Abstract:Background: The widespread adoption of smartphones has transformed global communication but raised health concerns like Text Neck Syndrome - a musculoskeletal condition arising from prolonged device use, causing discomfort in the cervical spine. This study investigates its prevalence and associated factors among smartphone users in Jeddah, Saudi Arabia, focusing on usage patterns, symptoms, and awareness. Methods: This cross-sectional online survey was conducted from June 1 to June 30, 2024. Data on demographics, smartphone habits, symptoms, and Text Neck Syndrome awareness were collected using a structured questionnaire. Statistical analysis involved descriptive statistics and chi-square tests. Results: The study included 421 participants, predominantly female (279, 66.3%) and Saudi nationals (397, 94.3%). The largest age group was 21-40 years (308, 73.2%), and most were single (251, 59.6%) with a college degree (236, 56.1%). Over 42.0% of participants used smartphones for more than five hours daily, with 39.4% adopting a 30-degree neck posture. The most prevalent symptoms reported were neck pain (272, 64.6%), headaches (203, 48.2%), and shoulder pain (178, 42.3%). Awareness of Text Neck Syndrome was reported by 197 participants (46.8%), but only 60 (14.3%) had been diagnosed. Concerns about long-term complications such as osteoporosis (105, 24.9%) and prolapsed intervertebral disc (120, 28.5%) were expressed. Despite these concerns, 97.9% of participants hesitated to reduce smartphone usage due to reported symptoms. Conclusion: This study highlights the significant musculoskeletal impact of smartphone use in Jeddah, underscoring the need for interventions promoting ergonomic practices and increasing awareness about associated risks. Public health strategies should focus on promoting ergonomic practices and educating users about preventive measures.
Prevalence of text neck syndrome, its impact on neck dysfunction, and its associated factors among medical students: A cross-sectional study

Abstract:BACKGROUND: Students increasingly rely on digital devices, leading to text neck syndrome, a common overuse syndrome caused by repetitive forward neck flexion. OBJECTIVE: This study aimed to determine the prevalence of text neck syndrome among medical students and the resulting neck dysfunction and to investigate the possible associated factors. METHODS: This cross-sectional study used an online self-developed questionnaire among medical students. Students’ characteristics and data about smartphone usage were evaluated for diagnosis. Individuals with at least 3 of the 6 text neck syndrome symptoms and a history of more than four hours a day spent on a smartphone were diagnosed with text neck syndrome. Neck dysfunction was measured using the neck disability index. Descriptive statistics and the chi-square test were used. P values < 0.05 were considered statistically significant. RESULTS: The study found that 31.7% of students with text neck syndrome have mild neck disabilities, with a higher proportion of females (40%). The characteristics that have a significant relation to text neck syndrome include being female (p < 0.0001), overweight (p = 0.025), being right-handed (p = 0.001), using four digital devices or more (p = 0.002), having low levels of physical activity (P = 0.018), and spending more than three hours a day sitting down (P = 0.027). CONCLUSION: More than a quarter of medical students had text neck syndrome, and most of them had a mild neck disability. Text Neck Syndrome was linked to an increased number of gadgets used, low exercise, and more time spent in a sitting position.

Abstract:Introduction: Neck pain has become a common issue among today's students, affecting their studies and daily activities. Prolonged use of laptops and cell phones, leading to incorrect head and cervical spine posture, is considered a major contributing factor. This study aimed to determine the frequency of neck pain among medical students who use these devices and identify the specific areas of discomfort and the duration of device usage that causes pain. Additionally, the study aimed to identify risk factors associated with symptoms during device use, particularly in sitting postures, and explore the occurrence of neck and shoulder problems along with potential risk factors. Methodology: The study involved an observational cross-sectional design conducted on a population of medical students. A self-administered, closed-ended questionnaire was used to collect data. The questionnaire focused on variables such as duration of device usage, positions adopted while using the devices, affected areas of discomfort, and the impact of smartphone use on sleep. Convenience sampling was employed to select participants, and statistical analysis was conducted using SPSS software, presenting the results in the form of graphs and tables. Results: A total of 500 students participated in the study. The findings revealed that 76% of the students experienced neck, shoulder, or hand pain. Among them, 39.5% reported symptoms appearing after 3-4 hours of device use, while 56% experienced discomfort lasting less than 30 minutes. Furthermore, 65.5% stated that their discomfort did not interfere with their sleep, and 59.5% reported recurring neck pain. Conclusion: This study demonstrates that neck discomfort is a prevalent issue among college students, with risk factors such as poor posture and inadequate body ergonomics. Managing these risk factors through posture adjustment and avoiding behaviors that exacerbate neck pain can help alleviate the issue.

Abstract:Mobile phones are reliable devices for communication and entertainment. However, their utilization for prolonged periods in flexed neck position is linked to neck and shoulders pain. The main purpose of this study is to investigate the association between neck pain and the duration of device use, taking into consideration gender, age, and the most frequent position in which students use their devices.Based on a self-administered online questionnaire, we filled 500 questionnaires between February 15th, 2017 and March 18th, 2017. The study sample included healthy students from health care faculties regardless of their age, gender, or handedness.Analysis of the predictors for pain severity showed that age (p = 0.04) and duration of use (p = 0.001) were significantly associated with the severity of neck pain, while only the duration of use was significantly associated with pain duration (p = 0.036). Subjects were divided into two groups according to the pain score, 75.8% had pain severity equal or less than 4/10 and 24.2% had pain severity more than 4/10. Of those with pain severity >4, 5.8% of students sought medical help at the emergency department and 12.4% visited clinics, compared to only 0.3% seeking medical advice at an emergency department and 4.2% visiting clinics in the group with pain severity of ≤4 (p<0.001). Regarding the use of analgesia in the two groups, 44.6% of subjects with pain severity of >4 used analgesia, compared to only 12.1% in subjects with pain severity of ≤4 (p< 0.001).This study demonstrates a significant positive correlation between the duration of mobile phone use and the duration and severity of neck pain. Furthermore, the increased severity of neck pain places a huge burden on the healthcare system.

Abstract:Background: The pervasive use of mobile phones has raised concerns about their impact on musculoskeletal health, particularly neck pain. This issue is notably relevant in the Eastern Province of Saudi Arabia, where high mobile phone usage intersects with demographic diversity. While extensive phone use has been linked to neck pain and other musculoskeletal disorders globally, specific data on this issue in the Eastern Province are limited. This study addresses this gap by examining phone use patterns, neck positions, and associated symptoms in the region. Methods: Using an online, self-administered survey, this cross-sectional study investigated the relationship between phone use and neck pain in the Eastern Province of Saudi Arabia. Participants aged 18 years and older were recruited via social media, community groups, and university networks. The survey collected data on demographics, phone use patterns, neck positions, awareness of health risks, and pain symptoms. It was pre-tested, administered through Google Forms (Google, Mountain View, CA), and available for four weeks. Data were analyzed using descriptive statistics and cross-tabulations with SPSS 26.0 (IBM Corp., Armonk, NY). Results: The study included 400 participants, with 273 females (68.3%) and 127 males (31.8%). Most participants were single (245, 61.3%) and held a university degree (301, 75.3%). Daily phone use varied: 228 participants (57.0%) used their phones for less than five hours daily, while 43 (10.8%) used them for 10-15 hours or more. Neck positions ranged from 0° to 60°, with 168 participants (42.0%) maintaining a 30° angle. Awareness of health risks associated with phone use was high, with 364 participants (91.0%) aware of these risks. Neck pain was reported by 244 participants (61.0%), with additional symptoms including headache (22 participants, 5.5%) and upper back pain (five participants, 1.3%). Conclusion: This study found a significant link between prolonged phone use and neck pain in the Eastern Province of Saudi Arabia. Despite high awareness of the risks, many individuals report discomfort. These findings underscore the need for public health interventions and ergonomic education to improve phone use practices and musculoskeletal health.

Abstract:Background: Smartphone usage has become ubiquitous in Saudi Arabia with concerns growing over its impact on musculoskeletal health. Globally, various musculoskeletal symptoms have been linked to smartphone use such as neck pain, headaches, and shoulder discomfort, often exacerbated by poor posture and prolonged screen time. Methods: This cross-sectional study was conducted in Riyadh, Saudi Arabia, to investigate the impact of smartphone use on musculoskeletal health among adults aged 18 years and older. Participants were recruited through convenience sampling from various settings such as universities, workplaces, and public areas. Data were collected using a structured questionnaire administered both online and in paper format, covering demographic characteristics, smartphone usage behaviors, awareness of smartphone-related health effects, and the prevalence and severity of musculoskeletal symptoms. Results: A total of 413 participants from Riyadh, Saudi Arabia, were surveyed to assess musculoskeletal symptoms associated with smartphone use. Demographic analysis revealed a predominantly young, single, and highly educated population, primarily Saudi nationals. Smartphone usage patterns showed extensive daily use, with over 41% using their devices for more than five hours daily. The majority were aware of smartphone-related health effects. Musculoskeletal symptoms were prevalent, notably neck pain (83.8%), arm pain (63.8%), and headaches (71.2%). The incidence of symptoms related to text neck syndrome was substantial, although awareness and diagnosis were limited. The severity of symptoms varied, with mild to moderate levels reported most frequently. Conclusion: This study highlights a substantial prevalence of musculoskeletal symptoms among smartphone users in Riyadh, Saudi Arabia, emphasizing the need for public health interventions to promote ergonomic practices and mitigate health risks associated with smartphone use. Future research should focus on longitudinal studies to establish causal relationships and evaluate intervention strategies aimed at reducing these symptoms effectively.

Abstract:As digital technology and online activities have become more widely accessible over the past few years, information and communication technology have grown in importance in all students' lives. Most of them routinely use digital devices for a range of activities, primarily for online learning, activities, assignments, conversing, and Internet browsing. This study elucidated the relationship between neck and low back pain intensity and the use of digital devices among Saudi nursing students. A cross-sectional descriptive exploratory research design was applied using a convenience sample of 120 nursing students enrolled in an undergraduate nursing program at a Saudi government university located in Riyadh, the capital city of Saudi Arabia. A valid and reliable self-administered survey was employed. Data about sociodemographic characteristics, the Neck Disability Index and the Roland–Morris Disability Questionnaire were collected. The participants reported 9.1 ± 4.6 study hours on average per week. A total of 82.5% of them do not receive enough exercise, and 87.5% indicated daily use of digital devices. Around half (54.2%) of participants reported having neck pain of a mild intensity. About 60% of the participants regularly shift positions to rest their backs, 39.2% are only able to stand for limited periods due to back issues, and 39.2% expressed concern for others about what might happen to their health. Although there was an association between neck pain intensity and the age or gender of all research participants, there was a significant association between neck pain severity and marital status (χ2 = 15.226, p = 0.019). Our findings suggest that nursing students should maintain neutral neck and back postures on a regular basis to reduce pain, which could be attributed to extensive reading on digital devices.

Abstract:Introduction: In the present era, the use of smartphones for teaching, learning and communication for education in India has raised manyfold. The increasing use of smartphones in our daily life has brought about numerous musculoskeletal problems. Impairments of neck are the most common when compared to other parts of the body. Aim: To find out the impact of smartphone addiction on neck pain and disability. Materials and Methods: This cross-sectional study was conducted on students from Garden City University aged between 18-25 years through non probability sampling technique. The data was collected from August to September 2019. The addiction level to a smartphone was measured using Smartphone Addiction Scale Short Version (SAS-SV) and the Neck disability was measured using the Neck Disability Index (NDI) and were circulated. The questionnaires were shared with the students through electronic media and 108 subjects participated in it but only 100 were considered. The data were analysed using IBM SPSS 20.0 statistical software to correlate smartphone addiction scale and NDI. Spearman’s correlation test was performed to analyse the relationship between SAS-SV and NDI. Results: The Spearman correlation analysis clearly showed that there was a moderate positive correlation (r=0.682) between smartphone addiction and neck pain and neck disability in university students and the level of significance was p<0.01. Conclusion: This study concluded that longer use of smartphones would result in significant neck pain and disability. The students who were under the category of smartphone addicts would require ergonomic advice and education about the ill-effects of smartphone and the various musculoskeletal problems that may arise due to overuse.

Abstract:A BSTRACT Background: The past decade has witnessed a revolution in smartphones owing to their growing importance and various applications. However, excessive usage can lead to addiction and joint pain in the wrist/thumb area. Despite these negative effects, smartphones offer medical students access to the resources they need. Objective: To determine the association between smartphone addiction and thumb/wrist pain among undergraduate students of Jazan University. Methods: A cross-sectional self-administrated, online survey and online convenience sample technique was used. The collected data were analyzed using SPSS Software. Results: This survey included 337 participants, of which two-thirds were female (66.8%) and the rest male (33.2%); 32.6% of the respondents were studying in the College of Medicine. Of the 337 participants, 32% reported experiencing pain in their wrists or at the back of their neck while using a smartphone. The study found an average Smartphone Addition Scale-Short Version (SAS-SV) score of 32.5 ± 8.0 out of 50 and a total Patient-Rated Wrist Evaluation score of 48.8 ± 14.7, with a pain score of 13.3 ± 11.0 and a function score of 35.5 ± 11.7. The study also revealed a significant association between smartphone addiction and thumb/wrist pain ( P value = 0.029), with females and students of the College of Applied Medical Science associated with higher SAS-SV scores, thereby indicating higher smartphone addiction. Conclusion: Significant correlation exists between smartphone addiction and thumb/wirst pain among medical students of Jazan University.

Abstract:Background: Addiction to smart phones is classified clinically as behavioral addiction resulted from an excessive problematic usage of smart phones that effect the daily life of the users. Therefore, this study aims to explore the prevalence of smart phone addiction, its associated psychological distress risk, and its associated predictors among university students in Jordan. Methods: Between November 2022 and January 2023, a cross-sectional online survey study was conducted in Jordan. In this study, we used previously developed questionnaire instruments, the psychological Distress scale of Kessler and the Smartphone Addiction Scale. A score of 30 was used to identify the dummy variable in the binary logistic regression analysis to identify predictors of severe psychological distress, and smartphone addiction score of 38.7 was used to to identify predictors of smartphone addiction. Results: A total of 2337 university students participated in this study. The mean psychological distress score for the study participants was 30.0 (SD: 8.9). More than half of the study participants (59.1%) had a psychological distress score of 30 and above, which indicates a severe mental disorder state. More than half of the study participants (56.7%) had a smartphone addiction score of 30 and above, which reflects a smartphone addiction state. Females, divorced, those who feel that their mental abilities have been negatively affected by the use of smart phones, those who feel that using smartphones has affected their sleep and made it harder to fall asleep, and those feel that everything requires effort and fatigue, and they do not want to do any activity that requires effort were more likely to have severe psychological distress compared to others (p < 0.05). Females, those who feel that using smartphones has affected their sleep and made it harder to fall asleep, and those feel that everything requires effort and fatigue, and they do not want to do any activity that requires effort were more likely to be smartphone addicted compared to others (p < 0.05). Conclusion: Mental diseases are a major public health concern in Jordan, especially among university students. Females, those who thought smartphone usage hurt their mental capacities, and those who had trouble sleeping and fatigue were more likely to develop serious psychological discomfort and smartphone addiction. Smartphones are indispensable, but excessive use can lead to addiction and harm university students' mental health.

Abstract:Background: Digital devices significantly contribute to vision-related problems. In addition, prolonged postural imbalance, owing to excessive use of digital devices, can lead to the tightening of head and neck muscles, resulting in neck pain, a common musculoskeletal discomfort that significantly affects individuals with poor vision. This study aimed to evaluate the prevalence and interrelationships of screen time, visual disorders, and neck pain among students at Majmaah University. Methods: A cross-sectional study was conducted at Majmaah University, Saudi Arabia, enrolling students aged 18 to 25 years. Exclusion criteria included neurological or musculoskeletal disorders. Demographic data and information on visual and neck pain symptoms were collected. The Neck Disability Index questionnaire was used to assess neck pain, and data were analyzed using SPSS version 24.0. Results: Among 263 participants, 53.6% were female. Nearsightedness (38.0%) and dry/itchy eyes (49.0%) were the most common visual disorders and symptoms, respectively. Visual disorders were prevalent in 62.0% of students, while neck pain was reported by 79.5%. Females and those studying for more than 5 h using electronic screens daily reported higher neck disability index scores. A significant association was found between >5 h of study duration [screen time] and neck disability (OR 3.703, 95% CI 1.500–9.144, p = 0.005). Conclusion: The study highlights a relationship between visual problems and neck pain among university students, emphasizing the need for addressing vision-related issues to reduce neck discomfort. High screen time could substantially increase the odds of developing neck disability. However, authors warrant cautious interpretation in the light of following limitations: cross-sectional study, small sample size, lack of statistical power, and self-reported data.

Abstract:Background: Cell phone usage is highly prevalent among young adults. They are used for multiple purposes including communication, studies, social networking and entertainment. However, its excessive usage has been associated with adverse health outcomes. Objective: The objective was to find the association of cell phone usage with musculoskeletal discomfort (MSD) and its associated factors. Methods: A cross-sectional study was conducted on young adult students from a low-middle income country over a period of 3 months from December 2018 to February 2019. A structured questionnaire based on the Disabilities of the Arm, Shoulder and Hand (QuickDASH scoring) was used to record the musculoskeletal discomfort. Results: Out of 803 questionnaires, data of 754 (94%) were entered and the remaining questionnaires (n = 49) were discarded due to incomplete answers. The mean age was 20.83 (1.62) years. In our study, there were 194 (25.7%) males and 560 (74.3%) female participants. Neck and shoulder were the most frequently affected regions. A significant difference in QuickDASH score was observed between genders (p-value p ≤ 0.001). Left-handed individuals scored significantly higher on QuickDASH score compared to right-handed individuals (p < 0.05). Increased musculoskeletal discomfort was reported by individuals with daily cell phone use of more than four hours (p < 0.05). A positive significant correlation was found between continuous one hour cell-phone use and higher mean QuickDASH scores (correlation coefficient 0.124, p value 0.001). Shorter eye-to-screen distance was significantly associated with MSD (p < 0.05). Conclusion: Musculoskeletal discomfort is associated with the female gender, duration of cell phone use and a small eye-to-screen distance.

Abstract:The mobile phone is essential in daily life, especially during the pandemic. Prolonged use can cause postural issues, leading to common neck pain. This study aims to determine the correlation between mobile phone use duration, addiction, neck muscle endurance, and neck pain in university students. The study included 62 participants (30 female, 32 male) aged 18–35 years. Inclusion criteria required participants to have experienced neck pain at least twice in the past year and to have no other concomitant issues, as well as to volunteer for the study. Demographic information and daily mobile phone usage time were collected. Neck pain was assessed with the Visual Analogue Scale, smartphone addiction with the Smartphone Addiction Scale, and cervical muscle endurance was evaluated. Correlation analysis reveals a moderate relationship between neck pain severity (NPS) and cervical extensor muscle endurance (CEME), a strong relationship between NPS and cervical flexor muscle endurance (CFME), as well as a strong relationship among daily phone usage time (DPUT), CFME, and NPS, with a moderate relationship between DPUT and CEME. Participants were divided into two groups based on their DPUT, revealing that those who used their phone for four hours or more showed significantly higher levels of pain (p < 0.05) and reduced endurance in cervical flexor muscles. Our study found a strong correlation between neck pain, muscle endurance, and daily phone usage. Participants using their phones for more than four hours daily reported increased neck pain and decreased muscle endurance. We suggest integrating phone usage duration into neck pain assessments, promoting ergonomic practices, and offering detailed usage guidelines for users.

Abstract:Background The use of smartphones among the general public and health care practitioners, in particular, is ubiquitous. The aim of this study was to investigate the relationship between smartphone addiction and sleep quality, psychological distress, and loneliness among health care students and workers in Saudi Arabia. Methods This cross-sectional study used an online questionnaire to collect data on smartphone addiction, sleep quality, psychological distress, and loneliness as well as demographic information. Results A total of 773 health care students and workers participated in the study, with an average age of 25.95 ± 8.35, and 59.6% female participants. The study found a positive significant association between smartphone addiction and psychological distress (F (1,771) = 140.8, P < 0.001) and emotional loneliness (F (1,771) = 26.70, P < 0.001). Additionally, a significant negative association between smartphone addiction and sleep quality was found (F (1,771) = 4.208, P = 0.041). However, there was no significant relationship between smartphone addiction and social loneliness (F (1,771) = 0.544, P < 0.461). Conclusion These findings suggest that smartphone addiction has a negative impact on psychological distress, sleep quality, and emotional loneliness among health care students and workers. It is important to promote strategies to reduce smartphone dependency in order to avoid the harmful consequences of smartphone addiction.

Abstract:Background: College students are leading an evolution of device use both in the type of device and the frequency of use. They have transitioned from desktop stations to laptops, tablets, and especially smartphones and use them throughout the day and into the night. Methods: Using a 35-min online survey, we sought to understand how technology daily usage patterns, device types, and postures affect pain and discomfort to understand how knowledge of that pain might help students avoid it. Data were analyzed from 515 students (69.5% male) who completed an internet-delivered survey (81.3% response rate). Results: Participants ranked smartphones as their most frequently used technology (64.0%), followed by laptops and tablets (both 53.2%), and desktop computers (46.4%). Time spent using smartphones averaged over 4.4 h per day. When using their devices, students were more likely to adopt non-traditional workplace postures as they used these devices primarily on the couch or at a chair with no desk. Conclusion: Recent trends in wireless academic access points along with the portability of small handheld devices, have made smartphones the most common link to educational materials despite having the least favorable control and display scenario from an ergonomic perspective. Further, the potential impact of transitions in work environments due to COVID-19 may further exacerbate ergonomic issues among millions highlighting the need for such work to be carried out.
Associations between exposure to common technology devices and reported neck pain among Iranian school-age adolescents: a cross sectional study

Abstract:Background: The advancement of technology has contributed to a more sedentary lifestyle, and the extensive use of handheld devices among adolescents may potentially result in neck pain. This study aimed to assess the association between exposure to common technology devices and self-reported neck pain in Iranian school-age adolescents. Methods: This cross-sectional study was conducted between June and October 2021, employing a randomized multi-stage cluster sampling approach. We enrolled 808 adolescent students aged 11 to 19 years old. We asked participants about any neck pain they experienced in the week leading up to the study. Additionally, we gathered demographic information and assessed participants' use of electronic devices using a questionnaire. Results: Our study comprised 73.5% female participants with an average age of 15.1 ± 1.7 years and 26.5% male participants with an average age of 14.5 ± 1.5 years. In the regression model, the female gender (p = 0.038), using mobile for more than 6 h (p = 0.04), and using electronic devices while sitting on the floor (p = 0.02) were associated with a higher prevalence of neck pain among participants. Conclusion: In our study, we observed a relatively high prevalence of neck pain, which was linked to extended daily mobile phone usage and body posture during electronic device use. Policymakers may consider interventions aimed at reducing mobile phone usage and promoting proper body posture while using electronic devices as potential strategies to alleviate the burden of neck pain among Iranian adolescents.

Abstract:Abstract Background Smartphone usage is an essential everyday tool in Iran, however problematic use has escalated and become a concern for the Iranian health policy system, particularly during and following the COVID-19 Pandemic. This study’s aim was investigation of the prevalence of smartphone addiction, patterns of use, and the relationship to specific demographic characteristics and associated musculoskeletal disorders during the COVID-19 pandemic. Methods A descriptive-analytical correlational study recruited participants from a population of convenience ( n = 2344) who were smartphone owners with > 1 year of use. For demographic information an electronic self-report questionnaire collected age, sex, marital status, usage for daily hours, and patterns. To assess addiction levels, the ‘Smartphone Addiction Scale-short version’ (SAS-SV) patient-reported outcome measure was used (cut-off = 31). For experienced discomfort, the Extended Nordic Musculoskeletal Questionnaire (ENMQ) was used. Results The participants (female = 66.6%, n = 1561, mean age = 29.07 ± 12.34 years, range 6–60 years) smartphone use averaged 5.75 ± 3.44 h/day. The general prevalence of smartphone addiction was 46.16% (females = 46.06%, males = 46.36%; married = 44.5%, single = 47.63%). School students had the greatest addiction (53.2%) and those with a higher education to or above a Master’s degree were the lowest (39.38%). The highest pattern of use was for social networks at 89.1% of participants (female = 88.34%, male = 90.54%). The areas of highest reported discomfort were the eyes (43.5%) and neck (43.3%). A significant correlation was found between smartphone addiction and hours of daily usage, and the amount of usage increased during the COVID-19 pandemic period. Conclusion A high level of smartphone addiction in the Iranian population was found to have occurred during the COVID-19 pandemic. Those most affected were unmarried individuals and school students, with the predominant areas being the eyes and neck. Health decision-makers should consider these findings when developing recommendations and plans for public health, particularly those focused on students.

Abstract:Smartphone use can lead to smartphone addiction, which is a growing concern worldwide. However, there are limited studies about smartphone addiction and its impacts on university students in Saudi Arabia. This study aims to fill this gap. This is a quantitative study conducted among undergraduate students in Umm Al-Qura University (UQU), Saudi Arabia from May 2019 and February 2021. Study data were collected using both online and hard copy administered surveys. A self-administered questionnaire, Grade point average, Smartphone Addiction Short Version, and Kessler Psychological Distress scales were used to assess the outcomes. A total of 545 undergraduate students, mostly females, aged ≤ 21 years old and lived with large family sizes. More than half owned a smartphone for 5–8 years and the majority used their smartphone on average 6–11 h per day for social networking (82.6%), entertainment (66.2%) and web surfing (59.6%). Most of the participants were smartphone-addicted (67.0%). Logistic regression analysis showed that age ≤ 21, not gainfully employed, small family size and high family income were the main significant socio-demographic predictors of smartphone addiction. Smartphone-addicted participants were more likely to: have lower academic performance (GPA); be physically inactive; have poor sleep; be overweight/obese; have pain in their shoulder (39.2%), eyes (62.2%) and neck (67.7%) and have a serious mental illness (30.7%). This finding has significant implications for decision makers and suggests that smartphone education focusing on the physical and mental health consequences of smartphone addiction among university students can be beneficial.

Abstract:Background “Text neck” is defined by the cervical flexion adopted when using mobile devices. The possible causal relationship between this posture and neck pain is still widely discussed. Objectives The aim of this study was to investigate this association. Methods It is a longitudinal observational study. The sample consisted of 396 volunteers without neck pain aged between 18 and 65 years. Sociodemographic, anthropometric, lifestyle (level of physical activity, smoking, sleep quality), psychosocial (anxiety, depression, social isolation) and smartphone use issues were assessed using a self-reported questionnaire. Text neck was assessed by measuring the cervical flexion angle of participants standing while typing text on their smartphones, using the cervical range of motion (CROM) device at baseline. Two questions were used to assess the point prevalence and frequency of neck pain one year after baseline: “Did you have neck pain today?” With the following answer options “yes” or “no” and “How often do you have neck pain?”, the answer options were “very often”, “often”, “from time to time”, “rarely” and “never”. Results Of the total, 84% (n=335) of participants completed the one-year follow-up. Neck pain was reported by 10% (n=40) of the sample. The average cervical flexion angle of the standing participants using the smartphone was 34º (SD=12). Multiple logistic regression analysis showed that participants' neck flexion angle while standing using a smartphone was not associated with neck pain (OR = 1.01; 95% CI: 0.98-1.04; p=0.64) or frequency of neck pain (OR = 1.01; 95% CI: 0.99-1.03; p=0.44) one year after baseline. Of the potential confounders, sleep quality was associated with neck pain (OR = 1.76; 95% CI: 1.18–2.62; p=0.006) and frequency of neck pain (OR = 1.53, CI 95 %: 1.19–1.96; p=0.001). When compared to active participants, insufficiently active participants increased the chances of neck pain (OR = 2.42; 95%CI: 1.04–5.63; p=0.04). Conclusion “Text neck” was not associated with neck pain or frequency of neck pain in adults. Implications These results challenge the belief that poor neck posture while using smartphones leads to neck pain and may help mitigate the impact of negative information on the cervical spine.
