Effect of Lifeway bar meal replacement on integrated health management in patients with overweight and obesity

Abstract:Objective To observe the effects of integrated health management on losing weight of the overweight and obesity adults and controlling of chronic metabolic disease. Methods One hundred and sixty cases with overweight and obesity were randomly divided into lifeway group and control group. Both groups were received integrated health management with diet,exercise and psychology for 6 months. Lifeway group was received one lifeway bar meal replacement per day on the basis of conventional treatment. The numbers with hypertension and diabetes who did not reach the target and fatty livers were recorded before integrated health management and at every month after integrated health management. The body mass index (BMI),waist hip rate (WHR), body fat percentage, triglyceride, total cholesterol and high-density lipoprotein cholesterol (HDL-C)were assessed 2 and 6 month after integrated health management were also recorded and compared. Results The numbers of hypertension who did not reach the target in both groups were significantly decreased at 4 months after integrated health management (χ2=5.78, 7.11, P<0.05). The numbers of diabetes who did not reach the target in lifeway group and control group were significantly decreased at 2 months and 4 months after health management respectively (χ2=12.38, 4.05, P<0.05). In lifeway group, the numbers of fatty liver were significantly decreased at 6 months after health management (χ2=3.66,P<0.05). With integrated health management, the numbers of diabetes who did not reach the target and the numbers of fatty liver in lifeway groups were gradually decreased. The difference were statistically significant when compared with control group (χ2=4.35, P<0.05). The levels of BMI, WHR, body fat percentage, triglyceride, total cholesterol and HDL-C in both groups were significant improved after the health management. There were no significant difference between the two groups (t=0.26,0.80,1.94,0.34,1.96,1.71, P>0.05). In lifeway group, there were significant difference in BMI, WHR, body fat percentage, triglyceride,total cholesterol and HDL-C between 2 months after health management and before management (t=7.62, 6.99, 6.18, 3.68, 5.80,-6.39, P<0.05) as well as compared with 6 months after health management (t=8.69, 14.24, 10.75, 4.30, 7.45,-5.88, P<0.05). In control group, there were significant difference in BMI, WHR, triglyceride and total cholesterol between 2 months after health management and before management (t=4.01, 2.27, 3.53, 2.19, P<0.05). The BMI,WHR, body fat percentage, triglyceride, total cholesterol and HDL-C between 2 months and 6 months after health management were significantly different (t=10.60,18.06,2.48,2.48,3.89,-3.71,P<0.05). Conclusion Integrated health management has obvious effect on losing weight and controlling of the metabolic syndrome in patients with overweight and obesity. Lifeway bar meal replacement is more efficacious in controlling blood glucose level in diabetic patients.
Real-World Assessment of Personalized Approach With Voglibose Fixed-Dose Combination in Type 2 Diabetes Mellitus

Abstract:BACKGROUND: Post-prandial hyperglycemia (PPHG) remains a complex aspect in the management of type 2 diabetes mellitus (T2DM) in the Indian population due to uncertainty in the optimal utilization of alpha-glucosidase inhibitors (AGIs) either as standalone therapy or in combination, whether initiated initially or as a sequential therapy.METHODS: This was a post-approval, observational, multicentric clinical study conducted at 50 centers according to principles of the International Council for Harmonisation of Technical Requirements of Pharmaceuticals for Human Use Guideline for Good Clinical Practice (ICH-GCP) and Declaration of Helsinki and local ethics approval. Descriptive and analytical statistics were applied for conclusion and categorical variables using SPSS version 29.0.1.0 (171) (Armonk, NY: IBM Corp.).RESULTS: Protocol analyses of 515 cases revealed baseline demographics as follows: age 57.35±10.04 years, weight 72.86±10.92 kg, and BMI 28.33±6.07 kg/m<sup>2</sup>. Comorbidities included hypertension (N=169, 32.82%), thyroid disorders (N=99, 19.22%), and heart failure (N=92, 17.86%). Concomitant oral antidiabetics (OADs) prescribed as DPP4i (9.50%), SGLT2i (19.20%), and DPP4i+SGLT2i (7.20%). Study drug reduced glycosylated hemoglobin (HbA1c) by 13.77% (1.25% mean change, p<0.01), fasting blood glucose (FBG) by 23.69% (44.61 mg/dL mean change, p<0.01), post-prandial blood glucose (PPBG) by 24.57% (70.46 mg/dL mean change, p<0.01), and body weight by 4.43% (3.21 kg mean change, p<0.01) over 12 weeks. A total of 161 patients accomplished targeted PPBG of <180 mg/dL (119.13 mg/dL mean change, p<0.01). Regression analysis considering PPBG and HbA1c ≤7.5% showed a weak correlation between them (R-value=0.13, R-squared value=0.02), whereas between PPBG and HbA1c ≤9% yielded moderate positive correlation (R-value=0.53, R-squared value=0.28). There were no adverse events reported or analyzed during the observation period.CONCLUSION: Voglibose fixed-dose combination (FDC) demonstrates significant effectiveness at the initial dosage when started early in the management of T2DM and high PPBG levels. Moreover, it exhibits good tolerability, thereby contributing to higher compliance among Indian patients who consume a high-carbohydrate diet.
Efficacy and Safety of Combination Therapy with Vildagliptin and Metformin Versus Metformin Uptitration in Chinese Patients with Type 2 Diabetes Inadequately Controlled with Metformin Monotherapy: A Randomized, Open-Label, Prospective Study (Vision)

Abstract:AimsTo compare the efficacy and safety of combination of vildagliptin and metformin therapy with metformin uptitration in Chinese patients with type 2 diabetes (T2DM) inadequately controlled with low‐dose metformin.MethodsIn this 24‐week prospective, randomized, multicentre, open‐label study, patients with T2DM inadequately controlled with metformin ≤1000 mg daily were divided 1 : 1 : 1 : 1 into four prespecified subgroups based on age and body mass index (BMI). Patients in each subgroup were randomized 5 : 1 to receive either vildagliptin (50 mg twice daily) plus metformin [500 mg twice daily; vildagliptin and low‐dose metformin (VLDM) group] or metformin uptitration [1000 mg twice daily; high‐dose metformin (HDM) group]. The primary endpoint was change in glycated haemoglobin (HbA1c) from baseline at week 24. The key secondary endpoints included percentage of patients achieving target HbA1c without adverse gastrointestinal (GI) events and mean change in fasting plasma glucose (FPG) from baseline to week 24.ResultsA total of 3084 patients were randomized. HbA1c reduction of 0.54% at week 24 in the VLDM group was non‐inferior and statistically superior compared with 0.40% in the HDM group (P < 0.0001). VLDM's non‐inferiority to HDM was confirmed in the four subgroups and its superiority was shown for all subgroups (p < 0.05) except for the subgroup of patients aged <60 years with a BMI of ≥24 kg/m2. Compared with HDM, VLDM significantly increased the percentage of patients achieving HbA1c ≤6.5% and HbA1c ≤6.5% without GI events. FPG levels in the VLDM group were lower at week 24 numerically than in the HDM group. The two treatment arms had similar safety profiles.ConclusionsVLDM was non‐inferior and statistically superior to HDM in glycaemic control in Chinese patients with T2DM inadequately controlled with low‐dose metformin.
Reversal of type 2 diabetes mellitus through integrated Ayurveda dietary protocol - A case report

Abstract:Type 2 Diabetes Mellitus (T2DM) is a chronic metabolic disorder of hyperglycemia has close link with obesity and insulin resistance. Standard approaches in T2DM management are lifestyle management and Pharmacotherapy. Dietary management in T2DM was found to be safe and effective. In India, studies on reversal of T2DM through diet were less found. Presenting a case of female aged 31 years with T2DM (HbA1c-13.5) was successfully reversed diabetes with dietary principles of Indian traditional medicine (Ayurveda diet) and therapeutic yoga practices. Pre and post assessment of Glycaemic parameters (HbA1C, FBS, and PPBS), Lipid profiles, Insulin resistance parameter (HOMA IR) were done. Major outcome of this case is greater reduction of HbA1c from inadequate control to near normal. Marked changes observed in weight, BMI and Waist circumference. A change in Homa IR indicates improvement in insulin resistance. Ayurveda dietary management training include Therapeutic fasting [Two meal method], meals include nutrient dense wholesome food, Indian millet based diet, herbal recipes [therapeutic food/herbal drinks, smoothies and plant juices] and eat only when hungry method. Yoga include complete sequel of body loosening practices, surya namaskara, yogic postures and breathing and meditative techniques practiced for specified period [60 min].This case showed effectiveness of Ayurveda diet and Yoga practices in reversing the insulin resistance, help to maintain the glycemic parameters .In addition, patient was able to loose body weight, waist circumference & serum triglyceride levels. Patient can able to withstand her diabetic profile in normal without any pharmacotherapy intervention.

Abstract:Background: Type 2 diabetes is a lifestyle-related disorder that affects around 422 million individuals in India. Integration of AYUSH (Ayurveda) with the National Programme for Prevention and Control of Cancer, Diabetes, Cardiovascular Diseases and Stroke (NPCDCS) was conceived on pilot basis at Gaya, Bihar, to provide integrative treatment for non-communicable disease patients and to manage the burden of non-communicable diseases in India. Objectives: The objective of this study was to analyze the effect of Ayurveda intervention, lifestyle modification and Yoga in the management of type 2 diabetes under NPCDCS-AYUSH integration project. Materials and Methods : A multi-centric, open-labeled, prospective, comparative clinical study was conducted at 17 community health centers and 1 district hospital. Population over 30 years of age was screened and prediabetic or type 2 diabetic individuals were enrolled in two cohorts, i.e., pre-diabetic (Cohort A) and type 2 diabetic (Cohort B). Each cohort was further divided into two groups: Group A1 was advised for lifestyle modification and Yoga and group A2 was given Ayurveda medication in addition to lifestyle modification and Yoga . Similarly, group B1 was advised for lifestyle modification and Yoga along with allopathic medication and group B2 was given Ayurveda medication, i.e., Mamajjaka , Amalaki and Guduchi powder in addition to lifestyle modification and Yoga along with allopathic medication. Treatment was given for 6 months. Data were analyzed through paired t -test. Results: A significant reduction was observed in fasting blood sugar level in groups A2 and B2 ( P = 0.001) and also in the postprandial blood sugar level in Groups A2 and B2 ( P = 0.001). Further, improvement in subjective symptoms such as polyuria, polydipsia, polyphagia, blurred vision and weakness was found in all the groups, while non-healing ulcer does not show any improvement. Conclusion: The study reveals that Ayurveda intervention, i.e., Mamajjaka Churna (1 g), Amalaki Churna (3 g) and Guduchi Churna (3 g) two times a day effectively controls blood sugar level in pre-diabetic and type 2 diabetic patients and improves the disease management with lifestyle modification and Yogasana as well as with allopathic treatment.

Abstract:It is of interest to evaluate the clinical effectiveness and safety of vildagliptin as monotherapy and combination therapy of vildagliptin and metformin for the management of type 2 diabetes mellitus (T2DM) patients in Indian settings. The study included patients with T2DM (aged >18 years) receiving vildagliptin monotherapy and vildagliptin in combination with metformin therapy of various strengths. Data related to demographics, risk factors, medical history, glycated hemoglobin (HbA1c) levels, and medical therapies were retrieved from medical records. Out of 9678 patients (median age, 52.0 years), 59.1% were men. A combination of vildagliptin and metformin (50/500 mg) was the most commonly used therapy (54.8%), and the median duration of therapy was 24.0 months. The predominant reason for selecting vildagliptin therapy was to improve HbA1c levels (87.8%). A total of 87.5% of patients required dosage up-titration. Vildagliptin therapy was used in patients with T2DM and associated complications (peripheral neuropathy, CAD, nephropathy, retinopathy, autonomous neuropathy, stroke/TIA, and peripheral artery disease). Among 5175 patients who experienced body weight changes, a majority of patients had lost weight (68.6%). The target glycemic control was achieved in 95.3% of patients. The mean HbA1c levels were significantly decreased post-treatment (mean change: 1.34%; p<0.001). Adverse events were reported in 0.4% of patients. Physicians rated the majority of patients as good to excellent on the global evaluation of efficacy and tolerability scale (98.9%, each). Vildagliptin with or without metformin was an effective therapy in reducing HbA1c helps in achieving target glycemic control, and was well-tolerated in Indian patients with T2DM continuum.
Efficacy of oral vitamin D on glycated haemoglobin (HbA1c) in type 2 diabetics having vitamin D deficiency - A randomized controlled trial

Abstract:Objective: To study the efficacy of oral vitamin D in improving glycaemic control of patients with type 2 diabetes by reducing glycated haemoglobin levels. Methods: This randomised control trial was carried out at Mayo Hospital, Lahore, from February 5 to August 5, 2016, and comprised type 2 diabetes patients aged 40-70 years visiting the outpatient clinics. They were randomly divided into two groups by using the lottery method. Group A received oral vitamin D along with metformin and group B received metformin only. Blood samples of both the groups were tested for glycated haemoglobin at three months to assess the change. SPSS 21 was used for data analysis. Results: There were 140 patients divided into two groups of 70(50%) each. Mean age in Group A was 54.80±8.55 years and 58.40±7.98 years in Group B. No significant difference was seen in glycated haemoglobin levels at baseline (p>0.05). However, after 3 months post-treatment the levels significantly differed (p<0.05) in favour of Group A. Conclusions: Vitamin D supplementation had a significant effect in lowering glycated haemoglobin level in patients with type 2 diabetes.

Abstract:We study breakdown in high-quality 2-7 nm gate dielectrics, and find that soft breakdown becomes more likely for thinner oxides and for oxides stressed at lower voltages. For 2 nm oxides, an increase in gate noise is the only precise indication of soft breakdown. For many applications, devices should remain functional with the level of gate noise we have observed, after soft breakdown.

Abstract:Objectives: This study assessed the effect of liraglutide as a monotherapy and add-on to metformin on weight loss and BMI, among patients with Type 2 diabetes (T2D) who are overweight or obese. Methods: The following databases were assessed to identify relevant papers published until July 2023: Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE (PubMed), clinicaltrial.gov, and Web of Science. All clinical trials evaluating the effect of liraglutide on weight loss and BMI in patients with T2D who are obese or overweight, treated for at least 2 months, were included in the review. All analysis and risk of bias assessment was done using Cochrane Review Manager software, version 5.4.1 (Cochrane, London, UK). A random-effects model with inverse variance was used to synthesise the results. Results: In total, 10 randomised controlled trials involving 945 participants were included in the meta-analysis. Treatment with liraglutide with or without metformin for more than 2 months led to a significant weight loss (mean difference: -4.75 kg; 95% confidence interval: -7.02–-2.48; p<0.01). Liraglutide supplementation also led to a significant decrease in BMI (mean difference: -2.07; 95% confidence interval: -2.75–-1.39; p<0.01). However, the decrease in weight and BMI was not statistically significant as compared to treatment with other oral hypoglycaemic drugs or placebo. Conclusion: Liraglutide used alone or as adjunctive therapy to metformin produces reduction in weight and BMI when administered in adult patients with T2D who are obese or overweight.

Abstract:Background: Combining oral anti-hyperglycaemic agents with different mechanism of action produces additive effects, allowing the use of submaximal doses of the agents, thereby reducing the side effects and increasing the complementary benets on cardiovascular risk factors. Objectives: To compare the efcacy and safety of triple drug combination of Voglibose 0.2 mg, Glimepiride 1mg/2mg and Metformin SR 500 mg in type 2 diabetes mellitus (T2DM) patients, inadequately controlled with dual therapy. Methodology: This was an open labelled, nonrandomized, multi-centric, phase IV clinical study where the subjects were divided into two groups: GM V Arm, subjects were given xed dose 1 combination of Voglibose (0.2 mg) + Glimepiride (1 mg) + Metformin SR (500 mg) and GM VArm,subjects were given xed dose combination of 2 Voglibose (0.2 mg) + Glimepiride (2 mg) + Metformin SR (500 mg) twice daily. The primary efcacy endpoint was the change in HbA1c (%) from baseline to end of the study. At 12 weeks, voglibose reduced HbA1c to 6.88 ± 0.85 in GM VArm and to 7.40 ± 1.24 in GM VArm, which Results: 1 2 was statistically signicant (P<0.001). The PPG reductions in both the Arms were signicant compared to baseline, but no statistically signicant changes in body weight, TC, HDL, LDL, VLDL, TG was observed. The reduction in the FPG at week 12 was signicant (P<0.001) in both GM1V Arm and GM VArm. The most common adverse event was fever in 2 subjects (1.98%) and headache, dryness of palms and vomiting in one subject 2 (0.99%). Conclusion: The combination of Voglibose (0.2 mg) + Glimepiride (1/2 mg) +Metformin SR (500 mg) is effective, safe and well tolerated, thus this is an important armamentarium in the management of patients with uncontrolled T2DM.

Abstract:Objective: Although the efficacy and safety of vildagliptin have been proven in randomized clinical trials, data regarding the improved and up-to-date understanding on the management of Type 2 diabetes mellitus from the context of Indian diabetic patients, are scarce. This study was conducted to assess the opinions of clinical experts on the current usage patterns of vildagliptin in the management of Type 2 diabetes among Indian patients. Methods: A cross-sectional, questionnaire-based study was conducted to collect opinion among physicians in endocrinology across India between June 2022 and December 2022. Convenient sampling method was used. Descriptive statistics were used to summarize the characteristics of the study by employing frequencies and percentages. Results: It was observed that 95 out of 188 clinicians preferred dipeptidyl peptidase-4 (DPP4) inhibitors (DPP4i) and only 35 out of 188 clinicians (18.6%) opted a combination of Metformin and DPP4 inhibitors as the first hypoglycemic agent for newly detected diabetes. In addition, 141 out of 188 (75%) prescribed vildagliptin only as their preferred drug in the class of DPP4 inhibitors. Further, the reason behind such a high-yielding response for vildagliptin among clinicians and their patients, was its weight-neutral property primarily and posing a low risk of adverse effects. Nearly, half of the clinicians reported a 0.6–1% drop in glycosylated hemoglobin (HbA1c) levels in patients within 3 months of vildagliptin use. Conclusion: This study concluded that the majority of the specialists preferred using vildagliptin among other DPP4 inhibitors and pointed out that vildagliptin lowered HbA1c within 3 months of use.
Efficacy and Safety of a Fixed-Dose Combination of Vildagliptin and Pioglitazone in Indian Patients With Type 2 Diabetes Mellitus: A Randomized, Open-Label, Comparative, Phase III Study

Abstract:Background Type 2 diabetes mellitus (T2DM) arises due to a range of pathological abnormalities, necessitating a combination therapy to achieve optimal glycemic control. Vildagliptin, an effective and selective DPP-4 inhibitor, and pioglitazone, an insulin sensitizer, offer distinct mechanisms of action. Hence, the integration of these medications represents a logical and justified therapeutic strategy Objective To compare the efficacy, safety, and tolerability of vildagliptin and pioglitazone 50 mg/15 mg fixed-dose combination (FDC) tablets with individual monotherapy vildagliptin 50 mg and pioglitazone 15 mg tablets in Indian T2DM patients who were inadequately controlled on metformin monotherapy. Methods This was a randomized, open-label, comparative, multicenter, phase III study involving 195 T2DM patients with inadequate glycemic control on metformin ≥ 1000 mg/day. Patients were randomly assigned in a 1:1:1 ratio to the test product group (n=65) (vildagliptin 50 mg + pioglitazone 15 mg FDC tablets), reference product group 1 (n=65) (vildagliptin 50 mg tablet), or reference product group 2 (n=65) (pioglitazone 15 mg tablet reference product). The primary endpoint was the mean change in HbA1c levels from baseline to end of the study visit (12 weeks (84 days ±2)). The secondary endpoints were the mean change in fasting plasma glucose (FPG) and 2-hr postprandial plasma glucose (2-hr PPG) levels. Safety parameters were assessed till the end of the study. Results A total of 178 patients completed the study. At 12 weeks, the mean HbA1c level in the test group reduced to 6.85 ± 1.27%, in the reference product 1 group to 7.56 ± 1.72%, and in the reference product 2 groups to 7.37 ± 1.59%. The mean change in Hb1Ac from baseline in the test group was statistically significant compared to the reference groups (p=0.037). Similarly, the mean changes in the FPG and 2hr-PPG with the test product were statistically significant compared to reference products (p=0.041). The adverse events were comparable across all the treatment groups. Conclusion In Indian T2DM patients inadequately controlled on a daily maximum dose of metformin, treatment with vildagliptin and pioglitazone FDC showed better glycemic control than either vildagliptin or pioglitazone along with a good tolerability profile.
Vitamin D Supplementation in Overweight/obese Asian Indian Women with Prediabetes Reduces Glycemic Measures and Truncal Subcutaneous Fat: A 78 Weeks Randomized Placebo-Controlled Trial (PREVENT-WIN Trial)

Abstract:Vitamin D deficiency may contribute to etiology of type 2 diabetes in Asian Indians. The objectives of this study was to evaluate effect of vitamin D supplementation on glycemic profile and body composition in prediabetic and vitamin D deficient overweight/obese Asian Indian women. In this open-label randomized placebo-controlled trial (78 weeks duration), 121 females (aged 20-60 years) with prediabetes and vitamin D deficiency were randomly allocated in intervention (n, 61) and placebo (n, 60) groups. The primary outcome variables were fasting blood glucose (FBG), 2-h blood glucose post OGTT (2-h BG), hemoglobin A1c (HbA1C), and reversal to normoglycemia. In Intention-to-treat analysis, at the end of intervention, we observed significant decrease in FBG [-5.0 (-12.6-2.4), p = 0.04], 2-h blood glucose post OGTT [-11(-49.3-26.9), p = 0.02], hemoglobin A1c [-0.41 (5.89, 6.55), p = 0.05] and increase in 25(OH) D [7.5 (-6.0-20.9), p = 0.002] levels in intervention as compared to the placebo group. Changes in glycemic category based on FBG were as follows; intervention group: normal FBG, 58.6%; impaired fasting glucose (IFG), 39%; and type 2 diabetes mellitus (T2DM), 2.4%; placebo group: normal FBG, 48.8%; IFG, 46.3%; and T2DM, 4.9%. Changes in category of 2-hour glucose post OGTT after intervention were as follows; intervention group: normal glucose tolerance (NGT) 51.2% and prediabetes, 48.8%; placebo group: NGT, 43.9%; prediabetes, 53.7% and T2DM, 2.4%. After intervention, subscapular skinfold (visit Ist compared to visit IIIrd) and suprailiac skinfold (visit IInd compared to visit IIIrd) were significantly lower in intervention group vs. control group. In conclusion, we observed significant reduction in FBG, 2-hour glucose post OGTT, HbA1c, and truncal subcutaneous fat and reversal to normoglycemia in overweight/obese prediabetic vitamin D deficient Asian Indian women after 78 weeks of vitamin D supplementation.
Assessment of Clinical Experience of Triple Drug Fixed-dose Combination of Glimepiride, Metformin and Voglibose in the Management of Type 2 Diabetes Mellitus: A Retrospective Study

Abstract:Introduction: Poor adherence of patients to the polypharmacy approach is a crucial challenge in the management of Type 2 Diabetes Mellitus (T2DM) and use of triple Fixed-Dose Combination (FDC) of metformin, glimepiride, and voglibose is effective in achieving glycaemic control and would aid in improved drug adherence. Aim: To analyse clinical profile and treatment patterns of FDC of glimepiride, metformin, and voglibose with or without other antidiabetic therapy in patients with T2DM. Materials and Methods: A retrospective, observational, multi- centric study conducted during August 2019 to March 2020. Included patients of either sex, ≥18 years of age with T2DM and who had received treatment with FDC of glimepiride, metformin, and voglibose of varying strengths with or without other antidiabetic therapy. Data extracted from medical records included demographic characteristics, duration of disease, co- morbidities, concomitant medications and dosage pattern. Data were analysed using Chi-square test and Mann-Whitney U test. Results: A total of 2650 patients with T2DM were included, of which 1689 (63.7%) were males. The mean (standard deviation {SD}) age was 54.2 (11.4) years. The average Body Mass Index (BMI) was 27.2 (4.3) kg/m2 and hypertension 1656 (62.5%) and dyslipidaemia 1109 (41.9%) were the most common co-morbidities. Dipeptidyl peptidase-4 inhibitors 908 (34.3%) and antihypertensives 1601 (60.4%) were the most common concomitant diabetic and non diabetic medications received, respectively. Glimepiride (2 mg)+metformin (500 mg)+voglibose (0.2 mg) FDC twice-a-day 878 (33.1%) was a common triple FDC. A total of 2449 (92.4%) patients were compliant and 2585 (97.9%) achieved glycaemic goal with triple FDC treatment. During the therapy, the majority of patients had decreased weight 1106 (67.2%). The mean Haemoglobin A1c (HbA1c) levels significantly decreased post-treatment (mean change 1.45%; p-value <0.001). Family history of diabetes mellitus, obesity, sedentary lifestyle were the most common risk factors and smoking being prevalent in males. Conclusion: Overall results demonstrate that triple FDC of glimepiride, metformin, and voglibose was effective in reducing HbA1c and weight and was well tolerated. Also, it improves compliance in Indian patients with T2DM.

Abstract:Managing complex disease like diabetes requires a systems and network medicine approach. Polyherbal formulations used in Indian Systems of Medicines (ISMs) like Ayurveda and their multi-targeted mode of action could be an effective way to bring out this systems biology pharmacology effect required for chronic diseases. These multi-component formulations are also served as sources for identifying novel ligands that interact with drug targets having systemic effects. The present study uses Varanadi Kashayam (VK), a polyherbal preparation used in Ayurveda clinical management of diabetes and its comorbidities, to demonstrate the systemic pharmacological action exerted by a formulation with a special focus on incretin modulation. In-vitro assays demonstrated the inhibitory potential of VA on α-glucosidase and DPP4 enzymes and adipogenesis. It also showed an increase in GLP-1 secretion from GLUTag cells. These observations were corroborated in the in-vivo high-fat fed model of SD rats where VA increased GLP-1 secretion, improved the oral glucose tolerance and reduced body weight. Bioinformatics and computational biology analysis further identified Chebulinic acid, Terchebin and Chebulagic acid as the three major molecules showing stable docking and interactions with DPP4 when compared to the standard drug Vildagliptin. Network pharmacology analysis of VA revealed the potential involvement of AGE-RAGE signalling, Insulin resistance, Insulin signalling and lipid and atherosclerosis pathway along with other important signalling like FOXO signalling, cAMP and PI3K-AKT signalling in exerting the anti-diabetic and anti-obesity action. The present work provided a biological insight into the multi-targeted mode of action of a poly-herbal formulation the possible pharmacological networking through which the formulation modulates diverse disease related targets.

Abstract:Background: The benefit of first-line use of sodium-dependent glucose transport 2 inhibitors (SGLT2i) and glucagon-like peptide-1 receptor agonists (GLP-1RAs) in type 2 diabetes mellitus (T2DM) with low risk of cardiovascular diseases are not clear. Methods: PubMed, EMBASE and Cochrane Library databases were searched to identify eligible randomized controlled trials. We used the odds ratio (OR) and mean difference (MD) and the corresponding 95% confidence interval (CI) to assess the dichotomous and continuous variable, respectively. Results: Thirteen studies involving 2,885 T2DM at low risk of cardiovascular diseases were included. Compared to placebo, first line use of SGLT2i significantly reduced glycosylated hemoglobin type A1C (HbA1c) (MD: -0.72), weight (MD: -1.32) and fasting plasma glucose (FPG) (MD: -27.05) levels. Compared with metformin, SGLT2i reduced body weight (MD: -1.50) and FPG (MD: -10.13) more effectively, with similar reduction for HbA1c (MD: -0.05). No significant increased safety adverse was found for SGLT2i, including nasopharyngitis (OR: 1.07), urinary tract infection (OR: 2.31), diarrhea (OR: 1.18) and hypoglycemia (OR: 1.06). GLP-1RAs significantly reduced HbA1c (MD: -1.13), weight (MD: -2.12) and FPG (MD: -31.44) levels as first-line therapy compared to placebo. GLP-1RAs significantly increased occurrence of diarrhea (OR: 2.18), hypoglycemia (OR: 3.10), vomiting (OR: 8.22), and nausea (OR: 4.41). Conclusion: First line use of SGLT2i and GLP-1RAs is effective in reducing HbA1c, weight, and FPG levels in T2DM patients at low risk for cardiovascular disease. SGLT2i may be superior to metformin in controlling body weight and FPG. GLP-1RAs may increase the occurrence of diarrhea, hypoglycemia, vomiting, and nausea. Abstract:Kidneys of rats treated with methylprednisolone show altered substrate requirements for sodium reabsorption when perfused in vitro. Such kidneys synthesize glucose from lactate at twice the rate of control. Optimum sodium reabsorption is not seen with glucose, which is normally the preferred substrate. Sodium reabsorption is restored toward normal by the combination of glucose and butyrate, by pyruvate, or by 3-mercaptopicolinate. All of these results point to a metabolic adaptation in the kidney; butyrate may improve sodium reabsorption by sparing glucose, pyruvate is a gluconeogenic precursor and an effective fuel of respiration, and 3-mercaptopicolinate is an inhibitor of gluconeogenesis. In kidneys from rats treated with methylprednisolone there is an increased requirement for metabolic energy because of the increased rate of gluconeogenesis. It is suggested that the availability of energy from glucose oxidation is limited in part by the diversion of pyruvate back to glucose. Under these special circumstances, gluconeogenesis competes with sodium reabsorption in the intact kidney.

Abstract:Lifestyle interventions to prevent type 2 diabetes in South Asians produced modest effects on weight and glycaemic control and will unlikely reduce diabetes incidence in this population. Diabetes remission trials could potentially be effective and acceptable, but further research is required to assess their success. Background People from South Asian heritage are at high risk of type 2 diabetes, but there are limited specific strategies to prevent and manage this condition. The aim was to assess the effectiveness of culturally bespoke lifestyle programmes in South Asians that target weight loss for the prevention or remission of type 2 diabetes mellitus (T2DM). Methods We performed a systematic review and meta‐analysis of intervention trials. PubMed, Scopus, MEDLINE (EBSCOhost), CINAHL, PsycINFO and CENTRAL were searched. Human intervention trials (randomised controlled trials and quasi‐experimental) investigating the effect of lifestyle interventions on the prevention and remission of T2DM in South Asians were included. Studies including participants at risk of T2DM (prevention trials) and having the disease (remission trials) with duration ≥12 weeks were eligible. For prevention trials, the primary outcome was change in weight (kg) from baseline; for remission trials, it was decrease in HbA1c to non‐diabetic levels (HbA1c ≤ 6.5%) without diabetes medications. Prevention trials were separated into (i) lifestyle modification advice and (ii) lifestyle modification advice including a supervised physical activity programme. Results Twenty‐four trials were eligible (21 prevention trials and 3 remission trials). In T2DM prevention trials involving only lifestyle modification advice, the mean postintervention difference in weight between intervention and control groups was −0.65 kg (95% confidence interval [CI]: −1.04, −0.26; p = 0.01). Lifestyle modification advice including a physical activity programme was associated with greater decreases in weight: −1.13 kg (95% CI: −2.04, −0.21; p = 0.02). Fasting blood glucose levels were slightly lower in intervention groups for both intervention subtypes, although there was no significant change in HbA1c levels or 2‐h plasma glucose levels. Diabetes remission trials showed potential acceptability but were limited in number and involved a small sample size, and some did not include a control group. Conclusions In South Asians, lifestyle interventions for prevention of T2DM offer only modest impacts on weight and glucose control and will unlikely reduce diabetes incidence. Alternative lifestyle interventions co‐designed with members of the communities and aimed at both prevention and remission of T2DM must be urgently considered.
