Abstract:Background and aim Depression is a common and complex psychiatric disorder, and lipid metabolism plays an important role in the development of psychiatric disorders such as depression. Cardiometabolic index (CMI) is a novel index that synthesizes two quantitative indicators of blood lipids (triglyceride(TG)/high-density lipoprotein cholesterol (HDL-C)) and human obesity-related parameters (waist height ratio (WHtR)). This study used NHANES data to explore the correlation between CMI and the incidence of depression. Methods and results Based on the data of the National Health and Nutrition Examination Survey (NHANES) 2011–2018, multivariate logistic regression, sensitivity analysis, and smooth curve fitting were used to study the relationship between CMI and depression. Subgroup analysis and interaction tests were used to investigate whether the association was stable in different populations. CMI was positively associated with depression in 7229 participants aged >20 years. In the fully adjusted model, each unit increase in CMI was associated with 36 % higher likelihood of depression symptoms [1.36(1.16,1.59)]. Participants in the highest quartile of CMI had a 62 % higher risk of depression than participants in the lowest quartile [1.62(1.17,2.23)]. This positive correlation was more pronounced in those with hypertension. Conclusions CMI was associated with a higher PHQ-9 score and an increased likelihood of depression among US adults. Further large-scale prospective studies are still need to analyze the role of CMI in depression.

Abstract:Objectives: Non-high-density lipoprotein cholesterol (non-HDL-C) has attracted attention because it is associated with a variety of diseases and is easy to measure. However, the relationship between non-HDL-C and depression is still unclear. Our aim was to assess the relationship between non-HDL-C and depression using the cross-sectional NHANES survey from 2005 to 2018. Methods: We examined the association between non-HDL-C and depression using weighted multivariable logistic regression models and subgroup analysis. Sensitivity analysis demonstrated the robustness of the results. Results: There were 42,143 participants in this study and 8.6% had depression (weighted 7.53%). Non-HDL-C was higher in participants with depression compared to those without depression (weighted mean 3.64 vs. 3.73, p < 0.01). There was a positive association between non-HDL-C and depression with a 95% OR of 1.22 adjusted for multifactorial (95% CI,1.03–1.45). In subgroup analyses, non-HDL-C was positively associated with depression in men (OR, 1.31; 95% CI, 1.01–1.70), normal BMI (OR: 0.93; 95% CI: 0.66–1.32) and in participants without hypertension (OR, 1.29; 95% CI, 1.01–1.66). Conclusion: Non-HDL-C positively correlated with depression, and further research may be better for clinical service.

Abstract:Objective: We aimed to evaluate whether depression is associated with increased risk of dietary inflammatory index (DII) or energy-adjusted DII (E-DII) and whether the association is partly explained by insulin resistance (IR). Methods: Base on the National Health and Nutrition Examination Survey (NHANES) 2005–2018. Univariate analyses of continuous and categorical variables were performed using t -test, ANOVA, and χ 2 test, respectively. Logistic regression was used to analyze the relationship between DII or E-DII and depression in three different models. Mediation analysis was used to assess the potential mediation effects of homeostatic model assessment-IR (HOMA-IR). Results: A total of 70,190 participants were included, and the DII score was higher in the depressed group. DII score was related to all participant characteristics except age ( p < 0.05). After being included in covariates (Model 3), participants in the highest quartile of DII score have increased odds of depression ( OR : 1.82, 95% CI : 1.28–2.58) compared with those in the first quartile of DII score. And, a significant dose–response relationship was found ( p -trend <0.05). No interaction between DII and HOMA-IR was observed in terms of the risk of depression, and HOMA-IR did not find to play a mediating role in the association between DII and depression. Similar results were obtained for the association between E-DII and depression. Conclusion: Our results suggest that a higher pro-inflammatory diet increases the risk of depression in U.S. adults, while there was no evidence of a multiplicative effect of DII or E-DII and HOMA-IR on disease risk, nor of a mediating effect of HOMA-IR.

Abstract:Research on the association between level of depression and coronary heart disease (CHD), stroke risk, and all-cause and cardiovascular mortality is lacking in large-scale or population-based studies incorporating cardiovascular disease (CVD) endpoints. We aim to assess the relationship between the level of a person's depression and their risk of CHD, stroke, and all-cause and cardiovascular mortality. Utilizing data from the United States National Health and Nutrition Examination Survey (NHANES), multicycle cross-sectional design and mortality linkage studies were conducted. The study sample included 30918 participants aged 20–85 years old during the 2005–2018 period. Depression was assessed using the nine-item Patient Health Questionnaire (PHQ-9), with scores of 5, 10, 15, and 20 being the cut-off points for mild, moderate, moderately severe, and severe depression, respectively. A series of weighted logistic regression analyses and Cox proportional hazards models were utilized to examine the relationship between the level of depression with the risk of CHD, stroke, all-cause, and cardiovascular mortality. Trend analyses were conducted by entering the level of depression as a continuous variable and rerunning the corresponding regression models. Weighted logistic regression models consistently indicated a statistically significant association between the level of depression and increased risk of CHD and stroke, and those linear trend tests were statistically significant (P for trend < 0.001). Furthermore, weighted Cox regression analyses consistently indicated that participants who had a more severe degree of depression were at a higher risk of all-cause death, and trend analyses suggested similar results (P for trend < 0.001). Another weighted Cox regression analysis also consistently indicated that except for severe depression, the hazard of cardiovascular death was increased with each additional level increase of depression. Our study confirmed that the level of depression was strongly associated with CHD, stroke, and all-cause and cardiovascular mortality, even after accounting for other factors that could impact risk, including variables of age, gender, ethnicity, income, education, body mass index (BMI), marital, and smoking status.

Abstract:Background: Emerging studies reveal a shared pathophysiological underpinning for metabolic problems and mental illnesses. The present study aimed to determine the association between atherogenic index of plasma (AIP) and the incidence of major depressive disorder (MDD). Methods: 7,951 subjects of US adults were collected from the National Health and Nutrition Examination Survey (NHANES) 2005-2018. MDD was evaluated through the Patient Health Questionnaire (PHQ-9). Multivariate logistic regression, sensitivity analysis, and spline smoothing plot method were used to identify the relationship between AIP and MDD. The cut-off point was calculated using recursive partitioning analysis when segmenting effects emerged. The area under the receiver operating characteristic (ROC) curve (AUC) and Hosmer-Lemeshow test were conducted to evaluate the performance of AIP in identifying MDD. Subgroup analyses and interaction tests were used to explore whether the association was stable in different populations. Results: A positive correlation between AIP and PHQ-9 score and MDD was both observed in 7,951 subjects included in the study, with a significant threshold of -0.42 determined using recursive partitioning analysis. In the fully adjusted model, a positive association between AIP and PHQ-9 score and MDD was observed (β=0.46, 95% CI 0.14~0.78; OR=1.42, 95% CI 1.04~1.93). Individuals in the highest AIP quartile had a 0.39-unit higher PHQ-9 score (β=0.39, 95% CI 0.12~0.66) and a significantly 33% greater risk of MDD than those in the lowest AIP quartile (OR=1.33, 95% CI 1.02~1.73). Spline smoothing plot analysis further confirmed the positive and non-linear association between AIP and PHQ-9 and MDD. ROC analysis (AUC=0.771) and the Hosmer-Lemeshow test (χ 2 = 14.239, P =0.076) suggested an excellent performance and goodness-of-fit of the relatively optimal model. DCA and CIC analysis also revealed a favorable overall net benefit and clinical impact of the model. Subgroup analyses and interaction tests revealed that the association between AIP and PHQ-9 score and MDD remained consistent across different subgroups and was not modified by other covariates, and this positive correlation was more pronounced in those with diabetes or hypertension. Conclusion: An elevated AIP is linked to a higher chance of MDD, especially in those with diabetes or hypertension. Resolving dyslipidemia and managing comorbidities may help reduce the likelihood of developing MDD.

Abstract:Objective: Depressive symptom is a serious mental illness often accompanied by physical and emotional problems. The prevalence of depressive symptom in older adults has become an increasingly important public health priority. Our study used cardiometabolic indicators to predict depressive symptom in middle-aged and older adults in China. Methods: The data came from the China Health and Retirement Longitudinal Study 2011 (CHARLS2011), which was a cross-sectional study. The analytic sample included 8,942 participants aged 45 years or above. The study evaluated the relationship between cardiometabolic indicators and depression by measuring 13 indicators, including body mass index (BMI), waist circumference, waist-height ratio (WHtR), conicity index, visceral adiposity index (VAI), Chinese visceral adiposity index (CVAI), lipid accumulation product (LAP), a body shape index (ABSI), body roundness index (BRI), triglyceride glucose index (TyG-index) and its correlation index (TyG-BMI, TyG-waist circumference, TyG-WHtR). Binary logistic regression analysis was used to examine the association between thirteen cardiometabolic indicators and depressive symptom. In addition, the receiver operating characteristic (ROC) curve analysis and area under curve (AUC) were used to evaluate the predictive anthropometric index and to determine the optimum cut-off value. Results: The study included 8,942 participants, of whom 4,146 (46.37%) and 4,796 (53.63%) were male and female. The prevalence of depressive symptom in mid-aged and older adults in China was 41.12% in males and 55.05% in females. The results revealed that BMI [AUC = 0.440, 95%CI: 0.422–0.457], waist circumference [AUC = 0.443, 95%CI: 0.425–0.460], WHtR [AUC = 0.459, 95%CI: 0.441–0.476], LAP [AUC = 0.455, 95%CI: 0.437–0.472], BRI [AUC = 0.459, 95%CI: 0.441–0.476], CVAI [AUC = 0.449, 95%CI: 0.432–0.467], TyG-BMI [AUC = 0.447, 95%CI: 0.429–0.465], and TyG-waist circumference [AUC =0.452, 95%CI: 0.434–0.470] were weak predictors of depressive symptom ( p < 0.05) in males. In females, BMI [AUC = 0.470, 95%CI: 0.453–0.486], LAP [AUC = 0.484, 95%CI: 0.467–0.500], TyG-BMI [AUC = 0.470, 95%CI: 0.454–0.487], and TyG-waist circumference [AUC =0.481, 95%CI: 0.465–0.498] were weak predictors of depressive symptom ( p < 0.05). On the other side, VAI, ABSI, conicity index and TyG index could not predict depressive symptom in middle-aged and older adults. Conclusion: Most cardiometabolic indicators have important value in predicting depressive symptom. Our results can provide measures for the early identification of depressive symptom in middle-aged and older adults in China to reduce the prevalence of depressive symptom and improve health.

Abstract:Background: Depression is a common mental illness with a high prevalence in overweight or obese individuals. Recent studies suggest that the Metabolic Score for Visceral Fat (METS-VF) is a novel metric for assessing visceral fat levels, potentially linking metabolic disturbances to depression. This study explores the association between METS-VF and depression severity in overweight or obese U.S. adults. Methods: Data were obtained from the National Health and Nutrition Examination Survey (NHANES) 2007-2018 dataset, including 9,415 overweight or obese participants. Depression severity was measured using the Patient Health Questionnaire-9 (PHQ-9). To assess the association between METS-VF and depression, the study methodology included multivariate logistic regression, subgroup analyses, generalized additive model (GAM), and smoothed curve fitting. This study also calculated BMI for the Non-Hispanic Asian population from 2011-2018 and incorporated this data as part of a sensitivity analysis. Results: Elevated levels of METS-VF in overweight or obese participants were significantly associated with increased PHQ-9 scores and an increased likelihood of depression. Notably, this association remained significant after adjustment for multiple covariates. Smoothed curve-fitting plots showed no nonlinear association between METS-VF and PHQ-9 scores. Subgroup analyses confirmed the robustness of these results across populations, particularly among people under the age of fifty. The sensitivity analyses confirmed the robustness of the results in this study. Conclusion: METS-VF levels were positively associated with depression severity and the likelihood of depression in overweight or obese individuals, with the association being particularly pronounced in people under 50 years of age.

Abstract:Background: We aimed to evaluate the association between depression and major cardiovascular events and test whether the relationship between depression and cardiovascular events is influenced by lifestyle or metabolic risk factors. Methods: The China Cardiometabolic Disease and Cancer Cohort (4C) Study was a nationwide, multicenter, prospective cohort study. About 92,869 participants without cardiovascular disease or cancer at baseline were included. Depression status was evaluated by the Patient Health Questionnaire-9 (PHQ-9). Lifestyle information was collected by the questionnaire, and metabolic risk factors including waist circumference, blood pressure, lipid profiles, and plasma glucose were measured. Major cardiovascular events including cardiovascular death, myocardial infarction, stroke, and hospitalized or treated heart failure events were validated based on medical records. Results: During an average of 3.8 years of follow-up, we detected 2,076 cardiovascular events and showed that participants with depressive symptoms had an increased risk for cardiovascular events after adjustments [hazard ratio (HR): 1.29; 95% confidence index (CI): 1.08–1.53]. Stratified on metabolic risk status, the relationship between depression and cardiovascular events tended to be stronger according to the increasing numbers of metabolic risk factors, with HR (95% CI) of 0.98 (0.72–1.35) in the category with 0–2 metabolic risk factors, 1.36 (0.996–1.87) and 1.47 (1.13–1.92) for those with 3, and 4–5 metabolic risk factors, respectively, indicating an interaction effect ( P = 0.039). Conclusion: Depression was independently associated with an increased risk of major cardiovascular events. The effect was particularly prominent among populations at higher metabolic risk.

Abstract:Background: Diet and dietary inflammation play an important role in depression. The aim of this study was to assess the association of SFAs with depression risk and the mediating role of DII. Method: Among 22, 478 U.S. adults (≥ 20, years old) according to the National Health and Nutrition Examination Survey (NHANES), univariate logistic regression, and multivariate logistic regression were used to evaluate the association between dietary intake of SFAs and the risk of depression. Dietary inflammation levels were evaluated using the DII. Mediation analysis was used to investigate the risk of DII and depression. The nonlinear relationship between SFAs and depression was assessed using restricted cubic spline (RCS). Results: There was a significant difference in SFA 6.0 dietary intake between depression and non-depression individuals. After adjusting for potential confounders, multifactorial logistic regression results showed that SFA 8.0 (Q3 1.58 (1.09, 2.30), P-value = 0.017; Q4 1.55 (1.00, 2.42), P-value = 0.050) may increase the prevalence factor for depression, SFA 14.0 (Q3 0.67 (0.47, 0.94), P-value = 0.020) may decrease the risk of depression. There were sex and age differences in the effects of different subtypes of SFAs on depression. Dietary intake of SFA 12.0 content showed a nonlinear relationship with the risk of depression (P-value = 0.005). Furthermore, DII was recognized as a mediator of the association between SFAs and the risk of depression. Conclusions: The findings suggest that dietary intake of SFAs is associated with the risk of depression in relation to the chain length of SFAs, and this may be due to the mediating effect of DII.

Abstract:Cardiovascular disease (CVD) and depression are common diseases that lead to adverse health outcomes. Depressive Symptoms may be a risk factor for CVD. But few studies focused on the impact of socioeconomic factors, common medical history and dietary intake about this association. This study analyzed National Health and Nutrition Examination Survey (NHANES) 2007–2016. Complex sampling-weighted logistic regression models were used to compare the odds ratios (ORs) of CVD in participants with different depressive symptoms. 11,516 NHANES participants aged ≥ 40 years were included in the final analysis, of whom 1842 had CVD. Compared with participants with no/minimal depression, participants with mild, moderate, and moderately severe/severe depression had OR values of 1.25 (95% CI 1.01–1.54), 1.98 (95% CI 1.32–2.96), and 2.41 (95% CI 1.63–3.57). The association of depressive symptoms with CVD follow a dose-dependent pattern. The interactions of depressive symptoms with gender (Interaction P = 0.009), diabetes (Interaction P = 0.010), household income level (Interaction P = 0.002), dietary cholesterol intake (Interaction P = 0.017) on CVD were observed. More severe depressive symptoms are associated with increased risk of CVD in US population. The association may be more pronounced in the female population, population with diabetes, low family income level, or high dietary cholesterol intake.

Abstract:BackgroundCardiometabolic diseases are prevalent in the older population worldwide and might have important roles in developing depression. However, few studies have investigated the associations between cardiometabolic multimorbidity and depression in older Chinese people. We aimed to examine the associations between cardiometabolic multimorbidity and depression in middle-aged and older Chinese people.MethodsWe included 15 162 participants aged 45 years or above from the China Health and Retirement Longitudinal Study (known as CHARLS). For this study, cardiometabolic multimorbidity included hypertension, heart problems, dyslipidaemia, and diabetes. We used the Center for Epidemiological Studies Depression Scale-10 to assess depression. We used generalised estimating equations to estimate the associations between cardiometabolic multimorbidity and depression.FindingsAfter adjusting for covariates, compared with no cardiometabolic multimorbidity, one (odds ratio 1·10; 95% CI 1·03–1·17), two (1·37; 1·25–1·50), three (1·55; 1·32–1·81), and four cardiometabolic multimorbidities (1·73; 1·33–2·25) were associated with depression. Compared with people with no cardiometabolic multimorbidity, those with hypertension only were not associated with depression (1·02; 0·94–1·10), whereas hypertension with one (1·34; 1·21–1·48), two (1·55; 1·31–1·82), and three cardiometabolic multimorbidities (1·73; 1·33–2·25) were associated with an increased risk of depression. For those with hypertension, compared with hypertension only, hypertension with heart problems only (1·50; 1·28–1·74), dyslipidaemia only (1·17; 1·01–1·37), diabetes only (1·32; 1·07–1·62), heart problems and dyslipidaemia (1·67; 1·38–2·01), heart problems and diabetes (2·14; 1·48–3·10), and all cardiometabolic multimorbidities (1·77; 1·35–2·31) were associated with depression.InterpretationIn middle-aged and older Chinese people, hypertension with no cardiometabolic multimorbidity is not associated with depression, whereas hypertension with cardiometabolic multimorbidity is associated with an increased risk of depression. Abstract:Objective: Exploring the relationship between the cardiometabolic index (CMI) and serum testosterone levels as well as testosterone deficiency in American adult males. Additionally, comparing the diagnostic value of the CMI with several common obesity and metabolism indices for identifying testosterone deficiency. Methods: This cross-sectional study was conducted using data from the National Health and Nutrition Examination Survey (NHANES) from 2011 to 2016. Serum testosterone levels and testosterone deficiency were used as dependent variables, with the cardiometabolic index as the independent variable. Multivariable regression was used to assess the relationship between the independent and dependent variables, while subgroup analyses were performed to ensure the stability of the results. Smooth curve fitting was utilized to evaluate the nonlinear relationship between the CMI and testosterone levels. Receiver operating characteristic curves (ROC) were plotted for several obesity and metabolism prediction indices and the area under the curve was calculated to compare the specificity and sensitivity of each diagnostic index in the diagnosis of testosterone deficiency. Results: Among 3541 adult male participants, CMI is negatively associated with serum testosterone levels and positively associated with testosterone deficiency. In the fully adjusted model, for every unit increase in CMI, serum testosterone decreased by 14.89 ng/dl. Comparing the highest quartile to the lowest quartile of CMI, each unit increase in CMI, serum testosterone decreased by 98.58 ng/dl. Furthermore, each unit increase in CMI was associated with a 16% increase in incidence of testosterone deficiency. By plotting the ROC curves, we found that the AUCs for Lipid Accumulation Product (LAP), Body Mass Index (BMI), Weight Adjusted Waist Index (WWI), CMI, Visceral Adiposity Index (VAI) and Triglyceride glucose index (TyG) were 0.73, 0.72, 0.71, 0.69, 0.66, and 0.66 respectively. Conclusion: Elevated levels of CMI are associated with lower testosterone levels and an increased risk of testosterone deficiency. The predictive value of the LAP was superior to that of CMI, while the predictive value of CMI was higher than VAI and TyG.
The relationship between cardiometabolic index and infertility in American adults: a population-based study

Abstract:Background: Infertility is a fertility disorder caused by various factors, with lipid metabolism playing a crucial role in its development. The cardiometabolic index (CMI), which combines blood lipids (TG/HDL-C) and obesity-related parameters (WHtR), is a new quantitative indicator. This study used NHANES data to investigate the relationship between CMI and the incidence of infertility. Methods: We utilized data from women who took part in the National Health and Nutrition Examination Survey (NHANES) from 2015 to March 2020 to calculate the CMI index. Subsequently, we used multivariate logistic regression, smooth curve fitting, and subgroup analysis to investigate the relationship between the CMI index and infertility. Results: The logistic regression model revealed a significant positive correlation between the CMI index and infertility, even after adjusting for all confounding variables (OR=3.23, 95%CI: 1.55-6.73, p=0.0017). This association remained consistent across all subgroups (p>0.05 for all interactions). Smooth curve fitting demonstrated a positive nonlinear relationship between CMI and infertility. However, the CMI index had limited diagnostic power for infertility (AUC=0.60, 95%CI: 0.56-0.65). However, the CMI-BMI index combined with BMI had good predictive performance (AUC=0.722, 95%CI: 0.676-0.767). Conclusion: The CMI index shows a positive correlation with infertility, but its diagnostic value is restricted. The combination with BMI has good diagnostic value. Further investigation is required to determine the effectiveness of the CMI index as an early indicator of infertility.

Abstract:The Cardiometabolic Index (CMI) represents an innovative metric that encompasses abdominal obesity and lipid levels, offering a comprehensive assessment of cardiometabolic health. It is derived through the multiplication of the waist-to-height ratio by the triglyceride-to-high-density lipoprotein cholesterol ratio. Although obesity and blood lipid levels are known factors affecting sleep quality, the direct correlation between CMI and sleep quality has yet to be elucidated. This study uses NHANES data to explore the potential correlation between CMI and sleep quality. Our research employed a cross-sectional design, utilizing data from the National Health and Nutrition Examination Survey (NHANES) spanning 2011 to 2014. CMI was used as the exposure variable, with sleep quality assessed through three self-reported measures: trouble sleeping, sleep disorders, and sleep duration. We employed multivariate logistic regression models and linear regression model as well as subgroup analyses to explore the independent relationship between CMI and sleep quality. Additionally, interaction tests were conducted to examine differences across various subgroups. The study included 4298 participants, comprising 2134 males and 2164 females. There was a positive correlation between CMI and trouble sleeping (OR = 1.63, 95% CI 1.28–2.08) and sleep disorders (OR = 1.96, 95% CI 1.43–2.67), while there was a negative correlation between CMI and sleep duration (β = − 0.17, 95% CI − 0.31 to − 0.03), indicating that a higher CMI was associated with shorter sleep duration. Subgroup analyses and interaction tests revealed a consistently positive association between CMI and sleep difficulty across various populations, including gender, age, hypertension, diabetes, and cardiovascular disease history. However, the relationship between CMI and sleep disorders was more prominent among participants aged 50 and above. In American adults, a higher CMI is linked to an increased prevalence of sleep disturbances. In clinical practice, CMI can be considered as a supplementary factor in the assessment and management of sleep problems. Our study also provided new insights for improving sleep quality.

Bidirectional Associations Between Cardiometabolic Multimorbidity and Depression and Mediation of Lifestyles: A Multicohort Study

Abstract:Background: Cardiometabolic multimorbidity (CMM) and depression are major health concerns, and the onset of either condition may heighten the risk of developing the other. Objectives: The goal of this study was to characterize the reciprocal associations between CMM and depression among middle-aged and older adults. Methods: This multicohort study used harmonized data from 5 prospective cohorts from China, South Korea, the United States, the United Kingdom, and Europe. Cardiometabolic diseases (CMDs) (including diabetes, heart diseases, and stroke) and depression were assessed at baseline and at 7 to 8 years' follow-up. Lifestyle factors, including physical activity, alcohol consumption, and smoking status, were regarded as potential mediators. Two sets of analyses, CMM-depression analyses (n = 67,188) and depression-CMM analyses (n = 65,738), were conducted to explore the bidirectional associations between CMM and depression. Results: In the CMM-depression analyses, 16,596 (24.7%) individuals developed depression. Participants with a single CMD (HR: 1.24; 95% CI:1.19-1.29) and CMM (HR: 1.52; 95% CI: 1.42-1.63) at baseline had higher risks of depression occurring. Physical activity and alcohol consumption significantly mediated 7.5% and 6.9% of the CMM-depression association, respectively. In the depression-CMM analyses, 1,461 (2.2%) participants developed CMM. The HR for developing CMM was 1.31 (95% CI: 1.14-1.50) in patients with depression, with increased risk of developing more CMDs. Physical activity and alcohol consumption mediated 12.0% and 7.1% of the depression-CMM association. The bidirectional relationships were more pronounced in Western countries than in Asian countries. Conclusions: CMM and depression were bidirectionally associated. The mediated effects of lifestyle factors were larger in the depression-lifestyle-CMM pathway than in the CMM-lifestyle-depression pathway.

Abstract:Abstract Background: Heart failure (HF) is a prevalent global health issue with increasing incidence due to aging populations and advancements in treatment. The Cardiometabolic Index (CMI), a new marker combining waist-to-height ratio and the triglycerides-to-HDL cholesterol ratio, has shown promise in predicting cardiovascular risks. However, the relationship between CMI and HF remains unclear, warranting further investigation. This study aims to examine the association between CMI and HF to better understand and potentially identify HF risks. Methods: This study included 101,316 participants, of whom 22,042 met the selection criteria to investigate the correlation between the CMI and heart failure. The CMI is calculated as the product of the waist-to-height ratio (WHtR) and the triglycerides-to-HDL cholesterol ratio (TG/HDL-C). Data collection involved personal interviews to gather heart failure information, with HF diagnosis based on specific questionnaire responses. Clinical and biochemical data encompassed a wide range of variables, including demographic details, health status, and biochemical markers. Statistical analyses leveraged complex survey design from the National Health and Nutrition Examination Survey (NHANES), using weighted regression and chi-square tests to compare groups and multivariate logistic regression to examine the CMI-HF relationship across adjusted models. An analysis of the threshold effect elucidated the nonlinear dynamics present between CMI and HF, incorporating subgroup analyses to investigate the interactions among variables and the Receiver Operating Characteristic (ROC) curve was employed to evaluate the diagnostic utility of CMI in comparison to Body Mass Index (BMI) for the detection of HF. Results: In this study, based on specific inclusion and exclusion criteria, 706 individuals were diagnosed with HF, representing 3.2% of the total population. The findings indicated a significant association between elevated CMI levels and an increased risk of HF (OR = 1.13; 95% CI, 1.07–1.17, p < 0.001), with each unit increment in CMI level being associated with a 13% increase in HF risk. Subgroup analyses revealed the stability of the CMI-HF relationship across various subgroups, identifying race, history of heart disease, and hypertension status as key modulators of the strength and direction of the CMI-HF association. Moreover, smooth curve fitting and threshold effect analysis demonstrated a non-linear relationship between CMI and HF, with an inflection point at a CMI level of 6.49. Below this threshold, the incidence of HF increased with rising CMI levels. Additionally, the diagnostic capabilities of CMI and Body Mass Index (BMI) in identifying HF were compared, with the area under the curve (AUC) values for CMI surpassing those for BMI, indicating a superior ability of CMI in identifying HF. Conclusion: Our research indicates that the level of CMI bears a substantial positive correlation with the incidence risk of HF, with the relationship between CMI and HF being non-linear. Additionally, the CMI is a better predictor of HF than the BMI.

Abstract:Background: Chronic obstructive pulmonary disease (COPD) is the fourth leading cause of death worldwide and constitutes a global health problem. The cardiometabolic index (CMI) is a new metric that combines abdominal obesity and lipid levels. Studies have shown that the prevalence of lipid metabolism disorders is greater among COPD patients and that the CMI can help reveal the potential role of lipid metabolism in disease progression by assessing the body's metabolic status; however, the association between the CMI and COPD is not known. Objective: To explore the association between the CMI and the prevalence of COPD. Methods: A cross-sectional study was conducted with 14,340 participants aged ≥ 20 years from the 2007-2018 NHANES databases. To assess the relationship between the CMI and the odds of COPD prevalence, we performed multivariate logistic regression analyses, subgroup analysis interaction tests, smoothed curve fitting, and threshold effect analyses. Results: The study included a total of 14,340 participants, 48.49 % male and 51.51 % female, and the average age was 49.75 ± 17.49 years. According to the regression model adjusted for all confounding variables, participants in the highest quartile of the CMI had 22 % greater odds of having COPD than did those in the lowest quartile (OR = 1.22, 95 % CI: 1.03, 1.21, p = 0.010). A nonlinear association was found between the CMI and COPD, with an inflection point of 0.26. The OR (95 % CI) before the inflection point was 1.27 (1.12, 1.44), p = 0.0002. The interaction was statistically significant only in the sex analysis. Conclusions: The level of the CMI and the odds of COPD prevalence were positively correlated in our study. These findings suggest that managing abdominal obesity and lipid levels may help prevent or mitigate COPD, emphasizing the potential value of the CMI as an indicator for early intervention and precision therapy.

Abstract:Disparities in socioeconomic status (SES) are associated with an increased risk of cardiovascular disease (CVD). Depression is a highly prevalent cardiovascular risk factor among individuals with low SES. The present study aimed to gain a better understanding of the association between SES and CVD by examining the mediating effects of depression. 30,127 participants, 50.77 % female, with a mean age of 47.37 years from the National Health and Nutrition Examination Survey (NHANES) 2005-2018, were included. Weighted multivariable logistic regression analyses estimated the association between SES and CVD. Mediation analyses tested the mediating effects of depression. The multivariable-adjusted odds ratio (OR) of high SES for CVD was 0.42 (95 % confidence interval (CI): 0.36, 0.50; P < 0.001). In parallel, depression was negatively associated with SES, in which the multivariable-adjusted β was -1.42 (95 % CI: -1.62, -1.22; P < 0.001) in high SES compared to low SES. For the association between depression and CVD, the multivariable-adjusted OR per 1-point increase of PHQ-9 score for CVD was 1.08 (95 % CI: 1.07, 1.09; P < 0.001). The mediation models indicated that depression mediated 10.92 % of the total association between SES and CVD when fully adjusted. Identifying individuals with depression for appropriate treatment is promising for reducing the CVD burden among populations with low SES.

Abstract:Background: This study aimed to analyze the associations of cardiometabolic index (CMI) with diabetic statuses and insulin resistance (IR) using data from the National Health and Nutrition Examination Survey (NHANES) and examined the potential mediating role of inflammation in these correlations. Methods: This study enrolled 9477 participants across four NHANES cycles from 2011 to 2018. The primary outcomes of the study included the risk of having prediabetes, diabetes and the level of the homeostasis model assessment of IR (HOMA-IR). Other outcomes including the levels of fasting blood glucose (FBG), hemoglobin A1c (HbA1c), oral glucose tolerance test (OGTT) results, fasting insulin, the risk of oral hypoglycemic medicine use, insulin use, and retinopathy were also collected and analyzed. Logistic regression model, subgroup analysis, restricted cubic spine (RCS), and Pearson correlation coefficients were conducted to assess the associations of CMI with diabetic statuses and IR. The mediating role of inflammation was evaluated to investigate the potential mechanism of the associations between CMI and diabetic statuses. Results: Among included participants, the CMI levels in normal participants, prediabetes and diabetes in this study were 0.48, 0.73 and 1.07. After multivariable adjustment, CMI was positively associated with the risk of prediabetes (OR = 1.49, 95% CI = 1.24-1.79), diabetes (OR = 2.14, 95% CI = 1.82-2.50) and the level of HOMA-IR (β = 2.57, 95% CI = 2.14-3.01). Besides, an increased CMI was correlated with higher levels of FBG, HBA1c, OGTT results and fasting insulin as well as the greater risk of oral hypoglycemic medicine use and insulin use. The RCS showed an inverted L-shaped association of CMI with prediabetes and diabetes (P for non-linearity < 0.001). According to Pearson correlation coefficients, higher CMI was linked to higher rises in HOMA-IR (r = 0.224, P < 0.001). Inflammation-related indicators including leukocyte and neutrophil demonstrated significant mediating effects in the associations of CMI with prediabetes (15.5%, 9.8%), diabetes (5.1%, 6.0%) and HOMA-IR (3.3%, 2.6%). Conclusion: CMI was positively associated with the risk of worse diabetic statuses and higher level of IR while the associations may be partially mediated by inflammation-related indicators, suggesting that CMI could be a promising indicator for the prediction of severe diabetes and IR.
