Microbiological Profile of Culture-Positive Fungal Keratitis

Abstract:Purpose: To examine the microbiological profile of cases of culture-positive fungal keratitis presenting to a tertiary eye care center in eastern India. Methods: Microbiology records of all culture-positive microbial keratitis patients presenting to L V Prasad Eye Institute, Bhubaneswar, between January 2020 and December 2021, were retrospectively reviewed. Collected data included smear results of culture-positive fungal or mixed infections, the species isolated, and the time taken for organisms to grow in each media. Results: Fungal keratitis formed 36% of all culture-positive microbial keratitis, whereas mixed infections (fungi and other organisms) formed 8.5%. The most common fungal species isolated was Fusarium spp. (25.8%). The most common bacteria involved in mixed infection with fungi was Staphylococcus spp. (54.8%). The positivity of potassium hydroxide+calcofluor white stain in detecting fungal filaments was 89.0% and that of Gram stain was 76.1%. Culture-positive cases of fungal keratitis showed most frequent growth on potato-dextrose agar (77.6%). A similar pattern was observed in culture-positive mixed infections (Sabouraud dextrose agar [SDA]: 84%). Most frequent growth of bacteria in mixed infections was seen in thioglycolate broth (54.7%). The shortest time to achieve significant fungal growth was observed in blood agar (BA) and chocolate agar (CA) (2.2/2.3 days, and 1.8/2 days for fungal keratitis and mixed infections, respectively). Filamentous hyaline fungi took the shortest time to achieve significant growth (2.8 days), whereas yeast forms took the longest (5 days). Conclusion: This study highlights the importance of combined use of both solid and liquid culture media, especially potato dextrose agar (PDA)/SDA and CA, to arrive at a definitive diagnosis of fungal keratitis and possible bacterial co-infection, which forms a significant proportion of cases with fungal keratitis. In resource-poor laboratories, two culture media, either SDA or PDA, along with BA, may be plated to detect mixed infections. Examination of stained smears of corneal samples provides an inexpensive method of rapid diagnosis of fungal keratitis when culture media is not available.

Abstract:Introduction: Microbial keratitis poses a significant threat to vision and is a common ocular infection. Its causative agents encompass a wide spectrum, including bacteria, fungi, viruses, and parasites. The microbiological profile of microbial keratitis is influenced by factors such as patient demographics, geographical location, climate, and occupational hazards and evolves over time. Methodology: Corneal scrapings were collected from 75 patients with a provisional diagnosis of microbial keratitis. The samples were processed in the microbiology laboratory, and the bacterial and fungal growth isolated in the study were identified according to standard procedures. Results: Among the 75 patients, 48 (64%) were male and 27 (36%) were female. Corneal ulceration was found in individuals of all age groups, with the highest prevalence of 77.33% (58/75) observed in the 21-60 age range. Farmers exhibited a higher susceptibility, constituting 66.67% (50/75) of the cases. The study noted a higher occurrence of keratitis from November to February, accounting for 69.33% (52/75). Microbial etiology was identified in 25.33% (19/75) of scrapings, with fungi accounting for 68.42% (13/19) and bacteria for 31.57% (6/19). The prevalent fungal species included Fusarium (7/13, 53.84%), Aspergillus (3/13, 23.07%), Colletotrichum (2/13, 15.38%), and Curvularia (1/13, 7.69%). Bacterial isolates featured Streptococcus pneumoniae (5/6, 83.33%) and Klebsiella pneumoniae (1/6, 16.66%). Conclusions: This study underscores the importance of regularly updating local microbial profiles and understanding antimicrobial resistance patterns. Such updates are critical for informed decision-making in selecting optimal topical treatments for microbial keratitis.
Clinical analysis of hypopyon culture in fungal keratitis

Abstract:Objective: To investigate the correlation among culture results of the hypopyon and the infected fungal strains, pathological examination, diseased course and progonosis in fungal keratitis. Methods: 33 cases of severe fungal keratitis which underwent therapeutic penetrating keratoplasty (PKP) were enrolled in this study. The diseased corneal button were bisected, one half of the corneal button and the hypopyon was taken for culture. Anothor half of the corneal button was taken for pathological study. Results: All 33 diseased corneal buttons had positive culture for fungus. 15/33 hypopyon were positive in which 6 eyes had corneal perforation. The fungal spectum is: Fusarium 12 eyes; Absidia coerulea 2 eyes; Aspergillus 1 eyes. Pathological examination: fungal hypha penetrated the desement membrane in 10 corneal buttons, 8 of these cases had a positive culture for fungus. Conclusion: Most of the hypopyon in fungal keratitis without perforation are aseptic. If cornea perforate the hypopyon is positive in fungal culture. PKP can save the eyes with infected hypopyon.
Demographic and microbiological profile of corneal ulcer patients presenting at a tertiary healthcare center of Eastern India during the COVID era: A hospital-based cross-sectional study

Abstract:Purpose: To determine the demographic and microbiological profile involved in the causation of corneal ulcers in Eastern India during the COVID era. Method: Patients presenting with corneal ulcers fulfilling the inclusion and exclusion criteria were taken as the case. The study duration was from January 2021 to December 2021. Socio-demographic details and information about risk factors were noted. A detailed corneal examination followed by corneal scraping was performed for microbiological evaluation. Results: In 1 year, 99 infective corneal ulcer patients were evaluated. Farmers (24.2%) were found to be maximally affected by corneal ulcers. The peak in cases was recorded from October to December (38.4%). Ocular trauma was the commonest risk factor (42.4%). The majority (80.8%) of patients were already on some topical antimicrobials. 22.2% of samples showed fungal filaments on KOH mount; 54.5% of these cases turned out to be culture-positive. 17.56% of KOH-negative samples turned out to be culture-positive (fungal). Overall, the culture positivity rate was 28.28% out of which fungal isolates were 89.28% and bacterial isolates were 10.72%. Fusarium species were identified as the most common organism contributing 42.85%, followed by Aspergillus fumigatus (14.28%). 10.72% of cases were culture positive for Pseudomonas aeuroginosa. Conclusion: Trauma with the organic matter was the predominant cause of fungal keratitis. In this study, fungal keratitis was found to be more common. Fusarium was the most common isolate.

Abstract:Objectives: To evaluate and compare the risk factors, presenting features, and outcomes of patients with culture-positive and culture-negative microbial keratitis (MK) who presented to a tertiary referral center.Materials and Methods: We conducted a retrospective review of the medical records of 314 patients who were diagnosed with MK in our clinic between 2012 and 2019.Results: Among 314 patients, 142 had positive cultures (45.2%). The mean ages of the culture-positive and -negative patients at the time of diagnosis were 51.39±21.31 (range, 14-90) years and 56.68±21.34 (7-94) years, respectively (p=0.028). The mean best corrected visual acuity (BCVA) of the culture-positive and -negative patients were1.74±1.25 (0-3.1) LogMAR and 1.91±1.23 (0-3.1) LogMAR prior to treatment and increased to 1.21±1.30 (0-3.1) LogMAR and 1.27± 1.29 (0-3.1) LogMAR at last visit, respectively. There was no statistically significant difference between culture-positive and -negative patients' BCVA levels at presentation or last visit. Ninety-two patients (64.7%) were infected with bacteria and 50 patients (35.2%) with fungi. The most common pathogen was Pseudomonas aeruginosa (18.3%), followed by Streptococcus pneumoniae (11.2%) and Fusarium spp. (11.2%). Keratitis foci were either centrally or paracentrally located in 105 eyes (73.9%) of culture-positive patients and 149 eyes (86.6%) of culture-negative patients. Multiple foci were present mostly in culture-positive patients (p=0.001). There was no significant difference between the culture-positive and -negative groups in terms of hypopyon presence (p=0.364). The proportion of contact lens (CL) wearers was 33% (n=47) among culturepositive MK patients and 13.3% (n=23) among culture-negative MK patients, respectively (p<0.001). Culture positivity was found to be significantly higher in keratitis associated with CL use (p=0.0001).Conclusion: Microbiological analysis and culture evaluation are important steps in order to manage proper treatment in microbial keratitis. Prognosis mostly depends on the infectivity of the microbiological agent.

Abstract:Purpose: To evaluate the microbiological profile of corneal ulcers presenting at a tertiary care eye hospital in eastern Uttar Pradesh (UP), India. Methods: This retrospective, observational study included patients with corneal ulcers who underwent corneal scraping and microbiological examination of the sample from May 2014 to October 2023. The demographic details, predisposing factors, and clinical examination data of the patients were noted. Microbiology report of staining and culture (blood agar and Sabaraud's dextrose agar) of corneal scraping sample was analyzed in percentage. Reports of antimicrobial susceptibility testing were also noted and analyzed. Results: A total of 695 corneal scraping samples were examined during the study period. The mean age of the patients was 45.95 years. Among the patients, 412 (59.28%) were males and 283 (40.72%) were females. Among the patients, 402 (57.84%) belonged to an agricultural background. Trauma was the most common predisposing factor associated with 350 (50.36%) cases. The corneal scraping samples were stain-positive in 455 (65.47%) cases, of which, 130 (28.57%) were gram-positive and 325 (71.43%) were positive on potassium hydroxide (KOH) mount. Culture was positive in 306 (44.03%) cases, of which, bacterial isolates were found in 78 (25.49%), and fungal isolates were found in 228 (74.51%). Fusarium was the most common fungal isolate in 72 (31.58%) cases followed by Aspergillus in 60 (26.32%) cases. Among the bacterial isolates, Staphylococcus aureus was the most common in 20 (25.64%) cases followed by Pseudomonas and Streptococcus. The antimicrobial susceptibility testing showed that 47 (60.26%) of the bacterial isolates were sensitive to fluoroquinolones while the rest 31 (39.74%) were resistant. All the Staphylococcus aureus strains (including four cases of methicillin-resistant Staphylococcus aureus) were susceptible to vancomycin and linezolid, while 7 (35%) were resistant to moxifloxacin. None of the Pseudomonas strains were multidrug resistant. Among the fungal isolates, 220 (96.49%) were susceptible to voriconazole and 189 (82.89%) were sensitive to amphotericin B. Conclusion: Fungal corneal ulcers are more common compared to bacterial ulcers in eastern India, particularly eastern UP and Bihar. This article highlights the importance of microbial testing and provides insight into the prevalent organisms and their antimicrobial susceptibility pattern in this geographic location, the knowledge of which will help clinicians in the appropriate management of these cases.
Microbiological culture versus 16S/18S rRNA gene PCR-sanger sequencing for infectious keratitis: a three-arm, diagnostic cross-sectional study

Abstract:Background: To compare the diagnostic performance of microbiological culture and 16S/18S rRNA gene polymerase chain reaction (PCR)-Sanger sequencing for infectious keratitis (IK) and to analyse the effect of clinical disease severity on test performance and inter-test concordance. Methods: This was a three-arm, diagnostic cross-sectional study. We included all eligible patients who presented with presumed bacterial/fungal keratitis to the Queen's Medical Centre, Nottingham, UK, between June 2021 and September 2022. All patients underwent simultaneous culture (either direct or indirect culture, or both) and 16S (pan-bacterial)/18S (pan-fungal) ribosomal RNA (rRNA) PCR-Sanger sequencing. The bacterial/fungal genus and species identified on culture were confirmed using matrix-assisted laser desorption/ionization-time-of-flight mass spectrometry. Relevant clinical data were also collected to analyze for any potential clinico-microbiological correlation. Main outcome measures included the diagnostic yield, test accuracy (including sensitivity and specificity), and inter-test agreement [including percent agreement and Cohen's kappa (k)]. Results: A total of 81 patients (86 episodes of IK) were included in this study. All organisms identified were of bacterial origin. Diagnostic yields were similar among direct culture (52.3%), indirect culture (50.8%), and PCR (43.1%; p = 0.13). The addition of PCR enabled a positive diagnostic yield in 3 (9.7%) direct culture-negative cases. Based on composite reference standard, direct culture had the highest sensitivity (87.5%; 95% CI, 72.4-95.3%), followed by indirect culture (85.4%; 95% CI, 71.6-93.5%) and PCR (73.5%; 95% CI, 59.0-84.6%), with 100% specificity noted in all tests. Pairwise comparisons showed substantial agreement among the three tests (percent agreement = 81.8-86.2%, Cohen's k = 0.67-0.72). Clinico-microbiological correlation demonstrated higher culture-PCR concordance in cases with greater infection severity. Conclusions: This study highlights a similar diagnostic performance of direct culture, indirect culture and 16S rRNA PCR for bacterial keratitis, with substantial inter-test concordance. PCR serves as a useful diagnostic adjuvant to culture, particularly in culture-negative cases or those with lesser disease severity (where culture-PCR concordance is lower).

Abstract:S ince 1996, emergency clinician researchers have conducted a relatively small number of clinical trials using the exception from informed consent (EFIC) regulations for emergency research (21 CFR 50.24). 1–3 Because of the sensitive nature of research that involves patients rendered vulnerable by their critical emergent conditions, the EFIC regulations include safeguards that were never before mandated for clinical trials, including the need for community consultation (CC). An exception was CC required for a trial of hypo-thermia for head injury conducted in the early 1990s, before the EFIC regulations. This was not FDA regulated ; it was conducted with a secretarial override that allowed a waiver of, and not an exception from, informed consent. 4 Community consultation is not community consent or veto. It is a two-way communication, in which the concerns of the local community regarding a proposed EFIC research study are heard and considered before the trial can begin locally. The effectiveness, community penetration, and additional safety provided by CC is under constant (and skeptical) debate by EFIC investigators. 5 Given that CC adds cost and time to the pre-study phase of a clinical trial, a measure of its value and effectiveness is important so that it is not just considered a necessary exercise or burden. If CC is to be meaningful, its purpose and its effects must be clear and convincing to the communities and investigators alike. When the EFIC regulations were developed, the intent of the added safeguard of CC was to prevent unanticipated harm to patient-subjects by soliciting from the community concerns that may not have occurred to the investigators. Subsequent questions by investigators and guidance by the FDA 2,6 seemed to concentrate more on the logistics of CC activities rather than their purpose. The definition of community and who could represent the community has been investigated. 7 The pros and cons of various methods of CC are under continuous discussion. 2,8 The added cost of (and time for) prestudy CC has been described. 9 However, there have been no formal reports describing the effects of CC on institutional review board (IRB) decision-making or how CC concerns are dealt with by investigators and IRBs. No metrics have been established to measure the effect of CC, and in fact, since the purpose is vague, metrics of success cannot really be defined. Abstract:Cav2.1 mediates voltage-gated Ca2+ entry into neurons and the release of neurotransmitters at synapses of the central nervous system. An inactivation process that is modulated by the auxiliary β-subunits regulates Ca2+ entry through Cav2.1. However, the molecular mechanism of this α1-β-subunit interaction remains unknown. Herein we report the identification of new determinants within segment IVS6 of the α12.1-subunit that markedly influence channel inactivation. Systematic substitution of residues within IVS6 with amino acids of different size, charge, and polarity resulted in mutant channels with rates of fast inactivation (k inact) ranging from a 1.5-fold slowing in V1818I (k inact = 0.98 ± 0.09 s− 1 compared with wild type α12.1/α2-δ/β1a k inact = 1.35 ± 0.25 s− 1) to a 75-fold acceleration in mutant M1811Q (k inact = 102 ± 3 s− 1). Coexpression of mutant α12.1-subunits with β2a resulted in two different phenotypes of current inactivation: 1) a pronounced reduction in the rate of channel inactivation or 2) an attenuation of a slow component in I Ba inactivation. Simulations revealed that these two distinct inactivation phenotypes arise from a β2a-subunit-induced destabilization of the fast-inactivated state. The IVS6- and β2a-subunit-mediated effects on Cav2.1 inactivation are likely to occur via independent mechanisms.

Abstract:Objectives: To determine the utility and use of corneal cultures in bacterial keratitis. Methods: Retrospective single-center chart review of patients with bacterial keratitis who showed a positive corneal culture result from January 2017 to December 2021. Results: Seventy-four cases of bacterial keratitis in 73 patients with positive cultures were identified. The most common organisms were Pseudomonas, Staphylococcus, and Streptococcus species. In total, 87.8% of cases were started on fortified antibiotics initially. All organisms isolated were susceptible to empiric antibiotics used (n=54). In 47.3% of cases, an antibiotic was discontinued once culture results became available, most often fortified vancomycin. Within the 27 Pseudomonas cases, consisting of predominantly contact lens wearers, at least one fortified antibiotic was discontinued in 66.7% of cases after a positive culture result. Conclusions: Multidrug resistance was infrequent in the study population. Empiric antibiotic therapy is still effective in bacterial corneal ulcers. Corneal cultures may have utility in antibiotic de-escalation.

Abstract:Purpose: To study the type of bacteria and their antibiotic susceptibility pattern from corneal scrapings of patients with infectious keratitis presenting with concurrent nasolacrimal duct obstruction. Methods: A retrospective analysis was carried out of patients with microbial keratitis and co-existing nasolacrimal duct obstruction during a 5-year period (January 2012 to December 2016). Antibiotic susceptibility profile data was collected from the microbiology records. Results: The study included 56 (M:F = 33:23; OD:OS = 34:22) eyes. The mean age of patients was 60.2 ± 12.3 years. Microscopic examination of corneal scrapings showed 52% of gram-positive (n = 29/55) and 2% of multiple (>1; n = 1/55) bacteria. Streptococcus pneumoniae (32%, n = 18/56), coagulase-negative Staphylococcus (25%, n = 14/56), and Staphylococcus aureus (11%, n = 6/56) were the most common bacteria. Streptococcus pneumoniae was found to be susceptible to ciprofloxacin (72.2%), ofloxacin (94.4%), gatifloxacin (61.1%), and moxifloxacin (83.3%). Conclusion: Corneal infection associated with nasolacrimal duct obstruction is commonly caused by gram-positive-bacteria. Irrigation of nasolacrimal duct should be mandatory test in microbial keratitis. The gram-positive bacteria isolated showed maximum susceptibility to vancomycin and cefazolin. Early treatment by surgery and appropriate medical management is recommended for its control.

Abstract:Purpose: To study the risk factors, clinical features, and treatment outcomes of patients with culture-negative keratitis (CNK). Methods: A retrospective data review of 933 patients with CNK was performed from January 2018 to December 2020. The variables such as the history of injury, visual acuity, slit-lamp findings with measurements of size and depth of ulcer, microbiological evaluation, duct patency, blood glucose levels, and treatment were considered, and clinical outcome was analyzed. Results: Of the 933 patients with CNK, 763 (81.8%) were medically managed, with a mean treatment duration of 2.08 ± 1.7 weeks. Among them, 622 (66.7%) were both smear and culture-negative, and 311 (33.3%) showed only smear positivity. Smear-positive patients showed a positive correlation with the history of injury. A higher incidence of fungal growth on repeat culture was observed. Surgical interventions were done only in 18.2% of the patients; the rest were treated with topical medications alone. Conclusion: High clinical suspicion, differentiation of causative organisms based on clinical findings, and initiating empirical therapy with broad-spectrum antibiotics and antifungals improve the ultimate prognosis in patients with CNK, even though a standard protocol for empirical medical treatment may differ among institutions and surgeons based on their clinical experience and geographical variations.
Microbial Keratitis Due to Aeromonas Species at a Tertiary Eye Care Center in Southern India

Abstract:Purpose: To review the incidence, microbiological profile, antibiogram, clinical course, treatment, and outcome in patients with microbial keratitis due to Aeromonas species at a tertiary eye care center in Southern India. Design: A retrospective, noncomparative study. Methods: This study included all patients of corneal ulcer with a significant growth of Aeromonas species on culture of corneal scrapings managed at an eye institute in India between January 2005 and October 2011. The patients underwent standard diagnostic microbiological evaluation, and their treatment was guided by microbiology results and clinical response to therapy. Data were analyzed for the incidence, predisposing factors, clinical picture, microbiology, and the treatment outcome. Results: Fifteen patients of microbial keratitis showed Aeromonas species to be the causative agent, constituting 0.5% of bacterial keratitis diagnosed during this period. Laboratory results showed that Aeromonas hydrophila was the predominant species isolated. Most of the isolates were susceptible to commonly used ocular antibiotics. Medical therapy resulted in resolution of infection in 9 (60%) of 15 cases. Five patients required application of cyanoacrylate tissue adhesive with bandage contact lens for corneal perforation. Two of the 5 patients needed penetrating keratoplasty for uncontrolled infection. Twelve of 15 patients had some local predisposing host factors for the infection. Conclusions: Aeromonas species are uncommon but important causal agents of microbial keratitis. The organisms most often cause infection in individuals with local predisposing factors. Aeromonas hydrophila is the predominant species associated with keratitis. Although the organisms are susceptible to most antibiotics, a third of the patients required additional surgical intervention.
Corneal biopsy for diagnosis of recalcitrant microbial keratitis

Abstract:PurposeTo document the findings of corneal biopsies for progressive microbial keratitis in a large tertiary referral institution.MethodsA retrospective medical records review of all patients who underwent at least one corneal biopsy for the diagnosis of microbial keratitis at Sydney Eye Hospital, Australia between January 1, 2010 and December 31, 2016 was performed.ResultsThirty-eight patients (18 men and 20 women) underwent a corneal biopsy for progressive microbial keratitis unresponsive to broad-spectrum topical antimicrobials. Risk factors for microbial keratitis included contact lens wear in 8 (21%), recent intraocular surgery in 5 cases (13%), recent agricultural trauma in 3 cases (8%), exposure keratopathy due to Graves’ orbitopathy in 1 case (3%), and profound systemic immunosuppression due to chemotherapy for leukaemia in 1 case (3%). The remaining 20 patients had no identifiable risk factors. Fifteen patients (39%) had a positive biopsy result, which identified bacteria in 6 cases and Mycobacteria in 1 case, both by culture of the biopsy specimen. Three cases of fungus were identified on culture of biopsy specimen, two of which were also confirmed on histopathology and an additional case was identified from histopathology alone. A single case of Acanthamoeba was diagnosed by culture and histopathology, and an additional 3 cases were diagnosed on histopathology alone. A corneal biopsy yielded new organisms in 73% (11/15) cases where the culture results of biopsy specimens were positive.ConclusionCorneal biopsy is an important tool in the diagnosis of progressive keratitis, often identifying causal organisms not found on corneal scraping alone.

Microbial keratitis in the subtropical region of Korea: a comprehensive 12-year retrospective review at a single referral center

Abstract:Objectives: To investigate the epidemiological characteristics and clinical outcomes of culture-proven bacterial and fungal keratitis at a single tertiary referral centre on Jeju Island, South Korea. Design: A retrospective study design. Setting: Data from a solitary referral centre on Jeju Island spanning January 2011 to December 2022. Participants: Among the 245 patients clinically diagnosed with infectious microbial keratitis, 110 individuals had culture-positive results. Primary and secondary outcome measures: The primary outcome was the identification of causative microbial profiles and epidemiological characteristics, while the secondary outcome was the correlation of these factors with treatment outcomes. Results: Of 245 patients, 110 (44.9%) had culture-positive infectious keratitis, showing 69 bacterial, 32 fungal, 4 superimposed bacterial and 5 cases with coinfection by bacteria and fungus. The most common pathogen was Pseudomonas species in 14.4% of the bacterial keratitis cases, followed by Staphylococcus epidermidis (9%), Staphylococcus aureus (8%) and Moraxella species (7%). The total treatment success rate for bacterial keratitis was 67.5%. The frequency of methicillin-resistant Staphylococcus on Jeju Island did increase during the study period. Fusarium species had the highest incidence at 22.2%, followed by Candida (16.7%) and Colletotrichum species (11.1%). 56.7% of fungal keratitis patients were successfully treated. An initial large corneal lesion (>3 mm) showed a statistically significant association with treatment failure. Conclusion: The incidence of Moraxella and Colletotrichum species in our study was higher than that reported in other districts with different climates and environments. The results reported here reflect the unique environmental features of Jeju Island, characterised by high humidity and temperatures.

Abstract:Fungal aetiology of keratitis/corneal ulcer is considered to be one of the leading causes of ocular morbidity, particularly in developing countries including India. More importantly, Fusarium and Aspergillus are reported commonly implicating corneal ulcer and against this background the present work was undertaken so as to understand the current epidemiological trend of the two fungal keratitis. During the project period, a total of 500 corneal scrapings were collected from suspected mycotic keratitis patients, of which 411 (82.2%) were culture positive for bacteria, fungi, and parasites. Among fungal aetiologies, Fusarium (216, 52.5% of 411) and Aspergillus (68, 16.5% of 411) were predominantly determined. While the study revealed a male preponderance with both the fungal keratitis , it further brought out that polyene compounds (natamycin and amphotericin B) and azoles were active, respectively, against Fusarium spp. and Aspergillus spp. Additionally, 94.1% of culture proven Fusarium keratitis and, respectively, 100% and 63.6% of A. flavus and A. fumigatus were confirmed by multiplex PCR. The sensitivity of the PCR employed in the present study was noted to be 10 fg/ μ l, 1 pg/ μ l, and 300 pg/ μ l of DNA, respectively, for Fusarium , A. flavus, and A. fumigatus. Alarming fact was that Fusarium and Aspergillus regionally remained to be the common cause of mycotic keratitis and the Fusarium isolates had a higher antifungal resistance than Aspergillus strains against most of the test drugs.

Abstract:Aims To report an epidemiological update of bacterial keratitis (BK) in a tertiary ophthalmology centre over 20 months compared with a previous study on the same timeframe from 1998 to 1999. Methods 354 patients with BK documented by microbiological corneal scraping or resolutive under antibiotics treatment from January 2020 to September 2021 were analysed retrospectively. Results One or several risk factors were found in 95.2% of patients: contact lens wear (45.2%), ocular surface disease (25.0%), systemic disease (21.8%), ocular trauma (11.9%) and ocular surgery (8.8%). The positivity rate of corneal scrapings was 82.5%, with 18.2% polybacterial. One hundred seventy-five (59.9%) bacteria were Gram-negative, and 117 (40.1%) were Gram-positive. The most common bacteria were Pseudomonas aeruginosa (32.5%), Moraxella spp (18.1%) and Staphylococcus aureus (8.2%). Final visual acuity (logarithm of the minimum angle of resolution) was associated with age (r=+0.48; p=0.0001), infiltrate size (r=+0.32; p<0.0001), ocular surface disease (r=+0.13; p=0.03), ocular trauma (r=-0.14; p=0.02) and contact lens wear (r=−0.26; p<0.0001). Gram-negative bacteria were responsible for deeper (r=+0.18; p=0.004) and more extensive infiltrates (r=+0.18; p=0.004) in younger patients (r=−0.19; p=0.003). Compared with the previous period, the positivity rate of corneal scrapings and the proportion of Gram-negative bacteria, especially Moraxella spp, increased. All P. aeruginosa and Moraxella spp were sensitive to quinolones, and all S. aureus were sensitive to both quinolones and methicillin. Conclusion Contact lens wear remained the leading risk factor. The bacteria distribution was reversed, with a predominance of Gram-negative bacteria and increased Moraxella spp.

[Evaluation of primary care from the perspective of users: adaptation of the EUROPEP instrument for major Brazilian urban centers].

Abstract:Satisfaction with health care is a multidimensional concept that considers aspects such as access, organization and professional-user interaction. The aim of this study was to adapt and apply an instrument in the Family Health Strategy (FHS) to assess user satisfaction with Primary Health Care (PHC) based on the European Task Force on Patient Evaluation of General Practice Care (EUROPEP), which refers to user satisfaction wth general and family medicine services. The instrument consists of five dimensions of satisfaction: relationship and communication, medical care, information and support, continuity and cooperation, and organization of services. The study was divided into phases: review and adjustment of the instrument and application to a representative sample of users of the FHS in Rio de Janeiro. The averages of the proportions of answers for each indicator were calculated to analyze the results. Relationship and communication between professionals and users received the best evaluation and Organization of Services eceived the worst appraisal. Regarding education level, good self-perceived health and more elderly were more satisfied. The instrument proved to be easy to apply, can be routinely used for monitoring of the FHS, and is a tool for the institutionalization of evaluation.
