Abstract:To the editor: Pulmonary hamartoma is one of the most common benign tumour-like lesions of the lung with the characteristic radiograph appearance of a well-defined, solitary, smooth or slightly lobulated, peripheral pulmonary nodule with the presence of popcorn-like calcification. We report a case of multiple pulmonary hamartomas in a patient with unusual radiology performance. A 30-year-old previously healthy man was admitted to our department for evaluation of abnormal pulmonary multiple nodules and masses in the right lung that have been detected on computed tomography (CT). He was a nonsmoker and reported no illness during the preceding year, felt well and had no complaints. Physical examinations and laboratory investigations were unremarkable. He was diagnosed as tuberculosis and was placed on an initial antibiotic regimen involving three drugs administered daily (isoniazid, rifampin, ethambutol) for 9 months. After anti-tuberculosis therapy a CT scan was performed again for re-evaluation (Figure 1A), showing that there was no significant change in radiology compared with the preceding CT scan results. Sputum was negative for malignant cells and acid-fast bacilli. Then the patient underwent a transthoracic fine needle aspiration biopsy (FNAB) of the largest mass in the right upper lobe and histologic examination revealed myxomatous and cartilage diagnostic of a chondromatous hamartoma. Nevertheless, surgical excision of the mass was indicated due to the size and number of the nodules. The video-assisted thoracoscopic surgery (VATS) operation was performed and the upper and lower lobe and part of middle lobe of right lung were removed. The pathologic finding of surgery was also suggestive for chondromatous hamartoma, consistent with the FNAB results (Figure 1B). B: Histologic examination revealed cartilage diagnostic of a chondromatous hamartoma (HE, original magnification ×100).Hamartomas are the most common benign tumour-like lesions of the lung, constituting 5% to 8% of all solitary pulmonary nodules.1 However, the multiple pulmonary chondromatous hamartomas are very rare and less than 17 cases have been reported in the world.2 Most pulmonary hamartomas are usually clinically silent and found incidentally on chest radiographs. The characteristic radiograph appearance appeared as round or oval nodule lesion. It is often solitary, well-defined and slightly lobulated. Most hamartomas are 4 cm in size and are peripheral. Popcorn-like calcification and intranodular fat are important diagnostic CT findings for the diagnosis with a frequency of around 34%.3 However, on the case we reported, the CT scan showed a huge mass on the right middle lobe, measuring 8.5 cm × 4.0 cm. Multiple nodules of various sizes were also present (Figure 1A). There was no typical popcorn-like calcification or intranodular fat. The radiology findings, the size and the number of mass made our report an unusual presentation. This case re-emphases the value and necessity of pathologic examination particularly when the radiology findings are not typical.
