Abstract:The aim of the present study was to examine the association between shift work and the metabolic syndrome (MetS) using a large-scale longitudinal study design. Data were collected from a historical cohort of health checkups' in the Japanese population. The baseline survey, which. involved 16,952 inhabitants of the Minami Saku area of the Nagano Prefecture, was started in 1978. A nested case-control study was conducted between 1987 and 1990. This analysis was restricted to 6,712 men and women (age range 25-59 years). A conditional logistic regression model was used to estimate the risk of MetS associated with shift work. Compared with the day workers, shift workers had a significantly higher risk of MetS (odds ratio 1.87; 95%CI, 1.13-3.08). Our results demonstrate that shift work was strongly associated with MetS. The study suggests appropriate dietary habits as a basis for managing the MetS risk of shift workers.

Abstract:Shift-work leads to many negative health outcomes among nurses, including overweight/obesity, high stress level, and sleep disturbances. This study purposed to evaluate the influence of shift-work on perceived stress, sleep quality, and Body Mass Index (BMI) among emergency department (ED) nurses in Jordan. A descriptive correlational design was employed. A structured self-reported questionnaire was used to collect data from emergency nurses in government and private hospitals. A total of 450 emergency nurses responded to the questionnaire. Findings found that around 81.1% and 14.0% of the study participants endorsed moderate and high levels of stress, respectively. The majority of the participants (94.5%) had poor sleep quality; 35.3% had overweight and 18.7% had obesity. The study findings indicated that shift-work influenced on perceived stress ( β = 0.18, p < .001 ) and BMI ( β = 0.15, p < .001 ). The mixed shift-workers had lower perceived stress and higher BMI than their counterparts who were doing other shift-work categories. Hence, shift-work had a negative influence on the levels of perceived stress and BMI. Hence, this influence should be taken into consideration when planning interventions and strategies to minimize the negative effects of shift-work.
Shiftwork sleep disorder and associated factors among nurses working at public hospitals in Harari Regional state and Dire Dawa Administration, Eastern Ethiopia: a cross-sectional study

Abstract:Background: Shiftwork sleep disorder is one of the most common health-related effects of Shiftwork, particularly among healthcare workers. It is a chronic condition that is directly related to a person's work schedule. In Ethiopia, although a mental health strategy is in place, little attention is given to studies that focus on shiftwork sleep disorders among nurses. This study aimed to determine the magnitude of shiftwork sleep disorder and associated factors among nurses working at public hospitals in Harari Regional State and the Dire Dawa Administration. Methods: Institutional based cross-sectional study was conducted from June 1-30, 2021 among 392 nurses selected by a simple random sampling technique. A structured interviewer-guided self-administered questionnaire was used for data collection. The International Classification of Sleep Disorders 3rd edition (ICSD-3), Bargen Insomnia Scale (BIS) and Epworth Sleepiness Scale were used to assess shift-work sleep disorder. The data were entered into EpiData and exported to SPSS for analysis. Bivariable logistic regression was used to see the association between the outcome and the explanatory variables. Bivariate and Multivariate analyses were performed, and AOR with 95% CI was used to measure the strength of the association. Those variables with a p-values of < 0.05 were considered as statistically significant. Results: In this study, the magnitude of shiftwork sleep disorder among nurses was 30.4% (95% CI: 25.4-34.5). Being female (AOR = 2.4, 95% CI: 1.3, 4.2), working an average number of nights > 11 per month in the last 12 months (AOR = 2.5, 95% CI: 1.3, 3.8), and khat use in the last 12 months (AOR = 4.9, 95% CI: 2.9, 8.7) were significantly associated with the shiftwork sleep disorder. Conclusions: The study revealed that about one-third of the nurses had a shiftwork sleep disorder implying a high burden of the problem among nurses in the study setting, which endangers nurses, patients, and the healthcare system. Being female, working an average number of nights > 11 per month in the last 12 months, and khat use showed statistically significantly associated with the shiftwork sleep disorder. Early detection of shiftwork sleep disorder, having a policy on khat use and considering rest/recovery while scheduling work time should be addressed to prevent shiftwork sleep disorder.

Abstract:Objective: Shift work is associated with risk for adverse health outcomes including cardiovascular disease, type 2 diabetes, cancer, and obesity. Short sleep duration combined with disruptions to the circadian system may alter factors involved with the behavioral regulation of energy intake and expenditure. We aimed to determine how shift work affects sleep, food intake, and physical activity.Methods: This was a field-based observational study using objective assessments of sleep and physical activity and a 24-hour dietary recall in shift workers. Day (n = 12) and night (n = 12) hospital shift workers (nurses and technicians) who were women had their free-living sleep and physical activity tracked via accelerometry, and completed a computer-assisted 24-hour food recall, during a series of work shifts.Results: Compared to day workers, night workers had significantly shorter sleep duration and reported more premature awakenings and feeling less refreshed upon awakening. Daily self-reported energy and macronutrient intakes were not different between groups, although the night shift workers reported a significantly longer total daily eating duration window than day workers. Objectively recorded physical activity levels were not different between groups.Conclusions: The present findings confirm that sleep is disturbed in women night workers, while there are relatively less effects on objectively recorded physical activity and self-reported food intake. We also observed a prolonged daily eating duration in night vs. day workers. These observations can help inform the design of novel behavioral interventions, including, potentially, time restricted feeding approaches (e.g., by limiting daily eating episodes to within a 10–12 h window), to optimize weight management in shift workers.

Abstract:Progressive nonfluent aphasia (PNFA) is one of the 3 clinical presentations of frontotemporal lobar degeneration (FTLD), the other 2 being frontotemporal dementia and semantic dementia (SD). PNFA and SD, both representing relentlessly progressive language impairment in the realm of FTLD, may share a large part with primary progressive aphasia (PPA). A salient distinction between PPA and PNFA or SD is that PPA includes another clinical type, namely, logopenic/phonemic aphasia (LPA), which is not represented in FTLD. This is primarily because LPA is usually caused by Alzheimer's disease (AD) and the brunt of the lesion is localized at the left temporo-parietal region of the brain. Further, PNFA/SD should be limited to the clinical consequencies of FTLD while PPA is more generous with regard to its causal pathology. By definition, PNFA is an expressive language impairment which is characterized by effortful speech, phonemic errors, grammatical impairment, and word-finding difficulties. Reading and writing may be comparatively impaired. Comprehension of single word meaning is normal, while comprehension of sentencies may sometimes be impaired. PNFA should be differentiated from SD, LPA, and pure progressive apraxia of speech (AOS or alternatively referred to as aphemia or anarthria). SD may be distinguished from PNFA by virtue of its fluency, characteristic loss of word meaning and absence of agrammatism. LPA is similar to PNFA, yet differs in that there is preservation of grammatical skills and speech motor function that is devoid of AOS and/or dysarthria. AOS is an impairment at the level of speech motor programming without language impairment. Thus, there may be a double dissociation between AOS and PNFA i. e., PNFA may or may not accompany AOS and vice versa. PNFA is associated with a localized lesion in the left frontotemporal area of the brain. Immunohistochemical investigations have revealed that ubiquitin/TAR DNA binding protein-43 (TDA-43) positive and tau negative pathology, mostly FTLD with ubiquitin-positive inclusions (FTLD-U) type 3, accounts for 90% of PNFA cases, while the remaining 10% may be caused by tauopathy. Therapeutic attempts for PNFA are currently unsuccessful.

Abstract:BACKGROUND AND PURPOSE:Efforts to improve nurses' physical and mental health are critical to ensuring the safety and quality of the healthcare system. Long-term studies targeting the relevancy of nurses' occupation characteristics with health conditions remain insufficient. This study aimed to examine the relationship between nurses' night shift and sleep problems and metabolic abnormalities risk.METHODS:This study was a part of the National Nurse Health Study, an ambispective cohort study in China, in 2021. Based on an integration physical examination data system, this study carried out a retrospective analysis of 730 nurses from 2018 to 2020 and combined with a questionnaire survey in 2021. The STROBE guidelines were adopted for reporting.RESULTS:In the 23 (23.0, 24.0) months follow-up, higher night shift load was associated with more sleep problems such as shortened sleep duration, sleep disorders, poor sleep quality, and sleep deprivation. Moreover, night shift load was associated with chronic diseases risk factors, increasing body mass index and body fat, with more night shift density, increasing the occurrence of low levels of high-density lipoprotein cholesterol, high triglyceride, triglyceride/high-density lipoprotein cholesterol ratio, and serum uric acid.CONCLUSION:The night shift load has become an occupational health concern, contributing to chronic diseases relevant metabolic risk factors and negative influence on sleep health. Abstract:Objective: The current study was conducted to assess the prevalence and associated factors of shift work sleep disorder among nurses working at Ethiopian federal government hospitals in Addis Ababa. Methods: An institutional-based, cross-sectional study was conducted from April to June 2018 using self-administered structured questionnaires. Computer-generated simple random sampling technique was used to select a total of 422 nurses as study participants. The International Classification of Sleep Disorders - Third Edition, Bargen Insomnia Scale and/or Epworth Sleepiness Scale were used to measure the outcome shift work sleep disorder. Descriptive statistics and bivariate and multivariate binary logistic regression were used. OR with 95% CI was used to measure the strength of association, and a p value <0.05 was considered statistically significant in multivariate analysis. Results: About a quarter (25.6%) of the study participants have shift work sleep disorder. The average number of nights per month for the last 1 year (adjusted OR=6.3, 95% CI 3.513 to 12.35) and working in three-shift rotation (adjusted OR=3.1, 95% CI 1.68 to 5.83) were significantly associated with shift work sleep disorder. Conclusion: A quarter of shift worker nurses are suffering from shift work sleep disorder. The number of nights on average per month for the last 1 year and working in three-shift rotation were significantly associated with shift work sleep disorder. Shift working conditions should be improved to provide less stressful conditions for nurses, ensuring at the same time an appropriate quality of care over a 24-hour period. More studies are necessary to better highlight the burden of shift work on the health and well-being of both nurses and patients.
[The relationship between self-awareness of fatigue symptoms and working conditions in female nurses]

Abstract:Objectives: The aim of this study was to clarify the relationship between self-awareness of fatigue symptoms and working conditions, in female nurses. Methods: We studied personal background, working conditions, physical symptoms, "Pittsburgh Sleep Quality Index - Japanese version (PSQI-J)", and 13 items that represented the parts related to self-awareness of the "the self-Diagnosis Checklist for Workers' Accumulated Fatigue" in 3,855 female nurses. The targets for analysis were 983 nurses ("all subjects") and among those, 765 "shift workers". The total score (fatigue symptoms self-awareness score) was calculated for 13 items that represented the parts related to self-awareness of the "the self-Diagnosis Checklist for Workers' Accumulated Fatigue". The Mann-Whitney U test and the Kruskal-Wallis test were performed, and the fatigue symptoms self-awareness score was studied, based on personal background, working conditions, physical symptoms and the PSQI-J score. Results: For "all subjects", there was a significant difference in the fatigue symptoms self-awareness score for department in which they work, the form the work takes, and the PSQI-J score. The fatigue symptoms self-awareness score of nurses with a PSQI-J score of 5.5 or higher was a significantly high value. For "shift workers", there was a significant difference in the fatigue symptoms self-awareness score for presence or absence of a caregiver role, department in which they work, the form the work takes, the form the nap takes during night work and PSQI-J score. There was a significantly high value in the fatigue symptoms self-awareness score of subjects with a caregiver role, subjects working in surgical wards or internal medicine wards, subjects working on a three-shift system, and subjects who were unable to take naps during night work, subjects with a PSQI-J score of 5.5 or higher. Discussion: These survey results suggest that it is important to set up a system that will ensure the ability to have naps during night work. It will also be necessary to consider the caregiver role and so on as items related to the fatigue of nurses.
Nurses’ Sleep Characteristics by Shift Type in a Tertiary Hospital With Flexible Working Arrangements

Abstract:Objectives: To improve the adaptation of shift work, various flexible shift patterns for nurses have introduced in addition to the existing eight-hour-three-shift system. This study aimed to compare the differences in rotating nurses’ sleep characteristics according to shift patterns. Methods: A total of 62 rotating nurses (all females, mean age 29.79±4.30 years) participated in sleep monitoring for consecutive two weeks. Objective sleep was obtained using Readiband (Fatigue Science, Inc., Vancouver, BC, Canada), a wrist-worn device that uses an accelerometer to distinguish sleep and wakefulness. Subjective sleep characteristics were measured using the Morningness-Eveningness questionnaire (MEQ), Bedtime Procrastination Scale (BPS), Epworth Sleepiness Scale (ESS), and Insomnia Severity Index (ISI). Results: Mean BPS was 30.42±5.10, ESS was 10.42±4.27, and ISI was 11.58±4.16, and there were no statistical differences in sleep parameters except for total sleep time (TST). Although significant differences were not found in sleep parameters according to shift patterns, 27.4% of rotating nurses report subjective sleep problems. More than half (59.7%) suffer from excessive daytime sleepiness. ISI was negatively correlated with age (rho=-0.275, p =0.031) and shift work period (rho=-0.278, p =0.028), but it was positively correlated with ESS (rho=0.306, p =0.015). Furthermore, BPS was negatively correlated with MEQ (rho=-0.351, p =0.005) and TST (rho=-0.307, p =0.016). Conclusions: To improve the sleep of rotating nurses, more active interventions, such as sleep education to reduce bedtime procrastination and short naps during night shifts, are needed.
Comparison of Sleep, Perceived Health, and Job Stress According to Symptoms of Shift Work Disorder in Shift Nurses

Abstract:Objectives: This study aimed to investigate the prevalence of symptoms of shift work disorder (SSWD) and its related factors in rapidly rotating three-shift nurses. Methods: We enrolled 344 nurses (mean age, 28.7 years) without prior history of sleep disturbance before starting shift work in a university-affiliated hospital. SSWD were defined using self-reported sleep questionnaires (insomnia severity index >14 and/or Epworth Sleepiness Scale ≥10) and without any sleep problem before shift work. Sleep pattern in each of the three shifts was measured using the Munich Chronotype Questionnaire for Shift-Workers. Mood (anxiety, depression) and job stress were also measured using self-reported questionnaires. Results: Our results showed that 62.2% of the participants presented SSWD. Compared to the non-SWD group, the SSWD group showed worse sleep hygiene, lower total sleep time (TST) during workdays, higher anxiety and depressive mood, and higher job stress. In the SSWD group, the evening chronotype individuals presented the shortest TST during day shifts, while the morning chronotype individuals presented the shortest TST during evening shifts. Conclusions: SSWD is highly prevalent in fast rotating three-shift nurses. This study suggests that intensive education on sleep hygiene and appropriate scheduling of shift work while considering individual chronotypes may improve sleep patterns and sleep quality of shift workers and minimize SSWD.
Association between shift work and obesity among nurses: A systematic review and meta-analysis

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>The association of shiftwork and the risk of obesity in nurses has been inconsistent in the literature.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>We therefore conducted a systematic review and meta-analysis to quantitatively summarize this association in nurses.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>We systematically searched PubMed, Ovid MEDLINE, Cochrane Library, EMBASE and Web of Science from inception until April 2020 for studies that examined the relationship between shift work and obesity in nurses. Pooled odds ratio (OR) was calculated using a fixed- or random-effect model. The quality of each study was evaluated by using the Risk Of Bias In Non-randomized Studies-of Interventions (ROBINS-I) tool. Two researchers independently assessed eligibility and extracted data. The whole process followed the PRISMA statement.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>We included 11 eligible studies with a total of 74,651 nurses in this study. All of the studies were found to be low or moderate risk of bias according to the ROBINS-I tool. The pooled estimate of the risk of obesity in shift work nurses as compared to non-shift work nurses did not achieve statistical significance (OR = 1.05, 95% CI = 0.97–1.14). However, the risk of obesity was significantly higher in the sub-analysis of night-only shift work nurses (OR = 1.12, 95% CI = 1.03–1.21). In a sub-analysis limited only to the studies of female nurses, the obesity risk was found not to be statistically significant (OR = 1.09, 95% CI = 0.84–1.35). For different regions of study population, shift work was associated with a 36% increased risk of obesity in America (OR = 1.36, 95% CI = 1.30–1.42) and 1% increased risk in Europe and Australia (OR = 1.01, 95% CI = 1.00–1.03). The BMI cut-off for obesity varied in the included studies. When using a cut-off of BMI ≥ 30 kg/m<sup>2</sup> for obesity, our subgroup analysis shows a positive relationship between obesity and nurse shift work (OR = 1.12, 95% CI = 1.03–1.20).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>This is the first systematic review and meta-analysis confirming that shiftwork may play a significant role in the development of obesity among nurses, especially in America, Europe and Australia. This was found to be particularly true for night only shift work nurses. Implementation of health policies and a better night shift schedule are needed in the hospital's management to effectively protect nurses from obesity, and the health risks associated with it.</p>

Abstract:Abstract A high proportion (20%–30%) of shift workers experience Shift Work Disorder (SWD), characterized by chronic sleepiness and/or insomnia associated with work schedules. The reasons for individual variation in shift work tolerance are not well understood, however. The aim of this study was to identify individual factors that contribute to the risk of SWD. Nurses (n = 202) were categorized as low or high risk of SWD based on the Shift Work Disorder Questionnaire. Participants provided demographic and lifestyle information and completed the Sleep Hygiene Index (SHI) and Morningness–Eveningness Questionnaire (MEQ). High risk of SWD was associated with poorer sleep hygiene (SHI, 35.41 ± 6.19 vs. 31.49 ± 7.08, p < .0001) and greater eveningness (MEQ, 34.73 ± 6.13 vs. 37.49 ± 6.45, p = .005) compared to low risk. No other factors, including body mass index, marital status, having children, or caffeine or alcohol intake were significant. Logistic regression showed that SHI was the most significant contributing factor to SWD risk (odds ratio [OR] = 1.09, 95% confidence interval [CI] = 1.04 to 1.14). Standardized odds ratio further revealed that with every unit increase on the SHI score, the odds of being at high risk of SWD increased by 80% (OR = 1.84). Most individuals at high risk of SWD reported “always” or “frequently” going to bed at different times (79%) and waking at different times (83%; compared to 58%, p = .017, and 61%, p = .002, respectively for the low-risk group), as well as going to bed stressed/angry (67% vs. 41%, p < .0001) and/or planning/worrying in bed (54% vs. 22%, p < .0001). Interventions aimed at improving sleep hygiene practices and psychological health of shift workers may help reduce the risk of SWD.

Abstract:Summary Chronotype may affect tolerance for circadian disruption induced by shift work. This study examines the association between chronotype, self‐reported sleep timing, shift type preference, and sleep problems among nurses, and studies chronotype stability over time. The study included 37,731 Dutch female nurses who completed a baseline (2011) and follow‐up questionnaire (2017), with information on shift work (e.g., job history, shift type preference [collected in 2017 only]), and sleep characteristics (e.g., chronotype, preferred sleep–wake time in a work‐free period [collected in 2017 only], and sleep problems between working days according to Medical Outcomes Study‐Sleep Problem Index II [MOS‐SPI‐II]). The association between chronotype and sleep timing was examined using (age‐adjusted) linear regression. Associations between chronotype and shift type preference and sleep problems (MOS‐SPI‐II >30) were examined using ordered logistic and Poisson regression, respectively. With later chronotype, midsleep time increased (definite evening vs. intermediate types [reference]: β = 55 min, 95% confidence interval [95% CI]: 54–55), the odds ratio (OR) for 1‐point increase in preference for night (2.68; 95% CI: 2.48–2.90) and evening shifts increased (OR 2.20; 95% CI: 2.03–2.38), while the odds for day (OR 0.17; 95% CI: 0.16–0.18) and morning shifts (OR 0.22; 95% CI: 0.21–0.24) decreased. Nonrestorative Sleep and Type 2 Diabetes Incidence: the Aichi Workers’ Cohort Study

Abstract:BACKGROUND: The term "nonrestorative sleep (NRS)" refers to unrefreshed feeling at wake-up and is a domain of poor sleep quality. Previous researches have demonstrated that NRS is linked to a number of diseases and adverse health outcomes, but less is known regarding the link between NRS and diabetes, particularly in Japanese.METHODS: We studied 3665 middle-aged male participants of the Aichi Workers' Cohort Study who were followed-up from 2002 to 2019. Cox proportional hazards models estimated hazard ratios (HRs) and 95% confidence intervals (CIs) of incident type 2 diabetes mellitus (T2DM) in relation to NRS adjusted for potential confounding variables.RESULTS: During a median follow-up of 14.6 years, 421 type 2 diabetes cases were identified. Participants with NRS had a higher crude incidence rate of T2DM (11.2/1,000 person-years), compared to participants without NRS (9.3/1,000 person-years). In the fully adjusted model, individuals who reported having NRS had a significantly higher risk of developing T2DM (HR: 1.36, 95% CI: 1.10-1.67). The association was observed only in participants under 50 years old (HR: 1.82, 95% CI: 1.36-2.43), not in the older (50 years or older) participants (P for interaction =0.025). In contrast, stratified analyses by the presence of shift work, obesity or sleep duration showed similar associations in all the strata.CONCLUSIONS: NRS was associated with higher risk of T2DM in middle-aged Japanese male workers independent of a variety of lifestyle factors and other sleep problems.

Abstract:INTRODUCTION: Sleep deprivation and altered circadian rhythm affects the cognitive performance of an individual. Quality of sleep is compromised in those who are frequently involved in extended working hours and shift work which is found to be more common among nurses. Cognitive impairment leads to fatigability, decline in attention and efficiency in their workplace which puts their health and patients' health at risk.AIM: To find out the prevalence of sleep deprivation and its impact on cognition among shift working nurses.MATERIALS AND METHODS: Sleep deprivation among 97 female and three male healthy nurses of age 20-50 years was assessed by Epworth sleepiness scale (ESS). Cognition was assessed by Montreal Cognitive Assessment (MoCA) questionnaire. Mobile applications were used to test their vigilance, reaction time, photographic memory and numerical cognition. The above said parameters were assessed during end of day shift and 3-4 days after start of night shift.RESULTS: Poor sleep quality was observed among 69% of shift working nurses according to ESS scores. The cognitive performance was analysed using Wilcoxon signed rank test. The MoCA score was found to be lesser among 66% of nurses during night (25.72) than day (26.81). During the night, 32% made more mathematical errors. It was also found that, 71%, 83% and 68% of the nurses scored lesser during night in the Stroop's colour test, vigilance test and memory tests respectively. Thus, impairment in cognitive performance was statistically significant (p<0.001) among shift working nurses.CONCLUSION: Cognitive performance was found to be impaired among shift working nurses, due to poor sleep quality and decreased alertness during wake state. Thus, shift work poses significant cognitive risks in work performance of nurses.

Abstract:ABSTRACT. A case of isolated histiocytosis X of the liver is described in a 12‐month‐old child. The patient's symptomcomplex were characterized by fever, tachycardia, hypertension and odd posturing resembling the clinical picture of pheochromocytoma. An extensive workup failed to demonstrate a catecholamine secreting tumor and only histiocytosis X of the liver was found.
Excessive Daytime Sleepiness and Associated Factors in Patients with Type 2 Diabetes Mellitus: a Cross-Sectional Study

Abstract:Aim: This study aimed to assess the prevalence and associated factors of excessive daytime sleepiness (EDS) in patients with type 2 diabetes mellitus (T2DM). Methods: We conducted a cross-sectional study in Beijing, China, from November 2015 to October 2016, and patients with T2DM were invited to participate. Structured questionnaires were used to collect data. EDS was assessed using the Epworth Sleepiness Scale (ESS), and anxiety and depression were assessed using the Hospital Anxiety and Depression Scale (HADS). Logistic regression analysis was used to evaluate factors associated with EDS. Results: Of the 224 patients with T2DM, 24 (10.7%) had EDS. The proportions of anxiety, depression, and comorbid anxiety and depression were significantly different between the EDS and non-EDS groups. ESS scores, anxiety scores, and depression scores were positively correlated. Logistic regression analysis showed that mild anxiety (OR 11.055; 95% CI 2.272–53.785; P = 0.003), moderate to severe anxiety (OR 33.223; 95% CI 4.896–225.440; P < 0.001), and mild depression (OR 6.227; 95% CI 1.319–29.399; P = 0.021) were associated with EDS in patients with T2DM. These associations remained significant after adjustment for age, sex, body mass index, diabetes duration, and apnoea–hypopnoea index. In addition, the severity of obstructive sleep apnoea–hypopnoea syndrome was not significantly associated with EDS, anxiety, depression, and comorbid anxiety and depression in this study. Conclusions: The prevalence of EDS was high in patients with T2DM, and anxiety and depression were significantly associated with EDS. When developing interventions to improve EDS in patients with T2DM, healthcare providers may need to consider interventions that target anxiety and depression.

Abstract:Sleep disturbance and fatigue are prevalent in nurses. Little is known about the characteristics of shift work nurses' sleep-wake features and their subsequent impact on work performance. The study aimed to describe the characteristics of the sleep-wake index, reaction time, saliva cortisol level, and fatigue severity among female shift work nurses. This is a cross‐sectional exploratory study. A convenience sample of 152 female nurses (8-hour day-evening-night, n = 70; 12-hour day–night, n = 82) participated in this study from nine intensive care units (ICUs) from two teaching hospitals in Beijing, China. A consecutive 7-day actigraphy data were used to analyse sleep-wake indexes, including total sleep time (TST) and circadian activity rhythms (CAR). Before and after shifts, the following data were collected, psychomotor vigilance task for reaction time, saliva cortisol for the level of alertness, and self-reported fatigue severity with the Lee Fatigue Scale-Short Form. All nurses reported clinically significant fatigue severity. Compared with the 8-hour shift nurses, the 12-hour shift nurses had significantly more TST (456 vs. 364 min), higher saliva cortisol levels before the day shift (0.54 vs. 0.31), but longer reaction time before the night shift (286 vs. 277 ms). In both shifts, those with better CAR had significantly longer TST. Female nurses experienced fatigue and desynchronized CAR, especially nurses on a 12-hour shift. The CAR-friendly shift work schedule is needed to minimize the health and safety impacts of circadian misalignment for nurses. This is the first use of consecutive 7-day actigraphy data to explore the link between sleep disturbances as a stressor to CAR, salivary cortisol, and reaction time among clinical nurses. CAR may be a helpful indicator for overworked nurses, and it can serve as a modifiable target for interventions to enhance nurses' well-being.

Abstract:Sleep disturbance is common among shift workers, and may be an important factor in the effect of shift work on chronic disease development. In this cross-sectional study, we described sleep patterns of 294 female hospital workers (142 alternating day-night shift workers, 152 day workers) and determined associations between shift work and sleep duration. Rest-activity cycles were recorded with the ActiGraph GT3X+ for 1 week. Analyses were stratified by chronotype of shift workers. Using all study days to calculate average sleep duration, shift workers slept approximately 13 min less than day workers during main sleep periods, while 24-h sleep duration did not differ between day workers and shift workers. Results from age-adjusted models demonstrated that all shift workers, regardless of chronotype, slept 20-30 min less than day workers on day shifts during main and total sleep. Early and intermediate chronotypes working night shifts slept between 114 and 125 min less than day workers, both with regard to the main sleep episode and 24-h sleep duration, while the difference was less pronounced among late chronotypes. When sleep duration on free days was compared between shift workers and day workers, only shift workers with late chronotypes slept less, by approximately 50 min, than day workers during main sleep. Results from this study demonstrate how an alternating day-night shift work schedule impacts sleep negatively among female hospital workers, and the importance of considering chronotype in sleep research among shift workers.
