Abstract:Simão, Carlos Eduardo De Paiva Campos Nogueira ; Silva, Ana Luiza Ferreira Da ; Araújo, Marcela Emílio De ; Caldas, Sergei Godeiro Fernandes Rabelo ; Objective: The objective of this study was to evaluate the shear bond strength of metal brackets bonded with indirect bonding, under different surface treatment protocols. Material and Methods: 40 bovine teeth were randomly divided into four groups (n = 10), according to the type of surface treatment: G1 = 70% alcohol, G2 = air/water spray, G3 = 100-μm aluminum oxide blasting, G4 = direct boning. After drying, the standard Edgewise central incisor brackets were bonded with light-cured resin. The brackets were moved from the plaster models by means of a transfer tray made with condensation silicone, and bonded to the surface of the enamel with self-curing adhesive. The samples were submitted to shear tests by a universal test machine. Data were analyzed with SPSS 20.0 by the one-way ANOVA test and the Tukey post-test. Results: No statistically significant difference (p=0.174) was observed between the mean forces measured between the group for shear strength values of the groups during the test: G1 (5.33 MPa), G2 (3.52 MPa) and G3 (4.58 MPa). Conclusion: The bracket surface treatment protocols presented similarities in shear bond strength test. However, alcohol 70% and oxide blasting presented higher absolute values of resistance than the water group.

Abstract:GSK3 has gained a considerable attention of researchers in the late 1970s as an inevitable drug target to treat diabetes. Furthermore, it was found to have a key role in the development of diseases like cancer and neurodegeneration (ND). A broad spectrum of GSK3 inhibitors have been discovered from time to time in order to curb these diseases. Inhibition of GSK3 by insulin boosts the dephosphorylation of glycogen synthase, hence its activation to convert UDP glucose into glycogen. Lack of insulin and insulin-resistance is supposed to be the cause of type 2 diabetes (Diabetes mellitus). Additionally, GSK3 stabilizes the components of beta-catenin complex, hence promotes oncogenesis. Phosphorylation of GSK3 by Akt and some other kinases also favours the carcinogenesis. However, in some cases GSK3 has tumor supressing character. GSK3 has been found to have a prominent role in the formation of amyloid plaques and neurofibrillary tangles (abnormal protein accumulations) which are the main suspects of Alzheimer's disease (AD). GSK3 inhibitors have been reported to have amyloidbeta disaggregation property and have been found to promote the adult hippocampal neurogenesis in vivo as well as in vitro. This manuscript thoroughly reviews the involvement of GSK3 in diabetes, cancer and ND. Furthermore, development of GSK3 inhibitors as antidiabetes, anticancer and antineurodegenerative agents focusing mainly on lead optimization has been discussed.
Enamel Deproteinization or Sandblasting for Enamel Reconditioning Before Acid Etching to Enhance the Shear Bond Strength of Metallic Brackets in a Third Bonding: An In Vitro Study

Abstract:Background: Enamel conditioning with 37% phosphoric acid is the most common technique during orthodontic bracket bonding procedures. However, due to the repeated de-bonding of the orthodontic brackets during treatment, other methods were needed to condition the enamel surface and increase the bond strength. This study aimed to compare the effect of conditioning the enamel surface by sandblasting with aluminum oxide particles or 5.25% sodium hypochlorite gel in combination with acid etching compared to acid etching alone on shear bond strength (SBS). Material and methods: One hundred eight extracted upper premolars were randomly divided into three groups according to the conditioning enamel surface method. After the first and second bonding of metal brackets, new metal brackets were bonded with a total-etching adhesive after enamel conditioning using different methods: acid etching only (37% phosphoric acid for 30 seconds) (AE group), sodium hypochlorite associated with acid etching (5.25% NaOCl gel for 60 seconds and then acid etching for 30 seconds) (NaOCl-AE group), and sandblasting associated with acid etching (sandblasting for five seconds and then acid etching for 30 seconds) (SB-AE group). The shear bond strengths of the brackets were tested with a universal testing machine. One-way analysis of variance (ANOVA) and Tukey's honestly significant difference (HSD) tests were used to detect significant differences in shear bond strength among groups at the third bonding. Repeated-measure ANOVA and Bonferroni's tests were used to detect significant differences in shear bond strength among the bonding attempts within each group. Results: 5.25% sodium hypochlorite associated with the acid etching method produced significantly greater shear bond strength than sandblasting associated with acid etching and acid etching only methods at the third bonding (16.40 ± 5.80 MPa, 13.60.47 ± 6.40 MPa, and 9.90 ± 4.40 MPa, respectively; P < 0.001). However, there was no significant difference between the AE and SB-AE groups (P = 0.247). In addition, we found a significant decrease in the shear bond strength within each group after each bonding attempt. Conclusion: Conditioning the enamel surface with 5.25% sodium hypochlorite associated with acid etching produced greater bond strength than conditioning by sandblasting associated with acid etching and acid etching only at the third bonding. The bond strength of the metal bracket decreased with increasing bonding attempts, even with the application of enamel surface conditioning methods.
In vitro Analysis of Shear Bond Strength and Adhesive Remnant Index of Stainless Steel Brackets with Different Adhesive Systems to Enamel.

Abstract:AIM This study aimed at evaluating and comparing the bond strength of stainless steel brackets bonded with multiple orthodontic adhesives. MATERIALS AND METHODS About 60 premolars extracted to create space during orthodontic treatment were included in the present study. Premolar stainless steel brackets of size 0.022" slot with the surface area of bracket base of 11.15 mm2 were used. The premolars were divided randomly into three groups of 20 in each. Group I: Conventional Acid Etching (Transbond XT); group II: Seventh-generation adhesive materials (Xeno V); and group III: Flowable composite (Filtek Z350 XT). Calculation of Adhesive Remnant Index (ARI) was done and shear bond strength was assessed. The probability level of 0.05 was considered as clinically significant. RESULTS The shear bond strength was highest in group II (17.46 ± 1.36), which was followed by group I (15.33 ± 2.78) and the least was shown by group III (13.96 ± 1.44). A statistically significant difference was found between group I vs II, group II vs III, and group III vs II. Utmost prevalence of about 35% was found in Transbond XT with an ARI score of 3 (followed by all adhesive left on the tooth), Filtek Z350 XT with 20%, and the least was found with Xeno V, about 5%. The probability was found to be p = 0.025 among different groups, which is statistically significant according to chi-square test. CONCLUSION The study concluded that the adhesive materials of the seventh generation showed higher shear bond strength than that of the flowable composite and conventional acid etching. The ARI indicated that a minimum amount of the residual adhesive was found on the surface of the enamel after debonding. CLINICAL SIGNIFICANCE This study helps orthodontists to select appropriate adhesive and orthodontic brackets for the benefit of the patient and to distinguish the distribution of stress homogeneously within the layer of cement while loading and subsequently minimizing the damage to the enamel during the debonding of orthodontic brackets.

Abstract:Abstract: Calcium release units (CRUs) are constituted of specialized junctional domains of the sarcoplasmic reticulum (jSR) that bear calcium release channels, also called ryanodine receptors (RyRs). In cardiac muscle, CRUs come in three subtypes that differ in geometry, but have common molecular components. Peripheral couplings are formed by a junction of the jSR with the plasmalemma; dyads occur where the jSR is associated with transverse (T)‐tubules; corbular SR is a jSR domain that is located within the cells and bears RyRs but does not associate with either plasmalemma or T‐tubules. Using transmission electron microscopy, this study followed the formation of CRUs and their accrual of four components: the L‐type channel dihydropyridine receptors (DHPRs) of plasmalemma/T‐tubules; the RyRs of jSR; triadin (Tr) and junctin (JnC), two homologous components of the jSR membrane; and calsequestrin (CSQ), the internal calcium binding proteins. During differentiation, peripheral couplings are formed first and the others follow. RyRs and DHPRs are targeted to subdomains of the CRUs that face each other and are acquired in a concerted manner. Overexpressions of either junction (JnC or Tr) and of CSQ, singly or in conjunction, shed light on the specific role of JnC in the structural development, organization, and maintenance of jSR cisternae and on the independent synthetic pathways and targeting of JnC and CSQ. In addition, the structural cues provided by the overexpression models allow us to define sequential steps in the synthetic pathway for JnC and CSQ and their targeting to the CRUs of differentiating myocardium.
Optimal Enamel Conditioning Strategy for Rebonding Orthodontic Brackets: a Laboratory Study.

Abstract:OBJECTIVE:To compare the conventional etching and primer method (CEP) and the self-etching primer method (SEP) in rebonding brackets.METHODS:Forty human maxillary second premolars extracted for orthodontic purpose were randomly divided into 4 equal groups. Group 1 and Group 2 were bonded using the CEP method; Group 3 and Group 4 using the SEP method. All the brackets were debonded and 40 new brackets were rebonded with four different protocols after surface cleaning: Group 1: CEP + adhesive; Group 2: CEP without etch step + adhesive; Group 3: SEP + adhesive; Group 4: non-acidic primer + adhesive. Then, the shear bond strength (SBS) of each group was tested and the measurements of adhesive remnant index scores (ARI) and SEM examination were performed.RESULTS:The mean SBSs for Group 1, 2, 3 and 4 were 14.18, 6.57, 11.90, 5.91 MPa, respectively. Statistical differences of the SBS existed between Group 1 and 2 (P < 0.05) and between Group 3 and 4 (P < 0.05). No difference was found between Group 1 and 3, or Group 2 and 4.CONCLUSION:Omission of the acid-etching step in rebonding orthodontic brackets may be adequate for the clinical requirement. No differences in SBS and ARI of the rebonded brackets were showed between CEP and SEP methods.
Experimental Bracket Design Performance on Bonding and Polymerization of Orthodontic Composite

Abstract:Objective. To evaluate the enamel bonding ability and orthodontic adhesive resin degree of conversion using the experimental bracket design. Material and Methods. Thirteen bovine teeth were used in the study. The experimental bracket was modified with a translucent region in the center of its body. After enamel etching, Orthocem orthodontic adhesive (FGM, Joinville, Brazil) was applied on the bracket base for bonding. The groups were divided as follows (n = 10 per group): (1) control (CB) with standard brackets and (2) spot bracket (SB) with experimental brackets featuring a 0.8 mm translucent region at the center using carbide bur. Shear bond strength (SBS) was evaluated after 24 hours in a universal testing machine and adhesive remnant index (ARI). The degree of conversion (DC) was analyzed using Raman spectroscopy (n = 3 per group). Data were then analyzed using Student's t‐test and Mann–Whitney statistical methods. Results. The SB group exhibited a higher mean SBS (10.33 MPa) compared to the CB Group (8.77 MPa). However, there was no statistical difference between the groups (p = 0.376). Both SB and CB groups had a mean ARI score of 1. Raman analysis revealed a higher degree of conversion in the SB group (49.3%) compared to the CB group (25.9%). Conclusions. The experimental support showed a higher degree of adhesive conversion, although there was no significant increase in bond strength.

The Effect of Surface Treatments on Shear Bond Strength between Orthodontic Metal Bracket and Porcelain Face

Abstract:Objectives: To report on the different approaches taken to improve the bonding of orthodontic brackets to porcelain surfaces and review the literature and test the available alternatives of hydrofluoric acid use. Materials and Methods: The search engine PubMed, Scopus, Google Scholar and Science Direct were used for this study to obtain relevant information. The sample comprises 18 Porcelain fused to metal crowns, etchant acids like Hydrofluoric Acid 9.6%; Orthophosphoric Acid 37%; Maleic Acid 10, 20, 30%. Acrylic tooth holder, high-speed hand piece, round bur, bonding agent, composite, brackets, bracket holder, light cure, and Universal Testing Machine were all used to conduct the study at Ajman University Prosthodontic Laboratory. Results: The highest shear bond strength value was observed for sandblasting without any acid etching. Maleic Acid 30% were found to have the highest shear strength amongst all other acids after 1-minute etching, while Maleic Acid 20% showed the highest value after etching for 2-minutes. Orthophosphoric Acid 37% showed the highest shear strength compared to the other acids after 3-minutes etching. Conclusions: Two essential factors to be considered when bonding in orthodontics is the concentration of acid being used and the amount of time subjected to the surface. Sandblasting alone without using any acid etching has the highest value, and it hails to be one of the most effective ways of increasing bond strength. Furthermore, compared with all the acids we have used, Maleic acid of 30% has shown the highest shear strength after 1-minute of etching.
Incorporation of Zirconium Oxide Nanoparticles in Adhesive Resin for Bonding of Brackets to Enamel Conditioned with Er,Cr:YSGG, Photodynamic Therapy, and Phosphoric Acid

Abstract:Objective: To evaluate the effect of enamel conditioning methods, that is, total-etch and rinse (TER), Er,Cr:YSGG (ECYL), and photodynamic therapy (PDT) on the shear bond strength (SBS) of orthodontic metallic brackets bonded using Zirconium oxide experimental adhesive (ZOEA). Methods: Sixty human incisor buccal surfaces were cleaned and allocated into three groups based on the method of enamel surface treatment, that is, TER using 37% phosphoric acid gel, methylene blue photosensitizer activated by PDT and ECYL (n = 20 each). Each group was further divided into two subgroups (n = 10) based on the type of adhesive, that is, ZOEA and experimental adhesive (EA). Metallic brackets were seated using composite resin. Samples were placed in a universal testing machine for SBS and failure mode using the ARI index. One-way analysis of variance and post hoc Tukey were used for multiple comparisons. ARI was presented in percentages in different investigated groups. Results: TER+ZOEA (17.16 ± 0.41 MPa) displayed the highest bond integrity. However, group PDT+EA (11.34 ± 0.25 MPa) demonstrated the lowest bond scores. The intergroup comparison revealed that the TER system displayed significantly higher SBS values than the PDT and ECYL groups (p < 0.05). The intragroup comparison revealed that enamel conditioned with TER and brackets bonded with ZOEA and EA displayed comparable bond integrity (p > 0.05). Conclusions: Enamel conditioned with TER bonded to a metallic bracket displayed better bond strength than PDT and ECYL. Zirconium oxide nanoparticles incorporated in adhesive have proved to be promising in improving the bond integrity of adhesive.
Assessment of enamel surface roughness and hardness with metal and ceramic orthodontic brackets using different etching and adhesive systems: An in vitro study

Abstract:Background: This study aimed to evaluate enamel surface roughness and microhardness following the use of different bracket materials (metal or ceramic), etchants (total- and self-etchants), and adhesive systems (precoated or flash-free). Method: A total of 99 extracted human premolars were selected for the analysis. The surface roughness was first assessed (roughness control). One specimen from each subgroup was examined using a scanning electron microscope to illustrate the surface topography. Eighty-eight teeth were prepared using total- or self-etchants and bonded to precoated or flash-free adhesive metal or ceramic brackets. The remaining 11 specimens were not bonded to brackets (microhardness controls). The brackets were debonded after immersion in distilled water for 24 h. The specimens were again scanned for surface roughness and topography imaging. Finally, the microhardness was assessed using a micro-Vickers hardness test at a force of 200 g for 10 s. Result: An overall statistically significant increase in surface roughness and reduced surface microhardness were observed in all experimental groups when compared with those in the control groups. The etchant type was the only variable found to contribute to the measured surface properties, with increased roughness and reduced microhardness introduced by total-etching compared to those by self-etching. Conclusion: Orthodontic brackets introduced a significant increase in enamel surface roughness and reduce microhardness compared with untreated enamel, regardless of the bracket material, etchant type, and adhesive system. The etchant type was the only variable contributing to these changes, with total etching having a more pronounced effect.

Abstract:Background: In the search for alternatives to increase the bond strength of brackets, when necessary, this study proposes to apply the enamel deproteinization protocol to eliminate the proteins in the surface enamel to achieve better etching patterns and thereby increase bond strength. The aim of this study was to evaluate the effect of sodium hypochlorite as a deproteinizing agent on the bond strength of metal brackets. Material and methods: Forty bovine teeth were randomly and equally divided into two groups. The experimental group (n=20) underwent a deproteinization treatment with sodium hypochlorite at 5.25% for 60 seconds prior to acid etching of the enamel with 37% phosphoric acid and bracket bonding with Transbond XTTM resin. The control group (n=20) underwent enamel etching with 37% phosphoric acid and bracket bonding with Transbond XTTM resin. Shear strength and the adhesive remnant index were evaluated and the Students t and Chi-square tests were used (P<0.05). Results: The bond strength values of the control group (27.72±6.42 Mpa) were not significantly lower compared to the experimental group (29.21± 7.96 Mpa) (p=0.259). The adhesive remnant index showed a similar behavior in both groups, with the amount of adhesive remaining on the enamel being less than 50% in most samples of both the control and experimental group. Conclusions: Deproteinization treatment of bovine tooth enamel with 5.25% sodium hypochlorite for 60 seconds prior to enamel acid etching does not improve the bond strength of a resin in orthodontic bracket bonding. Correlation between dental enamel chemical composition and bracket debonding, comparing adhesive systems through a scanning electron microscope

Abstract:Literature reports indicate that during bracket removal there can be enamel damage. We compare the shear bond strength (SBS) and tooth enamel loss of four adhesive systems and identify the Ca/P ratio. Then a total of 20 premolars were divided into four groups of five each. After prophylaxis, photographs were taken at 35× with a scanning electron microscope (SEM) and analyzed with X-ray spectroscopy (EDS) at 250×. Brackets were bonded with Transbond™ MIP(G1), Transbond™ PLUS SEP(G2), Enlight(G3) and Stylus®(G4) adhesives, 24 h after were debonded with a Instron universal testing machine at 1 mm/min. All the brackets were photographed with the SEM. The amount of lost enamel was measured with AutoCad. All the results were measured with a significance level p < .05. The SBS general average at debonding was 7.94 ± 2.26 MPa, meanwhile the SBS for G1, G2, G3 and G4 was 9.38 ± 1.46, 6.28 ± 0.69, 9.08 ± 2.45 and 7.04 ± 2.64 MPa respectively. 90% of the samples had no enamel loss, 10% had enamel loss. Only two samples in G1 presented an enamel loss area of 0.34mm2 and 0.80mm2 respectively. From EDS analysis, the Ca/P ratio was 1.6 ± 0.05, 1.61 ± 0.03, 1.64 ± 0.83 and 1.59 ± 0.07 for G1, G2, G3 and G4 respectively; no statistically significant differences were found. We conclude that no association was found between the Ca/P ratio and enamel damage when brackets are removed. HIGHLIGHTS: Where enamel is lost, we observe fractures, steps, horizontal and vertical enamel loss. There is a loss of tooth enamel from 0.34 to 0.80 mm2 with Transbond PLUS SEP. Structural loss of enamel is almost inevitable during the separation of the bracket.

Abstract:Objectives To evaluate the effects of the phosphoric acid (PA) etching, self-etching technique (SE) and blasting with Al 2 O 3 particles (BL) on the bonding of a dental adhesive to intact (INT) or abraded (ABR) enamel. Methods Enamel surfaces were treated as follows: 1- ABR-PA: INT was abraded with SiC paper and etched with PA (20 s) before Clearfil Universal Bond Quick adhesive application; 2- ABR-SE: ABR was SiC and adhesive applied in SE mode; 3- INT-PA: INT was etched with PA and adhesive applied; 4- INT-SE: the adhesive (SE mode) was applied to INT; 5- INT-BL: INT was BL and the adhesive was applied (SE mode), and 6- INT-BA: INT was BL, etched with PA and adhesive applied (SE mode). The enamel surface treated was examined with scanning electron microscopy (SEM) (n = 3) and Al 2 O 3 particles were characterized using SEM and EDX. The enamel bond strength was measured by microtensile test (24 h and 1 year) (n = 8) and the morphology of enamel-adhesive interfaces were analyzed by SEM (n = 3). Bond strength data were analyzed by two-way ANOVA and Tukey's test (α = 0.05). Results Al 2 O 3 particles had an irregular shape, their length varied (50–20 μm) and the perimeter mean was 38.8 μm. The enamel morphology significantly influenced the enamel bond strength. ABR-PA, INT-BL, and INT-BA provided greater and stable enamel-dentin interaction and bond strength. Significance The enamel morphology significantly influenced the enamel bond strength. Using the adhesive in etch-and-rinse mode, enamel must be abraded before etching and must be Al 2 O 3 -blasted when used in SE mode.

Abstract:Introduction: In orthodontics, shear bond strength plays an important role because it provides a good bond between the brackets and tooth surface; it avoids fracture of the tooth surface and prevents debonding of brackets from the tooth surface. All of these allow sufficient treatment time. Many factors, including the adhesive, its thickness, its strength, the bonding procedure, the clinician's ability, the base design, the geometry of the bracket, the material, and the kind of bracket all contribute to the shear bond strength. Brackets joined using conventional adhesive and adhesive pre-coated (APC) flash-free glue were the subjects of this comparison and evaluation research, which aimed to measure shear bond strength, enamel microfracture, and adhesive residual index. Method: 60 recently removed premolars from humans were used in this investigation. Before mounting on the acrylic block, the teeth were meticulously cleaned and preserved in artificial saliva. Two groups were formed from the collected premolars the control group and the experimental group. For the control group, we used American Orthodontics (AO) Master/Mini Master series brackets glued with resin composite kits. To make sure the adhesive was uniformly thick, we flashed extra adhesive around the brackets. In the meantime, samples were bonded using 3M Unitek APC flash-free technology in the experimental group. Results: The research indicated that there was a statistically significant difference between the two groups to the adhesive remnant index (ARI) and mean shear bond strength. The shear bond strength of the experimental group averaged 10.96 megapascals (MPa), whereas the control group's was 5.70 MPa. The control group's ARI score was 2.97, whereas the experimental group's score was 2.4. There was no statistically significant change seen in enamel microfracture. Conclusion: A more robust shear bond may be possible using APC flash-free brackets. Compared to conventional bonding techniques and brackets, APC flash-free brackets have a lower adhesive residual index. The APC flash-free bracket technology also causes more enamel microfracture than conventional bonding and bracketing methods.

Abstract:Introduction: With advances in materials for bonding in orthodontics, errors regarding amount of adhesive to be used can be controlled. Adhesive Precoated (APCTM) Brackets were introduced to reduce the step of applying conventional adhesive on base of the bracket. Moreover, APCTM Flash Free Adhesive System (FFAS) eliminated the step of removal of excessive adhesive around the brackets. Aim: To compare Shear Bond Strength (SBS), Bonding Time (BT) and Adhesive Remnant Index (ARI) between Conventional Adhesive System (CAS) and APCTM FFAS using metal brackets Materials and Methods: This in-vitro study was carried out in the Department of Orthodontics and Dentofacial Orthopaedics at Bharati Vidyapeeth (Deemed to be University) Dental College and Hospital, Sangli, Maharashtra, India. A total of 78 teeth with metal braces were split into two groups, with 39 samples each receiving treatment with the CAS and the APCTM FFAS. Shear bond forces were applied to each sample using a universal testing machine and recorded in Megapascals (MPa) to provide an indication of SBS. Both Groups’ BT was quantified in terms of seconds. The stereomicroscope indexes of Artun J and Bergland S were used to determine ARI. Microsoft Excel was used for data entry, while Statistical Package for Social Sciences (SPSS) version 24.0 was used for analysis. The normality of the data was tested using two different t-tests. In terms of mean and standard deviation, descriptive statistics were represented. A significant level of 0.05 was used. Results: SBS values were 10.35±3.55 MPa and 11.23±3.82 MPa in CAS and FFAS respectively. No significant difference was found among the two groups (p≤0.29) in SBS. BT was significantly (p≤0.001) less in FFAS (95.54±8.72 seconds) compared to CAS (140.85±16.62 seconds). ARI was significantly (p≤0.002) less in FFAS (1.79±0.80) in comparison with CAS (1.23±0.74). Conclusion: FFAS brackets perform better in comparison to CAS in case of BT and ARI. Both groups show no significant difference in SBS.

Abstract:Background: In view of the increasing demand of adult orthodontics for esthetic purposes, adult treatment with brackets has become an important issue. One essential factor for the quality of such treatment is bracket bonding on ceramics. For testing the adhesive bond between the bracket and the ceramic surface it is important to consider the static or cyclic loading that goes along with it. Methods: Metallic Brackets were adhesively fixed on zirconia ceramic blocks in a simulated leveling phase using two different primers (Monobond S and Monobond Etch & Prime). Half of the metallic brackets were activated using a 0.14-nickel titanium wire, while the other half remained non-activated. Shear bond testing (SBT) was performed after thermocycling. Furthermore the Adhesive Remnant Index (ARI) was analyzed. Results: SBT resulted in significantly higher shear bond values when Monobond Etch & Prime was used compared to the use of Monobond S. Activation of the brackets did not show different results in comparison to the non-activated brackets. The ARI did not indicate cement remnants on the ceramic surface, regardless of the primer and the activation status. Conclusions: The use of Monobond Etch & Prime has great potential for the bonding of brackets on dental zirconia ceramics.
Comparison of a recently developed nanofiller self-etching primer adhesive with other self-etching primers and conventional acid etching

Abstract:The purpose of this ex vivo study was to compare the bond strengths and modes of failure of brackets bonded with three self-etching products [Adper Prompt L-Pop (3M Espe), Futurabond NR (Voco) and Transbond Plus (3M Unitek)] and a conventional 38 per cent phosphoric acid-etching system Etch-Rite (Pulpdent Corp.) at two different time points. Ninety-six defect-free premolars were randomly allocated to eight groups (n = 12). The etching procedure was carried out according to the manufacturers' instructions. The shear bond strengths (SBSs) were measured after storage in distilled water for 12 and 24 hours. The brackets were debonded using a universal testing machine with a cross-head speed of 0.5 mm/minute. Multiple comparisons of the SBSs for the different etching types were performed using analysis of variance. The chi-square test was used to evaluate differences in adhesive remnant index (CARI) scores among the groups. The differences between SBSs analysed at 12 and 24 hours were not significantly different. Although a significant difference (P < 0.01) was observed with conventional and self-etching primer (SEP) adhesive systems, SEP revealed bond strengths higher than clinically accepted limits (6-8 MPa). SEP systems can achieve successful orthodontic bond strengths. The rate of development of the bond strength must be determined to ensure sufficient maturation of orthodontic adhesives before functional loading.
Shear bond strength in two types of indirect orthodontic cementation

Abstract:Background: To compare the shear bond strength of brackets cemented to dental enamel according to the cementation techniques. Material and methods: Experimental study. We used 90 premolars and placed them in printed polylactic acid (PLA) filament models to simulate the dental arch shape and to then cement brackets using the direct, indirect technique with Transbond™XT and indirect technique with Orthocem®. Then, we carried out a shear bond strength test using a universal testing machine, and we evaluated the enamel surface using the adhesive resin remaining index. Dunn's test was used for the inferential statistical analysis of shear bond strength, and Fisher's exact test was used for the adhesive resin remaining index. Results: The shear bond strength of the brackets recorded mean values of 16.74±4.48Mpa, 15.93±6.49Mpa and 12.09±5.07Mpa in the direct, indirect technique with Transbond™XT and indirect technique with Orthocem® respectively. At an inferential level, a lower statistically significant difference was found in the indirect group with OrthoCem® in contrast to the other two groups. In the evaluation of resin remaining after detachment, the direct technique group registered 46.7% of teeth with more than half of resin remaining and the indirect technique groups with Transbond™XT and Orthocem® registered less than half of resin remaining with an incidence of 53.3% and 43.3% respectively. At an inferential level, a statistically significant difference between groups was evidenced. Conclusions: The indirect cementation technique using Transbond™ XT is more recommended since it presents a higher shear bond strength than using Orthocem®. Abstract:PurposeBond strength which is as high as that of acid etching under in vitro conditions was provided by the laser which can be used more simply with a shorter clinical time than the acid etching. The aim of this study was to compare different orthodontic adhesives with the efficacy of Er: YAG laser for enamel conditioning. The effects of these adhesives’ types (single component orthodontic adhesives and glass carbomer cement) with lasers have not been studied previously.MethodsA total of 80 specimens were etched by irradiation of Er: YAG laser with 1.2-W power at 10 Hz and divided randomly into four groups (n = 20): group 1, bonding with Transbond XT primer with Transbond XT composite; group 2, Biofix; group 3, GC ortho connect; group 4; glass carbomer cement. Metal brackets were then bonded with adhesives. Shear bond strength tests were conducted on them in a universal testing machine after 24 h of water storage. A stereomicroscope was used to calculate the adhesive remnant index (ARI).ResultsThe statistical analysis of the data was done. The mean and standard deviation of shear bond strength were 11.67 ± 3.22, 9.78 ± 3.68, 7.85 ± 1.90, and 5.06 ± 1.72 MPa in groups 1 to 4, respectively. According to the ARI scores, there was a main mixture of the bond failure modes in all groups.ConclusionsThe optimal shear bond strength was yielded by Er: YAG lasers with the mentioned parameters, which may be used as another option for acid etching of the bracket bond to composite with single-component adhesive systems.

Abstract:PURPOSE Chemotherapy for non-small-cell lung cancer (NSCLC) remains controversial. We describe the two largest reported, randomized, parallel trials designed to determine whether the addition of chemotherapy influences duration and quality of life in localized, unresectable (mitomycin, ifosfamide, cisplatin [MIC]1 trial) and extensive (MIC2 trial) disease. PATIENTS AND METHODS Ambulatory patients with NSCLC, aged 75 years or younger, with localized disease, were randomized in MIC1 to receive up to four cycles of chemotherapy (CT: mitomycin 6 mg/m(2), ifosfamide 3 g/m(2), and cisplatin 50 mg/m(2)) every 21 days, followed by radical radiotherapy (CT + RT) or radiotherapy (RT) alone. Extensive-stage patients were randomized in MIC2 to identical chemotherapy plus palliative care (CT + PC) or palliative care (PC) alone. Short-term change in quality of life (QOL) was assessed in a subgroup of patients. Data from the two trials were combined to allow multivariate and stratified survival analyses. RESULTS Seven hundred ninety-seven eligible patients were randomized, 446 in MIC1 and 351 in MIC2. MIC CT improved survival in both trials (significantly in MIC2). The median survival time in MIC1 was 11.7 months (CT + RT) versus 9.7 months (RT alone) (P =.14); whereas in MIC2, median survival time was 6.7 months (CT + PC) compared with 4. 8 months (PC alone) (P =.03). QOL, assessed in 134 patients from start of trial to week 6, showed improvement with chemotherapy and deterioration with standard treatment. In the combined analysis of 797 randomized patients, the positive effect of MIC on survival was significant overall (P =.01) and after adjusting for prognostic factors (P =.01). CONCLUSION MIC chemotherapy prolongs survival in unresectable NSCLC without compromising QOL.
