Abstract:Background and Objective: Hemodialysis (HD) patients are more inactive, leading to poor functional capacity and quality of life; this may be reversed with intradialytic exercise training. To systematically evaluate the efficacy and safety of intradialytic exercise for HD patients, we conducted a metaanalysis of the published randomized controlled trials. Data Sources and Methods: Medline, Embase, and Cochrane Central Register of Controlled Trials were systematically searched up to February, 2014. The reference lists of eligible studies and relevant reviews were also checked. Results: 24 studies of 997 patients were included. Compared with control, intradialytic exercise significantly improve Kt/V (SMD = 0.27, 95% CI 0.01-0.53), peak oxygen consumption (VO2peak) (SMD = 0.53, 95% CI 0.30-0.76), and physical performance of physical function of life (SMD = 0.30, 95% CI 0.04-0.55). However, no significant improvements were found in the mental function of life. There was no significant difference with respect to musculoskeletal and cardiovascular complications between the intradialytic exercise groups and control groups. Further subgroup analysis found that, when the trial duration was more than 6 months, the intervention had significant effects on VO2peak (SMD = 0.89, 95% CI 0.56-1.22). However, when the trial duration was less than 6 months, the change of VO2peak was not significant (SMD = 0.19, 95% CI -0.13 to 0.51). Conclusion: Intradialytic exercise can improve Kt/ V, VO2peak, and the physical quality of life, and intradialytic exercise is safe for HD patients. Therefore, we put forward the suggestion that clinical guideline be updated to inform clinicians on the benefits of intradialytic exercise on HD patients. (C) 2014 S. Karger AG, Basel.
Effect of Intradialytic Exercise on Physical Performance and Cardiovascular Risk Factors in Patients Receiving Maintenance Hemodialysis: A Pilot and Feasibility Study

Abstract:Introduction:Although intradialytic exercise is considered a form of "nonpharmacological medicine" for patients receiving maintenance hemodialysis (MHD), this practice has not been widely implemented in most dialysis centers because of clinical limitations. We, therefore, aimed to design an intradialytic exercise training program to improve the implementation of this practice and determine its impact on physical performance and cardiovascular risk factors in patients receiving MHD.Methods:A total of 132 MHD patients at 4 outpatient dialysis units were enrolled and assigned randomly into exercise (n= 67) and control groups (n= 65). During a 2-year period, patients in the exercise group participated in 20-min exercise training sessions within dialysis sessions on 3 days per week. All patients underwent assessments of physical function (6-min walk test) and cardiovascular risk factors (blood pressure [BP], total cholesterol [TC], low-density lipoprotein [LDL], high-sensitivity C-reactive protein [hsCRP], albumin [Alb], hemoglobin [Hb], and erythropoietin [EPO] dose) at the baseline and annually thereafter.Results:Of the participants, 50.8% had completed the study after 2 years. No statistically significant intragroup or intergroup differences were observed in the measures of 6MD, BP, TC, hsCRP, Alb, Hb, and EPO dose.Conclusion:The results suggest that although this low-intensity, nonprogressive intradialytic exercise program may be practical, it was not sufficient to improve physiological function and reduce cardiovascular disease risk factors in patients receiving MHD.
Effects of intradialytic exercise on health-related quality of life in patients undergoing maintenance haemodialysis: a systematic review and meta-analysis

Abstract:Purpose: To summarize the current evidence regarding the effectiveness of intradialytic exercise (IDE) on the health-related quality of life (HRQOL) of patients undergoing maintenance haemodialysis. Methods: Five English databases (PubMed, EMBASE, Cochrane Library, Web of Science, and ScienceDirect) and four Chinese databases (VIP, WAN FANG, CNKI, CBM) were comprehensively searched from their inception to 18 March 2021. This study was reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. Two independent reviewers selected the papers and extracted the details of each study therein. Only randomized controlled trials were included. The risk of bias tool version 2.0 was used to evaluate the risk of bias of the included studies. A random-effects meta-analysis was conducted to pool the effect size. Results: Thirty-three eligible studies with 1481 participants were included. For the generic HRQOL, assessed by the Medical Outcomes Study Short-Form survey, IDE significantly improved most domains and the physical component summary compared with the control group. Furthermore, aerobic exercise alone significantly improved more domains compared to resistance exercise, combined aerobic and resistance exercise, and other types of exercise. Regarding the kidney-specific HRQOL, IDE improved three of eleven domains, including the symptom/problem list, the effect of kidney disease, and the quality of social interaction. No significant effect was found on other domains of kidney-specific HRQOL. Conclusion: Intradialytic exercise could benefit patients undergoing haemodialysis in improving most domains of generic HRQOL, but the effect on most domains of kidney-specific HRQOL is insufficient.
Effects of Different Exercises on Physical Function, Dialysis Adequacy, and Health-Related Quality of Life in Maintenance Hemodialysis Patients: A Systematic Review and Network Meta-Analysis

Abstract:Background: Clinical guidelines recommend exercise training for patients undergoing maintenance hemodialysis (MHD). However, the effectiveness of different types of exercise remains uncertain. Objectives: The aims of the study were to compare and rank the effect of different types of exercise on walking capacity, cardiorespiratory fitness, dialysis adequacy, and health-related quality of life (HRQOL) in patients undergoing MHD. Methods: Eight databases (four English and four Chinese) were searched from inception to January 1, 2022. Randomized controlled trials evaluating the efficacy of different exercises for patients undergoing MHD were included. Two independent reviewers screened the literature, extracted data, assessed the risk of bias, and evaluated the certainty of evidence. A frequentist random-effect network meta-analysis was conducted. Results: Ninety trials with 4,084 participants comparing 15 types of exercise were included, reporting on the six-minute walking test (45 trials), peak oxygen uptake (22 trials), dialysis adequacy (30 trials), and HRQOL (23 trials). Network meta-analysis showed that the most effective intervention for walking capacity was intradialytic aerobic exercise combined with blood flow restriction with a mean difference and 95% confidence interval of 97.35 (11.89-182.81), for peak oxygen uptake it was non-intradialytic combined aerobic and resistance exercise with a value of 4.35 (2.25-6.44), for dialysis adequacy it was intradialytic combined aerobic and resistance exercise with a value of 0.17 (0.06-0.28), for the physical component summary of HRQOL it was intradialytic aerobic exercise with a value of 4.93 (2.31-7.54), and for the mental component summary of HRQOL it was non-intradialytic combined aerobic and resistance exercise with a value of 6.36 (0.45-12.27). Ultimately, intradialytic combined aerobic and resistance exercise could improve all the above outcomes compared to usual care. Conclusions: This study concluded that intradialytic combined aerobic and resistance exercise is optimal for MHD patients due to its significant positive effects on multiple outcomes. Walking capacity can be further enhanced by combining blood flow restriction with exercise. For improving dialysis adequacy, intradialytic exercise proves to be more effective than non-intradialytic exercise. Further well-designed clinical trials are needed to investigate the effects of exercise with varying durations, intensities, and frequencies.

Abstract:Background: End-stage renal disease (ESRD) patients undergoing maintenance HD typically adopt a sedentary lifestyle which increases cardiometabolic risk and takes many years to be clinically detectable. Heart rate variability (HRV) is a non-invasive method to assess cardiac autonomic modulation. Interventions to increase HRV, such as exercise therapy, have been examined, given the theoretical plausibility that improvements in HRV may lead to improved outcomes. This study aimed at assessing the effects of aerobic exercise during hemodialysis sessions on the HRV of patients with ESRD. Methods: This research is a quasi-experimental study with pre and post-test randomized control group design. From the total of 20 ESRD patients undergoing maintenance HD randomly assigned to either exercise or control group, 2 subjects dropped out, leaving 18 ESRD patients. The patients in the exercise group were submitted to a low intradialytic aerobic exercise, performed during the first two hours of HD, twice weekly, for 12 weeks. At baseline and after 12 weeks, all participants underwent short-(for 5 min) term measurements of HRV. The data were analyzed in the SPSS package program. Results: There is a significant difference in post-pre changes (Δ) SDNN between the exercise and control group, but there were no significant differences in ΔRMSSD (p = 0.157) and ΔLF/HF ratio (p = 0.859), with the large effect size for ΔSDNN (Cohen’s D 1.01), medium effect size for ΔRMSSD (Cohen’s D 0.70), and very small effect size for ΔLF/HF ratio (Cohen’s D 0.09). Conclusion: Twelve weeks of supervised low-intensity intradialytic aerobic exercise has modified heart rate variability in maintenance hemodialysis patients.

Abstract:Introduction. The left atrial (LA) function plays a pivotal role in cardiac performance by modulating left ventricular (LV) function. Impairments in LV function are commonly reported during hemodialysis, but available data describing changes in LA function are limited. There is growing evidence of the cardioprotective effect of intradialytic exercise (IDE) on the LV function but studies analyzing its effect on LA function are scarce. Our aim was to evaluate whether IDE can limit the severity of hemodialysis-induced impairment in LA myocardial function. Methods. In this prospective, open-label, two-center randomized crossover trial, fifty-six stable individuals receiving hemodialysis participated in 2 hemodialysis sessions in a random order: standard hemodialysis (HD) and a session incorporating 30min of aerobic exercise. LA and LV global longitudinal (GLS) strains were obtained before and at peak stress of hemodialysis (i.e. 30min before hemodialysis-ending). Results. IDE totally abolished the decline in LA reservoir strain observed during HD (estimated difference: 3.1%, 95% CI 0.4/5.8, p=0.02) whereas it did not affect the other components of LA mechanics. A similar result favoring IDE intervention was also demonstrated on GLS changes over the HD procedure (p 34 mL/m2). Conclusion. Even a short duration of IDE at moderate intensity is effective in preventing hemodialysis-associated decline in LA reservoir function. Further research is needed to explore the long-term benefits of IDE on LA function.

Effect of Exercise on Fatigue in Patients Receiving Maintenance Hemodialysis Treatment: A Systematic Review and Network Meta-analysis

Abstract:Objective: This study aimed to evaluate the effect of different exercises for improving fatigue in maintenance hemodialysis patients. Design: PubMed, Embase, Web of Science, Cochrane, PEDro, CNKI, VIP, Wanfang data, CBM, and CINAHL were conducted from the establishment of the database to July 2023. We independently screened the literature, evaluated the quality, analyzed the data using Stata 15.0, and performed subgroup analysis on intervention time and exercise intervention adherence. Results: Twenty-three studies were analyzed, with 1867 patients and nine interventions. Based on the surface under cumulative ranking curves and pairwise comparisons effects, overall long-term, short-term, and exercise intervention adherence >90% effects have all presented that during dialysis aerobic-resistance combined exercise was ranked as the most effective compared with routine care (surface under cumulative ranking curves = 83.9%, standardized mean difference = -1.45, 95% CI = -2.23 to -0.67; surface under cumulative ranking curves = 85.9%, standardized mean difference = -1.38, 95% CI = -2.24 to -0.52; surface under cumulative ranking curves = 86.6%, standardized mean difference = -1.54, 95% CI = -2.58 to -0.49; surface under cumulative ranking curves = 83.6%, standardized mean difference = -1.45, 95% CI = -2.25 to -0.66, respectively), followed by interdialytic period muscle relaxation exercise, interdialytic period aerobic-resistance combined exercise, interdialytic period aerobic exercise, and interdialytic period Baduanjin. Conclusions: Dialysis aerobic-resistance combined exercise may be considered in practice when resources allow. However, high-quality, multicenter, and large-sample randomized control trials must further verify this conclusion.

Abstract:Introduction: Fatigue is a common and debilitating symptom experienced by patients undergoing hemodialysis. Intradialytic exercise, which involves physical activity performed during dialysis treatment, has been studied as a potential intervention to alleviate this fatigue. This literature review aims to determine the effects of intradialytic exercise on fatigue in patients undergoing hemodialysis. Methods: The literature review search was carried out using three databases, namely Google Scholar, PubMed, and Science Direct. The inclusion criteria for the literature review search were articles with publication years 2019-2024 in Indonesia and English. The articles could be open access. The keywords used in searching for the article were “intradialytic AND fatigue.” Results: The literature review identified fifteen studies for inclusion in the final analysis. Three articles used randomized control trials, seven articles used quasi-experiments, and five articles used pre-experiments. Many types of intradialytic exercises during hemodialysis were discovered. Conclusion: Intradialytic exercises have proven successful in reducing the level of weakness in patients undergoing hemodialysis. Fifteen research articles were found to support the provision of intradialytic exercises to patients undergoing hemodialysis. Intradialytic exercises can include active movement exercises, leg exercises, and relaxation movements.
Cardiovascular adaptations associated with exercise in patients on hemodialysis

Abstract:Patients on hemodialysis are physically inactive. Less than 50% of hemodialysis patients undertake exercise once a week and such patients have increased mortality compared to patients who undertake regular exercise. The reasons for physical inactivity and reduced functional capacity are complex and inter-related, with skeletal muscle catabolism, chronic inflammation, anemia, malnutrition, uremia, the burden of co-morbid diseases, and "enforced" sedentary time during hemodialysis all contributing. Many of these factors drive cardiovascular disease (CVD) processes in this cohort of patients and in the general population, exercise interventions have been shown to modify many of these risk factors. Whilst there is increasing evidence about the beneficial effects of exercise interventions on quality of life, functional capacity, aerobic fitness, and muscular strength, there are few compelling data on the effects of such programs on cardiovascular outcome measures. The reasons for this are manifold and include: limitations in study size; inconsistencies in study design; the heterogeneous nature of exercise interventions; assessment of nonstandardized outcome measures and; a lack of understanding of what changes in certain traditional measures of CVD (such as blood pressure or lipid profile) mean for patients on hemodialysis. This review summarizes the current evidence base for the effects of exercise on traditional and nontraditional cardiovascular risk factors and the effects of exercise interventions on cardiovascular structure and function, including a review of study limitations and future research priorities.
Exercise training reduces coronary risk and effectively rehabilitates hemodialysis patients

Abstract:This study examines the effects of 12 months of endurance exercise training (cycling, walking and jogging) on lipid profiles, glucose metabolism, blood pressure, anemia and psychological function in 14 hemodialysis patients. Maximal aerobic capacity (VO2max) increased 18% in the exercisers (p less than 0.01), but did not change in 11 controls. This was associated with a reduction in depression, a decrease in dosages of antihypertensive medications, a significant increase in hematocrit and hemoglobin levels (red cell mass rose, plasma volume did not change), a decrease in plasma triglyceride by 23% (p less than 0.05) and an increase in high-density lipoprotein cholesterol (HDL-C) levels by 21% (p less than 0.01) (both HDL-C and triglyceride levels worsened in the sedentary controls), and an 18% increase in glucose disappearance rates (p less than 0.05) in spite of a 52% decrease in fasting insulin levels (p less than 0.01), suggesting that insulin sensitivity improved. These results demonstrate that some of the complications present in hemodialysis patients may be caused by their sedentary life-style, rather than endstage renal disease itself. This suggests that rehabilitation through exercise is possible for these patients. By reducing coronary risk factors in hemodialysis patients, exercise training may also decrease their heightened morbidity and mortality from atherosclerotic complications. These possibilities need to be examined in a longitudinal study.
Effect of intradialytic exercise on geriatric issues in older patients undergoing hemodialysis: a single-center non-randomized controlled study

Abstract:Purpose: This study investigated the effect of 1 year of intradialytic exercise on older hemodialysis patients with geriatric issues. Methods: Forty-six patients aged ≥ 70 years were non-randomly assigned to two groups (exercise group: 27, control group: 19). Intradialytic exercise consisted of 30 min of aerobic exercise using a cycle ergometer, and resistance training comprising four exercises using an elastic tube three times per week for 1 year. Handgrip strength, leg extremity muscle strength, 10-m walk speed, short physical performance battery, serum albumin, Geriatric Nutritional Risk Index (GNRI), geriatric depression scale, frailty, and mobility were each assessed before and after the intervention. Results: The control group exhibited a significant reduction in handgrip strength, 10-m walking speed, serum albumin, and GNRI after intervention compared to baseline (p < 0.05). Conversely, no significant reductions were observed in the exercise group. The ΔGNRI (effect size, 0.69; 95% confidence interval [CI] - 5.21, - 0.1; p < 0.05) and Δserum albumin (effect size, 0.72; 95% CI - 0.31, - 0.02; p < 0.05) before and after the intervention declined significantly less in the exercise group than in the control group. Other between-group values were not significantly different. The number of frail patients and patients requiring walking assistance exhibited no significant intra-group or between-group differences before and after the intervention. Conclusion: Intradialytic exercise prevented the worsening of nutritional status and physical function in the exercise group compared to the control group. Exercise therapy during dialysis is an important aspect of patient care that helps prevent functional decline in older patients.

Abstract:Patients with chronic kidney disease (CKD) are at increased risk of cardiovascular disease. This increased risk cannot be fully explained by traditional risk factors such as hypertension. Endothelial dysfunction and arterial stiffness have been suggested as factors which explain some of the increased risk, and are independently associated with important cardiovascular outcomes in CKD patients. Studies in other disease populations have shown positive effects of exercise on vascular dysfunction. The aim of this review was to determine whether exercise training interventions improve measures of vascular function and morphology in patients across the spectrum of CKD and which exercise training interventions are most efficacious. A systematic search of Medline, Embase and the Cochrane Central Register identified 25 randomized, controlled trials. Only randomized control trials using an exercise intervention with non-exercising control group, and at least one measure of vascular function or morphology were included. Participants were non-dialysis CKD or transplant patients, or those requiring dialysis therapy. A systematic review was conducted according to PRISMA guidelines. A meta-analysis was completed for pulse wave velocity, augmentation index and measures of endothelial-dependent vasodilation. Data from 25 studies with 872 participants showed that exercise training reduced pulse wave velocity and augmentation index, but had no effect on vasodilation. Subgroup analyses suggested that exercise interventions of at least moderate intensity were more likely to be effective. Limitations included the absence observational studies, or other interventions aimed at increasing habitual physical activity. Further studies are warranted to investigate which are the most effective exercise interventions.

Aerobic Exercise Training and Nontraditional Cardiovascular Risk Factors in Hemodialysis Patients: Results from a Prospective Randomized Trial

Abstract:Introduction: Chronic kidney disease (CKD) patients have a high incidence of cardiovascular diseases (CVD) which increases their morbidity and mortality. A sedentary lifestyle in CKD is directly linked to the onset of CVD. Physical activity can bring beneficial effects to CKD patients. Aims: The aim of this study was assess the impact of aerobic training on nontraditional cardiovascular risk factors in CKD patients on hemodialysis. Materials and methods: This is a prospective, controlled, and randomized clinical trial with analysis of intention to treat. Thirty patients underwent an exercise treadmill test, an arterial stiffness evaluation, echocardiography and analysis of endothelial reactivity, and carotid ultrasound and laboratorial tests, including analysis of serum aldosterone. The intervention group (IG) (n =15) underwent aerobic exercise during hemodialysis 3 times a week for 4 months. The control group (CG) (n =15) had no intervention. All of the patients were reassessed after 4 months. Results: In the IG, there was a statistically significant improvement in flow-mediated vasodilation (FMV; p = 0.002) and a reduction in left ventricular hypertrophy (p = 0.006) and serum aldosterone (p = 0.016). There was an increase in C-reactive protein in the CG (p = 0.002). Conclusion: This aerobic training protocol was able to improve endothelial function with enhanced FMV and reduce left ventricular hypertrophy and serum aldosterone, which could have a positive impact on the reduction of nontraditional cardiovascular risk factors in CKD patients on hemodialysis.

Abstract:Abstract : It is unclear why some individuals develop PTSD while others do not. Pre-existing biological differences among individuals may predispose some and not others to develop PTSD. We are seeking to identify such biological differences using an existing rat model for PTSD where exposure to a predator stress scent (PSS) results in extreme behavioral responses for 25% of animals (EBR) thought to be analogous to PTSD, while 25% of the animals show minimal behavioral responses (MBR). Our objectives for Experiment 1 were to identify genes showing altered mRNA expression in blood and brain areas implicated in PTSD that relate to individual differences in fear reactivity. Our goal in Experiment 2 was to examine the prophylactic effects of exogenous glucocorticoids on the behavioral and gene expression effects of stress exposure. We have completed the behavioral work for Exp.1 and Exp.2 and successfully produced rats with the behavioral phenotypes that were required for our gene expression aims. Gene expression profiling of Exp.1 has been completed on all brain samples and on a portion of the blood. Among others, differential expression of genes (e.g. FKBP5) regulated by the glucocorticoid-receptor (GR) underlies the behavioral differences. The small overlap in gene expression between different tissue and gender demonstrated the existence of distinct tissue specific susceptibility pathways in males and females. Gene expression profiling of the samples of Exp.2 is ongoing.

Abstract:Objective: To explore the effects of resistance exercise on Intradialytic Hypotension episodes, nursing intervention times and pre-dialysis blood pressure. Methods: Totally 77 patients with frequent Intradialytic Hypotension from 2 hemodialysis centers were recruited and divided randomly into the intervention group (n=39) and the control group (n=38). The patients in the intervention group were guided to do resistance exercise 3 times per week for 12 weeks besides the routine care, while the patients in the control group received the routine care. The data were collected from August 2015 to March 2016. We collected the data of the two groups' Intradialytic Hypotension episodes, nursing intervention times and pre-dialysis blood pressure before and during intervention. Results: The results showed that the Intradialytic Hypotension episodes and nursing intervention times in the intervention group were significantly less than those in the control group (P<0.05). After 12-week resistance exercise, patients' diastolic pressure decreased and their systolic pressure were stable before hemodialysis . Conclusion: Resistance exercise is effective for the hemodialysis patients with frequent Intradialytic Hypotension to reduce Intradialytic Hypotension episodes, reduce the related nursing intervention times, and to improve blood pressure.
The Impact of Exercising During Haemodialysis on Blood Pressure, Markers of Cardiac Injury and Systemic Inflammation - Preliminary Results of a Pilot Study

Abstract:BACKGROUND/AIMS: Patients requiring haemodialysis have cardiovascular and immune dysfunction. Little is known about the acute effects of exercise during haemodialysis. Exercise has numerous health benefits but in other populations has a profound impact upon blood pressure, inflammation and immune function; therefore having the potential to exacerbate cardiovascular and immune dysfunction in this vulnerable population.METHODS: Fifteen patients took part in a randomised-crossover study investigating the effect of a 30-min bout of exercise during haemodialysis compared to resting haemodialysis. We assessed blood pressure, plasma markers of cardiac injury and systemic inflammation and neutrophil degranulation.RESULTS: Exercise increased blood pressure immediately post-exercise; however, 1 hour after exercise blood pressure was lower than resting levels (106±22 vs. 117±25 mm Hg). No differences in h-FABP, cTnI, myoglobin or CKMB were observed between trial arms. Exercise did not alter circulating concentrations of IL-6, TNF-α or IL-1ra nor clearly suppress neutrophil function.CONCLUSIONS: This study demonstrates fluctuations in blood pressure during haemodialysis in response to exercise. However, since the fall in blood pressure occurred without evidence of cardiac injury, we regard it as a normal response to exercise superimposed onto the haemodynamic response to haemodialysis. Importantly, exercise did not exacerbate systemic inflammation or immune dysfunction; intradialytic exercise was well tolerated.

Abstract:Abstract Dialysis patients show a high rate of reduced functional capacity, morbidity and mortality. Cardiovascular disorders, muscle atrophy and malnutrition play an essential role among the aetiological factors. Sedentary lifestyle characterizes them and contributes to the aggravation of the disorders. On the contrary, exercise training is an important preventive and therapeutic tool both for cardiovascular problems and for the appearance of muscle atrophy in dialysis patients. Regular exercise causes both central (cardiac) and peripheral (muscular) adaptations, improving functional capacity. In particular, circulatory system clinical trials in haemodialysis (HD) patients documented that exercise has favourable effects on heart function, promotes balance on the cardiac autonomic nervous system and contributes to the management of arterial hypertension. In the muscular system, it prevents muscle atrophy or contributes significantly to its treatment. The main preventive mechanisms of the beneficial effect of exercise on the muscles constitute the inhibition of the apoptotic processes and protein degradation. Exercise training in HD patients leads to an increase of muscle fibers, mitochondria and capillaries, and the combination of regular exercise and dietary strategies is even more effective in preventing or treating muscle atrophy. Finally, an improvement in functional capacity and quality of life was found also in peritoneal dialysis patients following exercise training.

Abstract:With the increasing prevalence of end-stage kidney disease, the number of patients requiring hemodialysis (HD) continues to rise. While life-sustaining, HD is often associated with adverse effects such as muscle loss, physical deconditioning, fatigue, and compromised health-related quality of life (HRQoL). Recent research suggests that intradialytic exercise (IDE) and home-based exercise (HBE) may mitigate these adverse effects and improve patient outcomes. However, the existing literature mainly focuses on the outcomes of both exercises, whereas the comparison of types is often omitted. Hence, this review consolidates findings from studies investigating the effectiveness, implementation, safety, feasibility, and adherence of different types of IDE and HBE in HD patients. Overall, the current literature bolsters the significance of IDE and HBE for improving health in HD patients. IDE and HBE enhance physical function, cardiopulmonary capacity, HRQoL, and cognitive well-being. Some research proposed an indirect link between IDE and survival rates. Despite these benefits, challenges remain in implementing these exercise modalities, including patient adherence and the feasibility of routine exercise during HD sessions. Integrating these exercises into routine care allows healthcare providers to enhance outcomes for HD patients. Further research is suggested to optimize exercise protocols and explore long-term effects and cost-effectiveness.
