Since the growth of meningioma is the principal indication of treatment, accurate and rapid measurement of the volume of intracranial meningiomas is essential in clinical practice to determine the growth rate of the tumor. It could be useful for the management of meningiomas given their increasing incidence and the wait-and-see policy currently in use for asymptomatic meningiomas. The aim of this study was to develop and validate a computational model for fully automated meningioma segmentation and volume measurement on contrast-enhanced MR scans using deep learning. Material and Methods The retrospectively collected axial contrast-enhanced T1-weighted section images from patients diagnosed with meningioma were manually segmented and used to construct automatic segmentation models with six U-Net- and nnU-Net-based architectures. The performance of each model was evaluated with the Sørensen-Dice similarity coefficient (DSC) with internal (IVS) and external validation sets (EVS), each consisting of 100 independent MRI examinations. Results A total of 12,909 section images from 459 patients were applied for the training (median age 58 [52-66] [IQR]; 385 women [83.9%]). The median tumor volume of the training set was 2.36 cm3. A 2D nnU-Net showed the highest median DSCs of 0.922 and 0.893 for the IVS and EVS, respectively. The nnU-Nets achieved superior performance in meningioma segmentation than the U-Nets. The DSCs of the 2D nnU-Net for small meningiomas less than 1 cm3 were 0.769 and 0.780 with the IVS and EVS, respectively. Conclusion We successfully developed a fully automated and accurate volumetric measurement tool for meningioma using nnU-Net. The volumetry performance for small meningioma was significantly better than that achieved in previous studies. The results of this study are clinically applicable and are expected to be of great use in the management of monitored meningioma patients.

Abstract:Purpose: In clinical work, accurately measuring the volume and the size of breast cancer is significant to develop a treatment plan. However, it is time-consuming, and inter- and intra-observer variations among radiologists exist. The purpose of this study was to assess the performance of a Res-UNet convolutional neural network based on automatic segmentation for size and volumetric measurement of mass enhancement breast cancer on magnetic resonance imaging (MRI). Materials and methods: A total of 1,000 female breast cancer patients who underwent preoperative 1.5-T dynamic contrast-enhanced MRI prior to treatment were selected from January 2015 to October 2021 and randomly divided into a training cohort (n = 800) and a testing cohort (n = 200). Compared with the masks named ground truth delineated manually by radiologists, the model performance on segmentation was evaluated with dice similarity coefficient (DSC) and intraclass correlation coefficient (ICC). The performance of tumor (T) stage classification was evaluated with accuracy, sensitivity, and specificity. Results: In the test cohort, the DSC of automatic segmentation reached 0.89. Excellent concordance (ICC > 0.95) of the maximal and minimal diameter and good concordance (ICC > 0.80) of volumetric measurement were shown between the model and the radiologists. The trained model took approximately 10-15 s to provide automatic segmentation and classified the T stage with an overall accuracy of 0.93, sensitivity of 0.94, 0.94, and 0.75, and specificity of 0.95, 0.92, and 0.99, respectively, in T1, T2, and T3. Conclusions: Our model demonstrated good performance and reliability for automatic segmentation for size and volumetric measurement of breast cancer, which can be time-saving and effective in clinical decision-making.

Evaluating the clinical applicability of neural networks for meningioma tumor segmentation on 3D MRI

Abstract:Magnetic resonance imaging (MRI), in both 2D and 3D configurations, serves as the leading non-invasive and non-ionizing technique for the detection and clinical assessment of brain tumors. 3D multiparametric MRIs (mpMRIs) offer enhanced spatial and biological context over other MRIs, which can help clinicians plan tailored treatment and therapy. Presently, surgical intervention remains the primary treatment method for brain tumors, requiring accurate segmentation of tumor tissue on MRI scans. However, manual tumor segmentation and traditional machine learning techniques are labor intensive and subject to observer bias. Deep learning models can provide enhanced accuracy and precision in tumor segmentation due to their ability to automatically extract intricate features and patterns from MRI scans. In this study, MRI scans from 358 meningioma patients were used to train a series of deep learning models to segment the tumor subregions. We hypothesized that a deep learning framework designed for meningioma tumor segmentation and trained utilizing 3D mpMRIs would demonstrate superior segmentation accuracy compared to a framework trained on 2D MRIs or single-sequence 3D MRIs. We compared the model predictions with the ground truth labels using similarity metrics to assess accuracy and clinical applicability. The model trained on 3D mpMRI scans showed reliable performance for meningioma subregion segmentation with a Dice Similarity Coefficient score of 0.91 and a median Sensitivity of 91.38%. It outperformed the other models due to the additional spatial context provided by 3D mpMRIs, suggesting it could be readily used in clinical practice to help guide treatment strategy.

Abstract:Abstract Background Manual segmentation of brain tumors requires expertise, is time-consuming, and is subject to inter-rater variability. Fully automatic brain tumor segmentation is possible for glioma and meningioma when volumetric T1, T1 contrast-enhanced (T1c), T2, and Fluid-attenuated inversion recovery (FLAIR) MRIs are available. In clinical care of meningiomas, however, often only volumetric T1c scans are available. In this work, we trained a deep learning network to segment meningiomas using only T1c scans for use in clinical research. Material and Methods NnU-Net, a deep learning model that is optimized for medical image segmentation, was trained to segment meningiomas from T1c images. This was performed on a large clinically collected meningioma dataset (n=374) of T1c scans with semi-automatically generated enhancing tumor masks and additional data from the BraTS2020 glioma dataset. Model performance was compared against inter-rater reliability, between different models, between anatomical tumor locations, and against models using multiple MRI modalities. Results The best performing model obtained a Dice score of 0.90. This performance was 0.03 points lower when compared to inter-rater reliability (Dice=0.93) and almost equal to models using multiple MRI modalities. Model performance split over anatomical tumor locations was between 0.90 and 0.97 (Dice). Conclusion Fully automatic meningioma segmentation using only T1c images is possible with an accuracy that is similar to inter-rater reliability and models using multiple imaging modalities.

Abstract:The Neuroradiology Journal, Ahead of Print. BackgroundMeningioma is the commonest primary brain tumour. Volumetric post-contrast magnetic resonance imaging (MRI) is recognised as gold standard for delineation of meningioma volume but is hindered by manual processing times. We aimed to investigate the utility of a model-based variational approach in segmenting meningioma.MethodsA database of patients with a meningioma (2007–2015) was queried for patients with a contrast-enhanced volumetric MRI, who had consented to a research tissue biobank. Manual segmentation by a neuroradiologist was performed and results were compared to the mathematical model, using a battery of tests including the Sørensen–Dice coefficient (DICE) and JACCARD index. A publicly available meningioma dataset (708 segmented T1 contrast-enhanced slices) was also used to test the reliability of the model.Results49 meningioma cases were included. The most common meningioma location was convexity (n = 15, 30.6%). The mathematical model segmented all but one incidental meningioma, which failed due to the lack of contrast uptake. The median meningioma volume by manual segmentation was 19.0 cm3 (IQR 4.9–31.2). The median meningioma volume using the mathematical model was 16.9 cm3 (IQR 4.6–28.34). The mean DICE score was 0.90 (SD = 0.04). The mean JACCARD index was 0.82 (SD = 0.07). For the publicly available dataset, the mean DICE and JACCARD scores were 0.90 (SD = 0.06) and 0.82 (SD = 0.10), respectively.ConclusionsSegmentation of meningioma volume using the proposed mathematical model was possible with accurate results. Application of this model on contrast-enhanced volumetric imaging may help reduce work burden on neuroradiologists with the increasing number in meningioma diagnoses.

Abstract:Abstract Background Longitudinal measurement of glioma burden with MRI is the basis for treatment response assessment. In this study, we developed a deep learning algorithm that automatically segments abnormal fluid attenuated inversion recovery (FLAIR) hyperintensity and contrast-enhancing tumor, quantitating tumor volumes as well as the product of maximum bidimensional diameters according to the Response Assessment in Neuro-Oncology (RANO) criteria (AutoRANO). Methods Two cohorts of patients were used for this study. One consisted of 843 preoperative MRIs from 843 patients with low- or high-grade gliomas from 4 institutions and the second consisted of 713 longitudinal postoperative MRI visits from 54 patients with newly diagnosed glioblastomas (each with 2 pretreatment “baseline” MRIs) from 1 institution. Results The automatically generated FLAIR hyperintensity volume, contrast-enhancing tumor volume, and AutoRANO were highly repeatable for the double-baseline visits, with an intraclass correlation coefficient (ICC) of 0.986, 0.991, and 0.977, respectively, on the cohort of postoperative GBM patients. Furthermore, there was high agreement between manually and automatically measured tumor volumes, with ICC values of 0.915, 0.924, and 0.965 for preoperative FLAIR hyperintensity, postoperative FLAIR hyperintensity, and postoperative contrast-enhancing tumor volumes, respectively. Lastly, the ICCs for comparing manually and automatically derived longitudinal changes in tumor burden were 0.917, 0.966, and 0.850 for FLAIR hyperintensity volume, contrast-enhancing tumor volume, and RANO measures, respectively. Conclusions Our automated algorithm demonstrates potential utility for evaluating tumor burden in complex posttreatment settings, although further validation in multicenter clinical trials will be needed prior to widespread implementation.

Abstract:Tumor burden assessment by magnetic resonance imaging (MRI) is central to the evaluation of treatment response for glioblastoma. This assessment is complex to perform and associated with high variability due to the high heterogeneity and complexity of the disease. In this work, we tackle this issue and propose a deep learning pipeline for the fully automated end-to-end analysis of glioblastoma patients. Our approach simultaneously identifies tumor sub-regions, including the enhancing tumor, peritumoral edema and surgical cavity in the first step, and then calculates the volumetric and bidimensional measurements that follow the current Response Assessment in Neuro-Oncology (RANO) criteria. Also, we introduce a rigorous manual annotation process which was followed to delineate the tumor sub-regions by the human experts, and to capture their segmentation confidences that are later used while training the deep learning models. The results of our extensive experimental study performed over 760 pre-operative and 504 post-operative adult patients with glioma obtained from the public database (acquired at 19 sites in years 2021-2020) and from a clinical treatment trial (47 and 69 sites for pre-/post-operative patients, 2009-2011) and backed up with thorough quantitative, qualitative and statistical analysis revealed that our pipeline performs accurate segmentation of pre- and post-operative MRIs in a fraction of the manual delineation time (up to 20 times faster than humans). The bidimensional and volumetric measurements were in strong agreement with expert radiologists, and we showed that RANO measurements are not always sufficient to quantify tumor burden.

Abstract:ObjectivesDevelop and evaluate a deep learning–based automatic meningioma segmentation method for preoperative meningioma differentiation using radiomic features.MethodsA retrospective multicentre inclusion of MR examinations (T1/T2-weighted and contrast-enhanced T1-weighted imaging) was conducted. Data from centre 1 were allocated to training (n = 307, age = 50.94 ± 11.51) and internal testing (n = 238, age = 50.70 ± 12.72) cohorts, and data from centre 2 external testing cohort (n = 64, age = 48.45 ± 13.59). A modified attention U-Net was trained for meningioma segmentation. Segmentation accuracy was evaluated by five quantitative metrics. The agreement between radiomic features from manual and automatic segmentations was assessed using intra class correlation coefficient (ICC). After univariate and minimum-redundancy-maximum-relevance feature selection, L1-regularized logistic regression models for differentiating between low-grade (I) and high-grade (II and III) meningiomas were separately constructed using manual and automatic segmentations; their performances were evaluated using ROC analysis.ResultsDice of meningioma segmentation for the internal testing cohort were 0.94 ± 0.04 and 0.91 ± 0.05 for tumour volumes in contrast-enhanced T1-weighted and T2-weighted images, respectively; those for the external testing cohort were 0.90 ± 0.07 and 0.88 ± 0.07. Features extracted using manual and automatic segmentations agreed well, for both the internal (ICC = 0.94, interquartile range: 0.88–0.97) and external (ICC = 0.90, interquartile range: 0.78–70.96) testing cohorts. AUC of radiomic model with automatic segmentation was comparable with that of the model with manual segmentation for both the internal (0.95 vs. 0.93, p = 0.176) and external (0.88 vs. 0.91, p = 0.419) testing cohorts.ConclusionsThe developed deep learning–based segmentation method enables automatic and accurate extraction of meningioma from multiparametric MR images and can help deploy radiomics for preoperative meningioma differentiation in clinical practice.Key Points• A deep learning–based method was developed for automatic segmentation of meningioma from multiparametric MR images.• The automatic segmentation method enabled accurate extraction of meningiomas and yielded radiomic features that were highly consistent with those that were obtained using manual segmentation.• High-grade meningiomas were preoperatively differentiated from low-grade meningiomas using a radiomic model constructed on features from automatic segmentation.

Fast meningioma segmentation in T1-weighted MRI volumes using a lightweight 3D deep learning architecture

Abstract:Automatic and consistent meningioma segmentation in T1-weighted MRI volumes and corresponding volumetric assessment is of use for diagnosis, treatment planning, and tumor growth evaluation. In this paper, we optimized the segmentation and processing speed performances using a large number of both surgically treated meningiomas and untreated meningiomas followed at the outpatient clinic. We studied two different 3D neural network architectures: (i) a simple encoder-decoder similar to a 3D U-Net, and (ii) a lightweight multi-scale architecture (PLS-Net). In addition, we studied the impact of different training schemes. For the validation studies, we used 698 T1-weighted MR volumes from St. Olav University Hospital, Trondheim, Norway. The models were evaluated in terms of detection accuracy, segmentation accuracy and training/inference speed. While both architectures reached a similar Dice score of 70% on average, the PLS-Net was more accurate with an F1-score of up to 88%. The highest accuracy was achieved for the largest meningiomas. Speed-wise, the PLS-Net architecture tended to converge in about 50 hours while 130 hours were necessary for U-Net. Inference with PLS-Net takes less than a second on GPU and about 15 seconds on CPU. Overall, with the use of mixed precision training, it was possible to train competitive segmentation models in a relatively short amount of time using the lightweight PLS-Net architecture. In the future, the focus should be brought toward the segmentation of small meningiomas (less than 2ml) to improve clinical relevance for automatic and early diagnosis as well as speed of growth estimates.

Abstract:Background and objective: Treatment for meningiomas usually includes surgical removal, radiation therapy, and chemotherapy. Accurate segmentation of tumors significantly facilitates complete surgical resection and precise radiotherapy, thereby improving patient survival. In this paper, a deep learning model is constructed for magnetic resonance T1-weighted Contrast Enhancement (T1CE) images to develop an automatic processing scheme for accurate tumor segmentation. Methods: In this paper, a novel Convolutional Neural Network (CNN) model is proposed for the accurate meningioma segmentation in MR images. It can extract fused features in multi-scale receptive fields of the same feature map based on MR image characteristics of meningiomas. The attention mechanism is added as a helpful addition to the model to optimize the feature information transmission. Results and conclusions: The results were evaluated on two internal testing sets and one external testing set. Mean Dice Similarity Coefficient (DSC) values of 0.886, 0.851, and 0.874 are demonstrated, respectively. In this paper, a deep learning approach is proposed to segment tumors in T1CE images. Multi-center testing sets validated the effectiveness and generalization of the method. The proposed model demonstrates state-of-the-art tumor segmentation performance.

Abstract:PURPOSE:This study aimed to investigate the effectiveness and practicality of using models like convolutional neural network and transformer in detecting and precise segmenting meningioma from magnetic resonance images.METHODS:The retrospective study on T1-weighted and contrast-enhanced images of 523 meningioma patients from 3 centers between 2010 and 2020. A total of 373 cases split 8:2 for training and validation. Three independent test sets were built based on the remaining 150 cases. Six convolutional neural network detection models trained via transfer learning were evaluated using 4 metrics and receiver operating characteristic analysis. Detected images were used for segmentation. Three segmentation models were trained for meningioma segmentation and were evaluated via 4 metrics. In 3 test sets, intraclass consistency values were used to evaluate the consistency of detection and segmentation models with manually annotated results from 3 different levels of radiologists.RESULTS:The average accuracies of the detection model in the 3 test sets were 97.3%, 93.5%, and 96.0%, respectively. The model of segmentation showed mean Dice similarity coefficient values of 0.884, 0.834, and 0.892, respectively. Intraclass consistency values showed that the results of detection and segmentation models were highly consistent with those of intermediate and senior radiologists and lowly consistent with those of junior radiologists.CONCLUSIONS:The proposed deep learning system exhibits advanced performance comparable with intermediate and senior radiologists in meningioma detection and segmentation. This system could potentially significantly improve the efficiency of the detection and segmentation of meningiomas.

Abstract:OBJECTIVES: To noninvasively differentiate meningioma grades by deep learning radiomics (DLR) model based on routine post-contrast MRI.METHODS: We enrolled 181 patients with histopathologic diagnosis of meningioma who received post-contrast MRI preoperative examinations from 2 hospitals (99 in the primary cohort and 82 in the validation cohort). All the tumors were segmented based on post-contrast axial T1 weighted images (T1WI), from which 2048 deep learning features were extracted by the convolutional neural network. The random forest algorithm was used to select features with importance values over 0.001, upon which a deep learning signature was built by a linear discriminant analysis classifier. The performance of our DLR model was assessed by discrimination and calibration in the independent validation cohort. For comparison, a radiomic model based on hand-crafted features and a fusion model were built.RESULTS: The DLR signature comprised 39 deep learning features and showed good discrimination performance in both the primary and validation cohorts. The area under curve (AUC), sensitivity, and specificity for predicting meningioma grades were 0.811(95% CI, 0.635-0.986), 0.769, and 0.898 respectively in the validation cohort. DLR performance was superior over the hand-crafted features. Calibration curves of DLR model showed good agreements between the prediction probability and the observed outcome of high-grade meningioma.CONCLUSIONS: Using routine MRI data, we developed a DLR model with good performance for noninvasively individualized prediction of meningioma grades, which achieved a quantization capability superior over the hand-crafted features. This model has potential to guide and facilitate the clinical decision-making of whether to observe or to treat patients by providing prognostic information.

Abstract:Purpose: Meningiomas are the most common type of primary brain tumor, accounting for ~30% of all brain tumors. A substantial number of these tumors are never surgically removed but rather monitored over time. Automatic and precise meningioma segmentation is, therefore, beneficial to enable reliable growth estimation and patient-specific treatment planning. Methods: In this study, we propose the inclusion of attention mechanisms on top of a U-Net architecture used as backbone: (i) Attention-gated U-Net (AGUNet) and (ii) Dual Attention U-Net (DAUNet), using a three-dimensional (3D) magnetic resonance imaging (MRI) volume as input. Attention has the potential to leverage the global context and identify features' relationships across the entire volume. To limit spatial resolution degradation and loss of detail inherent to encoder–decoder architectures, we studied the impact of multi-scale input and deep supervision components. The proposed architectures are trainable end-to-end and each concept can be seamlessly disabled for ablation studies. Results: The validation studies were performed using a five-fold cross-validation over 600 T1-weighted MRI volumes from St. Olavs Hospital, Trondheim University Hospital, Norway. Models were evaluated based on segmentation, detection, and speed performances, and results are reported patient-wise after averaging across all folds. For the best-performing architecture, an average Dice score of 81.6% was reached for an F1-score of 95.6%. With an almost perfect precision of 98%, meningiomas smaller than 3 ml were occasionally missed hence reaching an overall recall of 93%. Conclusion: Leveraging global context from a 3D MRI volume provided the best performances, even if the native volume resolution could not be processed directly due to current GPU memory limitations. Overall, near-perfect detection was achieved for meningiomas larger than 3 ml, which is relevant for clinical use. In the future, the use of multi-scale designs and refinement networks should be further investigated. A larger number of cases with meningiomas below 3 ml might also be needed to improve the performance for the smallest tumors.

Abstract:BACKGROUND: More than a million people are affected by brain tumors each year; high-grade gliomas (HGGs) and low-grade gliomas (LGGs) present serious diagnostic and treatment hurdles, resulting in shortened life expectancies. Glioma segmentation is still a significant difficulty in clinical settings, despite improvements in Magnetic Resonance Imaging (MRI) and diagnostic tools. Convolutional neural networks (CNNs) have seen recent advancements that offer promise for increasing segmentation accuracy, addressing the pressing need for improved diagnostic and therapeutic approaches. OBJECTIVE: The study intended to develop an automated glioma segmentation algorithm using CNN to accurately identify tumor components in MRI images. The goal was to match the accuracy of experienced radiologists with commercial instruments, hence improving diagnostic precision and quantification. METHODS: 285 MRI scans of high-grade gliomas (HGGs) and low-grade gliomas (LGGs) were analyzed in the study. T1-weighted sequences were utilised for segmentation both pre-and post-contrast agent administration, along with T2-weighted sequences (with and without Fluid Attenuation by Inversion Recovery [FAIRE]). The segmentation performance was assessed with a U-Net network, renowned for its efficacy in medical image segmentation. DICE coefficients were computed for the tumour core with contrast enhancement, the entire tumour, and the tumour nucleus without contrast enhancement. RESULTS: The U-Net network produced DICE values of 0.7331 for the tumour core with contrast enhancement, 0.8624 for the total tumour, and 0.7267 for the tumour nucleus without contrast enhancement. The results align with previous studies, demonstrating segmentation accuracy on par with professional radiologists and commercially accessible segmentation tools. CONCLUSION: The study developed a CNN-based automated segmentation system for gliomas, achieving high accuracy in recognising glioma components in MRI images. The results confirm the ability of CNNs to enhance the accuracy of brain tumour diagnoses, suggesting a promising avenue for future research in medical imaging and diagnostics. This advancement is expected to improve diagnostic processes for clinicians and patients by providing more precise and quantitative results.

Abstract:The purpose of this study was to determine whether a deep-learning-based assessment system could facilitate preoperative grading of meningioma. This was a retrospective study conducted at two institutions covering 643 patients. The system, designed with a cascade network structure, was developed using deep-learning technology for automatic tumor detection, visual assessment, and grading prediction. Specifically, a modified U-Net convolutional neural network was first established to segment tumor images. Subsequently, the segmentations were introduced into rendering algorithms for spatial reconstruction and another DenseNet convolutional neural network for grading prediction. The trained models were integrated as a system, and the robustness was tested based on its performance on an external dataset from the second institution involving different magnetic resonance imaging platforms. The results showed that the segment model represented a noteworthy performance with dice coefficients of 0.920 ± 0.009 in the validation group. With accurate segmented tumor images, the rendering model delicately reconstructed the tumor body and clearly displayed the important intracranial vessels. The DenseNet model also achieved high accuracy with an area under the curve of 0.918 ± 0.006 and accuracy of 0.901 ± 0.039 when classifying tumors into low-grade and high-grade meningiomas. Moreover, the system exhibited good performance on the external validation dataset.

Abstract:One of the main requirements of tumor extraction is the annotation and segmentation of tumor boundaries correctly. For this purpose, we present a threefold deep learning architecture. First, classifiers are implemented with a deep convolutional neural network (CNN) and second a region-based convolutional neural network (R-CNN) is performed on the classified images to localize the tumor regions of interest. As the third and final stage, the concentrated tumor boundary is contoured for the segmentation process by using the Chan–Vese segmentation algorithm. As the typical edge detection algorithms based on gradients of pixel intensity tend to fail in the medical image segmentation process, an active contour algorithm defined with the level set function is proposed. Specifically, the Chan–Vese algorithm was applied to detect the tumor boundaries for the segmentation process. To evaluate the performance of the overall system, Dice Score, Rand Index (RI), Variation of Information (VOI), Global Consistency Error (GCE), Boundary Displacement Error (BDE), Mean Absolute Error (MAE), and Peak Signal to Noise Ratio (PSNR) were calculated by comparing the segmented boundary area which is the final output of the proposed, against the demarcations of the subject specialists which is the gold standard. Overall performance of the proposed architecture for both glioma and meningioma segmentation is with an average Dice Score of 0.92 (also, with RI of 0.9936, VOI of 0.0301, GCE of 0.004, BDE of 2.099, PSNR of 77.076, and MAE of 52.946), pointing to the high reliability of the proposed architecture.

Abstract:Background: Meningiomas are the most common primary brain tumors. While most are benign (WHO grade 1) and have a favorable prognosis, up to one-fourth are classified as higher-grade, falling into WHO grade 2 or 3 categories. Recently, an integrated risk score (IRS) pertaining to tumor biology was developed and its prognostic relevance was validated in a large, multicenter study. We hypothesized imaging data to be reflective of the IRS. Thus, we assessed the potential of a machine learning classifier for its noninvasive prediction using preoperative magnetic resonance imaging (MRI). Methods: In total, 160 WHO grade 2 and 3 meningioma patients from 2 university centers were included in this study. All patients underwent surgery with histopathological workup including methylation analysis. Preoperative MRI scans were automatically segmented, and radiomic parameters were extracted. Using a random forest classifier, 3 machine learning classifiers (1 multiclass classifier for IRS and 2 binary classifiers for low-risk and high-risk prediction, respectively) were developed in a training set (120 patients) and independently tested in a hold-out test set (40 patients). Results: Multiclass IRS classification had a test set area under the curve (AUC) of 0.7, mostly driven by the difficulties in clearly separating medium-risk from high-risk patients. Consequently, a classifier predicting low-risk IRS versus medium-/high-risk showed a very high test accuracy of 90% (AUC 0.88). In particular, "sphericity" was associated with low-risk IRS classification. Conclusion: The IRS, in particular molecular low-risk, can be predicted from imaging data with high accuracy, making this important prognostic classification accessible by imaging.
