Abstract:Experimental Biology and Medicine, Ahead of Print. Arboviruses cause millions of infections each year; however, only limited options are available for treatment and pharmacological prevention. Mosquitoes are among the most important vectors for the transmission of several pathogens to humans. Despite advances, the sampling, viral detection, and control methods for these insects remain ineffective. Challenges arise with the increase in mosquito populations due to climate change, insecticide resistance, and human interference affecting natural habitats, which contribute to the increasing difficulty in controlling the spread of arboviruses. Therefore, prioritizing arbovirus surveillance is essential for effective epidemic preparedness. In this review, we offer a concise historical account of the discovery and monitoring of arboviruses in mosquitoes, from mosquito capture to viral detection. We then analyzed the advantages and limitations of these traditional methods. Furthermore, we investigated the potential of emerging technologies to address these limitations, including the implementation of next-generation sequencing, paper-based devices, spectroscopic detectors, and synthetic biosensors. We also provide perspectives on recurring issues and areas of interest such as insect-specific viruses.

Abstract:Arboviruses represent major challenges to public health, particularly in tropical, and subtropical regions, and a substantial risk to other parts of the world as respective vectors extend their habitats. In recent years, two viruses transmitted by Aedes mosquitoes, Chikungunya and Zika virus, have gathered increased interest. After decades of regionally constrained outbreaks, both viruses have recently caused explosive outbreaks on an unprecedented scale, causing immense suffering and massive economic burdens in affected regions. Chikungunya virus causes an acute febrile illness that often transitions into a chronic manifestation characterized by debilitating arthralgia and/or arthritis in a substantial subset of infected individuals. Zika infection frequently presents as a mild influenza-like illness, often subclinical, but can cause severe complications such as congenital malformations in pregnancy and neurological disorders, including Guillain–Barré syndrome. With no specific treatments or vaccines available, vector control remains the most effective measure to manage spread of these diseases. Given that both viruses cause antibody responses that confer long-term, possibly lifelong protection and that such responses are cross-protective against the various circulating genetic lineages, the development of Zika and Chikungunya vaccines represents a promising route for disease control. In this review we provide a brief overview on Zika and Chikungunya viruses, the etiology and epidemiology of the illnesses they cause and the host immune response against them, before summarizing past and current efforts to develop vaccines to alleviate the burden caused by these emerging diseases. The development of the urgently needed vaccines is hampered by several factors including the unpredictable epidemiology, feasibility of rapid clinical trial implementation during outbreaks and regulatory pathways. We will give an overview of the current developments.

Abstract:Arboviruses such as yellow fever, dengue, chikungunya and zika are transmitted mainly by the mosquito vector Aedes aegypti. Especially in the tropics, inefficacy of mosquito control causes arboviruses outbreaks every year, affecting the general population with debilitating effects in infected individuals. Several strategies have been tried to control the proliferation of A. aegypti using physical, biological, and chemical control measures. Other methods are currently under research and development, amongst which the use of nanotechnology has attracted a lot of attention of the researchers in relation to the production of more effective repellents and larvicides with less toxicity, and development of rapid sensors for the detection of virus infections. In this review, the utilization of nano-based formulations on control and diagnosis of mosquito-borne diseases were discussed. We also emphasizes the need for future research for broad commercialization of nano-based formulations in world market aiming a positive impact on public health.
Exploring the urban arbovirus landscape in Rio de Janeiro, Brazil: transmission dynamics and patterns of disease spread

Abstract:Background: This study focuses on urban arboviruses, specifically dengue (DENV), chikungunya (CHIKV), and Zika (ZIKV), which pose a significant public health challenge in Rio de Janeiro state, Southeast Brazil. In our research, we highlight critical findings on the transmission dynamics of these arboviruses in Rio de Janeiro, identifying distinct patterns of disease spread. Methods: By combining genomic data with case reports from the Brazilian Ministry of Health, we have analysed the phylogenetics, prevalence and spatial distribution of these endemic viruses within the state. Findings: Our results revealed sustained DENV transmission primarily in the northern part of the state, a significant ZIKV epidemic in 2016 affecting all mesoregions, and two major CHIKV outbreaks in 2018 and 2019, predominantly impacting the northern and southern areas. Our analysis suggests an inverse relationship between arboviral case incidence and urban density, with less populous regions experiencing higher transmission rates, potentially attributed to a complex interplay of factors such as the efficacy of vector control measures, environmental conditions, local immunity levels, and human mobility. Furthermore, our investigation unveiled distinct age and gender trends among affected individuals. Notably, dengue cases were predominantly observed in young adults aged 32, while chikungunya cases were more prevalent among individuals over 41. In contrast, cases of ZIKV were concentrated around the 33-year age group. Intriguingly, females accounted for nearly 60% of the cases, suggesting a potential gender-based difference in infection rates. Interpretation: Our findings underscore the complexity of arbovirus transmission and the need for interventions tailored to different geographical mesoregions. Enhanced surveillance and genomic sequencing will be essential for a deeper, more nuanced understanding of regional arbovirus dynamics. Identifying potential blind spots within the state will be pivotal for developing and implementing more effective public health strategies, specifically designed to address the unique challenges posed by these viruses throughout the state. Abstract:BACKGROUND: Estimates of the spatiotemporal distribution of different mosquito vector species and the associated risk of transmission of arboviruses are key to design adequate policies for preventing local outbreaks and reducing the number of human infections in endemic areas. In this study, we quantified the abundance of Aedes albopictus and Aedes aegypti and the local transmission potential for three arboviral infections at an unprecedented spatiotemporal resolution in areas where no entomological surveillance is available.METHODS: We developed a computational model to quantify the daily abundance of Aedes mosquitoes, leveraging temperature and precipitation records. The model was calibrated on mosquito surveillance data collected in 115 locations in Europe and the Americas between 2007 and 2018. Model estimates were used to quantify the reproduction number of dengue virus, Zika virus, and chikungunya in Europe and the Americas, at a high spatial resolution.FINDINGS: In areas colonised by both Aedes species, A aegypti was estimated to be the main vector for the transmission of dengue virus, Zika virus, and chikungunya, being associated with a higher estimate of R0 when compared with A albopictus. Our estimates highlighted that these arboviruses were endemic in tropical and subtropical countries, with the highest risks of transmission found in central America, Venezuela, Colombia, and central-east Brazil. A non-negligible potential risk of transmission was also estimated for Florida, Texas, and Arizona (USA). Abstract:Chikungunya virus (CHIKV) is an arthropod-borne virus (arbovirus) of epidemic concern, transmitted by Aedes ssp. mosquitoes, and is the etiologic agent of a febrile and incapacitating arthritogenic illness responsible for millions of human cases worldwide. After major outbreaks starting in 2004, CHIKV spread to subtropical areas and western hemisphere coming from sub-Saharan Africa, South East Asia, and the Indian subcontinent. Even though CHIKV disease is self-limiting and non-lethal, more than 30% of the infected individuals will develop chronic disease with persistent severe joint pain, tenosynovitis, and incapacitating polyarthralgia that can last for months to years, negatively impacting an individual's quality of life and socioeconomic productivity. The lack of specific drugs or licensed vaccines to treat or prevent CHIKV disease associated with the global presence of the mosquito vector in tropical and temperate areas, representing a possibility for CHIKV to continually spread to different territories, make this virus an agent of public health burden. In South America, where Dengue virus is endemic and Zika virus was recently introduced, the impact of the expansion of CHIKV infections, and co-infection with other arboviruses, still needs to be estimated. In Brazil, the recent spread of the East/Central/South Africa (ECSA) and Asian genotypes of CHIKV was accompanied by a high morbidity rate and acute cases of abnormal disease presentation and severe neuropathies, which is an atypical outcome for this infection. In this review, we will discuss what is currently known about CHIKV epidemics, clinical manifestations of the human disease, the basic concepts and recent findings in the mechanisms underlying virus-host interaction, and CHIKV-induced chronic disease for both in vitro and in vivo models of infection. We aim to stimulate scientific debate on how the characterization of replication, host-cell interactions, and the pathogenic potential of the new epidemic viral strains can contribute as potential developments in the virology field and shed light on strategies for disease control.

Abstract:Arbovirus is a vital, life-threatening disease worldwide and continues to be a significant problem while the world is dealing with the major coronavirus (COVID-19) pandemic. Vectors, mostly mosquitoes and ticks, transmit this disease. Dengue fever, chikungunya, and Zika viruses are the major threats because of their high incidence, public health burden, and clinically significant disease spectrum. These vector-borne disease causes one-fourth of annual deaths, leading to various infectious diseases. The arbovirus represents eight different families and 14 genera; most viruses belong to the family Bunyaviridae, and some also belong to Togaviridae, Reoviridae, and Flaviviridae. The arbovirus disease was isolated first in tropical and subtropical regions of South America and Africa and has high significance because of suitable environmental conditions for virus transmission and vector expansion. Its transmission cycle ranges from simple to highly complex. DENV is the most prevalent, results in febrile illness, and has transmission in 128 different countries. CHIKV causes infection in asymptomatic people, and the problems include nephritis, arthritis, myelitis, and acute encephalopathy. ZIKV-infected 80% of people are asymptomatic and may cause rashes, myalgia, fever, headache, and conjunctivitis. Vaccines for DENV are not clinically available; it is a primary arboviral infection in the world nowadays. The exposure of arbovirus diseases continues to be a global health problem regardless of continuing efforts. This review article will overview major arbovirus diseases and their diagnosis, treatment, and prevention strategies.

Abstract:The spread of arboviruses can be influenced by factors such as climate, socioeconomic and spatial distribution. The identification of populations at highest risk of exposure to arboviruses is an important topic in the public health scenario. Therefore, the present study is a review of the literature, which aimed to identify the association between the spatial distribution of Dengue Virus, Zika Virus and Chikungunya Virus and the social, environmental and economic aspects of individuals affected by arboviruses, the based on the analysis of published literature on the topic. Articles published in Portuguese, Spanish and English between 1997 and 2019 were included; and studies that addressed the clinic, infection, molecular biology, genetics, entomology and microbiology of viruses and mathematical modeling studies and reviews were excluded. The search for articles was carried out in the IBECS databases; CINAHL; MEDLINE; PubMed; Web of Science. After data mapping, PRISMA was used to extract the data. Therefore, of the 32 articles included in the analysis, the results showed that climatic factors such as temperature and precipitation affect the origin, evolution and distribution of vectors; and, consequently, the transmission of related pathogens. Furthermore, when considering socioeconomic aspects, populations from regions with greater social vulnerability are at greater risk of becoming ill from an arbovirus. Therefore, these results suggest that climatic factors and socioeconomic aspects are associated with the spatial distribution of arboviruses, making it essential to develop efficient control and prevention measures for arboviruses, especially in the most vulnerable regions.

Abstract:Some properties of the optimal mean-square error uniform quantizer are treated. It is shown that the mean-square error (mse) is given by the input variance minus the output variance. Abstract:The World Health Organization recently declared a global initiative to control arboviral diseases. These are mainly caused by pathogenic flaviviruses (such as dengue, yellow fever and Zika viruses) and alphaviruses (such as chikungunya and Venezuelan equine encephalitis viruses). Vaccines represent key interventions for these viruses, with licensed human and/or veterinary vaccines being available for several members of both genera. However, a hurdle for the licensing of new vaccines is the epidemic nature of many arboviruses, which presents logistical challenges for phase III efficacy trials. Furthermore, our ability to predict or measure the post-vaccination immune responses that are sufficient for subclinical outcomes post-infection is limited. Given that arboviruses are also subject to control by the immune system of their insect vectors, several approaches are now emerging that aim to augment antiviral immunity in mosquitoes, including Wolbachia infection, transgenic mosquitoes, insect-specific viruses and paratransgenesis. In this Review, we discuss recent advances, current challenges and future prospects in exploiting both vertebrate and invertebrate immune systems for the control of flaviviral and alphaviral diseases.

Abstract:Arboviral infections are major public health threats, with 100 million people estimated to get sick annually from dengue infection alone. Globally, the risk of arboviruses is likely to further increase both within, and outside of, affected regions due to a combination of factors including climate change, human mobility, and other societal factors. Despite the availability of vaccines for some arbovirus infections, there is a lack of specific antiviral treatment options. Professor Johan Neyts at the University of Leuven, Belgium, has been working on developing antiviral strategies for more than 30 years. His current research focuses on developing antiviral drugs and vaccines against emerging and neglected viruses many of which are arboviruses. In this Q&A, he discusses the risks associated with vector-borne virus infections, challenges in developing efficient drugs for treatment, and current promising efforts to address these challenges.

Abstract:Dengue is currently the highest and rapidly spreading vector-borne viral disease, which can lead to mortality in its severe form. The globally endemic dengue poses as a public health and economic challenge that has been attempted to suppress though application of various prevention and control techniques. Therefore, broad spectrum techniques, that are efficient, cost-effective, and environmentally sustainable, are proposed and practiced in dengue-endemic regions. The development of vaccines and immunotherapies have introduced a new dimension for effective dengue control and prevention. Thus, the present study focuses on the preventive and control strategies that are currently employed to counter dengue. While traditional control strategies bring temporary sustainability alone, implementation of novel biotechnological interventions, such as sterile insect technique, paratransgenesis, and production of genetically modified vectors, has improved the efficacy of the traditional strategies. Although a large-scale vector control strategy can be limited, innovative vaccine candidates have provided evidence for promising dengue prevention measures. The use of tetravalent dengue vaccine (CYD-TDV) has been the most effective so far in treating dengue infections. Nonetheless, challenges and limitation hinder the progress of developing integrated intervention methods and vaccines; while the improvement in the latest techniques and vaccine formulation continues, one can hope for a future without the threat of dengue virus.

Abstract:Background: Research has been conducted on interventions to control dengue transmission and respond to outbreaks. A summary of the available evidence will help inform disease control policy decisions and research directions, both for dengue and, more broadly, for all Aedes-borne arboviral diseases. Method: A research-to-policy forum was convened by TDR, the Special Programme for Research and Training in Tropical Diseases, with researchers and representatives from ministries of health, in order to review research findings and discuss their implications for policy and research. Results: The participants reviewed findings of research supported by TDR and others. Surveillance and early outbreak warning. Systematic reviews and country studies identify the critical characteristics that an alert system should have to document trends reliably and trigger timely responses (i.e., early enough to prevent the epidemic spread of the virus) to dengue outbreaks. A range of variables that, according to the literature, either indicate risk of forthcoming dengue transmission or predict dengue outbreaks were tested and some of them could be successfully applied in an Early Warning and Response System (EWARS). Entomological surveillance and vector management. A summary of the published literature shows that controlling Aedes vectors requires complex interventions and points to the need for more rigorous, standardised study designs, with disease reduction as the primary outcome to be measured. House screening and targeted vector interventions are promising vector management approaches. Sampling vector populations, both for surveillance purposes and evaluation of control activities, is usually conducted in an unsystematic way, limiting the potentials of entomological surveillance for outbreak prediction. Combining outbreak alert and improved approaches of vector management will help to overcome the present uncertainties about major risk groups or areas where outbreak response should be initiated and where resources for vector management should be allocated during the interepidemic period. Conclusions: The Forum concluded that the evidence collected can inform policy decisions, but also that important research gaps have yet to be filled.

Abstract:Objective: To identify and synthesize evidence of the effectiveness and cost-effectiveness of the Wolbachia-Aedes methodology for reducing cases of arboviral infection. Methodology: Rapid systematic review. A search of five online portals and bibliographic databases was conducted in February 2022. Two reviewers conducted the selection stages, with any disagreements resolved by a third investigator. Data were extracted by one reviewer and subsequently checked by another. The inclusion criteria were studies describing the cost-effectiveness and effectiveness of the Wolbachia-Aedes methodology, based on field interventions, published in Portuguese, English, and Spanish, with no restrictions on date of publication. Results: Four studies were included: one cluster randomized trial, two quasi-experimental studies, and one pre-post study, published between 2019 and 2021 in four countries. The included studies reported effectiveness for some of the outcomes of interest, such as a reduction in the incidence of dengue, Zika, and chikungunya cases. Although no experimental studies addressing the cost-effectiveness of the Wolbachia-Aedes strategy were identified, some modeling studies have demonstrated potential cost-benefit of this methodology. Conclusion: Considering the results of the included studies, Wolbachia has the potential to be an economically effective strategy that leads to reductions in the incidence of dengue, Zika, and chikungunya. Abstract:Diseases transmitted by mosquitoes impose enormous burden towards human morbidity and mortality. Over the last three decades, Brazil has suffered from severe Dengue epidemics. In September 2014, this situation is further complicated by the introduction of two other viruses, Zika and Chikungunya, placing Brazil in a triple epidemic. In this article, we discuss the biology of Aedes aegypti Linnaeus, and the principal initiatives currently used to control mosquito populations and the diseases they transmit. Aedes aegypti has broad global distribution and is involved in the transmission of various arboviral diseases such as Dengue, Zika, and Chikungunya. Several factors contribute to the success of the species, particularly behavioral plasticity, rapid development, desiccation-resistant eggs, resistance to the principle insecticide classes currently available on the market, preference for the urban environment, and proximity to humans. Vector control programs are the best way to reduce the burden of mosquito-borne diseases. Chemical control is most commonly used in recent times, and unfortunately, the results have not been satisfactory but instead, there is increased vector dispersal and, subsequently, the spread of disease epidemics. Investigations of alternative control methods such as release of Wolbachia-infected mosquitoes for blocking vector-borne pathogens, release of transgenic mosquitoes carrying a lethal gene for offspring, and the use of insecticide-dispersing mosquitoes are under way in Brazil, and some have shown promising results. Special emphasis should be placed on integrated management of all available tactics, so as to maximize efforts towards mosquito control. Finally, we emphasize that continuous actions and community participation control initiatives are critically important for success.

Abstract:Mosquito-borne diseases are persistent and potentially severe posing a threat to global pandemic preparedness. The risk of mosquito-borne virus transmission is rapidly increasing due to the unprecedented spread of viruses such as dengue and chikungunya, the disruption of global mosquito-borne disease control efforts following the emergence of coronavirus diseases 2019 (COVID-19) in 2019, global warming, and human activities. To address this global challenge, various innovative mosquito control technologies are being developed worldwide. This paper summarizes the latest advances in mosquito vector control, focusing on China's latest mosquito control strategies, to provide insights into implementing novel mosquito-borne disease control measures.
Global risk mapping for major diseases transmitted by Aedes aegypti and Aedes albopictus

Abstract:Objectives: The objective of this study was to map the global risk of the major arboviral diseases transmitted by Aedes aegypti and Aedes albopictus by identifying areas where the diseases are reported, either through active transmission or travel-related outbreaks, as well as areas where the diseases are not currently reported but are nonetheless suitable for the vector. Methods: Data relating to five arboviral diseases (Zika, dengue fever, chikungunya, yellow fever, and Rift Valley fever (RVF)) were extracted from some of the largest contemporary databases and paired with data on the known distribution of their vectors, A. aegypti and A. albopictus. The disease occurrence data for the selected diseases were compiled from literature dating as far back as 1952 to as recent as 2017. The resulting datasets were aggregated at the country level, except in the case of the USA, where state-level data were used. Spatial analysis was used to process the data and to develop risk maps. Results: Out of the 250 countries/territories considered, 215 (86%) are potentially suitable for the survival and establishment of A. aegypti and/or A. albopictus. A. albopictus has suitability foci in 197 countries/territories, while there are 188 that are suitable for A. aegypti. There is considerable variation in the suitability range among countries/territories, but many of the tropical regions of the world provide high suitability over extensive areas. Globally, 146 (58.4%) countries/territories reported at least one arboviral disease, while 123 (49.2%) reported more than one of the above diseases. The overall numbers of countries/territories reporting autochthonous vector-borne occurrences of Zika, dengue, chikungunya, yellow fever, and RVF, were 85, 111, 106, 43, and 39, respectively. Conclusions: With 215 countries/territories potentially suitable for the most important arboviral disease vectors and more than half of these reporting cases, arboviral diseases are indeed a global public health threat. The increasing proportion of reports that include multiple arboviral diseases highlights the expanding range of their common transmission vectors. The shared features of these arboviral diseases should motivate efforts to combine interventions against these diseases.
[Atypical presentation of delirium: risk of misdiagnosis and undertreatment].

Abstract:An atypical presentation of delirium increases the risk of misdiagnosis and undertreatment. Three cases are presented in this article. A 65-year-old woman, admitted for malnutrition, has significant mood-related symptoms that resemble depression. A 50-year-old male, admitted with an abscess, necrotizing fasciitis and sepsis, appears to be suicidal. A 61-year-old male, admitted with pneumonia, has auditory hallucinations. All three patients turned out to have a delirium. The authors advise ruling out a delirium in all patients who have predisposing factors and who develop an acute psychiatric disorder while staying in the hospital, before considering other psychiatric diagnoses. Advice on how to improve the diagnostic process is given.
