Abstract:Noncommunicable diseases (NCDs) have become the leading cause of death worldwide. NCDs' chronicity, hiddenness, and irreversibility make patients' disease self-awareness extremely important in disease control but hard to achieve. With an accumulation of electronic health record (EHR) data, it has become possible to predict NCDs early through machine learning approaches. However, EHR data from latent NCD patients are often irregularly sampled temporally, and the data sequences are short and imbalanced, which prevents researchers from fully and effectively using such data. Here, we outline the characteristics of typical short sequential data for NCD early prediction and emphasize the importance of using such data in machine learning schemes. We then propose a novel NCD early prediction method: the short sequential medical data-based early prediction method (SSEPM). The SSEPM network contains two stacked subnetworks for multilabel enhancement. In each subnetwork, long short-term memory (LSTM) and attention layers are implemented to extract both temporal and nontemporal embedded features. During training, with prior clinical knowledge of the NCD characteristics, a random connection (RC) process is proposed for data augmentation. Comparative experiments involving ten-fold cross-validation are performed with real-world medical data to predict 5 NCDs. The result shows that the SSEPM outperforms the state-of-the-art NCD early prediction algorithms and works well in dealing with short sequential data. The results also suggest that the direct use of short sequential data could be more effective than formatting datasets with temporal exclusion limitations.

Abstract:Predictive modeling with electronic health record (EHR) data is anticipated to drive personalized medicine and improve healthcare quality. Constructing predictive statistical models typically requires extraction of curated predictor variables from normalized EHR data, a labor-intensive process that discards the vast majority of information in each patient’s record. We propose a representation of patients’ entire raw EHR records based on the Fast Healthcare Interoperability Resources (FHIR) format. We demonstrate that deep learning methods using this representation are capable of accurately predicting multiple medical events from multiple centers without site-specific data harmonization. We validated our approach using de-identified EHR data from two US academic medical centers with 216,221 adult patients hospitalized for at least 24 h. In the sequential format we propose, this volume of EHR data unrolled into a total of 46,864,534,945 data points, including clinical notes. Deep learning models achieved high accuracy for tasks such as predicting: in-hospital mortality (area under the receiver operator curve [AUROC] across sites 0.93–0.94), 30-day unplanned readmission (AUROC 0.75–0.76), prolonged length of stay (AUROC 0.85–0.86), and all of a patient’s final discharge diagnoses (frequency-weighted AUROC 0.90). These models outperformed traditional, clinically-used predictive models in all cases. We believe that this approach can be used to create accurate and scalable predictions for a variety of clinical scenarios. In a case study of a particular prediction, we demonstrate that neural networks can be used to identify relevant information from the patient’s chart.

Abstract:In the healthcare sector, the application of deep learning technologies has revolutionized data analysis and disease forecasting. This is particularly evident in the field of diabetes, where the deep analysis of Electronic Health Records (EHR) has unlocked new opportunities for early detection and effective intervention strategies. Our research presents an innovative model that synergizes the capabilities of Bidirectional Long Short-Term Memory Networks-Conditional Random Field (BiLSTM-CRF) with a fusion of XGBoost and Logistic Regression. This model is designed to enhance the accuracy of diabetes risk prediction by conducting an in-depth analysis of electronic medical records data. The first phase of our approach involves employing BiLSTM-CRF to delve into the temporal characteristics and latent patterns present in EHR data. This method effectively uncovers the progression trends of diabetes, which are often hidden in the complex data structures of medical records. The second phase leverages the combined strength of XGBoost and Logistic Regression to classify these extracted features and evaluate associated risks. This dual approach facilitates a more nuanced and precise prediction of diabetes, outperforming traditional models, particularly in handling multifaceted and nonlinear medical datasets. Our research demonstrates a notable advancement in diabetes prediction over traditional methods, showcasing the effectiveness of our combined BiLSTM-CRF, XGBoost, and Logistic Regression model. This study highlights the value of data-driven strategies in clinical decision-making, equipping healthcare professionals with precise tools for early detection and intervention. By enabling personalized treatment and timely care, our approach signifies progress in incorporating advanced analytics in healthcare, potentially improving outcomes for diabetes and other chronic conditions.

Abstract:Early detection of preventable diseases is important for better disease management, improved inter-ventions, and more efficient health-care resource allocation. Various machine learning approacheshave been developed to utilize information in Electronic Health Record (EHR) for this task. Majorityof previous attempts, however, focus on structured fields and lose the vast amount of information inthe unstructured notes. In this work we propose a general multi-task framework for disease onsetprediction that combines both free-text medical notes and structured information. We compareperformance of different deep learning architectures including CNN, LSTM and hierarchical <a class="link-external link-http" href="http://models.In" rel="external noopener nofollow">this http URL</a> contrast to traditional text-based prediction models, our approach does not require disease specificfeature engineering, and can handle negations and numerical values that exist in the text. Ourresults on a cohort of about 1 million patients show that models using text outperform modelsusing just structured data, and that models capable of using numerical values and negations in thetext, in addition to the raw text, further improve performance. Additionally, we compare differentvisualization methods for medical professionals to interpret model predictions.

SCOPE: predicting future diagnoses in office visits using electronic health records

Abstract:We propose an interpretable and scalable model to predict likely diagnoses at an encounter based on past diagnoses and lab results. This model is intended to aid physicians in their interaction with the electronic health records (EHR). To accomplish this, we retrospectively collected and de-identified EHR data of 2,701,522 patients at Stanford Healthcare over a time period from January 2008 to December 2016. A population-based sample of patients comprising 524,198 individuals (44% M, 56% F) with multiple encounters with at least one frequently occurring diagnosis codes were chosen. A calibrated model was developed to predict ICD-10 diagnosis codes at an encounter based on the past diagnoses and lab results, using a binary relevance based multi-label modeling strategy. Logistic regression and random forests were tested as the base classifier, and several time windows were tested for aggregating the past diagnoses and labs. This modeling approach was compared to a recurrent neural network based deep learning method. The best model used random forest as the base classifier and integrated demographic features, diagnosis codes, and lab results. The best model was calibrated and its performance was comparable or better than existing methods in terms of various metrics, including a median AUROC of 0.904 (IQR [0.838, 0.954]) over 583 diseases. When predicting the first occurrence of a disease label for a patient, the median AUROC with the best model was 0.796 (IQR [0.737, 0.868]). Our modeling approach performed comparably as the tested deep learning method, outperforming it in terms of AUROC (p < 0.001) but underperforming in terms of AUPRC (p < 0.001). Interpreting the model showed that the model uses meaningful features and highlights many interesting associations among diagnoses and lab results. We conclude that the multi-label model performs comparably with RNN based deep learning model while offering simplicity and potentially superior interpretability. While the model was trained and validated on data obtained from a single institution, its simplicity, interpretability and performance makes it a promising candidate for deployment.

Abstract:Background: The cost of health care in many countries is increasing rapidly. There is a growing interest in using machine learning for predicting high health care utilizers for population health initiatives. Previous studies have focused on individuals who contribute to the highest financial burden. However, this group is small and represents a limited opportunity for long-term cost reduction. Objective: We developed a collection of models that predict future health care utilization at various thresholds. Methods: We utilized data from a multi-institutional diabetes database from the year 2019 to develop binary classification models. These models predict health care utilization in the subsequent year across 6 different outcomes: patients having a length of stay of ≥7, ≥14, and ≥30 days and emergency department attendance of ≥3, ≥5, and ≥10 visits. To address class imbalance, random and synthetic minority oversampling techniques were employed. The models were then applied to unseen data from 2020 and 2021 to predict health care utilization in the following year. A portfolio of performance metrics, with priority on area under the receiver operating characteristic curve, sensitivity, and positive predictive value, was used for comparison. Explainability analyses were conducted on the best performing models. Results: When trained with random oversampling, 4 models, that is, logistic regression, multivariate adaptive regression splines, boosted trees, and multilayer perceptron consistently achieved high area under the receiver operating characteristic curve (>0.80) and sensitivity (>0.60) across training-validation and test data sets. Correcting for class imbalance proved critical for model performance. Important predictors for all outcomes included age, number of emergency department visits in the present year, chronic kidney disease stage, inpatient bed days in the present year, and mean hemoglobin A1c levels. Explainability analyses using partial dependence plots demonstrated that for the best performing models, the learned patterns were consistent with real-world knowledge, thereby supporting the validity of the models. Conclusions: We successfully developed machine learning models capable of predicting high service level utilization with strong performance and valid explainability. These models can be integrated into wider diabetes-related population health initiatives.

Abstract:Urban living in modern large cities has significant adverse effects on health, increasing the risk of several chronic diseases. We focus on the two leading clusters of chronic disease, heart disease and diabetes, and develop data-driven methods to predict hospitalizations due to these conditions. We base these predictions on the patients' medical history, recent and more distant, as described in their Electronic Health Records (EHR). We formulate the prediction problem as a binary classification problem and consider a variety of machine learning methods, including kernelized and sparse Support Vector Machines (SVM), sparse logistic regression, and random forests. To strike a balance between accuracy and interpretability of the prediction, which is important in a medical setting, we propose two novel methods: <i>K</i>-LRT, a likelihood ratio test-based method, and a Joint Clustering and Classification (JCC) method which identifies hidden patient clusters and adapts classifiers to each cluster. We develop theoretical out-of-sample guarantees for the latter method. We validate our algorithms on large datasets from the Boston Medical Center, the largest safety-net hospital system in New England.

Exploring Long-Term Prediction of Type 2 Diabetes Microvascular Complications

Abstract:Electronic healthcare records (EHR) contain a huge wealth of data that can support the prediction of clinical outcomes. EHR data is often stored and analysed using clinical codes (ICD10, SNOMED), however these can differ across registries and healthcare providers. Integrating data across systems involves mapping between different clinical ontologies requiring domain expertise, and at times resulting in data loss. To overcome this, code-agnostic models have been proposed. We assess the effectiveness of a code-agnostic representation approach on the task of long-term microvascular complication prediction for individuals living with Type 2 Diabetes. Our method encodes individual EHRs as text using fine-tuned, pretrained clinical language models. Leveraging large-scale EHR data from the UK, we employ a multi-label approach to simultaneously predict the risk of microvascular complications across 1-, 5-, and 10-year windows. We demonstrate that a code-agnostic approach outperforms a code-based model and illustrate that performance is better with longer prediction windows but is biased to the first occurring complication. Overall, we highlight that context length is vitally important for model performance. This study highlights the possibility of including data from across different clinical ontologies and is a starting point for generalisable clinical models.

Abstract:Background: Electronic health records (EHRs) contain patients' health information over time, including possible early indicators of disease. However, the increasing amount of data hinders clinicians from using them. There is accumulating evidence suggesting that machine learning (ML) and deep learning (DL) can assist clinicians in analyzing these large-scale EHRs, as algorithms thrive on high volumes of data. Although ML has become well developed, studies mainly focus on engineering but lack medical outcomes. Objective: This study aims for a scoping review of the evidence on how the use of ML on longitudinal EHRs can support the early detection and prevention of disease. The medical insights and clinical benefits that have been generated were investigated by reviewing applications in a variety of diseases. Methods: This study was conducted according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. A literature search was performed in 2022 in collaboration with a medical information specialist in the following databases: PubMed, Embase, Web of Science Core Collection (Clarivate Analytics), and IEEE Xplore Digital Library and computer science bibliography. Studies were eligible when longitudinal EHRs were used that aimed for the early detection of disease via ML in a prevention context. Studies with a technical focus or using imaging or hospital admission data were beyond the scope of this review. Study screening and selection and data extraction were performed independently by 2 researchers. Results: In total, 20 studies were included, mainly published between 2018 and 2022. They showed that a variety of diseases could be detected or predicted, particularly diabetes; kidney diseases; diseases of the circulatory system; and mental, behavioral, and neurodevelopmental disorders. Demographics, symptoms, procedures, laboratory test results, diagnoses, medications, and BMI were frequently used EHR data in basic recurrent neural network or long short-term memory techniques. By developing and comparing ML and DL models, medical insights such as a high diagnostic performance, an earlier detection, the most important predictors, and additional health indicators were obtained. A clinical benefit that has been evaluated positively was preliminary screening. If these models are applied in practice, patients might also benefit from personalized health care and prevention, with practical benefits such as workload reduction and policy insights. Conclusions: Longitudinal EHRs proved to be helpful for support in health care. Current ML models on EHRs can support the detection of diseases in terms of accuracy and offer preliminary screening benefits. Regarding the prevention of diseases, ML and specifically DL models can accurately predict or detect diseases earlier than current clinical diagnoses. Adding personally responsible factors allows targeted prevention interventions. While ML models based on textual EHRs are still in the developmental stage, they have high potential to support clinicians and the health care system and improve patient outcomes.

Abstract:A novel dynamic HyperGraph‐based deep learning model is proposed for disease prediction (DHGL) here. The proposed DHGL model is evaluated on real‐world electronic health record datasets and it is demonstrated that it outperforms several state‐of‐the‐art baseline methods in terms of predictive accuracy. Electronic health record (EHR) data is crucial in providing comprehensive historical disease information for patients and is frequently utilized in health event prediction. However, current deep learning models that rely on EHR data encounter significant challenges. These include inadequate exploration of higher‐order relationships among diseases, a failure to capture dynamic relationships in existing relationship‐based disease prediction models, and insufficient utilization of patient symptom information. To address these limitations, a novel dynamic HyperGraph‐based deep learning model is introduced for disease prediction (DHGL) in this study. Initially, pertinent symptom information is extracted from patients to assign them with an initial embedding. Subsequently, sub‐hypergraphs are constructed to consider distinct patient cohorts rather than treating them as isolated entities. Ultimately, these hypergraphs are dynamized to gain a more nuanced understanding of patient relationships. The evaluation of DHGL on real‐world EHR datasets reveals its superiority over several state‐of‐the‐art baseline methods in terms of predictive accuracy.

Abstract:Augmentation of disease diagnosis and decision-making in healthcare with machine learning algorithms is gaining much impetus in recent years. In particular, in the current epidemiological situation caused by COVID-19 pandemic, swift and accurate prediction of disease diagnosis with machine learning algorithms could facilitate identification and care of vulnerable clusters of population, such as those having multi-morbidity conditions. In order to build a useful disease diagnosis prediction system, advancement in both data representation and development of machine learning architectures are imperative. First, with respect to data collection and representation, we face severe problems due to multitude of formats and lack of coherency prevalent in Electronic Health Records (EHRs). This causes hindrance in extraction of valuable information contained in EHRs. Currently, no universal global data standard has been established. As a useful solution, we develop and publish a Python package to transform public health dataset into an easy to access universal format. This data transformation to an international health data format facilitates researchers to easily combine EHR datasets with clinical datasets of diverse formats. Second, machine learning algorithms that predict multiple disease diagnosis categories simultaneously remain underdeveloped. We propose two novel model architectures in this regard. First, DeepObserver, which uses structured numerical data to predict the diagnosis categories and second, ClinicalBERT_Multi, that incorporates rich information available in clinical notes via natural language processing methods and also provides interpretable visualizations to medical practitioners. We show that both models can predict multiple diagnoses simultaneously with high accuracy.

Abstract:Background Deep learning models have attracted significant interest from health care researchers during the last few decades. There have been many studies that apply deep learning to medical applications and achieve promising results. However, there are three limitations to the existing models: (1) most clinicians are unable to interpret the results from the existing models, (2) existing models cannot incorporate complicated medical domain knowledge (eg, a disease causes another disease), and (3) most existing models lack visual exploration and interaction. Both the electronic health record (EHR) data set and the deep model results are complex and abstract, which impedes clinicians from exploring and communicating with the model directly. Objective The objective of this study is to develop an interpretable and accurate risk prediction model as well as an interactive clinical prediction system to support EHR data exploration, knowledge graph demonstration, and model interpretation. Methods A domain-knowledge–guided recurrent neural network (DG-RNN) model is proposed to predict clinical risks. The model takes medical event sequences as input and incorporates medical domain knowledge by attending to a subgraph of the whole medical knowledge graph. A global pooling operation and a fully connected layer are used to output the clinical outcomes. The middle results and the parameters of the fully connected layer are helpful in identifying which medical events cause clinical risks. DG-Viz is also designed to support EHR data exploration, knowledge graph demonstration, and model interpretation. Results We conducted both risk prediction experiments and a case study on a real-world data set. A total of 554 patients with heart failure and 1662 control patients without heart failure were selected from the data set. The experimental results show that the proposed DG-RNN outperforms the state-of-the-art approaches by approximately 1.5%. The case study demonstrates how our medical physician collaborator can effectively explore the data and interpret the prediction results using DG-Viz. Conclusions In this study, we present DG-Viz, an interactive clinical prediction system, which brings together the power of deep learning (ie, a DG-RNN–based model) and visual analytics to predict clinical risks and visually interpret the EHR prediction results. Experimental results and a case study on heart failure risk prediction tasks demonstrate the effectiveness and usefulness of the DG-Viz system. This study will pave the way for interactive, interpretable, and accurate clinical risk predictions.

Abstract:Early prediction of patient outcomes is important for targeting preventive care. This protocol describes a practical workflow for developing deep-learning risk models that can predict various clinical and operational outcomes from structured electronic health record (EHR) data. The protocol comprises five main stages: formal problem definition, data pre-processing, architecture selection, calibration and uncertainty, and generalizability evaluation. We have applied the workflow to four endpoints (acute kidney injury, mortality, length of stay and 30-day hospital readmission). The workflow can enable continuous (e.g., triggered every 6 h) and static (e.g., triggered at 24 h after admission) predictions. We also provide an open-source codebase that illustrates some key principles in EHR modeling. This protocol can be used by interdisciplinary teams with programming and clinical expertise to build deep-learning prediction models with alternate data sources and prediction tasks.

Abstract:Multimodal healthcare data provides a huge opportunity for big-data-based disease prediction, supporting the diagnosis and treatment decision-making process for doctors. Many deep learning based methods are developed to yield better performance of multimodal data based disease prediction considering their powerful representation abilities. However, the black-box nature of deep learning methods results in many serious concerns: e.g. the reliability of the prediction performance is questionable; the end-users (i.e., doctors) can not understand the reasons behind the prediction. These issues make it difficult for deep learning based disease prediction systems to apply in practice. Therefore, we aim to tackle the aforementioned challenges and propose an explainable hierarchical association regularized deep learning method to produce interpretable prediction results while maintaining prediction accuracy. The method takes the multimodal healthcare data into consideration for the disease prediction task and constructs a hierarchical association path for each sample. An ingenious loss function is designed to learn consistent features among different data modalities and the disease prediction path with a hierarchical structure. The experimental results based on a public dataset show the superiority of our proposed method. The ablation study and sensitivity analysis verify the effectiveness and necessity of the method design.

Abstract:Leveraging health administrative data (HAD) datasets for predicting the risk of chronic diseases including diabetes has gained a lot of attention in the machine learning community recently. In this paper, we use the largest health records datasets of patients in Ontario,Canada. Provided by the Institute of Clinical Evaluative Sciences (ICES), this database is age, gender and ethnicity-diverse. The datasets include demographics, lab measurements,drug benefits, healthcare system interactions, ambulatory and hospitalizations records. We perform one of the first large-scale machine learning studies with this data to study the task of predicting diabetes in a range of 1-10 years ahead, which requires no additional screening of individuals.In the best setup, we reach a test AUC of 80.3 with a single-model trained on an observation window of 5 years with a one-year buffer using all datasets. A subset of top 15 features alone (out of a total of 963) could provide a test AUC of 79.1. In this paper, we provide extensive machine learning model performance and feature contribution analysis, which enables us to narrow down to the most important features useful for diabetes forecasting. Examples include chronic conditions such as asthma and hypertension, lab results, diagnostic codes in insurance claims, age and geographical information.

Abstract:Managing patients with chronic diseases is a major and growing healthcare challenge in several countries. A chronic condition, such as diabetes, is an illness that lasts a long time and does not go away, and often leads to the patient's health gradually getting worse. While recent works involve raw electronic health record (EHR) from hospitals, this work uses only financial records from health plan providers (medical claims) to predict diabetes disease evolution with a self-attentive recurrent neural network. The use of financial data is due to the possibility of being an interface to international standards, as the records standard encodes medical procedures. The main goal was to assess high risk diabetics, so we predict records related to diabetes acute complications such as amputations and debridements, revascularization and hemodialysis. Our work succeeds to anticipate complications between 60 to 240 days with an area under ROC curve ranging from 0.81 to 0.94. In this paper we describe the first half of a work-in-progress developed within a health plan provider with ROC curve ranging from 0.81 to 0.83. This assessment will give healthcare providers the chance to intervene earlier and head off hospitalizations. We are aiming to deliver personalized predictions and personalized recommendations to individual patients, with the goal of improving outcomes and reducing costs

Abstract:Traditional Machine Learning (ML) methods face unique challenges when applied to healthcare predictive analytics. The high-dimensional nature of healthcare data necessitates labor-intensive and time-consuming processes when selecting an appropriate set of features for each new task. Furthermore, ML methods depend heavily on feature engineering to capture the sequential nature of patient data, oftentimes failing to adequately leverage the temporal patterns of medical events and their dependencies. In contrast, recent Deep Learning (DL) methods have shown promising performance for various healthcare prediction tasks by specifically addressing the high-dimensional and temporal challenges of medical data. DL techniques excel at learning useful representations of medical concepts and patient clinical data as well as their nonlinear interactions from high-dimensional raw or minimally-processed healthcare data. In this paper we systematically reviewed research works that focused on advancing deep neural networks to leverage patient structured time series data for healthcare prediction tasks. To identify relevant studies, we searched MEDLINE, IEEE, Scopus, and ACM digital library for relevant publications through November 4 th , 2021. Overall, we found that researchers have contributed to deep time series prediction literature in ten identifiable research streams: DL models, missing value handling, addressing temporal irregularity, patient representation, static data inclusion, attention mechanisms, interpretation, incorporation of medical ontologies, learning strategies, and scalability. This study summarizes research insights from these literature streams, identifies several critical research gaps, and suggests future research opportunities for DL applications using patient time series data.
Hi-Le and HiTCLe: Ensemble Learning Approaches for Early Diabetes Detection Using Deep Learning and Explainable Artificial Intelligence

Abstract:Diabetes is a metabolic disease caused by the body's failure to use insulin or break down meals correctly. Every year, an alarming number of new cases of diabetes are recorded. A poor lifestyle and an unfavorable environment are the two main causes of diabetes. If it is not treated at early stages, it becomes a lifelong disease and further leads to failure of important organs such as the kidneys, heart, eyes, and so on. This danger can be decreased with timely and precise identification. Deep Learning (DL) is the best method for illness prediction, as demonstrated by recent developments in DL for clinical use. We have proposed two ensemble learning approaches: blending and hybrid by using the Diabetes Prediction Dataset (DPD), which is a highly imbalanced dataset. The number of diabetic patients in it are 8500 whereas, the number of non-diabetic individuals are 91500. To overcome the class imbalance problem, a Proximity-Weighted Synthetic Oversampling (ProWSyn) technique is implemented. We have proposed a hybrid of highway and LeNet model, named Hi-Le, for early and accurate diabetes detection. Hi-Le model achieves an accuracy of 94%, a F1-Score of 96%, precision score of 94% and recall of 95% and beats its individual models in terms of accuracy, F1-Score, precision and recall. We have also proposed a blending model named HiTCLe using Highway, LeNet, and a Temporal Convolutional Network (TCN) to detect and predict diabetes at an early stage. HiTCLe performs best, beats its individual models, highway, TCN and LeNet, and achieves an accuracy score of 94% and a F1-Score of 94%, whereas individual models achieve an accuracy score between 89% and 91% on 10 epochs. To validate models' results, we have implemented K-Fold Cross Validation (K-FCV). Also, to know the features contributions, we have implemented Shapley Additive eXplanations (SHAP) post processing technique. Both ensemble learning models outperform their individual models in term of accurate diabetes detection and prediction.
