College Students' Reflections on Their Experience Facilitating a Photovoice Research Project with BIPOC Older Adults and Frontline Healthcare Workers

Abstract:The photovoice literature has focused mainly on research collaborations between researchers in academia and community partners. There is limited evidence of undergraduate students facilitating photovoice research projects with underserved Black, Indigenous, and persons of color (BIPOC). Students' participation in photovoice research increases their understanding of local issues and can empower them to engage with local communities to create change. This qualitative study explored students' experiences facilitating photovoice research with BIPOC older adults and frontline healthcare workers. In this study, we trained college students in photovoice methodological skills by introducing photovoice as a participatory qualitative methodology, explained ethics issues and the informed consent process, and clarified the steps and requirements for the photovoice project. Students were paired with BIPOC older adults and frontline healthcare workers. They facilitated the photovoice research project in four stages. Throughout the study, students critically reflected and wrote about the various aspects of their experience facilitating the photovoice research activities based on weekly structured reflection questions. In their weekly journal, student facilitators commented on how both older and younger BIPOC participants were friendly and kind, flexible with the research activity scheduling, inspiring with their stories, and selfless. Students' reflections indicated that their participants experienced greater emotional and psychological burdens during the COVID-19 pandemic, exacerbated by the long-standing epidemic of systemic racism. Student facilitators overwhelmingly recognized participants' resilience and perseverance despite these life challenges. Our findings highlight the importance of photovoice in developing and strengthening college students' qualitative research skills and stimulating critical thinking and creativity, a sense of critical consciousness about social issues and society, and a sense of empowerment. Our results will be relevant to hospital/long-term care managers, policymakers, and researchers.
Participatory Action Research and Knowledge Dissemination in Virtual Photovoice: Methodological Insights

Abstract:Qualitative Health Research, Ahead of Print. Despite the methodological spread of virtual photovoice, alignments to and potential advances for the participatory action research (PAR) and knowledge dissemination (KD) components of in-person photovoice are poorly understood. Detailing the PAR and KD processes, practices, and products drawn from a virtual photovoice study examining men's experiences of and perspectives about equitable intimate partner relationships, the current article offers three thematic findings. The first theme Processes and pragmatics for selecting representative photographs describes adapting established analytics of preview, review, and cross-photo comparisons to categorize and select images from a large collection of participant-produced photographs (n = 714). Specifically, detailed are the reconciling of researchers deciding which images and accompanying narratives to include guided by PAR principles. Theme 2, Democratizing and disrupting in-person PAR with virtual focus group polls (VFGPs), chronicles participant voting through Zoom to collectively decide and subsequently discuss their favorite photographs. While anonymity for the poll was democratizing in terms of participant equality for voting on the photographs, connecting men virtually from diverse locales could differentiate cultural norms. The third theme KD pledges and pitfalls with online photovoice exhibitions details the potential benefits and challenges for reaching diverse end-users. Evident was the importance of marketing and media for driving traffic to the online exhibition, and the centrality of interactivity for fostering engagement to build and adjust photovoice e-health interventions. With virtual photovoice continuing to grow in popularity post COVID-19, this article offers important methodological lessons for adapting and advancing components of in-person PAR and KD.

Abstract:Journal of Applied Gerontology, Ahead of Print. This paper examines how older adults who participated in an online photovoice-based group intervention program reported their experience. In a qualitative-phenomenological study, in which 13 older-adult people participated, data were collected through semi-structured in-depth interviews and analyzed through content analysis. The findings point to three central themes: a) Challenges—technical difficulties, difficulties in finding a subject for photography, investing time in photography, and an emotional-intellectual effort to put their experience into photography; b) Growth: New knowledge and skills—acquiring new knowledge, acquiring skills, experiencing skills regardless of age, and empowerment; c) Meaning—reflexivity, the ability to project feelings onto images, connection to the outside world, mindfulness, ability to choose, creativity, and critical consciousness. The findings share the way in which the use of creative visual engagement with photography contributed to coping with various challenges and enabled various gains within the process among the older-adult participants.

Abstract:Background The cocreation of eHealth solutions with potential users, or co-design, can help make the solution more acceptable. However, the co-design research approach requires substantial investment, and projects are not always fruitful. Researchers have provided guidelines for the co-design approach, but these are either applicable only in specific situations or not supported by empirical data. Ways to optimize the experience of the co-design process from the point of view of the participants are also missing. Scientific literature in the co-design field generally provides an extrinsic description of the experience of participants in co-design projects. Objective We addressed this issue by describing a co-design project and focusing on the participants’ experiences looking at what was significant from their point of view. Methods We used a qualitative situated cognitive anthropology approach for this study. Data were collected on a co-design research project that aimed to support the help-seeking process of caregivers of functionally dependent older adults. The methodology was based on the perspective of experience by Dewey and used the course-of-action theoretical and methodological framework. Data collection was conducted in 2 phases: observation of participants and recording of sessions and participant self-confrontation interviews using the session recordings. We interviewed 27% (20/74) of the participants. We analyzed the data through nonexclusive emerging categorization of themes using the constant comparative method. Results In total, 5 emerging themes were identified. The perception of extrinsic constraints and the effects of the situation was central and the most important theme, affecting other themes (frustrating interactions with others, learning together, destabilization, and getting personal benefits). Co-occurrences between codes allowed for a visual and narrative understanding of what was significant for the participants during this project. The results highlighted the importance of the role of the research team in preparing and moderating the sessions. They also provided a detailed description of the interactions between participants during the sessions, which is a core aspect of the co-design approach. There were positive and negative aspects of the participants’ experiences during this co-design project. Reflecting on our results, we provided potential affordances to shape the experience of participants in co-design. Conclusions Potential users are an essential component of the co-design research approach. Researchers and designers should seek to offer these users a positive and contributory experience to encourage participation in further co-design initiatives. Future research should explore how the proposed affordances influence the success of the intervention.
A community-academic partnered approach to designing a Virtual Village to address the needs of older adults living with HIV

Abstract:ABSTRACTMany older adults living with HIV face unique challenges, including comorbidities, loneliness, and isolation. This community-academic partnered study elicited viewpoints from older adults living with HIV about the characteristics of a digital environment ("Virtual Village") to combat against loneliness and isolation. We utilized Choice-Based Conjoint Analyses to determine preferred attributes of a Virtual Village. We also conducted focus groups and interviews with older adults living with HIV and used an iterative, data-driven approach to systematically identify emergent themes. Participants (N = 82) were aged 50-82 years and racially/ethnically diverse. The majority were men (78%), gay (66%), and lived with HIV for ≥15 years (83%). Cost was the factor that most drove participants' preference for joining a Virtual Village. Thematic concerns included lack of technological confidence, internet access, potential for harassment in digital environments, privacy, and preference for in-person interactions. Praises centered on convenience and making connections across geographic distances. Participants emphasized the need for purposive strategies to form a cohesive and supportive community for older adults living with HIV. A moderated environment was recommneded to create a safe, structured, and comfortable digital environment for older adults living with HIV. A Virtual Village should be viewed as a bridge to in-person interactions.
Co-designing action-oriented mental health conversations between care providers and ageing Canadians in the community: a participatory mixed-methods study protocol

Abstract:Introduction The mental health of ageing Canadians is a growing concern, particularly post-pandemic. Older adults face systemic ageism and mental health stigma as pervasive barriers to seeking needed mental health support, care and treatment within health and social care systems. These barriers are exacerbated when service providers focus on physical healthcare needs or lack the skills and confidence to talk about and/or address mental health during routine visits. This study aims to co-design and test an evidence-based approach to mental health conversations at the point-of-care in home and community settings with older adults, family and friend caregivers and health and social care providers that could facilitate help-seeking activities and care access. Methods and analysis A participatory mixed-methods study design will be applied, guided by a Working Group of experts-by-experience (n=30). Phase 1 engages ageing Canadians in four online workshops (n=60) and a national survey (n=1000) to adapt an evidence-based visual model of mental health for use with older adults in home and community care. Phase 2 includes six co-design workshops with community providers (n=90) in rural and urban sites across three Canadian provinces to co-design tools, resources and processes for enabling the use of the adapted model as a conversation guide. Phase 3 involves pilot and feasibility testing the co-designed conversations with older adult clients of providers from Phase 2 (n=180). Ethics and dissemination Phases 1 and 2 of this study have received ethics clearance at the University of Waterloo (ORE #44187), University of British Columbia (#H22-02306) and St. Francis Xavier University (#26075). While an overview of Phase 3 is included, details will rely on Phase 2 outcomes. Knowledge mobilisation activities will include peer-reviewed publications, conference presentations, webinars, newsletters, infographics and policy briefs. Interested audiences may include community organisations, policy and decision-makers and health and social care providers.

Abstract:Introduction: People with lived experience of a health issue can be engaged in research to address issues related to social justice, informing change through partnerships and an understanding of community perspectives and needs. Although photovoice has been applied to various disciplines and topics across the health sciences, the concrete design of the photovoice process and participants' experience of engaging in photovoice is not always well documented or understood. Objective: This paper describes youth participants' experiences and perspectives with a youth-led photovoice design process on a study regarding COVID-19 vaccine confidence. Method: The sample consisted of 27 youth aged 14-24 who reported experiencing mental health and/or substance use challenges [MHSU] during the COVID-19 pandemic and some degree of COVID-19 vaccine confidence. Youth participated in a series of photography workshops, then each attended one of the six focus groups about both the topic and experience of the photovoice project. Results: Four themes were constructed from the data: 1) Participating in a photovoice project was an enjoyable experience that had a positive effect on participants; 2) Shared group experiences contributed to building a safe space for participants; 3) Photography and the photovoice process served as a catalyst for reflection; 4) Photovoice shifted participants' perspectives on both the COVID-19 vaccine and photography. Conclusions: This project, a youth-engaged and youth-led photovoice study, describes how the photovoice methodology can be applied in a public health context to meaningfully involve young people and impact their lives. By involving youth in the co-construction of the study design and implementation, photovoice research can represent positive and empowering experiences for participants. Bringing together a diverse and multifaceted lived experience engagement research team structure strengthened the design, delivery, analysis, and interpretation of the project.
The use of photovoice to explore the physical disability experience in older adults with mild cognitive impairment/early dementia

Abstract:Although co-occurring cognitive impairment and physical disability in older adults is common, there is little understanding of how this group perceives their ability to do their daily activities. This study used photovoice to explore how older adults with MCI/early dementia and physical disability without and with care partners (dyads) perceive challenges with their daily activities. Photovoice is a visual research methodology to capture participants' insight on aspects of their daily lives. No known studies have taken this approach to explore the experiences of older adults with MCI/early dementia and co-occurring physical disability. We used a cross-sectional, exploratory research design to understand participants' (n = 12) experiences in their home environment. Photos and participant thoughts on the photos were categorized based on the Blackfoot Breath of Life Theory and the Hierarchy Model of Needs in Dementia, an adaptation of Maslow's Model. Notable findings included: awareness of physical/cognitive difficulties by older adults, solutions for ADL difficulty in persons with MCI/early dementia, care partners' difficulty recognizing pain without the presence of severe emotional or physical responses, reducing mood severity, and self-esteem needs met with structured, memory-issue adapted, meaningful activities. Older adults with co-occurring physical disability and MCI/early dementia identified both physiological and psychological needs despite challenges impacting their cognition. Photovoice evoked daily situations of these individuals and revealed the importance of developing individualized intervention elements for older adults with dementia and physical disability.

Abstract:This paper visually explores older aged care resident's day‐to‐day lived experience, as well as providing a brief introduction to the participatory documentary photography method of photovoice.Ten residents from one Australian residential aged care facility collaborated with researchers and care staff to photograph, reflect on and share their lives. Photographs were shortlisted in individual and group discussions.Residents' photographs and narratives provide a visual understanding of what they value. Photographs captured three core themes: the restorative joy of nature (frangipanis); active engagement through leisure activities (football); and the value of social interactions and connections, especially with fellow residents and staff (friendship).As old age and aged care remain virtually invisible within popular visual culture, this research highlights the communicative value of participatory photography for challenging stereotypes, as well as the opportunities, challenges and value of visual methods with this cohort.

Abstract:Journal of Applied Gerontology, Ahead of Print. Participation in society is a cornerstone for quality of life, active aging, and aging in place. While the majority of older adults prefer aging in place, health and financial challenges can hinder this vision. Conducted in suburban South Jersey, this study utilized mixed methods, including surveys and interviews with older adult participants. Surveys indicated that satisfaction with participation in the community is influenced by functional ability, healthcare service availability, and information access. Interviewees identified suggestions to overcome barriers (e.g., improve access to community and address ageism). Combined results provide a theory of change which suggests older adults' empowerment in community participation hinges on home living support, participation options aligning with ability and interest, and accessible information on community events. This person-centered planning approach emphasizes the importance of older adult and stakeholder participation in foundational community planning, offering translational foundational tools for evidence-based strategies to engage them in future community action plans (CAPs).

Abstract:Abstract There is an emerging body of research on the co-design of public services, including co-design with vulnerable adults. However, what has been less explored has been the impact of digital technology and virtual environments upon the co-design process in this context. This paper analyses the contingencies of virtual co-design through a case study of a project to develop supportive local communities for vulnerable elderly people. This project was initially planned to use traditional co-design methods within a face-to-face environment, in the context of the local public service ecosystem. The CoVid-19 pandemic made this impossible. Consequently, an innovative approach to co-design was developed that shifted the process from a face-to-face to a virtual environment. This exploratory paper reports and evaluates this approach and its implications for the future of the theory and practice of the co-design of public services for vulnerable adults. Theoretically the paper evolves a model of co-design in a virtual space that is embedded within a public service ecosystem framework of value creation. At a practice level, the paper provides insight into the strategic and operational management of co-creation in a virtual space. It evolves the ‘Relay’ model of asynchronous co-creation across time and considers it key contingencies.

Abstract:Background: Participatory Design (PD), albeit an established approach in User-Centered Design, comes with specific challenges when working with older adults as research participants. Addressing these challenges relates to the reflection and negotiation of the positionalities of the researchers and research participants and includes various acts of giving and receiving help. During the COVID-19 pandemic, facets of positionalities and (mutual) care became particularly evident in qualitative and participatory research settings. Objective: The aim of this paper was to systematically analyze care practices of participatory (design) research, which are to different extents practices of the latter. Using a multiyear PD project with older people that had to take place remotely over many months, we specify different practices of care; how they relate to collaborative work in the design project; and represent foundational practices for sustainable, long-term co-design. Our research questions were "How can digitally-mediated PD work during COVID-19 and can we understand such digital PD as 'care'?" Methods: Our data comes from the Joint Programming Initiative "More Years, Better Lives" (JPI MYBL), a European Union project that aims to promote digital literacy and technology appropriation among older adults in domestic settings. It targeted the cocreation, by older adults and university researchers, of a mobile demo kit website with cocreated resources, aimed at improving the understanding of use options of digital tools. Through a series of workshops, a range of current IT products was explored by a group of 21 older adults, which served as the basis for joint cocreative work on generating design ideas and prototypes. We reflect on the PD process and examine how the actors enact and manifest care. Results: The use of digital technology allowed the participatory project to continue during the COVID-19 pandemic and accentuated the digital skills of older adults and the improvement of digital literacy as part of "care." We provide empirically based evidence of PD with older adults developing digital literacy and sensitizing concepts, based on the notion of care by Tronto for differentiating aspects and processes of care. The data suggest that it is not enough to focus solely on the technologies and how they are used; it is also necessary to focus on the social structures in which help is available and in which technologies offer opportunities to do care work. Conclusions: We document that the cocreation of different digital media tools can be used to provide a community with mutual care. Our study demonstrates how research participants effectively enact different forms of care and how such "care" is a necessary basis for a genuinely participatory approach, which became especially meaningful as a form of support during COVID-19. We reflect on how notions of "care" and "caring" that were central to the pandemic response are also central to PD.
Co‐designing a virtual reality social connecting space for older adults living with dementia

Abstract:Background Older adults living with dementia often experience reduced social connectedness and must be supported to enhance or maintain their social connections. Digital technology is increasingly being used to address this area. In particular, virtual reality (VR) has shown promising benefits for sociability despite not being designed for this purpose. Research has highlighted the need to design social VR spaces which can enable multi‐user experiences to foster social connectedness. Such a design process needs to meaningfully involve older adults living with dementia and their support persons from the outset. Methods Action research was used to co‐design a VR social connecting space for older adults living with dementia. The research aimed to explore one's experience and perception of social connectedness and how VR may be used to help older adults living with dementia stay socially connected. Using participatory methods of data collection, older adults living with dementia and their support person had the opportunity to use a VR technology probe and participate in a series of semi‐structured interviews, group meetings and design workshops. Reflexive thematic analysis was used to analyze transcripts alongside observational field notes taken by the researcher. The findings from each phase of data collection were used to inform subsequent research activities. Results The meaning and importance of social connectedness is dynamic and transient. Older adults living with dementia experience several barriers and facilitators to social connectedness and their support person often plays an important role in maintaining existing connections. VR was considered a novel and enticing technology to enhance and maintain one's connections. Older adults living with dementia and their support person expressed preferences for shared experiences and these were translated into the virtual environment (VE). Design preferences to facilitate co‐location and co‐presence in the VE were also integrated into the social connecting space. Conclusion VR was considered an enticing technology to enhance or maintain the social connectedness of older adults living with dementia. Older adults living with dementia and their support person contributed to the design decisions which shaped the development of the social connecting space.

Antipyrine Kinetics in Patients with Primary Biliary Cirrhosis

Abstract:Fourteen antimitochrodrial antibody‐positive patients (13 women, 1 man) with biopsy‐proven primary biliary cirrhosis, aged 40 to 71 years (mean, 57 years) weighing 43 to 102 kg (mean, 63 kg), along with 14 age‐ and sex‐matched healthy controls, received a single 1.0‐ to 1.2‐g dose of intravenous antipyrine. Plasma antipyrine levels were determined during a 12‐ to 24‐hour period. Patients' mean serum chemistry values were: albumin, 3.9 g/dL (range, 3.1–4.4) and total bilirubin, 1.9 mg/dL (range, 0.3–10.9). Seven of the fourteen patients had cirrhosis. Mean kinetic variables for antipyrine in controls and primary biliary cirrhosis patients were: Vd, .54 versus .49 L/kg; half‐life, 12.0 versus 15.1 hours (P < .07); clearance, .55 versus .41 mL/min/kg (P < .04). Within the primary biliary cirrhosis group, there was no correlation between total bilirubin and clearance (r = .09), nor did clearance vary significantly among histologic categories. Clearance of antipyrine in primary biliary cirrhosis patients is reduced by an average of 25%, but the clinical prognosticators of serum bilirubin levels and histologic grade do not correlate with or predict the degree of clearance impairment.
Photovoice: A Strategy to Better Understand the Reproductive and Sexual Health Needs of Young Mothers

Abstract:Study objective: Adolescent women face significant sexual and reproductive health challenges and are more vulnerable than their male peers. Photovoice methodology might allow them to provide more meaningful and accurate representations of the health challenges they encounter. Our objectives were to: (1) use Photovoice to understand how young mothers frame reproductive and sexual health within the context of their lives; (2) explore how they define reproductive and sexual health; (3) identify youth perspectives on how their life situations influence their ability to affect their health; and (4) connect their perspectives to social determinants of health framework to facilitate implementation of effective programs and policies to address their needs. DESIGN, SETTING, PARTICIPANTS, INTERVENTIONS, AND MAIN OUTCOME MEASURES: This was a prospective qualitative community-based participatory research study involving young women (ages 15-25 years) recruited from a local youth outreach center. A 9-step validated qualitative participatory approach that combined documentary photography with focus groups was used. Qualitative analysis was conducted with NVivo version 10 software (QSR International Inc., Burlington, MA, USA). Data were coded and themes were developed. Results: Thirty women were recruited and nine women completed the study. Key themes included: personal sexual health practices and coping skills, influence of poverty, physical environments, community resources and sexual health services, education, and stigma of pregnancy. Participating in community-based participatory research empowered participants to advocate for their own health. Conclusion: Photovoice methodology contributes to understanding complex factors influencing sexual and reproductive health of young mothers. This participatory-based methodology highlights their individual situations, allowing us to seek connections, create analytical perspectives from which to relate their situations to root causes, and consider strategies for change.

Abstract:Photovoice is a process by which people can identify, represent, and enhance their community through a specific photographic technique. As a practice based in the production of knowledge, photovoice has three main goals: (1) to enable people to record and reflect their community's strengths and concerns, (2) to promote critical dialogue and knowledge about important issues through large and small group discussion of photographs, and (3) to reach policymakers. Applying photovoice to public health promotion, the authors describe the methodology and analyze its value for participatory needs assessment. They discuss the development of the photovoice concept, advantages and disadvantages, key elements, participatory analysis, materials and resources, and implications for practice.

From Our Lab to Their Homes: Learnings from Longitudinal Field Research with Older Adults

Abstract:Conducting research with older adults in their home environments presents unique opportunities and challenges that differ significantly from traditional lab-based studies. In this paper, we share our experiences from year-long research activities aiming to design and evaluate conversational voice assistants for older adults through longitudinal deployment, interviews, co-design workshops, and evaluation studies. We discuss the benefits of bringing the lab to their home, including producing realistic and contextual interactions, creating stronger researcher-participant bonds, and enabling participant growth with the research over time. We also detail the difficulties encountered in various aspects of the research process, including recruitment, scheduling, logistics, following study protocols, and study closure. These learnings highlight the complex, yet rewarding, nature of longitudinal home-based research with older adults, offering lessons for future studies aiming to achieve real-world applicability.

Abstract:Increasingly researchers are partnering with citizens and communities in research; less is known about research impacts of this engagement. EMBOLDEN is an evidence-informed, mobility-promoting intervention for older adults co-designed by a 26-person Strategic Guiding Council (SGC) of health/social service providers and older adult citizens. This study evaluated research partners' perceptions of engagement strategies, the engagement context, strengths, areas for improvement, as well as the impacts of the guiding council on older-adult identified priority areas.

Abstract:Journal of Intellectual Disabilities, Ahead of Print. People with intellectual disabilities are often excluded from participation in research, whilst research has the potential to enhance positive change in their lives. In the current study, using a guided photovoice procedure, the experiences of people with intellectual disabilities regarding participation in research are evaluated. A total of 14 participants with intellectual disabilities were interviewed using guided photovoice. Through thematical analyses the experiences of people with intellectual disabilities regarding photovoice are discussed. Benefits were taking time, visual cues and the opportunity to speak out. Challenges were abstract concepts and aftercare. Guided photovoice helps to better understand the perspective of people with intellectual disabilities and thus can help to improve their quality of life. People with intellectual disabilities were interested in taking part in the current research project and experienced participating as a positive experience. However careful planning and expertise in communication with people with intellectual disabilities is necessary to include people with intellectual disabilities in research.
