Abstract:Objective To review the relationships between 25 hydroxyvitamin D3[25(OH)D3]levels and chronic hepatitis C.Methods We searched Cochrane Library,Medline,PubMed,Chinese Biomedical Literature Database(CBM),VIP,CNKI,and Wanfang database of English and Chinese studies reporting the relationship between 25 hydroxyvitamin D levels and chronic hepatitis D form establish of the database to March 1,2017.Results Twelve studies(n=1 144)were finally included,737 cases in CHC group and 407 in control group.The results of meta-analysis showed that 25(OH)D3 in patients with CHC was lower than the control group individuals(SMD=-1.59,95％CI-2.37～-0.80,P<0.01).The subgroup analysis of ethnicity,the CHC group was lower than the control group in the serum 25(OH)D3 levels for both Asian and Egyptian(SMD=-2.48,95％CI-3.58～-1.38,P<0.01;SMD=-2.10,95％CI-3.23～-0.96,P<0.01),while there was no statistical difference for White(SMD=-0.20,95％CI-1.42～-1.03,P=0.75).In the subgroup analysis of detection method for 25(OH)D3,both of ELISA and chromatography showed that the serum 25(OH)D3 levels in CHC group was lower than the control group(SMD=-2.39,95％CI-3.22～-1.56,P<0.01;SMD=-1.75,95％CI-2.06～-1.44,P<0.01).The result of the study showed no bias and reliable according to publication bias analysis and sensitivity analysis.Conclusion The 25 hydroxyvitamin D level was lower in chronic hepatitis C patients than health control.Due to the small of sample sizes,more high quality,large sample studies are needed to verify the results.
Association of 25-Hydroxyvitamin D with Liver Cancer Incidence and Chronic Liver Disease Mortality in Finnish Male Smokers of the ATBC Study.

Abstract:Background: Although circulating 25-hydroxyvitamin D [25(OH)D] concentrations were linked to liver cancer and chronic liver disease (CLD) in laboratory studies, few epidemiologic studies have addressed the associations.Methods: Within the Alpha-Tocopherol, Beta-Carotene Cancer Prevention Study, we measured 25(OH)D in baseline serum of 202 incident liver cancer cases and 225 CLD deaths that occurred during nearly 25 years of follow-up, and 427 controls. ORs and 95% confidence intervals (CIs) were estimated using unconditional logistic regression. We examined predetermined clinically defined cut-points, and season-specific and season-standardized quartiles.Results: Low serum 25(OH)D concentrations were associated with higher risk of liver cancer (<25 nmol/L vs. ≥50 nmol/L: 1.98; 95% CI, 1.22-3.20; Ptrend across categories = 0.003) and CLD mortality (1.93; 95% CI, 1.23-3.03; Ptrend = 0.006) in models adjusted for age and date of blood draw. After additional adjustment for body mass index, diabetes, smoking, and other potential confounders, the association remained statistically significant for liver cancer (1.91; 95% CI, 1.16-3.15; Ptrend = 0.008), but was somewhat attenuated for CLD mortality (1.67; 95% CI, 1.02-2.75; Ptrend = 0.05). Associations were similar for analyses using season-specific and season-standardized quartiles, and after excluding participants with diabetes, or hepatitis B or C.Conclusions: Our results suggest a possible preventive role for vitamin D against liver cancer and CLD, although the importance of the liver for vitamin D metabolism and the lack of information about underlying liver disease makes reverse causality a concern.Impact: Future studies are needed to evaluate associations of vitamin D with liver cancer and liver disease in other populations, particularly those with a different constellation of risk factors. Association between serum vitamin D and severity of liver fibrosis in chronic hepatitis C patients: a systematic meta-analysis.

Abstract:To conduct a systematic review of group studies assessing the association of serum vitamin D status with the severity of liver fibrosis in chronic hepatitis C patients using meta-analysis. The relevant research literatures were identified by searching PubMed and EMBASE databases prior to October 2013 with no restrictions. We included group studies that reported odds ratio (OR) estimates with 95% confidence intervals (CIs) or a mean with standard deviation (SD) for the association between serum vitamin D status and the severity of liver fibrosis in chronic hepatitis C patients. Approximately 8321 participants from several countries were included in this analysis. Six studies on serum vitamin D status and the severity of liver fibrosis were included in this meta-analysis. ORs with 95% CIs were extracted from four studies and the pooled ORs were 0.866 (95% CI, 0.649 to 1.157). The means with SDs were extracted from three studies and the pooled means were -0.487 (95% CI, -0.659 to -0.315). There was statistically significant heterogeneity among the mean data extracted studies (P=0.029; I(2)=71.8%) but not among the OR data extracted studies (P=0.061; I(2)=55.6%). Finally, results from the mean data extracted studies suggest that lower serum vitamin D is a risk factor for the severity of liver fibrosis in chronic hepatitis C patients. However, there is no conclusive evidence on this association because of inconsistencies between the OR data extracted studies and the mean data extracted studies.

The mean follow-up time was 411 ± 397 days with a range of 1-1382 days. 30 (12.0%) patients underwent liver transplantation and 85 (33.8%) individuals died within the study. The mean serum 25(OH)D3 concentration was 8.93 ± 7.1 ng/ml with a range of 1.0 to 46.0 ng/ml. 25(OH)D3 levels differed significantly between Child Pugh scores and showed a negative correlation with the model of end stage liver disease (MELD) score. Patients with decompensated cirrhosis and infectious complications, had significantly lower 25(OH)D3 levels compared to subjects without complications. Abstract:Objective To investigate the correlations of serum 25-hydroxyvitamin D [25(OH)D] concentrations with hepatitis C virus ( HCV ) RNA loads and cirrhosis severity in hepatitis C patients . Methods A total of 271 hepatitis C patients and 218 healthy subjects were enrolled , and their serum samples were collected .Serum 25( OH) D concentrations and HCV RNA loads were determined .Moreover, serum alpha 2-macroglobulin (α2M), haptoglobin (HPT), apolipoprotein A1 (apo A1), total bilirubin (TBil) and gamma-glutamyl transpeptidase (GGT) concentrations were determined .According to Fibrotest classification , the severity of cirrhosis was evaluated . The 25 ( OH ) D concentrations between hepatitis C and healthy control groups were compared .Furthermore, the difference of serum 25( OH) D between HCV RNA positive group and HCV RNA negative group was also evaluated .The correlation between serum 25( OH) D concentrations and HCV RNA loads in HCV RNA positive group was analyzed .Hepatitis C patients were classified according to the quartile concentrations of 25(OH)D, and the correlation between serum 25(OH)D concentrations and Fibrotest classification was investigated .Results Serum 25(OH)D concentrations were significantly higher in healthy control group than in hepatitis C group [ means were ( 60.42 ±1.34 ) nmol/L and ( 49.31 ± 1.39) nmol/L, P<0.01].The proportions of patients with the deficiency of 25(OH)D (<50 nmol/L) and the ones with severe deficiency (<25 nmol/L) in hepatitis C group were 41.33%(112/271) and 14.40%(39/271), which were significantly higher than those in healthy control group [27.98%(61/218) and 3.67% (8/218)] (P<0.01). No significant difference for serum 25(OH)D concentrations was observed between HCV RNA positive and HCV RNA negative groups [means were 48.98 ±21.59 and (49.89 ±25.27) nmol/L, P>0.05].Linearity regression revealed that there was no correlation between the logarithmic of HCV RNA loads and the concentration of 25 ( OH) D in HCV RNA positive group (r =0.056, P =0.412).Moreover, there was no statistical significance for the proportion of patients with different Fibrotest classifications (F0-F1, F1-F3 and F3-F4) between the highest quartile concentration and the lowest quartile concentration of 25(OH)D [means were 49.12 ±5.80, 48.27 ±4.90 and (49.89 ±8.20) nmol/L, P>0.05].Conclusions The hepatitis C patients are prone to encounter 25(OH)D deficiency compared with healthy subjects, which implys that these patients are suggested to uptake vitamin D supply .Furthermore, no clinical correlation is found between the logarithmic of HCV RNA loads , cirrhosis severity and the concentrations of 25(OH)D.

Abstract:Background: Hepatic encephalopathy is the most common and debilitating complication among the patients with cirrhosis. Electrolyte derangement is greatly related to recurrence and outcome of patients hospitalized with hepatic encephalopathy. Objective: This study was aimed to find out the association between these two variables in our setting. Methodology: The study was a cross-sectional study and conducted among 100 patients admitted with hepatic encephalopathy. Diagnosis and staging of hepatic encephalopathy were done based on West Haven staging, and severity of liver cirrhosis by the MELD (Model for End-Stage Liver Disease) scoring, and the outcomes were determined by the condition of the patients during discharge. Baseline serum electrolyte level was done for all the study participants. Results: Among 100 patients, male-female ratio was 7:3 with an average (standard deviation) age 56.25(±14.8) years. Most common causes of hepatic encephalopathy were infection (40.0%), gastrointestinal bleeding (30.0%), electrolyte abnormality (30.0%). Outcomes of hepatic encephalopathy were complete recovery (75.0%), needed ICU admission (15.0%) and death (10.0%). This study found no co-relation between baseline electrolyte and complete recovery of these patients and no association with the outcome of the patients. Conclusion: Baseline serum electrolyte have no significant influence on recovery of the patients with hepatic encephalopathy with liver cirrhosis. Abstract:Abstract Introduction: Vitamin D has generated great interest as a potential preventative agent of chronic disease. For example, laboratory studies suggest that vitamin D has anti-proliferative, anti-inflammation, pro-differentiation, and pro-apoptotic properties. Epidemiologic studies have linked high vitamin D levels with some cancers, yet few studies have examined possible associations with liver cancer or chronic liver disease. Therefore, we evaluated the association of serum 25-hydroxyvitamin D (25(OH)D) with incident liver cancer and chronic liver disease mortality in a case-control study nested within the prospective Alpha-Tocopherol and Beta-Carotene Cancer Prevention (ATBC) Study, a cohort of Finnish male smokers. Methods: The ATBC Study was a randomized primary prevention trial of α-tocopherol and β-carotene supplementation where 29,133 Finnish male smokers, aged 50-69, were recruited from 1985 to 1988 and randomized to daily supplementation of α-tocopherol (50 mg), β-carotene (20 mg), both, or placebo. Supplementation ended in April 1993 (median 6.1 years); however, participants have been followed since that time. At baseline, participants underwent a physical examination by registered nurses to measure height and weight, and to collect an overnight fasting blood sample. Participants also completed a baseline questionnaire concerning their tobacco smoking, general risk factors, diet and their medical history. Serum 25(OH)D levels was measured for 202 incident liver cancer cases (ICD-9=155 and ICD-10=C22), 225 chronic liver disease deaths (CD-9=571 and ICD-10=K70, K73, or K74) occurring between baseline and December 31, 2009, and 427 liver cancer-free controls matched with cases by age and date of blood draw. Odds ratios (ORs) and 95% confidence intervals (CIs) were estimated using unconditional logistic regression models, adjusting for age, date of blood draw, BMI, history of diabetes, number of cigarettes smoked per day, daily intakes of alcohol and coffee, serum α-tocopherol, β-carotene, and cholesterol. Results: High serum 25(OH)D levels were inversely associated with both incident liver cancer and chronic liver disease in models adjusted for age and date of blood draw. After multivariate adjustment, associations remained similar for incident liver cancer (season-specific quartile 4 (Q4) vs. quartile 1 (Q1) OR=0.53, 95% CI=0.31-0.89, P-trend=0.007) but were attenuated somewhat for chronic liver disease mortality (Q4 vs. Q1 OR=0.71, 95% CI=0.41-1.25, P-trend=0.23). Associations were similar in analyses using clinical vitamin D cutpoints and season-standardized quartiles, and in analyses stratified by season of blood collection and intervention group. Similar associations were also observed among cases occurring more than fifteen years after baseline and in analyses performed after excluding cases seropositive for hepatitis B and C viruses. Conclusions: Men with higher serum 25(OH)D levels had a lower risk of incident liver cancer. A similar pattern was observed for chronic liver disease mortality, although associations for this outcome were not statistically significant. Abstract:Background: Nonalcoholic fatty liver disease (NAFLD) is a condition, in which triglyceride accumulates in hepatic cells without a history of alcohol consumption and is strongly associated with insulin resistance, obesity, diabetes mellitus, hypertension and dyslipidemia. The potential role of vitamin D in the pathogenesis of NAFLD has been reported. Objectives: The aim of this study was to determine the optimal vitamin D levels for prevention of NAFLD. Methods: In a cross-sectional study, 2,160 cases who referred to a university-affiliated health center were randomly selected and their demographic information, anthropometric and metabolic indices and also vitamin D levels were collected. Fatty liver was assessed by fatty liver index (FLI) and confirmed by FibroScan using controlled attenuation parameter (CAP). Based on the NAFLD score, the subjects were divided into two groups and the vitamin D cutoff point was calculated by ROC curve. Results: Based on the results, 745 patients (34.5%) had different degrees of fatty liver. Significant differences in the stiffness of liver tissue were observed between vitamin D categories (285.10 ± 30.56 for severe, 251.82 ± 42.63 for moderate and 201.02 ± 36.08for mild deficiency). According to the multivariate analysis, age, fasting insulin and vitamin D levels were found as the most significant factors in NAFLD pathogenesis. Vitamin D cutoff point was obtained 18 nmol/L in women and 21 nmol/L in men. Conclusions: The results indicated a significant association between vitamin D level and NAFLD score. Association between serum 25-hydroxyvitamin D3 level and cognitive impairment in older chronic kidney disease patients

Abstract:This study aims to examine whether hypovitaminosis D was associated with cognitive impairment among chronic kidney patients with different level of albuminuria. This population-based cross-sectional study was conducted on elderly (over 60 years old) with urine albumin to creatinine ratio (UACR) ≥ 30 mg/g from 2011 to 2014 in the US National Health and Nutrition Examination Survey (NHANES). Cognitive function was assessed by the Consortium to Establish a Registry for Alzheimer's Disease Word List Learning (CERAD). Subjects were divided into 2 groups according to the absence or presence of cognitive impairment and a propensity score matching (PSM) was further conducted. The association was assessed with Spearman correlation and logistic regression analysis. The positive association of 25-hydroxyvitamin D3 (25(OH)D3) and cognitive score was presented. PSM analysis revealed that a higher level of 25(OH)D3 correlated to a better cognitive function in CKD patients with albuminuria, especially in patients with 30 mg/g ≤ UACR < 300 mg/g. This study indicated that a low 25(OH)D3 level was associated with poor cognitive performance, especially in patients with microalbuminuria. Thus, early diagnosis of vitamin D insufficiency and an effective intervention might be a useful therapeutic strategy to prevent cognitive decline in patients with the progression of renal dysfunction.

Independent association of hypovitaminosis d with non-alcoholic fatty liver disease in people with chronic spinal cord injury: a cross-sectional study

Abstract:Purpose: Non-alcoholic fatty liver disease (NAFLD) and hypovitaminosis D are highly prevalent in people with spinal cord injury (SCI) and could exert an unfavorable influence on cardiovascular profile and rehabilitation outcomes. We aimed to assess the independent association between low 25-hydroxy vitamin D (25(OH)D) levels and NAFLD in people with chronic (> 1 year) SCI. Methods: One hundred seventy-three consecutive patients with chronic SCI (132 men and 41 women) admitted to a rehabilitation program underwent clinical/biochemical evaluations and liver ultrasonography. Results: NAFLD was found in 105 patients (60.7% of the study population). They were significantly older and exhibited a poorer leisure time physical activity (LTPA) and functional independence in activities of daily living, a greater number of comorbidities and a higher prevalence of metabolic syndrome (MetS) and its correlates, including lower HDL and higher values of body mass index (BMI), systolic blood pressure, HOMA-index of insulin resistance and triglycerides. 25(OH)D levels were significantly lower in NAFLD (median: 10.6 ng/ml, range: 2.0-31.0) than in non-NAFLD group (22.5 ng/ml, 4.2-51.6). When all these variables were included in a multiple logistic regression analysis, a significant independent association with NAFLD only persisted for lower 25(OH)D levels, a greater number of comorbidities and a poorer LTPA. The ROC analysis revealed that 25(OH)D levels < 18.25 ng/ml discriminated patients with NAFLD with a sensitivity of 89.0% and a specificity of 73.0% (AUC: 85.7%; 95%CI: 79.6-91.7%). NAFLD was exhibited by 83.9% of patients with 25(OH)D levels < 18.25 ng/ml and by 18% of those with 25(OH)D levels ≥ 18.25 ng/ml (p < 0.0001). Conclusion: In people with chronic SCI, 25(OH)D levels < 18.25 ng/ml may represent a marker of NAFLD independent of MetS-related features. Further studies are warranted to define the cause-effect relationships of this association.

Abstract:BACKGROUND/AIMS: A low vitamin D level has been associated with metabolic syndrome and diabetes. However, an association between a low vitamin D level and nonalcoholic fatty liver disease (NAFLD) has not yet been definitively established. This study aimed to characterize the relationship between a vitamin D level and NAFLD in Korea.METHODS: A cross-sectional study involving 6,055 health check-up subjects was conducted. NAFLD was diagnosed on the basis of typical ultrasonographic findings and a history of alcohol consumption.RESULTS: The subjects were aged 51.7 ± 10.3 years (mean ± SD) and 54.7% were female. NAFLD showed a significant inverse correlation with the vitamin D level after adjusting for age and sex [odds ratio (OR)=0.85, 95% confidence interval (CI)=0.75-0.96]. The age- and sex-adjusted prevalence of NAFLD decreased steadily with increasing vitamin D level [OR=0.74, 95% CI=0.60-0.90, lowest quintile (≤14.4 ng/mL) vs highest quintile (≥28.9 ng/mL), p for trend <0.001]. Multivariate regression analysis after adjusting for other metabolic factors revealed that NAFLD showed a significant inverse correlation with both the vitamin D level (>20 ng/mL) [OR=0.86, 95% CI=0.75-0.99] and the quintiles of the vitamin D level in a dose-dependent manner (p for trend=0.001).CONCLUSIONS: The serum level of vitamin D, even when within the normal range, was found to be inversely correlated with NAFLD in a dose-dependent manner. Vitamin D was found to be inversely correlated with NAFLD independent of known metabolic risk factors. These findings suggest that vitamin D exerts protective effects against NAFLD.

Abstract:The report in this issue of CCI by Kalyanasundaram and co-investigators [1] provides the most recent data analysis from the pivotal TARGET trial. This multicenter study intended to document clinical outcomes and identify their determinants in patients with ischemic syndromes who underwent PCI, mainly of native coronary arteries, with stenting and 2b/3a inhibition. The TARGET subgroup analysis of 200 SVG interventions by Kalyanasundaram and colleagues is interesting and its observations are relevant to clinical practice. Intriguingly, as the title of this editorial implies, a new conclusion can be drawn from their data. The patients received either tirofiban or abciximab without the use of distal protection. Multivariate analysis identified only thrombus as a predictor a 30-day major adverse coronary event. While no difference in outcome was noted between the two-receptor antagonists used in the SVGs, a previous report encompassing the entire TARGET population of 4,809 patients revealed that tirofiban offered less protection than abciximab [2]. The discrepancy between the results of these two reports could be attributed to the much smaller size of the SVG group. An important aspect needing focused evaluation is the commonly found large, multilayered thrombus in SVGs associated with impaired antegrade flow. Unfortunately, the investigators did not report detailed assessment of preand post-intervention thrombus burden and did not study the effect of different grades of thrombus burden on the yield of the 2b/3a antagonists and the MACE rate. A much desired QCA analysis by an independent core laboratory is absent. Other studies that used independent QCA core laboratory for this task found angiographic grading of thrombus burden [3] by interventionalists to correlate well with QCA thrombus assessment [4] and to assist in target lesion management. Despite an almost instinctive, prevailing belief among interventionalists in the necessity of protection devices to reduce adverse ischemic events, the lack of the protection devices in the TARGET study likely increased MACE rates. As the investigators and others have noted, morphologic characteristics unique for SVGs preclude application of protection devices. Furthermore, the absolute need to cross a tight, thrombotic lesion with a protection device is a limiting factor because crossing the thrombus and underlying plaque frequently causes distal embolization. Another concern is the deleterious effect of the deployed device on the wall of the SVG. The question of ‘‘why does a dedicated treatment with the useful 2b/3a inhibitors still leave thrombus as a major determinant of MACE?’’ becomes important. The answer lies with a proposed new conclusion from the TARGET reports: these pharmacologic agents ‘‘miss’’ the target thrombus. Studying the structural morphology of thrombi under scanning electron microscope demonstrates that elastic fibrin strands crisscross forming netlike scaffolding with platelets anchored to the fibrin. These elastic fibrin fibers consist of various sizes and exhibit marked resilience to external forces [such as balloon deployment] thereby fortifying the clot with structural support [5]. Blood cells, procoagulants, and vasoactive reactants are also present within the fibrin net. Because 2b/3a inhibitors exert their main selective effect only on the platelets receptors, they leave untouched the fibrin net and associated constituents. These uninterrupted components continue to constitute a flow barrier and serve as a substrate for further thrombus expansion and continuous mediators discharge. Previous studies have shown that in the presence of angiographic thrombus, 2b/3a inhibitors face an ‘‘efficiency ceiling’’ and do not contribute to a successful thrombus management and subsequent improvement in clinical results [6]. Abstract:Background: NAFLD and vitamin D deficiency often coexist and epidemiologic evidence has shown that both of these conditions share several risk factors. Recent studies investigating the relationship between vitamin D levels and severity of NAFLD showed conflicting results. Thus we conducted a systematic review and meta-analysis to evaluate association between vitamin D and NAFLD histologic severity. Methods: A comprehensive search of the databases of the MEDLINE and EMBASE was performed from inception through November 2016. Observational studies compared serum vitamin D levels among NAFLD patients with high and low histologic severity, which was determined by NAFLD activity score (NAS) and fibrosis score. We calculated pooled mean difference (MD) of 25-hydroxyvitamin D levels with 95% confidence intervals (CI) using random-effects model. Results: Data were extracted from 6 studies involving 974 NAFLD patients. There was no difference in 25-hydroxyvitamin D levels among NAFLD patients with high NAS (score of ≥5) versus low NAS (pooled MD=-0.93, 95%CI -2.45 to 0.58, I2=0%) and also high fibrosis score (score of ≥3) versus low fibrosis score (pooled MD=0.88, 95%CI -2.65 to 4.42, I2=64%). Conclusions: Despite evidence implicating vitamin D in NAFLD pathogenesis, serum 25-hydroxyvitamin D may not be associated with NAFLD histologic severity.
Relationship Between Vitamin D Deficiency and Non-alcoholic Fatty Liver Disease: A Cross-Sectional Study From a Tertiary Care Center in Northern India

Abstract:Background Vitamin D levels are strongly associated with myocardial infarction, coronary artery disease, heart dysfunction, and even mortality. Non-alcoholic fatty liver disease (NAFLD) is a prevalent hepatic illness whose incidence has grown dramatically over the past several decades. Methodology This observational, cross-sectional study was conducted over 1.5 years (January 2019 to June 2020) at the Department of General Medicine of a tertiary care hospital in northern India on 100 adult patients with NAFLD admitted to the emergency ward, intensive care unit, and medical ward. Results In our study, of the 100 patients, 45.0%, 16.0%, and 39.0% of patients exhibited vitamin D deficiency, insufficiency, and sufficiency, respectively. Vitamin D deficiency was the highest among those aged 41-50 (54.2%) and lowest among those aged 30-40 (29.0%). We observed that vitamin D deficiency was less prevalent in people with a normal body mass index (39.1%) than in those who were overweight (91.7%). There was a significant (p < 0.05) association between the severity of vitamin D deficiency and the presence of hepatomegaly, splenomegaly, and ascites. Overall, the incidence of fatty liver was 49% among patients. There was a significant (p = 0.0001) correlation between fatty liver and serum vitamin D levels. The association between the proportion of patients with fatty liver and the degree of vitamin D deficiency was found to be significant (p = 0.04). The relationship between the distribution of patients according to insulin resistance and the degree of vitamin D deficiency was also statistically significant (p < 0.001). Conclusions Vitamin D deficiency is associated with an increased risk of NAFLD, as well as with the severity of NAFLD.

Abstract:Introduction/ObjectivesPrevious studies have shown conflicting results regarding the association between hypovitaminosis D and non-alcoholic fatty liver disease (NAFLD). The aim of this study is to explore the individual and combined effect of hypovitaminosis D and metabolic syndrome (MS) on NAFLD.Materials and methodsIn this cross-sectional study, 874 subjects were enrolled. 25(OH)D was assessed by a sequential competitive immuno-fluoro-assay method. The Fatty Liver Index (FLI) was used for NAFLD screening. Binary logistic regression and additive interaction were performed to investigate the association between vitamin D status, MS and NAFLD.ResultsSevere vitamin D deficiency was found to be positively related to NAFLD, with a higher risk in women than in men (OR = 6.4, 95% CI [2.8-15], p < 0.0001 vs. OR = 5.8, 95% CI [1.9-17.7], p = 0.002). In men, this association was partially masked by obesity. The additive interaction with MS was significant in women but not in men, the relative excess risk due to interaction was of 7.2, 95% CI [1.3-12.9], p = 0.02), the attributable proportion due to the combined effect was of 0.6, 95% CI [0.4-0.8], p < 0.0001. The interaction mechanism is synergistic; the synergy index: was of 2.9, 95% CI [1.6-5.3], p = 0.0006.ConclusionA positive association has been found between severe vitamin D deficiency and NAFLD. Moreover, an excess risk in women combining both MS and severe vitamin D deficiency was quantified.

Abstract:Objectives: This study aimed to investigate the potential correlations among serum 25-hydroxyvitamin D [25(OH) D] levels, white matter hyperintensity (WMH) and cognitive function in patients with non-disabling ischemic cerebrovascular events (NICE). Methods: This was a prospective investigation of 160 NICE patients with age of 40 years or older. Cognitive function was evaluated by the Montreal Cognitive Assessment (MoCA). White matter lesions were evaluated by WMH using Fazekas scores. Spearman correlation analysis and linear regression models were used to identify the associations between serum 25(OH)D levels and cognitive function. Binary logistic regression analysis models were used to evaluate the predictable value of serum 25(OH)D levels and WMH for cognitive impairment.Results: Patients with inadequate 25(OH)D levels had lower MoCA score (P=0.008), and a higher proportion of severe WMH (P=0.043). Spearman correlation analysis demonstrated that serum 25(OH)D concentrations were positively associated with MoCA score (rs=0.185, P=0.019) while negatively related to the proportion of severe WMH (sWMH) (rs=-0.166, P=0.036).The association between 25(OH)D concentrations and MoCA score remained significant in linear regression (adjusted & beta;=0.012, 95%CI:0.001-0.203).Adjusted binary logistic regression analysis showed that the odds ratio of cognitive impairment with insufficient 25(OH)D concentration was 5.038 (95%CI:1.154-21.988) compared with the sufficient group and the sWMH (OR=2.728, 95%CI:1.230-6.051) was identified as an independent risk factor for cognitive decline in NICE patients. Conclusion: Serum 25(OH)D levels and white matter lesions were independently and significantly associated with cognitive impairment in NICE patients.

Abstract:AIM: To investigate the relationship between serum vitamin D3 levels and liver fibrosis or inflammation in treatment-naive Chinese patients with chronic hepatitis C(CHC).METHODS: From July 2010 to June 2011, we enrolled 122 CHC patients and 11 healthy controls from Dingxicity, Gansu Province, China. The patients were infected with Hepatitis C virus(HCV) during blood cell retransfusion following plasma donation in 1992-1995, and had never received antiviral treatment. At present, all the patients except two underwent liver biopsy with ultrasound guidance. The Scheuer Scoring System was used to evaluate hepatic inflammation and the Metavir Scoring System was used to evaluate hepatic fibrosis. Twelve-hour overnight fasting blood samples were collected in the morning of the day of biopsy. Serum levels of alanine aminotransferase, aspartate aminotransferase, total bilirubin, direct bilirubin, cholineste rase, prothrom binactivity, albumin, γ-glutamyl transpeptidase, hemoglobin, calcium and phosphorus were determined. Serum HCV RNA levels were measured by real-time PCR. Serum levels of 25-hydroxyvitamin D3 [25(OH)D3] and 24,25-dihydroxyvitamin D3 [24,25(OH)2D3] were measured by high-performance liquid chromatography tandem mass spectrometry.R E S U LT S : Serumlevels of 2 5(OH) D3 but not 24,25(OH)2D3 were significantly lower in CHC patients than in control subjects. Serum 25(OH)D3 levels did not correlate with liver fibrosis, inflammation, patient age, or levels of alanine aminotransferase, aspartate aminotransferase, total bilirubin, direct bilirubin, prothrombin activity, cholinesterase or HCV RNA. However, serum 25(OH)D3 levels did correlate with serum 24,25(OH)2D3 levels. Serum 25(OH)D3 and 24,25(OH)2D3 levels, and the 25(OH)D3/24,25(OH)2D3 ratio, have no difference among the fibrosis stages or inflammation grades.CONCLUSION: We found that serum levels of 25(OH)D3 and its degradation metabolite 24,25(OH)2D3 did not correlate with liver fibrosis in treatment-naive Chinese patient with CHC.

Abstract:Objective: To determine the association of serum Ammonia levels with severity of grades of hepatic encephalopathy in patients presenting to the department. Study Design: Comparative cross-sectional study. Place and Duration of Study: Emergency Department, Pak Emirates Military Hospital Rawalpindi Pakistan, from Dec 2020 to May 2021. Methodology: This study was conducted on 100 patients presenting with hepatic encephalopathy at the emergency department of our hospital. Based on clinical findings and relevant investigations, a consultant gastroenterologist or emergency medicine physician diagnosed hepatic encephalopathy or the underlying cause. In addition, serum Ammonia levels were done on all the patients at the time of presentation and associated with grades of hepatic encephalopathy and other factors. Results: Out of 100 patients included in the final analysis, 67 were males, and 33 were females. Infection (29%) was the commonest aetiology of hepatic encephalopathy, followed by Constipation (23%). In addition, 59% had normal serum ammonia levels, while 41% had raised ammonia levels in their serum. High grade of hepatic encephalopathy and raised Child-Turcotte-Pugh score were statistically significantly associated with raised ammonia levels in our study participants. Conclusion: Raised serum ammonia levels were a common finding among patients with hepatic encephalopathy. Therefore, patients with a high grade of hepatic encephalopathy and a high Child-Turcotte-Pugh score at the time of presentation should be considered at a higher risk of having hyperammonemia.
