Abstract:Gastric cancer is a significant health problem owing to its high incidence and mortality rate. The risk factors for gastric cancer include both uncontrollable (e.g., age, sex, and genetic predisposition) and controllable factors (e.g., Helicobacter pylori infection, smoking, alcohol consumption, obesity, and high-salt diets). Although treatment of H. pylori infections has been implemented as a primary preventive measure, the risk of gastric cancer may persist, highlighting the need for additional preventive measures. This review discusses various risks and protective factors associated with gastric cancer, studies conducted on these factors, and chemoprevention. Smoking is a risk factor for this disease; thus, is not recommended and current smokers are encouraged to quit because cessation can reduce the risk of gastric cancer. Excessive alcohol intake has been reported to increase the risk of gastric cancer as has the consumption of high-salt foods that can damage the gastric mucosa. Additionally, increasing the intake of fruits and vegetables, known for their protective effects against gastric cancer, can aid in its prevention. Studies on chemopreventive agents, including nonsteroidal anti-inflammatory drugs (e.g., aspirin), statins, and metformin, have been reported; however, evidence of their effectiveness remains insufficient to recommend these agents as preventive treatments. Additional well-planned studies on preventive medications and dietary approaches are necessary.

Abstract:Effective prevention and early diagnostic strategies are the most important public health interventions in gastric cancer, which remains a common malignancy worldwide. Preventive strategies require identification and understanding of environmental risk factors that lead to carcinogenesis. Helicobacter pylori (H. pylori) is the primary carcinogen as this ancient bacterium has a complex ability to interact with its human host. Smoking and salt are strong independent risk factors for gastric cancer whereas alcohol is only a risk when it is heavily consumed. Red meat and high fat increase the risk of gastric cancer however fresh fruits, vegetables (allium family) and certain micronutrients (selenium, vitamin C) reduce the risk, with evidence lacking for fish, coffee and tea. Foods that inhibit H. pylori viability, colonization and infection may reduce cancer risk. Obesity is increasingly recognized as a contributory factor in gastric cardia carcinogenesis. Therefore, modest daily physical activities can be protective against cancer. Foundry workers are at risk for developing gastric cancer with dust iron being an important cause. Other risk factors include Epstein-Barr virus (EBV), possibly JC virus and radiation but the effects of these are likely to remain small.

Abstract:The development of sequencing technology has expanded our knowledge of the human gastric microbiome, which is now known to play a critical role in the maintenance of homeostasis, while alterations in microbial community composition can promote the development of gastric diseases. Recently, carcinogenic effects of gastric microbiome have received increased attention. Gastric cancer (GC) is one of the most common malignancies worldwide with a high mortality rate. Helicobacter pylori is a well-recognized risk factor for GC. More than half of the global population is infected with H. pylori , which can modulate the acidity of the stomach to alter the gastric microbiome profile, leading to H. pylori -associated diseases. Moreover, there is increasing evidence that bacteria other than H. pylori and their metabolites also contribute to gastric carcinogenesis. Therefore, clarifying the contribution of the gastric microbiome to the development and progression of GC can lead to improvements in prevention, diagnosis, and treatment. In this review, we discuss the current state of knowledge regarding changes in the microbial composition of the stomach caused by H. pylori infection, the carcinogenic effects of H. pylori and non- H. pylori bacteria in GC, as well as the potential therapeutic role of gastric microbiome in H. pylori infection and GC.

Abstract:Gastric cancer (GC) is a globally important disease. The discovery of Helicobacter pylori (H. pylori) demonstrated that the human stomach is not a sterile environment, and recent advances in molecular biology have led to the detection of large populations of microorganisms in the stomach. A growing number of studies have elucidated differences in the microbiota of patients at various stages of GC development. Evidence from insulin-gastrin transgenic (INS-GAS) and human gastric microbiota-transplanted mouse models have further demonstrated the potential causality of microbiota in the development of GC. To date, H. pylori is still thought to be the strongest risk factor for GC. H. pylori interacts with non-H. pylori commensals and affects the composition of the gastric microbiota. This review provides an overview of the relationship between the gastric microbiota and GC, including the mechanisms of microbe-associated carcinogenesis, the clinical value of the microbiota as a GC biomarker, and the potential of modulating the microbiota for GC prevention or therapy.

Abstract:Gastric cancer remains one of the most common and deadly cancers worldwide, especially among older males. Based on GLOBOCAN 2018 data, stomach cancer is the 5th most common neoplasm and the 3rd most deadly cancer, with an estimated 783,000 deaths in 2018. Gastric cancer incidence and mortality are highly variable by region and highly dependent on diet and Helicobacter pylori infection. While strides in preventing and treating H. pylori infection have decreased the overall incidence of gastric cancer, they have also contributed to an increase in the incidence of cardia gastric cancer, a rare subtype of the neoplasm that has grown 7-fold in the past decades. A better understanding of the etiology and risk factors of the disease can help reach a consensus in approaching H. pylori infection. Dietary modification, smoking cessation, and exercise hold promise in preventing gastric cancer, while genetic testing is enabling earlier diagnosis and thus greater survival.

Abstract:Gastric cancer (GC) is a global health problem, with more than 1 million people newly diagnosed with GC worldwide each year. GC is more prevalent in less developed countries than in more developed countries. About half of all GC cases worldwide occur in East Asia, notably China. Globally, overall incidence rates of GC are declining, which is potentially attributed to a decrease in Helicobacter pylori (H. pylori) infection and the use of refrigeration to preserve foods rather than salt. GC is a multifactorial disease, and its occurrence and development were impacted by environmental and genetic factors. H. pylori infection is the primary risk factor for GC, especially for non-cardia. The prognosis of GC is poor due to stages at the first diagnosis. The 5-year survival rate is less than 10% when patients are diagnosed at an advanced stage, but the rate is as high as 85% if patients are detected at an earlier stage. Endoscopic screening can potentially prevent GC by early diagnosis and early treatment and has been widely adopted in screening programs in East Asian countries, such as Japan and Korea. This review summarizes updated epidemiological aspects, risk factors, and prevention strategies of GC in recent years to help researchers determine the most effective intervention strategies for reducing risk of GC.

Abstract:Overall, the past century has seen a substantial decline in gastric cancer, attributable to decreases in risk factors such as H. pylori infection, tobacco smoking, and the intake of salt-preserved food. One potential preventive strategy for those at high risk is H. pylori eradication for infected subjects, but confirmation of this effect awaits longer follow-up. Obesity continues to advance and may cause increases in cardia cancer, particularly in Western populations, and careful monitoring of this outcome is warranted in both Western and Asian populations. These changes in gastric cancer epidemiology foreshadow a new era in gastric cancer control and warrant further monitoring of descriptive patterns and risk factors.

Abstract:Risks for gastric cancer in relation to diet and other environmental factors are receiving renewed attention. New developments include the emerging relationship between Helicobacter pylori infection and an increased risk for gastric cancer, and positive results of chemoprevention trials in decreasing gastric cancer risk with the use of beta-carotene and vitamin E supplements. Factors that may enhance risk include consumption of nitrites, nitrates, alcohol, and highly salted, pickled, fermented, or smoked foods. Other environmental factors which may promote cancer are H. pylori infection, inappropriate food storage, metal and cement dust exposure, and cigarette smoking. High intakes of fruits and vegetables or of antioxidants, such as beta-carotene, vitamin E, and vitamin C may decrease risk.

Abstract:Gastric cancer is the second most common fatal malignancy in the world. In China, gastric cancer is now the second most common malignancy, while in Hong Kong, the mortality rate ranked fourth among all cancers in 1995. Dietary factors in gastric carcinogenesis came mostly from case-control studies. N-Nitroso compounds from dietary sources such as preserved, smoked and salted foods were found to be associated with gastric cancer. ß-Carotene, selenium and α-tocopherol have been shown in an intervention study to be favourable in the reduction of stomach cancer mortality. Fruits and vegetables showed the most consistent results of inverse association with gastric cancer. Dietary salt intake in preserved or salted foods is also shown to be associated with gastric cancer. Tea drinking, especially green tea, has a protective effect against gastric cancer as shown in some studies. Prospective case-control studies of the association between Helicobacter pylori infection and the subsequent development of gastric cancer showed that the odds ratio ranged from 2.8 to 6.0. However, results of similar case-control studies in countries with a high frequency of gastric cancer are controversial. Infection with H. pylori leads to changes in the vitamin C content of gastric juice, reactive oxygen metabolites, epithelial cell proliferation and apoptosis. Recently, CagA-positive strains were found to be associated with gastric cancer and also duodenal ulcers. The exact role of H. pylori in gastric carcinogenesis is still under investigation. Large-scale intervention studies are underway to examine dietary supplementation, H. pylori infection and gastric cancer. Helicobacter pylori eradication for gastric cancer prevention is being conducted in China and other parts of the world. In high-risk areas, for example in China, a combination approach including H. pylori eradication and dietary supplementation may be necessary.

Abstract:Genetic susceptibility seems to play an important role in some gastric cancers. In some rare cases, gastric cancer can be one part of familial cancer syndromes. A well-known example is the family of Napoleon since the Emperor himself as well as his father and his grandfather had gastric cancer. Generally, hereditary factors include germline mutations of E-cadherin, APC, p53 or mismatch repair genes like hMLH1 or hMSH2. Gatekeeper genes like Ecadherin and APC encode signaling proteins important to cellular proliferation and differentiation while protein products from caretaker genes like p53 or mismatch repair genes are important to maintain the integrality and fidelity of the genome. In addition to gene mutations, gene polymorphisms have been shown to be associated with the susceptibility of gastric cancer (El-Omar et al., 2000; Persson et al., 2011). Interestingly, most of these genes are encoding proinflammatory cytokines, strongly indicating the importance of inflammation to the initiation and progression of gastric cancer. Environment factors play important roles in the pathogenesis of gastric cancer by affecting the inflammation in the stomach. The infection of Helicobacter pylori is apparently one of the primary risk factors of gastric cancer. In 1994, the World Health Organization and the International Agency for Research on Cancer Consensus group classified H. pylori as a type I carcinogen in human beings. Although numerous studies reported the growth promoting effect of H. pylori or its derived proteins in vitro, no direct evidence about the interaction of H. pylori with cancer initiating or stem cells have been presented. In contrast, accumulating reports indicated that the inflammation induced by H. pylori infection is the key to gastric carcinogenesis. Thus, it was believed that H. pylori infection is the initial step in the stepwise process of gastric carcinogenesis starting from chronic gastritis, gastric atrophy, intestinal metaplasia, dysplasia to gastric carcinoma. Consistently, diets rich in fruit and vegetables is protective factors of gastric cancer since vitamin C are helpful to attenuate inflammation while inflammation-promoting factors such as smoking, foods with high salts and nitrite could facilitate the development of gastric cancer by promoting chronic inflammation. Microenvironments in chronic inflammatory tissues are rich in growth factors and cytokines important to promote the perpetuation of tumor growth by epigenetically reprogramming cancer stem cells. Abstract:Gastric cancer is one of the most common causes of cancer-related mortality worldwide. The objective of this article is to review the epidemiology and biology of gastric cancer risk. This literature review explores the biological, clinical, and environmental factors that influence the rates of this disease and discuss the different intervention methods that may not only increase the awareness of gastric cancer but also increase screening in efforts to reduce the risk of gastric cancer. Helicobacter pylori infection is the primary risk factor for gastric cancer. Additional risk factors include geographical location, age, sex, smoking, socioeconomic status, dietary intake, and genetics. Primary and secondary prevention strategies such as dietary modifications and screenings are important measures for reducing the risk of gastric cancer. Interventions, such as H. pylori eradication through chemoprevention trials, have shown some potential as a preventative strategy. Although knowledge about gastric cancer risk has greatly increased, future research is warranted on the differentiation of gastric cancer epidemiology by subsite and exploring the interactions between H. pylori infection, genetics, and environmental factors. Better understanding of these relationships can help researchers determine the most effective intervention strategies for reducing the risk of this disease.
Carcinogenesis, prevention and early detection of gastric cancer: where we are and where we should go

Abstract:Helicobacter pylori is the most common cause of gastric cancer (GC), though smoking, alcohol, diet, genetics and epigenetic factors may also have a role in the occurrence of the disease. Why H. pylori cause GC in only a minority of those infected remains unknown. Although mechanisms of H. pylori-induced carcinogenesis are not yet well understood, several genotypes of H. pylori have been associated with strain virulence and disease risk. Primary prevention of GC should be addressed by avoiding exposure to factors that increase the risk and to promote factors associated with decrease risk. Vaccines against H. pylori are an ongoing promise and not yet available. Chemoprevention through vitamin supplementation has shown no benefit. Screening and eradication of H. pylori in the general population is not advised. Given that GC is a multiple-steps process, the identification of patients with preneoplastic lesions with high risk of progression, and periodic endoscopic surveillance of them represents the most effective way for early diagnosis of GC. However, clinical guidelines for surveillance are lacking and there are no clear criteria to classify patients into high or low risk of progressing to GC. No study has shown the potential usefulness of combining the information on the type of preneoplastic lesions, genetic and epigenetic, lifestyle and virulence bacterial factors in order to identify high risk patients who need more intensive surveillance. The integration of all this information, in a prediction model requires further research and could be the most important contribution for reducing the burden of GC.
Gastric Cancer Prevention: A Pathologist's Perspective

Abstract:Gastric cancer is heterogeneous in pathology and remains as one of the fetal primary cancers with a high mortality rate in China. Despite the fact that the tumorigenesis of gastric cancer remains elusive, a growing body of evidences in epidemiology, clinicopathology, and molecular pathology accumulated in recent years suggests the existence of 3 types of gastric carcinoma by location: proximal (ie, cardia), non-proximal, and diffuse type. The risk factors and pathogenesis may vary among the 3 types of gastric carcinoma. In China, the majority of gastric carcinoma are non-proximal, related to Helicobacter pylori infection, and occur primarily in the antrum and angularis incisura. It is known that environmental factors play a critical role in the development of non-proximal gastric carcinoma. Therefore, this type of carcinoma can be effectively prevented by strategies such as public health education and promotion of healthy life styles, national campaign for ban of smoking, and annual upper endoscopy surveillance with biopsy for high-risk population in order to detect early premalignant mucosal lesions and administrating curative management. This requires a close collaboration between healthcare providers and patients.
Diet and the risk of gastric cancer: review of epidemiological evidence

Abstract:There are geographic and ethnic differences in the incidence of gastric cancer around the world as well as with its trends for each population over time. The incidence patterns observed among immigrants change according to where they live. All of these factors serve to indicate the close association of gastric cancer with modifiable factors such as diet. This review presents epidemiological evidence on the association between dietary factors and gastric cancer based on previous systematic reviews and subsequent updates. Infection with Helicobacter pylori is a strong and established risk factor of gastric cancer but is not a sufficient cause for its development. Substantial evidence from ecological, case-control, and cohort studies strongly suggests that the risk may be increased with a high intake of various traditional salt-preserved foods and salt per se and decreased with a high intake of fruit and vegetables, particularly fruit. However, it remains unclear which constituents in fruit and vegetables play a significant role in gastric cancer prevention. Among them, vitamin C is a plausible candidate supported by a relatively large body of epidemiological evidence. Consumption of green tea is possibly associated with a decreased risk of gastric cancer, although the protective effects have been, for the most part, identified in Japanese women, most of whom are nonsmokers. In contrast, processed meat and N-nitroso compounds may be positively associated with the risk of gastric cancer. In conclusion, dietary modification by reducing salt and salted food intake, as well as by increasing intake of fruit and vitamin C, represents a practical strategy to prevent gastric cancer.

Abstract:Background Gastric cancer is a common malignancy and a leading cause of cancer-related deaths worldwide. Cardia gastric cancer originates in the area where the esophagus meets the stomach, while non-cardia gastric cancer develops in other parts of the stomach. These subtypes have distinct risk factors, epidemiological patterns, and prognoses, necessitating tailored approaches for prevention, early detection, and treatment. This study aimed to investigate the latest global incidence of gastric cancer by anatomical subtypes using international and national cancer registries. Methods Data on gastric cancer incidence by anatomical subtypes in 2022 were obtained from the Global Cancer Observatory (GLOBOCAN) and Cancer Incidence in Five Continents (CI5) Volume XII. Age-standardised rates for gastric cancer incidence by anatomical subtypes were adjusted using the world standard population. Results The global incidence of gastric cancer in 2022 was estimated to be 9.2 per 100,000 persons, with a total of 968,488 cases. Cardia gastric cancer accounted for 32.7% of cases, while non-cardia gastric cancer represented 67.3%. Cardia gastric cancer was more common among males (36.0%) than females (26.7%), particularly in the older population (35.1%) compared to the younger population (29.0%). The highest proportion of cardia gastric cancer cases was observed in South Central Asia (52.4%), Oceania (47.0%), and Northern America (42.6%). In contrast, the highest proportion of non-cardia gastric cancer cases was observed in the Caribbean (81.1%), Southern Europe (78.7%), and Eastern Europe (76.2%). Conclusions Non-cardia gastric cancer remains the predominant subtype of gastric cancer globally. However, cardia gastric cancer was found to be more prevalent in South Central Asia. Additionally, a higher proportion of cardia gastric cancer cases were observed among males and the older population compared with their counterparts. Further studies are needed to elucidate the underlying reasons for these distinct patterns.

Abstract:Despite the decline in incidence and mortality rates, gastric cancer (GC) is the fifth leading cause of cancer deaths worldwide. The incidence and mortality of GC are exceptionally high in Asia due to high H. pylori infection, dietary habits, smoking behaviors, and heavy alcohol consumption. In Asia, males are more susceptible to developing GC than females. Variations in H. pylori strains and prevalence rates may contribute to the differences in incidence and mortality rates across Asian countries. Large-scale H. pylori eradication was one of the effective ways to reduce GC incidences. Treatment methods and clinical trials have evolved, but the 5-year survival rate of advanced GC is still low. Efforts should be put towards large-scale screening and early diagnosis, precision medicine, and deep mechanism studies on the interplay of GC cells and microenvironments for dealing with peritoneal metastasis and prolonging patients' survival.

Abstract:Gastric cancer patients who smoke after surgery are more likely to develop cardiovascular disease (CVD) irrespective of their prior smoking status, while smoking cessation could potentially mitigate CVD risk to levels observed in non‐smokers. Female gastric cancer patients, who started consuming alcohol after gastrectomy, have higher CVD risk compared to non‐drinking females. Background While cancer patients are at an increased risk of cardiovascular disease (CVD), the role of modifiable risk factors remains poorly understood. This study investigated whether lifestyle modifications affect CVD development in gastric cancer patients who undergo surgery. Methods Using data from the Korean National Health Insurance Service (NHIS), gastric cancer patients who underwent surgery from 2010 to 2017 were identified. Lifestyle behaviours, surveyed within 2 years before and after surgery were analysed. Incident CVD, defined as a composite of myocardial infarction and stroke, was compared among subgroups of lifestyle behaviour changes. Results Among 22,211 gastrectomy patients, 628 (2.8%) developed CVD (5.68/1000 person‐years). Persistent smokers (HR: 1.72, 95% CI: 1.33–2.22) and new smokers (HR: 1.85, 95% CI: 1.04–3.30) faced higher CVD risks than non‐smokers, with an especially pronounced risk in persistent‐smoking females (HR: 3.89, 95% CI: 1.20–12.62). Smoking cessation showed no significant risk difference compared to non‐smokers (HR: 1.16, 95% CI: 0.93–1.43). Female new drinkers had a higher CVD risk than non‐drinking females (HR: 2.89, 95% CI: 1.06–7.88), while men did not show such association. Changes in physical activity, when compared to physical inactivity, were not associated with CVD risk. Conclusion Gastric cancer patients who smoked after surgery were more likely to develop CVD irrespective of their prior smoking status, with a notable vulnerability in persistent female smokers. Smoking cessation could potentially mitigate CVD risk to levels observed in non‐smokers. Alcohol intake should be avoided following surgery, especially for female gastric cancer patients.

Abstract:Numerous studies have investigated the associations between dietary components, behavioral patterns, and susceptibility to gastric cancer (GC). Diet and lifestyle cover a spectrum of both protective and harmful factors associated with GC. Additionally, non-modifiable factors such as age, gender, blood type, family history, and genetic predispositions may influence GC development. This review aims to explore the interplay between modifiable and non-modifiable factors, along with dietary habits and lifestyle practices, in relation to GC risk and the potential underlying mechanisms. We have synthesized the primary findings of observational studies (case-control and cohort), systematic reviews, and meta-analyses pertaining to preventive and deleterious factors affecting the incidence of gastric cancer. A literature search was conducted on Google Scholar, MEDLINE (PubMed), ScienceDirect, and Scopus for articles published in English from 2001 to 2024. The main search terms included body weight and body fat; diet; fruits and vegetables; meats and processed meats; fried and fast foods; milk and dairy products; salty foods; food and dietary patterns; fat and sweets intake; alcohol consumption; smoking; physical activity; age; sex; family history; blood type; genetics; and medication, and the risk of gastric cancer. Unhealthy dietary patterns, consumption of fried and fast foods, salty foods, alcohol, and smoking have been associated with an increased risk of GC. Non-modifiable factors such as advanced age, male sex, family history, genetics, and blood type A were linked to an elevated risk of GC. Conversely, physical activity and high consumption of fresh fruits and vegetables may prevent GC occurrence due to the presence of antioxidants, fiber, and polyphenols. While many studies have demonstrated that dietary patterns loaded with red and processed meats were associated with a high risk of GC, others have yielded inconclusive results. Controversial findings regarding the relationship between body weight and body fat, medications, milk and dairy products, and fat and sweets consumption with the risk of GC were also observed. Adequate diet modification and addressing preventable factors may play a pivotal role in reducing the incidence of gastric cancer.
