Abstract:Objective: To examine the time course of aqueous-deficient and meibomian gland dysfunction (MGD) in patients with primary Sjogren's Syndrome (pSS). Methods: This prospective study was conducted on pSS female patients in the Department of Rheumatism of the Second Affiliated Hospital, School of Medicine, Zhejiang University. The age-matched MGD female patients without pSS (non-SS-MGD) were recruited as MGD controls from the Eye Center of the Second Affiliated Hospital, School of Medicine, Zhejiang University. After providing written informed consent, the patients underwent an eye examination and completed an Ocular Surface Disease Index questionnaire that assessed the symptoms of dry eye disease. The subjects were evaluated using Schirmer I test (SIt), tear meniscus height (TMH), noninvasive keratographic break-up time (NIKBUT), corneal fluorescein staining (CFS), and meibomian gland evaluation (meibomian gland infrared, lid margin score, expressible meibomian glands number and the secretions quality). The patients were divided into two groups: early stage (≤3 years) and late stage (>3 years) according to their medical history of dry eye. The data were analyzed using SPSS 20.0. Results: There were 49 pSS and 52 non-SS-MGD female patients enrolled in this study from 1 January 2018 to 30 December 2018. There were no differences in age (49.38 ± 10.32 and 48.69 ± 13.57 years) and dry eye medical history (48.44 ± 40.16 and 47.79 ± 37.85 months) between the two groups. When the medical history was ≤3 years, the average SIt and TMH of the pSS patients were significantly smaller than those of the patients with MGD. However, the signs related to the MGD did not show a significant difference between the two groups. When the medical history was >3 years, both the SIt and TMH and the signs related to MGD in pSS group were significantly more severe than the MGD group. Conclusions: Our results demonstrated that 3 years may be an important time node for the dry eye development in pSS patients, before this, the lacrimal glands received a greater influence, and then the meibomian glands began to be greatly affected.

Abstract:Mucosal chemokines have antimicrobial properties and play an important role in mucosal immunity. However, little is known about their expression on the ocular surface. This study aimed to analyze the expression of the mucosal chemokines CCL28, CXCL14 and CXCL17 in corneal and conjunctival epithelial cells under in vitro dry eye (DE) conditions, and in conjunctival samples from healthy subjects and DE patients. Human corneal epithelial cells (HCE) and immortalized human conjunctival epithelial cells (IM-HConEpiC) were incubated under hyperosmolar (400-500 mOsM) or inflammatory (TNF-α 25 ng/mL) conditions for 6 h and 24 h to measure CCL28, CXCL14, and CXCL17 gene expression by RT-PCR and their secretion by immunobead-based analysis (CCL28, CXCL14) and ELISA (CXCL17). Additionally, twenty-seven DE patients and 13 healthy subjects were included in this study. DE-related questionnaires (OSDI, mSIDEQ and NRS) evaluated symptomatology. Ocular surface integrity was assessed using vital staining. Tactile sensitivity was measured with Cochet-Bonnet esthesiometer, and mechanic and thermal (heat and cold) sensitivity using Belmonte's non-contact esthesiometer. Subbasal nerve plexus and dendritic cell density were analyzed by in vivo confocal microscopy. Conjunctival cells from participants were collected by impression cytology to measure mucosal chemokines gene expression by RT-PCR. Our results showed that HCE and IM-HConEpiC cells increased CCL28, CXCL14, and CXCL17 secretion under hyperosmolar conditions. The gene expression of CCL28 was significantly upregulated in conjunctival samples from DE patients. CCL28 expression correlated positively with symptomatology, corneal staining, heat sensitivity threshold, and dendritic cell density. CXCL14 expression correlated positively with age, ocular pain, conjunctival staining, tactile sensitivity, and image reflectivity. CXCL17 expression correlated positively with corneal staining. These results suggest that corneal and conjunctival epithelial cells could be a source of CCL28, CXCL14, and CXCL17 on the ocular surface and that CCL28 might be involved in DE pathogenesis.

Analysis of the correlations of mucins, inflammatory markers, and clinical tests in dry eye.

Abstract:Purpose: To investigate the expression of mucin and inflammatory factors in the conjunctival epithelium of subjects with dry eye and determine a correlation between clinical laboratory tests and subjective symptoms. Methods: The ocular surface disease index questionnaire, corneal fluorescein staining, tear film breakup time, and Schirmer I tests were administered in 66 subjects. According to questionnaire scores, the subjects were divided into mild, moderate, and severe symptom groups. Then, the subjects were divided based on the clinical tests into negative and positive groups. Impression cytology specimens from all patients were immunolabeled with antibodies directed against human mucin 5AC (MUC5AC) and interleukin-6 (IL-6) and the staining density was measured. Results: MUC5AC was lower in the severe symptom group compared with the mild and moderate symptom groups (P < 0.001, respectively). IL-6 was higher in the severe symptom group compared with the mild and moderate symptom groups (P < 0.001, respectively), and also in the moderate symptom group compared with the mild symptom group (P < 0.001). A decrease of MUC5AC was found in the positive group compared with the negative group (P < 0.001). IL-6 was higher in the positive group compared with the negative group (P = 0.029). MUC5AC was negatively correlated with the ocular surface disease index scores (r = -0.697, P < 0.001), whereas IL-6 positively correlated the scores (r = 0.895, P < 0.001). There were weak correlations for MUC5AC and IL-6 with tear film breakup time (r = 0.158 and r = -0.195, respectively; P = 0.038 and P = 0.041, respectively), and no correlations with other clinical tests. Conclusions: Dry eye disease is associated with decreased levels of mucin and an overexpression of IL-6, which correlate to the symptomatic severity of disease.
Correlation Study Between Abnormal Morphology of Meibomian Glands and Meibum in Patients With Dry Eye Disease Under in vivo Confocal Microscopy

Abstract:Objective: To observe differences in meibomian gland morphology among patients with meibomian gland dysfunction (MGD) with liquid meibum, with solid meibum, and a non-MGD group by laser scanning in vivo confocal microscopy (IVCM), and then analyze the correlation between meibomian gland abnormalities and dry eye disease. Methods: Twenty-one patients with MGD (42 eyes) with liquid meibum, 21 patients with MGD (38 eyes) with solid meibum, and 24 non-MGD patients (39 eyes) were enrolled in the study. IVCM examination and Schirmer II test were performed, and non-invasive tear-film breakup time (NIBUT) was measured. Results: Data obtained from all the patients were normally distributed; therefore, one-way analysis of variance was performed. The meibomian gland opening diameter and gland opening area of the patients with MGD were greater than those of the non-MGD patients, and values of the liquid meibum group were greater than those of the solid meibum group ( F = 17.96, p < 0.001; F = 8.84, p < 0.001, respectively). Due to changes in meibomian gland opening diameter and dilation of meibomian gland acini, the acinar longest diameter and unit area of the patients with MGD were also greater than those of the non-MGD patients, and the values of the solid meibum group were greater than those of the liquid meibum group ( F = 36.52, p < 0.001; F = 27.81, p < 0.001, respectively). In the aspect of acinar shortest diameter, there was no difference among the three groups ( F = 0.24, P > 0.05). Highest inflammatory cell density was observed in the solid meibum group, followed by the liquid meibum group, and the non-MGD group ( F = 111.54, p < 0.001). Similarly, the results of the Schirmer II test and NIBUT showed that the condition of the patients with MGD in the solid meibum group was worst, followed by that of the liquid meibum group and the non-MGD group ( F = 99.57, p < 0.001; F = 11.87, p < 0.001, respectively). Conclusions: The different meibum in the patients with MGD is accompanied by different meibomian gland morphologies. Compared with the patients with liquid meibum, those with solid meibum have more obvious dilatation of meibomian glands under confocal microscopy and in these patients, the density of inflammatory cells among glands is higher, and the quality of tears is worse.

Analysis of Tear Inflammatory Molecules and Clinical Correlations in Evaporative Dry Eye Disease Caused by Meibomian Gland Dysfunction

Abstract:Purpose To compare the levels of inflammatory molecules in tear samples between patients with meibomian gland dysfunction (MGD)-related evaporative dry eye (EDE) and healthy subjects and to analyze the correlations between the levels of tear inflammatory molecules and ocular surface parameters. Methods A total of 30 MGD-related EDE patients (48 eyes) and ten healthy volunteers (15 eyes) were enrolled. Dry eye-related examinations and questionnaires were obtained from all participants. The levels of nine inflammatory molecules were determined through multiplex bead analysis. Results Inflammatory molecules including ICAM-1, IFN-γ, CXCL8/IL-8, IL-6, TNF-α and IL-12p70 were detected in 100% of the patients, while IL-1α, IL-1β and IL-10 were detected in 56.25%, 13.60% and 45.83% of the patients, respectively. Moreover, ICAM-1, IL-8, IL-6, TNF-α, IL-12p70 and IFN-γ were detected in 86.67–100% of the healthy subjects, and the detection rates of IL-10, IL-1α and IL-1β were below 50%. The levels of IL-8, IL-6, IFN-γ and ICAM-1 were significantly higher in the patient group compared with the control group. In addition, IL-8 and IL-6 were negatively correlated with Schirmer I test. Besides, IFN-γ was negatively correlated with tear film breakup time. Furthermore, ICAM-1 and IL-6 were positively correlated with meibography score. Conclusions Collectively, patients with MGD-related EDE had higher levels of inflammatory molecules in their tears, and some molecules were correlated with ocular surface parameters. These findings suggested that inflammation played an important role in MGD-related EDE, and several inflammatory molecules could be used in the diagnosis and the treatment of MGD-related EDE.

Abstract:AIM: Tear proteins have an important role in the maintenance of the ocular surface and modulation of biological processes, including inflammation, in dry eye. Meibomian gland disease (MGD) is a condition that can increase inflammation in the ocular surface, but tear protein changes due to MGD have not been documented. This study evaluated the possible association of tear proteins with severity of MGD in dry eye.METHODS: Twenty-four patients with dry eye were evaluated. The panel of proteins found previously to be of interest and evaluated in this study were α-enolase, α-1-acid glycoprotein 1, S100A8 (calgranulin A), S100A9 (calgranulin B), S100A4 and S100A11 (calgizzarin), prolactin-inducible protein (PIP), lipocalin-1, lactoferrin and lysozyme. Tear protein ratios for each of 24 patients were calculated relative to pooled control from 18 healthy people, using isobaric tagging for relative and absolute quantification (iTRAQ)-based proteomics combined with two-dimensional-nanoliquid chromatography (LC)-nano-electrospray ionisation (ESI)-mass spectrometry (MS)/MS. The severity of MGD was clinically classified into grades 0-3 based on biomicroscopic signs.RESULTS: The levels of S100A8 and S100A9 were correlated to MGD severity. The level of S100A8 protein was significantly correlated to grittiness, whereas S100A8 and S100A9 were correlated to symptoms of redness and transient blurring. Lipocalin-1 was associated with heaviness of the eyelids and tearing.CONCLUSIONS: Distinct tear proteins are associated with MGD in dry eye patients, and some proteins were associated with distinct dry eye symptoms. These findings suggest that MGD may independently contribute to the symptomatology of dry eye patients.

Is Meibomian Gland Dysfunction One Disease? Heterogeneity Among Phenotypes

Abstract:Purpose: The purpose of this study was to examine ocular surface symptoms, tear metrics, and tear cytokines by Meibomian gland dysfunction (MGD) features. Methods: Symptom questionnaires and an ocular surface evaluation were performed on 40 individuals with varied MGD signs [Meibomian gland (MG) plugging, eyelid vascularity, meibum quality, and MG dropout]. Tear proteins were extracted off Schirmer strips and analyzed for 23 human inflammation-related proteins. Statistical analysis was performed to examine associations between dry eye metrics inflammatory proteins and MGD features. Results: The study involved 40 South Florida veterans with a mean age of 61 ± 13 years; most individuals were male (95%), White (31%), and non-Hispanic (85%). MGD features differentially related to dry eye signs. Eyelid vascularity, meibum quality, and MG dropout, but not MG plugging, correlated with higher corneal staining and lower tear production. MGD features also differentially related to tear cytokines. Eyelid vascularity most closely related to inflammation with significant correlations for interferon-gamma-γ (r = 0.36, P = 0.02), interleukin-4 (IL-4) (r = 0.43, P = 0.006), IL-17A (r = 0.42, P = 0.007), matrix metalloproteinase-2 (r = 0.39, P = 0.01), C-X-C motif chemokine ligand 5 (Regulated upon Activation, Normal T-Cell Expressed and presumably Secreted [RANTES]) (r = 0.32, P = 0.04), and tumor necrosis factor α (r = 0.36, P = 0.02). The other 3 MGD signs were less related to inflammation. Multivariable models revealed IL-4 to be most closely related to eyelid vascularity (standardized β = 0.39, P < 0.0001). Conclusions: Eyelid vascularity was the MGD sign most closely related to inflammatory cytokines, suggesting that different MGD features may be driven by different pathophysiological mechanisms.

Abstract:Meibomian Glands (MG) are sebaceous glands responsible for the production of meibum, the main component of the Tear Film Lipid Layer (TFLL). The TFLL facilitates the spread of the tear film over the ocular surface, provides stability and reduces tear evaporation. Alterations in meibum composition lead to different ocular alterations like Meibomian Gland Dysfunction (MGD) and subsequent Evaporative Dry Eye (EDE). The aim of the present study was to investigate the composition and abundance of meibum lipids and their relationship with eyelid margin abnormalities, lipid layer patterns and MG status. The study utilizes a lipidomic approach to identify and quantify lipids in meibum samples using an Elute UHPLC system. This system considered all four dimensions (mass/charge, retention time, ion mobility and intensity) to provide the accurate identification of lipid species. Samples were categorized as healthy or low/no signs of alteration (group 1) or severe signs of alteration or EDE/MGD (group 2). The current investigation found differences in Variable Importance in Projection lipid abundance between both groups for the MGD signs studied. Changes in meibum composition occur and are related to higher scores in eyelid margin hyperaemia, eyelid margin irregularity, MG orifice plugging, MG loss and lipid layer pattern.

Identification of Meibomian Phospholipids in Meibomian Gland Dysfunction Patients Using Liquid Chromatography/tandem Mass Spectrometry

Abstract:Objective To evaluate the differences in three kinds of meibomian phospholipid composition in healthy subjects and in patients suffering from meibomian gland dysfunction (MGD).Methods In this prospective study, 22 MGD patients (MGD group) and 30 age and sex matched heahhy individuals (control group) were enrolled.One eye of each subject was included in the study.MGD patients underwent sequential examinations as follows: the property of meibomian secretion, tear film break-up time (BUT), corneal fluorescein staining (CFS), Schirmer Ⅰ test and lid margin and ocular surface examinations by slit lamp.Meibomian gland secretions were collected using sterile stainless steel curettes in the healthy control group and MGD group after gentle massage of the lid margin with sterile cotton swabs.The meibomian phospholipids, mainly lysophosphatidylcholine,phosphatidylethanolamine, and sphingomyelin, were detected using liquid chromatography/tandem mass spectrometry (LC-MS/MS), and comparisons were made between the MGD group and the healthy control group.The data from the two groups were compared by an independent-samples t test and a Pearson correlation analysis was used to assess the correlation between the relative composition concentrations of the three kinds of phospholipids with the clinical parameters of the objective signs of MGD.Results The relative concentration level of lysophosphatidylcholine in the MGD group was significantly higher than that in the healthy controls (t=-2.45, P＜0.05), while phosphatidylethanolamine and sphingomyelin levels were lower in the MGD group than in healthy controls (t=16.73, P＜0.01 and t=18.59, P＜0.01, respectively).The relative concentration of lysophosphatidylcholine was positively correlated with the property of meibomian secretion, CFS, lid margin and ocular surface examination by slit lamp (r=0.602, 0.716, 0.710, P＜0.01, respectively);while a negative correlation was found with the BUT and Schirmer Ⅰ test (-0.628,-0.477, P＜0.05, respectively).The relative concentrations of phosphatidylethanolamine and sphingomyelin were negatively correlated with the properties of the meibomian secretion, CFS, lid margin and ocular surface examination (r=-0.565,-0.724,-0.702;r=-0.597,-0.719,-0.698, P＜0.01, respectively).However, a positive correlation was detected with the BUT and Schirmer Ⅰ test (r=0.659, 0.498, P＜0.05;r=0.631, 0.468, P＜0.05, respectively).Conclusion The profile of the three kinds of meibomian phospholipids (lysophosphatidylcholine, phosphatidylethanolamine and sphingomyelin) in MGD patients is dramatically different from healthy subjects.Its significant correlation with the clinical parameters of the signs of meibomian gland dysfunction suggest that a change in phospholipids composition in meibomian gland secretions is a probable reason for the development of MGD.
Conjunctivochalasis and Tear Osmolarity Are Associated With Reduced Conjunctival Epithelial Thickness in Dry Eye

Abstract:Purpose: To investigate the hypothesis that ocular surface epithelial thickness is correlated with tear osmolarity, conjunctivochalasis (CCh), and dry eye clinical tests. Design: A case-control study. Methods: A total of 50 patients with different types of dry eye disease (DED) and 15 age- and sex-matched control subjects were enrolled in this study. We performed a detailed diagnostic assessment of tear film and ocular surface parameters, including Ocular Surface Disease Index questionnaire, visual analog scale questionnaire, topographic surface regularity and surface asymmetry indices, tear film break-up time, corneal and conjunctival dye staining, Schirmer I test, and tear osmolarity. The corneal and bulbar conjunctival epithelial thickness (CET) were obtained using the latest version of spectral domain optical coherence tomography (SDOCT). Results: Patients with aqueous-deficient DED (ADDED) had lower bulbar CET values, particularly in the temporal region, than those of normal subjects and patients with evaporative-type DED (EDED); however, the difference did not quite reach a statistically significant level. Patients with DED and CCh had lower bulbar CET values in temporal (38.52 ± 9.58 µm) and inferior regions (50.79 ± 9.10 µm) compared to those with DED without CCh (47.39 ± 11.71 µm, 60.38 ± 14.36 µm, respectively, P < .02). In the DED group, tear osmolarity was found to be negatively correlated with CET values in temporal bulbar region (P = .006 and r = -0.403) and central corneal epithelial thickness values (P = .029 and r = -0.325). Conclusions: CCh and tear osmolarity are associated with reduced conjunctival epithelial thickness in DED.
Rete Ridges in Eyelid Margin and Inflammatory Cytokines in Meibomian Gland Dysfunction Associated with Dry Eye Symptom

Abstract:Purpose: To evaluate the morphology and function of rete ridges in eyelid margin and inflammatory cytokines in meibomian gland dysfunction (MGD) associated with dry eye symptom. Methods: A total of 63 subjects with OSDI score ≥13 were enrolled in MGD group and no-MGD group. Main measurements included tear cytokines levels and the rete ridges morphology. Results: Meibomian gland loss (MGL), corneal staining score (CSS), and IL-6 and TNF-α increased, meibomian gland secretion (MGS) decreased, the density of the rete ridges was lower, the longest diameters and shortest diameters of the rete ridges were longer in MGD group. The MGD group showed a negative correlation between MGL and BUT and MGS, but it showed a positive correlation with CSS. CSS was negatively correlated with IL-6. LLT was negatively correlated with IL-2 and IL-4. The shortest diameters of rete ridges in eyelid margin had a significant positive correlation with IL-2, IL-4, IL-10, and IFN-γ levels. Conclusion: Change of meibomian gland function and the rete ridges morphology might have some correlation with the injury to ocular surface. Some inflammatory cytokines were correlated with the change of the rete ridges morphology, which might in turn affect the ocular surface function.
Clinical ocular surface characteristics and expression of MUC5AC in diabetics: a population-based study

Abstract:Objectives: To investigate the clinical characteristics and the expression of mucin 5AC (MUC5AC) in in diabetic and non-diabetic subjects with or without dry eye disease. Methods: A total of 399 participants (796 eyes) aged 50-80 years were enrolled in this study. Participants were divided into 4 groups: non-diabetic without dry eye group (normal group), non-diabetic with dry eye group, diabetic without dry eye group and diabetic with dry eye group. Demographic information, fasting plasma glucose (FBG), and glycated haemoglobin A1C (HbA1C) data were collected. Additionally, ocular surface disease index (OSDI) questionnaire, signs of dry eye, tear osmolarity, and meibomian glands were evaluated. Tear MUC5AC expression and conjunctival goblet cells density (GCD) were tested. Results: Compared with non-diabetic with dry eye group, diabetic with dry eye group showed significantly lower tear film osmolarity (TFO), but higher corneal fluorescein and conjunctival lissamine green staining scores. In comparison with diabetic without dry eye group, diabetic with dry eye group showed significantly higher TFO, corneal fluorescein and conjunctival lissamine green staining scorers. The MUC5AC concentration and GCD of diabetic with dry eye group was significantly lower than those of the non-diabetic with dry eye group. Diabetic subjects with higher HbA1c levels (≥7.8%) showed higher TFO and shorter fluorescein tear break time. Conclusion: Diabetics with dry eye exhibited notably higher corneal fluorescein and conjunctival lissamine green staining scores. Conjunctival goblet cells and MUC5AC were significantly reduced in diabetics. Higher TFO was associated with the duration of diabetes and HbA1c levels.
Meibomian Gland Alteration in Patients with Primary Chronic Dacryocystitis: an in Vivo Confocal Microscopy Study

Abstract:Purpose: To evaluate meibomian gland (MG) alterations in patients with primary chronic dacryocystitis (PCD) by in vivo confocal microscopy (IVCM), and to correlate the finding with clinical presentation.Methods: Twenty-eight eyes with the diagnosis of PCD and their contralateral unaffected eyes were studied and compared with 27 normal controls. All subjects completed an Ocular Surface Disease Index questionnaire (OSDI) and underwent slit-lamp biomicroscopy examination, tear break-up time (BUT) measurements, fluorescein staining, Schirmer test I, and an IVCM examination of the MG. IVCM parameters, including the MG acinar unit density (MGAUD), periglandular inflammatory cell density (ICD), MG acinar unit longest diameter (MGALD), and MG acinar unit shortest diameter (MGASD) and their correlation with clinical data were analyzed.Results: The mean MG expressibility scores, BUT values, and staining scores were significantly worse in eyes with PCD compared with the contralateral clinically unaffected eyes and controls (p<0.05). A significant decrease in MGAUD was observed in PCD eyes compared with the controls and the contralateral clinically unaffected eyes. Conversely, the mean ICD and MGASD values were significantly higher in the PCD eyes. There were no significant differences in mean MGALD value between the PCD eyes and the contralateral clinically unaffected eyes. In addition, there were significant changes in the IVCM parameters in the contralateral unaffected eyes compared with the controls, including MGAUD, ICD, MGALD, and MGASD. All IVCM parameters showed a strong, significant correlation with MG dropout grades, MG expressibility, fluorescein staining scores, and OSDI values (all p<0.05).Conclusions: Patients with unilateral PCD demonstrated significant changes in MG as compared with the contralateral clinically unaffected eyes and controls. The MG function should be closely observed in these patients.

Abstract:Meibomian gland dysfunction (MGD) is a highly prevalent condition and the most common cause of evaporative dry eye disease. Studying the proteome of MGD can result in important advances in the management of the condition. Here, we collected tear film samples from treatment naïve patients with MGD (n = 10) and age-matched controls (n = 11) with Schirmer filtration paper. The samples were analyzed with label-free quantification nano liquid chromatography—tandem mass spectrometry. The proteins were considered differentially expressed if p < 0.05. A total of 88 proteins were significantly regulated. The largest change was observed in cystatin-SN, which was downregulated in MGD and correlated negatively with tear meniscus height. The downregulation of cystatin-SN was confirmed with targeted mass spectrometry by single reaction monitoring (SRM). Eighteen immunoglobulin components involved in B cell activation, phagocytosis, and complement activation were downregulated in MGD including Ig alpha-1 chain C region, immunoglobulin J chain, immunoglobulin heavy variable 3–15, and Ig mu chain C region. The changes in cystatin-SN and immunoglobulin chains are likely to result from the inflammatory changes related to tear film evaporation, and future studies may assess their association with the meibum quality.

Changes in Human Meibum Lipid Composition Related to the Presence and Severity of Meibomian Gland Dysfunction

Abstract:Purpose: Changes in meibum composition and quantity in meibomian gland dysfunction (MGD) result in tear film instability and dry eye. This exploratory study aimed to identify changes in (O-acyl)-ω-hydroxy fatty acid (OAHFA) and hydrocarbon chain (HC) unsaturation levels in meibum related to the presence and severity of MGD. Methods: Meibum samples were collected from 3 cohorts of adults with no MGD, mild-to-moderate MGD, and severe MGD in a noninterventional clinical trial (NCT01979887). OAHFAs, cholesterol esters (CE), HC unsaturation, and HC length in the meibum samples were quantified with 1H-nuclear magnetic resonance spectroscopy using 2 methods of normalization. Results: Meibum samples from 62 subjects were analyzed: 21 non-MGD, 21 mild-to-moderate MGD, and 20 severe MGD. Meibum OAHFA and CE levels and HC unsaturation were reduced with increasing severity of MGD, with most pairwise comparisons significant (P < 0.05, t-tests), following the order non-MGD > mild-to-moderate MGD > severe MGD. Regardless of the resonances used for normalization, each pairwise comparison of OAHFA, CE, and HC unsaturation levels in MGD (combined severities) versus non-MGD samples was significant (P < 0.01, t-test). Analysis using various normalization equations showed reductions of 20%-22% for OAHFAs, 51%-57% for CE, and 36%-66% for HC unsaturation in MGD (combined severities) compared with non-MGD. HC length was not altered in MGD (combined severities) compared with non-MGD samples (t-test). Conclusions: Meibum OAHFA, CE, and HC unsaturation levels were reduced in MGD and were lowest in the severe MGD cohort. These findings may contribute to the understanding of the pathophysiology of MGD.

Abstract:OBJECTIVE:To explore the composition of the ocular microbiome in normal subjects and patients with Meibomian gland dysfunction (MGD).SUBJECTS AND METHODS:Seventy subjects (140 eyes) were enrolled in our study. Signs of dry eye were evaluated and bacterial species in the conjunctival sac (CS) and Meibomian gland (MG) secretions were then identified by 16S rRNA gene sequencing. Additionally, 17 subjects (34 eyes) were further evaluated to determine differences in the microbiomes in the surface and deep layers of MG using a segmental secretion analysis.RESULTS:The positive bacterial isolation rate was markedly higher in MG secretions than in the CS. The bacterial composition of the control and mild group was simple, whereas the composition of bacteria was more complex as the severity of MGD increased. The positive bacterial isolation rate and number of bacterial types were significantly higher in the severe MGD group than those in the control, mild and moderate MGD groups. Corynebacterium macginleyi was only detected in the severe MGD group, with an isolation rate of up to 26.3%. Furthermore, a new grading system for bacterial severity of MGD was proposed and the severity of MGD appeared to be positively correlated with a higher grade of bacterial severity. The segmental secretion analysis showed severe MGD had a significantly higher incidence of bacterial discordance rate.CONCLUSION:The severity of MGD was positively correlated with a higher isolation rate, a greater number of bacterial species, and a higher grade of bacterial severity, which implied that MGD might be correlated with bacterial changes. This study provided some basis for the indications of antibiotic in clinical practice.

Abstract:Sixty-seven patients (38 woman; median age, 69 years) were enrolled to assess complement activation products (CAPs) in tear fluid with/without dry eye (DE) and with/without meibomian gland dysfunction (MGD). Patients were divided into four groups based on the presence/absence of DE and MGD: group DM had both DE and MGD, group DN had DE without MGD, group NM had MGD without DE, and group NN had neither DE nor MGD. The levels of C3a and C5a in the collected tears were analyzed using a cytometric bead array. The C3a concentrations in the DM, DN, NM, and NN groups were 2326 pg/ml, 1411 pg/ml, 1821 pg/ml, and 978 pg/ml, respectively. The C5a concentrations in the DM, DN, NM, and NN groups were 24.7 pg/ml, 15.3 pg/ml, 24.1 pg/ml, and 12.9 pg/ml, respectively. The concentrations of C3a and C5a in the DM and NM groups were significantly higher than in the NN group ( P < 0.05 for both comparisons). The CAPs in the tear fluid in MGD and DE increased. Local dysregulation of the innate immune system can be associated with the development of MGD and DE in elderly patients.

Abstract:Purposes: This study is to examine the efficacy of vitamin A palmitate eye gel in improving the density of conjunctival goblet cells and mucin-5 subtype AC (MUC5AC) production. Methods: Dry eye model was established in 60 rabbits. The rabbits were then randomly divided into 3 groups: group A - untreated; group B - treated with carboxymethylcellulose sodium eye drops three times a day for 28 days; group C - treated with vitamin A palmitate eye gel three times a day for 28 days. Break-up time, fluorescein and rose bengal staining, and Schirmer test were performed. Conjunctival impression cytology was used to detect conjunctival goblet cell density. Immunofluorescence and Western blotting was performed to detect MUC5AC. Results: On day 3, break-up time and the wet length for group C were lower than those for group B (P < 0.01). Fluorescein score of group C was higher than that of group A and B (P < 0.01). On day 14, fluorescein scores were decreased in groups B and C compared with that in group A (P < 0.01). On day 28, break-up time was significantly increased in group C (P < 0.05). On days 14 and 28, rose bengal scores were decreased in group C. Goblet cell density and MUC5AC production was significantly increased in group C on day 28, higher than that in group A and B. Conclusions: Vitamin A palmitate eye gel is effective in improving conjunctival goblet cell density and mucin production in dry eye syndrome.
