Abstract:Objectives: The aim of this study is to assess Heparin-binding protein (HBP) as a diagnostic and prognostic biomarker of severe sepsis in the pediatric intensive care unit (PICU).Methods: A multicenter, prospective study was conducted among children with sepsis in nine PICUs in China from October 2019 to June 2021. Plasma levels of HBP, procalcitonin (PCT), C-reactive protein (CRP), lactate, and white blood cell (WBC) count were determined at enrollment and 72 h after enrollment.Results: Of 355 included patients, 132 patients were diagnosed with non-severe sepsis (referred to as sepsis), 223 patients had severe sepsis. Patients with severe sepsis had significantly elevated levels of HBP compared with sepsis (median 170.5 vs. 74.1 ng/mL, P < 0.001). Adding HBP to a diagnostic model with PCT and lactate could significantly improve the diagnostic capability for severe sepsis. The plasma levels of HBP correlated positively with the number of dysfunctional organs. After adjusting for confounding factors, the declined levels of HBP at 72 h had a significant association with decreased in-hospital mortality (adjusted odds ratio (aOR) 0.242, P < 0.001). The levels of HBP showed weak positive correlations with PCT, CRP, WBC, and no correlation to lactate.Conclusions: HBP at enrollment can be an independent indicator for severe sepsis and the dynamic changes at 72 h can be a predictor for in-hospital mortality in PICU.
EFFECT OF HEPARIN ON ACTIVATION OF PLATELET FUNCTION IN PATIENTS DURING RADIOFREQUENCY CATHETER ABLATION

Abstract:1. Thromboembolism occurs in 0.4-2.0% of patients undergoing radiofrequency catheter ablation (RFCA). Some studies have shown that treatment with heparin inhibits the activation of coagulation and fibrinolysis. No study has directly measured the activation of platelet aggregation to investigate the effect of heparin on platelet function. The purpose of the present study was to observe the inhibitory effect of heparin on platelet activity in patients undergoing RFCA.2. Sixty-two patients with supraventricular tachycardia were observed and divided into a heparin-treatment group and a control group. Changes in platelet aggregability (PAG) and thromboxane B-2 (TXB2) in the blood samples of all patients at different times (before, after electrophysiological study, immediately after and 10 and 30 min after the RFCA procedure) were observed.3. No indication of clinically symptomatic thromboembolism and no major differences in baseline characteristics and procedure were apparent in either group. The levels of PAG and TXB2 were all clearly increased after the electrophysiological study (all P < 0.05). Immediately after RFCA, PAG and TXB2 levels were significantly increased in both groups and remained elevated 30 min after the procedure (all P < 0.05). In the heparin-treatment group, the increases in PAG (54.69 +/- 3.24%) and TXB2 (29.01 +/- 1.84%) caused by RFCA were lower than changes observed in the control group (70.92 +/- 3.45 and 44.70 +/- 3.28%, respectively; both P < 0.01). Moreover, treatment with heparin normalized the elevated level of PAG 30 min after RFCA more clearly.4. The results of the present study suggest that intravenous heparin treatment during the operation inhibits the activation of platelets induced by RFCA.
Assessing the impact of different heparin dosing regimens for cardiopulmonary bypass on anticoagulation: the HepDOSE pilot study

Abstract:Purpose: It has been suggested that a larger heparin dose during cardiopulmonary bypass (CPB) is associated with reduced perioperative coagulopathy and thromboembolic complications. We investigated the effect of different heparin doses during routine elective cardiac surgery. Our primary outcomes include blood loss and transfusion and secondary outcomes investigate the effects on coagulation biomarkers. Methods: In this prospective pilot trial, we allocated 60 patients undergoing cardiac surgery on CPB in a single tertiary cardiac centre into three groups to receive an initial dose of 300, 400, or 500 units (U) per kilogram of intravenous heparin prior to the commencement of CPB. Blood was sampled after induction of anesthesia, at 30 and 60 min of CPB, and three minutes after heparin reversal with protamine. Samples were analyzed for fibrinopeptide A (FPA), fibrinopeptide B (FPB), D-dimer, and thrombin-antithrombin (TAT) complexes. Postoperative blood loss and transfusion was measured for the first 24-hr period after surgery. Results: The total mean (95% CI) administered heparin dose in the 300 U·kg-1, 400 U·kg-1, and 500 U·kg-1 groups were 39,975 (36,528 to 43,421) U, 43,195 (36,940 to 49,449) U and 47,900 (44,807 to 50,992) U, respectively. There were no statistically significant differences in FPA, FPB or D-dimer levels at the measured time intervals. There was a trend towards lower TAT levels while on CPB with greater heparin dosing, which was statistically significant after the administration of protamine. The clinical significance appears to be negligible, as there is no difference in overall blood loss and amount of packed red blood cell transfusion or other blood product transfusion. Conclusion: This pilot study indicates that, while larger heparin dosing for routine cardiac surgery results in subtle biochemical changes in coagulation, there is no demonstrable benefit in postoperative blood loss or transfusion requirements.
Can Heparin Therapy Reduce 28-Day Mortality in Adult Severe Sepsis Patients?

Abstract:A study by Wang and colleagues [1] in a previous issue of Critical Care shows that heparin may reduce 28-day mortality in severe sepsis patients without causing an increased risk of bleeding. Therefore, the authors recommend the use of heparin for sepsis and severe sepsis. Although their result may significantly influence clinical practice, several confounding factors should be noticed. In this study, eight studies performed analyses of mortality and seven studies performed analyses of bleeding events. From Additional file two, three non-randomized controlled trials (NRCTs) account for 88.42% of weight on mortality [2-4]; and from Figure three, two NRCTs account for 92.27% of weight on bleeding events [2,3]. In other words, the results of this meta-analysis are almost equivalent to those of the meta-analysis of these five NRCTs. Given the above analysis, the subgroup analyses in this study were unnecessary. However, the three studies were only placebo groups in three large multi-center randomized controlled trials [2-4]. Patients in these placebo groups were not randomly assigned to receive heparin or other therapy. In addition, it is possible that patients receiving heparin were less severe, because the Acute Physiology and Chronic Health Evaluation II score was not reported in these placebo groups. Moreover, as the initial times of heparin treatment were not defined clearly in the above three studies, we cannot rule out the possibility that patients who had died early would have no chance to receive heparin, whereas patients who had a longer survival would be more inclined to receive heparin. Finally, the dose of heparin in the three trials, and within each trial, was not controlled.
Elevated plasma levels of heparin-binding protein in intensive care unit patients with severe sepsis and septic shock

Abstract:IntroductionRapid detection of, and optimized treatment for, severe sepsis and septic shock is crucial for successful outcome. Heparin-binding protein (HBP), a potent inducer of increased vascular permeability, is a potentially useful biomarker for predicting outcome in patients with severe infections. Our aim was to study the systemic release and dynamics of HBP in the plasma of patients with severe sepsis and septic shock in the ICU.MethodsA prospective study was conducted of two patient cohorts treated in the ICU at Karolinska University Hospital Huddinge in Sweden. A total of 179 patients was included, of whom 151 had sepsis (126 with septic shock and 25 patients with severe sepsis) and 28 a non-septic critical condition. Blood samples were collected at five time points during six days after admission.ResultsHBP levels were significantly higher in the sepsis group as compared to the control group. At admission to the ICU, a plasma HBP concentration of ≥15 ng/mL and/or a HBP (ng/mL)/white blood cell count (109/L) ratio of >2 was found in 87.2% and 50.0% of critically ill patients with sepsis and non-septic illness, respectively. A lactate level of >2.5 mmol/L was detected in 64.9% and 56.0% of the same patient groups. Both in the sepsis group (n = 151) and in the whole group (n = 179), plasma HBP concentrations at admission and in the last measured sample within the 144 hour study period were significantly higher among 28-day non-survivors as compared to survivors and in the sepsis group, an elevated HBP-level at baseline was associated with an increased case-fatality rate at 28 days.ConclusionsPlasma HBP levels were significantly higher in patients with severe sepsis or septic shock compared to patients with a non-septic illness in the ICU. HBP was associated with severity of disease and an elevated HBP at admission was associated with an increased risk of death. HBP that rises over time may identify patients with a deteriorating prognosis. Thus, repeated HBP measurement in the ICU may help monitor treatment and predict outcome in patients with severe infections.

Abstract:Introduction: Heparin-binding protein (HBP) is an antimicrobial protein stored in neutrophil granules and plays a role in endothelial permeability regulation. The aim was to assess the diagnostic and prognostic value of measuring HBP in patients with acute lung injury (ALI)/acute respiratory distress syndrome (ARDS).Methods: Plasma HBP was collected from 78 patients with ALI/ARDS, 28 patients with cardiogenic pulmonary edema (CPE) and 20 healthy volunteers at enrollment. Levels of HBP were measured by ELISA.Results: Patients with ALI/ARDS had significantly higher median levels of HBP compared with patients with CPE (17.15 (11.95 to 24.07) ng/ml vs. 9.50 (7.98 to 12.18) ng/ml, P < 0.001) at enrollment. There was no significant difference between CPE patients and healthy subjects in terms of HBP value (P = 0.372). The HBP levels of nonsurvivors was significantly higher than that of survivors (23.90 (14.81 to 32.45) ng/ml vs. 16.01 (10.97 to 21.06) ng/ml, P = 0.012) and multivariate logistic regression showed HBP (odds ratio = 1.52, P = 0.034) was the independent predictor for 30-day mortality in patients with ALI/ARDS.Conclusions: Plasma HBP levels of ALI/ARDS patients were significantly higher than that of CPE patients. HBP was a strong prognostic marker for short-term mortality in ALI/ARDS.
Effect of bydroxyethyl starch with different degrees of substitution on expression of platelet membrane glycoprotein

Abstract:Objective To investigate the effect of hydroxyethyl starch (HES) with different degrees of substitution on coagulation of platelets.Methods Sixty ASA classⅠpatients of both sexes (26 males, 34 females) aged 25-45 yrs weighing 51-70 kg undergoing elective minor surgery were randomized to receive lactated Ringer's solution (LR group) or HES 200/0.5 (HES group) or HES 130/0.4 (Voluven group) 20 ml·kg-1 after induction of anesthesia ( n = 20 each) . Venous blood samples were taken before surgery (T1, baseline) , at 15 min (T2) and 6 h (T3) after LR or HES infusion for determination of the expression of platelet membrane glycoprotein (GP), CD42b, CD61/41, CD62p by flow cytometry. Results There was no significant change in the expression of GP on the resting platelet after infusion of LR or HES in all patients. The expression of CD42b, CD61/41 and CD62p on the platelet activated by ADP was significantly decreased at 15 min after HES infusion as compared to the baseline at T1 in both HES and Voluven groups (P < 0.05 or 0.01) . The expression of CD42b, CD61/41 and CD62p was still significantly lower at 6 h after HES infusion than the baseline values in HES group (P< 0.01) while in Voluven group the expression of GP returned to the preoperative baseline level. Conclusion HES 200/0.5 and to a lesser degree HES 130/0.4 can both inhibit coagulation of platelet. Fluid therapy with HES 130/0.4 can decrease the risk of hemorrhage in postoperative period.

Abstract:BackgroundElevated plasma levels of heparin-binding protein (HBP) are associated with risk of organ dysfunction and mortality in sepsis, but little is known about causality and mechanisms of action of HBP. The objective of the present study was to test the hypothesis that HBP is a key mediator of the increased endothelial permeability observed in sepsis and to test potential treatments that inhibit HBP-induced increases in permeability.MethodsAssociation between HBP at admission with clinical signs of increased permeability was investigated in 341 patients with septic shock. Mechanisms of action and potential treatment strategies were investigated in cultured human endothelial cells and in mice.ResultsFollowing adjustment for comorbidities and Acute Physiology and Chronic Health Evaluation (APACHE) II, plasma HBP concentrations were weakly associated with fluid overload during the first 4 days of septic shock and the degree of hypoxemia (PaO2/FiO2) as measures of increased systemic and lung permeability, respectively. In mice, intravenous injection of recombinant human HBP induced a lung injury similar to that observed after lipopolysaccharide injection. HBP increased permeability of vascular endothelial cell monolayers in vitro, and enzymatic removal of luminal cell surface glycosaminoglycans (GAGs) using heparinase III and chondroitinase ABC abolished this effect. Similarly, unfractionated heparins and low molecular weight heparins counteracted permeability increased by HBP in vitro. Intracellular, selective inhibition of protein kinase C (PKC) and Rho-kinase pathways reversed HBP-mediated permeability effects.ConclusionsHBP is a potential mediator of sepsis-induced acute lung injury through enhanced endothelial permeability. HBP increases permeability through an interaction with luminal GAGs and activation of the PKC and Rho-kinase pathways. Heparins are potential inhibitors of HBP-induced increases in permeability.
Release of Hepatocyte Growth Factor Mediated by Heparin

Abstract:OBJECTIVE:To observe the effects of sodium heparin and low molecular weight heparin on the release of plasma hepatocyte growth factor (HGF) in senior coronary heart disease patients.METHODS:Fifty-four senior patients with coronary heart disease were divided into three groups: intravenous sodium heparin, subcutaneous sodium heparin, and subcutaneous low molecular weight heparin (LMWH). Plasma HGF and vascular endothelial growth factor (VEGF) were measured before and after injection.RESULTS:Plasma HGF was increased rapidly and significantly after intravenous injection of sodium heparin, reaching its peak level (about 48 fold) after approximately 10 minutes. Plasma HGF was also increased rapidly and significantly after subcutaneous injection of sodium heparin and LMWH, reaching its peak level (about 4 and 5 fold in sodium heparin and LMWH respectively) after approximately 2-3 hours.CONCLUSION:The rise of plasma HGF after heparin treatment suggests that heparin has some other biological effects in addition to its anticoagulant property through HGF. By this mechanism, the administration of heparin may be of some importance in the reparation of cardio-vascular diseases.
Assessment of the effect of unfractionated heparin administered either by intravenous infusion vs. subcutaneous injection on heparin-binding protein, and plasminogen activator inhibitor-1 in critically ill septic patients: a randomized controlled trial

Abstract:Objective: The study compared the impact of unfractionated heparin (UFH) administered via two routes (infusion and subcutaneous injection) on heparin-binding protein (HBP) and plasminogen activator inhibitor-1 (PAI-1) levels in critically ill sepsis patients. Patients and methods: Forty critically ill sepsis patients were randomly assigned to receive either a low-dose intravenous infusion of UFH (500 units/hour) or subcutaneous UFH (5,000 units/8 hours) for seven days. HBP and PAI-1 were measured at baseline and on days one, two, and seven. Results: Intravenous administration of UFH showed a significant reduction in percentage change of HBP compared to subcutaneous administration on days one [(-35% vs. -13%, p = 0.03*) (*indicates a significant result *p < 0.05, relative to the subcutaneous group)] and seven (-62% vs. -39%, p = 0.02*). Also, the percentage change of PAI-1 was significantly reduced in the infusion group compared to the subcutaneous group on days one (-28% vs. -3%, p = 0.008*), two (-42% vs. -3%, p = 0.001*), and seven (-62% vs. 27%, p = 0.001*), respectively. Furthermore, a significant improvement in the 14-day survival was observed in the infusion group compared to the subcutaneous group (p = 0.008*). Conclusions: Intravenous infusion was the route of choice for UFH administration in critically ill septic patients, with a promising effect on HBP, PAI-1, and survival.
Heparin binding protein in severe COVID-19—A prospective observational cohort study

Abstract:Background and aims Neutrophil-derived heparin binding protein (HBP; also known as azurocidin or CAP-37) is a key player in bacterial sepsis and a promising biomarker in severe infections. The aims of this study were to assess whether HBP is involved in the pathophysiology of COVID-19 and, if so, whether it can be used to predict severe disease preferably using a point-of-care test. Methods This was a prospective convenience sample study of biomarkers in patients admitted to Skåne University hospital in Sweden with a confirmed COVID-19 diagnosis. Plasma samples and clinical data were collected within 72h after admission, during hospital stay and at discharge. Plasma HBP concentrations samples were measured both with enzyme-linked immunosorbent assay (ELISA) and with a novel dry immunofluorescence analyzer (Joinstar) point-of-care test. Results Thirty-five COVID-19 patients were enrolled in the study. Twenty-nine patients had blood samples taken within 72h after admission. We compared the highest HBP value taken within 72h after admission in patients who eventually developed organ dysfunction (n = 23) compared to those who did not (n = 6), and found that HBP was significantly elevated in those who developed organ dysfunction (25.0 ng/mL (interquartile range (IQR) 16.6–48.5) vs 10.6 ng/mL (IQR 4.8–21.7 ng/mL), p = 0.03). Point-of-care test measurements correlated well with ELISA measurements (R = 0.83). HBP measured by the POC device predicted development of COVID-induced organ dysfunction with an AUC of 0.88 (95% confidence interval (CI) 0.70–1.0). Conclusions HBP is elevated prior to onset of organ dysfunction in patients with severe COVID-19 using a newly developed point-of-care test and hence HBP could be used in a clinical setting as a prognostic marker in COVID-19.

Abstract:BackgroundDetection of impending circulatory failure in a patient with sepsis is critical for modification of the treatment. Heparin-binding protein (HBP) is an inflammatory mediator with an important role in central sepsis mechanism.Design This study was a prospective, randomized, comparative one.Objective The aim of this study was to investigate the serum levels of HBP in patients with sepsis and to assess its value in the early detection of circulatory failure compared with the currently used biomarkers of sepsis.Patients and methods A total of 90 patients with signs of sepsis were prospectively enrolled in this study. After a 12-h study period, 41 patients had signs of sepsis (the sepsis group), 29 patients developed severe sepsis (the severe sepsis group), and 20 patients progressed to septic shock (the septic shock group). Blood samples were collected at enrollment and after 12 h for the measurement of white blood cell count and the serum levels of HBP, procalcitonin, lactate, and C-reactive protein. Vital signs were recorded at the same predetermined times.Results The serum levels of HBP were elevated up to 12 h before signs of circulatory failure were detected in patients with severe sepsis and septic shock. HBP serum level with an area under the curve value of 0.95 at a cutoff point of more than 31.6 ng/ml, sensitivity of 85.7%, specificity of 87.8%, positive predictive value of 89.4%, and negative predictive value of 83.7% was better indicator of circulatory failure compared with the other investigated biomarkers. After 12 h, the mean values of serum HBP were significantly increased in the severe sepsis group (75.67±11.85 ng/ml) and in the septic shock group (92.23 ± 16.13 ng/ml) compared with patients in the sepsis group (23.20 ± 5.26 ng/ml) (P Conclusion The increase in serum levels of HBP in patients with sepsis is an early predictor and diagnostic marker for the development of circulatory failure compared with the other investigated biomarkers.
Inverse Relationship of Bleeding Risk with Clot Burden During Pulmonary Embolism Treatment with LMW Heparin.

Abstract:IntroductionClinically relevant bleeding occurs three times as frequently as recurrent venous thromboembolism in the modern early treatment of pulmonary embolism (PE) with fixed-dose, unmonitored anticoagulants. Unfractionated heparin (UFH) is monitored and adjusted to assure efficacy and minimize bleeding risk, but low molecular weight heparin (LMWH) is not. PE requires more anticoagulant than isolated deep venous thrombosis. Speculating that PE with low clot burden could lead to excess bleeding with unadjusted LMWH treatment but not with UFH, we compared PE patients receiving either UFH or LMWH with high and low clot burden for clinically significant bleeding in an observational study.Materials and MethodsPatients with acute PE at multiple Chinese teaching hospitals had been randomized to UFH or LMWH for initial treatment. These treatment cohorts had baseline measurement of pulmonary artery obstruction (PAO) score, which was prospectively separated into quartiles, lowest to highest PAO. All patients were followed for bleeding episodes, which were subsequently analyzed by quartile of PAO.ResultsTwo hundred seventy-four patients divided between the two groups had similar efficacy and safety outcomes (12 clinically significant bleeds in the UFH group vs 15 in the LMWH group). LMWH recipients with the smallest clot burdens (lowest PAO quartiles) had highest bleeding rates (Cochran-Armitage trend test, P trend=0.048), but there was no such trend for UFH recipients.ConclusionsFor UFH, excess anticoagulant pro-hemorrhagic potential is down-adjusted via activated partial thromboplastin time monitoring, but for LMWH it is not. For PE patients at high bleeding risk, UFH may be safer if the clot burden is small.

Abstract:Background: The outcome of patients undergoing major surgery treated with HES for hemodynamic optimization is unclear. This post-hoc analysis of a randomized clinical pilot trial investigated the impact of low-molecular balanced HES solutions on the coagulation system, blood loss and transfusion requirements. Methods: The Trial was registered: EudraCT 2008-004175-22 and ethical approval was provided by the ethics committee of Berlin. Patients were randomized into three groups receiving either a 10% HES 130/0.42 solution, a 6% HES 130/0.42 solution or a crystalloid following a goal-directed hemodynamic algorithm. Endpoints were parameters of standard and viscoelastic coagulation laboratory, blood loss and transfusion requirements at baseline, at the end of surgery (EOS) and the first postoperative day (POD 1). Results: Fifty-two patients were included in the analysis (HES 10% (n = 15), HES 6% (n = 17) and crystalloid (n = 20)). Fibrinogen decreased in all groups at EOS (HES 10% 338 [298;378] to 192 [163;234] mg dl-1, p<0.01, HES 6% 385 [302;442] to 174 [163;224] mg dl-1, p<0.01, crystalloids 408 [325;458] to 313 [248;370] mg dl-1, p = 0.01). MCF FIBTEM was decreased for both HES groups at EOS (HES 10%: 20.5 [16.0;24.8] to 6.5 [5.0;10.8] mm, p = <0.01; HES 6% 27.0 [18.8;35.2] to 7.0 [5.0;19.0] mm, p = <0.01). These changes did not persist on POD 1 for HES 10% (rise to 16.0 [13.0;24.0] mm, p = 0.88). Blood loss was not different in the groups nor transfusion requirements. Conclusion: Our data suggest a stronger but transient effect of balanced, low-molecular HES on the coagulation system. Despite the decline of the use of artificial colloids in clinical practice, these results may help to inform clinicians who use HES solutions.

Abstract:Unfractionated heparin (UFH) was uncovered in 1916, has been used as an anticoagulant since 1935, and has been listed in the World Health Organization's Model List of Essential Medicines. Despite the availability of many other anticoagulants, the use of heparin (either low molecular weight heparin [LMWH] or UFH) is still substantial. Heparin has pleotropic effects including anticoagulant and several nonanticoagulant properties such as antiproliferative, anti-inflammatory activity, and anticomplement effects. Although UFH has been widely replaced by LMWH, UFH is still the preferred anticoagulant of choice for patients undergoing cardiopulmonary bypass surgery, extracorporeal membrane oxygenation, and patients with high-risk mechanical cardiac valves requiring temporary bridging with a parenteral anticoagulant. UFH is a highly negatively charged molecule and binds many positively charged molecules, hence has unpredictable pharmacokinetics, and variable anticoagulant effect on an individual patient basis. Therefore, anticoagulant effects of UFH may not be proportional to the dose of UFH given to any individual patient. In this review, we discuss the anticoagulant and nonanticoagulant activities of UFH, differences between UFH and LMWH, when to use UFH, different methods of monitoring the anticoagulant effects of UFH (including activated partial thromboplastin time, heparin anti-Xa activity level, and activated clotting time), while discussing pros and cons related to each method and comparison of clinical outcomes in patients treated with UFH monitored with different methods based on available evidence. Abstract:The effectiveness of Oxytetracycline 250 mg. 6 hourly for 2 weeks and erythromycin stearate 500 mg. 12 hourly for 2 weeks in the treatment of non-gonococcal urethritis (NGU) have been compared; cell culture for Chlamydia trachomatis was performed before and after treatment. There was no significant difference between the results obtained with the two antimicrobials. Both were clinically effective in the treatment of Chlamydia-positive NGU. Response to therapy was rapid; 2 weeks after treatment began, only 5 (14%) of 35 men treated with Oxytetracycline and 4 (13%) of 30 men treated with erythromycin stearate still had urethritis. Clinical improvement was accompanied in the majority of cases by failure to re-isolate C. trachomatis, but a small number of men who had not been exposed to the risk of reinfection yielded chlamydiae after therapy with Oxytetracycline or erythromycin stearate. The results of treatment of men with NGU from whom C. trachomatis had not been isolated were similar to those obtained in isolation-positive patients; again, there was no significant difference in the efficacy of the two antimicrobials. It is concluded that erythromycin stearate is a useful alternative to tetracyclines in the treatment of NGU.
Heparin Binding Protein and Endothelial Glycocalyx Markers in Severe COVID-19 – A Prospective Observational Cohort Study

Abstract:Abstract Background: The pathophysiology of severe COVID-19 has been implicated to involve neutrophil activation in the blood and in the lungs and endothelial dysfunction. Heparin binding protein (HBP) is a neutrophil protein that plays an important role in bacterial sepsis and is a promising biomarker in severe infections. Syndecans and glypicans are potential markers of sheeding of the glycocalyx and endothelial dysfunction.The primary aims of this study were to assess whether HBP or syndecans and glypicans are involved in the pathophysiology of COVID-19 and if so, whether they can be used to predict severe disease preferably using a point-of-care test (POC) that can substitute more time-consuming analysis with enzyme-linked immunosorbent assay (ELISA). Methods: A prospective convenience sample study of biomarkers. The main cohort consisted of patients admitted to hospital with a confirmed COVID-19 diagnosis. Samples and clinical data were collected at admission, during admission and at discharge and samples were analyzed with ELISA kit (Axis-Shield Diagnostics) for measuring HBP concentration and a novel dry immunofluorescence analyzer (Jet-iStar 800) (Joinstar) for point-of-care testing. Results: Thirty-five COVID-19 patients were prospectively enrolled in the study. HBP was significantly elevated in COVID-19 patients with organ dysfunction (n= 23) compared to those without organ dysfunction (n=6), 24.7 ng/mL (95% CI 17.3-48.4) vs 10.6 ng/mL (95% CI 6.2-17.1 ng/mL), p =0.03. Syndecan-1 and Glypican-4 were not significantly elevated in patients with organ dysfunction. Syndecan-1, 62.1 ng/mL (44.4-102.0) vs 57.5 ng/mL (95% CI 46.0- 63.7), p =0.44 and glypican-4, 3292.4 pg/mL (95% CI 1707.5- 6790.6) vs 3962.7 pg/mL (95% CI 2653.6- 5823.5), p =0.80. The point-of-care (POC) HBP test showed good correlation to the standard ELISA with an R-value of 0.83. HBP measured by the POC device predicted development of COVID-induced organ dysfunction within 72 hours with an AUC of 0.88. Conclusions: The neutrophil-derived HBP is elevated prior to onset of organ dysfunction in patients with severe COVID-19 using a newly developed point-of-care test and hence HBP could be used in a clinical setting as a prognostic marker in COVID-19.

Abstract:Heparin-binding protein (HBP) has been shown to be a robust predictor of the progression to organ dysfunction from sepsis, and we hypothesized that dynamic changes in HBP may reflect the severity of sepsis. We therefore aim to investigate the predictive value of baseline HBP, 24-h, and 48-h HBP change for prediction of 30-day mortality in adult patients with sepsis. This is a prospective observational study in an intensive care unit of a tertiary center. Patients aged 20 years or older who met SEPSIS-3 criteria were prospectively enrolled from August 2019 to January 2020. Plasma levels of HBP were measured at admission, 24 h, and 48 h and dynamic changes in HBP were calculated. The Primary endpoint was 30-day mortality. We tested whether the biomarkers could enhance the predictive accuracy of a multivariable predictive model. A total of 206 patients were included in the final analysis. 48-h HBP change (HBPc-48 h) had greater predictive accuracy of area under the curve (AUC: 0.82), followed by baseline HBP (0.79), PCT (0.72), lactate (0.71), and CRP (0.65), and HBPc-24 h (0.62). Incorporation of HBPc-48 h into a clinical prediction model significantly improved the AUC from 0.85 to 0.93. HBPc-48 h may assist clinicians with clinical outcome prediction in critically ill patients with sepsis and can improve the performance of a prediction model including age, SOFA score and Charlson comorbidity index.

Abstract:Objective To investigate the association of plasma heparin-binding protein (HBP) with the development of acute gastrointestinal injury (AGI) in critically ill patients. Design Clinical retrospective cross-sectional study. Setting A general teaching hospital in China. Participants Adult patients (age ≥18 years) admitted to our department with an intensive care unit (ICU) stay ≥5 days. Main outcome measures HBP levels were recorded twice or more within 5 days after admission. The initial AGI grades and the worst AGI grades within 5 days after admission, the number of patients receiving total enteral nutrition (TEN) and the number of patients with feeding intolerance (FI) and with sepsis were also recorded, along with some clinical severity and outcome variables. Results From June 2018 to May 2019, 221 patients were enrolled in this study. We divided patients into four groups based on the HBP values: HBP ≤20 ng/mL, 20<HBP ≤50 ng/mL, 50 100 ng/mL. Significant differences were found in the ratios of AGI deterioration and TEN and the incidence rates of FI and sepsis among the four groups. Differences were also found among the groups regarding the worst AGI grades. The area under receiver operating characteristic curves for AGI deterioration, severe AGI (grades II or above), TEN and FI were 0.738 (p=0.001), 0.774 (p<0.001), 0.810 (p<0.001) and 0.729 (p=0.001), respectively. The optimal HBP cut-off values for AGI deterioration and severe AGI were 53.27 ng/mL and 41.26 ng/mL, respectively. However, no differences in ICU duration or 28-day mortality were found. Conclusions HBP levels were associated with gastrointestinal dysfunction in critically ill patients. Increased HBP was positively correlated with sepsis but it was not correlated with 28-day mortality.

Abstract:Background . Heparin, used clinically as an anticoagulant, also has anti-inflammatory properties. The purpose of this systematic review was to provide a comprehensive review regarding the efficacy and safety of heparin and its derivatives as anti-inflammatory agents. Methods . We searched the following databases up to March 2012: Pub Med, Scopus, Web of Science, Ovid, Elsevier, and Google Scholar using combination of Mesh terms. Randomized Clinical Trials (RCTs) and trials with quasi-experimental design in clinical setting published in English were included. Quality assessments of RCTs were performed using Jadad score and Consolidated Standards of Reporting Trials (CONSORT) checklist. Results . A total of 280 relevant studies were reviewed and 57 studies met the inclusion criteria. Among them 48 studies were RCTs. About 65% of articles had score of 3 and higher according to Jadad score. Twelve studies had a quality score > 40% according to CONSORT items. Asthma ( n = 7 ), inflammatory bowel disease ( n = 5 ), cardiopulmonary bypass ( n = 8 ), and cataract surgery ( n = 6 ) were the most studied disease condition. Forty studies use unfractionated heparin (UFH) for intervention; the remaining studies use low molecular weight heparin (LMWH). Conclusion . Despite the conflicting results, heparin seems to be a safe and effective anti-inflammatory agent; although it is shown that heparin can decrease the level of inflammatory biomarkers and improves patient conditions, still more data from larger rigorously designed studies are needed to support use of heparin as an anti-inflammatory agent in clinical setting. However, because of the association between inflammation, atherogenesis, thrombogenesis, and cell proliferation, heparin and related compounds with pleiotropic effects may have greater therapeutic efficacy than compounds acting against a single target.
