Abstract:Objective This study aimed to explore the preferences and influencing factors of self-management behaviors in patients with recurrent gout and provide a theoretical basis for developing targeted strategies to improve self-management preferences. Methods A total of 10 patients with recurrent gout were recruited from the gout specialist outpatient clinic at a tertiary hospital in Shenyang, Liaoning Province, China. Semi-structured interviews were conducted with these patients, and the Kawakita Jiro (KJ) method was used to analyze the interview data. Results After four rounds of screening by five researchers, 35 codes were selected from an initial 132. After three rounds of discussion and induction, the KJ method identified seven domains of self-management behavior preferences in patients with recurrent gout: 1) extensive knowledge of gout, yet difficulty in distinguishing between accurate and inaccurate information; 2) a passive attitude of "no pain, no management"; 3) the challenge of changing entrenched daily habits; 4) the optimistic but unrealistic belief of "self-delusion"; 5) a tendency to seek medical attention late due to hopelessness of cure and familiarity with recurrences; 6) preference for analgesics for gout recurrences while neglecting long-term urate-lowering therapy (ULT); and 7) gout-related stigma. Conclusion The results of this study showed that the self-management behavior of patients with recurrent gout could be improved, especially in the aspects of medical seeking behavior, medication compliance, daily management and emotional management. At the same time, we found that gout stigma, difficulty in distinguishing true and false knowledge of gout and negative attitude of "ignoring pain" were significantly associated with self-management behavior.

Illness perception and treatment experience in patients with gout: a descriptive qualitative study

Abstract:Objective: To explore the illness perception and treatment experience of gout patients in China. Methods: A descriptive qualitative approach was used. In-depth semi-structured individual interviews were conducted among 18 adults with gout between August 2020 and December 2020. The Common-Sense Model of Self-Regulation was used as the conceptual framework for developing interview guide and data analysis. Interviews were audio recorded, transcribed, independently coded, and analyzed for themes. Results: The 18 gout patients aged 23-77 years old, with 16 being male. Four themes were generated to reflect the experience of patients with gout: limited knowledge of gout, various but inadequate sources of gout knowledge, multifaceted influences of gout, and seeking for diversified disease treatment methods. Conclusion: There is a gap between patients' anticipation and practical service that is closely linked with subsequent poor gout management among the participants. The findings indicate a need of strengthening health education about gout by considering building nurse-led support groups and developing mobile health applications, as well as addressing the long-term influence of the Chinese alcoholic drinking culture to promote patients' effective disease management. Key points: • This qualitative study fills a knowledge gap on illness perception and treatment experience among Chinese gout patients, exploring how these patients perceived gout and manage gout. • The gout patients in this study reported disparity between personal anticipation and healthcare service that largely contributes to their poor gout management. • The Chinese alcoholic drinking culture embedded in personal, professional, and social activities posed extra difficulties and challenges on modifying lifestyle for gout patients that needs to be addressed for promoting gout management among this group.
General practitioners' perspectives on the management of gout: a qualitative study

Abstract:Background: Many quantitative studies globally have identified suboptimal management of gout. Purpose: To explore management of gout from the perspective of general practitioners (GPs), while identifying the barriers and possible strategies for improvement. Study design: This descriptive qualitative study used semistructured interviews with 14 purposely selected GPs from four separate general practices in Townsville. The questions focused on management strategies, practicalities in managing gout, barriers and possible strategies to improve management. Results: Indomethacin was commonly reported to be used in acute gout with progression to allopurinol after the acute stage had subsided. There were differences with the initial allopurinol dose and follow-up periods. GPs reported lack of patient adherence to allopurinol and lifestyle modifications, mainly due to lack of education. Most suggested the need for allied health input and improved patient education. Conclusions: Tailor-made plan in terms of education and lifestyle advice could help adherence to gout management.

Abstract:Background: The prevalence of gout has increased in the Western societies due to ageing and increasing BMI. Recently, lifestyle and dietary factors have been linked in epidemiological studies with an alteration of the risk of gout; however, there remains a lack of data on patient knowledge of these factors. The purpose of this survey-based study was to determine the knowledge of gout and its treatment both in the community and specialist care settings. Methods: Participants were recruited from a hospital rheumatology outpatient department, consumer organization and a random sample of participants from a population-based cohort who had self-reported gout in South Australia. Participants completed a survey regarding basic demographics, the Single Item Literacy Screener, use of medication and diet for treatment of their gout and knowledge of gout. Results: Seventy-four people were recruited (87% male) with a mean age of 66 years (range 35-88). The mean duration of gout was 16.6 years (range 0-60). On screening with SILS, 19.0% were identified as having limited reading ability. Most gout was managed by the family practitioner (81.1%) and/or rheumatologist (18.9%). In regard to current gout medications, 52.7% were taking allopurinol, 17.6% colchicine, 9.5% non-steroidal anti-inflammatory drugs, 6.8% prednisolone and 5.4% herbal preparations. For further information regarding gout, participants would most commonly approach their general practitioner (85.1%). Most participants correctly identified certain triggers to gout attacks and almost half of participants (41.9%) reported that they had altered their diet due to gout. Conversely, participants often incorrectly identified common risk or protective factors for gout. Conclusion: Gout remains a common, yet undertreated, chronic condition. Our study highlights a lack of knowledge amongst patients of risk and protective factors in relation to gout. The increasing prevalence of gout within the population indicates a need to improve education and understanding among those with the condition.

Abstract:Introduction Gout management remains suboptimal despite safe and effective urate‐lowering therapy. Self‐monitoring of urate may improve gout management, however, the acceptability of urate self‐monitoring by people with gout is unknown. The aim of this study was to explore the experiences of urate self‐monitoring in people with gout. Methods Semistructured interviews were conducted with people taking urate‐lowering therapy (N = 30) in a 12‐month trial of urate self‐monitoring in rural and urban Australia. Interviews covered the experience of monitoring and its effect on gout self‐management. Deidentified transcripts were analysed thematically. Results Participants valued the ability to self‐monitor and gain more understanding of urate control compared with the annual monitoring ordered by their doctors. Participants indicated that self‐monitoring at home was easy, convenient and informed gout self‐management behaviours such as dietary modifications, hydration, exercise and medication routines. Many participants self‐monitored to understand urate concentration changes in response to feeling a gout flare was imminent or whether their behaviours, for example, alcohol intake, increased the risk of a gout flare. Urate concentrations were shared with doctors mainly when they were above target to seek management support, and this led to allopurinol dose increases in some cases. Conclusion Urate self‐monitoring was viewed by people with gout as convenient and useful for independent management of gout. They believed self‐monitoring achieved better gout control with a less restricted lifestyle. Urate data was shared with doctors at the patient's discretion and helped inform clinical decisions, such as allopurinol dose changes. Further research on implementing urate self‐monitoring in routine care would enable an evaluation of its impact on medication adherence and clinical outcomes, as well as inform gout management guidelines. Patient or Public Contribution One person with gout, who was not a participant, was involved in the study design by providing feedback and pilot testing the semistructured interview guide. In response to their feedback, subsequent modifications to the interview guide were made to improve the understandability of the questions from a patient perspective. No additional questions were suggested.

Abstract:Objective: To explore the perceptions of patients toward the impacts of gout, in an Australian context. Methods: Semi-structured interviews were conducted with Australian patients with gout (n = 17). Interviews focused on the impacts of gout on patients' day-to-day lives. The interviews were audio-recorded, transcribed verbatim and analyzed thematically by 2 independent reviewers. Results: Patients with gout reported variable but significant impacts of gout on their daily lives. Patients were primarily concerned with the pain and poor mobility associated with gout flares. Participants also reported significant social impacts related to their inability to attend events and the need for family and friends to look after them. Productivity was also negatively affected due to pain and poor mobility, and emotional wellbeing was reduced both by the symptoms of gout flares and anxiety between flares. Diet, daily planning, hobbies, exercise, sleep and selection of attire were also reported as being impacted by their gout. Conclusions: In Australia, gout has significant and preventable impacts beyond the physical symptoms of gout flares, across a variety of domains. By identifying patients' key concerns, this can potentially aid clinicians to tailor gout education and encourage medication adherence, in order to improve gout management.
Determinants of self‐management behavior in gout: a scoping review

Abstract:Objective This study aimed to identify modifiable determinants of self‐management behavior in gout. Methods Four databases (MEDLINE, Embase, PsycINFO and CINAHL) were searched using terms related to gout, self‐management and determinants of behavior as described in the Theoretical Domains Framework (TDF). Two reviewers independently selected relevant studies via screening of title/abstract and full text. Thematic synthesis was performed for qualitative data; quantitative data was summarized using cross‐tabulation displaying the investigated associations of determinants with self‐management behavior. The TDF facilitated identification and grouping of determinants. Results From 2087 unique articles found, 56 studies were included in this review, of which 27 qualitative and 29 quantitative studies. Eight themes were identified: knowledge and skills for self‐management, acceptance of disease, beliefs about necessity of self‐management to improve gout‐related health, resistance and reluctance for medication adherence and dietary alteration/changes, negative emotions influencing self‐management, social support and interactions, environmental context, and self‐regulation of behavior. Quantitative determinants associated with self‐management behavior, predominantly medication adherence, were mapped to 12 of the 14 domains of the TDF. No determinants regarding skills and goals have been identified in quantitative research. Conclusion Intervention targets for self‐management behavior in gout mainly included determinants related to knowledge, implicit and explicit beliefs and attitudes, the environmental context and resources, (social) support and reinforcement.

Abstract:Background: Both acute and long-term management of gout in primary care in UK is reportedly suboptimal1, with patients rarely being offered lifestyle advice or urate Lowering Therapy (ULT). Further, ULT is often not titrated according to the patient‘s serum uric acid (sUA) levels, so that they fail to achieve target sUA levels. Objectives: To audit and evaluate the management of gout in patients in a primary care setting against the current guidelines treat to target guidelines set by the British Society of Rheumatology (BSR)2. Methods: Audit criteria were derived from the latest BSR gout guideline (Hui et al; 2017). A randomised sample of adult patients with a read code for the diagnosis of gout from Jan 2006-Jan 2018 was chosen from six large general practices in Leicestershire County of the United Kingdom. The data collected included demographics, provision of patient information, management of acute attacks and prophylactic treatment, screening of appropriate co-morbidities, dosing of urate-lowering therapy (ULT) and titration of doses against measurement of uric acid levels. Results: Data was obtained for 861 patients The mean age was 60 years and 91% were male, 21.5% were recorded as being provided with written information about gout and 60.5% of patients were treated with NSAIDs and COXIBs for acute attacks of gout. When colchicine was prescribed to patients, 71% had no dose recorded in their clinical records. 323 (37.5%) of patients were prescribed a ULT and the recorded starting dose of allopurinol was 100mg daily for 73.8%. Titration of subsequent allopurinol doses was recorded in only 21% of patients. 539 patients (62.6%) had no record of a serum urate level check after starting ULT. Conclusion: Clinical records indicate that the management of gout by UK General Practitioners in Primary Care is suboptimal in concordance with the BSR guidelines. It was clear that general practices did not employ the treat to target strategy. There is a clear need for increased GP awareness and adherence to the BSR guidelines in order to optimise deficient areas of care, particularly in patient education, initiation and titration of ULT and monitoring of serum urate levels in gout patients. Appropriate patient recording templates are needed so that key information is captured during a patient consultation in order to enable medicines optimisation for those with gout. Abstract:Urease is an inducible virulence factor of uropathogenic Proteus mirabilis. Although eight contiguous genes necessary for urease activity have been cloned and sequenced, the transcriptional organization and regulation of specific genes within the Proteus gene cluster has not been investigated in detail. The first gene, ureR, is located 400 bp upstream and is oriented in the direction opposite the other seven genes, ureDABCEFG. The structural subunits of urease are encoded by ureABC. Previously, UreR was shown to contain a putative helix-turn-helix DNA-binding motif 30 residues upstream of a consensus sequence which is a signature for the AraC family of positive regulators; this polypeptide is homologous to other DNA-binding regulatory proteins. Nested deletions of ureR linked to either ureD-lacZ or ureA-lacZ operon fusions demonstrated that an intact ureR is required for urea-induced synthesis of LacZ from either ureA or ureD and identified a urea-regulated promoter in the ureR-ureD intergenic region. However, lacZ operon fusions to fragments encompassing putative promoter regions upstream of ureA and ureF demonstrated that no urea-regulated promoters occur upstream of these open reading frames; regions upstream of ureR, ureE, and ureG were not tested. These data suggest that UreR acts as a positive regulator in the presence of urea, activating transcription of urease structural and accessory genes via sequences upstream of ureD. To address the role of the nonstructural regulatory and accessory genes, we constructed deletion, cassette, and linker insertion mutations throughout the ure gene cluster and determined the effect of these mutations on production and regulation of urease activity in Escherichia coli. Mutations were obtained, with locations determine by DNA sequencing, in all genes except ureA and ureE. In each case, the mutation resulted in a urease-negative phenotype.
Factors influencing medication adherence in patients with gout: A descriptive correlational study

Abstract:AIMS AND OBJECTIVES: To examine the factors influencing adherence to urate-lowering therapy in patients with gout in Singapore.BACKGROUND: Gout is the most common type of chronic inflammatory arthritis. Urate-lowering therapy is used to treat gout by reducing serum uric acid levels. However, adherence to urate-lowering therapy among patients remains poor. To date, there have been no available studies based on a conceptual framework that examined factors influencing medication adherence in patients with gout.DESIGN: Cross-sectional, descriptive correlational study.METHODS: A convenience sample of outpatients (n = 108) was recruited between October 2014-January 2015 from a tertiary hospital in Singapore. Outcomes were measured by relevant valid and reliable instruments. Descriptive statistics and parametric tests including multiple linear regression were used to analyse the data.RESULTS: Although 44.4% of the participants were high adherers to urate-lowering therapy, the mean adherence level was moderate. Significant differences in medication adherence scores were found among the subgroups of gender, ethnicity, marital status, employment status and presence of comorbidity. Medication adherence was positively significantly correlated with age, number of comorbidities and beliefs about medicines. Linear regression showed that higher level of beliefs about medicines, presence of comorbidity and being married were factors positively influencing medication adherence.CONCLUSIONS: This study revealed moderate adherence to urate-lowering therapy in patients with gout in Singapore, indicating the need for strategies to improve adherence by considering its main influencing factors. Future research should be conducted to develop interventions targeted at modifying patients' beliefs about medicines in order to improve medication adherence.RELEVANCE TO CLINICAL PRACTICE: Findings from this study allow healthcare providers to quickly and easily identify patients who may have low adherence. Nurses should take the lead in educating patients on the mechanism of urate-lowering therapy and highlight the importance of adhering to it.

Abstract:Congenital blepharophimosis (Phimosis Palpebrum, von Ammon, 1841)1is described as a general diminution of the palpebral aperture in all its dimensions but with the eyelids normally differentiated.2It is exemplified by Atkinson's report3of an adult woman with a palpebral aperture 8 mm. long and 3 mm. wide. Individual face-to-face semi-structured interviews were conducted with patients recruited from West China Hospital in Sichuan, China. The qualitative data were transcribed verbatim and analyzed by thematic analysis. Results: Ten male patients with gout were included in the final analysis. Instrumental, psychological, and relational support emerged as themes describing the needs of patients for gout self-management support. The instrumental support included health education, hospital registration, setting reminders, and shopping. Psychological support referred to helping patients mitigate various negative emotions. Relational support included interactions with healthcare providers and fellow patients. Conclusion: The present findings indicate that healthcare providers or companies should develop mHealth APPs for gout self-management based on the needs of patients. Keywords: gout, self-management, mobile applications, China Gout is the most common type of inflammatory arthritis. 1 Although it is more prevalent among men aged >40 years (accounting for 90.0–95.0% of cases), it also occurs in women after menopause. 2 A meta-analysis reported that the pooled prevalence of gout in mainland China was 1.1% (95% confidence interval: 0.8–1.5%), which is lower than that observed in Taiwan Province (6.2%). 3,4 The incidence of gout also shows an increasing trend in China. 4 Most patients require self-management with lifelong urate-lowering therapy (ULT), self-medication following the occurrence of flare-warning symptoms, management of chronic comorbidities, and lifestyle adjustment. 2,5–7 However, evidence showed that 77.9% of patients with gout had poor self-management, with only 15.4% achieving clinical remission. 8,9 With the increasing use of electronic devices, mobile health (mHealth) applications (APPs) are attracting considerable research attention as an effective approach to promoting self-management for patients with chronic diseases. 10,11 The NEWZOO 2020 Global Mobile Market Report 12 reported that the number of smartphone users reached 3.6 billion in 2020. China accounted for more than a quarter of these users (nearly 850 million). By 2023, there will be 4.1 billion smartphone users globally, increasing between 2018 and 2023 at a compound annual growth rate of +6.2%. Furthermore, the number of downloads of mHealth APPs nearly doubled between 2013 and 2016. According to Research2Guidance, 2.6 billion users were expected to download mHealth APPs using mobile phones or tablets by 2020. A survey found that 86.1% of patients with gout were willing to use mHealth APPs for gout self-management. 13 Empirical evidence reported that using disease-related APPs could significantly improve disease-related knowledge, diet and medication adherence, and perceived social support, as well as reduce anxiety and depression, and ultimately improve self-management and health outcomes. 14–16 In 2019, we reported on existing APPs related to gout in China. 17 However, to the best of our knowledge, there is no related study investigating the development of gout-related APPs in China. Abstract:Background: Improved outcomes for patients with gout are associated with the control of urate levels; yet, less than 40% of patients in the UK are currently offered urate lowering therapy, and it is typically offered without titration to target. Supported self-management has been shown to benefit patients with chronic health conditions; therefore, we aimed to determine whether a supported gout self-management approach, incorporating treatment-to-target urate, helped participants reach target urate levels. Methods: In this randomised controlled feasibility study, conducted in one hospital in Edinburgh, UK, we included patients (aged ≥18 years) with gout and a physician recommendation for initiation or escalation of urate lowering therapy. We randomised participants in a 2:1 ratio to a supported self-management group or a usual care group using the GoutSMART smartphone app. Participants in the self-management group were given a urate self-testing meter and received direct advice from clinicians on escalation of urate lowering therapy through the app. Participants were identified following referral to rheumatology, or using the Scottish Health Research Register, and they were screened and offered a detailed management plan before randomisation. Participants in the usual care group had a limited version of the app, which only allowed it to function as a health diary, and their gout management plan was implemented by their general practitioner. The primary outcome was the percentage of participants achieving a urate target of 0·30 mmol/L or less at 24 weeks, and analysis was by intent to treat. The trial was registered with ClinicalTrials.gov, NCT03274063, and there is an extension study ongoing. There was no masking of participants or assessors. Findings: Between April 5, 2019, and March 19, 2020, 60 (65%) of 92 patients screened were enrolled to the study. The mean age was 52.8 years (SD 14.6); 56 (93%) of the participants were male, and 4 (7%) were female. 58 (97%) of participants were White. 40 participants were assigned to the self-supported management group and 20 participants were assigned to the usual care group. A urate target of 0·30 mmol/L or less at 24 weeks was reached by 29 (73%) participants in the supported self-management group compared with 3 (15%) participants in the usual care group (risk difference 0·58 [95% CI 0·37-0·78]; p<0·0001). 90% of participants completed the study with no difference in the drop-out rate or adverse events between the two groups. Interpretation: Supported self-management of gout results in substantially improved attainment of urate targets compared with usual care, and it is well tolerated. Larger trials will be needed to fully evaluate the clinical and cost-effectiveness of this approach. Funding: Edinburgh & Lothians Health Foundation.
Gout management and outcomes during established COVID-19 pandemic in 2020-2021: a cross-sectional Internet survey

Abstract:Objective: To assess the management of gout in established COVID-19 pandemic. Methods: We assessed medication use, health care utilization, gout-specific health-related quality of life (HRQOL), psychological distress using Patient Health Questionnaire-4 (PHQ-4), resilience, illness perception, and health literacy in people with physician-diagnosed self-reported gout in established COVID-19 pandemic in a cross-sectional Internet survey. Results: Among the 130 survey respondents with gout, the mean age was 62.8 years, 65% were male, 83% were White, 59% were prescribed urate-lowering therapy (ULT), and health literacy was adequate in 80%. A third of survey respondents reported more difficulty with their gout management since September 2020. Gout-specific HRQOL deficits were evident. Moderate-severe psychological distress was seen in 22%, and resilience score was 6.5 [standard deviation (SD), 1.9; range, 0-8]. Adjusted for age and sex, compared with no/mild psychological distress, moderate-severe psychological distress was associated with significantly higher odds ratio (OR; 95% confidence interval) of more difficulty with (1) getting health care for gout in clinic, 3.7 (1.0, 13.2); emergency room/urgent care, 8.1 (1.4, 45.0); and in the hospital, 9.8 (1.6, 59.6); (2) getting gout flares treated, 6.6 (1.6, 26.8); (3) avoiding gout complications, 4.5 (1.2, 16.7); and (4) daily activities at home, 4.2 (1.3, 14.1), and performing work, 4.1 (1.2, 13.6). Conclusion: Respondents with gout reported health care gaps, low rates of ULT prescription, high psychological distress, and HRQOL deficits during established COVID-19 pandemic. Moderate-severe psychological distress was associated with difficulties in health care access and gout management. Interventions to address these challenges in gout management are needed.

Abstract:Background: Diabetes Self-Management Education (DSME) is a fundamental aspect of diabetes care, but no standard program exists in Thailand. Understanding current patterns of illness perceptions (concerns) and self-management practices among patients with diabetes in Thailand is vital to develop culturally tailored DSME programs. This study sought to explore the association between reported self-management practices and diabetes perceptions on glycemic control among patients with type 2 diabetes in Chiang Mai Province, Thailand. Specifically, the study examined whether the association between illness perceptions and diabetes control was mediated by self-management. Methods: This was a cross-sectional study conducted among type 2 diabetes patients on outpatient care and follow-up in four districts hospitals in Chiang Mai, Thailand. Illness perceptions was measured by the Brief Illness Perceptions Questionnaire (BIPQ). Self-management practices were measured by Summary Diabetes Self-Care activities (SDSCA). For illness perceptions and self-management practices, patients were classified into two groups, high level and low level based on the median values. Univariate and multivariable analyses were done to determine the association between the determinant factors: self-care practices and illness perceptions and the outcome of interest- good glycemic control (HbA1c < 7%). Results: Of the 200 participants recruited into the study, 180 completed the questionnaire. Only 35% of participants had good glycemic control (HBA1c < 7.0). Both illness perceptions and self-management practices were independently linked to glycemic control. Among illness perceptions, a sense of personal control was strongly associated with good glycemic control (p = 0.01). For self-management, appropriate diet (p = 0.03) and medication adherence (p = 0.05) were associated with good glycemic control. After adjustments for key baseline characteristics, patients with high levels of illness perceptions were less likely to achieve glycemic control (OR 0.55, 95% CI 0.29 to 1.14, p = 0.11) and those with high level of self-management were more likely to achieve glycemic control (OR 2.11, 95% CI 1.04 to 4.30, p = 0.04). The effect size for illness perception attenuated when further adjusted for levels of self-management (OR 0.88, 95% CI 0.39 to 1.96, p = 0.75) while the effect size for self-management and glycemic control did not materially change (OR 2.30, 95% CI 1.06 to 5.02, p = 0.04). Conclusion: Illness perceptions and self-management practices are associated with glycemic control. Future culturally tailored interventions in Thailand aimed at improving glycemic should focus on personal control, improving diet and treatment adherence as these are more likely to help improve diabetes control as demonstrated in this study.
