Abstract:Inadequate persistence of tumor-infiltrating natural killer (NK) cells is associated with poor prognosis in cancer patients. The solid tumor microenvironment is characterized by the presence of immunosuppressive factors, including prostaglandin E2 (PGE2), that limit NK cell persistence. Here, we investigate if the modulation of the cytokine environment in lung cancer with IL-2 or IL-15 renders NK cells resistant to suppression by PGE2. Analyzing Cancer Genome Atlas (TCGA) data, we found that high NK cell gene signatures correlate with significantly improved overall survival in patients with high levels of the prostaglandin E synthase (PTGES). In vitro, IL-15, in contrast to IL-2, enriches for CD25+ /CD54+ NK cells with superior mTOR activity and increased expression of the cAMP hydrolyzing enzyme phosphodiesterase 4A (PDE4A). Consequently, this distinct population of NK cells maintains their function in the presence of PGE2 and shows an increased ability to infiltrate lung adenocarcinoma tumors in vitro and in vivo. Thus, strategies to enrich CD25+ /CD54+ NK cells for adoptive cell therapy should be considered.
Comprehensive Analysis of the Tumour Immune Microenvironment in Canine Urothelial Carcinoma Reveals Immunosuppressive Mechanisms Induced by the COX-Prostanoid Cascade

Abstract:A comprehensive understanding of the tumour immune microenvironment (TIME) is essential for advancing precision medicine and identifying potential therapeutic targets. This study focused on canine urothelial carcinoma (cUC) recognised for its high sensitivity to cyclooxygenase (COX) inhibitors. Using immunohistochemical techniques, we quantified the infiltration of seven immune cell populations within cUC tumour tissue to identify clinicopathological features that characterise the TIME in cUC. Our results revealed several notable factors, including the significantly higher levels of CD3+ T cells and CD8+ T cells within tumour cell nests in cases treated with preoperative COX inhibitors compared to untreated cases. Based on the immunohistochemistry data, we further performed a comparative analysis using publicly available RNA-seq data from untreated cUC tissues (n = 29) and normal bladder tissues (n = 4) to explore the link between COX-prostanoid pathways and the immune response to tumours. We observed increased expression of COX-2, microsomal prostaglandin E2 synthase-1 (mPGES-1) and mPGES-2 in cUC tissues. However, only mPGES-2 showed a negative correlation with the cytotoxic T-cell (CTL)-related genes CD8A and granzyme B (GZMB). In addition, a broader analysis of solid tumours using The Cancer Genome Atlas (TCGA) database revealed similar patterns in several human tumours, suggesting a common mechanism in dogs and humans. Our results suggest that the COX-2/mPGES-2 pathway may act as a cross-species tumour-intrinsic factor that weakens anti-tumour immunity, and that COX inhibitors may convert TIME from a 'cold tumour' to a 'hot tumour' state by counteracting COX/mPGES-2-mediated immunosuppression.
Neutrophils as key regulators of tumor immunity that restrict immune checkpoint blockade in liver cancer

Abstract:Objective: Liver cancer is a deadly malignancy associated with high mortality and morbidity. Less than 20% of patients with advanced liver cancer respond to a single anti-PD-1 treatment. The high heterogeneity of neutrophils in the tumor immune microenvironment in liver cancer may contribute to resistance to immune checkpoint blockade (ICB). However, the underlying mechanism remains largely unknown. Methods: We established an orthotopic liver cancer model by using transposable elements to integrate the oncogenes Myc and KrasG12D into the genome in liver cells from conditional Trp53 null/null mice (pTMK/Trp53−/−). Flow cytometry and immunohistochemistry were used to assess the changes in immune cells in the tumor microenvironment. An ex vivo coculture assay was performed to test the inhibitory effects of tumor-associated neutrophils (TANs) on CD8+ T cells. The roles of neutrophils, T cells, and NK cells were validated through antibody-mediated depletion. The efficacy of the combination of neutrophil depletion and ICB was evaluated. Results: Orthotropic pTMK/Trp53−/− mouse liver tumors displayed a moderate response to anti-Ly6G treatment but not PD-1 blockade. Depletion of neutrophils increased the infiltration of CD8+ T cells and decreased the number of exhausted T cells in the tumor microenvironment. Furthermore, depletion of either CD8+ T or NK cells abrogated the antitumor efficacy of anti-Ly6G treatment. Moreover, the combination of anti-Ly6G with anti-PD-L1 enhanced the infiltration of cytotoxic CD8+ T cells and thereafter resulted in a significantly greater decrease in tumor burden. Conclusions: Our data suggest that TANs may contribute to the resistance of liver cancer to ICB, and combining TAN depletion with T cell immunotherapy synergistically increases antitumor efficacy.

Negative Effects of Stromal Neutrophils on T Cells Reduce Survival in Resectable Urothelial Carcinoma of the Bladder

Abstract:Urothelial carcinoma of the bladder (UCB) is a major type of bladder cancer with a distinct tumor microenvironment (TME). Although neutrophils are the main component of myeloid cells in the TME, the clinical significance and function of the neutrophils remain unclear in UCB. Here, we observed CD66b+ neutrophils were predominantly enriched in the stroma of UCB tissues and their levels emerged as an independent prognostic factor for overall survival ( P = 0.006, n = 237), and were positively associated with age ( P = 0.033), tumor stage ( P < 0.0001), nodal metastasis ( P = 0.045), and histological grade ( P < 0.0001). Furthermore, we found that CD66b + neutrophils were frequently co-localized with CD4 + T cells (R=0.35, P = 0.0067), CD8 + T cells (R=0.52, P <0.0001) and Cleaved Caspase-3 + apoptosis cells (R=0.44, P = 0.0007) in the stroma of UCB tissue. In addition, better effects of T cells on patients' survival were markedly reduced by neutrophils and T cells co-infiltration. Moreover, we confirmed bladder tumor cell supernatant treated neutrophils suppressed T cell proliferation and activation, and promoted T cell apoptosis through GM-CSF induced PD-L1 in vitro . The expression of PD-L1 by neutrophils was also detected in fresh UCB tissues by using flow cytometric analysis. These data suggested that stromal CD66b+ neutrophils may potentially represent a reliable marker of poor prognosis for UCB patients, and neutrophils might play an immunosuppressive role on T cell immunity partially via the expression of PD-L1.

Synergistic antitumor activity of pan-PI3K inhibition and immune checkpoint blockade in bladder cancer

Abstract:Background Immune checkpoint blockade (ICB) induces durable response in approximately 20% of patients with advanced bladder urothelial cancer (aUC). Over 50% of aUCs harbor genomic alterations along the phosphoinositide 3-kinase (PI3K) pathway. The goal of this project was to determine the synergistic effects and mechanisms of action of PI3K inhibition and ICB combination in aUC. Methods Alterations affecting the PI3K pathway were examined in The Cancer Genome Atlas (TCGA) and the Cancer Dependency Map databases. Human and mouse cells with Pten deletion were used for in vitro studies. C57BL/6 mice carrying syngeneic tumors were used to determine in vivo activity, mechanisms of action and secondary resistance of pan-PI3K inhibition, ICB and combination. Results Alterations along the PI3K pathway occurred in 57% of aUCs in TCGA. CRISPR (clustered regularly interspaced short palindromic repeats) knockout of PIK3CA induced pronounced inhibition of cell proliferation (p=0.0046). PI3K inhibition suppressed cancer cell growth, migration and colony formation in vitro. Pan-PI3K inhibition, antiprogrammed death 1 (aPD1) therapy and combination improved the overall survival (OS) of syngeneic mice with PTEN-deleted tumors from 27 days of the control to 48, 37, and 65 days, respectively. In mice with tumors not containing a PI3K pathway alteration, OS was prolonged by the combination but not single treatments. Pan-PI3K inhibition significantly upregulated CD80, CD86, MHC-I, and MHC-II in dendritic cells, and downregulated the transforming growth factor beta pathway with a false discovery rate-adjusted q value of 0.001. Interferon alpha response was significantly upregulated with aPD1 therapy (q value: <0.001) and combination (q value: 0.027). Compared with the control, combination treatment increased CD8 + T-cell infiltration (p=0.005), decreased T reg -cell infiltration (p=0.036), and upregulated the expression of multiple immunostimulatory cytokines and granzyme B (p<0.01). Secondary resistance was associated with upregulation of the mammalian target of rapamycin (mTOR) pathway and multiple Sprr family genes. Conclusions The combination Pan-PI3K inhibition and ICB has significant antitumor effects in aUC with or without activated PI3K pathway and warrants further clinical investigation. This combination creates an immunostimulatory tumor milieu. Secondary resistance is associated with upregulation of the mTOR pathway and Sprr family genes.

Abstract:The colorectal-cancer (CRC) incidence rate and mortality have remained high for several years. In recent years, immune-checkpoint-inhibitor (ICI) therapy has rapidly developed. However, it is only effective in a few CRC patients with microsatellite-instability-high (MSI-H) or mismatch-repair-deficient (dMMR) CRC. How to improve the efficiency of ICI therapy in CRC patients with microsatellite stability (MSS) remains a huge obstacle. Tumor-associated neutrophils (TANs), which are similar to macrophages, also have N1 and N2 phenotypes. They can be recruited and polarized through different cytokines or chemokines, and then play an antitumor or tumor-promoting role. In CRC, we find that the prognostic significance of TANs is still controversial. In this review, we describe the antitumor regulation of TANs, and their mechanism of promoting tumor progression by boosting the transformation of inflammation into tumors, facilitating tumor-cell proliferation, metastasis and angiogenesis. The targeting of TANs combined with ICIs may be a new treatment model for CRC. Relevant animal experiments have shown good responses, and clinical trials have also been carried out in succession. TANs, as "assistants" of ICI treatment, may become the key to the success of CRC immunotherapy, although no significant results have been obtained.

Tumor-associated neutrophils activated by tumor-derived CCL20 (C-C motif chemokine ligand 20) promote T cell immunosuppression via programmed death-ligand 1 (PD-L1) in breast cancer

Abstract:Breast cancer is the leading cause of cancer-related death among women despite the significant improvement in diagnosis and treatment. Tumor-associated neutrophils have been shown to suppress antitumor functions of the host. However, how breast cancer tumor microenvironment influences the phenotype and functions of neutrophils to potentiate T cell immunosuppression is unknown. Herein, neutrophils isolated from peripheral blood of healthy donors were treated with supernatants from breast cancer cell lines or recombinant human CCL20. PD-L1 expression on neutrophils was then evaluated by immunofluorescence and flow cytometry. Neutrophils and Jurkat T cells were cocultured to evaluate the effect of tumor-associated neutrophils on T cell functions. Finally, immunohistochemical staining was performed to evaluate the clinical relevance of neutrophils infiltrating breast tumor tissues. Tumor-derived CCL20 activated and upregulated PD-L1 expression on neutrophils. A significant positive correlation was found between CCL20 and CD66b+ neutrophils in tumor tissues. Through in vitro experiment, tumor-associated neutrophils (TANs) effectively suppressed T cell immunity which was reversed upon PD-L1 blockade.Moreover, a high density of TANs was associated with short disease free survival in breast cancer patients. Furthermore, receiver operating curve showed that the density of TANs could accurately predict disease-free survival in breast cancer patients. Our findings suggest that targeting TANs via CCL20 immunosuppressive pathway may be a novel therapeutic strategy for breast cancer treatment.

Single-cell atlases link macrophages and CD8 + T-cell subpopulations to disease progression and immunotherapy response in urothelial carcinoma

Abstract:Rationale: Immune checkpoint inhibitors (ICIs) have revolutionized the management of locally advanced or metastatic urothelial carcinoma. Strikingly, compared to urothelial carcinoma of the bladder (UCB), upper tract urothelial carcinoma (UTUC) has a higher response rate to ICIs. The stratification of patients most likely to benefit from ICI therapy remains a major clinical challenge. Methods: In this study, we performed the first single-cell RNA sequencing (scRNA-seq) study of 13 surgical tissue specimens from 12 patients with UTUC. The key results were validated by the analysis of two independent cohorts with bulk RNA-seq data for UCB (n = 404) and UTUC (n = 158) and one cohort of patients with metastatic urothelial carcinoma (mUC) who were treated with atezolizumab (n = 348). Results: Using scRNA-seq, we observed a higher proportion of tumor-infiltrating immune cells in locally advanced UTUC. Similar prognostically relevant intrinsic basal and luminal-like epithelial subtypes were found in both UTUC and UCB, although UTUC is predominantly of the luminal subtype. We also discovered that immunosuppressive macrophages and exhausted T-cell subpopulations were enriched in the basal subtype and showed enhanced interactions. Furthermore, we developed a gene expression signature (Macro-C3 score) capturing the immunosuppressive macrophages that better predicts outcomes than the currently established subtypes. We also developed a computational method to model immune evasion, and the Macro-C3 score predicted therapeutic response of mUC treated with first-line anti-PD-L1 inhibitors in patients with lower basal scores. Conclusions: Overall, the distinct microenvironment and Macro-C3 score provide an explanation for ICI efficacy in urothelial carcinoma and reveal new candidate regulators of immune evasion, suggesting potential therapeutic targets for improving antitumor immunity in the basal subtype.

Abstract:Unresolved inflammation plays a critical role in bladder cancer initiation and progression. Acute inflammation leads to eicosanoid‐driven cytokine production. Controlling the local and systemic inflammatory response in bladder cancer is critical to preventing bladder cancer progression. Chemotherapy (e.g. gemcitabine and cisplatin), the standard of care for advanced bladder cancer, disrupts the resolution of inflammation and is only partially effective in preventing tumor recurrence after treatment. Although immunotherapy induces a durable response in 15 to 30% of patients in advanced bladder cancer, a majority of patients do not respond due to drug resistance mechanisms. Therefore, there is a critical unmet medical need to improve chemotherapy and immunotherapy in bladder cancer. To address this, we have developed dual COX‐2/sEH inhibitors (e.g. PTUPB). This lead compound is representative of a newly patented class of “dual acting” molecules that target two important enzymes in the arachidonic acid cascade with nanomolar binding affinity: cyclooxygenase‐2 (COX‐2) and soluble epoxide hydrolase (sEH). Oral dosing of PTUPB is well‐tolerated in animal models. Since chemotherapy and immunotherapy both induce tumor‐promoting inflammation via an eicosanoid and cytokine storm, we hypothesized that dual COX‐2/sEH inhibition using PTUPB would enhance immunotherapy in experimental bladder cancer via anti‐inflammatory mechanisms. When syngeneic (MB49) bladder tumors reached ~200 mm3 in immunocompetent mice, treatment was initiated with PTUPB (oral, 30‐60 mg/kg), anti‐CTLA‐4 (200 µg initial dose, 100 µg Q3D), anti‐PD1 (200 µg Q3D), gemcitabine (75 mg/kg Q3D), cisplatin (5 mg/kg Q5D), or various combinations thereof. After 9 days of treatment, mice were sacrificed and bladder tumor tissues were analyzed for sEH and COX‐2 expression, as well as endoplasmic reticulum (ER) stress. Here we demonstrate that chemotherapy stimulated sEH expression in tumor tissue which was counter‐regulated by PTUPB. Importantly, PTUPB enhanced immunotherapy in the MB49 bladder cancer model. While monotherapy treatment with PTUPB, gemcitabine/cisplatin, anti‐PD1 or anti‐CTLA4 suppressed tumor growth compared to control, the tumors eventually escaped by the end of the study. Dual COX‐2/sEH inhibition in combination with chemotherapy (gemcitabine and cisplatin) and/or immune checkpoint blockade (anti‐CTLA‐4 or anti‐PD1) induced tumor regression via synergistic anti‐tumor activities. Chemotherapy induced the expression of ER stress response genes (e.g. BiP and CHOP), COX‐2, and pro‐inflammatory cytokines in bladder cancer tissue which were repressed by PTUPB. Taken together, our results demonstrate dual COX‐2/sEH inhibition as a novel therapeutic approach to enhance immunotherapy in bladder cancer without overt toxicity.

Abstract:Abstract Maintenance of CTL-, Th1-, and NK cell-mediated type-1 immunity is essential for effective anti-tumor responses. Unexpectedly, we observed that IFNγ and TNFα, the critical soluble mediators produced by type-1 immune effector cells, synergize in the induction of cyclooxygenase 2 (COX2), the key enzyme in prostaglandin (PG)E2 synthesis, and the hyper-activation of myeloid-derived suppressor cells (MDSCs) within the tumor microenvironment (TME) of ovarian cancer patients. This MDSC hyper-activation by type-1 immune cells and the subsequent over-expression of indoleamine 2,3-dioxygenase (IDO), inducible nitric oxide synthase (iNOS/NOS2), IL-10, and additional COX2 result in strong feedback suppression of type-1 immune responses. Importantly, this self-limiting feedback could be eliminated by COX2 blockade, allowing amplification of type-1 immunity and prolonged survival of tumor bearing mice. Our data demonstrate a new mechanism underlying the self-limiting nature of type-1 immunity in the human TME and provide rationale for targeting the COX2-PGE2 axis to enhance the effectiveness of cancer immunotherapies.

Abstract:Regulatory T cells (Tregs) play a major role in the development of an immunosuppressive tumor microenvironment. Systemic Treg depletion is not favored because of the critical role of Tregs in maintaining immune homeostasis and preventing the autoimmunity. Recently, CCR8 has been identified as an important chemokine receptor expressed on intratumoral Tregs and is known to be critical for CCR8(+)Treg-mediated immunosuppression. However, the inherent molecular mechanisms and clinical significance of intratumoral CCR8(+)Tregs remain poorly understood. In this study, a retrospective analysis of 259 muscle-invasive bladder cancer (MIBC) patients from two independent clinic centers was conducted to explore the prognostic merit of CCR8(+)Tregs via immunohistochemistry. Eighty-three fresh MIBC samples and data from the Cancer Genome Atlas were used to evaluate the proportion and function of immune cells via flow cytometry, ex vivo intervention experiments and bioinformatics analysis. It was found that the CCR8 expression by intratumoral Tregs maintained the stability and potentiated their suppressive function by upregulating the expression of transcript factors FOXO1 and c-MAF. High level of CCR8(+)Tregs was associated with the immune tolerance and predicted poor survival and inferior therapeutic responsiveness to chemotherapy. Moreover, it was revealed that CCR8 blockade could destabilize intratumoral Tregs into a fragile phenotype accompanied with reactivation of antitumor immunity and augment of anti-PD-1 therapeutic benefits in MIBC. In summary, those results suggested that CCR8(+)Tregs represented a stable Treg subtype and a promising therapeutic target in the immunotherapy of MIBC.

METHODS: 259 MIBC patients from two independent clinic centers were included in the study. 83 fresh MIBC tumor tissues were used to evaluate the proportion and function of immune cells via flow cytometry and ex vivo intervention experiments. 396 MIBC patients of TCGA were applied for bioinformatics analysis. RESULTS: We found that the CCR8 expression by intratumoral Tregs maintained the stability and potentiated their suppressive function by upregulating the expression of transcript factors Foxo1 and c-Maf. High level of CCR8+ Tregs is associated with immune tolerance and predicts poor survival and inferior therapeutic responsiveness to chemotherapy. Moreover, we revealed that CCR8 blockade using neutralizing antibody destabilized intratumoral Tregs into a fragile phenotype, reactivated the antitumor immunity and augmented the therapeutic benefits of the anti-PD-1 antibody in MIBC. CD8+ T effector and immune checkpoint signatures predict prognosis and responsiveness to immunotherapy in bladder cancer

Abstract:Immune-checkpoint blockade (ICB) has been routinely implemented to treat bladder cancer; however, most patients have little or no clinical benefit. In this study, 348 pretreated metastatic urothelial cancer samples from the IMvigor210 cohort were used to identify important genes significantly associated with CD8+ T effector and immune checkpoint signatures. The immune checkpoint inhibitor score (IMS) scoring system was constructed to predict the immunotherapy responsiveness. Transcriptome analysis confirmed that the high IMS score group had significant immune activation with better prognosis and higher immunotherapy responsiveness, which was a powerful biomarker for predicting the prognosis and responsiveness of ICB. Tumor immune dysfunction and exclusion (TIDE) scores were calculated using 2031 external bladder cancer samples for further validation. We selected the important Hub genes as potential therapeutic targets, and validated the genes using genomic, transcriptomic, immunomic, and other multi-omics methods. In addition, we construct a risk prediction model which could stratify patients with bladder cancer and predict patient prognosis and ICB treatment responsiveness. In conclusion, this study identified effective biomarkers for the prediction of immune checkpoint inhibitor treatment responsiveness in bladder cancer patients and provided new immunotherapeutic targets.

Abstract:Abstract Cancer immunotherapy can elicit robust tumor-specific immune responses that can generate durable tumor regression or eradication. Immune checkpoint blockade specific to programmed death-1/ligand-1 (PD1/PDL1) have shown durable clinical responses in various cancer types but benefit only a small portion of patients. Interleukin 12 (IL-12) and granulocyte macrophage colony-stimulator factor (GM-CSF) have shown potent antitumor activities through activating innate and adaptive immunities, and reprogramming tumor-promoting myeloid cells from immune-suppressive to immune-active phenotypes. In this study, we showed the effect of radiation, which increased release of tumor antigens so that augment the efficacy of immunotherapy, in combination with IL-12/GM-CSF (RT/12GM) or anti-PDL1 antibody (RT/PDL1) on TAN in advanced 14-day-CT26 colorectoral cancer. We found that both combination treatment significantly suppressed tumor growth, only RT/12GM but not RT/PDL1 can cure mice; radiation or immunotherapy alone single therapies were much less effective. Mechanistic analysis showed that RT/12GM, but not RT/PDL1, greatly increased accumulation of TAN. These TANs were not only impaired in their suppressive activities, but also able to induce T cell proliferation, in accordance with their increased expressions of MHC class I and II, CD40 and CD86, indicating they presented in a status with higher Ag presentation ability. Antibody depletion experiments showed that TAN was required for tumor regression in RT/12GM therapy. Together, our data demonstrate that combination of radiotherapy and IL-12/GM-CSF improved antitumor immunity through enhancing Ag presentation activity and reducing immunosuppressive functions of TAN.

Abstract:Background: Immune checkpoint blockade (ICB) has emerged as a promising therapy for both resectable urothelial carcinoma (UC) patients preparing for radical surgery and unresectable UC patients, whereas the objective response rate of ICB remains unsatisfactory due to various factors. Antibiotic (ATB) use can influence intra-tumoral bacteria, which may further reduce ICB efficacy. The study aims to evaluate the effects of ATB use on prognosis and response in UC patients undergoing ICB, and explore potential molecular mechanisms of ATBs and intra-tumoral bacteria impacting UC immune microenvironment. Materials and methods: Pooled analyses, synthesizing evidence from 12 studies and 3496 UC patients with ICB treatment, was conducted via a meta-analysis. In addition, single-cell RNA and single-cell microbiome data were analyzed based on eight UC samples and 63185 single cells. Bulk RNA sequencing and clinical data from a single-arm, multi-center, atezolizumab-treated, phase 2 trial, IMvigor210, were used for validation. The study is registered at PROSPERO (XXX) and at Research Registry (XXX). Results: ATB use exhibited worse overall survival (HR=1.46, 95%CI=[1.20, 1.77], P<0.001, heterogeneity I²=51%) and lower objective response (OR=0.43, 95%CI=[0.27, 0.68], P<0.001, heterogeneity I²=0%) in UC patients receiving ICB. Single-cell transcriptome and single-cell microbiome analyses identified the presence of intra-tumoral bacteria was obviously related to elevated anti-bacterial immune functions; and anti-bacterial immunity was positively correlated to anti-tumor immunity in UC immune microenvironment. Intra-tumoral bacteria could up-regulate CD74-MIF/COPA signaling of immune cells and activation of CD74-MIF/COPA mediated the promotion of T cell anti-tumor function induced by anti-bacterial immune cells. UC patients with higher CD74-MIF/COPA signaling carried better overall survival (HR=1.60, 95%CI=[1.19, 2.15], P=0.002) in IMvigor210 immunotherapy cohort. Conclusion: ATB use reduces overall survival and objective response to ICB in UC patients. Anti-bacterial immune cell functions induced by intra-cellular bacteria in UC microenvironment might up-regulate the function of anti-tumor T immune cells via activating CD74-MIF/COPA, whereas ATB could inhibit the above process through killing intra-cellular bacteria and result in poorer clinical benefit of ICB. The use of ATB should be considered carefully during neoadjuvant immunotherapy period for resectable UC patients preparing for radical surgery and during immunotherapy period for unresectable UC patients.

Abstract:Background: Immunotherapy for upper tract urothelial carcinoma (UTUC) shows potential, but only a small subset of patients benefits from the treatment. This disadvantage can be attributed to additional immunosuppressive mechanisms in vivo. Indoleamine 2,3-dioxygenase 1 (IDO1) can be potentially used as an immune checkpoint target. We conducted this study to determine the prognostic role of IDO1 in UTUC and further construct an immune classifier, which can provide additional prognostic information and personalized treatment for UTUC.Methods: Two hundred and fifty-one tumor tissues of UTUC from three independent cohorts were analyzed. The expression of IDO1, programmed cell death protein 1-ligand 1 (PD-L1) and the infiltration of CD4+/CD8+/Foxp3+ T cells were evaluated using immunohistochemistry staining. Kaplan-Meier curves and Cox model were used for comparing the clinical outcomes. The Lasso Cox regression model was used to construct an IDO1-based immune classifier. Internal and external validation cohorts were included to validate the classifier. The immune contexture of different risk group stratified by the immune classifier were measured by RNA sequencing and immunofluorescence.Results: High IDO1 expression on tumor cells (TC) indicated a poorer overall survival and disease-free survival compared with low IDO1 expression in all cohorts. Patients with high expression of IDO1 TC possessed increased infiltration of Foxp3+ regulatory cells, CD8+ and CD4+ T cells. However, the distribution of IDO1 expression was irrelevant with that of PD-L1. An immune classifier based on intratumoral CD8+ lymphocytes, IDO1 TC and stromal PD-L1 expression status was developed, with its AUC values for OS at 5 years being 0.79 (95% CI 0.65-0.93) in the discovery cohort, 0.75 (95% CI 0.58-0.92) and 0.78 (95% CI 0.65-0.92) in the internal and external validation cohort, respectively. Abstract:Abstract Unresolved inflammation plays a critical role in bladder cancer initiation and progression. Chemotherapy (e.g. gemcitabine and cisplatin), the standard of care for advanced bladder cancer, disrupts inflammation resolution and is only partially effective in preventing tumor recurrence after treatment. Immunotherapy has emerged as a potential treatment option for bladder cancer patients, however it is ineffective in 80% of patients and can induce a pro-inflammatory cytokine storm. Therefore, there is a critical unmet medical need to improve chemotherapy and immunotherapy in bladder cancer. To address this, we have developed dual COX-2/sEH inhibitors (e.g. PTUPB). This lead compound is representative of a newly patented class of “dual acting” molecules that target two important enzymes in the arachidonic acid cascade with nanomolar binding affinity: cyclooxygenase-2 (COX-2) and soluble epoxide hydrolase (sEH). Oral dosing of PTUPB is well-tolerated in animal models. Since chemotherapy and immunotherapy both induce tumor-promoting inflammation via an eicosanoid/cytokine storm, we hypothesized that dual COX-2/sEH inhibition would enhance immunotherapy in experimental bladder cancer via anti-inflammatory mechanisms. When syngeneic (MB49) bladder tumors reached ~200 mm3 in immunocompetent mice, treatment was initiated with PTUPB, anti-CTLA-4, anti-PD1, gemcitabine & cisplatin, or various combinations thereof. Here we demonstrate that chemotherapy and/or immunotherapy stimulated sEH and COX-2 expression in tumor tissue which was counter-regulated by PTUPB. While monotherapy treatment with PTUPB, gemcitabine & cisplatin, anti-PD1 or anti-CTLA-4 suppressed tumor growth compared to control, the tumors escaped single treatments by the end of the study. Remarkably, dual COX-2/sEH inhibition in combination with chemotherapy (gemcitabine and cisplatin) and/or immune checkpoint blockade (anti-CTLA-4 or anti-PD1) induced sustained tumor regression via synergistic anti-tumor activities. Chemotherapy and/or immunotherapy induced the expression of ER stress response genes (e.g. BiP and CHOP) and angiogenic factors (e.g. EGF and VEGF-C) in bladder cancer tissue, which were repressed by PTUPB. PTUPB also prevented chemotherapy-induced toxicity and prolonged survival in an orthotopic MB49 tumor model. Taken together, our results demonstrate dual COX-2/sEH inhibition as a novel therapeutic approach to enhance immunotherapy in bladder cancer without overt toxicity. Programmed Death Ligand‐1 is Associated with Tumor Infiltrating Lymphocytes and Poorer Survival in Urothelial Cell Carcinoma of the Bladder

Abstract:Drugs blocking programmed death ligand‐1 (PD‐L1) have shown unprecedented activity in metastatic and unresectable bladder cancer. The purpose of the present study was to investigate the expression, clinical significance and association of PD‐L1 with tumor‐infiltrating lymphocytes (TIL) in resectable urothelial cell carcinoma of the bladder (UCB). In this retrospective study, 248 UCB patients who received radical cystectomy or transurethral resection were examined. Immunohistochemistry was used to evaluate PD‐L1 expression and stromal CD8+ TIL, Th1 orientation T cell (T‐bet+) and PD‐1+ TIL densities within the intratumoral regions and associated stromal regions. Of the 248 specimens, 23% showed PD‐L1 expression in tumor cells and 55% in tumor‐infiltrating immune cells. CD8+ TIL, T‐bet+ TIL and PD‐1+ TIL were distributed throughout the tumor tissues and were more frequently distributed in stromal regions than in intratumoral regions. PD‐L1+ tumor cells and PD‐L1+ immune cells were positively associated with aggressive clinical features (all P < .05). Both PD‐L1+ tumor cells and PD‐L1+ immune cells were associated with poorer recurrence‐free and overall survival (all P < .05). Multivariate analysis showed that PD‐L1+ immune cells were an independent prognostic factor for overall (P = .001) and recurrence‐free survival (P = .024). Notably, high stromal CD8+ TIL and PD‐1+ TIL density were associated with poorer overall survival (P = .031 and P = .001, respectively). In the stroma, CD8+ TIL density has strong positive association with PD‐L1+ immune cells and PD‐1+ TIL density (all P < .0001). These results suggested that an exhausted immune state occurred in the tumor stroma in UCB. Further clinical development of immune‐checkpoint inhibitors may be effective for resectable patients with UCB.

Abstract:Introduction: Bladder cancer (BLCA) is a highly malignant tumor of the urinary system, but the prognosis and survival rates have little improvement based on current therapeutic strategy. Immune checkpoint inhibitors (ICIs) therapy revolutionized the treatment of BLCA, but the clinical application of ICIs is limited by low response rate. Oxaliplatin (OXP), a second line chemotherapy drug for BLCA, may reshape the tumor immune microenvironment (TIME) via recruiting immune cells. Here, we conducted the study of oxaliplatin combined with anti-PD-1 inhibitor in BLCA mice models. Methods: The 6-8 weeks old female C57BL/6J mice were used to establish subcutaneous model of bladder tumor. After tumors developed, mice were given tail vein injections of PBS or oxaliplatin (2.5 mg/kg) and/or anti-PD-1 antibody (10 mg/kg). Tumor tissue samples and peripheral blood mononuclear cell (PBMC) were collected to systemically evaluate the efficiency and safety of combination OXP and anti-PD-1 inhibitor. The change of immune cells populations and the corresponding phenotypic diversity in TIME and PBMC were analysed by flow cytometry. Results: Tumor growth experiments clarified that the combination therapy was more efficient than medication alone. Flow cytometry analysis of tumor samples showed significant differences between untreated and treated mice. Oxaliplatin influences the TIME by increasing immune cells infiltration, including CD3 + T cells, CD4 + T cells, CD8 + T cells, dendritic cells (DC cells) and natural killer cells (NK cells). As for infiltrating cells, oxaliplatin upregulated the expression of CD134 and downregulated TIM-3 of CD4 + T cells, downregulated the PD-L1 expression of DC cells, which contributed to improve the anti-tumor effect and the treatment response of ICIs. Additionally, the evaluation of PBMC found that there were no significant changes in immune cell subsets and phenotypes, which validated the safety of the combination therapy. These results show the therapeutic potential for the combination of OXP and anti-PD-1 inhibitor in BLCA. Conclusion: OXP could increase the infiltration of immune cells in TIME to promote the anti-tumor activity of anti-PD-1 inhibitor. The present research provided an appropriate rationale of combination chemotherapy and immunotherapy therapy for BLCA.
