Abstract:A 60-year-old Chinese man was admitted to our hospital with chronic cough for > 2 months. His cough was paroxysmal and nonirritating, occasionally productive with some small amounts of white phlegm. He had had a low-grade fever for half a month. There were no night sweats, joint swelling on limbs, pain, rash, or any other discomfort. The patient denied weight loss and decreased appetite.
Non-surgical Management of an Abrupt Cavitation and Large Oval-Shaped Lung Abscess Secondary to Acute Thromboembolic Pulmonary Infarction: a Case Report

Abstract:Infected cavitating pulmonary infarction is a rare complication of pulmonary embolism with a high mortality rate. Surgical excision for this complication has been used in past decades. Abrupt cavitation and a large oval-shaped lung abscess caused by acute thromboembolic pulmonary infarction during anticoagulation are rare. We present a 70-year-old man who suffered from pleuritic pain and breathlessness, accompanied by nausea and vomiting for 1 day. A physical examination showed tachycardia and tachypnea with moist rales in the left upper chest. High D-dimer levels, leukocytosis, respiratory failure and left upper lobe consolidation were found on plain computed tomography (CT). CT pulmonary angiography was performed 2 days after the previous CT scan because pulmonary embolism was suspected. This scan showed emboli in the main, right upper, middle, lower and left upper pulmonary arteries with deteriorated left upper lobe consolidation and cavitation. Thromboembolic pulmonary infarction and an abscess were diagnosed. Enoxaparin 60 mg was administered every 12 hours for 10 days, followed by rivaroxaban, antibiotics and drainage of the hydrothorax. The patient improved after the strategy of non-surgical treatment and was discharged approximately 1 month later. The patient had an uneventful course during rivaroxaban 20 mg once daily for 1 year.
A Giant Ganglion Cyst of Hip Joint Causing Lower Limb Edema

Abstract:Lower limb edema caused by a giant ganglion cyst of hip joint, rather than deep venous insufficiency or lymphedema, is unusual particularly for guiding clinical practice. In this study, we present a rare case of a 76-year-old man with a 3-month history of progressive swelling of right lower extremity and 2-month history of a known inguinal mass, which was initially thought as hemangioma by using ultrasound. But, subsequent computed tomography examination and exploration surgery revealed a giant ganglion cyst compressing the right femoral vein. Resection of the cyst led to complete resolution of symptoms postoperatively. To the best of our knowledge this is a very rare case of giant ganglion cyst of hip joint origin causing lower limb edema in the literature.
Vanishing Lung Syndrome of Uncommon Size

Abstract:A 48-year-old female nonsmoker was admitted to our hospital with a 10-year history of chronic chest tightness and dyspnea. Physical examination revealed decreased breath sounds and hyperresonance to percussion in left lung. chest x-ray showed a hyperlucent left hemithorax with a severe rightward medias-tinal shift, which was suggestive of tension pneumothorax. But high-resolution computed tomography of the chest revealed that several giant bullae occupying almost the entire left hemithorax with severe mediastinal displacement to the right (Figure 1), which was consistent with vanishing lung syndrome. This patient successfully underwent video-assisted thoracoscopic resection of the left giant bullae and is currently without residual symptoms.

Abstract:A 70-year-old man was diagnosed with mid-thoracic esophageal squamous cell carcinoma (distance from incisors, 27-30 cm) because of progressive dysphagia and underwent thoracic laparoscopic esophagectomy at a local hospital. He was transferred from the ICU 4 days after surgery; however, a large amount of purulent fluid exuded from the neck incision after oral drinking, which was consistent with cervical anastomotic leakage. Later, the patient experienced difficulty breathing and expelling sputum; he was then transferred back to the ICU for treatment. A CT scan showed massive fluid collection in the mediastinum and left pleural cavity. Thoracentesis yielded yellowish fluid, and the patient's general condition gradually improved after placement of a closed chest drainage system. The patient's cervical anastomotic fistula persisted and did not heal, and he was subsequently transferred to our medical center with the closed chest drainage system left in place.

Abstract:As you would be aware, COVID-19 has rapidly spread to a number of countries around the world. In our letter, we have reviewed chest imaging from a 36-year male with severe COVID-19. We found that his lung lesions started to gradually diminish after about a month. Besides, we found that invasive mechanical positive pressure ventilation had both positive and negative effects. Lung-protective ventilation strategies should be adopted and full attention given to iatrogenic injuries in the diagnosis and treatment of patients with COVID-19. This case report may have a significant impact on the clinical management of patients with COVID-19.

Abstract:A 77-year-old man was in good health until he complained of fatigue 3 weeks before presentation. Two weeks before admission, he developed gradually worsening shortness of breath. One week before admission, he developed a cough that initially was nonproductive but later was associated with hemoptysis.His past medical history was remarkable for a history of colon cancer (Dukes' stage III), for which he underwent a hemicolectomy and treatment with adjuvant chemotherapy in 1993. He had a myocardial infarction in 1986 and underwent coronary artery bypass surgery in 1999. He also had a history of hypertension, type 2 diabetes, and gout. He smoked in the past but had stopped more than 30 years ago.He was initially evaluated by his primary care physician, who noted that he complained of diaphoresis but denied fevers, chills, or contact with others who were ill. His physical examination was remarkable for bilateral crackles that were more pronounced on the right. A chest radiograph demonstrated bilateral pulmonary infiltrates (Figure 1). He was treated with amoxicillin. The next day, however, his physician noted that his dyspnea had worsened and that his oxygen saturation on room air was poor. He was therefore admitted for further evaluation. The amoxicillin was discontinued, and he was treated with levofloxacin, followed by ceftriaxone and azithromycin as his pulmonary status continued to deteriorate. He received intravenous diuretic agents, which failed to improve his respiratory status. During the initial phase of hospitalization, he was anemic, with a hematocrit of 21.3%. His serum creatinine level, which had been 1.0 mg/dL in 1999, was now 2.5 mg/dL. Urinalysis was remarkable for the presence of numerous red blood cells. His oxygen requirement increased, and he eventually required a 100% nonrebreather mask. A computed tomographic scan of the chest demonstrated prominent alveolar opacities throughout the right upper, middle, and lower lobes, with similar opacities in the left upper and left lower lobes (Figure 2). An echocardiogram showed an ejection fraction of 50%, as well as mild mitral regurgitation. Serologies were remarkable for an antinuclear antibody titer of 1:320 and a P-antineutrophil cytoplasmic antibody (P-ANCA) titer of greater than 1:320. C-ANCA was negative. Anti-glomerular basement membrane and anti-human immunodeficiency virus antibodies were undetectable.

Abstract:A 19-year-old boy with shortness of breath and chest pain after strenuous exercise presented to emergency department . On physical examination, the neck and shoulders appeared to be swollen. There was crepitus on skin palpation. Chest X-ray disclosed diffuse subcutaneous emphysema and pneumomediastinum. CT showed additional finding of air in epidural space. The patient was discharged after 2 days of hospitalisation with conservative treatment uneventfully. Pneumorrhachis is usually caused by abrupt increase in intrathoracic pressure in instance of forceful vomiting, cough or asthma attack in an otherwise healthy young adult. It is usually accompanied with pneumomediastinum. The management of epidural pneumatosis should be tailored according to its primary cause. For most patients with pneumorrhachis associated to a spontaneous pneumomediastinum without neurological symptoms, this condition is generally self-limited. For epidural free air of large volume that causes neurological deficits, surgical laminectomy may be indicated.

Abstract:A 17-year-old man with ulcerative colitis (UC) presented to our hospital with neck pain and fever after vomiting. On examination, a snowflake sensation was noted in the neck. A chest radiograph showed extensive subcutaneous emphysema in the chest. CT scans showed extensive subcutaneous emphysema in the neck, shoulders and axilla, as well as pneumomediastinum and pneumothorax. A diagnosis of pneumomediastinum with exacerbation of UC was made, and he was fasted and treated with antibiotics. Intensive granulocyte and monocyte adsorption apheresis (GMA) was started for exacerbation of UC. His symptoms and the radiological findings of pneumomediastinum improved. He remained in remission on azathioprine. UC is a chronic inflammatory bowel disease (IBD) associated with extraintestinal manifestations (EIM). Very few cases have been complicated by pneumomediastinum. The increase in alveolar pressure due to vomiting and systemic inflammation-related pleural or esophageal damage may cause pneumomediastinum in this case. Prevention of progression to mediastinitis and treatment of exacerbated UC are contradictory. GMA was successful because it was not an immunosuppressive therapy. Our case highlights that rare EIM may complicate exacerbation of UC.

Abstract:Clinical introduction A 63-year-old man with HIV presented with 1 month of dyspnoea and productive cough without orthopnoea. He was afebrile with normal blood pressure, borderline tachycardia and mild pulmonary wheezing. He had exertional hypoxia requiring 4 L per minute of oxygen. No murmurs, jugular venous distention or lower extremity oedema was noted. Clinical evaluation included transthoracic echocardiography (TTE), which showed mild left ventricular hypertrophy with normal size and systolic function. The right ventricle was normal. There was mild aortic insufficiency; other valves were normal. An abnormality was incidentally seen at the aortic root. Abstract:Clinical introduction A man in his 60s presented after syncope associated with palpitations, dyspnoea and fatigue. History was notable for hypertrophic obstructive cardiomyopathy (HOCM) and stage IV non-small cell lung cancer treated with durvalumab (discontinued 4 months prior due to pneumonitis). Two weeks prior, he was diagnosed with influenza A pneumonia and recovered. Vitals demonstrated borderline hypotension (95/66 mm Hg) and tachycardia (103 bpm). Electrocardiography revealed sinus tachycardia with ST-elevation in aVR and diffuse ST-depression in V3-V6. Troponin-I was elevated at 174.14 ng/mL. Cardiac catheterisation revealed no significant coronary disease, 73 mm Hg peak-to-peak left ventricular outflow tract (LVOT) gradient, grade 3 mitral regurgitation, left ventricular hyperkinesis and aortic pressure 65/46 mm Hg. Transthoracic echocardiography demonstrated a peak resting LVOT gradient of 32 mm Hg, increasing to 70 mm Hg during Valsalva manoeuvre. Abstract:Background: Pulmonary sequestration is an uncommon pulmonary disorder. We presented an adult case with recurrent pulmonary infection firstly misdiagnosed as pneumonia, which proved as pulmonary sequestration by enhanced CT scan and CT angiography. Methods: Appropriate laboratory tests, chest CT scan, bronchoscopy, and CT angiography were performed for diagnosis. Results: The white blood cells detected by routine blood test were 11.8 x 10(9)/L, the plain chest CT scan showed the volume of the lower lobe of the left lung decreased and the density increased. Enhanced CT and maximum intensity projection (MIP) algorithms were used for three-dimensional (3D) reconstruction of the images: no abnormally enhanced shadows were seen in the reduced lower lobe of the left lung, and tortuous vascular shadows were seen in the mediastinum. Bronchoscopy showed a narrowing of the opening in the dorsal segment of the lower lobe of the left lung. Thoracic aortography revealed an abnormal arterial supply to the lower left lung, the pathological results of thoracoscopic resection of the lower left lung were pulmonary sequestration. Conclusions: Pulmonary consolidation may be more than a simple pulmonary infection. Physicians should consider the possibility of pulmonary sequestration in patients with recurrent or refractory pneumonia. Enhanced CT findings of abnormal blood vessel supply are helpful for pulmonary sequestration diagnosis, and CT angiography is the gold standard for diagnosis.

Abstract:CASE PRESENTATION: A 44-year-old man with a history of asthma presented with intermittent convulsion of the right limb, fever in the late afternoon, and decreased exercise tolerance over 2 months. Occasional productive cough, no hemoptysis, and weight loss of nearly 6 kg were observed during this period. Neither chemotherapy nor oral immunosuppressive drugs had been administered, and no exposure to toxic substances was known. He was a cook and had smoked approximately one pack of cigarettes per day for the past 20 years. The living environment was relatively humid. The patient presented to a local hospital, where the workup was notable for low-density shadows in the left parieto-occipital lobe and a cavity in the right upper lobe of the lung with bilateral diffuse interlobular septal thickening and multiple patchy ground-glass opacities. The brain and lung lesions were F-18-fluorodeoxyglucose avid on PET/CT scan. Bronchoscopy with BAL and transbronchial biopsy were nondiagnostic. While preparing for another diagnostic procedure, the patient gradually developed increasing dyspnea and more frequent convulsions with the progression of lesions on the follow-up chest CT scan. The patient was transferred to our hospital.

Abstract:A 58-year-old man was admitted with a typical presentation of acute left heart failure. However, the patient showed a partial response to the anti-heart failure therapy. Following admission, a continuous fever was monitored, and a CT scan revealed that multiple opacities on bilateral lungs had progressed. Bronchoscopy was performed, and Coxiella burnetii was detected by Metagenomic next-generation sequencing (mNGS) in bronchoalveolar lavage (BALF), and transbronchial lung biopsy showed organizing pneumonia. Considering that the patient had a history of rabbit breeding and delivery, with some newborn rabbits dying before he became ill, organizing pneumonia secondary to Q fever pneumonia was diagnosed. Anti-Q fever treatment was initiated and the patient's temperature returned to normal. Glucocorticoid was administered after adequate treatment for Q fever. The patient's symptom of dyspnea relieved soon and opacities on CT scan were absorbed remarkably. The final diagnosis was organizing pneumonia secondary to Q fever pneumonia accompanied with left heart failure.

Abstract:A 69-year-old man of Indian origin presented to the chest clinic with an excessive dry cough for 3 months. He did not report any weight loss, haemoptysis or fever. He had no history of recurrent cough, asthma, tuberculosis (TB) or close TB contact. He denied any nasal symptoms or significant reflux-related symptoms. Strange Saucer-Shaped Lung Collapse Successfully Salvaged in Spite of Delayed Intervention

Abstract:A middle-aged woman presented in November 2023 with exertional dyspnea and a chronic cough for three months. She had undergone a repeat mitral valve replacement (MVR) surgery five months prior. She had a tissue MVR in 2016, which degenerated, making her symptomatic, and hence had to be replaced with a metallic valve. As the respiratory symptoms recurred two months post-op, she was evaluated for the integrity of the newly placed mitral valve, which was found to be functioning well. The left ventricular function was well preserved, and she had no vegetation or clots. She was therefore investigated further for other possible causes. Chest X-ray showed a strange saucer-shaped (or disc-shaped) opacity above the middle portion of the right hemidiaphragm, which itself was found to be elevated. An ultrasound of the chest ruled out subpulmonic pleural effusion and confirmed right hemidiaphragm palsy. A computed tomography (CT) scan of the thorax was suggestive of a strange-shaped collapse of the right lower lobe with tortuous air bronchograms and a small intraluminal soft tissue shadow in the right lower lobe bronchus. A bronchoscopy confirmed collapsing segments of the right lower lobe due to external compression. It also ruled out any intrabronchial pathology causing obstruction, effectively confirming that the tissue shadow was probably just a mucus plug. A possible phrenic nerve injury during thoracotomy at the time of MVR was thus concluded. A diaphragmatic plication was advised considering that she had significant orthopnea and low peripheral oxygen saturation and that the collapsed lung would not possibly expand beyond six months or so and could in itself act as a focus for further mucus stagnation, leading to infection and further bronchiectasis, which had probably already started developing. It was difficult to persuade the patient for a third thoracotomy and she took much time to decide. Good counselling, rapport building, and assurance that, although the lung may not fully expand beyond six months, at least the orthopnea would significantly improve, she finally consented to diaphragmatic plication, which was done after 10 months of the MVR surgery. Aggressive post-operative chest physiotherapy and rehabilitation were promptly initiated. The lung completely expanded one month post-op and was thus successfully salvaged.

Abstract:Vanishing lung syndrome (VLS) is an uncommon condition characterized by idiopathic giant bullous emphysema, resulting in the lungs appearing abnormally small on radiological scans. Some case reports have suggested a potential association between the development of this condition in young males, individuals with a history of heavy tobacco smoking, methamphetamine, and marijuana use, and those previously diagnosed with HIV. The primary diagnostic tools for vanishing lung syndrome include initial x-rays and high-resolution CT scans, which play a crucial role in confirming the diagnosis. The management of vanishing lung syndrome varies based on several factors, including the patient's functional status and the size and location of the bullae, with treatment options ranging from conservative approaches to surgical interventions. In this case report, we present the case of a 42-year-old male who was a heavy tobacco smoker, had a history of methamphetamine and marijuana use, and was previously diagnosed with HIV. He initially presented to the emergency department seeking poly-substance detoxification but was incidentally found to have giant bullous emphysema on chest imaging. After stabilizing the patient, he was discharged with instructions to follow up with a pulmonologist in two months.

Abstract:A 47-year-old man was referred to the pulmonary clinic with a 2-year history of productive cough and 3-month history of hemoptysis. Two years ago, his chest CT scan revealed a 2 cm×2 cm well-defined nodule in the right upper lung. His cough was alleviated without treatment. Three months ago, he had a productive cough with the bloody sputum after a running to catch the bus. Physical examination was normal. Complete blood count (CBC) showed an elevated eosinophil count (42.61%). Chest CT scan showed that the enlargement of the right upper lobe nodule (3.4 cm×3.3 cm), with bilateral pathy lesions distributed in the right upper lobe and the left lower lobe. Pathological study of needle specimen biopsy showed the lamellated cyst wall of hydatid cyst, brood capsule formation and hooklet. Pulmonary hydatidosis was diagnosed. It was recommended that the patient should be treated by surgery combined with albendazole. His symptoms relieved and lung nodules were shrinking without treatment after 5 years follow-up.
