Abstract:The real-time city is now real! The increasing deployment of sensors and hand-held electronic devices in recent years is allowing a new approach to the study and exploration of the built environment. The WikiCity project deals with the development of real-time location-sensitive tools for the city. It follows the Real Time Rome project that was developed for the 2006 Venice Biennale of Architecture and which aggregated data from cell phones, buses and taxis in Rome to better understand urban dynamics in real-time. The WikiCity project is concerned with the real-time mapping of city dynamics. This mapping however is not limited to representing the city but becomes also instantly an instrument for city inhabitants to base their actions and decisions upon in a better informed manner, leading to an overall increased efficiency and sustainability in making use of the city environment.

Abstract:BACKGROUND: Surgical removal of lesions around the rolandic cortex remains a challenge for neurosurgeons owing to the high risk of neurological deficits. Evaluating the risk factors associated with motor deficits after surgery in this region may help reduce the occurrence of motor deficits. OBJECTIVE: To report our surgical experience in treating epileptic lesions involving the rolandic and perirolandic cortices. METHODS: We performed a single-center retrospective review of patients undergoing epilepsy surgeries with lesions located in the rolandic and perirolandic cortices. Patients with detailed follow-up information were included. The lesion locations, resected regions, and invasive exploration techniques were studied to assess their relationship with postoperative motor deficits. RESULTS: Forty-one patients were included. Twenty-three patients suffered from a transient motor deficit, and 2 had permanent disabilities after surgery. Six patients with lesions at the posterior bank of the precentral sulcus underwent resection, and 5 experienced short-term motor deficits. Two patients with lesions adjacent to the anterior part of the precentral gyrus, in whom the adjacent precentral gyrus was removed, experienced permanent motor deficits. Lesions located at the bottom of the central sulcus and invading the anterior bank of the central sulcus were observed in 3 patients. The patients did not experience permanent motor deficits after surgery. CONCLUSION: The anterior bank of the central sulcus is indispensable for motor function, and destruction of this region would inevitably cause motor deficits. The anterior bank of the precentral gyrus can also be removed without motor impairment if there is a preexisting epileptogenic lesion.

Abstract:Objective: : To summarize th e preliminary experience in microelectrode-guided multi-targets lesion sterota ctic surgery for advanced Parkinson's disease. Methods: The hea ling effects of 24 patients with rigidity and tremor in unilateral and/or bilate ral extremities were observed and evaluated according to the Unified Parkinson's Disease Rating Scale (UPDRS) one week before and after microelectrode-guid ed sterotactic surgery of multi-targets lesion. Follow-up ranged from 1 to 17 months.Results: Following surgery,rigidity tremor and poor reci procal movements were significantly improved. The patients obtained good results during 1-17 months follow-up after surgery. Preoperative and postoperative UP DRS score improved significantly. Conclusion: Microelectrode-g uided sterotactic surgery of multi-targets lesion is safe and feasible for adva nced Parkinson's disease.

Abstract:Introduction: Bilateral stimulation of the subthalamic nucleus (STN) seems to be one of the most effective treatments for advanced forms of Parkinson's disease. Success of the postoperative clinical outcome depends on the quality of the inclusion of patients and the optimal targeting and stimulation of the cerebral nucleus.<p> Case report: We describe the case of a 68 years old male with intracerebral hemorrhage, cognitive deterioration and generalized convulsions in the early postoperative period of a bilateral deep brain stimulation (DBS) of STN. Conclusions: Although this kind of surgery is included in the concept of "minimal invasive" surgery, there are risks and side effects, some of them very serious, as in the case report we present. Perioperative management is complex because of the nature of the surgery and the type of patient, so that some of the aspects are still under debate for both the anesthesiologist and the rest of specialists involved.

Abstract:The photodecomposition of bilirubin in chloroform was studied by spectrophotometry and chromatography. A fraction of the pigment decomposed with retention of the tetrapyrrolic structure, giving biliverdin, chrysin, and choletelin. The rest of the bilirubin decomposed to propentdyopents, imides, a pyrrolic acid, and simple aliphatic acids. Verdins, mesobiliviolin, and i-urobilin treated similarly retained the tetrapyrrolic structure for a longer time.

Abstract:Sustained improvements in surgical technique now allow safe resection of JP with minimal mortality. Yet a major cause of morbidity remains the cranial nerve deficits resulting from both tumor growth and surgical extirpation [1]. These impairments may dramatically influence multiple defining elements of the human condition: vision, hearing, speech, and deglutition. Recovery may be incomplete and often requires a prolonged period of rehabilitation. When multiple cranial nerves are affected, the effects on overall quality of life can be devastating. While those deficits produced by tumor growth generally occur progressively over many months, the acute postsurgical deficit is not nearly as well tolerated.

Abstract:It has been described that treatment of cells with high dose psoralen and UVA induce the production of reactive oxygen species (ROS) leading to DNA damage. Transcription factor nuclear factor kappa B (NFĸB) plays a crucial role in regulating not only cell growth but also cell differentiation, and ROS seem to be partly involved in these mechanisms. The aim of this research was to find out the effect of a combined treatment with trioxsalen (TMP)/UVA on NFĸB binding activity in HaCaT keratinocytes. HaCaT keratinocytes were treated with 27 µg/l TMP. This concentration did not affect the proliferation rates, nor was it toxic, as shown by cytotoxicity assays. After treatment with TMP with or without UVA (1 J/cm2), NFĸB binding activity in nuclear protein extracts was measured by electrophoretic mobility shift assays. The effect on cytokines and cytokine receptor genes was investigated using cDNA expression arrays. An inhibitory effect on NFĸB binding activity was found between 30 and 60 min after TMP supplementation of the culture media. UVA irradiation induced a 2-fold increase in NFĸB binding activity in TMP supplemented HaCaT keratinocytes compared with the non-irradiated control. In addition, NFĸB binding activity was higher after UVA irradiation with TMP than in UVA irradiated cells in the absence of TMP. TGF-α, IL-1R, IL-2Rα, IL-12β and PDGF expression was induced by UVA. However, all of them except PDGF were inhibited by combined TMP/UVA treatment. Using an inhibitor of NFĸB activation, we found out that under these conditions, these cytokines or cytokine receptor genes are apparently not regulated by NFĸB. Our results indicate that a combined TMP/UVA treatment of HaCaT keratinocytes induces NFĸB binding activity, and that this is a synergistic effect. The investigated cytokines, and cytokine receptor genes do not seem to be NFĸB regulated; however, TMP shows anti-inflammatory capacities in vitro.
Surgical Treatment of Propriospinal Myoclonus: A Case Report

Abstract:Propriospinal myoclonus (PSM) is a rare segmental movement disorder characterized by repetitive irregular myoclonic jerks of the trunk and/or axial limbs at the resting state. It is imperative to make a correct diagnosis as other movement disorders can be mistaken for or mask PSM. Therefore, a battery of neuroimaging and neurophysiological testing must be undertaken. In our case report, we discuss a patient who was diagnosed with PSM concurrently with cervical degenerative stenosis and then had a successful outcome via surgical decompression and arthrodesis of the cervical spine. We documented the patient's postoperative course and achievement of complete remission, sustained at a 41-month follow-up. We then grouped our case together with five other PSM cases in the literature to offer readers a broader context of the role of surgical spinal intervention in ameliorating PSM.

Abstract:Essential Tremor is the most common neurological movement disorder. This progressive disease causes uncontrollable rhythmic motions -most often affecting the patient's dominant upper extremity- that occur during volitional movement and make it difficult for the patient to perform everyday tasks. Medication may also become ineffective as the disorder progresses. For many patients, deep brain stimulation (DBS) of the thalamus is an effective means of treating this condition when medication fails. In current use, however, clinicians set the patient's stimulator to apply stimulation at all times- whether it is needed or not. This practice leads to excess power use, and more rapid depletion of batteries that require surgical replacement. In the work described here, for the first time, neural sensing of movement (using chronically-implanted cortical electrodes) is used to enable or disable stimulation for tremor. Therapeutic stimulation is delivered only when the patient is actively using their effected limb, thereby reducing the total stimulation applied, and potentially extending the lifetime of surgically-implanted batteries. This work, which involves both implanted and external subsystems, paves the way for the future fully-implanted closed-loop deep brain stimulators.

Abstract:Background: Based on data for a large population of patients from a single center in China accumulated over 22 years, we revealed an overall profile of movement disorders (MDs) in terms of their demographic features and surgical treatment. Methods: This study was conducted on 21,005 outpatients (2008–2019) and 5,126 inpatients (1998–2019) with MDs who underwent radiofrequency ablation (RFA) or deep-brain stimulation (DBS). Patients were categorized into those with Parkinson’s disease (PD), essential tremor (ET), dystonia, tics, or involuntary movements (IMs). The clinical data of these patients were analyzed retrospectively. Findings: PD accounted for the largest proportion (75·4%) among surgically treated MD patients, and was associated with the highest surgical rate (18·8%), followed by ET (proportion: 12·1%, surgical rate: 10·4%) and dystonia (proportion: 5·7%, surgical rate: 5·7%). The male:female ratio was higher in surgical patients than in outpatients (1·49 vs 1·27 p< 0·001). The surgery ages in patients with PD, ET, dystonia, tics, or IMs was 62 (54–67), 61 (46–68), 40 (26–53), 21 (19–25), and 21 (17–28) years; and the duration of illness (DOI was 6 (4–10), 14 (9–20), 4 (2–10), 11 (7–17), and 17 (10–22) years, respectively. Between 1998 and 2019, the percentage of patients undergoing DBS among surgically treated MD patients increased from 0% to 85%. Interpretation: PD is the most common MD that necessitates surgical treatment, followed by ET and dystonia. Surgical timing is individualized and varies among different types of MDs. Abstract:Purpose: Brain plasticity after multistep surgery in low-grade glioma is highly variable; the neurosurgical approach must be individualised and functional imaging can be used for this purpose. In supplementary motor area complex (SMAC) tumors, the early and adequate functional recovery of patients raises the possibility of a latent bihemispheric or "mirror" cortico-subcortical network, which would develop depending on the needs of each patient. Methods: Functional and DTI-MR data from 4 right-handed patients with left frontal low grade gliomas near the SMAC, who were operated at least in two occasions were collected. The time of the reintervention was variable (18 months- 8 years), related to the tumor growth. All patients were evaluated by a neuropsychologist and imaged before each surgery, in a 3 T MR, with a 24 multichanel head coil Motor and expressive language task-fMRI and DTI sequences were obtained to evidence the main cortico-subcortical components of the SMAC. Data were processed with Brainwave (GE Medical Systems) and with an Iplan Fiber Tracking tool (MEDTRONIC), respectively RESULTS: None of our patients presented permanent neurological deficits after the first or second functional surgery. Three patients with partial or complete resection of the left middle and / or inferior frontal gyrus, and the left frontal aslant tract evidenced new right hemispherical cortical activity. This right shift were not observed in the patient without left middle gyrus resection, indeed with partial absent of the left frontal aslant tract. Conclusion: SMAC is a latent cortico-subcortical bihemispheric network that allows it to reorganize itself in response to specific neurological deficits. We highlight the importance in the cortical reorganization of the left middle frontal gyrus in the SMAC, closely connected with the essential language areas of this region, but also we focused in the potential cortioco-subcortical changes to compensate the functionality of the FAT.

Abstract:Background Surgical resection of vestibular schwannoma (VS) and other benign posterior fossa tumors is often curative if gross total resection is achieved; however, these lesions pose unique challenges due to their intricate location and proximity to critical structures. Approaches for resection often utilize lateral head rotation and neck flexion to optimize the surgical field. However, this may inadvertently occlude cerebral venous drainage, elevating intracranial pressure (ICP) and increasing intraoperative bleeding, potentially increasing the operative risk to patients. Methods We review relevant literature regarding the effects of head rotation and neck flexion on internal jugular vein (IJV) occlusion and ICP and highlight the notion that head rotation and flexion may occlude the ipsilateral IJV, increasing ICP. Subsequently, we propose a novel technique using continuous, real-time monitoring of jugular bulb pressure (JBP) to detect obstructions in jugular venous flow and guide optimal head positioning prior to VS resection. Results As proof of concept, we present a single-patient case in which JBP monitoring was employed to optimize head positioning prior to a VS resection, which shows a significant reduction in JBP compared with traditional positioning. Conclusion This innovative approach offers promise in enhancing the safety and efficacy of intracranial surgery for VS and other neurosurgical procedures. The role of deep brain stimulation as a surgical treatment for Parkinson's disease

Abstract:Patients with advanced Parkinson's disease (PD) frequently suffer disabling motor complications that cannot be satisfactorily controlled with medical therapy. Deep brain stimulation (DBS) has recently been introduced by Benabid and his colleagues in Grenoble, France, as a new surgical procedure for the treatment of PD patients. DBS simulates the effects of a lesion without the need to make a destructive brain lesion. In this procedure, an electrode is implanted in the brain target and connected to a subcutaneous pacemaker. DBS of the ventro-intermediate (Vim) nucleus of the thalamus has been shown to ameliorate tremor in patients with tremor-dominant PD. DBS of the subthalamic nucleus (STN) and globus pallidus pars interna (GPi) have been shown to improve all of the cardinal features of PD and to markedly reduce dyskinesia and motor fluctuations. Adverse events are associated with the surgical procedure, the device, and stimulation, but the procedure is usually well tolerated. On the basis of these findings, the FDA has recently approved unilateral DBS of the Vim for treatment of tremor in PD and is currently considering approval of DBS for STN and GPi. This article reviews existing information with respect to DBS.

Abstract:Patients with anarchic hand (AH) syndrome exhibit involuntary but seemingly purposeful controlesional upper limb movements. Here we report on the case of a patient (AC) presenting with a right AH following a left medial frontal lesion. Previous literature indicated that endogenous movements, particularly in the presence of distractors, are impaired in AH, whereas exogenous movements are spared. In this study we examined exogenous and endogenous (or sequential) movements using a new experimental procedure. Our main aim was to investigate whether the ability to perform sequential movements improves under verbal command as anecdotally observed in patients with AH. Results showed that the performance of AC's right AH was impaired in sequential tasks and that this impairment was improved by verbal command. The observed reduction in errors in sequential tasks under external verbal command was coupled with a compensatory increase in response times.
Characterization of anti-seizure medication reduction and discontinuation rates following epilepsy surgery

Abstract:Objective: Many patients pursue epilepsy surgery with the hope of reducing or stopping anti-seizure medications (ASMs), in addition to reducing their seizure frequency and severity. While ASM decrease is primarily driven by surgical outcomes and patient preferences, preoperative estimates of meaningful ASM reduction or discontinuation are uncertain, especially when accounting for the various forking paths possible following intracranial EEG (iEEG), including resection, neuromodulation, or even the absence of further surgery. Here, we characterize in detail the ASM reduction in a large cohort of patients who underwent iEEG, facilitating proactive, early counseling for a complicated cohort considering surgical treatment. Methods: We identified a multi-institutional cohort of patients who underwent iEEG between 2001 and 2022, with a minimum of two years follow-up. The total number of ASMs prescribed immediately prior to surgery, choice of investigation modality, and subsequent surgical treatment were extracted for each patient. Primary endpoints included decreases in ASM counts from preoperative baseline to various follow-up intervals. Results: A total of 284 patients were followed for a median of 6.0 (range 2,22) years after iEEG surgery. Patients undergoing resection saw an average reduction of ∼ 0.5 ASMs. Patients undergoing neuromodulation saw no decrease and trended towards requiring increased ASM usage during long-term follow-up. Only patients undergoing resection were likely to completely discontinue all ASMs, with an increasing probability over time approaching ∼ 10 %. Up to half of resection patients saw ASM decreases, which was largely stable during long-term follow-up, whereas only a quarter of neuromodulation patients saw a reduction, though their ASM reduction decreased over time. Conclusions: With the increasing use of stereotactic EEG and non-curative neuromodulation procedures, realistic estimates of ASM reduction and discontinuation should be considered preoperatively. Almost half of patients undergoing resective surgery can expect to reduce their ASMs, though only a tenth can expect to discontinue ASMs completely. If reduction is not seen early, it likely does not occur later during long-term follow-up. Less than a third of patients undergoing neuromodulation can expect ASM reduction, and instead most may require increased usage during long-term follow-up.
Intraoperative neurophysiological assessment of disabling symptoms in DBS surgery

Abstract:Introduction: Neurophysiological assessment can provide quantitative measures for the selected motor signs that have been targeted for surgery and may be helpful in predicting the therapeutic effects of deep brain stimulation (DBS) on pathological tremor, motor performance, and rigidity. Objective: To present a survey and demonstrate the contribution of neurophysiological assessment of side effects and effects on disabling motor symptoms at various steps of DBS surgery, and to confirm its role for optimal target localization, as an adjuvant to anatomic imaging. Material and methods: The data result from 192 nuclei in 118 procedures on patients with Parkinson's disease (84), essential tremor (24), Hallenvorder Spatz dystonia (4), multiple sclerosis (4), and Holmes tremor (2). The intraoperative neurophysiological monitoring (IOM) protocol consists of semimicroelectrode recording (for subthalamic nuclei), whereas accelerotransducers and spectral analysis allow assessment of tremor, finger tapping (FT), diadochokinesis (DDK), and determination of the distance between DBS electrodes and internal capsule (IC). Rigidity is assessed by surface EMG recordings in combination with a goniometer. Results: The determination of the functional distance between the DBS electrode and the IC is based on the activation functions of axons in the IC. We show the high sensitivity of accelerometers for tremor over a large part of the body, the relationship between clinical scores and spectral frequencies of FT and DDK. Parkinsonian rigidity can be assessed from surface EMG (sEMG) by means of a balance coefficient, which can detect negative rigidity, for low unified Parkinson's disease rating scale (UPDRS) scores (0-2) and quantified EMG when negative rigidity is excluded. Conclusion: Accelerometer and sEMG recording have shown their value for intraoperative assessment of disabling motor symptoms and side effects during surgery, to optimize the target position electrodes for DBS. The combination with contemporary signal analyzing techniques permit intraoperative monitoring without a significant delay. IONM improves sensitivity and adds objective neurophysiological data.

Abstract:The authors describe a rare case of a pediatric patient with focal inhibitory motor seizures (ictal paralysis) with the seizure focus in the primary motor cortex. Seizures were characterized by ictal paralysis of the contralateral limbs without loss of consciousness. The seizure focus and progression were examined using long-term video electroencephalography (EEG) monitoring and MR imaging. Invasive recordings of the ictal EEG revealed spike rhythms arising from the primary motor cortex, suggesting that seizures associated with paralysis can arise from hyperactivity in either positive or negative motor areas. Resection of the cortical area related to ictal onset resulted in a seizure-free status for the last 3 years.
