PurposeTo report a challenging Descemet Membrane Endothelial Keratoplasty (DMEK) case, complicated by intraoperative aqueous misdirection and spontaneous anterior chamber fibrin reaction.MethodsA 70-year-old female affected by corneal edema due to Fuchs endothelial dystrophy underwent a triple procedure (cataract extraction - IOL implantation - DMEK surgery) in her left eye. This report illustrates the management of the intraoperative complications of aqueous misdirection syndrome and anterior chamber fibrin reaction.ResultsDespite the optimal management of the posterior pressure and the thorough removal of the fibrinous reaction during the case, the DMEK graft was not completely unfolded and centred at the end of the surgical procedure. Nonetheless, the patient showed good long-term anatomical and functional recovery: at the last follow-up (2 years after surgery), central corneal thickness was 526 μm with a best corrected visual acuity of 20/25 and an endothelial cell density of 1112 cell/mm2.ConclusionEarly recognition and prompt management of intraoperative aqueous misdirection syndrome and anterior chamber fibrin reaction during DMEK surgery is essential to ensure good functional and anatomical outcomes.

Abstract:We present an alternative surgical procedure including simultaneous deep anterior lamellar keratoplasty (DALK) and Descemet membrane endothelial keratoplasty (DMEK) in a case with endothelial failure and stromal scarring. A 62-year-old woman presented with vision loss caused by pseudophakic bullous keratopathy. While waiting for a corneal transplant, the patient developed infectious keratitis, which was treated with medication. Although the keratitis healed, it left a scar. To improve the patient's vision, a corneal transplant surgery that included simultaneous DALK and DMEK was performed. Postoperatively, the corneal graft was clear, and the Descemet membrane was well attached. However, there was an interface haze because of residual stromal tissue. The patient's best-corrected visual acuity improved from hand motion to 0.2 (decimal). This combined procedure allows for lamellar keratoplasty in cases with coexistence of corneal endothelial and stromal involvement.

Abstract:Objective: To investigate causes and management of surgical complications of Descemet membrane endothelial keratoplasty (DMEK) in initial cases. Methods: A retrospective study. A total of 52 eyes of 52 patients underwent DMEK by one surgeon in Peking University Third Hospital from February 2017 to December 2019, including 17 males and 35 females, aged (66±12) years. All patients' demographic characteristics, previous surgical history, intraoperative and postoperative complications were recorded. Results: Intraoperative complications included difficulties in unfolding the graft in 6 cases (11.5%), inverted grafts in 5 cases (9.6%), grafts rushing out of the anterior chamber in 2 cases (3.8%), grafts injected behind the iris in 3 cases (5.8%) and hemorrhage in 1 case. Partial graft detachment that occurred in 28 cases (53.8%) was the most frequent postoperative complication. Rebubbling was performed in 25 cases (48.1%). Pupillary block occurred in 9 cases (17.3%) and was managed by reducing air under a slit lamp. Primary graft failure occurred in 3 cases (5.8%) and viral infection occurred in 2 cases (3.8%). Conclusions: The rate of complications of DMEK in initial cases is higher than that of Descemet stripping automated endothelial keratoplasty. Intraoperative graft inversion and postoperative graft detachment are the most common complications. It is important to select appropriate indications and discover or manage intraoperative and postoperative complications in time to reduce the incidence of complications. Abstract:Purpose: To compare the postoperative complications and clinical outcomes of Descemet membrane endothelial keratoplasty (DMEK) and Descemet stripping automated endothelial keratoplasty (DSAEK) in eyes with previous glaucoma filtering surgery. Methods: In this retrospective comparative case series, we analysed postoperative visual acuity and intraocular pressure, graft survival, rate of graft detachment and/or dislocation, number of rebubbling and/or graft repositioning procedures, and graft rejection or failure (primary and secondary). Results: Sixteen eyes with DMEK and 80 eyes with DSAEK with previous glaucoma surgery were studied. The results were recorded at 3 and 12 months postoperatively. No statistically significant differences in postoperative visual acuity were found between the two groups at any stage of the follow-up. Intraocular pressure was lower in the DMEK group at the follow-up stage of 3 (p = 0.0022) and 12 months (p = 0.0480). Visually significant graft detachment was recorded in 31.3% and 22.5% of DMEK and DSAEK cases, respectively (p = 0.4541). All DMEK detachments (n = 5) were managed with slit-lamp rebubbling. Out of 18 graft detachments in the DSAEK group, 2 grafts were observed due to small graft detachment, 6 large graft detachments underwent rebubbling performed in the operating theatre, and 10 eyes needed primary graft repositioning for graft dislocation. Conclusions: DMEK is a feasible option to treat endothelial failure in complex eyes with previous glaucoma surgery. In the DMEK group, visual acuity outcomes and possibly postoperative intraocular pressure control were better compared with the DSAEK group.

Abstract:Purpose: To analyze the feasibility and outcome of Descemet membrane endothelial keratoplasty (DMEK) for treatment of endothelial failure in primary angle closure suspect (PACS) eyes. Methods: Retrospective, single-center case series of eyes treated by DMEK for endothelial failure caused by PACS. Main study parameters were success rate of DMEK, best-corrected visual acuity, anterior chamber depth, central corneal thickness, and endothelial cell density. Mean follow-up time was 16 ± 13 months. Results: Ten eyes of 9 patients receiving DMEK for the treatment of corneal endothelial failure because of PACS were included. Except for 2 eyes that had undergone cataract surgery, none of the eyes had previous ocular surgery. DMEK combined with cataract surgery was performed in 5 eyes, DMEK alone with second-step cataract surgery in 2 eyes. The eyes with corneal edema after cataract surgery received DMEK only. DMEK surgery was successful in nine out of 10 eyes, 1 patient required repeat DMEK because of primary graft failure. In the group of phakic eyes, mean preoperative internal anterior chamber depth was 1.74 ± 0.18 mm. In eyes with corneal edema, central corneal thickness was 849 ± 205 μm before DMEK surgery, and 517 ± 24 μm at the final postoperative visit ( P = 0.002). Conclusions: DMEK is a feasible option in eyes with endothelial failure because of primary angle closure. In case of advanced corneal edema, a second-step procedure (first DMEK, second cataract surgery) is a possible approach if visibility of the lens is too poor for simultaneous cataract surgery.

A miniaturized autonomous microsystem for hydrogen gas sensing applications

Abstract:This paper presents a fully integrated, ultra-low power microsystem that is used for hydrogen gas sensing in an autonomous wireless sensor node. The proposed circuit harvests solar energy from a micro-power photovoltaic module to measure temperature and hydrogen concentration and transmits the measured value using wireless data transmission. A rechargeable NiMH microbattery is used to store harvested energy. Photovoltaic module charges this microbattery, using a highly area- and power-efficient power management circuit. In order to measure hydrogen concentration, conductance change of a miniaturized palladium nanowire sensor is measured and converted to a digital signal with 12-bit resolution, using an area-efficient readout circuit. The proposed microsystem has been implemented in a 0.18μm CMOS process and occupies a core area of only 0.47mm2. This circuit features a low current consumption of 200nA for power management circuit and an additional 1.1μA for sensor interface circuit. It operates with low power supply voltage in the 0.8V to 1.6V range.

Abstract:Purpose: The aim of this review is to examine the techniques, complications, and outcomes of Descemet membrane endothelial keratoplasty (DMEK) in aphakic, aniridic, and vitrectomized eyes. Methods: A literature search was conducted in the MEDLINE database (via PubMed), using as keywords “(DMEK) AND (aphakia OR aniridia OR vitrectomy OR vitrectomized).” The research was limited to 10 years (January 2014–March 2024), in view of lack of literature before 2014. Articles, including case reports and case series, were included. Results: Twenty articles were included. No randomized controlled trials were found nor comparative studies with more than 1 technique used. Mean rebubbling rate in complex eyes was 29%, whereas mean endothelial cell loss at 6 months was 37%. The mean postoperative visual acuity improved from 1.47 logarithm of the minimal angle of resolution to 0.7 logarithm of the minimal angle of resolution. Conclusions: Despite being more challenging, compared with the techniques reported in literature, DMEK can be considered a valid option for the management of endothelial decompensation in complex eyes, with rebubbling rate and endothelial cell loss at 6 months, which are similar to non-complex eyes.

Abstract:Purpose: Several techniques have been developed for graft unfolding approaches in Descemet membrane endothelial keratoplasty (DMEK). However, despite these techniques, graft deployment and configuration in eyes with deep anterior chambers remain challenging in some cases. Therefore, in this study, we described a modified technique for DMEK, known as the “double-bubble technique assisted by holding forceps.” Methods: This was a retrospective interventional case series. Patients who underwent DMEK between August 2022 and July 2023, including cases with a history of vitrectomy and scleral fixation of intraocular lens, were enrolled in this study. Two experienced surgeons performed DMEK. In brief, after graft insertion into the anterior chamber, the first bubble with a small volume of air was injected above the graft to open the tight roll, and the graft edge was held using a 25-gauge graft manipulator. The second bubble was injected underneath the graft for fixation, while the graft edge was grasped using forceps during gas injection. The graft was released from the forceps. Best spectacle corrected visual acuity, central corneal thickness, endothelial cell density, and incidence of postoperative complications were measured before and after DMEK. Results: Eleven eyes of 11 patients were included in this study (mean follow-up period, 4.5 ± 4.4 months). Best spectacle corrected visual acuity and central corneal thickness significantly improved postoperatively ( P < 0.001). Rebubbling was required in 2 eyes; no other postoperative complications or primary graft failure were observed. Conclusions: The present technique enables safe and feasible DMEK surgery in vitrectomized eyes with scleral fixated IOLs and in those with a deep anterior chamber.

Abstract:A 68-year-old female underwent Descemet membrane endothelial keratoplasty (DMEK) in her right eye using a 3D visualization system with the surgeon looking directly to a digital screen instead of through the eyepieces of the surgical microscope. The procedure was uneventful. Five weeks after the surgery the DMEK graft was in good position and totally adhered, the cornea clear and uncorrected distance visual acuity 20/50. This is the first reported case of DMEK using 3D augmented reality visualization system. It seems to offer advantages for the corneal surgeon in critical steps of the endothelial grafting procedure.
Graft Failure after PKP and DMEK: What is the Best Option?

Abstract:Corneal transplantation is the most commonly performed human tissue transplantation procedure worldwide. Due to the large number of grafts, corneal graft failure has become one of the most common indications for corneal transplantation for immunological and non-immunological reasons (e.g. recurrence of underlying disease, high intraocular pressure, grafted guttae, transmitted HSV or CMV infection). The relatively recently developed lamellar grafting techniques have introduced certain potential complications that may lead to graft failure and require approaches other than penetrating keratoplasty for re-grafting. On the other hand, these new lamellar techniques also offer new possibilities for salvaging failed penetrating grafts, with potential advantages over successive penetrating keratoplasties, such as lower intraoperative risks, faster visual rehabilitation and reduced risk of immune reaction. Today, the patient with good healing who is satisfied with his graft before endothelial decompensation, with low astigmatism and no stromal scars, represents the optimal condition for DMEK after PKP. This can also be combined with phacoemulsification (so-called triple DMEK). Otherwise, a penetrating re-keratoplasty with a larger graft (typically excimer laser repeat PKP 8.5/8.6 mm) is performed to treat edema, scars and irregular astigmatism simultaneously. The medical history carries weight in this decision! Re-DMEK in case of graft failure after DMEK and DSAEK does not require any modification of the standard technique and leads to good visual acuity results if performed quickly. If there is clear stromal scarring after multiple (external) DMEKs, PKP can also be considered to rectify the situation. Otherwise causeless recurrent graft failures must suggest herpetic or CMV endotheliitis and, after PCR analysis of the aqueous humour aspirate, be treated appropriately with medication.

Abstract:OBJECTIVE:To investigate the clinical efficacy, prevention and management complications and choice of indication with Descemet membrane stripping with automated endothelium keratoplasty (DSAEK).METHODS:Clinical case series. Nine patients (9 eyes) with bullous keratopathy underwent DSAEK procedure from Peking University Eye Center, Peking University Third Hospital during September to December in 2007. Visual acuity, cornea transparency, graft position, corneal thickness, curvature, astigmatism and endothelial cells density (ECD) were observed over 3-7 month follow-up period.RESULTS:One patient with iris cornea endothelium syndrome was fail to insert graft into anterior chamber and changed procedure to penetrating keratoplasty. Eight patient successfully performed procedure. One graft dislocation in was managed in first day after surgery. All 8 grafts remained transparent and improved visual acuity. After surgery, mean corneal thickness was (775 +/- 30)microm. Mean corneal curvature was (44.19 +/- 2.28) D. Mean corneal astigmatism was (2.20 +/- 0.83) D. Mean endothelial cells density was (1439 +/- 296) cells/mm(2).CONCLUSIONS:DSAEK is a less damage, faster recovery vision, safer procedure. It is expected to be the important procedure to treat bullous keratopathy.

Abstract:Purpose: The aim of this study was to describe the efficacy of Descemet membrane endothelial keratoplasty (DMEK) in patients with corneal endothelial decompensation secondary to a forceps-induced corneal birth injury. Methods: This was a retrospective, noncomparative, interventional case series. Four eyes of 4 patients (1 female and 3 males; mean age, 64.0 ± 4.7 years) with corneal endothelial decompensation due to forceps-induced corneal birth injury were included. DMEK was performed in all cases, using a combined technique, including the use of intraoperative optical coherence tomography, vital staining of Descemet membrane of both host and donor, removal of scarred Descemet membrane with side-port forceps and vitreous cutter to smoothen the posterior corneal surface, epithelial peeling, and illumination for visualization. The examination included preoperative and postoperative ophthalmologic examinations: best-corrected visual acuity (converted to logarithm of the minimum angle of resolution [logMAR]), intraocular pressure, endothelial cell density (ECD), and central corneal thickness. Results: No postoperative complications were noted, and corneal transparency was maintained during follow-up (mean follow-up period, 32.0 ± 27.0 months; range, 3-71 months). The mean best-corrected visual acuity was 0.52 ± 0.35 logMAR preoperatively and 0.15 ± 0.09 logMAR at the last visit. The mean postoperative ECD was 1632 ± 631 cells/mm 2 (mean ECD at baseline, 3167 cells/mm 2 ). Central corneal thickness decreased from 640 ± 67 μm preoperatively to 576 ± 58 μm postoperatively. Conclusions: This study suggests that DMEK can be performed uneventfully in eyes with a forceps-induced corneal birth injury. The combination of surgical techniques may be an effective approach for DMEK.

Abstract:Introduction: The aim of the study was to report a unique case with excellent clinical outcomes after late endophthalmitis following Descemet's membrane endothelial keratoplasty (DMEK) surgery requiring donor graft removal without replacement. Case presentation: A 67-year-old female with a prior ocular history of bilateral cataract surgery, Fuchs endothelial dystrophy, and pseudophakic DMEK in the left eye presented with endophthalmitis 2 months after keratoplasty. DMEK graft removal without replacement with an intracameral washout, pars plana vitrectomy, intracameral, and intravitreal antibiotics resulted in an excellent visual outcome (20/25). Conclusion: This is a unique case of late endophthalmitis following DMEK surgery requiring graft removal and pars plana vitrectomy with excellent visual recovery without donor replacement.

Abstract:PURPOSE: To share the experience and provide a standardized protocol for Descemet membrane endothelial keratoplasty (DMEK) graft preparation.METHODS: A retrospective study based on 527 prestripped DMEK tissues that were prepared between 2014 and 2017. The experience of using different instruments and techniques has been described, and a standardized technique for preparing DMEK grafts has been identified. The tissues in general were prepared by superficially tapping the endothelial side with a Moria trephine (9.5 mm diameter). The plane of cleavage was identified using a cleavage hook, and the DMEK graft was deadhered from the trephined site throughout the circumference for ease of excising the graft. The DMEK graft was peeled using either one or multiple quadrant methods depending on the challenges faced during excision. The graft was finally marked with the letter "F" to identify the orientation during surgery. Data on endothelial cell loss (ECL) and challenging cases were observed, monitored, and recorded during this period.RESULTS: Less than 1 percent trypan blue-positive cells with tissue wastage of <6% was observed during the study period. Our standardized stripping technique has resulted in an overall ECL of 4.6%. Marking Descemet membrane showed 0.5% cell mortality.CONCLUSIONS: Standardizing DMEK technique using specific tools and simple techniques would help new surgeons to decide the instruments and improve their tissue preparation skills also in challenging cases such as previous cataract incisions or horseshoe-shaped tears, further reducing ECL or tissue wastage.

Case Report: Role of Anterior Segment Optical Coherence Tomography for Managing Failed Endothelial Keratoplasty Graft

Abstract:Significance: Some patients show poor visual outcomes after Descemet stripping automated endothelial keratoplasty. In such cases, secondary Descemet membrane endothelial keratoplasty can be performed to achieve complete visual recovery. Anterior segment optical coherence tomography (AS-OCT) is a valuable tool for the follow-up of posterior lamellar keratoplasty outcomes and complications. Purpose: This study aimed to report the clinical outcome of secondary Descemet membrane endothelial keratoplasty for managing poor visual results in a patient with graft failure after a previous Descemet stripping automated endothelial keratoplasty, highlighting the importance of AS-OCT in the follow-up of endothelial keratoplasty. Case report: A 38-year-old woman with high myopia underwent Descemet stripping automated endothelial keratoplasty for bullous keratopathy after explantation of an angle-supported phakic intraocular lens. Two years after keratoplasty, the patient experienced poor visual acuity (counting fingers), and significant corneal edema was observed on clinical examination hindering visualization of the anterior chamber structures. Anterior segment optical coherence tomography showed a failed and thickened graft adhering well to the recipient cornea in an anterior chamber without other comorbidities. Therefore, the graft was removed and replaced with a Descemet membrane endothelial keratoplasty graft without any complications. One year later, the clinical outcome was evaluated by comparing the pre-operative and post-operative best-corrected visual acuity, biomicroscopy findings, endothelial cell density, and corneal central thickness. Conclusions: Anterior segment optical coherence tomography is an important tool when deciding on the surgical technique to be applied and for the post-surgical monitoring of endothelial corneal grafts. This case demonstrates the successful management of Descemet stripping automated endothelial keratoplasty graft failure with Descemet membrane endothelial keratoplasty graft, highlighting the importance of AS-OCT in detecting complications such as graft dislocation and primary graft failure. In addition, corneal thickness measured using AS-OCT serves as a critical predictor of graft failure, as observed in this case.

Abstract:Sir, Median raphe cyst is a rare, benign congenital cyst presenting most commonly on the ventral aspect of penile shaft. These lesions present mostly in young men, whereas some cases in children have also been reported. It was first described by Mermet in 18951 and by Lantin and Thompson in 1956.2 It usually presents anywhere along the median raphe, i.e., in the midline from the urethral meatus extending ventrally to the anus.3,4

Abstract:Purpose Comparison of safety and clinical results of Descemet membrane endothelial keratoplasty (DMEK) in topical, peribulbar, or general anesthesia. Methods Retrospective, post hoc matched study of 346 patients who received DMEK surgery with different types of anesthesia (n = 54 topical, n = 137 peribulbar, n = 155 general anesthesia). Outcome criteria were intraoperative complications, endothelial cell count (ECC), central corneal thickness (CCT) and graft rejection rate, rebubbling rate, and visual acuity (VA). Mean follow-up time was 9.4 ± 2.8 months. Results The group with topical anesthesia showed intraoperative difficulties such as vitreous pressure (p = 0.01) and difficult graft unfolding (p = 0.4), possibly leading to a higher rebubbling rate (p = 0.03) and therefore graft failure (p = 0.39). However, rebubbling and graft failure occurred more often when the graft preparation was more difficult (p = 0.2, p = 0.13, respectively), which was independent of anesthesia. All three groups achieved comparable functional results regarding VA, ECC, and CCT after 6 months. Conclusion DMEK under topical anesthesia is feasible and shows comparable final visual outcomes but should be limited to selected cooperative patients and performed by experienced surgeons due to the potential for increased intraoperative challenges. Fragestellung Vergleich von Tropf-, Parabulbär- und Allgemeinanästhesie im Rahmen einer Descemet-Membran-Endothel-Keratoplastik (DMEK) im Hinblick auf Sicherheit und klinisches Outcome. Methoden Retrospektive, post-hoc-gematchte Studie von 346 Patienten, bei denen eine DMEK mittels verschiedener Anästhesieverfahren erfolgte (n = 54 Tropfenanästhesie, n = 137 Parabulbäranästhesie, n = 155 Allgemeinanästhesie). Entscheidende und berücksichtigte Faktoren für das schlussfolgernde Outcome waren dabei intraoperative Komplikationen, die Endothelzellzahl, die zentrale Hornhautdicke, sowie die Abstoßungsrate, Re-Bubbling-Rate und bester korrigierter Visus. Der durchschnittliche Beobachtungszeitraum nach erfolgter DMEK betrug 9,4 ± 2,8 Monate. Ergebnisse Intraoperative Komplikationen wie erhöhter Druck aus dem Glaskörperraum (Vis-à-tergo) (p = 0,01) und erschwerte Transplantatentfaltung (p = 0,4) traten in der Gruppe der in Tropfenanästhesie operierten Patienten auf, was möglicherweise zu einer höheren Rate an Re-Bubbling- (p = 0.03) und Transplantatversagen (p = 0,39) führte. Insgesamt traten ein Re-Bubbling oder ein Transplantatversagen häufiger bei erschwerter Transplantatpräparation auf (respektiv p = 0,2, p = 0,13), wobei die Transplantatpräparation insgesamt unabhängig von der gewählten Anästhesieform war. Unter Berücksichtigung von Visus, Endothelzellzahl und zentraler Hornhautdicke zeigten alle 3 Anästhesiegruppen nach 6 Monaten vergleichbare Ergebnisse. Fazit Die DMEK ist unter Tropfenanästhesie durchführbar und zeigt letztendlich vergleichbare Visus-Ergebnisse mit DMEKs unter Parabulbär- und Allgemeinanästhesie. Dennoch sollte die Tropfenanästhesie aufgrund des höheren Risikos für intraoperative Komplikationen ein Verfahren für besonders kooperative Patienten und erfahrene Operateure bleiben. Already known: Technical refinements such as no-touch preparation techniques, clear cornea cartridge implantation, and bimanual irrigation to ensure stable intraocular pressure have made DMEK a minimally invasive procedure. Newly described: Topical anesthesia is feasible for DMEK and yields comparable visual results when compared with other more invasive forms of anesthesia. Intraoperative difficulties such as vitreous pressure and difficult graft unfolding occurred more often with topical anesthesia. We recommend performing DMEK with topical anesthesia only in selected cooperative patients and solely by very experienced surgeons.
