Abstract:Abstract Background: Pulmonary arteriovenous malformations are uncommon conditions of abnormal communications between pulmonary arteries and veins, which are most commonly congenital in nature. Although such condition is not extremely rare, it is an challenge in the differential diagnosis of pulmonary problems such as hypoxemia and pulmonary lesions.Case presentation: We report a meaningful case of a 23-year-old male presented with elevated hemoglobin (23.0 g/dl) on admission. Physical examination revealed cyanosis, digital clubbing and low oxygen saturation on room air. The patient was initially diagnosed as polycythemia vera while the subsequent result of bone marrow aspiration was negative. During further assessment, pulmonary arteriovenous malformations were detected by computed tomographic angiography of pulmonary artery. Lobectomy was successfully performed with significant increase of oxygen saturation from 86% to 98%. The patient received an uneventful postoperative recovery and was followed up for nearly 5 years without recurrence.Conclusions: Pulmonary arteriovenous malformations should be suspected in the patients with central cyanosis, digital clubbing, polycythemia and pulmonary lesion and without cardiac malformations. Embolization or surgery is highly recommended to reduce the risks caused by pulmonary arteriovenous malformations.

Abstract:A 39-year-old woman was admitted to our hospital for a pure ground-glass opacity that had been detected in the right lung during a regular examination. A computed tomography scan showed a pure ground-glass opacity beneath the pleura of the right upper lobe of the lung that had enlarged over time. As a consequence, a lung adenocarcinoma was suspected. Meanwhile, three-dimensional computed tomography scans revealed a tracheal bronchus originating directly from the lateral wall of the trachea. The patient consequently underwent posterior segmental resection and mediastinal lymph node sampling by video-assisted thoracic surgery. During surgery, in addition to the tracheal bronchus, a variable central vein was found entering the left atrium dorsal to the right pulmonary artery trunk. We submit that, to the best of our knowledge, this is the first case of its kind ever reported. (C) 2014 by The Society of Thoracic Surgeons

Abstract:Background Displaced anterior segmental bronchus and pulmonary artery is extremely rare. A keen knowledge of such variations is required in the field of pulmonary segmentectomy, for unawareness of the structural variation could lead to intra- and postoperative complications. Case presentation A 50-year-old female presented to our department with suspected lung adenocarcinoma. Preoperative 3-dimensional computed tomographic bronchography and angiography revealed anterior segmental bronchus and anterior segmental pulmonary artery variation: the anterior segmental bronchus derived from the middle lobe bronchus, accompanied by a distally distributed anterior segmental pulmonary artery branch. A right apical-posterior segmentectomy using inflation-deflation technique was performed successfully. Conclusions The keen observation and proper application of modern imaging technology and operative technique could greatly aid segmentectomy, preventing intra- and postoperative complications from happening.

Abstract:Background: Video-assisted thoracoscopic surgery segmentectomy is increasingly being used to resect peripheral small lung cancer. However, to manage some lesions which locate between segment deep inside the parenchyma is still challenging. Generally, wedge resection and segmentectomy are optional for peripheral small lung cancer. However, it is hard to achieve safe surgical margins if the lesion is located in the segment plane deep inside the parenchyma, especially close to the segment hilum. In that situation, lobectomy and bi-segmentectomy are usually performed for the sake of safe margins with the price of excessive normal lung tissue lost. To ensure safe surgical margins and preserve normal lung tissue as far as possible, combined subsegmentectomy is feasible and it is required precise preoperative plan including surgical margin delineation, bronchi and vessels variation and surgical procedure. The variation of bronchi and segment vein in our current case are rare and the branching pattern of pulmonary artery-inferior trunk (Tr.inf) is firstly reported in our case. Case Description: In 2019, a 41-year-old female presented to the Thoracic Clinic with a history of a 7-mm sized, mixed-density ground-glass opacity (GGO) in the right upper lobe. The lesion located in the segmental plane between S3b and S1b deep inside the parenchyma. We performed precise preoperative planning with 3-dimension pulmonary bronchi and vessels reconstruction and resected RS3b + S1bi via single-port approach. The patient was discharged from hospital on the 5th postoperative day without any complications. Chest CT scans in the 12th and 24th months after surgery showed good lung recovery, and no atelectasis or pulmonary congestion was observed. Notably, we observed a new branching pattern of A1b, which came from the inferior trunk, combined with A3a. Additionally, we performed a literature review to analyze the variation patterns of segmental structures in the right upper lobe, and the indications, effects, and safety of combined subsegmentectomy. Conclusions: Our case and review of literature showed that combined subsegmentectomy was feasible for lesion deep inside parenchyma if a detailed preoperative plan and delicate procedures during surgery were implemented.

Abstract:A detailed knowledge of anomalies of the anatomical course of the pulmonary vessels is extremely important for video-assisted thoracic surgery (VATS). Inadvertent vascular ligation and dissection are catastrophic for patients undergoing lobectomy. We describe a case with a rare anomaly of the left pulmonary artery and highlight the importance of identifying the courses of pulmonary vessels for VATS lobectomy.

Abstract:OBJECTIVES:We summarize our experiences of video-assisted thoracoscopic surgery (VATS) sleeve lobectomy with bronchoplasty for non-small-cell lung cancer and discuss the indications and technical details of the operation.METHODS:From September 2011 to December 2012, 15 patients underwent VATS sleeve lobectomy with bronchoplasty at our institution (right upper lobe 10, right middle and lower lobes 1, left lower lobe 2 and left upper lobe 2), with mediastinal lymphadenectomy. Three incisions were utilized. The utility incision was made at the fourth intercostal space, anterior axillary line. Simple continuous and simple interrupted suturing of the membranous and cartilaginous portions of the bronchus were performed for the anastomosis.RESULTS:All procedures were uneventful, with a median operative time of 165 min, a median bronchial anastomosis time of 44 min and a median blood loss of 150 ml. There were no conversions to thoracotomy. There were 14 squamous carcinomas and 1 adenocarcinoma. All patients recovered well, and 1 experienced a minor complication. The median duration of chest tube drainage was 5.4 days, and the median length of hospital stay was 7 days. All patients were followed postoperatively for a range of 1-16 months without tumour recurrence.CONCLUSIONS:VATS sleeve lobectomy with bronchoplasty is safe and effective. The utility incision placed at the fourth intercostal space, anterior axillary line, is convenient for the anastomosis, and the suturing technique is expeditious and secure. Preserving the azygos vein does not compromise exposure for the anastomosis. This technique is very suitable for centrally located lung cancers <3 cm in diameter, particularly when the cancers are located within the brachial lumen.
Strategies for Completely Thoracoscopic Lobectomy

Abstract:Objective To discuss the technical strategies for thoracoscopic lobectomy.Methods Between September 2006 and May 2008,a total of 91 patients underwent thoracoscopic lobectomy in our hospital.The lobectomy and lymph node resection were completed via three mini incisions with the same procedures as those in an open surgery.Among the cases,75 had primary or metastatic malignancies,and 16 showed benign tumor.By thoracoscopy,upper right lobectomy was carried out in 21 patients,right middle lobectomy in 12,lower right in 20,upper left in 18,and lower left in 20.Results Only two cases were converted to open thoracotomy.In the other 89 patients,the mean operation time for the thoracoscopy was(185.8±52.9)minutes(ragne,60-300 minutes),and the mean blood loss was 213.2 ml(range,50-650 ml).In this series,the chest drainage lasted(6.9±2.9)days;the patients were discharged from the hospital in(9.4±3.2)days after the surgery.No severe complications or perioperative death occurred in the cases except in one patient,who developed chylothorax after the treatment.Two patients,who had primary lung cancer,showed distant metastasis at 15 and 3 months respectively after the surgery.No recurrence or metastasis was found in the other cases.Conclusions Thoracoscopic lobectomy is safe and effective for patients with indications for the surgical procedure.Surgical skills for dissection of vessels and lymph nodes are the key to the operation.
Thoracoscopic Sublobectomy for Lung Cancer in a Patient with Unilateral Absence of Pulmonary Artery: Case Report and Narrative Review

Abstract:Unilateral absence of a pulmonary artery (UAPA) is an uncommon congenital anomaly. Among the rarer conditions, UAPA with lung cancer has been previously reported in 13 cases; however, there remains controversy regarding the surgical approach and precautions. Herein, we present a case study of a 56-year-old female patient who was incidentally diagnosed with a nodule in the right lower lobe of the lung during a routine physical examination and subsequently found to have an absent right pulmonary artery upon preoperative evaluation. A wedge resection of the right lower lobe was performed as treatment. Postoperative pathology confirmed invasive adenocarcinoma (pT1N0M0). We provide a narrative review of existing literature on these patients and discuss optimal surgical management strategies.

Abstract:Ectopia of the right upper pulmonary vein (RUV) is rare and sometimes combined with tracheal bronchus. Such anomalies can affect surgical procedures. We report the case of a 66-year-old woman with lung cancer in which preoperative three-dimensional computed tomography reconstruction and intraoperative images displayed a combined ectopic variation of the RUV and bronchus. The RUV was absent from the anterior hilum, but instead was present in the superior and posterior hilum dispersedly. In addition, the right upper lobe bronchus was ectopic to the lower edge of the "bronchus intermedius." Based on this discovery, video-assisted thoracoscopic lobectomy was successfully performed.

Abstract:While the role of low-dose computed tomography (CT) in lung cancer screening is established, its limitations in detailing pulmonary vascular variations are less emphasized. Three-dimensional reconstruction technology allows surgeons to reconstruct a patient's bronchial and pulmonary vascular structures using CT scan results. However, low-dose CT may not provide the same level of clarity as enhanced CT in displaying pulmonary vascular details. This limitation can be unfavorable for preoperative detection of potential pulmonary vascular variations, especially in cases involving planned segmentectomy.

Abstract:BACKGROUND:Video-assisted thoracic surgery (VATS) is a new technology for nearly 30 years in the field of thoracic surgery most watched. However, there are still some controversy concerning the technical difficulties, operation duration, the extent of lymph node dissection and perioperative complications for VATS sleeve bronchial lobectomy when handling the locally advanced central lung cancer (involving the trachea and/or main bronchus).METHODS:A 66 years old man was admitted for coughing for 2 months. He had smoked for 30 years, 20 packs a day. Chest computed tomography (CT) revealed a 2.5 cm × 4.5 cm mass in the right upper lobe. Bronchoscopy demonstrated the tumor obstructing the right upper lobe bronchus and involved the right main bronchus and bronchus intermedius. Pathology was squamous cell carcinoma. His pulmonary function result was forced expiratory volume in 1 second (FEV1): 1.91 L (64.7% predicted), forced vital capacity (FVC): 4.36 L. He received general anesthesia with double-lumen endotracheal intubation and left lung ventilation. Left lateral decubitus position was chosen. The first 1.5 cm incision was selected in the eighth intercostal space in the midaxillary line, and was used for the camera. A 4 cm long incision was made in the 3rd intercostal space in the preaxillary line. A third 1.5 cm incision was performed in the 9th intercostal space in the postaxillary line for assistant. Pulmonary ligament and the entire right hilum were mobilized. Pulmonary vein is the most forward hilar structure, sometimes immediately prior pulmonary trunk. The right upper lobe vein was transected with a vascular stapler. Truncus and posterior ascending pulmonary artery were then divided and transected with a vascular stapler. Major and minor fissures were stapled by 60 mm green linear stapler. Following clearance of the mediastinal lymph nodes of level 7, 4R and 2R, the bronchial sleeve resection and reconstruction began. The distal right main bronchus and bronchus intermedius were fully mobilized to ensure adequate surgical exposure. Traction sutures were routinely placed on the lateral walls and to reduce tension. Interrupted sutures were chosen for bronchial anastomosis. Bronchial membrane was sutured first, and then circumference end-to-end anastomoses were carried out using 3-0 absorbable sutures.RESULTS:There were no complications and the patient was discharged 8 days postoperatively.CONCLUSIONS:The 3rd intercostal space of the anterior axillary line was suggested for right upper lobe bronchial sleeve resection. This incision can reduce the distance and angle between the anastomosis to the incision, providing convenient conditions for easy anastomosis. And avoid the operator fatigue for keeping the posture for a long time. Clearance of the mediastinal lymph nodes before cutting the bronchus was helpful for satisfied explosion of the right main bronchus, the upper lobe bronchus and bronchus intermedius. And this would avoid pulling bronchial anastomosis for mediastinal lymph nodes clearance. Interrupted suture was safe and effective for VATS bronchial anastomosis.

Abstract:Three‐dimensional illustration of the multi‐intersegmental vein V1+2d and their clinical significance. Background: To investigate the distribution pattern of V1+2d in the left superior pulmonary vein and its clinical significance. Methods: A retrospective analysis was conducted using three‐dimensional computed tomographic bronchography and angiography (3D‐CTBA) data from 500 lung cancer patients. Statistical analyses were performed to evaluate the incidence and drainage patterns of the three sub‐branches of V1+2d, namely V1+2d1, V1+2d2 and V1+2d3. Furthermore, clinical data from 10 patients' lesions involving V1+2d were reviewed to illustrate the impact of adjacency to V1+2d on the surgical approach. Results: The incidences of V1+2d1, V1+2d2 and V1+2d3 were 100%, 76.4% and 100% respectively. The relative interlobar distribution sizes of B3a and B1+2c and the left upper division (LUD) vein type influenced the incidence of V1+2d2 (p B3a patterns. V1+2d2 was entirely absent in the B3a > B1+2c pattern. V1+2d2 exhibited a higher incidence in both the central vein (CV) type and the noncentral vein (NCV) type when compared to the semi‐central vein (SCV) type (100% vs. 100% vs. 64.8%). The most prevalent venous drainage pattern was the three sub‐branches of V1+2d constituting a major trunk to drain (41.2%). All 10 cases with lesions involving V1+2d successfully underwent sublobar resection with no complications, and the surgical margin was ≥2 cm. Conclusion: The three sub‐branches of V1+2d exhibit a high incidence with diverse distribution patterns, yet a discernible pattern exists. For inter‐ or multi‐intersegmental nodules involving V1+2d, combined segmentectomy and subsegmentectomy or combined subsegmentectomy can ensure the safe margin.

Abstract:We report a rare case of early‐stage lung cancer along with partial anomalous pulmonary venous return with a small left‐to‐right shunt. Robot‐assisted right S3 segmentectomy was performed safely without postoperative cardiac and respiratory complications in low respiratory patients with partial anomalous pulmonary venous return in the same lobe. Partial anomalous pulmonary venous return (PAPVR) is a rare congenital malformation where the pulmonary vein partially refluxes into the venous system. Here, we present the first robotic‐assisted right S3 segmentectomy in a 70‐year‐old male with early‐stage lung cancer and PAPVR in the right upper pulmonary vein. The patient, with suspected primary lung cancer (11 mm diameter, pure solid appearance in right S3 segment), exhibited clinical stage T1bN0M0 stage IA2. Preoperative computed tomography revealed severe lung emphysema, and right V1–3 returned directly to the superior vena cava. However, no signs of right‐sided heart failure were observed, and echocardiogram was normal with a pulmonary‐to‐systemic blood flow ratio of 1.4. Successful robot‐assisted right S3 segmentectomy with hilar nodal dissection was performed, and the patient was discharged on the sixth postoperative day without complications. One year postoperatively, there has been no recurrence of lung cancer or respiratory/right‐sided heart failure symptoms.

Abstract:A 61-year-old male patient was referred to the thoracic surgery department due to repeated hemoptysis for more than one year. The computed tomography revealed a 5.2 cm × 3.1 cm mass in the right lower lobe and the nature of the mass was confirmed to be chronic inflammation by trans percutaneous lung biopsy. Bronchiectasis of the right lower lobe was considered based on the symptoms, signs, and imaging findings. Surgery for bronchiectasis is used only as part of a multimodality treatment approach. After the adequate pretreatment with a targeted antimicrobial, thoracoscopic resection of superior segment of the right lower pulmonary lobe was finally performed in the order of the superior segmental artery, the superior segmental vein, the superior segmental bronchus, and the pulmonary tissues of the superior segment. Total surgery time was 70 min and blood loss was 100 mL. The chest tube was removed on the 3(th) postoperative day. The patient was discharged home on the 8(th) postoperative day.

Abstract:A 60 years old man with right upper lobe squamous cell carcinoma was admitted in the department of thoracic surgery after receiving two cycles of chemotherapy. Chest computed tomography (CT) revealed the mass reduce to 2.0×1.5 cm. No contraindications or metastasis was found after routine preoperative examination. It took 3 hours to complete this operation. Final pathology stage was T3N0M0, stage IIA. This patient discharged on the 7th postoperative day with no complications.
Management of Middle Lobe Veins During Single-Portal Video-Assisted Thoracoscopic Surgery Lobectomy Via the Fifth Intercostal Approach (case Series): Why the Subcarinal Triangular Right Base Angle is Helpful

Abstract:Background: The fifth intercostal space is the preferred approach during uniportal video-assisted thoracoscopic surgery (VATS) lobectomy. However, managing the right middle lobe pulmonary vein (RML PV) through this approach is technically challenging for inexperienced surgeons. To facilitate the surgical procedure, we describe our surgical strategy for managing the middle lobe vein via the fifth intercostal space and define the approach [subcarinal triangular right base angle (SCT-RBA)] utilized to manage the middle lobe vein. Case Description: Based on the characteristics of uniportal surgery, we designed a new method of managing middle lobar veins via the fifth intercostal approach, which also facilitates the dissection of the subcarinal lymph nodes. We described the short-term surgical outcomes of 7 patients who underwent singleport middle lobe resection from January 2021 to January 2022 in the Department of Thoracic Surgery, Ruijin Hospital North Campus, Shanghai Jiaotong University School of Medicine. No conversion and mortality were observed in 7 patients who underwent single-port VATS middle lobe resection. One patient had bronchial asthma and air leakage, which led to delayed drainage and hospitalization. There were no complications or delayed discharge reported among the other patients. Conclusions: Our initial results indicate that this new technique is a feasible strategy to manage the middle lobe veins and facilitate the dissection of subcarinal lymph nodes.
VATS Right Upper Lobe Bronchial Sleeve Resection.

Abstract:BACKGROUND:The aim of this study is to discuss video-assisted thoracic surgery (VATS) sleeve bronchial lobectomy when handling the locally advanced central lung cancer (involving the trachea and/or main bronchus).METHODS:A 2.5 cm × 1.0 cm mass was found in the right upper lobe. Bronchoscopy demonstrated the tumor obstructing the right upper lobe bronchus and involved the right main bronchus and bronchus intermedius. Interrupted sutures were chosen for bronchial anastomosis. Bronchial membrane was sutured first, and then circumference end-to-end anastomoses were carried out using 3-0 absorbable sutures.RESULTS:There were no complications and the patient was discharged 8 days postoperatively.CONCLUSIONS:The third intercostal space of the anterior axillary line was suggested for right upper lobe bronchial sleeve resection. This incision can reduce the distance and angle between the anastomosis to the incision, and facilitate anastomosis. This approach can also prevent operator from fatigue for keeping one posture for a long time. Clearance of the mediastinal lymph nodes before cutting the bronchus was helpful for exposing the right main bronchus, the upper lobe bronchus and bronchus intermedius satisfied. And this option would avoid pulling bronchial anastomosis during mediastinal lymph nodes clearance. Interrupted suture was safe and effective for VATS bronchial anastomosis.
