Abstract:Abstract Objective: To establish a comprehensive and accurate assessment model of periodontal alveolar bone loss based on panoramic images.Methods:A total of 640 panoramic images were included, and 3 experienced periodontal physicians marked the key points needed to calculate the degree of periodontal alveolar bone loss and the specific location and shape of the alveolar bone loss. A deep learning architecture based on UNet and YOLO-v4 was proposed to localize the tooth and key points, and the percentage and stageof periodontal alveolar bone loss were accurately calculated. The ability of the model to recognize these features was evaluated and compared with that of general dental practitioners.Results: The overall classification accuracy of the model was 0.77, and the performance of the model varied for different tooth positions and categories;model classification was generally more accurate than that of general practitioners.Conclusion: It is feasible to establish deep learning model forassessmentand staging radiographicperiodontal alveolar bone loss using two-stage architecture based on UNet and YOLO-v4.
Simulation of hydrological processes in the Simiyu River, tributary of Lake Victoria, Tanzania

Abstract:A spatially-distributed hydrologic model (WetSpa) is used to simulate hydrologic processes in the Simiyu River, a tributary of Lake Victoria, Tanzania. The model combines digital maps of topography, land-use and soil texture with observed daily meteorological time series to predict discharge hydrographs and spatial distribution of hydrologic parameters in the basin. The model was calibrated using 3 years of daily observed discharge measured at the mouth of the river at Lake Victoria. The estimated average travel time of the runoff to the outlet of the basin is about 2.4 days and a maximum of 8 days for the most remote areas. The model results show that the surface runoff and interflow provide, respectively, 38.6% and 61.4% of the total discharge, while the contribution of groundwater drainage is about nil. The absence of groundwater drainage is likely due to the high evaporative demand of the atmosphere, which accounts for about 90% of the total precipitation being lost by evapotranspiration. The annual water balance estimated with the model reveals that the total outflow to Lake Victoria is about 475 x 106 m3 per year, which occurs mainly in the wet seasons, i.e. from March to May and from November to January. The discharge volume produced by agricultural land amounts to about 43 x 106 m3 and may carry agrochemicals to Lake Victoria.

Abstract:AIM: The goal was to use a deep convolutional neural network to measure the radiographic alveolar bone level to aid periodontal diagnosis.MATERIALS AND METHODS: A deep learning (DL) model was developed by integrating three segmentation networks (bone area, tooth, cemento-enamel junction) and image analysis to measure the radiographic bone level and assign radiographic bone loss (RBL) stages. The percentage of RBL was calculated to determine the stage of RBL for each tooth. A provisional periodontal diagnosis was assigned using the 2018 periodontitis classification. RBL percentage, staging, and presumptive diagnosis were compared with the measurements and diagnoses made by the independent examiners.RESULTS: The average Dice Similarity Coefficient (DSC) for segmentation was over 0.91. There was no significant difference in the RBL percentage measurements determined by DL and examiners ( <math><mi>p</mi><mo>=</mo><mn>.65</mn></math> ). The area under the receiver operating characteristics curve of RBL stage assignment for stages I, II, and III was 0.89, 0.90, and 0.90, respectively. The accuracy of the case diagnosis was 0.85.CONCLUSIONS: The proposed DL model provides reliable RBL measurements and image-based periodontal diagnosis using periapical radiographic images. However, this model has to be further optimized and validated by a larger number of images to facilitate its application.

Abstract:Periodontitis is a serious oral disease that can lead to severe conditions such as bone loss and teeth falling out if left untreated. Diagnosis of radiographic bone loss (RBL) is critical for the staging and treatment of periodontitis. Unfortunately, the RBL diagnosis by examining the panoramic radiographs is time-consuming. The demand for automated image analysis is urgent. However, existing deep learning methods have limited performances in diagnosis accuracy and have certain difficulties in implementation. Hence, we propose a novel two-stage periodontitis detection convolutional neural network (PDCNN), where we optimize the detector with an anchor-free encoding that allows fast and accurate prediction. We also introduce a proposal-connection module in our detector that excludes less relevant regions of interests (ROIs), making the network focus on more relevant ROIs to improve detection accuracy. Furthermore, we introduced a large-scale, high-resolution panoramic radiograph dataset that captures various complex cases with professional periodontitis annotations. Experiments on our panoramic-image dataset show that the proposed approach achieved an RBL classification accuracy of 0.762. This result shows that our approach outperforms state-of-the-art detectors such as Faster R-CNN and YOLO-v4. We can conclude that the proposed method successfully improves the RBL detection performance. The dataset and our code have been released on GitHub. (https://github.com/PuckBlink/PDCNN).

Abstract:Background/purpose Artificial Intelligence (AI) can optimize treatment approaches in dental healthcare due to its high level of accuracy and wide range of applications. This study seeks to propose a new deep learning (DL) ensemble model based on deep Convolutional Neural Network (CNN) algorithms to predict tooth position, detect shape, detect remaining interproximal bone level, and detect radiographic bone loss (RBL) using periapical and bitewing radiographs. Materials and methods 270 patients from January 2015 to December 2020, and all images were deidentified without private information for this study. A total of 8000 periapical radiographs with 27,964 teeth were included for our model. AI algorithms utilizing the YOLOv5 model and VIA labeling platform, including VGG-16 and U-Net architecture, were created as a novel ensemble model. Results of AI analysis were compared with clinicians' assessments. Results DL-trained ensemble model accuracy was approximately 90% for periapical radiographs. Accuracy for tooth position detection was 88.8%, tooth shape detection 86.3%, periodontal bone level detection 92.61% and radiographic bone loss detection 97.0%. AI models were superior to mean accuracy values from 76% to 78% when detection was performed by dentists. Conclusion The proposed DL-trained ensemble model provides a critical cornerstone for radiographic detection and a valuable adjunct to periodontal diagnosis. High accuracy and reliability indicate model's strong potential to enhance clinical professional performance and build more efficient dental health services.

Deep Convolutional Neural Network for Automated Staging of Periodontal Bone Loss Severity on Bite-wing Radiographs: An Eigen-CAM Explainability Mapping Approach

Abstract:Periodontal disease is a significant global oral health problem. Radiographic staging is critical in determining periodontitis severity and treatment requirements. This study aims to automatically stage periodontal bone loss using a deep learning approach using bite-wing images. A total of 1752 bite-wing images were used for the study. Radiological examinations were classified into 4 groups. Healthy (normal), no bone loss; stage I (mild destruction), bone loss in the coronal third (< 15%); stage II (moderate destruction), bone loss is in the coronal third and from 15 to 33% (15-33%); stage III-IV (severe destruction), bone loss extending from the middle third to the apical third with furcation destruction (> 33%). All images were converted to 512 × 400 dimensions using bilinear interpolation. The data was divided into 80% training validation and 20% testing. The classification module of the YOLOv8 deep learning model was used for the artificial intelligence-based classification of the images. Based on four class results, it was trained using fivefold cross-validation after transfer learning and fine tuning. After the training, 20% of test data, which the system had never seen, were analyzed using the artificial intelligence weights obtained in each cross-validation. Training and test results were calculated with average accuracy, precision, recall, and F1-score performance metrics. Test images were analyzed with Eigen-CAM explainability heat maps. In the classification of bite-wing images as healthy, mild destruction, moderate destruction, and severe destruction, training performance results were 86.100% accuracy, 84.790% precision, 82.350% recall, and 84.411% F1-score, and test performance results were 83.446% accuracy, 81.742% precision, 80.883% recall, and 81.090% F1-score. The deep learning model gave successful results in staging periodontal bone loss in bite-wing images. Classification scores were relatively high for normal (no bone loss) and severe bone loss in bite-wing images, as they are more clearly visible than mild and moderate damage.

Abstract:Abstract In this study, a deep learning-based method for developing an automated diagnostic support system that detects periodontal bone loss in the panoramic dental radiographs is proposed. The presented method called DeNTNet not only detects lesions but also provides the corresponding teeth numbers of the lesion according to dental federation notation. DeNTNet applies deep convolutional neural networks(CNNs) using transfer learning and clinical prior knowledge to overcome the morphological variation of the lesions and imbalanced training dataset. With 12,179 panoramic dental radiographs annotated by experienced dental clinicians, DeNTNet was trained, validated, and tested using 11,189, 190, and 800 panoramic dental radiographs, respectively. Each experimental model was subjected to comparative study to demonstrate the validity of each phase of the proposed method. When compared to the dental clinicians, DeNTNet achieved the F1 score of 0.75 on the test set, whereas the average performance of dental clinicians was 0.69.

Application of Deep Learning in Differential Diagnosis of Ameloblastoma and Odontogenic Keratocyst Based on Panoramic Radiographs

Abstract:Objective To evaluate the accuracy of different convolutional neural networks (CNN),representative deep learning models,in the differential diagnosis of ameloblastoma and odontogenic keratocyst,and subsequently compare the diagnosis results between models and oral radiologists. Methods A total of 1000 digital panoramic radiographs were retrospectively collected from the patients with ameloblastoma (500 radiographs) or odontogenic keratocyst (500 radiographs) in the Department of Oral and Maxillofacial Radiology,Peking University School of Stomatology.Eight CNN including ResNet (18,50,101),VGG (16,19),and EfficientNet (b1,b3,b5) were selected to distinguish ameloblastoma from odontogenic keratocyst.Transfer learning was employed to train 800 panoramic radiographs in the training set through 5-fold cross validation,and 200 panoramic radiographs in the test set were used for differential diagnosis.Chi square test was performed for comparing the performance among different CNN.Furthermore,7 oral radiologists (including 2 seniors and 5 juniors) made a diagnosis on the 200 panoramic radiographs in the test set,and the diagnosis results were compared between CNN and oral radiologists. Results The eight neural network models showed the diagnostic accuracy ranging from 82.50% to 87.50%,of which EfficientNet b1 had the highest accuracy of 87.50%.There was no significant difference in the diagnostic accuracy among the CNN models (P=0.998,P=0.905).The average diagnostic accuracy of oral radiologists was (70.30±5.48)%,and there was no statistical difference in the accuracy between senior and junior oral radiologists (P=0.883).The diagnostic accuracy of CNN models was higher than that of oral radiologists (P<0.001). Conclusion Deep learning CNN can realize accurate differential diagnosis between ameloblastoma and odontogenic keratocyst with panoramic radiographs,with higher diagnostic accuracy than oral radiologists.
An End-to-end Entangled Segmentation and Classification Convolutional Neural Network for Periodontitis Stage Grading from Periapical Radiographic Images

Abstract:Periodontitis is a biofilm-related chronic inflammatory disease characterized by gingivitis and bone loss in the teeth area. Approximately 61 million adults over 30 suffer from periodontitis (42.2%), with 7.8% having severe periodontitis in the United States. The measurement of radiographic bone loss (RBL) is necessary to make a correct periodontal diagnosis, especially if the comprehensive and longitudinal periodontal mapping is unavailable. However, doctors can interpret X-rays differently depending on their experience and knowledge. Computerized diagnosis support for doctors sheds light on making the diagnosis with high accuracy and consistency and drawing up an appropriate treatment plan for preventing or controlling periodontitis. We developed an end-to-end deep learning network HYNETS (Hybrid NETwork for pEriodoNTiTiS STagES from radiograpH) by integrating segmentation and classification tasks for grading periodontitis from periapical radiographic images. HYNETS leverages a multi-task learning strategy by combining a set of segmentation networks and a classification network to provide an end-to-end interpretable solution and highly accurate and consistent results. HYNETS achieved the average dice coefficient of 0.96 and 0.94 for the bone area and tooth segmentation and the average AUC of 0.97 for periodontitis stage assignment. Additionally, conventional image processing techniques provide RBL measurements and build transparency and trust in the model's prediction. HYNETS will potentially transform clinical diagnosis from a manual time-consuming, and error-prone task to an efficient and automated periodontitis stage assignment based on periapical radiographic images.

Abstract:Abstract Background This study aims to propose the combinations of image processing and machine learning model to segment the maturity development of the mandibular premolars using a Keras-based deep learning convolutional neural networks (DCNN) model. Methods A dataset consisting of 240 images (20 images per stage per sex) of retrospect digital dental panoramic imaging of patients between 5 and 14 years of age was retrieved. In image preprocessing, abounding box with a dimension of 250 × 250 pixels was assigned to the left mandibular first (P1) and second (P2) permanent premolars. The implementation of dynamic programming of active contour (DP-AC) and convolutions neural network on images that require the procedure of image filtration using Python TensorFlow and Keras libraries were performed in image segmentation and classification, respectively. Results Image segmentation using the DP-AC algorithm enhanced the visibility of the image features in the region of interest while suppressing the image's background noise. The proposed model has an accuracy of 97.74%, 96.63% and 78.13% on the training, validation, and testing set, respectively. In addition, moderate agreement (Kappa value = 0.58) between human observer and computer were identified. Nonetheless, a robust DCNN model was achieved as there is no sign of the model's over-or under-fitting upon the learning process. Conclusions The application of digital imaging and deep learning techniques used by the DP-AC and convolutions neural network algorithms to segment and identify premolars provides promising results for semi-automated forensic dental staging in the future.

Abstract:This retrospective study is aimed at developing a web‐based artificial intelligence (AI) software (DiagnoCat) for periodontal bone loss detection on panoramic radiographs and evaluating the model's performance by comparing it with clinicians' results. Separate models are trained for tooth and periodontal bone loss detection. The first model's objective was to detect teeth, segmenting their masks, and to define their numbering and developed with Mask R‐CNN using pretrained ResNet‐101 as a backbone. The second model was based on Cascade R‐CNN architecture and used for bone loss prediction. Around 100 radiographs are evaluated by three clinicians regarding tooth identification and periodontal bone loss, separately. Ground truth is determined by the consensus and model's performance is evaluated with kappa, precision, recall, and F‐score statistics. For tooth conditions, the overall F‐score, accuracy, and Cohen's kappa coefficients were found to be 0.948, 0.977, and 0.933 for the binary, and 0.992, 0.988, and 0.961 for the multiclass results. For bone loss detection, the overall F‐score, accuracy, and Cohen's kappa coefficients were found to be 0.985, 0.980, and 0.956 for the binary, and 0.996, 0.993, and 0.974 for the multiclass results. The results of this study suggest that the use of a web‐based AI software (DiagnoCat) can be beneficial in detecting periodontal bone loss on panoramic radiographs.

Detection of Proximal Caries Lesions on Bitewing Radiographs Using Deep Learning Method

Abstract:This study aimed to evaluate the validity of a deep learning-based convolutional neural network (CNN) for detecting proximal caries lesions on bitewing radiographs. A total of 978 bitewing radiographs, 10899 proximal surfaces, were evaluated by two endodontists and a radiologist, of which 2719 surfaces were diagnosed and annotated with proximal caries and 8180 surfaces were sound. The data were randomly divided into two datasets, with 818 bitewings in the training and validation dataset and 160 bitewings in the test dataset. Each annotation in the test set was then classified into 5 stages according to the extent of the lesion (E1, E2, D1, D2, D3). Faster R-CNN, a deep learning-based object detection method, was trained to detect proximal caries in the training and validation dataset and then was assessed on the test dataset. The diagnostic accuracy, sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and receiver operating characteristic curve (ROC) were calculated. The performance of the network in the overall and different stages of lesions was compared with that of postgraduate students on the test dataset. A total of 388 carious lesions and 1435 sound surfaces were correctly identified by the neural network; hence, the accuracy was 0.87. Furthermore, 27.6% of lesions went undetected, and 7% of sound surfaces were misdiagnosed by the neural network. The sensitivity, specificity, PPV and NPV of the neural network were 0.72, 0.93, 0.77 and 0.91, respectively. In contrast with the network, 52.8% of lesions went undetected by the students, yielding a sensitivity of only 0.47. The F1-score of the students was 0.57, while the F1-score of the network was 0.74 despite the accuracy of 0.82. A significant difference in the sensitivity was found between the model and the postgraduate students when detecting different stages of lesions (P<0.05). For early lesions which limited in enamel and the outer third of dentin, the neural network had sensitivities all above or at 0.65, while students showed sensitivities below 0.40. From our results, we conclude that the CNN may be an assistant in detecting proximal caries on bitewings.
Evaluation of accuracy of deep learning and conventional neural network algorithms in detection of dental implant type using intraoral radiographic images: A systematic review and meta-analysis

Abstract:Statement of problem With the growing importance of implant brand detection in clinical practice, the accuracy of machine learning algorithms in implant brand detection has become a subject of research interest. Recent studies have shown promising results for the use of machine learning in implant brand detection. However, despite these promising findings, a comprehensive evaluation of the accuracy of machine learning in implant brand detection is needed. Purpose The purpose of this systematic review and meta-analysis was to assess the accuracy, sensitivity, and specificity of deep learning algorithms in implant brand detection using 2-dimensional images such as from periapical or panoramic radiographs. Material and methods Electronic searches were conducted in PubMed, Embase, Scopus, Scopus Secondary, and Web of Science databases. Studies that met the inclusion criteria were assessed for quality using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) tool. Meta-analyses were performed using the random-effects model to estimate the pooled performance measures and the 95% confidence intervals (CIs) using STATA v.17. Results Thirteen studies were selected for the systematic review, and 3 were used in the meta-analysis. The meta-analysis of the studies found that the overall accuracy of CNN algorithms in detecting dental implants in radiographic images was 95.63%, with a sensitivity of 94.55% and a specificity of 97.91%. The highest reported accuracy was 99.08% for CNN Multitask ResNet152 algorithm, and sensitivity and specificity were 100.00% and 98.70% respectively for the deep CNN (Neuro-T version 2.0.1) algorithm with the Straumann SLActive BLT implant brand. All studies had a low risk of bias. Conclusions The highest accuracy and sensitivity were reported in studies using CNN Multitask ResNet152 and deep CNN (Neuro-T version 2.0.1) algorithms.

Abstract:Determining the peri-implant marginal bone level on radiographs is challenging because the boundaries of the bones around implants are often unclear or the heights of the buccal and lingual bone levels are different. Therefore, a deep convolutional neural network (CNN) was evaluated for detecting the marginal bone level, top, and apex of implants on dental periapical radiographs. An automated assistant system was proposed for calculating the bone loss percentage and classifying the bone resorption severity. A modified region-based CNN (R-CNN) was trained using transfer learning based on Microsoft Common Objects in Context dataset. Overall, 708 periapical radiographic images were divided into training (n = 508), validation (n = 100), and test (n = 100) datasets. The training dataset was randomly enriched by data augmentation. For evaluation, average precision, average recall, and mean object keypoint similarity (OKS) were calculated, and the mean OKS values of the model and a dental clinician were compared. Using detected keypoints, radiographic bone loss was measured and classified. No statistically significant difference was found between the modified R-CNN model and dental clinician for detecting landmarks around dental implants. The modified R-CNN model can be utilized to measure the radiographic peri-implant bone loss ratio to assess the severity of peri-implantitis.

Abstract:Background Dental caries is one of the most prevalent conditions in dentistry worldwide. Early identification and classification of dental caries are essential for effective prevention and treatment. Panoramic dental radiographs are commonly used to screen for overall oral health, including dental caries and tooth anomalies. However, manual interpretation of these radiographs can be time-consuming and prone to human error. Therefore, an automated classification system could help streamline diagnostic workflows and provide timely insights for clinicians. Methods This article presents a deep learning-based, custom-built model for the binary classification of panoramic dental radiographs. The use of histogram equalization and filtering methods as preprocessing techniques effectively addresses issues related to irregular illumination and contrast in dental radiographs, enhancing overall image quality. By incorporating three separate panoramic dental radiograph datasets, the model benefits from a diverse dataset that improves its training and evaluation process across a wide range of caries and abnormalities. Results The dental radiograph analysis model is designed for binary classification to detect the presence of dental caries, restorations, and periapical region abnormalities, achieving accuracies of 97.01%, 81.63%, and 77.53%, respectively. Conclusions The proposed algorithm extracts discriminative features from dental radiographs, detecting subtle patterns indicative of tooth caries, restorations, and region-based abnormalities. Automating this classification could assist dentists in the early detection of caries and anomalies, aid in treatment planning, and enhance the monitoring of dental diseases, ultimately improving and promoting patients' oral healthcare.

Abstract:Background Changes in healthy and inflamed pulp on periapical radiographs are traditionally so subtle that they may be imperceptible to human experts, limiting its potential use as an adjunct clinical diagnostic feature. Aim This study aimed to investigate the feasibility of an image‐analysis technique based on the convolutional neural network (CNN) to detect irreversible pulpitis in primary molars on periapical radiographs (PRs). Design This retrospective study was performed in two health centres. Patients who received indirect pulp therapy at Peking University Hospital for Stomatology were retrospectively identified and randomly divided into training and validation sets (8:2). Using PRs as input to an EfficientNet CNN, the model was trained to categorise cases into either the success or failure group and externally tested on patients who presented to our affiliate institution. Model performance was evaluated using sensitivity, specificity, accuracy and F1 score. Results A total of 348 PRs with deep caries were enrolled from the two centres. The deep learning model achieved the highest accuracy of 0.90 (95% confidence interval: 0.79–0.96) in the internal validation set, with an overall accuracy of 0.85 in the external test set. The mean greyscale value was higher in the failure group than in the success group (p = .013). Conclusion The deep learning‐based model could detect irreversible pulpitis in primary molars with deep caries on PRs. Moreover, this study provides a convenient and complementary method for assessing pulp status.

Abstract:Gingival inflammation grade serves as a well-established index in periodontitis. The aim of this study was to develop a deep learning network utilizing a novel feature extraction method for the automatic assessment of gingival inflammation. T-distributed Stochastic Neighbor Embedding (t-SNE) was utilized for dimensionality reduction. A convolutional neural network (CNN) model based on DenseNet was developed for the identification and evaluation of gingival inflammation. To enhance the performance of the deep learning (DL) model, a novel teeth removal algorithm was implemented. Additionally, a Grad-CAM + + encoder was applied to generate heatmaps for computer visual attention analysis. The mean Intersection over Union (MIoU) for the identification of gingivitis was 0.727 ± 0.117. The accuracy rates for the five inflammatory degrees were 77.09%, 77.25%, 74.38%, 73.68% and 79.22%. The Area Under the Receiver Operating Characteristic (AUROC) values were 0.83, 0.80, 0.81, 0.81 and 0.84, respectively. The attention ratio towards gingival tissue increased from 37.73% to 62.20%, and within 8 mm of the gingival margin, it rose from 21.11% to 38.23%. On the gingiva, the overall attention ratio increased from 51.82% to 78.21%. The proposed DL model with novel feature extraction method provides high accuracy and sensitivity for identifying and grading gingival inflammation.
