They show that P. goldsteinii and its metabolite 7-keto-lithocholic acid promoted repair of aspirin-associated intestinal damage by suppressing the bile acid receptor FXR and maintaining Wnt signaling in intestinal stem cells.

Abstract:Background The gut microbiota, a pivotal regulator in colorectal cancer (CRC) development, is profoundly involved in drug metabolism. This study aims to decipher the role of the gut microbiota in aspirin-mediated chemoprevention of CRC in mice. Methods Two models, azoxymethane (AOM, 10 mg/kg) and dextran sulfate sodium (DSS, 2.0%) induced CRC and APCmin/+ induced CRC, were used in this study. Mice were orally administered aspirin (400 mg/L) with or without periodical antibiotics cocktail (0.1 g/L of vancomycin, 0.2 g/L of ampicillin, metronidazole and neomycin). AOM/DSS-treated germ-free mice and conventionally reared germ-free mice (conventionalized mice) were used for validation. Aspirin concentration was estimated by ultra-high performance liquid chromatography (UHPLC). Aspirin-degrading microbes were identified by UHPLC and16S rRNA gene sequencing. Results The tumor number and load were significantly reduced after aspirin treatment in antibiotics-treated mice in both AOM/DSS-treated and APCmin/+ models, but not significantly changed in non-antibiotics-treated mice. Consistently, aspirin-treated germ-free mice exhibited significantly less tumor number and load compared to those not receiving aspirin, which was not seen in conventionalized mice, suggesting that gut microbiota impairs the chemopreventive efficacy of aspirin on CRC. UHPLC analysis revealed that plasma levels of aspirin were significantly higher in microbiota-depleted mice compared to microbiota-intact mice, indicating that gut microbiota limits aspirin bioavailability. Co-incubation of aspirin-containing medium and bacteria showed that aspirin levels in medium were significantly reduced when incubated with the fecal commensal bacteria from microbiota-intact mice compared to those from microbiota-depleted mice. Screening 1,093 bacterial colonies isolated from the feces of microbiota-intact mice revealed that Lysinibacillus sphaericus significantly reduced the aspirin levels in aspirin-containing medium. Moreover, the germ-free mice monocolonized with Lysinibacillus sphaericus showed significantly lower plasma aspirin levels post aspirin administration relative to the intact germ-free mice, confirming a role of this bacterium in reducing aspirin bioavailability. Conclusions Gut microbiota modulates the chemopreventive efficacy of aspirin on CRC through impact on aspirin bioavailability in mice. Lysinibacillus sphaericus may play a critical role in aspirin degradation in the gut.

Abstract:Background Aspirin, the most widely used nonsteroidal anti-inflammatory drug, damages the gastrointestinal tract. Interactions between Aspirin and gut microbiota have been reported. However, the impact of microbiome altered by Aspirin in intestinal homeostasis remains unclear. We aim to determine the roles of Aspirin-altered microbiota in intestinal injury. Methods Healthy volunteers (N=23) were administrated with Aspirin (100mg/day) for 30 days (IDDF2022-ABS-0097 Figure 1A. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis). Shotgun metagenomics analysis on paired fecal samples before (BA) and after Aspirin (AA) administration was performed. Mice were treated with Asprin and fecal samples were subjected to 16s-sequencing and metabolomics by gas chromatography coupled to time-of-flight mass spectrometry. Fecal Microbiome Transplantation (FMT) for mice were conducted and intestinal injury was assessed by Chiu’s grades. Cell proliferation and apoptosis were determined through Ki67 and TUNEL staining. Intestinal permeability was evaluated by FITC-labeled dextran. Results Partial least squares discriminant analysis and linear discriminant analysis showed differences in the gut microbiome profiles (IDDF2022-ABS-0097 Figure 1B-C. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis). The abundance of Parabacteroide goldsteinii (P. goldsteinii) were found to be decreased in AA group (IDDF2022-ABS-0097 Figure 1 D-E. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis). Similar results were also found by 16s-sequencing in Aspirin-induced mice models (IDDF2022-ABS-0097 Figure 1F-G. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis). Scores of intestinal injury were negatively correlated with the abundance of P. goldsteinii (IDDF2022-ABS-0097 Figure 1H, J-M. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis). Intestinal concentrations of bile acids in mice were correlated with the abundance of P. goldsteinii (IDDF2022-ABS-0097 Figure 1I. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis). More interestingly, FMT (IDDF2022-ABS-0097 Figure 1N. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis) from donating mice treated with Aspirin to NC mice induced intestinal injury, whereas FMT of NC mice to Aspirin-treated mice, helped intestinal repairment and decreased the intestinal injury score (IDDF2022-ABS-0097 Figure 1O-P. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis). Further, gavage of P. goldsteinii rescued Aspirin-induced intestinal injury (IDDF2022-ABS-0097 Figure 1Q-R. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis), promoted the proliferation of cells in crypts (IDDF2022-ABS-0097 Figure 1S-T. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis), decreased apoptosis (IDDF2022-ABS-0097 Figure 1U-V. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis) and preserved intestinal permeability (IDDF2022-ABS-0097 Figure 1W. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis). These suggest that the Aspirin-suppressed microbiome, such as P. goldsteinii may be involved in the maintenance of intestinal homeostasis, which was disturbed by Aspirin treatment. Conclusions Aspirin leads to dysbiosis of gut microbiota, which impairs the homeostasis of the intestinal mucosa (IDDF2022-ABS-0097 Figure 1X. Aspirin impairs intestinal homeostasis through gut microbiota metabolites axis). Supplement of probiotics P. goldsteinii may be a promising strategy to prevent Aspirin-induced GI damage.

Effects of Long-term, Low-dose, Enteric-coated Aspirin on Gastroduodenal Mucosa: A Case-control Study

Abstract:Background: The safety of long-term, low-dose, enteric-coated aspirin for the purpose of prevention or treatment of certain illness has attentively focused upon. Aims: To determine whether gastroduodenal mucosa could be injured by longterm, daily administration of low-dose (≤100 mg/d), enteric-coated aspirin. Methods: A total of ninety-two patients with dyspeptic symptoms, taking low dose, enteric-coated aspirin daily for over three months, without history of peptic ulcer and alimentary tract hemorrhage, without taking other non-steroidal anti-inflammatory drug (NSAID), antacid, antibiotic and bismuth, and without Helicobacter pylori (H. pylori) eradication were endoscopied. Results: Of all patients, 40 (43.5%) had mild mucosal injury at endoscopy, but no ulcers were observed. There were no differences in age, sex, smoking, mean dose of aspirin, and common dyspeptic symptoms between the mucosal injury and non-mucosal injury groups (P>0.05); H. pylori infection was more frequent in the mucosal injury group than that in the non-mucosal injury group (P<0.05), but in smoking and age stratified analysis, the role of H. pylori had not been confirmed. However, a significant correlation was found between intensity of H. pylori infection and severity of mucosal injury (r6=0.308, P<0.05). Conclusions: Long-term, low-dose, enteric-coated aspirin could cause gastroduodenal mucosal injury. H. pylori infection, smoking and age are all risk factors, but H. pylori infection seems to be the major risk factor.

Effect of a proton-pump inhibitor on intestinal microbiota in patients taking low-dose aspirin

Abstract:Background and aim: Low-dose aspirin (LDA) administration prevents cerebral infarction and myocardial infarction, but many studies found an association with mucosal injury. Proton-pump inhibitors (PPIs) can prevent gastric and duodenal mucosal damage, but they may exacerbate small-intestinal mucosal injury by altering the microbiota. We aimed to assess the effect of PPIs on the intestinal flora of LDA users. Methods: Thirty-two recruited patients, who received LDA (100 mg/day) but did not take PPIs, were divided into 15 patients additionally receiving esomeprazole (20 mg/day) and 17 patients additionally receiving vonoprazan (10 mg/day). On days 0, 30, 90, and 180, the microbiota of each patient was examined by terminal restriction fragment length polymorphism analysis, and the serum gastrin, hemoglobin, and hematocrit levels were measured. Results: Additional PPI administration increased the proportion of Lactobacillales in the microbiota of LDA users. This trend was more prevalent in the vonoprazan group (p < 0.0001) than in the esomeprazole group (p = 0.0024). The Lactobacillales proportion was positively correlated with the gastrin level (r = 0.5354). No significant hemoglobin or hematocrit level reduction was observed in subjects receiving LDA with additional PPI. Conclusions: Additional PPI administration increased the Lactobacillales proportion in the microbiota of LDA users. The positive correlation between the gastrin level and the proportion of Lactobacillales suggested that the change in the intestinal flora was associated with the degree of suppression of gastric acid secretion. Additional oral PPI did not significantly promote anemia, but the risk of causing PPI-induced small-intestinal mucosal injury in LDA users should be considered.

Lactobacillus complex capsules ameliorate aspirin-related small intestinal mucosal injury: a prospective, randomized, controlled clinical trial

Abstract:Objectives: Aspirin can reduce cardiovascular disease risk; however, it can increase the risk of gastrointestinal injury. Lactobacilli have some protective effects; however, there are few studies on their effects on humans. This study investigates the effects of Lactobacillus complex capsule treatment on the aspirin-related small intestinal mucosal injury. Methods: This single-center, prospective, randomized controlled clinical trial included 69 patients using enteric-coated aspirin for >1 month between May and December 2019. After baseline magnetically controlled capsule endoscopy (MCCE), patients with aspirin-related small intestinal mucosal injury were randomly assigned (1:1) to receive enteric-coated aspirin and Lactobacillus complex capsules containing a combination of Lactobacillus rhamnosus I, Lactobacillus rhamnosus II, and Enterococcus faecium (probiotics group) or enteric-coated aspirin only (control group) for 2 months. After treatment, the patient underwent MCCE again. The primary outcome was the change in small intestinal mucosal injury scores from baseline to post-intervention. Results: Twenty-five patients in the probiotics group and 28 in the control group completed the trial. The decrease in small intestinal mucosal injury scores from baseline to post-intervention was significantly greater in the probiotics group than that in the control group (p < .001). The improvement rates of red spots and erosions in the probiotics group were higher compared with the control group (p = .027 and .022, respectively), and the improvement rate of small intestinal ulcers in the probiotics group was 75.0%; however, there was no improvement in the control group. Conclusion: Lactobacillus complex capsules can ameliorate aspirin-related small intestinal mucosal injury.

However, the reasons why some people develop serious lesions, whereas most only have minor, clinically irrelevant lesions are poorly understood. Areas covered in this review: A number of risk factors can be used to determine which patients are more likely to develop aspirin-associated GI bleeding, mainly in the upper GI tract; these include a previous GI ulcer, ulcer complications, dyspepsia, and concomitant drug therapy with non-steroidal anti-inflammatory drugs (NSAIDs) or clopidogrel. The possible role of Helicobacter pylori infection is also considered. What the reader will gain: Aspirin-induced GI damage can be reduced, and a number of strategies can be implemented to shift the risk-benefit ratio in favour of aspirin. Proton pump inhibitors are more effective than H(2)-receptor antagonists in preventing dyspeptic symptoms, peptic ulcers and bleeding ulcers in aspirin users. Although H. pylori infection may be a risk factor of aspirin-induced ulcer bleeding, the role of its eradication in the prevention of this outcome requires further investigation. Take home message: The individual assessment of the benefits and risks with aspirin, based on the underlying GI and cardiovascular risk factors, is the key to successful therapy. Understanding the effect of aspirin on colorectal cancer can also alter the risk-benefit ratio in at-risk aspirin users.

Abstract:Chronic use of non-steroidal anti-inflammatory drugs (NSAIDs) causes gastrointestinal adverse effects (GIAE), such as peptic ulcer disease (25%-50%), gastrointestinal bleeding (2%-4%), and perforation. In the outpatient setting, data has shown varying results regarding the use of prophylactic gastric acid suppression medication (GASM) in patients using chronic NSAIDs. We aim to study the association of prophylactic GASM in patients with or without GIAE.
