Abstract:Abstract Background: Post-Contrast acute kidney injury (PC-AKI) is a severe complication of coronary angiography (CAG) and percutaneous coronary intervention (PCI). At present, the mechanism and effect of statins on PC-AKI and its mechanism are still unclear. Methods: It was a multicenter retrospective obeservational study. We reviewed 4386 patients who underwent CAG or PCI, with serum creatinine recorded pre- or post-procedure within 72hrs after PCI, from December, 2006 to December, 2019 admitted to Sir Run Run Shaw Hospital and its medical consortium hospitals. Multivariate logical regression was used to explore whether preoperative use of statins was a protective factor for PC-AKI, and then path analysis model was used to explore the specific mechanism of statins. Results: In total, 4386 patients were enrolled in the study, the mean age was 67 yrs old, 17.9% with PC-AKI, 83.3% with pre-operative statin therapy. The incidence of PC-AKI was significantly lower in statin therapy group than non-statins therapy group. Multivariate regression indicated that pre-operative statin therapy was significantly negatively associated with percentage of elevated creatinine (β: -0.118, p<0.001) and PC-AKI (OR: 0.575, p<0.001). In pre-operative statin therapy group, no statistically significant deference was detected between atorvastatin and rosuvastatin group (OR: 1.052, p=0.558). Pathway model analysis indicated the direct protective effectiveness of pre-operative statin therapy on PC-AKI (p<0.001), but neither with its lipid-lowering effect (p=0.277) nor anti-inflammatory effect (p=0.596). Furthermore, It was found that “LDL-C→CRP” mediated the relationship between pre-operative statin therapy and PC-AKI (p=0.007); however, only explained less than 1% of the pre-operative statin therapy’s protective effects on PC-AKI. Conclusion: Pre-operative statin therapy is an independent protective factor of PC-AKI, which is not affected by the type of statins and not achieved by lipid-lowering effect or anti-inflammatory effect. Association of postprocedural neutrophil count,ST-segment resolution and NT-proBNP in ST elevation myocardial infarction patients after percutaneous coronary intervention

Abstract:Objective To explore the correlation of postprocedural neutrophil count,ST-segment resolution(STR) and NT-proBNP in ST elevation myocardial infarction patients(STEMI) after percutaneous coronary intervention(PCI).Methods A total of 195 STEMI patients who underwent successful PCI were enrolled.Electrocardiograms were recorded before PCI and two hours after PCI.Neutrophil counts and NT-proBNP were measured within 12 hours after PCI.Cardiac main adverse cardiac events(MACE) were also recorded during the follow-up survey after six months(P<0.05).According to the level of neutrophil after PCI,the patients were divided into three groups:group A(neutrophil<4.78×109/L),group B(neutrophil 4.78-8.79)×109/L,group C(neutrophil>8.79×109/L).Results Compared with group A,patients in group C less frequently had complete STR(17.8% vs 43.8%,P<0.05).The percentages of high neutrophil and plasma NT-proBNP were significantly higher in patients with STR<30%,compared those in patients with STR>70%(33.3% vs 13.8%,34.9% vs 12.3%,P<0.05 or <0.01).Linear relativity analysis showed that postprocedural neutrophil count was positively correlated with the level of NT-proBNP in group C.No statistical differences were found between the groups at incidence of six months' MACE(P>0.05).Conclusion There is a strong association between postprocedural neutrophil count,STR and NT-proBNP,suggesting the early measurement of postprocedural neutrophil count is helpful in evaluating the reperfusion of myocardium and the prognosis of patients with acute myocardial infarction.

Methods A total of 195 STEMI patients underwent successful PCI procedure were enrolled.Electrocardiograms were recorded before and 2 hours after PCI.Neutrophil counts and BNP were measured within 12 hours after PCI.Main adverse cardiac events (MACE) were also recorded during the follow-up survey at 6 months.According to neutrophil counts after PCI,patients were divided into 3 groups: group 1 (<4.78×10~9/L) ; group 2 (4.78-8.79×10~9/L) and group 3 (>8.79×10~9/L).Results Compared with group 1,patients in group 3 had higher occurrence rate of complete STR (P<0.01).The percentages of high neutrophil and plasma BNP were significantly higher in patients with STR >70% (13.8% and 11.1%) than in patients with STR 30% -70% and STR<30% (20.96% and 33.33%; 28.7% and 35.3%,all P<0.01).Logistic regression analysis showed that postprocedural neutrophil count was positively correlated with level of BNP.Compared with group 1 and 2 (6.25% and 6.78%),patients in group 3 (13.33%) showed higher incidence of MACE at 6 months.Conclusions There is a strong relation between postprocedural neutrophil count and STR or BNP,suggesting the early measurement of postprocedural neutrophil count is helpful in evaluating the reperfusion of myocardium and the prognosis of patients with STEM1.

Abstract:Objective:To study the effect of preoperative loading dose atorvastatin on the Hs-CRP,BNP levels and cardiac funetion of patients with STEMI emergency PCI.Methods:96 cases of patients with ST elevation myocardial infarction (STEMI) who were treated with emergency percutaneous coronary intervention (PCI) in our hospital from September 2014 to April 2016 were selected and randomly divided into the control group and the observation group with 48 cases in each group.The observation group was treated with atorvastatin 80 mg before the operation and 40mg/d after the operation,while the control group was treated with atorvastatin 40mg/d after the operation.Then the postoperative perfusion situation,serum high-sensitivity C reactive protein (hs-CRP),B type natriuretic peptide (BNP) and matrix metalloproteinase 9 (MMP-9) levels in the two groups before and after the treatment were observed and compared.Results:The ST segment drop rate,TIMI3 and TMP3 ratio in the observation group were significantly higher than those of the control group (P＜0.01);CTFC was significantly higher than that of the control group (t=4.43,P=0.00).The serum levels of hs-CRP,BNP and MMP-9 in the observation group were significantly lower than those of the control group (P＜0.01).The serum levels of CK-MB and cTnI in the observation group were significantly lower than those of the control group (P＜0.01).Conclusion:Administration of 80 mg atorvastatin was effective for the STEMI patients after PCI,which could promote the recovery of myocardial perfusion,inhibit the inflammation and improve the ventricular systolic function.

Abstract:【Objective】 To investigate the influence of high dose statin pretreatment on serum C-reactive protein(CRP) and lipid levels for percutaneous coronary intervention(PCI) patients with acute coronary syndrome(ACS).【Methods】 150 patients with ACS who were admitted to suffering from PCI from May 2010 to October 2011 were randomly divided into intensive therapy group and the conventional therapy group with 75 cases in each group.Patients in the intensive therapy group were given atorvastatin 80 mg orally,qd;but patients in the conventional therapy group received atorvastatin 20 mg orally,qd.A 7-day pretreatment with atorvastatin was given to the subjects.The two groups were identical in other medication treatment.CRP and lipid levels were measured at baseline,1 and 7 day after the procedure.【Results】 At baseline,1 and 7 day after PCI,CRP levels were respectively(8.1±3.1),(12.6±3.2),(7.9±2.7) mg/L in the intensive therapy group,and(8.21±3.0),(13.0±3.3),(11.8 ± 3.1) mg/L in the conventional therapy group.Postoperative CRP levels were first increased and then decreased,with no significant difference at 1 day after treatment(P >0.05) and significant differences at 7 day after treatment(P <0.05).Before treatment,TC,TG,HDL and LDL levels were not significantly different.After treatment,TC,TG and LDL levels were decreased,but HDL were increased,and the intensive therapy group compared with the conventional treatment group were significantly different(P <0.05).【Conclusion】 High-dose short-term intensive statin pretreatment for acute coronary syndrome underwent PCI can not only strengthen decreasing the lipid,but also can significantly reduce inflammation of PCI,thereby reducing the incidence of postoperative cardiovascular events.
Efficacy of High-dose Statins Pretreatment on PCI-related Inflammation and Myocardial Injury

Abstract:Objective To explore the effect of atorvastat in pretreatment on percutaneous coronary intervention ( PCI )-related inflammation and myocardial injury. Methods Eighty six patients with coronary heart diseases undergoing PCI were randomized into A group (n=30), B group (n=26) and C group (n=30). They were given 20, 80 and 0 mg atorvastatin tablet for 2 days before PCI, respectively. A and C group patients took 20 mg atorvastatin every night after PCI, well the B group took 40 mg per night. The neutrocyte count, the levels of high sensitivity C reactive protein (hs-CRP), enzyme, troponin T (cTnT), liver function, kidney function, blood lipids and electrocardiogram were determined before and after PCI. The incidence of perioperative myocardial infarction (MI) after successful PCI was also recorded. Results The changes of hs- CRP level, neutrocyte count were significant after PCI (P0.05). The degree of △hs-CRP, creatine kinase MB (CK-MB), cTnT elevation in B group was significantly less than that of A and C group (P0.05). Incidence of perioperative MI was lowest in the B group (P0.05). The highest compliance rate of LDL-C postoperative also in the B group (P0.05). The changes of liver and kidney function, creatine kinase were not significant before and after PCI in 3 groups (P0.05). Conculsion Pretreatment with atorvastatin 80 mg for patients under going PCI for 48 hours can inhibit PCI-related inflammation and have a protective effect against myocardial injury. Safety did not differ between high-dose and low-dose atorvastatin.

Abstract:Although several studies have suggested that intensive statin pretreatment could reduce the incidence of procedure-related myocardial infarction in western population, the data on the effect in Asian patients have been still limited. The aim of the study was to investigate the efficacy and safety of intensive atorvastatin load in Chinese patients undergoing elective PCI. A total of 1202 patients with stable angina or non-ST-segment elevation acute coronary syndrome (NSTE-ACS) scheduled to undergo PCI received either intensive statin treatment (80 mg atorvastatin daily x 2 days before PCI and 40 mg daily x 30 days after PCI) or usual care. The primary endpoint was incidence of major adverse cardiac events (cardiac death, myocardial infarction, or unexpected target vessel revascularization) within 30 days after PCI. Safety endpoints include the incidence of contrast induced nephropathy (CIN), ALT/AST >3 upper limit of normal (ULN), CK >5 ULN. The incidence of 30-day MACE did not significantly differ between the intensive group and control group (19.4 vs 18.3%, P = 0.63). Multivariate analysis revealed age (OR = 1.024, 95% CI 1.003-1.045, P = 0.023) and total stent length as an independent predictor of 30-day MACE (OR = 1.012, 95% CI 1.007-1.018, P < 0.0001). The incidence of CIN was comparable between intensive group and control group (4.09 vs 4.39%, P = 0.795). No significant differences were observed in other safety profile at all follow-ups between treatment groups. The ISCAP trial demonstrated that serial intensive atorvastatin therapy did not improve the clinical outcome with similar safety profile comparing with usual care among Chinese patients undergoing elective PCI.
Clinical effect of postconditioning in ST-elevation myocardial infarction patients treated with primary percutaneous coronary intervention: a meta-analysis of randomized controlled trials.

Abstract:OBJECTIVE:To evaluate the clinical effect of postconditioning on patients with ST-elevation myocardial infarction (STEMI) treated with primary percutaneous coronary intervention (PCI). METHODS:Randomized controlled trials were identified by searching relevant databases published up to April 2nd, 2014. A meta-analysis of eligible studies was performed by Stata 12.0 and Review Manager 5.2 with a fixed-effect model. RESULTS:Ten studies providing adverse cardiac events in a total of 1346 STEMI patients treated with primary PCI were identified. The occurrence of heart failure was significantly reduced in patients treated with postconditioning compared with usual care (risk ratio (RR) 0.533; 95% confidence intervals (CI) 0.368-0.770), whereas non-fatal reinfarction slightly increased in the postconditioning group (RR 2.746; 95% CI 1.007-7.488). No significant difference in total major adverse cardiac events (MACEs) was observed between the two groups (RR 0.876; 95% CI 0.671-1.144). CONCLUSIONS:Postconditioning in STEMI patients undergoing primary PCI significantly reduces the risk of heart failure, but fails to decrease the incidence of total MACEs and the risk of non-fatal reinfarction.
The Effects of Atorvastatin on Interventional Therapy in Patients with Acute Myocardial Infarction

Abstract:BACKGROUND: The aim of this study was to analyze the changes of interleukin-6 (IL-6), C-reactive protein (CRP), blood lipids and myocardial indexes after treatment of patients with acute myocardial infarction (AMI) with intensive atorvastatin and interventional therapy, and its clinical significance. METHODS: A total of 78 patients diagnosed with AMI in our hospital from March 2016 to February 2017 were selected and divided into treatment group (N.=39) and control group (N.=39). Patients in treatment group were treated with intensive atorvastatin based on conventional therapy before and after percutaneous coronary intervention (PCI), while those in control group were treated with conventional therapy before and after PCI. The levels of serum IL-6, CRP, blood lipids (total cholesterol [TC], triglyceride [TG], high-density lipoprotein cholesterol [HDL-C] and low-density lipoprotein cholesterol [LDL-C]) and myocardial enzyme indexes (troponin T [TnT] and creatine kinase-MB [CK-MB]) at different time points were detected. The correlations among serum CRP, TC, and TnT in treatment group before treatment were detected using the linear regression analysis, and changes in serum inflammatory factors, blood lipids and myocardial enzyme indexes in treatment group before and after treatment were analyzed. RESULTS: There were no statistically significant differences in demographics and clinical data between treatment group and controls (P>0.05). There were no significant differences, either, in levels of serum IL-6 and CRP before treatment between treatment group and control group, but they were decreased after treatment, and the curative effect in treatment group was significantly superior to that in control group. The differences were statistically significant (P<0.05). There were no significant differences in serum TC, TG, HDL-C and LDL-C levels before treatment between the two study groups. TC, TG and LDL-C were significantly decreased and HDL-C was significantly increased after treatment compared to before treatment. TnT and CK-MB were significantly increased at 24 hours after treatment (P<0.05). At 1 week, TnT was still higher, whereas CK-MB has returned to normal; at 2 weeks, they were both decreased. The curative effect in the treatment group was significantly superior to that in control group. CRP (r=0.793, P<0.001) and TC (r=0.668, P<0.001) were found to be positively correlated with TnT. The levels of serum inflammatory factors and blood lipids in the treatment group before treatment and at 24 hours, 1 week, and 2 weeks showed a decreasing trend, and TnT was increased at first, and then decreased. CONCLUSIONS: The application of intensive atorvastatin for AMI patients, especially before PCI, has high safety, which can effectively reduce levels of serum inflammatory factors and blood lipids, protect myocardial cells after PCI and avoid injury.
Combined Intracoronary Prourokinase Thrombolysis on Myocardial Perfusion and Vascular Endothelial Function in STEMI

Abstract:Background: This study aimed to investigate the effects of intracoronary prourokinase thrombolysis combined with emergency percutaneous coronary intervention (PCI) on myocardial perfusion and vascular endothelial function in patients with acute ST-segment elevation myocardial infarction (STEMI). Methods: A total of 104 patients with STEMI were collected from August 2020 to August 2022, and were divided into control group and observation group in a random manner. The control group received PCI directly, and the observation group received intracoronary prourokinase thrombolytic therapy before PCI. The treatment effects were evaluated by measuring the cardiac function indexes, including left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), and left ventricular ejection fraction (LVEF), the TIMI myocardial perfusion grade, the vascular endothelial indexes, including soluble intercellular adhesion molecule-1 (sICAM-1) and soluble vascular cell adhesion molecule-1 (sVCAM-1), the von Willebrand factor (vWF), the myocardial injury indexes, including cardiac troponin I (cTnI), creatine kinase isoenzyme MB (CK-MB), and lactate dehydrogenase (LDH), and the inflammatory factors, including myeloperoxidase (MPO), C-reactive protein (CRP), and interleukin-6 (IL-6). Furthermore, the treatment safety was assessed by recording the incidence of major MACE events, 6 months after the operation. Results: After treatment, LVEDD and LVESD were lower in the observation group than in the control group, and LVEF was higher (p < 0.05). The TIMI myocardial perfusion grade in the observation group was higher than in the control group, after treatment (p < 0.05). The levels of sICAM-1, sVCAM-1, and vWF were higher in the observation group than in the control group (p < 0.05). The levels of cTnI, CK-MB, and LDH in the observation group were lower than those in the control group, 24 hours after surgery. At 3 days after surgery, MPO was lower in the observation group than in the control group, and CRP and IL-6 were higher (p < 0.05). The incidence of major MACE events in the observation group was lower than that in the control group, 6 months after surgery (p < 0.05). There was 1 case of puncture site bleeding in the observation group, 1 case of puncture site bleeding and 1 case of subcutaneous ecchymosis in the control group, but no serious bleeding events, such as internal bleeding or cerebral hemorrhage, in the two groups. Conclusions: Intracoronary prourokinase thrombolytic therapy combined with emergency PCI can promote the recovery of cardiac function, improve myocardial perfusion and vascular endothelial function, and reduce inflammation and the incidence of major postoperative MACE events in acute STEMI patients.
Effect of Statins Pretreatment on Periprocedural Myocardial Infarction in Patients Undergoing Percutaneous Coronary Intervention: a Meta-Analysis

Abstract:Background. Periprocedural myocardial injury remains the most common complication associated with percutaneous coronary intervention (PCI). Previous studies have demonstrated that even a small elevation of cardiac enzymes is associated with higher risk of mortality during follow-up.Objective. We performed a meta-analysis based on all currently available randomized controlled trials (RCT) to evaluate the beneficial effects of hydroxymethylglutaryl-CoA reductase inhibitors (statins) given before PCI on preventing periprocedural myocardial infarction (MI).Methods. The published literature was scanned by formal searches of electronic databases (PubMed, EMBASE, and the Cochrane Central Register of Controlled Trials) and conference proceedings up through August 2009. RCTs were eligible for inclusion if they compared preprocedural statins versus placebo treatment in patients not taking statins previously but scheduled for PCI and had the data of periprocedural MI reported by the trial investigators.Results. Prespecified criteria were met by 6 RCTs involving 2,088 patients. During the periprocedural period, 81 of 1,051 patients (7.7%) in the statins pretreatment group developed periprocedural MI, significantly less than 147 of 1,037 (14.2%) patients assigned to the control group (OR 0.51, 95% CI 0.38-0.67; P < 0.001). During 1-month follow-up, only 4 deaths, 7 non-periprocedural Q-wave MIs, and 4 revascularizations occurred in all 2,088 enrolled patients. The composite of death, MI, or target vessel revascularization at 1 month, essentially driven by periprocedural MI, was reported in 8.0% in the statins pretreatment group and 15.3% in the control group (OR 0.48, 95% CI 0.36-0.64; P < 0.001). Conclusions. This meta-analysis supports the effectiveness of statins pretreatment on reducing the rate of periprocedural MI in patients undergoing PCI.

Abstract:Objective This study sought to investgate the impact of high loading-dose atorvastatin pre-treatment in patients with ST-segment elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PCI) on coronary reperfusion, short-term clinical outcome and safety.Methods We collected consecutive patients with STEMI in our hospital from January 2010 to March 2011,83 patients who did not receive any statin therapy before admission and only received atorvastatin after admission were included. Patients were divided in 2 groups :those who received atorvastatin 80 mg before primary PCI (n=41) and those who did not (n=42).The primary end point included thrombolysis in myocardial infarction flow grade, corrected thrombolysis in myocardial infarction frame count, and myocardial blush grade after PCI. Secondary end point was 30-day incidence of MACE including cardiovascular death, nonfatal MI, and target vessel revascularization. The safety end point was the level of liver enzyme, creatine kinase and serum creatinine.Results Baseline clinical and procedural characteristics were similar between the 2 groups. The proportion of thrombolysis in myocardial infarction flow grade 3 was higher in the 80-mg atorvastatin arm(87.8% vs. 64.3%,χ2=6.28,P=0.02), and corrected thrombolysis in myocardial infarction frame count was also lower in the 80-mg atorvastatin arm (21.72±9.42 vs. 32.64±15.51, t=-3.87,P<0.001). Simultaneously, myocardial blush grade were higher in the 80 mg atorvastatin arm (2.71±0.64 vs.2.15±0.74,t=3.68,P<0.001). MACE occurred in 2 (4.9%) and 2 (4.8%) patients in the atorvastain 80 mg pre-treatment and control arm, respectively (χ2=0.001,P=0.926).There was not significant difference of liver enzyme, creatine kinase and serum creatinine between 2 groups. Multivariate analysis showed that high loading-dose atorvastatin pre-treatment was associated with an odds ratio of 0.22 (95% confidence interval 0.06 to 0.87, P=0.03) for slow reflow or no reflow after PCI.Conclusions High loading-dose atorvastatin pre-treatment may produce an optimal result for STEMI patients undergoing PCI by improving microvascular myocardial perfusion without increasing the risk of side-effect.

Abstract:Previous studies have demonstrated the beneficial effect of statin loading prior to elective and early percutaneous coronary intervention (PCI), in which the ‘pleiotropic effects’ of statins may contribute to these clinical benefits. The aim of the present study was to examine the potential effects of atorvastatin loading prior to primary PCI on coronary endothelial function and inflammatory factors in patients with acute ST-segment elevation myocardial infarction (STEMI). A total of 60 patients with STEMI were randomized into three groups: Loading dose (80 mg atorvastatin prior to PCI; n=20), regular dose (20 mg atorvastatin prior to PCI; n=20) and control (without atorvastatin prior to PCI; n=20). The plasma samples were collected prior to, and immediately, 6 and 24 h after PCI in all the patients. The plasma concentrations of endothelial nitric oxide synthase (eNOS), nitric oxide (NO), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and intercellular adhesion molecule-1 (ICAM-1) were examined using ELISA. The plasma eNOS levels immediately and 24 h after PCI were significantly higher in the regular dose group compared with the other groups. However, there were no significant differences in the plasma eNOS concentration prior to and 6 h after PCI, or in the plasma NO concentration at any of the time-points among the three groups. The plasma IL-6 levels prior to PCI were significantly lower in the loading dose group compared with the other groups; however, there were no significant differences in the plasma concentration of IL-6 following PCI or in the concentrations of TNF-α and ICAM-1 at any of the time-points among the three groups. In conclusion, atorvastatin loading in patients with STEMI undergoing primary PCI may not have protective effects on endothelial function and the inflammatory reaction.

Abstract:Objective To investigate the effect of simvastatin on blood levels of high-sensitive c-reactive protein (hs-CRP) and interleukin 6 (IL-6) in patients with unstable angina post percutaneous coronary intervention (PCI). Method Eighty patients who underwent PCI from January 2007 to August 2008 in our hospital were randomised into two groups: control group and simvastatin group (40 mg/day). Serum levels of hs-CRP, and IL-6 were measured before and after one week treatment and at 24 h after PCI. Results The serum levels of hs-CRP and IL-6 in simvastatin group were distinctly lower than those in control group before (p<0.05) and after the PCI (p<0.05), respectively. Conclusions The result suggested that PCI could lead to a detectable increase in serum levels of hs-CRP and IL-6 in patients with coronary heart disease; Simvastatin could significantly decrease the serum levels of hs-CRP and IL-6 in patients with coronary heart disease; Simvastatin could also decrease the serum levels of hs-CRP and IL-6 in patients with PCI.
Clinical Value of Different Cardiac Rehabilitation Programs Applied to Patients with Acute Myocardial Infarction Early after Percutaneous Coronary Intervention

Abstract:Objective To investigate the clinical value and impact of cardiac rehabilitation programs applied to patients with acute myocardial infarction(AMI) early after percutaneous coronary intervention(PCI).Methods The enrolled participants were 90 cases of AMI who received PCI in Shaoxing People′s Hospital from January 2014 to May 2016.They were randomized into traditional routine group,traditional strengthening group and new optimization group with 30 cases in each.The cardiac rehabilitation program for traditional routine group was developed based on the rehabilitation program applied to patients one week after emergency PCI included in 2006 Chinese Cardiac Rehabilitation Programs after PCI,mainly consisting of simple forms of exercise such like walking.Both the traditional strengthening group and new optimization group received the cardiac rehabilitation program developed based on the rehabilitation program applied to patients after selective PCI included in 2006 Chinese Cardiac Rehabilitation Programs after PCI.In addition,the new optimization group received the early-applied,personalized and accurate cardiac rehabilitation program developed in accordance with the latest concepts in cardiac rehabilitation received by Avans University,UMC St RAdboud,Queen Elizabeth Hospital and other hospitals in Hong Kong.The traditional strengthening group and new optimization group had greater basal activities and total activities than the traditional routine group.The intervention period for all the groups was six months.Color Doppler echocardiography was performed in the groups immediately after PCI(before the cardiac rehabilitation intervention) and six months after the starting of cardiac rehabilitation program(after the cardiac rehabilitation intervention),respectively for measuring the left ventricular ejection fraction(LVEF),left ventricular end-systolic volume(LVESV),left ventricular end-diastolic volume(LVEDV) and wall motion score index(WMSI).Levels of serum tumor necrosis factor-alpha(TNF-α),nitric oxide(NO),endothelin 1(ET-1) and soluble vascular cell adhesion molecule 1(sVCAM-1) were measured before and after the cardiac rehabilitation intervention.The incidences of malignant arrhythmia,angina pectoris,heart failure and sudden death during the cardiac rehabilitation were observed.Results No significant differences in LVEF,LVESV,LVEDV,WMSI,TNF-α,NO,ET-1 and sVCAM-1 levels were found among the three groups before the cardiac rehabilitation(P>0.05).After the cardiac rehabilitation intervention,the WMSI was lower in the traditional strengthening group than that in the routine traditional group,LVEF was higher in the new optimization group than that in the routine traditional group and traditional strengthening group,WMSI was lower in the new optimization group than that in the routine traditional group and traditional strengthening group(P<0.05).LVEF was higher after the cardiac rehabilitation intervention than that before the cardiac rehabilitation intervention in the three groups(P<0.05).After the cardiac rehabilitation intervention,the levels of TNF-α,ET-1 and sVCAM-1 were lower in the traditional strengthening group than those in the routine traditional group,the level of NO was higher in the traditional strengthening group than that in the routine traditional group,the levels of TNF-α,ET-1 and sVCAM-1 were lower in the new optimization group than those in the routine traditional group and traditional strengthening group,the level of NO was higher in the new optimization group than that in the routine traditional group and traditional strengthening group(P<0.05).The levels of TNF-α,ET-1 and sVCAM-1 were lower,but the level of NO was higher after the cardiac rehabilitation intervention than that before the cardiac rehabilitation intervention in the three groups(P<0.05).During the cardiac rehabilitation intervention,the incidence of malignant arrhythmia differed significantly among the three groups(P<0.05);three groups had similar incidences of angina pectoris and heart failure(P>0.05);no sudden death occurred in all groups.Conclusion Cardiac rehabilitation training applied to patients with AMI early after PCI can obviously improve the cardiac function and prognosis.The humanitarian cardiac rehabilitation program based on personal conditions used in the new optimization group can achieve the optimal effect of rehabilitation,which can also protect the cardiac function to the utmost and improve the prognosis tangibly.
Effect of early rehabilitation nursing care after emergency PCI on hemodynamics and serum inflammatory factors in patients with acute myocardial infarction (AMI)

Abstract:Objective: To analyse the effect of early rehabilitation nursing after emergency PCI on hemodynamics, quality of life, serum inflammatory factors, lipoprotein related phospholipase A2 (Lp-PLA2), IL-6 and TNF-α in patients with AMI. Methods: 120 cases of AMI patients treated with PCI were enrolled and divided into control group and study group according to the visiting sequence, and with 60 cases in each group. Patients in the control group received routine nursing care, while patients received early rehabilitation nursing in the study group. Postoperative hemodynamics, quality of life, levels of serum inflammatory factors, lipoprotein associated phospholipase A2 (Lp-PLA2), IL-6 and TNF-α were compared between the two groups. Results: Before the intervention, there were no significant differences between the two groups in cardiac output, systolic blood pressure, diastolic blood pressure, pulse pressure and heart rate (p>0.05). After the intervention, cardiac output of patients in the two groups significantly increased (p 0.05). Before the intervention, there was no significant difference on the quality of life and pain scores between the two groups (P>0.05). After the intervention, quality of life scores increased significantly and pain scores decreased significantly in the two groups (P 0.05). After the intervention, the levels of serum Lp-PLA2, IL-6 and TNF-α were significantly lower than the control group (p<0.05). Conclusion: Early rehabilitation nursing care after emergency PCI in patients with AMI can improve hemodynamics and the quality of life. It can also reduce serum Lp-PLA2, IL-6 and TNF- levels and has certain clinical application value.

Abstract:This study aimed to investigate the effect of the therapy of amiodarone combined with atorvastatin on cardiac function of patients with acute myocardial infarction after percutaneous coronary intervention (PCI). A total of 90 patients with acute myocardial infarction who underwent PCI in the tertiary care hospital from January 2019 to January 2020 were selected as the subjects and randomly assigned into the control group and the study group, with 45 cases in each group. All the subjects had undergone PCI. The control group received amiodarone while those the study group received atorvastatin additionally. Comparison was done on the clinical efficacy, cardiac function, myocardial injury indicator and inflammatory factor between the two groups. The overall response rate (ORR) in the study group was significantly higher than that in the control group (P<0.05); patients in the study group had markedly better cardiac function compared with those in the control group (P<0.001); patients in the study group had considerably lower creatine kinase (CK) index, creatine kinase-MB (CK-MB) index, tumor necrosis factor (TNF-α) and interleukin-6 (IL-6) as opposed to those in the control group (P<0.001). It was observed that the therapy of amiodarone combined with atorvastatin could effectively improve the clinical indicators and cardiac function of patients with acute myocardial infarction after PCI. It is effective and worthy of wide promotion and application.

Abstract:Objective: To investigate the effect of thrombus aspiration with intra-infarct-related artery(IRA) tirofiban and intravenous tirofiban injection on cardiac function and no-reflow in patients of acute ST-elevation myocardial infarction(STEMI) with emergent PCI treatment. Methods: A total of 90 STEMI patients with emergent PCI treated in our hospital were enrolled and randomly divided into 2 groups. Experimental group, in which the patients received thrombus aspiration with IRA tirofiban injection during PCI, n=44, and Control group, the patients received intravenous tirofiban injection during PCI, n=46. The post procedural plasma levels of NT-proBNP and left ventricular end-systolic diameter(LVESD), LVEDD, left ventricular ejection fraction(LVEF) and major adverse cardiovascular events(MACE) were examined and compared between two groups. Results: Compared with Control group, more patients in Experimental group achieved TIMI 3 flow and had lower level of plasma NT-proBNP, both P0.05. At 30 days after the operation, the Experimental group also showed higher LVEF, lower LVESD, LVEDD and MACE, all P0.05. Conclusion: Emergent PCI with IRA tirofiban injection may reduce the incidence of no-reflow, during PCI, therefore improve the cardiac function and early effect of treatment in STEMI patients.
Effect of Statin Therapy Before Percutaneous Coronary Intervention on Periprocedural Myocardial Injury

Abstract:Objective:To study the effect of statin in reduction of myocardial injury in patients undergoing percutaneous coronary intervention(PCI).Methods:Total of 137 patients undergoing PCI were included,66 received statins before PCI,while 71 were not given statins at the time of PCI. Creatine kinase-MB and troponin I were measured at baseline and at 8 and 24 hours after the procedure.Results:Postprocedural peak levels of troponin I (0.15±0.23) versus (0.48±1.08)ng·mL-1,P0.05,and creatine kinase-MB (2.44±3.05) versus (4.98±10.89) ng·mL-1,P0.05 were lower in the statin than in the control group. Detection of markers of myocardial injury above the upper normal limit was significantly lower in the statin group versus the control group:24% versus 48% for troponin I (P0.05),10% versus 20% for creatine kinase-MB (P0.05). Myocardial infarction by troponin I determination was detected after procedure in 12% of patients in the statin group and in 25% of those in the control group (P0.05).Conclusion:Pretreatment with statin for 7 days significantly reduces procedural myocardial injury in elective PCI.

Abstract:Objectives The Novel approaches for preventing or limiting events (NAPLIES) and The Atorvastatin for Reduction of Myocardial Damage during Angioplasty (ARMYDA) studies demonstrated a beneficial effect of statin loading in preventing major adverse cardiac events (MACE) after elective percutaneous coronary intervention (PCI) for stable angina, unstable angina, non-ST-segment-elevation myocardial infarction (NSTEMI). The so called ‘pleiotropic effects’ of statins include modulation of endothelial function, inhibition of inflammation, and attenuation of thrombosis, all of which could provide clinical benefits in the setting of elective PCI by reducing postprocedural incidence of myocardial and MACE. So far, the efficacy of atorvastatin loading in patients with acute ST-segment-elevation myocardial infarction (STEMI) undergoing primary PCI has not been confirmed. Also, whether the ‘pleiotropic effects’ of statins could explain the possible mechanism(s) needs to be discussed. This study sought to explore potential protective effects of statin loading before primary PCI on coronary endothelial function, inflammation, and MACE. Methods A total 60 patients with STEMI were randomised into loading dose group (80 mg atorvastatin before PCI, n=20), regular dose group (20 mg atorvastatin before PCI, n=20), and control group (without atorvastatin before PCI, n=20). All patients received primary PCI and routine treatment. The plasma samples were collected before, immediately after, 6 h after and 24 h after PCI in all the patients. Plasma concentrations of endothelial nitric oxide synthase (eNOS), Nitric Oxide (NO), interleukin-6 (IL-6), tumour necrosis factor (TNF-а), intercellular adhesion molecule-1 (ICAM-1) were tested by ELISA. The results of coronarography, electrocardiogram, myocardial enzyme, high-sensitivity C-reactive protein (hs-CRP), amino terminal-pro brain natriuretic peptide (NT-proBNP), echocardiography, MACE, and the safety of statin loading were also collected. Results Plasma eNOS immediately and 24 h after PCI were higher in the regular dose group (p<0.05). Plasma eNOS before and 24 h after PCI, along with plasma NO at any time point did not show significant differences among the 3 groups. Plasma IL-6 before PCI were lower in the loading dose group (90.773±7.646 pg/ml vs 95.592±4.269 pg/ml vs 94.324±3.692 pg/ml, p=0.023). Plasma IL-6 after PCI, plasma TNF-a and ICAM-1 at any time point did not show significant differences among the 3 groups. MACE occurred in 2 (10.0%) patients in the loading dose group, 2 (10.0%) patients in the regular dose group, 3 (15.0%) patients in the control group, respectively (p=0.855). Conclusions Atorvastatin loading in patients with STEMI undergoing primary PCI may not have protective effects on coronary endothelial function, inflammation, and MACE.
