Comparative Evaluation of Disinfection Protocols for Dental Impressions in Prosthodontics

Abstract:Background: In prosthodontics, dental impressions are essential for creating precise dental restorations. However, these impressions are susceptible to microbial contamination, which can pose a risk of infection to patients. Consequently, effective disinfection methods are crucial to prevent postoperative infections. This study aims to evaluate the effectiveness of various disinfection techniques for dental impressions used in prosthodontics. Materials and methods: A total of 148 poured dental impressions were randomized into three disinfection groups: immersed in 0.5% sodium hypochlorite, 2% glutaraldehyde, or 0.2% chlorhexidine solution. The bacterial contamination was evaluated by direct colony-forming unit (CFU) counting, while the dimensional accuracy and surface detail duplication of each resin sample were determined as physical properties. Data were analyzed using SPSS version 23.0 (IBM Corp., Armonk, NY). Either analysis of variance (ANOVA) with the option for post-hoc or non-parametric tests was used to investigate and compare the efficacy of the better disinfection protocols where the p-value was considered significant if less than 0.05. Results: Glutaraldehyde showed the lowest mean CFU count (2.5 log10 CFUs), followed by sodium hypochlorite (3.2 log10 CFUs) and chlorhexidine (3.5 log10 CFUs). All disinfection protocols were able to significantly reduce microbial contamination when compared with the control group (p < 0.05). The results of the physical property assessment demonstrated acceptable dimensional accuracy in all tested protocols, with slight differences recorded between them regarding the reproduction of surface detail. More specifically, the mean dimensional deviation was in the range between 0.02 and 0.04 mm, while scores for surface detail reproduction ranged from 2 to 4. The ANOVA results revealed significant differences in microbial contamination levels (F(2, 145) = 5.72, p = 0.007) and dimensional accuracy (F(2, 145) = 3.45, p = 0.032) between the various disinfection protocols. Conclusion: This study enlightens the effective sterilization protocol to be adopted in prosthodontics for dental impressions. Glutaraldehyde was most effective in microbial reduction, while sodium hypochlorite and chlorhexidine were equally effective. Therefore, clinicians must be vigilant in assessing the type of microbial flora that can be encountered during prosthodontic procedures while choosing disinfection protocols for patient safety and quality of impressions.
Effect of Disinfectant with Benzethon Chloramine and Isopropanol As Main Active Ingredients on the Accuracy of Dental Impression

Abstract:OBJECTIVE To assess the effect of disinfectant (Cavicide) with benzethon chloramine and isopropanol as main active ingredients disinfectant on dental impression accuracy. METHODS The effect of Cavicide on three impression materials (alginate, polyether and vinylpolysiloxane) were assessed using a standard model. The standard model was digitized by an extraoral scanner (IScan D103i, Imetric). For each kind of impression materials, thirty impressions were taken following the manufactures' instruction in the same conditions. Subsequently, the impressions were randomly divided into three groups, with ten impressions in each group. After the impression taking was completed, the three groups underwent pure water rinse for 1 min (blank control, BC), 2% glutaraldehyde solution immersion disinfection for 30 min (glutaraldehyde, GD), and Cavicide solution spray disinfection for 5 min (Cavicide, CC), respectively. All the impressions were digitized by the extraoral scanner (IScan D103i, Imetric) after disinfection and exported to a dedicated three-dimensional analysis software (Geomagic Qualify 2014, Geomagic, USA). In the software, the digital models of the impressions were trimmed to teeth and then superimposed with the digitized standard model via best-fit alignment. Root mean square (RMS) was used to evaluate the deviations between the impression and the standard model. The deviation in the anterior and posterior regions was evaluated respectively. One-way ANOVA test and the LSD post-hoc test were used to compare the deviations between the three groups (P < 0.05). The color map of each superimposition was saved for visual analysis. RESULTS For the polyether and vinylpolysiloxane materials, the difference between the three groups was not statistically significant (P=0.933, P=0.827). For the alginate material, the difference in posterior region between group GD and group BC, as well as group GD and group CC were statistically significant (GD vs. BC, P=0.001; GD vs. CC, P=0.002), while the difference between group BC and group CC was not statistically significant (P=0.854). The visual analysis showed an obvious deviation in the buccal-lingual direction in group GD. CONCLUSION Disinfectant (Cavicide) with benzethon chloramine and isopropanol as main active ingredients using spray disinfection has no effect on the accuracy of the alginate, polyether and vinylpolysiloxane impressions.
Evaluation of the Efficacy of Novel Self Disinfectant Solution in Irreversible Hydrocolloid Impression Material - An Experimental Study

Abstract:Background: Irreversible hydrocolloid materials are widely used for both diagnostic and definitive impression procedures. Impressions can be disinfected by immersion or spraying in any compatible disinfectant. Disinfectants should not adversely affect the dimensional stability of the impression and physical properties of impression material and subsequent dental cast. Objective: The study aims to evaluate the efficacy of a hypochlorous disinfectant solution mixed with irreversible hydrocolloid on setting time, antibacterial efficacy, and dimensional stability. Methodology: Three groups were made with one control group and three alginate impression was made for each participants in the maxillary arch with 24-hours intervals between one another using the same brand of commercially available chromatic alginate. The working and setting time were noted. Bacterial swabs were collected using a dry sterile cotton swab in the mid palatal region. Dental casts were made using type III gypsum. Result: The working and setting time had significant differences whereas the dimension among the cast has no significant difference. Microbial growth analysis had distinct differences among the groups. Conclusion: The self disinfection method using a pioneer solution of hypochlorous HOCl (100 ppm) to mix the alginate impression material rather than the water with the same powder-liquid ratio prescribed by the manufacturer had the same dimensional stability with improved antimicrobial action.
Impression Disinfection and Its Effect on Dimensional Accuracy and Surface Detail in the Times of COVID-19: An In Vitro Study

Abstract:Introduction The disinfection of impressions is crucial to eliminate the viral and other microbial loads to prevent the cross contamination of diseases. The aim of this study was to compare the effect of different virucidal disinfecting methods on the dimensional accuracy and surface detail reproduction (SDR) of impression materials. Methods A total of 160 samples were fabricated with different impression materials using zinc oxide eugenol (Group 1), alginate (Group 2), polyether (Group 3), and addition silicone (Group 4) impression materials, each containing 40 samples (n=40). These groups were further divided into Subgroups A, B, C, and D (n=10) based on the disinfecting method used. Disinfection was carried out using 0.2% peracetic acid (A), a natural polymer of glucosamine (B), ultraviolet (UV) radiation (C), and ozonated water (D). The disinfected impressions were poured in type IV gypsum, and the obtained casts were checked for dimensional accuracy and surface detail reproduction (SDR). For dimensional accuracy, a one-way analysis of variance (ANOVA) test and, for surface detail reproduction, the chi-square test were used to compare the different subgroups of each impression material separately. Results Zinc oxide eugenol samples showed the lowest mean dimensional change when disinfected with 0.2% peracetic acid (1A=154.1 µm), and alginate showed the lowest mean dimensional change when disinfected using ozonated water (2D=134.9 µm). On the other hand, the lowest mean dimensional change observed in polyether and addition silicone samples was those which were disinfected using UV radiation (3C=100.9 µm and 4C=113.5 µm). Surface detail was reproduced adequately in most of the samples. Conclusion A 0.2% peracetic acid could be used to disinfect zinc oxide eugenol impressions, ozonated water for alginate impressions, and UV radiation for polyether and addition silicone impressions.

Abstract:The need to use a natural alternative for disinfecting dental impression materials, which should be biocompatible and effective, led us to evaluate the efficacy of Aloe vera as a disinfectant by immersion and spray method on alginate impression material and its effect on the dimensional stability of the resultant gypsum cast. The efficacy of the disinfection procedures against Staphylococcus aureus, Pseudomonas aeruginosa, and Candida albicans was evaluated by determining the number of colony-forming units (CFU) recovered after disinfection of alginate discs inoculated with 1x106 CFU for defined intervals with aloe vera solution (99.96%). Dimensional stability was determined from the mean percentage deviation of three measurements that were taken between the fixed points on the casts using a traveling microscope and compared with corresponding measurements from the master model and controls. Statistical analysis of data was determined by analysis of variance. We found out that there is a mean percentage reduction in colony count of S. aureus, P. aeruginosa, and C. albicans after 3 minutes of immersion in aloe vera and after 3 minutes spray disinfection. Complete elimination of all the microorganism cells after 7 min immersion and spray disinfection. There was a statistically significant difference in the increase of the mean anteroposterior (AP) and cross arch (CA) dimension after 3 and 7 minutes immersion in aloe vera. Spraying with aloe vera for 7 minutes was proved to be the most effective disinfection procedure without altering dimensional stability.
Effect of disinfectants on the colour stability of heat cure acrylic resin

Abstract:Background: Contaminated dentures need to be disinfected as a part of denture hygiene regimen. This study was conducted to determine the colour stability of heat cure acrylic resin after treatment with two disinfectants. Methods: This in-vitro experimental study was conducted at Dr Ishrat-ul- Ebad Khan Institute of Oral Health Sciences, Dow University of Health Sciences and Al Karam Textiles Karachi, from January to April 2012. Total 72 rectangular shaped specimens; 18 specimens were measured at baseline (control group) of the study (0 day), 18 specimens were immersed in distilled water. Eighteen (18) specimens were placed in 1% Sodium Hypochlorite solution for 20 minutes and eighteen (18) specimens were placed in Alkaline Gluteraldehyde for 10 minutes. All specimens were polished, stored in distilled water for 24 hours prior to experiment. All the specimens were immersed twice daily for total of 60 days. After 60 days of immersion the specimens were tested for colour changes with spectrophotometer. SPSS-16 was used for statistical analysis. Results: There was statistically significant difference in the colour change (AE) between all groups (p<0.001) after 60 days of immersion. At baseline (0 day), trace amount of colour change was observed whereas when specimens immersed in distilled water undergo slight change in colour after 60 days of immersion. Furthermore, specimens immersed in 1% Sodium hypochlorite for 20 minutes and 2%AlklcalineGluteraldehyde for 10 minutes for 60 days twice a day showed noticeable changes in colour. Conclusion: There results of the study advocated that the colour stability of denture base acrylic resin is affected by the disinfectants used.
Disinfection Efficacy of Sodium Hypochlorite and Glutaraldehyde and Their Effects on the Dimensional Stability and Surface Properties of Dental Impressions: a Systematic Review

Abstract:Objective: To systematically evaluate the disinfection efficacy of the two most frequently used disinfectants, sodium hypochlorite and glutaraldehyde, and their effects on the surface properties of four different dental impression materials.Methods: A systematic literature search was performed in 4 databases until May 1st, 2022 to select the studies which evaluated disinfection efficacy of disinfectants or surface properties of dental impressions after chemical disinfection.Main results: 50 studies were included through electronic database searches.Of these studies, 13 studies evaluated disinfection efficacy of two disinfectants, and 39 studies evaluated their effects on the surface properties of dental impressions.10-minute disinfection with 0.5-1% sodium hypochlorite or 2% glutaraldehyde was effective to inactivate oral flora and common oral pathogenic bacteria.With regard to surface properties, chemical disinfection within 30 minutes could not alter the dimensional stability, detail reproduction and wettability of alginate and polyether impressions.However, the wettability of addition silicone impressions and the dimensional stability of condensation silicone impressions were adversely affected after chemical disinfection, while other surface properties of these two dental impressions were out of significant influence. Conclusions:Alginate impressions are strongly recommended to be disinfected with 0.5% sodium hypochlorite using spray disinfection method for 10 minutes.Meanwhile, elastomeric impressions are strongly recommended to be disinfected with 0.5% sodium hypochlorite or 2% glutaraldehyde using immersion disinfection method for 10 minutes, however, polyether impression should be disinfected with 2% glutaraldehyde.

Abstract:Introduction. Bacteria in biofilms (BFs) have increased resistance to antibacterial agents, including disinfectants; however, the efficacy level varies depending on the chosen treatment. Therefore, evaluation of efficacy of main disinfectants against BF-residing microorganisms is of scientific and practical interest. The purpose of the study was to explore the effect of disinfectants from various chemical groups on gram-positive and gram-negative bacteria residing in BFs. Materials and methods. The effect of the following disinfectants has been evaluated: alkyldimethylbenzylammonium chloride (ADBAC), tertiary amine (TA), polyhexamethylene guanidine chloride (PHMG), hydrogen peroxide (HP), chloramine (CA), dichloroisocyanuric acid sodium salt (Na DCC), sodium hypochlorite (HC), ethyl alcohol (EA), glutaraldehyde (GA)) against Pseudomonas aeruginosa ATCC 15442 and Staphylococcus aureus ATCC 6538-P BFs. BFs were grown in 96-well plates at 37oC for 24 hours and then exposed to biocide solutions. The efficacy of disinfectants was evaluated by the number of remaining viable cells and BF relative density. Results. The analyzed bacterial strains formed moderate BFs; the average number of viable cells in BFs was 6.51 ± 0.19 lg. The viable bacterial cell counts in BFs reduced by more than 4 lg when exposed to HP solutions at a concentration of 6%, Na DCC solution — 0.1% (by active chlorine), HC — 1% (by active chlorine), CA – 1% (by product), PHMG — 0.05%, TA — 1.0 %. The BF density decreased by more than 70%. ADBAC solutions at concentrations of 0.1–1.0%, TA — 0.05%, HP — 3%, Na DCC solution — 0.05% (by active chlorine) caused a 2-lg reduction in viable cell counts in BFs. The efficacy of chlorine-active compounds and HP increased when 0.5% sulfonol was added. GA (0.25–1.00%) and EA (40–70%) solutions were ineffective against BF microorganisms. Conclusion. A promising potential in combating microbial biofilms is demonstrated by disinfectants from the group of oxidizing agents (chlorine-active and oxygen-containing), TA and PHMG; using ADBAC as an individual compound is ineffective; aldehydes and alcohols are unable to destroy BFs and eliminate microorganisms in them.

Abstract:INTRODUCTION: This randomized clinical trial assessed, by using microbial culture and scanning electron microscopy (SEM), the contamination by mutans streptococci (MS) colonies/biofilms on acrylic baseplates of removable orthodontic appliances and evaluated the efficacy of antimicrobial sprays (Periogard [Colgate-Palmolive Ind. Brasileira, Osasco, SP, Brazil], Cepacol [Merrell Lepetit Farmacêutica e Industrial Ltda, Santo Amaro, SP, Brazil], and sterile tap water [control]) on their disinfection.METHODS: Seventeen children were randomly enrolled in a 3-stage changeover system with a 1-week interval between each stage. All solutions were used in all stages by a different group of children. The acrylic baseplates were worn full time except at meals. At the end of each week of the trial, the baseplates were submitted to a randomized disinfection protocol and were sent for microbiologic analysis. New baseplates were constructed, and the same sequence of procedures was repeated 2 more times. Acrylic baseplates representing each test solution were examined by SEM. The Friedman test assessed differences at the 5% significance level among the solutions for MS biofilm formation on acrylic surface.RESULTS: Cepacol and Periogard reduced the formation of MS colonies/biofilms, and both solutions differed statistically from sterile tap water (P <.001). However, Periogard was significantly more effective against MS than Cepacol (P <.001). When MS colonies/biofilms were detected on acrylic surfaces under stereomicroscopy, this was confirmed with SEM.CONCLUSIONS: Acrylic baseplates of removable orthodontic appliances worn by children were contaminated by MS colonies/biofilms in all cases after 1 week. Although Cepacol had better results than sterile tap water, spraying with Periogard showed significantly greater efficacy in reducing MS colonies/biofilms on acrylic surfaces.

Abstract:A BSTRACT Background: One of the major problems of everyday dental practice is cross-contamination. It can place office personnel, dentists, and patients at risk of acquiring serious illness. Disinfection helps in controlling this cross-contamination to an extent. The evaluation was done to find the efficient disinfection method on gypsum casts. Aims: The aim of this study is to evaluate and compare the efficacy of three methods of disinfection of gypsum casts, namely, chemical disinfection by immersion, spray method, and microwave method, and also to evaluate and compare changes in surface roughness and dimensional accuracy between the three methods after disinfection. Materials and Methods: Ex vivo and experimental study. Thirty participants were selected, and impressions of the maxillary arch were made using polyvinyl siloxane impression material. Ninety type IV die stone gypsum casts were poured. It was divided into three groups and was subjected to chemical disinfection by immersion and spray methods, and microwave method. The disinfected casts were evaluated for microbial growth, surface roughness, and dimensional accuracy. It was performed by using the one-way analysis of variance test and paired t -test followed by the Kruskal − Wallis test and Wilcoxon signed rank test (α = 0.05). Results: Microwave disinfection was more effective than both immersion and spray chemical disinfection methods ( P < 0.010 and <0.001). The surface roughness of the microwave-irradiated casts had significantly increased after disinfection. However, there were no significant dimensional changes by any of the methods of disinfection. Conclusion: Within the limitations of the study, the microwave method of disinfection is more effective in eradicating microorganisms when compared to chemical methods of disinfection by immersion and spray methods.

Abstract:Introduction: Orthodontist must take precautionary measures to protect patients from cross-infection by contaminated materials received from the manufacturers. To minimize the incidence of cross-infection prior to bonding an invitro study was done to assess the antibacterial activity of chlorhexidine, povidone iodine and glutaraldehyde. Aim: To compare the antibacterial activity of 2% chlorhexidine, 5% povidone iodine and 2% glutaraldehyde on orthodontic materials prior to bonding. Materials and methods: A total of 36 samples were included in this study. The main group consists of 3 disinfectants and one control group (n =4). Group -1(Chlorhexidine gluconate), Group - 2 (Povidone Iodine), Group - 3 (Glutaraldehyde) and Group - 4 (Control group –untreated). Each group consists of 3 subgroups of the tested samples namely group A (brackets), group B (archwires) and group C - molar bands (n=9). which were contaminated by 3 bacterial species streptococcus, staphylococcus, lactobacillus and the number of colonies were counted before disinfection. The contaminated samples were subjected to disinfection with 2% chlorhexidine for 5 minutes 2% glutaraldehyde and 5% Povidone Iodine for 10 minutes. 20μl of sample of the disinfected samples were incubated and the number of colonies were counted after disinfection. Results: There were no colonies formed when 5% Povidone iodine was used to disinfect archwires and molarbands in few tested samples. 2% glutaraldehyde and 2% chlorhexidine were not effective against lactobacillus species. Povidone iodine was effective when compared to glutaraldehyde and chlorhexidine gluconate and effective against all the three species namely Streptococcus, Staphylococcus and Lactobacilli species. The obtained p-value is < 0.001 which is statistically significant when all groups were compared overall. Conclusion: Disinfection with 5% povidone iodine was most effective when compared with 2% chlorhexidine and 2% glutaraldehyde.

Abstract:Abstract Objective: The abundance and prevalence of dry-surface biofilms (DSBs) in hospitals constitute an emerging problem, yet studies rarely report the cleaning and disinfection efficacy against DSBs. Here, the combined impact of treatments on viability, transferability, and recovery of bacteria from DSBs has been investigated for the first time. Methods: Staphylococcus aureus DSBs were produced in alternating 48-hour wet–dry cycles for 12 days on AISI 430 stainless steel discs. The efficacy of 11 commercially available disinfectants, 4 detergents, and 2 contactless interventions were tested using a modified standardized product test. Reduction in viability, direct transferability, cross transmission (via glove intermediate), and DSB recovery after treatment were measured. Results: Of 11 disinfectants, 9 were effective in killing and removing bacteria from S. aureus DSBs with >4 log 10 reduction. Only 2 disinfectants, sodium dichloroisocyanurate 1,000 ppm and peracetic acid 3,500 ppm, were able to lower both direct and cross transmission of bacteria ( 2 days. Treatments not involving mechanical action (vaporized hydrogen peroxide and cold atmospheric plasma) were ineffective, producing <1 log 10 reduction in viability, DSB regrowth within 1 day, and 100% transferability of DSB after treatment. Conclusions: Reduction in bacterial viability alone does not determine product performance against biofilm and might give a false sense of security to consumers, manufacturers and regulators. The ability to prevent bacterial transfer and biofilm recovery after treatment requires a better understanding of the effectiveness of biocidal products.

Abstract:Environmental surfaces play a key role in transmitting pathogens that can survive on surfaces for long durations. The interest in long-lasting or residual disinfectants are, therefore, growing as it might protect surfaces for longer than traditional disinfectants. In this study, a quat-based product claiming residual disinfecting performance against bacteria, among other microorganisms, was tested using an approved standardized method, in a controlled laboratory study and on environmental surfaces in an office building. The results obtained showed that the residual disinfectant can reduce the bacterial counts significantly compared to a traditional quat-based disinfectant when used on horizontal surfaces, twenty-four hours after application. During the standardized test method, the residual disinfectant provided a 6-log reduction, whereas the traditional disinfectant provided only a 1.9-log reduction. Similarly, the residual disinfectant provided a 2.5 log reduction in the laboratory study, whereas the traditional disinfectant had too-numerous-to-count colonies. When tested on environmental surfaces, an ANOVA statistical analysis indicated that surfaces treated with the residual disinfectant had significantly less bacteria present twenty-four hours after application. The antibacterial performance of the residual disinfectant showed to be limited by the orientation of the treated surface, and the thickness of the product film dried on the surface. This study showed the potential of residual disinfectants that warrants further investigation and could potentially aid the further development of the technology.

Abstract:Objective:Pathogenic microbes may be transmitted directly from the orthodontist to the patient or from the patient to the doctor, and indirectly from patient to patient. The latter may occur via contaminated instruments or surfaces, and is referred to as cross-contamination. The objective of this study was to evaluate the extent of bacterial contamination of orthodontic pliers and the efficacy of the disinfection techniques applied after clinical use. We also sought to examine under standardized conditions the virucidal, bactericidal and fungicidal effects of disinfection techniques used in practice.Materials and Methods:The efficacy of various disinfection methods was determined after clinical use in-vivo on 10 test subjects and in-vitro with deliberate contamination. The following disinfection methods were tested:1. Iso-Septol spray2. Incidur® spray3. Trough disinfection in combination with 5% Sekusept® Plus solution4. Ultrasound bath in combination with 5% Sekusept® Plus solution5. Thermal disinfectionFor in-vitro contamination we used the test organisms Staphylococcus aureus, Escherichia coli, Candida albicans, Coxsackie virus B4, HSV 1, and Adenovirus type 5. The tests were carried out six to eight times for each organism. The Weingart pliers and distalend cutters were tested. The criteria for effective disinfection were a reduction in infectiosity of five log steps (for bacteria and fungi) or four log steps (viruses). Statistical analysis was carried out using the Wilcoxon and Whitney U-test.Results:The presence of contamination following clinical use was not adequately eliminated with all disinfection methods. The spray methods exhibited shortcomings in disinfection. For the type of contamination defined, trough disinfection with 5% Sekusept® Plus and the Incidur® and Iso-Septol spray disinfection methods provided insufficient disinfection. Conversely, the ultrasound bath with 5% Sekusept® Plus solution and steam disinfection met the criteria for effective disinfection for all microbes. No statistically significant difference was found between the oiled and unoiled states. In some cases, there were slightly higher rates of contamination with the Weingart pliers as with the distalend cutters. However, these were not statistically significant.Conclusions:It should be possible to disinfect lipophilic viruses and the usual bacterial infections adequately with all methods, provided that the use of sprays and trough disinfection is preceded by cleaning with brush and water, followed by drying. With hydrophilic viruses, however, the spray and trough disinfection methods are limited in their efficacy and cannot be considered adequate. Exclusively chemical methods are therefore less effective than thermal or physical-chemical methods.Thermal disinfection and the ultrasound bath in combination with 5% Sekusept® Plus are clearly superior to spray disinfection and trough disinfection alone. The ultrasound bath and thermal disinfection can therefore be recommended for the disinfection of orthodontic pliers. We recommend that the pliers be cleaned beforehand due to their uneven surfaces.

Abstract:PURPOSE: Dental prosthetic and orthodontic appliances are transported from the clinic to the laboratory for additions and repairs. These appliances, containing microbes from the oral flora, are a high risk for cross-contamination. The aim of this study was to evaluate the effect of chemical and ultrasound disinfection against two in vitro biofilms and an in vivo formed biofilm grown on unprepared and polished polymethyl methacrylate (PMMA) surfaces.MATERIALS AND METHODS: Rough and polished self-curing PMMA surfaces were infected with strains of both Candida albicans and Streptococcus oralis. After incubation, the samples were treated with different disinfection methods, including ultrasound treatment for both 15 and 30 seconds, and immersion in glutaraldehyde and alcohol-based chemical disinfectants (MD520 and Minuten, respectively). The disinfecting efficacy was assessed by colony forming units (CFU) analysis and by scanning electron microscopy (SEM). Furthermore the adequacy of bacterial elimination of application of 30-second ultrasound and MD520 was assessed on PMMA retrieved from ten volunteers by CFU analyses. ANOVA with p = 0.05 followed by the Tukey post hoc test and the Student t-test was used to analyze the data.RESULTS: The ultrasound treatment for 30 seconds, MD520, and Minuten were the most effective disinfectant methods as they reduced the microbial counts compared to the control (p < 0.05) as shown in the in vitro analyses. S. oralis adhered more to rough acrylic resin surfaces (p < 0.05). Ultrasound treatment was the most effective way to reduce microbial counts on PMMA exposed to oral flora (p = 0.043).CONCLUSION: Ultrasound treatment for 30 seconds was effective against C. albicans, S. oralis, and the oral flora as shown by testing microbial growth on agar plates and SEM.

Abstract:Milbemycins have received considerable interest in agricultural chemistry due to a special action mode, extremely high activity against arachnoide pests, low toxicity to mammals, and environmentally benign characteristics. Two series of novel milbemycin analogues (4Ia-6IIc) containing alkyl and aryl groups at the 4'- and 13-positions were designed and synthesized by five schemes. These analogues were identified by (1)H NMR, (13)C NMR, and elemental analysis (or HRMS). Their insecticidal activities against carmine spider mite, oriental armyworm, and black bean aphid were evaluated. The results showed that all of the title compounds had low acaricidal activity against carmine spider mite. However, most of them exhibited good insecticidal activities against oriental armyworm and black bean aphid at a concentration of 200 mg L(-1). The most potent substituents of 2,2-dimethylbutanoyl (4Ib), phenylacetyl (4IIm), and (Z)-1-(methoxyimino)-1-phenylacetyl (4IIn) exhibited the highest larvicidal activities, and its insecticidal LC(50) values against oriental armyworm were 0.250, 0.204, and 0.350 mg L(-1), while its insecticidal LC(50) values against black bean aphid were 0.150, 0.070, and 0.120 mg L(-1), respectively. These substituents provided some hints for further investigation on structure modification.
Effectiveness of supplementary antimicrobial procedures in disinfecting lateral canals as evaluated by a novel ex vivo analytical approach

Abstract:This ex vivo study devised an analytical ex vivo method for infection/disinfection of simulated lateral canals located in the middle and apical segments of the root. The antibacterial effects of supplementary approaches were tested in this model. Extracted mandibular premolars had their main root canals enlarged and then two lateral canals (100 μm in diameter) were created in the root, one in the apical and the other in the middle portion. Micro-computed tomography was used for specimen selection and to confirm the quality of the simulated ramifications. The specimens were contaminated with a mixed bacterial culture from subgingival bacterial biofilm added to pure Enterococcus faecalis strain ATCC 29212 grown overnight, using special strategies to facilitate culture medium penetration within the lateral canals. The following procedures were tested for disinfection: NaOCl/passive ultrasonic irrigation (PUI), NaOCl/XP-endo Finisher, ozonated water/continuous ultrasonic irrigation (CUI), and NaOCl/conventional irrigation with 30-G needles (control). Bacteriological samples were taken from the main canal before (S1) and after (S2) each supplementary protocol, and also from each lateral canal after treatment (S3). DNA extracted from the samples was subjected to quantitative real-time polymerase chain reaction. All S1 main canal samples were positive for bacterial presence. Bacterial counts in the main root canal substantially decreased by 99.2% after PUI, 99.1% after ozone/CUI, 99% after XP-endo Finisher, and 96% in the control group (P < 0.01 for all groups). There were no significant differences between groups (P > 0.05). The same was observed when comparing the effects of the supplementary approaches in the apical and middle lateral canals (P > 0.05). Only a few lateral canals showed no detectable bacteria. The method proposed here proved effective for ex vivo infection/disinfection studies. All supplementary approaches induced a substantial bacterial reduction in the main canal, with no significant differences between them. However, in terms of lateral canal disinfection, none of the tested approaches showed significant effects when compared to the control group.
Evaluation of home disinfection protocols for acrylic baseplates of removable orthodontic appliances: A randomized clinical investigation

Abstract:INTRODUCTION: In this randomized clinical trial, we investigated, using the microbial culture technique and scanning electron microscopy, the contamination of acrylic baseplates of removable orthodontic appliances by mutans streptococci (MS) and evaluated the efficacy of different home disinfection protocols with a 0.12% chlorhexidine gluconate spray (Periogard, Colgate-Palmolive, São Bernardo do Campo, Brazil).METHODS: Fifteen dental students were randomly enrolled in a 3-stage changeover system with a 1-week interval between each stage. The acrylic baseplates were worn full time except at meals to simulate the routine use of removable appliances under clinical conditions. Three 1-week home disinfection protocols were tested in all stages by a different group of students: protocol I, toothbrushing + baseplate brushing + sterile tap water spraying once a day; protocol II, toothbrushing + baseplate brushing + Periogard spraying on the seventh day after appliance placement; and protocol III, toothbrushing + baseplate brushing + Periogard spraying on the fourth and seventh days after appliance placement. After the first week, the volunteers received new baseplates, toothbrushes, and dentifrices, and the regimens were repeated 2 more times. At the end of each week, the baseplates had a randomized disinfection protocol and were sent for microbiologic analysis. A scanning electron microscope was used to examine 3 acrylic baseplates representing each home protocol. The Friedman test (α = 0.05) compared the home protocols for the formation of MS colonies or biofilms on the acrylic surfaces.RESULTS: MS colonies or biofilms were found on all acrylic baseplates after protocol I. Protocols II and III reduced significantly (P <0.05) the number of MS colonies and biofilms on the acrylic surfaces. No significant difference (P >0.05) was observed between protocols II and III. The scanning electron microscope analysis confirmed the results of the microbiologic cultures.CONCLUSIONS: Disinfection of baseplates of removable orthodontic appliances by using 0.12% chlorhexidine spray once or twice a week reduced the contamination by MS on the acrylic surface in vivo.

Abstract:Background: Surface detail reproduction (SDR) and dimensional stability of elastomeric impression material play a crucial role in fixed prosthodontics. Aim and Objectives: The aim of this study was to compare the effect of chemical disinfection on SDR and dimensional stability of polyvinyl siloxane (PVS) and polyether (PE) with a new vinyl polyether silicone (VPES) elastomeric impression material. Materials and Methods: A stainless steel mold was made to fabricate the study specimens for nonaqueous PVS, PE, and VPES elastic dental impression materials. Single mix impression techniques with light- and heavy-body consistency of all three materials were used to prepare the test specimens. The specimens were immersed in glutaraldehyde (Cidex) 2.45% and sodium hypochlorite (Hypo) 3.0% disinfection for 15 min (T1) and 12 h (T2) immersion after which dimensional stability and SDR were recorded using stereomicroscope and digital Vernier caliper. Statistical Analysis: The data were analyzed using the one-way ANOVA, paired t-test, Kruskal–Wallis test, and Wilcoxon signed-rank test. Results: Dimensional stability at the T2 time interval showed a highly significant difference for control and glutaraldehyde groups (P < 0.001), whereas a statistically significant difference for Hypo group (P < 0.05). SDR of the three materials when compared at T1 time interval showed a statistically significant difference (P = 0.015). A comparison between two disinfectants at T1 time interval revealed a highly significant difference (P < 0.001), while at T2 interval significant difference was obtained (P = 0.009). Conclusion: VPES impressions display acceptable dimensional stability and SDR for clinical use with immersion disinfection. Although some statistically significant differences in linear dimensional stability and semidefinite programming were observed among VPES, PE, and PVS, but the clinical impact of these differences is minor considering the overall accuracy of casts which was high.

Abstract:Paranuclear blue inclusions (PBIs) are frequently identified within metastatic undifferentiated small cell carcinoma (SCC) cells on air-dried bone marrow aspirates stained with Wright's stain. To determine the sensitivity and specificity of this finding, 116 bone marrow aspirates containing metastatic neoplasms were evaluated for the presence and frequency of PBIs. Bone marrow specimens included 47 cases of metastatic SCC of the lung, 13 cases of large cell lymphoma, 19 cases of neuroblastoma, five cases of small, noncleaved cell lymphoma, seven cases of rhabdomyosarcoma, three cases of Ewing's sarcoma, three cases of other sarcomas, and 19 cases of non-small cell carcinoma (adenocarcinoma). PBIs were identified in 40 of 47 (85%) cases of SCC and their frequency varied from 0 to 24% of tumor cells among different cases. In approximately half the cases of SCC, PBIs were identified in 1 to 4% tumor cells; and in eight cases, PBIs were present in 5% or more of tumor cells. PBIs were also identified in two of seven (29%) cases of rhabdomyosarcoma and one case of malignant peripheral nerve sheath tumor, but they were not seen in Ewing's sarcoma, small non-cleaved cell lymphoma, large cell lymphoma, neuroblastoma, or non-small cell carcinoma. In addition, PBIs were not seen in alcohol-fixed, Papanicolaou-stained cytology specimens containing SCC. Ultrastructurally, PBIs may represent phagocytized nuclear/cellular material. PBIs are a feature of small cell carcinoma on air-dried, cytologic material stained with Romanowsky type stains. Their presence may provide diagnostic information with regard to the differential diagnosis of metastatic SCC in the bone marrow. Future studies evaluating non-bone marrow Wright's stained fine-needle aspiration specimens are needed to determine if PBIs are useful in distinguishing SCC from other poorly differentiated tumors in the cytology laboratory.
