Abstract:Introduction:Stroke etiology may affect the prognosis of acute basilar artery occlusion (BAO). This study aimed to estimate the efficacy and safety of endovascular treatment (EVT) in BAO strokes due to the underlying stroke etiologies of large artery atherosclerosis (LAA) and cardioembolism (CE). Patients and methods:Patients with LAA and CE subtypes from the registry EVT for Acute Basilar Artery Occlusion Study (BASILAR) were selected for this analysis. We estimated the EVT treatment effect relative to standard medical treatment (SMT) in these patients by using a propensity score approach with inverse probability of treatment weighted estimation. Outcomes included 90-day favorable functional outcomes (modified Rankin scale (mRS) 0-3), functional independence (mRS 0-2), all-cause mortality, and symptomatic intracranial hemorrhage (sICH). Results:A total of 744 patients were included in the final analysis. After weighting with inverse probability of treatment estimation, the patients who, based on their characteristics, were treated with EVT rather than SMT, demonstrated higher rates of favorable functional outcomes in both the LAA (29.2% vs 11.7%, adjusted OR with 95% CI: 4.34 [2.15-8.78], p < 0.001) and the CE subtype (36.0% vs 8.1%, adjusted OR with 95% CI: 9.14 [1.96-42.55], p = 0.005). A similar finding was also observed for functional independence. EVT patients also demonstrated lower rates of mortality than SMT. Among EVT patients, no significant difference was observed in mortality or sICH between LAA and CE groups, but LAA patients had lower rates of favorable functional outcome and functional independence (29.2% vs 37% and 24.2% vs 32.9%, respectively), where the latter remained significant after adjustment for imbalances in baseline data (p = 0.04). Conclusions:In acute BAO strokes, both LAA and CE patients who, based on their characteristics, were treated with EVT rather than SMT, demonstrated better outcomes. Among EVT patients, LAA and CE subtypes displayed important baseline and treatment differences, and LAA patients were less likely to achieve functional independence at 3 months, but mortality and sICH were similar between LAA or CE subtypes. These results need to be confirmed in future clinical trials.

Abstract:Objective To observe the efficacy of edaravone in treatment of patients with cerebral infarction if atherosclerotic thrombus(ATCI).Methods Ninety six ATCI patients enrolled within 48h of onset were divided into two groups:control group and Edaravone group,48 patients in each.In treatment group edaravone was infused at a dose of 30 mg,twice a day for 14 days,in addition to hydroxyethylrutin and aspirin as basic treatment.The patients in control group were given the same regime except edaravone.The concentrations of serum CRP in 96 patients were measured by immunoturbidity method before treatment and at 5th,10th and 15th day after treatment.The head CT scanning was performed,before treatment and at 5th,15th day after treatment.Results As compared with the controls,the concentration of serum CRP was lower and size of cerebral infarct lesion was reduced significantly.Conclusion Edaravone is effective in decreasing the concentration of serum CRP and deflating cerebral infarct volume of patients with atherosclerotic thrombus cerebral infarction.
Efficacy and association of hyperbaric oxygen therapy combined with butylphthalide and oxiracetam with serum levels of inflammatory markers in vascular cognitive impairment after acute ischemic stroke

Abstract:Objectives: This study evaluated the efficacy of hyperbaric oxygen therapy (HBOT) combined with butylphthalide (NBP) and oxiracetam (OXR) for vascular cognitive impairment after acute ischemic stroke and investigated the association between such combination therapy and the serum levels of inflammatory markers. Methods: This was a prospective study which included eighty patients with post-AIS cognitive impairment (PAISCI) treated in Dongguan City People's Hospital from January 2020 to January 2022. They were randomized into study group and control group. The control group was provided with conventional therapy consisting of NBP for intravenous transfusion and oral OXR, while the study group received combination therapy of HBOT, NBP, and OXR. A comparison was drawn between the two groups regarding clinical outcomes, levels of recovery of cognitive and neurological function and intelligence, changes in inflammatory markers, and incidence of adverse drug reactions (ADRs). Results: The response rate of the study group was significantly higher than that of the control group (p=0.04). The cognitive function scores of the study group were significantly better than those of the control group at the end of treatment (p<0.05). The post-treatment levels of inflammatory markers were significantly reduced in the study group when compared with the control group (p<0.05). At two weeks after treatment, the ADR rate of study group was significantly lower than the control group (p=0.03). Conclusions: The combination therapy of HBOT, NBP, and OXR demonstrates robust efficacy in patients with PAISCI. It is deemed to be a safe and effective treatment regimen.
Endovascular Treatment for Minor Acute Ischemic Strokes with Large Vessel Occlusion

Abstract:Background It remains uncertain whether patients with minor acute ischemic stroke with large vessel occlusion benefit from endovascular treatment (EVT). We aim to evaluate the outcomes of EVT in minor acute ischemic stroke with anterior circulation large vessel occlusion. Methods and Results Based on a nationwide prospective stroke registry, patients with minor acute ischemic stroke with anterior circulation large vessel occlusion within 24 hours of onset were divided into groups receiving standard medical treatment plus EVT or standard medical treatment alone. Primary outcome was excellent functional outcome defined as modified Rankin Scale score 0 to 1 at 90 days. In addition, a multivariable logistic regression model was used to analyze the effect of EVT guided by perfusion imaging. A total of 572 patients with median age 68 years (interquartile range=60–77) and median National Institutes of Health Stroke score 3 (interquartile range =2–4) were identified and 123 patients were treated with standard medical treatment plus EVT. EVT was not associated with excellent functional outcome (unadjusted odds ratio [OR], 0.771 [95% CI, 0.516–1.151]; adjusted OR, 0.793 [95% CI, 0.515–1.219]; P=0.290). However, therapy selection guided by perfusion imaging was a modifier of EVT effect on outcomes, as EVT was significantly associated with excellent functional outcome (60.0% versus 50.8%, unadjusted OR, 1.451 [95% CI, 0.643–3.272]; adjusted OR, 2.849 [95% CI, 1.006–8.067]; P=0.049) but not with symptomatic intracerebral hemorrhage in the imaging‐guided group. Conclusions Although functional outcomes in minor acute ischemic stroke caused by anterior circulation large vessel occlusion were not improved from the routine use of EVT, our results suggested that EVT guided by perfusion imaging could be beneficial for those patients. Clinical Study of Atorvastatin Combined with Clopidogrel in Patients with Acute Cerebral Infarction

Abstract:OBJECTIVE To investigate the therapeutic efficacy of atorvastatin combined with clopidogrel on acute cerebral infarction and the improvement on the prognosis of the disease.METHODS 120 cases with acute cerebral infarction were randomly divided into two groups:60 cases were treated with atorvastatin combined with clopidogrel;60 cases were given clopidogrel only.The scores of NIHSS,Barthel,mRS and MMSE were estimated before and 3 and 6 months after treatment.RESULTS In these two groups,all of the scores of NIHSS,Barthel and mRS were improved 3 and 6 months after the therapy(P0.05).The scores of NIHSS,Barthel and mRS were improved more in atorvastatin plus clopidogrel therapy group than those in clopidogrel therapy group.The significant statistical difference was found(P0.05).No alternation of MMSE estimation was found with no statistical difference(P0.05).CONCLUSION Atorvastatin plus clopidogrel therapy may be beneficial for restoring the nervous function and improving the ability of daily living and the prognosis.Atorvastatin plus clopidogrel can be used in acute cerebral infarction as a therapeutic approach.

Abstract:Objective To study the effects of atorvastatin on cognition and blood lipids in patients with different stages of vascular cognitive impairment (VCI).Methods 140 patients with VCI were randomly divided into the treatment group [n = 75,VCI without dementia(VCIND) :47,vascular dementia (VaD):28] and the control group (n= 65 ,VCIND:41,VaD:24 ).The control group received conventional therapy,while the treatment group were administrated orally with 20 μg of atorvastatin every evening.The indices of TC,TG,HDL-C,LDL-C,as well as the scores of MMSE and clock-drawing test were compared between the groups before and 24 weeks after treatment.Results The plasma TC,TG,LDL-C decreased and the scores of MMSE and clock-drawing test improved significantly in the treatment group.The improvement in the scores of MMSE and clock-drawing test was greater in the VCIND treatment group than in the VaD treatment group.Conclusion Atorvastatin may improve cognitive function in patient with VCI,especially in VCIND patients.

Abstract:Objectives: Cognitive impairment, anxiety, depression, fatigue, and dependence in instrumental activities of daily living (ADL) are common after stroke; however, little is known about how these outcomes may differ following treatment with endovascular clot retrieval (ECR), intravenous tissue plasminogen activator (t-PA), or conservative management. Methods: Patients were recruited after acute treatment and invited to participate in an outcome assessment 90–120 days post-stroke. The assessment included a cognitive test battery and several questionnaires. The COVID-19 pandemic led to significant disruptions in recruitment and data collection, and the t-PA and conservative management groups were combined into a standard medical care (SMC) group. Results: Sixty-two participants were included in the study (ECR = 31, SMC = 31). Mean age was 66.5 (20–86) years, and 35 (56.5%) participants were male. Participants treated with ECR had significantly higher National Institutes of Health Stroke Scale scores at presentation and significantly lower education. After adjusting for stroke severity, premorbid intellectual ability, and age, treatment with ECR was associated with significantly better performances on measures of cognitive screening, visual working memory, and verbal learning and memory. Participants treated with ECR also experienced less fatigue and were more likely to achieve independence in basic and instrumental ADLs. Despite this, cognitive impairment and fatigue were still common among participants treated with ECR and anxiety and depression symptoms were experienced similarly by both groups. Conclusions: Cognitive impairment and fatigue were less common but still prevalent following treatment with ECR. This has important practical implications for stroke rehabilitation, and routine assessment of cognition, emotion, and fatigue is recommended for all stroke survivors regardless of stroke treatment and functional outcome.

Abstract:Aim This study aimed to explore the effect of enriched rehabilitation (ER) on cognitive function and serum glutamate levels in patients with stroke. Methods Forty patients diagnosed with post-stroke cognitive impairment (PSCI), according to the inclusion criteria, and undergoing inpatient rehabilitation were enrolled in the study. Patients were randomly assigned to receive 8 weeks of ER treatment (ER group; n = 20) or conventional medical treatment (CM group; n = 20). In addition, 20 age-matched healthy subjects who were outpatients in our hospital during the same period formed the healthy control (HC) group. In- and between-group differences in cognitive function were assessed during pre-intervention and post-intervention based on the Montreal Cognitive Assessment (MoCA), the Symbol Digit Modalities Test (SDMT), and the Trail Making Test (TMT). The serum levels of glutamate, tumor necrosis factor (TNF), and malondialdehyde (MDA) levels were also detected pre-intervention and post-intervention. Results Pre-intervention cognitive function and the levels of all the serum parameters assessed significant difference between the HC group and the PSCI group (both ER and CM groups) (p < 0.05), but not between the two groups of patients with PSCI (p > 0.05). Significant improvements were observed in cognitive function in both the ER and the CM groups post-intervention compared with pre-intervention, as evidenced by the measured improvement in MoCA, SDMT, and TMT scores. Similar improvements were seen for serum glutamate, the degree of oxidative damage, and the level of inflammation in both the treatment groups (p < 0.05). More enhancements in cognitive function, including MoCA, SDMT, TMT scores, and the serum levels of glutamate, the degree of oxidative damage, and the level of inflammation were shown in the ER group compared with the CM group post-intervention (p < 0.05). Conclusions ER can improve cognitive function in patients with PSCI. The associated mechanism may be related to the negative regulatory effect of ER on serum glutamate, TNF, and MDA levels, which is likely to enhance synaptic plasticity and alleviate oxidative stress- and inflammation-related damage, at least to some extent.

Abstract:Background and objectives: The effect of endovascular therapy (EVT) for large vessel occlusion stroke on cognitive outcomes is not well understood. We evaluated the effect of EVT on cognitive function in the Endovascular Treatment for Small Core and Anterior Circulation Proximal Occlusion With Emphasis on Minimizing CT to Recanalization Times (ESCAPE) trial. Methods: Patient data from the ESCAPE randomized trial were analyzed. Cognitive assessments completed at 90 days after stroke were the Montreal Cognitive Assessment (MoCA), the Sunnybrook Neglect Assessment Procedure (SNAP), the Boston Naming Test (BNT), Trail-making test A (Trails A), and Trail-making test B (Trails B). We used logistic regression to evaluate the association between EVT and favorable cognitive outcome on the 5 separate tests, adjusting for demographic and clinical factors. We used generalized estimating equations and ordinal regression to determine the odds of favorable outcome with EVT on global cognition incorporating the 5 tests. We added final infarct volume (FIV) to the models to assess the relationship of FIV with cognitive outcome. Results: The ESCAPE trial included 315 patients, 165 randomized to EVT and 150 randomized to control. There was higher odds of favorable outcome with EVT for MoCA (adjusted odds ratio [aOR] 2.32, 95% CI 1.30-4.16), SNAP (aOR 3.85, 95% CI 2.00-7.45), BNT (aOR 2.33, 95% CI 1.30-4.17), trails A (aOR 3.50, 95% CI 1.93-6.36), and trails B (aOR 2.56, 95% CI 1.46-4.48). There was higher odds of favorable outcome with EVT on global binary (aOR 2.57, 95% CI 1.67-3.94) and ordinal analyses (aOR 2.83, 95% CI 1.68-4.76) of cognitive function. After adding FIV to the models, both FIV and EVT were significantly associated with cognitive outcome. There was a significant correlation between global cognitive performance and mRS at day 90 (r = -0.78, p < 0.001), with the largest reductions in favorable cognitive outcome from mRS score 4 to 5 and from mRS 2 to 3. Discussion: In this secondary analysis of the ESCAPE trial, EVT was associated with favorable outcome on 5 separate cognitive tests and in global analyses of cognitive benefit. These results provide novel evidence for the effect of EVT on cognition and support the global benefit of treatment with EVT. Classification of evidence: This study provides Class II evidence that in patients with acute ischemic stroke due to intracranial internal carotid artery (ICA) or M1 segment MCA occlusion, including tandem extracranial ICA occlusions, EVT compared with best medical therapy increased odds of favorable cognitive outcome.

Abstract:To explore the efficacy of ginkgo diterpene lactone (GDLM) on cognitive function in patients with acute ischemic stroke (AIS). A total of 126 patients with AIS in Shaanxi Provincial People's Hospital from July 2019 to December 2020 were collected and randomly divided into the control group and treatment group ( n = 63). All patients received conventional treatment, on which 25 mg/day GDLM was administered in the treatment group. Coagulation and inflammation indexes, National Institutes of Health Stroke Scale (NIHSS) and activities of daily living scale (ADL) scores were measured before and 14 days after treatment. NIHSS and ADL scores were performed again after 3 months. Cognitive function was assessed by Montréal Cognitive Assessment (MoCA) score, Mini-Mental State Examination (MMSE) score, and potential P300. After 14 days of treatment, all biochemical indices were lower than before treatment ( P < 0.05). The NIHSS and ADL scores of the treatment group were significantly better than those of the control group after treatment ( P < 0.05). The MoCA and MMSE scores of the treatment group improved more significantly compared with the control group ( P < 0.05). After treatment, the P300 indexes of both groups were significantly better than before treatment ( P < 0.05). Conventional treatment of AIS combined with GDLM can effectively improve the cognitive function of patients, which is worthy of clinical recommendation.

Abstract:Objective To intestate the effect of different doses of atorvastatin on early neurological deterioration and short-term outcomes in patients with acute ischemic stroke.Methods The patients with acute ischemic stroke were enrolled prospectively.They were randomly assigned to either a standard therapy group (atorvastatin 20 mg/d) or an intensive treatment group (atorvastatin 40 mg/d).The primary outcomes were early neurological deterioration within 1 week of treatment and the good outcome of evaluation at 1 month after treatment (the modified Rankin Scale score 0-2); the secondary outcomes were the National Institutes of Health Stroke Scale (NIHSS) score and adverse events at 1 month.Results A total of 125 patients with acute ischemic stroke were enrolled,including 62 in the standard therapy group and 63 in the intensive treatment group.The incidence of early neurological deterioration at 1 week after treatment in the standard therapy group was significantly higher than that in the intensive treatment group (16.13％ vs.4.76％;x2=4.333,P=0.038); the proportion of good outcome in the standard therapy group was significantly lower than that in the intensive treatment group at 1 month after treatment (53.23％ vs.71.43％ ;x2 =4.413,P=0.036).During the treatment,no significant liver damage,muscle toxicity and other adverse events of causing atorvastatin reduction or withdrawal occurred in the patients of both groups.Conclusions Using high-dose atorvastatin in the acute phase of ischemic stroke may decrease the incidence of early neurological deterioration compared with the conventional dose,and improve short-term clinical outcomes.

Abstract:Activation of the tyrosine kinase focal adhesion kinase (FAK) upon cell stimulation by the extracellular matrix initiates integrin outside-in signaling. FAK is directly recruited to active integrins, which enhances its kinase activity and triggers downstream signaling like activation of PI3K. We recently described that Gα-interacting, vesicle-associated protein (GIV), a protein up-regulated in metastatic cancers, is also required for outside-in integrin signaling. More specifically, we found that GIV is a non-receptor guanine nucleotide exchange factor that activates trimeric G proteins in response to integrin stimulation to enhance PI3K signaling and tumor cell migration. In contrast, previous reports have established that GIV is involved in phosphotyrosine (Tyr(P))-based signaling in response to growth factor stimulation; i.e. GIV phosphorylation at Tyr-1764 and Tyr-1798 recruits and activates PI3K. Here we show that phosphorylation of GIV at Tyr-1764/Tyr-1798 is also required to enhance PI3K-Akt signaling and tumor cell migration in response to integrin stimulation, indicating that GIV functions in Tyr(P)-dependent integrin signaling. Unexpectedly, we found that activation of FAK, an upstream component of the integrin Tyr(P) signaling cascade, was diminished in GIV-depleted cells, suggesting that GIV is required to establish a positive feedback loop that enhances integrin-FAK signaling. Mechanistically, we demonstrate that this feedback activation of FAK depends on both guanine nucleotide exchange factor and Tyr(P) GIV signaling as well as on their convergence point, PI3K. Taken together, our results provide novel mechanistic insights into how GIV promotes proinvasive cancer cell behavior by working as a signal-amplifying platform at the crossroads of trimeric G protein and Tyr(P) signaling.

Abstract:Objective: This study evaluated the possible effect of the combined intervention of physical exercise and cognitive training on cognitive function in stroke survivals with vascular cognitive impairment. Design: A single-blind (investigator-blinded but not subject-blinded) randomized controlled trial. Setting: Medical Rehabilitation Center of Shanghai General Hospital, China. Subjects: A total of 225 patients (mean age 64.59 years, SD = 4.27) who exhibited vascular cognitive impairment were included in this study. Interventions: Patients were randomly allocated into one of the four groups: (1) physical exercise ( n = 56; 50-minute session), (2) cognitive training ( n = 57; 60-minute session), (3) combined intervention of physical exercise and cognitive training ( n = 55; 50-minute session + 60-minute session), or (4) control groups ( n = 57; 45-minute session). All participants received training for 36 sessions, three days per week, for 12 weeks. Primary measures: Measures were recorded at baseline, after the intervention and at a six-month follow-up. Primary measurements included the Trail Making Part B, Stroop, forward digit span, and mental rotation tests. Results: A total of 179 participants (79.56% response rate) completed the study. Cognitive performances on all four tasks in the combined training group improved significantly after the intervention ( P < 0.01). Changes in cognitive performance were greater in the combined intervention group than those in the physical exercise group (e.g. forward digit span, 13.61% vs. 2.18%, P = 0.003), the cognitive training group (e.g. mental rotation, 17.36% vs. 0.87%, P = 0.002), and the control group (e.g. Stroop, −4.11% vs. −0.72%, P = 0.026). Conclusion: The combined intervention produced greater benefits on cognitive function compared to either training alone in stroke survivors with vascular cognitive impairment.

Abstract:Background: Cognitive impairment is one of the common sequelae after stroke, which not only hinders the recovery of patients but also increases the financial burden on families. In the absence of effective therapeutic measures, acupuncture treatment has been widely used in China to treat post-stroke cognitive impairment (PSCI), but the specific efficacy is unclear. Therefore, this review aimed to evaluate the true efficacy of acupuncture treatment in patients with PSCI. Methods: We searched eight databases [PubMed, Embase, Web of Science, Cochrane Central Register of Controlled Trials, China Biomedical Literature Database (CBM), China Science and Technology Journal (VIP) database, the China National Knowledge Infrastructure (CNKI) database, and Wan fang database] from the inception to May 2022 for randomized controlled trials (RCTs) related to acupuncture treatment combined with cognitive rehabilitation (CR) for PSCI. Two investigators independently used a pre-designed form to extract valid data from eligible RCTs. The risk of bias was assessed through tools provided by the Cochrane Collaboration. The meta-analysis was implemented through Rev Man software (version 5.4). The strength of the evidence obtained was evaluated using GRADE profiler software. Adverse events (AEs) were collected by reading the full text and used to evaluate the safety of acupuncture treatment. Results: Thirty-eight studies involving a total of 2,971 participants were included in this meta-analysis. Overall, the RCTs included in this meta-analysis were poor in methodological quality. The combined results showed that acupuncture treatment combined with CR showed significant superiority compared to CR alone in terms of improving cognitive function [Mean Difference (MD) = 3.94, 95% confidence intervals (CI): 3.16–4.72, P 0.05). The certainty of the evidence was rated low level because of flaws in the study design and considerable heterogeneity among the included studies. Conclusion: This review found that acupuncture treatment combined with CR may have a positive effect on improving cognitive function and self-care ability in PSCI patients. However, our findings should be treated with caution owing to the existence of methodological quality issues. High-quality studies are urgently required to validate our results in the future. Study on Effect of Ginkgo Ketone on Inlfammatory Response and Cognitive Function in Patients with Acute Ischemic Stroke

Abstract:Objective To investigate the effect of Ginkgo ketone ester dispersible tablets on the inlfammatory reaction and cognitive function after ischemic stroke and its clinical signiifcance. <br> Methods This study was a prospective, multi-center, clinical randomized and controlled trial. Patients with acute ischemic stroke from Neurological Department in Nanjing Drum Tower Hospital and Yangzhou First People's Hospital during October 2012 to June 2014 were recruited and randomly divided into therapeutic group (Ginkgo ketone ester dispersible tablets (0.15 tid) combined with aspirin group (0.1 qd) and control group (aspirin group (0.1 qd), medication 180 d). Fresh blood was harvested before treatment and 13±1 days after treatment from enrolled patients. Serum inlfammatory cytokines (IL-1β, IL-6, IL-15, IL-17A, IL-23 and TNF-α) were detected by Milliplex Map multiple detection techniques. National Institutes Health Stroke Scale (NIHSS), modiifed Rankin Scale (mRS), Mini-Metal State Examination (MMSE) and Montreal Cognitive Assessment (MOCA) were applied to evaluate neurological deifcits and cognitive decline before treatment and (13±1) d, (30±7) d, (90±7) d, (180±7) d after treatment, and to compare scale scores changes in two groups before and after treatment. <br> Results A total of 60 patients were enrolled into study. Among whom, 8 cases were excluded due to non-eligible blood specimen; 27 cases of study group and 25 cases of control group were taken into statistical analysis. Treatment of (13±1) d, the serum IL-1 beta IL-1β ([1.55±0.43] pg/mlvs [2.05±0.74] pg/ml,P<0.001), IL-15 ([1.88±0.82] pg/mlvs [3.17±1.93] pg/ml,P<0.001), IL-6 ([5.57±4.96] pg/mlvs [8.81±8.00] pg/ml,P=0.042), IL-17A ([5.11±1.51] pg/mlvs [6.67±2.24] pg/ml,P<0.001), IL-23 ([0.42±0.88] pg/mlvs [0.67±0.98] pg/ml,P<0.001), TNF-alpha ([15.12±6.97] pg/mlvs [18.31±6.61] pg/ml,P=0.009) levels in therapeutic group were decreased significantly than that before treatment; while in the control group, only IL-15 ([2.01±0.72] pg/mlvs [2.53±1.20] pg/ml,P=0.036) level was signiifcantly decreased compared with that of before treatment. After treatment (30±7) d, (90±7) d, (180±7) d in the therapeutic group, MMSE and MOCA scores were improved more signiifcantly than that before treatment compared with the control group (P=0.036, 0.012 and 0.048), though the two scores were of no signiifcant differences in two groups. <br> Conclusion The combination of Ginkgo ketone ester dispersible tablets and aspirin can more obviously reduce serum inlfammatory cytokines and improve their cognitive function compared with using aspirin alone in ischemic stroke patient.
Influences of Butylphthalide Injection in Combination with Edaravone on Clinical Efficacy and Cytokines in Elderly Acute Cerebral Infarction Patients

Abstract:Background: Acute cerebral infarction threatens human health and life safety. The edaravone is a new antioxidant and hydroxyl radical scavenger, which is used in the treatment of cardiovascular diseases. However, long-term use of edaravone is prone to drug resistance, resulting in limited therapeutic effect. Butylphthalein can improve collateral circulation, protect the integrity of vascular endothelial cells, improve vasospasm, increase blood perfusion in the ischemic area of brain tissue, and promote the recovery of cerebral nerve function. Objective: To measure the influences of butylphthalide injection in combination with edaravone on clinical efficacy and cytokines in elderly acute cerebral infarction patients, aiming to provide reference for the treatment of acute cerebral infarction. Design: This was a retrospective study. Setting: This study was performed in Yantaishan Hospital. Participants: A total of 168 elderly acute cerebral infarction patients who accepted therapy in our hospital from February 2022 to February 2023 were chosen and allocated into a combination group and a control group. Interventions: The control group accepted the edaravone injection treatment. The combination group was treated with edaravone injection combined with butylphthalide injection. Primary outcome measures: (1) Therapeutic effect (2) Cytokines (3) Vascular endothelial function (4) Oxidative stress (5) Degree of neurological impairment (6) Living ability. Results: The total effective rate in the combination group presented elevation when in contrast to the control group (P < .05). After therapy, the levels of cytokines in the combination group presented reduction relative to the control group (P < .05), nitric oxide level in the combination group presented elevation when in contrast to the control group (P < .05), vascular endothelial growth factor level in the combination group presented lessened relative to the control group (P < .05), glutathione peroxidase along with superoxide dismutase levels in the combination group presented higher relative to the control group (P < .05) and malondialdehyde level in the combination group presented lower relative to the control group (P < .05). After therapy, the National Institute of Stroke Scale score in the combination group presented reduced when compared with the control group (P < .05). In contrast, the activity of daily living score in the combination group presented higher relative to the control group (P < .05). Conclusion: Butylphthalide injection in combination with edaravone can effectively inhibit the inflammatory response and oxidative stress, promote vascular endothelial function, improve daily behavior ability as well as promote the neurological function of elderly acute cerebral infarction patients.
Therapeutic Effects Of Aspirin Combined With Atorvastatin On Ischemic Strokes

Abstract:Objective: The aim of this study was to investigate the efficacy of aspirin combined with atorvastatin in the treatment of ischemic stroke. Methods: A total of 127 patients with ischemic strokes, admitted to The Fifth Affiliated Hospital, Harbin Medical University, from January 2016 to December 2017, were randomly divided into an observation group (n=66) and control group (n=61). In the control group, patients were treated with aspirin alone. Patients in the observation group were treated with aspirin combined with atorvastatin. Oxidized low-density lipoprotein (OX-LDL), transforming growth factor-beta 1 (TGF-beta 1), vascular cell adhesion molecule-1 (VCAM-1), high-sensitivity C-reactive protein (hs-CRP), and lipid levels in serum were measured before treatment and at 6 months after treatment. Modified RANKIN scale (mRS) scores, modified Barthel index (MBI) scores, carotid intimal plaque area, and carotid intima-media thickness were compared before treatment and 6 months after treatment for the two groups. Recurrence rates of ischemic strokes, incidence of cerebrovascular disease, and incidence of adverse reactions were compared between the two groups. Results: After 6 months of treatment, levels of OX-LDL, VCAM-1, hs-CRP, total cholesterol, low-density lipoprotein cholesterol, and triglycerides in serum in the observation group were significantly lower than those before treatment (all P < 0.05). Levels of serum TGF-beta 1 and high-density lipoprotein cholesterol were significantly higher than those before treatment (both P < 0.05). Serum levels of OX-LDL, TGF-beta 1, hs-CRP, VCAM-1, and lipids were not significantly different before and after treatment in the control group as well as between the two groups before treatment (all P > 0.05). However, there were significant differences in serum levels of the above five indicators between the two groups at 6 months after treatment (all P < 0.05). There were no significant differences in mRS scores, MBI scores, carotid intimal plaque area, and carotid intima-media thickness between the two groups before treatment (all P > 0.05). After 6 months of treatment, mRS scores of the observation group were significantly lower than that of the control group. MBI scores were significantly higher than that of the control group. Carotid intimal plaque area and carotid intima-media thickness were both less than those of the control group. Differences were statistically significant (all P < 0.05). Recurrence rate of ischemic stroke and incidence of cerebrovascular disease in the observation group were significantly lower than those in the control group (both P < 0.05). There were no significant differences in incidence of adverse reactions between the two groups (P > 0.05). Conclusion: Aspirin combined with atorvastatin has synergistic effects in the treatment of ischemic stroke. It has significant effects on regulating lipids, improving atherosclerotic plaque, and improving quality of life. It is also safe and worthy of clinical application.
Electroacupuncture improves cognitive impairment after ischemic stroke based on regulation of mitochondrial dynamics through SIRT1/PGC-1α pathway

Abstract:In recent years, the mechanism of acupuncture in the treatment of post-stroke cognitive impairment (PSCI) has not been fully elucidated. The balance between mitochondrial fission and fusion is important for PSCI. Our previous research demonstrated that electroacupuncture can improve learning and memory in middle cerebral artery ischemia reperfusion (MCAO/R) rats. However, the specific mechanism by which electroacupuncture improves learning and memory in MCAO/R rats by regulating mitochondrial fission and fusion needs to be further investigated. The MCAO/R rats was developed using the line-bolt method. The rats were randomly divided into sham-operated (Sham), model (MCAO/R), electroacupuncture (MCAO/R + EA) and sham-electroacupuncture (MCAO/R + sham EA) groups. Investigating the effects of EA on the expression of Sirtuin1 (SIRT1), peroxisome proliferator-activated receptor γ coactivator-1α (PGC-1α), Optic atrophy 1R + (OPA1) and Dynamin-related protein 1 (DRP1) in hippocampal neurons and on the morphology and function of hippocampal neurons and mitochondria. EA was able to reduce neurologic deficit scores and cerebral infarct volume and improve new object discrimination in MCAO/R rats, but there were no significant changes in these indices in the sham-electroacupuncture group. Moreover, EA increased the expression of SIRT1, PGC-1α, and OPA1 in hippocampal tissues, inhibited the expression of DRP1, attenuated neuronal and mitochondrial damage, and reduced mitochondrial fragmentation. The mechanism by which EA improves learning memory deficits in MCAO/R rats may be related to the inhibition of SIRT1/PGC-1α expression, the enhancement of mitochondrial fusion and the obstruction of its fission, and the reduction of hippocampal neuronal damage.
Abstract WMP111: Intra-Arterial Administration of Cerebral Endothelial Cell Derived Small Extracellular Vesicles Reduces Neurovascular Damage and Improves Neurological Outcome After Acute Ischemia in the Aged Rat

Abstract:Stroke is a leading cause of death and disability worldwide, mainly affecting the elderly. Endovascular thrombectomy (EVT) is a standard of care therapy for stroke patients with large-vessel occlusion, however, the majority of these patients do not achieve full functional improvement. The present study tested the hypothesis that intra-arterial (IA) administration of cerebral endothelial cell derived small extracellular vesicles (CEC-sEVs) following transient middle cerebral artery occlusion (tMCAO) improves stroke outcome in the aged rat. CEC-sEVs (1x10 11 particles/injection) isolated from healthy adult rats were employed. Male aged rats (18 months, n=10/group) subjected to 3h tMCAO were randomly divided into the experimental groups. To examine whether IA injection of CEC-sEVs improves stroke outcome, CEC-sEVs were administered via the internal carotid artery (ICA) immediately following reperfusion by withdrawal of the occlusion filament. To examine whether intravenous administration (IV) of CEC-sEVs during tMCAO and then followed by IA CEC-sEVs upon reperfusion further enhances stroke outcome, CEC-sEVs were given at 30 min of tMCAO via a tail vein and upon reperfusion via the ICA. The ischemic rats treated with IA saline were used as a control. Neurological outcome and infarct volume were assessed in a blinded manner. IA CEC-sEVs upon reperfusion significantly reduced infarct volume (24±5% vs 31±6% in saline, p<0.05) which was associated with robust improvement of neurological function from weeks 2 to 4 after tMCAO compared to saline treated rats. Moreover, rats treated with CEC-sEVs IV at 30min followed by IA treatment upon reperfusion further reduced infarct volume (16±6%) compared to rats treated with IA saline or CEC-sEVs. These rats also exhibited robust and early improvement of neurological function starting at 1d after tMCAO. In conclusion, IA administration of CEC-sEVs upon reperfusion significantly reduces ischemic damage, while an additional early (30min post stroke) dose of IV CEC-sEVs further enhances and accelerates the therapeutic effect of IA CEC-sEVs in the aged rats after tMCAO. Our data suggest that CEC-sEVs could potentially may be a therapy to amplify the efficacy of EVT even for elderly patients with acute ischemic stroke.
