Abstract:Total hip arthroplasty (THA) is a common procedure that is performed for elderly people who suffer from hip pain and other hip-related problems. THA is an operation where an artificial joint is implanted in the femur and acts as the hip joint. THA has seen advancement to a greater extent in the 20th century. In this work, a novel hip implant design is proposed based on the existing literature data. The hip implants were provided with radial clearance between the head and stem junction. This implant design is analyzed by applying a walking gait that has been clinically prerecorded. Linear and volumetric wear rates are estimated at the head-stem junction, head-liner, and liner to acetabulum using Archard's wear law. Total deformation, von Mises stress, contact stress, linear and volumetric wear rates were reported. The volumetric wear rate was found to be less than the existing literature, achieving the goal of increasing the life expectancy of the total hip prosthesis. The maximum wear rate is found at the head-stem junction for press-fit. It is observed that when the acetabular cup and femoral head are made of CoCr (Cobalt-Chromium) alloy, the liner with Ultra high molecular weight polyethylene (UHMWPE) and the stem with titanium alloy have a lesser wear rate when compared to other material combinations. Further, it is also found that the fatigue life of Ti (Titanium) alloy is higher than that of CoCr alloy and Ti alloy material combinations have the maximum factor of safety.

Abstract:Total Hip Arthroplasty has been a revolutionary technique in restoring mobility to patients with damaged hip joints. The introduction of modular components of the hip prosthesis allowed for bespoke solutions based on the requirements of the patient. The femoral stem is designed with a conical trunnion to allow for assembly of different femoral head sizes based on surgical requirements. The femoral head diameters for a metal-on-polyethylene hip prosthesis have typically ranged between 22 mm and 36 mm and are typically manufactured using Cobalt–Chromium alloy. A smaller femoral head diameter is associated with lower wear of the polyethylene, however, there is a higher risk of dislocation. In this study, a finite element model of a standard commercial hip arthroplasty prosthesis was modelled with femoral head diameters ranging from 22 mm to 36 mm to investigate the wear evolution and material loss at both contacting surfaces (acetabular cup and femoral stem trunnion). The finite element model, coupled with a validated in-house wear algorithm modelled a human walking for 10 million steps. The results have shown that as the femoral head size increased, the amount of wear on all contacting surfaces increased. As the femoral head diameter increased from 22 mm to 36 mm, the highly cross-linked polyethylene (XLPE) volumetric wear increased by 61% from 98.6 mm 3 to 159.5 mm 3 while the femoral head taper surface volumetric wear increased by 21% from 4.18 mm 3 to 4.95 mm 3 . This study has provided an insight into the amount of increased wear as the femoral head size increased which can highlight the life span of these prostheses in the human body.

Abstract:Objective:To study the stress distribution of the femoral hip prosthesis after the hip joint replacement.Methods:After the hip joint replacement,when the femur and prosthesis are considered as concentric cylinders with perfectly banded interface,a relatively perfect theoretical model of simulating the interfacial stress transfer is established.Results:The maximum interfacial shear stress occured at Z=0.At the cross-section of the femoral neck,interfacial shear stress decreased exponentially with the increases of the Z.Shear stress became very small at Z0.1 m,which meant that the shear stress at the far end of the femoral hip prosthesis was very small.In order to avoid the stress concentration and femoral hip prosthesis sinking,interfacial stress must remain constant and balanced with the pressure load at Z=0.The radius of the femoral hip prosthesis changed with interfacial shear stress.The maximum value of the radius occured at Z=0,then it decreased at m.Specially,a=18.2 mm at Z=10 mm,a=5.36 mm at Z=98 mm,these are ideal radius.Conclusion:A theoretical model of simulating the interfacial stress is established when the femur and prosthesis are considered as concentric cylinders.The distributions of the interfacial shear and radial stresses with the axial positions are obtained.A theoretical reference for the design of the prosthesis is provided.
Investigation of contact behavior on a model of the dual-mobility artificial hip joint for Asians in different inner liner thicknesses

Abstract:Background: The four components that make up the current dual-mobility artificial hip joint design are the femoral head, the inner liner, the outer liner as a metal cover to prevent wear, and the acetabular cup. The acetabular cup and the outer liner were constructed of 316L stainless steel. At the same time, the inner liner was made of ultra-high-molecular-weight polyethylene (UHMWPE). As this new dual-mobility artificial hip joint has not been researched extensively, more tribological research is needed to predict wear. The thickness of the inner liner is a significant component to consider when calculating the contact pressure. Aim: To make use of finite element analysis to gain a better understanding of the contact behavior in various inner liner thicknesses on a new model of a dual-mobility artificial hip joint, with the ultimate objective of determining the inner liner thickness that was most suitable for this particular type of dual-mobility artificial hip joint. Methods: In this study, the size of the femoral head was compared between two diameters (28 mm and 36 mm) and eight inner liner thicknesses ranging from 5 mm to 12 mm. Using the finite element method, the contact parameters, including the maximum contact pressure and contact area, have been evaluated in light of the Hertzian contact theory. The simulation was performed statically with dissipated energy and asymmetric behavior. The types of interaction were surface-to-surface contact and normal contact behavior. Results: The maximum contact pressures in the inner liner (UHMWPE) at a head diameter of 28 mm and 36 mm are between 3.7-13.5 MPa and 2.7-10.4 MPa, respectively. The maximum von Mises of the inner liner, outer liner, and acetabular cup are 2.4-11.4 MPa, 15.7-44.3 MPa, and 3.7-12.6 MPa, respectively, for 28 mm head. Then the maximum von Mises stresses of the 36 mm head are 1.9-8.9 MPa for the inner liner, 9.9-32.8 MPa for the outer liner, and 2.6-9.9 MPa for the acetabular cup. A head with a diameter of 28 mm should have an inner liner with a thickness of 12 mm. Whereas the head diameter was 36 mm, an inner liner thickness of 8 mm was suitable. Conclusion: The contact pressures and von Mises stresses generated during this research can potentially be exploited in estimating the wear of dual-mobility artificial hip joints in general. Contact pressure and von Mises stress reduce with an increasing head diameter and inner liner's thickness. Present findings would become one of the references for orthopedic surgery for choosing suitable bearing geometric parameter of hip implant.
How accurate is the Archard law to predict wear of UHMWPE in hard-on-soft hip implants? A numerical and experimental investigation

Abstract:Advanced finite element wear models have been recently proposed for hard-on-soft hip implants, but their high computational cost and validation are still open issues. This study aims to develop a fast and accurate wear model for in-silico pre-clinical trials. A finite element model of a ceramic-on-UHMWPE hip implant, based on the Archard wear law, was calibrated and validated using experimental wear maps, as rarely done in the literature. Predicted wear volume and depths deviated from the experimental ones of about 0.2 % and 12–25 %, respectively, comparable to more advanced models including the cross-shearing. Results support the development of relatively simple models requiring limited simulation times, thus making feasible thousands of simulations requested by in-silico trials.

Abstract:In the present study a direct comparison was made between in vitro total hip wear testing and a computational analysis considering the effects of time and a nonlinear stress-strain relationship for ultrahigh molecular weight polyethylene (UHMWPE) at 37°C. The computational simulation was made correct through calibration to experimental volumetric wear results, and the predicted damage layout on the acetabular liner surface was compared with results estimated from laser scanning of the actual worn specimens. The wear rates for the testing specimens were found to be 17.14±1.23 mg/106 cycles and 19.39±0.79 mg/106 cycles, and the cumulative volumetric wear values after 3×106 cycles were 63.70 mm3 and 64.02 mm3 for specimens 1 and 2, respectively. The value of the calibrated wear coefficient was found to be 5.32(10−10) mm3/N mm for both specimens. The major difference between the computational and experimental wear results was the existence of two damage vectors in the experimental case. The actual location of damage was virtually the same in both cases, and the maximum damage depth of the computational model agreed well with the experiment. The existence of multiple wear vectors may indicate the need for computational approaches to account for multidirectional sliding or strain hardening of UHMWPE. Despite the limitation in terms of describing the overall damage layout, the present computational model shows that simulation can mimic some of the behavior of in vitro wear.
Analysing contact conditions in hard-on-hard hip replacements: effectiveness of current analytical methods and novel data-driven approach

Abstract:Contact mechanics models that can accurately estimate the contact conditions and, consequently predict wear must be developed for hip implants. This study analyses and verifies the existing Hertz and Fang analytical models applicable to hard-on-hard hip implants. The contact parameters, such as the maximum contact pressure, contact radius and the maximum deformation, are considered for the validation with FEM. Both analytical models fail to predict the contact conditions throughout a gait cycle. A novel data-driven ANN model is developed to comprehensively predict the contact conditions considering different input parameters. The analysis shows that cup thickness significantly affects the output contact conditions. Therefore, it is recommended to consider cup thickness in the analytical model interpretation for hard-on-hard hip implants.

Abstract:Total hip arthroplasty (THA) is one of the most successful orthopaedic interventions globally, with over 450,000 procedures annually in the U.S. alone. However, issues like aseptic loosening, dislocation, infection and stress shielding persist, necessitating complex, costly revision surgeries. This highlights the need for continued biomaterials innovation to enhance primary implant integrity and longevity. Implant materials play a pivotal role in determining long-term outcomes, with titanium alloys being the prominent choice. However, emerging evidence indicates scope for optimized materials. The nickel-free β titanium alloy Ti-27Nb shows promise with excellent biocompatibility and mechanical properties. Using finite element analysis (FEA), this study investigated the biomechanical performance and safety factors of a hip bone implant made of nickel-free titanium alloy (Ti-27Nb) under actual loading during routine day life activities for different body weights. The FEA modelled physiological loads during walking, jogging, stair ascent/descent, knee bend, standing up, sitting down and cycling for 75 kg and 100 kg body weights. Comparative analyses were conducted between untreated versus 816-hour simulated body fluid (SBF) treated implant conditions to determine in vivo degradation effects. The FEA predicted elevated von Mises stresses in the implant neck for all activities, especially stair climbing, due to its smaller cross-section. Stresses increased substantially with a higher 100 kg body weight compared to 75 kg, implying risks for heavier patients. Safety factors were reduced by up to 58% between body weights, although remaining above the desired minimum value of 1. Negligible variations were observed between untreated and SBF-treated responses, attributed to Ti-27Nb's excellent biocorrosion resistance. This comprehensive FEA provided clinically relevant insights into the biomechanical behaviour and integrity of the Ti-27Nb hip implant under complex loading scenarios. The results can guide shape and material optimization to improve robustness against repetitive stresses over long-term use. Identifying damage accumulation and failure risks is crucial for hip implants encountering real-world variable conditions. The negligible SBF effects validate Ti-27Nb's resistance to physiological degradation. Overall, the study significantly advances understanding of Ti-27Nb's suitability for reliable, durable hip arthroplasties with low revision rates.
Finite element submodeling technique to analyze the contact pressure and wear of hard bearing couples in hip prosthesis

Abstract:Finite element (FE) simulation plays a major role in computing stress and predicting the failure of biomedical components. Normally in past, researchers focused on developing a global computational model from the scanned data of patients to analyze the stresses and deformations. To compute the wear of the hip prosthesis, mostly the global model (GM) is used to predict the expected life for million cycles using nodal updating technique which leads to high computational effort and time. The proposed work utilizes a submodeling finite element technique to analyze the contact pressure and wear of biomaterials for three different combinations in hip prosthesis including metal, ceramic and polycrystalline diamond materials. Initially the global model boundary and loading conditions are transferred to the submodel. The mesh is refined further using finer mesh and then the results are computed which consumes lesser time. The contact stress as well as the linear wear of biomaterials is found to be quite high for the local model (LM) when compared with the global model. However, no changes in volumetric wear of these biomaterials are observed when compared with previous experimental results. The computational time as well as accuracy in estimating the contact stress and the wear of bearings is improved effectively. Among local model with different element sizes, 0.75 mm element size of local model showed improved results in estimating the contact stress and linear wear of bearing.

Measurement of polyethylene wear and its clinical significance in total hip arthroplasty

Abstract:Objective To measure the linear polyethylene wear with use of a computer-assisted two-dimensional method, and to evaluate the relationship between wear and long-term clinic-radiological findings. Methods Radiographs of 104 cementless total hip arthroplasties were examined for osteolysis, radiolucent line, cup abduction angle, cup migration, cancellous condensation, stem subsidence, distal pedestal formation and so on. To measure the linear polyethylene wear, migration in the femoral head center relative to the cup shell center was calculated on digitized consecutive radiographs with special computer software program, the degree and direction of wear were also determined. Patients were followed-up for an average period of 8.4 years. Results The average Harris Hip Score was 53.8 points preoperatively and 88.5 points at the latest follow-up. Fourteen hips ( 13.5 % ) were loose. Total linear wear was 1. 14 mm,wear rate was 0. 15 mm/year. Acetabular osteolysis was correlated to polyethylene wear, and a high wear rate was associated with loosening of the components. Multivariate regression analysis showed a lack of correlation between wear rate and patients' age, body weight, cup size, cup abduction angle, activity level and Harris Hip Score. Conclusion The method is preferable for measuring the polyethylene wear of metal-backed acetabular component and other components of various designs. There was clear relationship between wear and acetabular osteolysis or loosening of the prostheses. Therefore, polyethylene wear is important in evaluating the clinical and radiological outcomes of total hip arthroplasty.
Effect of geometrical, operational and material parameters in the lubrication regime of hard-on-hard hip implants

Abstract:Several factors, such as the material, geometry, roughness, relative angular movement and loading conditions, influence the hip implant's lubrication regime, thereby affecting the friction and wear performance including longevity. In this study, a ball-on-plane model is considered to predict the lubrication regime across the entire gait cycle for the metal-on-metal and ceramic-on-ceramic hip implants. According to ISO 14242-1, a normal walking gait cycle is considered for the loads and rotations in the hip implant for this study. A comprehensive analysis is done to estimate the lubrication regime by considering different material combinations, body weights, femoral head sizes, clearances, and roughness across the entire gait cycle. The correlation coefficients show that the geometrical parameters are dominant in affecting the lubrication film thickness compared to the operational and material parameters. The femoral head size and body weight are found to be the most dominant and least dominant parameter respectively. Among the hard-on-hard tribo-pair, ceramics operate predominantly in the full-film lubrication regime compared to metals due to its fine surface finish. The present research suggests that in order to maximise the longevity of a hip implant, an orthopaedic surgeon should choose one with a larger femoral head diameter, less clearance, and an ultra-fine surface finish, regardless of any tribo-pair.

Finite element analysis of elliptical shaped stem profile of hip prosthesis using dynamic loading conditions

Abstract:Patient-specific dynamic loadings are seldom considered during the evaluation of hip implants. The primary objective of this study is to check for the feasibility of the use of UHMWPE as the material for an acetabular cup o CoCr Alloy that is reported to produce a squeaking sound after replacement. An elliptical shaped stem with three different cross-sectional profiles is considered for simulation. Using a commercial finite element method, patient-specific dynamic forces were applied for the quantitative analysis. The loading and boundary conditions are used as per ISO and ASTM standards. The walking gait cycle is used with two widely used biocompatible materials: titanium and cobalt-chromium. Initially, only the stem is considered for the analysis to finalize the best out of the three profiles, along with the better material for the stem. Later the complete implant is used for the analysis. Profile 1 exhibits 1.25 and 1.17 times greater stress than Profile 2 for CoCr Alloy and Ti-6Al-4V, respectively. Similarly, Profile 3 displays stresses 1.26 and 1.25 times greater than Profile 2 for CoCr Alloy and Ti-6Al-4V, respectively. Comparatively, displacement in stem Profile 2 is 1.75 times higher in Ti-6Al-4V than CoCr Alloy. The full implant displacement at 14% gait cycle is 1.15% higher for the CoCr-acetabular column material combination when compared to UHMWPE. It can be concluded that UHMWPE can be used as the acetabular cup material instead of CoCr for the Profile 2 elliptical shaped hip implant to prevent squeaking after replacement.

Abstract:Hip joint prostheses are used to replace hip joint function in the human body. The latest dual-mobility hip joint prosthesis has an additional component of an outer liner that acts as a cover for the liner component. Research on the contact pressure generated on the latest model of a dual-mobility hip joint prosthesis under a gait cycle has never been done before. The model is made of ultrahigh molecular weight polyethylene (UHMWPE) on the inner liner and 316L stainless steel (SS 316L) on the outer liner and acetabular cup. Simulation modeling using the finite element method is considered static loading with an implicit solver for studying the geometric parameter design of dual-mobility hip joint prostheses. In this study, simulation modeling was carried out by applying varying inclination angles of 30°, 40°, 45°, 50°, 60°, and 70° to the acetabular cup component. Three-dimensional loads were placed on femoral head reference points with variations of femoral head diameter used at 22 mm, 28 mm, and 32 mm. The results in the inner surface of the inner liner, the outer surface of the outer liner, and the inner surface of the acetabular cup showed that the variations in inclination angle do not have a major effect on the maximum contact pressure value on the liner component, where the acetabular cup with an inclination angle of 45° can reduce contact pressure more than the other studied inclination angle variations. In addition, it was found that the 22 mm diameter of the femoral head increases the contact pressure. The use of a larger diameter femoral head with an acetabular cup configuration at a 45° inclination can minimize the risk of implant failure due to wear.

Abstract:The wear of artificial joints can lead to joint noise and tissue pathology within the human body, which is a primary factor affecting their service life. In response to the issue of wear in ceramic–titanium alloy artificial hip joints, this study employed hip joint wear simulations and experimental wear testing on hip joint specimens to investigate the impact of different contact surface parameters on the wear of ceramic–titanium alloy articulating surfaces. The objective was to provide guidance for joint surface treatment to minimize wear. The simulation results demonstrated that the contacting surfaces of the articulating components exhibited a crescent-shaped surface composition before and after wear. The initial variation in the surface friction coefficient had minimal influence on the wear rate after stabilization, whereas excessively high friction coefficients led to erratic changes in wear depth. Based on the simulation results, experimental research was conducted to compare the wear results of different surface roughness values ranging from 60 to 550 nm. The findings revealed that a surface roughness of 150 nm exhibited the least amount of wear and the best anti-wear performance. Furthermore, an exploration of the mechanism behind the influence of different surface friction coefficients on the wear of the articulating surfaces provided valuable insights for surface processing and wear analysis of artificial joints.

Abstract:Abstract This paper proposes the replacement of artificial hip joints which is one of the successful methods used to restore the movement of damaged hip bones. The main contribution of this work is to reduce wear caused by contact pressure between the femoral head and acetabular cup surfaces, where the average wear rate is very high. This can be achieved through the use of dimple on the femoral head surface from the artificial hip joint and subsequently computed by using the Archard equation . To validate the contact pressure accuracy of the proposed model, mesh sensitivity that used with 3 levels of mesh size are 1.2, 1.1 and 1.0. In this study for more accurate result, the mesh sensitivity used is with the mesh size of 1.0. The performance calculation results from wear are shown that in a hip joint using dimple the maximum contact pressure and the wear depth are decreased compared to the hip joint without dimple which are 67.25 MPa and 1.064 x10 −10 mm from the previous value of 77.44 MPa and 1.185 x10 −10 mm. Therefore, based on differences in the distribution of contact pressure that occurs shows volumetric wear on the hip joints using a dimple lower than without a dimple.

Abstract:Objective Due to the limitation of manufacturing techniques,possible design optimization selecting and influence of its wearing in clinical application,the bearing surface of hip joint replacements is presented as non-spherical geometry,and the finite element method can be used to study the contact mechanics behavior in such kind of non-spherical hip joint replacement.Methods The reconstructing of non-spherical articular surface based on spherical-grid-data model(SGDM) was developed to investigate the effect of contact mechanics of an ellipsoidal head against a spherical cup in a typical metal-on-metal hip joint replacement.Results The maximum contact pressure of the non-spherical bearing was decreased effectively,and meanwhile the contact area was increased when curvature radius of the ellipsoidal head around the centre of the contact zone was increased,while the effects of the cup inclination angle on the maximum contact pressure and contact area of the non-spherical bearing under the same load showed relatively small,but the contact pressure distributions were different.Conclusions A well-controlled non-sphericity can improve the magnitude and distribution of contact pressures on metal-on-metal hip joint replacements.In addition,the developed model and evaluation method in this paper can be used for simulation of dynamic contact and wear prediction of non-spherical hip joint replacements.

Abstract:The present work covers the analytical design process of three dimensional (3-D) hip joint prosthesis with numerical fatigue stress analysis. The analytical generation equations describing the different stem constructive parts (ball, neck, tour, cone, lower ball) have been presented to reform the stem model in a mathematical feature. The generated surface has been introduced to FE solver (Ansys version 11) in order to simulate the induced dynamic stresses and investigate the effect of every design parameter (ball radius, angle of neck, radius of neck, neck ratio, main tour radius, and outer tour radius) on the max. equivalent stresses for hip prosthesis made from titanium alloy. The dynamic loading case has been studied to a stumbling case. The load has been applied on the cap tip as a concentrated load distributed on the interface of ball and socket. The results show that the decreasing of max. Fatigue stress by (175) MPa could be obtained by increasing the outer tour radius from (10)mm to (15) mm and that will change the max. Fatigue zone location from the tour section to the neck. The ball radius and neck angle must be as lower as possible to decrease the fatigue stresses. The most dominate parameter to increase the safety factor is the radius of neck.
Estimation of two wear factors for total hip arthroplasty: A simulation study based on musculoskeletal modelling

Abstract:Background: Primary causes of surgical revision after total hip arthroplasty are polyethylene wear and implant loosening. These factors are particularly related to joint friction and thus patients' physical activity. Assessing implant wear over time according to patients' morphology and physical activity level is key to improve follow-up and patients' quality of life. Methods: An approach initially proposed for tibiofemoral prosthetic wear estimation was adapted to compute two wear factors (force-velocity, directional wear intensity) using a musculoskeletal model. It was applied on 17 participants with total hip arthroplasty to compute joint angular velocity, contact force, sliding velocity, and wear factors during common daily living activities. Findings: Differences were observed between gait, sitting down, and standing up tasks. An incremental increase of both global wear factors (time-integral) was observed during gait from slow to fast speeds (p ≤ 0.01). Interestingly, these two wear factors did not result in same trend for sitting down and standing up tasks. Compared to gait, one cycle of sitting down or standing up tends to induce higher friction-related wear but lower cross-shear-related wear. Depending on the wear factor, significant differences can be found between sitting down and gait at slow speed (p ≤ 0.05), and between sitting down (p ≤ 0.05) or standing up (p ≤ 0.05) and gait at fast speed. Furthermore, depending on the activity, wear can be fostered by joint contact force and/or sliding velocity. Interpretation: This study demonstrated the potential of wear estimation to highlight activities inducing a higher risk of implant wear after total hip arthroplasty from motion capture data.
