Abstract:Introduction Delayed Cerebral Ischemia (DCI) is a significant complication following aneurysmal subarachnoid hemorrhage (aSAH) that can lead to poor outcomes. Machine learning techniques have shown promise in predicting DCI and improving risk stratification. Methods In this study, we aimed to develop machine learning models to predict the occurrence of DCI in patients with aSAH. Patient data, including various clinical variables and co-factors, were collected. Six different machine learning models, including logistic regression, multilayer perceptron, decision tree, random forest, gradient boosting machine, and extreme gradient boosting (XGB), were trained and evaluated using performance metrics such as accuracy, area under the curve (AUC), precision, recall, and F1 score. Results After data augmentation, the random forest model demonstrated the best performance, with an AUC of 0.85. The multilayer perceptron neural network model achieved an accuracy of 0.93 and an F1 score of 0.85, making it the best performing model. The presence of positive clinical vasospasm was identified as the most important feature for predicting DCI. Conclusions Our study highlights the potential of machine learning models in predicting the occurrence of DCI in patients with aSAH. The multilayer perceptron model showed excellent performance, indicating its utility in risk stratification and clinical decision-making. However, further validation and refinement of the models are necessary to ensure their generalizability and applicability in real-world settings. Machine learning techniques have the potential to enhance patient care and improve outcomes in aSAH, but their implementation should be accompanied by careful evaluation and validation.

Abstract:Intracranial hemorrhage, bleeding that occurs inside the cranium, is a serious health problem requiring rapid and often intensive medical treatment. Such a condition is traditionally diagnosed by highly-trained specialists analyzing computed tomography (CT) scan of the patient and identifying the location and type of hemorrhage if one exists. We propose a neural network approach to find and classify the condition based upon the CT scan. The model architecture implements a time distributed convolutional network. We observed accuracy above 92% from such an architecture, provided enough data. We propose further extensions to our approach involving the deployment of federated learning. This would be helpful in pooling learned parameters without violating the inherent privacy of the data involved.

Abstract:The amount of biomedical data continues to grow rapidly. However, collecting data from multiple sites for joint analysis remains challenging due to security, privacy, and regulatory concerns. To overcome this challenge, we use federated learning, which enables distributed training of neural network models over multiple data sources without sharing data. Each site trains the neural network over its private data for some time and then shares the neural network parameters (i.e., weights and/or gradients) with a federation controller, which in turn aggregates the local models and sends the resulting community model back to each site, and the process repeats. Our federated learning architecture, MetisFL, provides strong security and privacy. First, sample data never leave a site. Second, neural network parameters are encrypted before transmission and the global neural model is computed under fully homomorphic encryption. Finally, we use information-theoretic methods to limit information leakage from the neural model to prevent a "curious" site from performing model inversion or membership attacks. We present a thorough evaluation of the performance of secure, private federated learning in neuroimaging tasks, including for predicting Alzheimer's disease and for brain age gap estimation (BrainAGE) from magnetic resonance imaging (MRI) studies in challenging, heterogeneous federated environments where sites have different amounts of data and statistical distributions.
Feasibility Analysis of Federated Neural Networks for Explainable Detection of Atrial Fibrillation

Abstract:Early detection of atrial fibrillation (AFib) is challenging due to its asymptomatic and paroxysmal nature. However, advances in deep learning algorithms and the vast collection of electrocardiogram (ECG) data from devices such as the Internet of Things (IoT) hold great potential for the development of an effective solution. This study assesses the feasibility of training a neural network on a Federated Learning (FL) platform to detect AFib using raw ECG data. The performance of an advanced neural network is evaluated in centralized, local, and federated settings. The effects of different aggregation methods on model performance are investigated, and various normalization strategies are explored to address issues related to neural network federation. The results demonstrate that federated learning can significantly improve the accuracy of detection over local training. The best performing federated model achieved an F1 score of 77\%, improving performance by 15\% compared to the average performance of individually trained clients. This study emphasizes the promise of FL in medical diagnostics, offering a privacy-preserving and interpretable solution for large-scale healthcare applications.

Abstract:Distributed training can facilitate the processing of large medical image datasets, and improve the accuracy and efficiency of disease diagnosis while protecting patient privacy, which is crucial for achieving efficient medical image analysis and accelerating medical research progress. This paper presents an innovative approach to medical image classification, leveraging Federated Learning (FL) to address the dual challenges of data privacy and efficient disease diagnosis. Traditional Centralized Machine Learning models, despite their widespread use in medical imaging for tasks such as disease diagnosis, raise significant privacy concerns due to the sensitive nature of patient data. As an alternative, FL emerges as a promising solution by allowing the training of a collective global model across local clients without centralizing the data, thus preserving privacy. Focusing on the application of FL in Magnetic Resonance Imaging (MRI) brain tumor detection, this study demonstrates the effectiveness of the Federated Learning framework coupled with EfficientNet-B0 and the FedAvg algorithm in enhancing both privacy and diagnostic accuracy. Through a meticulous selection of preprocessing methods, algorithms, and hyperparameters, and a comparative analysis of various Convolutional Neural Network (CNN) architectures, the research uncovers optimal strategies for image classification. The experimental results reveal that EfficientNet-B0 outperforms other models like ResNet in handling data heterogeneity and achieving higher accuracy and lower loss, highlighting the potential of FL in overcoming the limitations of traditional models. The study underscores the significance of addressing data heterogeneity and proposes further research directions for broadening the applicability of FL in medical image analysis.

Abstract:Background and Purpose: Delayed cerebral ischemia (DCI) after aneurysmal subarachnoid hemorrhage negatively impacts long-term recovery but is often detected too late to prevent damage. We aim to develop hourly risk scores using routinely collected clinical data to detect DCI. Methods: A DCI classification model was trained using vital sign measurements (heart rate, blood pressure, respiratory rate, and oxygen saturation) and demographics routinely collected for clinical care. Twenty-two time-varying physiological measures were computed including mean, SD, and cross-correlation of heart rate time series with each of the other vitals. Classification was achieved using an ensemble approach with L2-regularized logistic regression, random forest, and support vector machines models. Classifier performance was determined by area under the receiver operating characteristic curves and confusion matrices. Hourly DCI risk scores were generated as the posterior probability at time t using the Ensemble classifier on cohorts recruited at 2 external institutions (n=38 and 40). Results: Three hundred ten patients were included in the training model (median, 54 years old [interquartile range, 45–65]; 80.2% women, 28.4% Hunt and Hess scale 4–5, 38.7% Modified Fisher Scale 3–4); 101 (33%) developed DCI with a median onset day 6 (interquartile range, 5–8). Classification accuracy before DCI onset was 0.83 (interquartile range, 0.76–0.83) area under the receiver operating characteristic curve. Risk scores applied to external institution datasets correctly predicted 64% and 91% of DCI events as early as 12 hours before clinical detection, with 2.7 and 1.6 true alerts for every false alert. Conclusions: An hourly risk score for DCI derived from routine vital signs may have the potential to alert clinicians to DCI, which could reduce neurological injury.

Abstract:Federated learning (FL) is widely used in privacy sensitive applications for isolated data islands, with the aim of achieving distributed model training, privacy enhancement and model sharing. Electromyographic (EMG) signals are a type of data collected from wearable sensors of subjects which are distributed on multiple devices, highly personalized and play an important role in several applications including prosthetic hand control, sign languages, grasp recognition, etc. This paper utilizes the FL method to detect single and combined finger movements based on EMG signals. The existing research on FL for wearable healthcare faces challenges of variable probability distributions of data, the need for prerequisite knowledge of server model and computational burdens in parameter transmission. To address these problems, this paper proposes a communication efficient FL framework in which each device only needs to transmit the weight matrices of local models to the server for model aggregation. To further reduce the FL transmission delay, a joint learning and resource allocation problem is formulated via optimizing transmit power of each device, time allocation, and user selection. To solve the delay minimization problem, the objective function is first converted to a tractable expression and then the difference of two convex functions programming is adopted. Simulation results using real EMG signals show that the proposed FL framework with personalized training process successfully detects single and combined finger movements for distributed users. Two public EMG datasets with 10 and 15 different finger movements are employed. Over 98% overall test accuracy is achieved in both datasets which surpasses the conventional learning framework by 1.6% and 0.5% on average. Different scenarios with regard to access points and users are investigated and the convexity of the proposed model is discussed.
FedBrain: A robust multi-site brain network analysis framework based on federated learning for brain disease diagnosis

Abstract:In recent years, deep learning models have shown their advantages in neuroimage analysis, such as brain disease diagnosis. Unfortunately, it's usually difficult to acquire numerous brain networks at a single centralized site to effectively train a high-quality deep learning model. To address this issue, federated learning (FL) has gained popularity in brain disease diagnosis, which allows deep learning models to be trained without centralizing data. However, most FL-based works might still face two following challenges. Firstly, the high-dimensional features of brain networks are often far larger than sample size, which might lead to poor performance due to the curse of dimensionality. Secondly, differences in data distributions across different sites can impact the communication efficiency and performance of FL models. To overcome these challenges, we design a novel FL framework for diagnosing brain disorders, named FedBrain. Firstly, FedBrain proposes data augmentation based on L1 regularization to select significant features shared by all clients. The domain alignment loss based on the maximum mean discrepancy criterion is introduced to minimize differences in the marginal and conditional distributions between local clients. Furthermore, FedBrain proposes a personalized predictor based on mixture of experts to adapt to different clients, using a global and private predictor as two experts. Eventually, FedBrain integrates the above modules with differential privacy and homomorphic encryption into a unified FL framework. Experimental results on the Autism Brain Imaging Data Exchange (ABIDE) dataset demonstrate its effectiveness and robustness, which shows that FedBrain can reduce the communication burden of FL and achieve the highest average accuracy of 79% against other counterparts.

Abstract:Recent advancements in neuroimaging have led to greater data sharing among the scientific community. However, institutions frequently maintain control over their data, citing concerns related to research culture, privacy, and accountability. This creates a demand for innovative tools capable of analyzing amalgamated datasets without the need to transfer actual data between entities. To address this challenge, we propose a decentralized sparse federated learning (FL) strategy. This approach emphasizes local training of sparse models to facilitate efficient communication within such frameworks. By capitalizing on model sparsity and selectively sharing parameters between client sites during the training phase, our method significantly lowers communication overheads. This advantage becomes increasingly pronounced when dealing with larger models and accommodating the diverse resource capabilities of various sites. We demonstrate the effectiveness of our approach through the application to the Adolescent Brain Cognitive Development (ABCD) dataset.

A Comparative Study of Federated Learning Models for COVID-19 Detection

Abstract:Deep learning is effective in diagnosing COVID-19 and requires a large amount of data to be effectively trained. Due to data and privacy regulations, hospitals generally have no access to data from other hospitals. Federated learning (FL) has been used to solve this problem, where it utilizes a distributed setting to train models in hospitals in a privacy-preserving manner. Deploying FL is not always feasible as it requires high computation and network communication resources. This paper evaluates five FL algorithms' performance and resource efficiency for Covid-19 detection. A decentralized setting with CNN networks is set up, and the performance of FL algorithms is compared with a centralized environment. We examined the algorithms with varying numbers of participants, federated rounds, and selection algorithms. Our results show that cyclic weight transfer can have better overall performance, and results are better with fewer participating hospitals. Our results demonstrate good performance for detecting COVID-19 patients and might be useful in deploying FL algorithms for covid-19 detection and medical image analysis in general.

Abstract:Purpose: To investigate the efficacy of federated learning (FL) compared to industry-level centralized learning (CL) for segmenting acute infarct and white matter hyperintensity. Materials and Methods: This retrospective study included 13,546 diffusion-weighted images (DWI) from 10 hospitals and 8,421 fluid-attenuated inversion recovery images (FLAIR) from 9 hospitals for acute (Task I) and chronic (Task II) lesion segmentation. The mean ages (SD) for the training datasets were 68.1 (12.8) for Task I and 67.4 (13.0) for Task II. The frequency of male participants was 51.5% and 60.4%, respectively. We trained with datasets from 9 and 3 institutions for Task I and Task II, respectively, and externally tested them in datasets from 1 and 9 institutions each. For FL, the central server aggregated training results every four rounds with FedYogi (Task I) and FedAvg (Task II). A batch clipping strategy was tested for the FL models. Performances were evaluated with the Dice similarity coefficient (DSC). Results: In Task I, the FL model employing batch clipping trained for 360 epochs achieved a DSC of 0.754±0.183, surpassing an equivalent CL model (DSC 0.691 ± 0.229; p<0.001) and comparable to the best-performing CL model at 940 epochs (DSC 0.755 ± 0.207; p=0.701). In Task II, no significant differences were observed amongst FL model with clipping, without clipping, and CL model after 48 epochs (DSCs of 0.761 ± 0.299, 0.751 ± 0.304, 0.744 ± 0.304). Few-shot FL showed significantly lower performance. Task II reduced training times with batch clipping (3.5 to 1.75 hours). Conclusion: Comparisons between CL and FL in identical settings suggest the feasibility of FL for medical image segmentation.

Abstract:Machine Learning (ML) is considered a promising tool to aid and accelerate diagnosis in various medical areas, including neuroimaging. However, its success is set back by the lack of large-scale public datasets. Indeed, medical institutions possess a large amount of data; however, open-sourcing is prevented by the legal requirements to protect the patient's privacy. Federated Learning (FL) is a viable alternative that can overcome this issue. This work proposes training an ML model for Alzheimer's Disease (AD) detection based on structural MRI (sMRI) data in a federated setting. We implement two aggregation algorithms, Federated Averaging (FedAvg) and Secure Aggregation (SecAgg), and compare their performance with the centralized ML model training. We simulate heterogeneous environments and explore the impact of demographical (sex, age, and diagnosis) and imbalanced data distributions. The simulated heterogeneous environments allow us to observe these statistical differences' effect on the ML models trained using FL and highlight the importance of studying such differences when training ML models for AD detection. Moreover, as part of the evaluation, we demonstrate the increased privacy guarantees of FL with SecAgg via simulated membership inference attacks.

Integrating Clinical Data and Radiomics and Deep Learning Features for End-to-End Delayed Cerebral Ischemia Prediction on Noncontrast CT

Abstract:Background and purpose: Delayed cerebral ischemia is hard to diagnose early due to gradual, symptomless development. This study aimed to develop an automated model for predicting delayed cerebral ischemia following aneurysmal SAH on NCCT. Materials and methods: This retrospective study included 400 patients with aneurysmal SAH (156 with delayed cerebral ischemia) who underwent NCCT. The study used ATT-Deeplabv3+ for automatically segmenting hemorrhagic regions using semisupervised learning. Principal component analysis was used for reducing the dimensionality of deep learning features extracted from the average pooling layer of ATT-DeepLabv3+. The classification model integrated clinical data, radiomics, and deep learning features to predict delayed cerebral ischemia. Feature selection involved Pearson correlation coefficients, least absolute shrinkage, and selection operator regression. We developed models based on clinical features, clinical-radiomics, and a combination of clinical, radiomics, and deep learning. The study selected logistic regression, Naive Bayes, Adaptive Boosting (AdaBoost), and multilayer perceptron as classifiers. The performance of segmentation and classification models was evaluated on their testing sets using the Dice similarity coefficient for segmentation, and the area under the receiver operating characteristic curve (AUC) and calibration curves for classification. Results: The segmentation process achieved a Dice similarity coefficient of 0.91 and the average time of 0.037 s/image. Seventeen features were selected to calculate the radiomics score. The clinical-radiomics-deep learning model with multilayer perceptron achieved the highest AUC of 0.84 (95% CI, 0.72-0.97), which outperformed the clinical-radiomics model (P = .002) and the clinical features model (P = .001) with multilayer perceptron. The performance of clinical-radiomics-deep learning model using AdaBoost was significantly superior to its clinical-radiomics model (P = .027). The performance of the clinical-radiomics-deep learning model and the clinical-radiomics model with logistic regression notably exceeded that of the model based solely on clinical features (P = .028; P = .046). The AUC of the clinical-radiomics-deep learning model with multilayer perceptron (P < .001) and the clinical-radiomics model with logistic regression (P = .046) were significantly higher than the clinical model with logistic regression. Of all models, the clinical-radiomics-deep learning model with multilayer perceptron showed best calibration. Conclusions: The proposed 2-stage end-to-end model not only achieves rapid and accurate segmentation but also demonstrates superior diagnostic performance with high AUC values and good calibration in the clinical-radiomics-deep learning model, suggesting its potential to enhance delayed cerebral ischemia detection and treatment strategies.
Preserving Specificity in Federated Graph Learning for fMRI-based Neurological Disorder Identification

Abstract:Resting-state functional magnetic resonance imaging (rs-fMRI) offers a non-invasive approach to examining abnormal brain connectivity associated with brain disorders. Graph neural network (GNN) gains popularity in fMRI representation learning and brain disorder analysis with powerful graph representation capabilities. Training a general GNN often necessitates a large-scale dataset from multiple imaging centers/sites, but centralizing multi-site data generally faces inherent challenges related to data privacy, security, and storage burden. Federated Learning (FL) enables collaborative model training without centralized multi-site fMRI data. Unfortunately, previous FL approaches for fMRI analysis often ignore site-specificity, including demographic factors such as age, gender, and education level. To this end, we propose a specificity-aware federated graph learning (SFGL) framework for rs-fMRI analysis and automated brain disorder identification, with a server and multiple clients/sites for federated model aggregation and prediction. At each client, our model consists of a shared and a personalized branch, where parameters of the shared branch are sent to the server while those of the personalized branch remain local. This can facilitate knowledge sharing among sites and also helps preserve site specificity. In the shared branch, we employ a spatio-temporal attention graph isomorphism network to learn dynamic fMRI representations. In the personalized branch, we integrate vectorized demographic information (i.e., age, gender, and education years) and functional connectivity networks to preserve site-specific characteristics. Representations generated by the two branches are then fused for classification. Experimental results on two fMRI datasets with a total of 1,218 subjects suggest that SFGL outperforms several state-of-the-art approaches.

Abstract:Deep learning has proven to be highly effective in diagnosing COVID-19; however, its efficacy is contingent upon the availability of extensive data for model training. The data sharing among hospitals, which is crucial for training robust models, is often restricted by privacy regulations. Federated learning (FL) emerges as a solution by enabling model training across multiple hospitals while preserving data privacy. However, the deployment of FL can be resource-intensive, necessitating efficient utilization of computational and network resources. In this study, we evaluate the performance and resource efficiency of five FL algorithms in the context of COVID-19 detection using Convolutional Neural Networks (CNNs) in a decentralized setting. The evaluation involves varying the number of participating entities, the number of federated rounds, and the selection algorithms. Our findings indicate that the Cyclic Weight Transfer algorithm exhibits superior performance, particularly when the number of participating hospitals is limited. These insights hold practical implications for the deployment of FL algorithms in COVID-19 detection and broader medical image analysis.

Abstract:Segmentation models for brain lesions in MRI are commonly developed for a specific disease and trained on data with a predefined set of MRI modalities. Each such model cannot segment the disease using data with a different set of MRI modalities, nor can it segment any other type of disease. Moreover, this training paradigm does not allow a model to benefit from learning from heterogeneous databases that may contain scans and segmentation labels for different types of brain pathologies and diverse sets of MRI modalities. Additionally, the sensitivity of patient data often prevents centrally aggregating data, necessitating a decentralized approach. Is it feasible to use Federated Learning (FL) to train a single model on client databases that contain scans and labels of different brain pathologies and diverse sets of MRI modalities? We demonstrate promising results by combining appropriate, simple, and practical modifications to the model and training strategy: Designing a model with input channels that cover the whole set of modalities available across clients, training with random modality drop, and exploring the effects of feature normalization methods. Evaluation on 7 brain MRI databases with 5 different diseases shows that such FL framework can train a single model that is shown to be very promising in segmenting all disease types seen during training. Importantly, it is able to segment these diseases in new databases that contain sets of modalities different from those in training clients. These results demonstrate, for the first time, the feasibility and effectiveness of using Federated Learning to train a single 3D segmentation model on decentralised data with diverse brain diseases and MRI modalities, a necessary step towards leveraging heterogeneous real-world databases. Federated GNNs for EEG-Based Stroke Assessment

Abstract:Machine learning (ML) has the potential to become an essential tool in supporting clinical decision-making processes, offering enhanced diagnostic capabilities and personalized treatment plans. However, outsourcing medical records to train ML models using patient data raises legal, privacy, and security concerns. Federated learning has emerged as a promising paradigm for collaborative ML, meeting healthcare institutions' requirements for robust models without sharing sensitive data and compromising patient privacy. This study proposes a novel method that combines federated learning (FL) and Graph Neural Networks (GNNs) to predict stroke severity using electroencephalography (EEG) signals across multiple medical institutions. Our approach enables multiple hospitals to jointly train a shared GNN model on their local EEG data without exchanging patient information. Specifically, we address a regression problem by predicting the National Institutes of Health Stroke Scale (NIHSS), a key indicator of stroke severity. The proposed model leverages a masked self-attention mechanism to capture salient brain connectivity patterns and employs EdgeSHAP to provide post-hoc explanations of the neurological states after a stroke. We evaluated our method on EEG recordings from four institutions, achieving a mean absolute error (MAE) of 3.23 in predicting NIHSS, close to the average error made by human experts (MAE $\approx$ 3.0). This demonstrates the method's effectiveness in providing accurate and explainable predictions while maintaining data privacy.

Abstract:In recent times, automated detection of diseases from pathological images leveraging Machine Learning (ML) models has become fairly common, where the ML models learn detecting the disease by identifying biomarkers from the images. However, such an approach requires the models to be trained on a vast amount of data, and healthcare organizations often tend to limit access due to privacy concerns. Consequently, collecting data for traditional centralized training becomes challenging. These privacy concerns can be handled by Federation Learning (FL), which builds an unbiased global model from local models trained with client data while maintaining the confidentiality of local data. Using FL, this study solves the problem of centralized data collection by detecting deformations in images of Red Blood Cells (RBC) in a decentralized way. To achieve this, RBC data is used to train multiple Deep Learning (DL) models, and among the various DL models, the most efficient one is considered to be used as the global model inside the FL framework. The FL framework works by copying the global model's weight to the client's local models and then training the local models in client-specific devices to average the weights of the local model back to the global model. In the averaging process, direct averaging is performed and alongside, weighted averaging is also done to weigh the individual local model's contribution according to their performance, keeping the FL framework immune to the effects of bad clients and attacks. In the process, the data of the client remains confidential during training, while the global model learns necessary information. The results of the experiments indicate that there is no significant difference in the performance of the FL method and the best-performing DL model, as the best-performing DL model reaches an accuracy of 96% and the FL environment reaches 94%-95%. This study shows that the FL technique, in comparison to the classic DL methodology, can accomplish confidentiality secured RBC deformation classification from RBC images without substantially diminishing the accuracy of the categorization. Finally, the overall validity of the classification results has been verified by employing GradCam driven Explainable AI techniques.
