Abstract:Background: With the evolution of facial rejuvenation procedures, thread lifts have gained space among western patients who seek cosmetic improvement without undergo to surgeries. Absorbable wedge-shaped polydioxanone (PDO) sutures are currently available, and in mid-lower face and mandibular jowl lift, threads are most often implanted in a diagonal vector force from medial-inferior up to lateral-superior site, near the temple. Herein, the authors introduce a new technique based on this oblique sum of anti-aging vector force to reposition and anchorage facial soft tissues, aimed for natural youth rejuvenation. Objective: Describe a novel technique for counteracting the facial ptosis and laxity skin, as well as the duration of clinical lifting result and of skin quality improvement. Methods: More than 84 patients were underwent in this thread technique once. The outcomes were assessed objectively using serial photography and subjectively based on the patients’ spontaneous satisfaction through over these 4 years. Complications were also reported. Results: Most patients considered the results satisfactory. The incidence of complications was low, and most of them were minor and temporary. Conclusion: Wedge-shaped PDO thread repositioning and anchorage soft tissue is safe and effective method for rejuvenation.

Abstract:Abstract Minimally invasive cosmetic procedures are in increasing demand worldwide. Patients are constantly searching for office-based techniques that can help improve the signs of aging but without the morbidity of a surgical procedure. Thread lifting with polydioxanone (PDO) sutures is a cosmetic procedure where lax tissue is lifted and repositioned trying to create a more youthful looking facial contour. The threads are absorbable and come in different shapes and lengths. The rate of complications and the downtime of the procedure are low, which are something patients are looking for. Ideally, these procedures should be performed by trained specialists who have complete knowledge of the facial anatomy, thus enhancing and improving results. Thread lifting with PDO absorbable threads has become an excellent option for minimally invasive facial rejuvenation procedures with a high satisfaction among patients and very few complications. It is a new tool that can be introduced in the armamentarium of specialists who work in the field of facial plastic surgery and who can offer this option to their patients with improved outcomes.

Abstract:As interest in skin aesthetics increases, treatments to suppress aging are increasing. Among them, a facelift is the most effective procedure for improving wrinkles. However, side effects including inflammatory reactions occur due to the limitations of the PDO thread itself used during the procedure. In this paper, to improve the function of PDO thread, inorganic particles such as magnesium hydroxide (MH) and zinc oxide (ZO) and a biologically active agent, asiaticoside, were coated on the surface of PDO thread using ultrasonic coating technology. The coated thread exhibited excellent biocompatibility, promoted collagen synthesis, reduced inflammation, and stimulated angiogenesis in vitro and in vivo. The multifunctional PDO thread has shown promising potential for skin regeneration without inducing fibrosis. Such a practical coating system and the developed multifunctional PDO thread suggest new possibilities for developing safer and more effective materials in cosmetic and regenerative medicine to prevent aging and improve skin aesthetics.

Abstract:Wired facial lifting is a minimally invasive technique that allows for a quick lifting effect. The aim of this study is to address the effectiveness of spiked Polydioxanone (PDO) threads (cog) for facial rejuvenation of the lower third of the face, through a clinical case report. The methodology was characterized by a brief bibliographic review, through an electronic search in the databases: Pubmed, Lilacs, Cochrane and bibliographic bases published in the USA and Brazil, associated with a clinical case report, making it possible to compare it with data from the literature on complications, safety and efficacy in the short and medium term. It is concluded that the aesthetic treatments seek to soften the signs of time and rescue the elasticity and firmness of the dermis. For this, it is necessary to elucidate the physiological effects, providing opportunities for safe treatment.

Abstract:The facial aging process is progressive, and rejuvenation of the oral region is challenging, as the skin alterations also reflect the changes in the fat, muscles, ligaments, and bone. Therefore, the procedures for improving this region must be combined to optimize the result. The use of polydioxanone (PDO) threads has been increasing in cosmetic dermatology in the last few years. Different types of threads, barbed, molded, non-barbed, straight, screwed, filler, etc., are available in the market bringing new options for treatment. Here, we want to present a new technique using a particular type of PDO thread, called solid filler thread, which was developed to fill folds in the face, replacing hyaluronic acid (HA) gel in some cases. Based on its purpose, it could also act as HA filler for myomodulation with the advantage of not promoting volume next to modiolus. In our technique, we associated HA filler gel in the areas where volume was desired, such as lips and mental area. We will report some cases to illustrate our technique and show the improvement in static and dynamic wrinkles, restructuring the oral region.

Abstract:BACKGROUND: Thread lifting is a minimally invasive technique for facial rejuvenation. Various devices for thread lifting using polydioxanone (PDO) are popular in aesthetic clinics in Korea, but there have been a few studies regarding its use.OBJECTIVE: To describe PDO thread and techniques adopted to counteract the descent and laxity of the face.METHODS: A retrospective chart review was conducted over a 24-month period. A total of 31 thread lifting procedures were performed. On each side, 5 bidirectional cog threads were used in the procedure for the flabby skin of the nasolabial folds. And, the procedure was performed on the marionette line using 2 twin threads.RESULTS: In most patients (87%), the results obtained were considered satisfactory. Consensus ratings by 2 physicians found that objective outcomes were divided among "excellent," "good," "fair," and "poor." Texture wise, the outcome ratings were 13 as excellent and 9 as good. Lifting wise, ratings were 11 as excellent and 6 as good. The incidence of complications was low and not serious.CONCLUSION: Facial rejuvenation using PDO thread is a safe and effective procedure associated with only minor complications when performed on patients with modest face sagging, fine wrinkles, and marked facial pores.

Abstract:BACKGROUND: Thread lifting with absorbable sutures has recently gained prominence for rejuvenating ptotic and sagging soft tissues in aging face. A barbed, delayed absorbable polyglyconate monofilament shows significantly high tensile loads with superior flexibility and elasticity.OBJECTIVE: To evaluate the safety and efficacy of novel thread lifting regimen with barbed polyglyconate suture for counteracting the descent and laxity of the aging face.METHODS: A retrospective chart review was conducted for 25 Korean patients with mild to moderate facial skin laxity receiving thread lifting procedures using barbed polyglyconate suture. Dermatologists' objective assessments using a 5-point Global Aesthetic Improvement Scale (GAIS), 3D imaging system, and subjective satisfaction were recorded at baseline and 4 months after treatments.RESULTS: In most patients (84%), GAIS scores were more than score 3 ("improved"), with 40% of score 4 ("much improved") and 24% of score 5 ("excellent") at the follow-up point. Subjective assessments paralleled these patterns. Three-dimensional analysis quantitatively showed volume migration of low face upward, clinically interpreted as facial lifting. No serious adverse effect was observed.CONCLUSION: This novel thread lifting procedure using barbed polyglyconate suture is a safe and effective method for tightening and lifting for aging face with mild to moderate face sagging.

The novel approach involves employing PDO volumizing threads to achieve physical augmentation akin to a "solid filler." The study introduces a more convenient insertion method for these threads, prioritizing ease and efficacy. Clinical cases demonstrate the efficacy of volumizing threads in addressing midface concerns, such as nasolabial folds and midcheek grooves. Additionally, the integration of volumizing threads to provide support in sagging areas is examined for achieving natural-looking enhancements. While highlighting positive outcomes, potential side effects like thread protrusion are addressed, along with strategies for their mitigation. Volumizing threads are presented as a suitable procedure for patients wary of traditional fillers or seeking subtle enhancements, with the recommendation of combining them with cog threads for those desiring more pronounced changes in facial contour. In summary, volumizing thread offers a minimally invasive alternative with fewer side effects for midface rejuvenation.

Abstract:Abstract Background The longevity of polydioxanone (PDO)-barbed lifting threads remains controversial. Objectives The authors sought to assess the longevity extension effect of a crisscross implantation pattern in PDO-barbed thread lifting. Methods To acquire the desired outcome in PDO-barbed thread lifting, the authors suggested a paradigm shift to incorporate biochemical factors in enforcing the physico-mechanical lift. A nude mouse model was employed to evaluate their theory to compare the conventional fan-shaped protocols in barbed thread lifting with an architectural construction of intersections of fibrous capsule in a crisscross pattern. Three fragments of monofilament PDO-barbed–lifting threads were implanted in the dorsal skin of 12 nude mice. The pattern of implantation was fan-shaped in the control group and crisscross in the experimental group. Tissue specimens containing tangential areas of threads were harvested, fixed, and paraffin-embedded. Samples were horizontally cut and histologically analyzed employing hematoxylin and eosin, Massons’ Trichrome, and Sirius red staining. Fibrotic areas and the width of fibrosis from the thread were also analyzed. Results Fibrous capsulations around the barbed area of the PDO-barbed lifting threads were threefold greater than those around the barb-free areas of the threads. In the crisscross implantation pattern, width and density of the fibrotic areas were fivefold greater than those of the fan-shaped areas. Induction of fibrous capsules around the PDO-barbed thread was markedly condensed in the crisscross areas. Conclusions This study provides the basis for a more logical implantation pattern in PDO-barbed lifting threads for facial rejuvenation. By generating controlled multiple crisscross patterns, we can create more intense fibrogenesis, reduce tension applied on each barbed thread, and, therefore, extend the longevity of the result. Abstract:Background: Thread lifting procedures for facial rejuvenation have made great strides with the development of absorbable threads. Although they have gained fame among plastic surgeons and dermatologists, there have been very few studies published in scientific articles on the effects of absorbable threads in facial rejuvenation, as well as few studies published by aesthetic physicians. The most effective and efficient techniques to determine the adequate place to insert the reabsorbable thread, and the different ways to evaluate the effectiveness of these aesthetic procedures, are still unknown. Objectives: This review aims to identify, in the scientific literature, the evaluation techniques for a safe and correct insertion of PDO threads in facial rejuvenation procedures. Methodology: A review of scientific literature was performed using the following list of keywords, descriptors, or thesauri: PDO threads, aesthetics, facial rejuvenation. Scopus, PubMed, and Web of Science databases were used for the literature search. Articles from years 2012-2022 were selected. The reference lists of the identified articles were included. (16) out of (35) articles associated with the topic were selected. Simple and compound searches were performed with the keywords and very few rigorous studies about the use of PDO threads for aesthetic procedures were found. Results: Very few rigorous scientific studies on the use of PDO threads for facial rejuvenation procedures were found. There is a strong theoretical and methodological gap on this subject, and, also, on the evaluation techniques that could be used for the safe and correct insertion of the threads. Conclusion: The bibliographic information found shows a huge theoretical and methodological gap in the subject of procedures for facial rejuvenation with PDO threads, as well as the techniques and tools employed to guarantee the correct insertion of the threads.

Abstract:Aims: To evaluate the efficacy of PDO thread lifting in lower face (marionette line) rejuvenation. Study Design: Quasi experimental study. Place and Duration of Study: Dermatology and Venereology Department, Tanta University Hospital, between January 2018 and April 2019. Methodology: The study included 10 patients asking for lower face rejuvenation who were treated by PDO absorbable threads (2 threads in each side of marionette line). Results: The mean validated grading scale for marionette line score before treatment was of 3.30 ± 0.82 while the mean score immediately after treatment was 1.70 ± 0.82. There was statistically significant improvement according to validated grading scale of marionette line after compared to before. These results were preserved after 1, 3 and 6months. Conclusion: PDO threads is a simple, rapid, immediately effective and office technique procedure.

Abstract:Facial lifting with polydioxanone barbed threads has been widely used in aesthetic treatment for years. However, gravity resists the thread and continuously pulls the face downward. This study aims to determine methods to lift the skin more efficiently with longer longevity. The quality of the thread is important and is defined by the pulling and pullout strengths. Moreover, the method of using threads is also important. We compared five thread-implantation techniques and six angles for the V-shaped implantation methods using a polydimethylsiloxane model to simulate thread migration in tissues. The results of the simulated thread-lift techniques can provide valuable information for physicians, enabling a more precise design of facelift surgery techniques.

Abstract:Purpose of reviewThis review focuses on prospective studies of vertigo and balance therapy in the past 3 years, including advances in vertigo-habituation exercises for adults, pediatric intervention, and virtual reality techniques, and, in more depth, the literature pertinent to driving motor vehicles. Recent findingsIncreased support has been generated for the efficacy of a minimal, home-based vertigo-habituation program for adults with peripheral vestibular disorders. Vestibular rehabilitation has been shown to be associated with improvements in independence and dynamic visual acuity. Community-based vestibular rehabilitation has been shown to be efficacious for selected patients, after careful screening, when trained personnel provide intervention. Vestibular rehabilitation has been incorporated into the rehabilitation program for head-injured military personnel who will be returned to duty, and multifactorial balance rehabilitation has been shown to be useful for children with hearing and balance impairments. Virtual reality techniques have made significant advances, so immersive environments have potential for rehabilitation for patients with vestibular disorders and for developing training regimens for astronauts to ameliorate some effects of exposure to microgravity. Driving skill, in general, is affected by use of benzodiazepines. For many patients with vestibular impairments driving is a particularly problematic activity of daily living. SummaryProgress has been made in studies of acute care, community-based, and pediatric vestibular rehabilitation. Work on simulator-based paradigms has moved toward readiness for implementation. Studies of driving have provided some insight into the problems of these patients. More work remains to be done on all of these problems.
Clinical applications of a novel poly-L-lactic acid microsphere and hyaluronic acid suspension for facial depression filling and rejuvenation

Abstract:Background: Poly L-lactic acid (PLLA) can stimulate fibrous tissue regeneration to exert a filling effect. However, severe inflammatory reactions and unsatisfactory effects remain a concern. Objective: Herein, we describe the mechanism of action, efficacy, and safety of PLLA microspheres in suspension (PLLA-b-PEG/HA) for facial contouring and soft tissue augmentation. Methods: PLLA-b-PEG/HA, ssynthesized by copolymerization with ethylene glycol, were suspended in hyaluronic acid (HA). Physiological verification was performed using scanning electron microscopy and X-ray computed tomography. PLLA-b-PEG/HA were subcutaneously injected into the dorsal region of 4-month-old rabbits. Ultrasound assessed volumetric capacity at 3 days and 1, 2, 4, and 12 weeks. The inflammatory response, collagen production, and HA degradation were evaluated. A retrospective case series of 10 patients who received PLLA-b-PEG/HA injections was conducted to assess long-term efficacy and safety. Results: PLLA-b-PEG exhibited a spherical structure with a smooth surface (20-45 μm diameter). In rabbits, implant site volume increased within 4 weeks, gradually decreasing thereafter. Fibrous capsules, microvessel density, and new collagen fiber formation progressively increased at 4, 12, and 26 weeks after injection. Clinical data demonstrated significant improvements in face contouring at months 3 and 12 after injection. All patients showed improved internal contours based on the Global Aesthetic Improvement Scale. After 12 months, 90% of the patients retained good shaping and support effects with minimal adverse reactions. Conclusions: PLLA-b-PEG/HA demonstrated superior biocompatibility and facial regeneration potential, with outstanding dual collagen-stimulating properties. The clinical efficacy and safety of PLLA-b-PEG/HA have been validated and established as a promising therapeutic option.

Abstract:Introduction: The facial aging process produces changes that are characteristic of the superficial and deep fat framework and skin layers. Subdermal suspension with threads enables the sagging tissues to be lifted by means of a minimally invasive, closed procedure without surgical dissection. This observational study has been carried out on the basis of standardized tridimensional photographic analysis and measurement, aimed at determining objective, repetitive, and reliable evaluation of the soft tissue suspension technique. Materials and Methods: Eight participants were enrolled in this pilot study presenting with mild to moderate ptosis of the jawline tissues. Patient photographs were taken before (t0), immediately after threads implantation (t1), and at the following visit (t2). Each image captured before thread insertion was registered by the software and surface linear lengths in between the mentioned points were calculated. Results: The result showed an overall average improvement in the "tragus-to-marionette distance" (C-A) and the "tragus-to-jowl distance" after a mean follow-up time of 8.16 months (t0-t2). All analyzed parameters improved significantly (P < 0.05) at t1 and at t2 with respect to t0. Conclusions: This pilot study suggest that facial tissue suspension by means of poli-lactic/poli-caprolactone threads is safe and effective in treating skin flaws that affect mild-to-moderate ptosis of the jawline up to 8 months.

Polydioxanone (PDO) biomaterials address aging concerns but produce acidic byproducts, causing inflammation. Inorganic particles and nitric oxide (NO) play crucial roles in inhibiting inflammation and promoting skin regeneration. Stem cell-derived extracellular vesicles (EVs) contribute to intercellular communication, offering the potential to enhance cell functions. The study proposes a method to enhance PDO-based medical devices by incorporating inorganic particles and immobilizing EVs, focusing on facial rejuvenation, anti-inflammatory response, collagen formation, and angiogenesis.
