Abstract:Purpose of review In this study, we review the evidence for both long-standing and newer oral antiplatelet agents as secondary prevention in coronary artery disease, and give our opinion on where each agent's treatment role lies. Recent findings Platelets play a pathological role in acute coronary syndromes and are therefore a major therapeutic target. The caveat to this is that their physiological haemostatic role means there must be a careful balance between preventing ischaemia and not promoting bleeding. In addition to accepted oral agents (aspirin and clopidogrel), more potent antiplatelet agents have recently become available (prasugrel and ticagrelor) at a cost of increased bleeding. Summary There is now a choice of three antiplatelet agents to be used in conjunction with aspirin for secondary prevention with dual antiplatelet therapy. Clinicians must now ‘tailor’ the correct therapy for each patient, depending on their presentation, clinical features and stage of risk.
Enzymatic Preparation of Immunomodulating Hydrolysates from Soy Proteins

Abstract:Soy protein hydrolysates with lower molecular weight were enzymatically prepared by several commercially available proteases (Alcalase 2.4L, Flavourzyme, Trypsin, Papain, Protease A and Peptidase R) with protein recovery varied from 42.59% to 79.87%. Relative content of positively charged peptides was determined on SP Sephadex C-25 using gradient sodium chloride solution as eluents. Immunomodulating properties were evaluated by measuring their effect on in vitro proliferation of murine spleen lymphocytes and phagocytic activity of peritoneal macrophages. The results showed that soy protein hydrolysates (SPHs) prepared with Alcalase and insoluble soy protein (InSP), preferable to other enzymes and soy proteins, have the highest immunomodulating activity and the optimum conditions were determined as follows: E/S=2% (Alcalase), 60 degrees C, pH 8.0, InSP concentration 6% and 225min. Positive correlations were obtained between the immunomodulating activity and content of positively charged peptides. The results suggested that lower molecular weight and positively charged peptides released from soy protein were effective in stimulating immunomodulating activity, thus provided insights into the preparation of potent immunomodulating products.
Identification of the Critical Amino Acids of IgE-Binding Epitopes in α-Lactalbumin

Abstract:α-Lactalbumin represents one of the major allergens causing cow milk allergy.The identification of the epitopes of α-lactalbumin and the critical amino acids for its allergenicity is of great significance for understanding the mechanism of action of α-lactalbumin and developing hypoallergenic dairy products.In this study,a series of peptides were synthesized by a solid phase method for the characterization of immunological epitopes and critical amino acids.The immunoglobulin (Ig) E-binding epitopes were immunolabeled with individual sera from cow milk-allergic patients as probes by enzyme linked immunosorbent assay (ELISA).Alanine scanning of immunodominant epitopes was used to identify the critical amino acids (aa).The results showed that IgE-binding epitopes were located within the sequences of aa1-15,aa6-20,aa46-60,aa71-85 and aa101-115.Our initial data revealed that Val8,Phe9,Arg10,Tyr103,Leu105 and His107 were the critical amino acids for IgE-binding epitopes.This study will provide the necessary information to alter the cDNA to encode a protein capable of activating milk-specific T cells,but with reduced IgE-binding capacity.

Abstract:β-Lactoglobulin represents one of the major allergens causing cow milk allergy. Few studies have clearly evaluated immunological relationships between IgE- and IgG-binding epitopes of β-lactoglobulin. For characterization of immunological epitopes, peptides of 15 amino acids (AA) in length were synthesized. Immunoglobulin E- and IgG-binding epitopes were immunolabeled with individual sera from cow milk-allergic patients. Alanine scanning of immunodominant epitopes was used to identify the critical AA of IgE- and IgG-binding epitopes. The results showed that 4 IgE-binding epitopes were identified. Our initial data revealed IgE-binding epitopes at AA 17 to 31, AA 72 to 86, AA 92 to 106, and AA 152 to 166. Threonine 20, Met23, and Asp27 are the critical AA of IgE-binding epitopes. Two IgG epitopes were identified, which were located at AA 22 to 36 and AA 127 to 141. The critical AA of IgG-binding epitopes were Leu26 and Val31. Results obtained from this study will provide necessary information to alter the cDNA to encode a protein capable of activating milk-specific T cells, but with reduced IgE- or IgGbinding capacity.
Lactic acid bacteria modulate the gastrointestinal digestive behavior of soy glycinin and correlation with its immunoreactivity: A peptidomics study

Abstract:Lactic acid bacteria fermentation is a highly promising bioprocess for reducing the immunoreactivity of soy protein. Nevertheless, the effect of lactic acid bacteria fermentation on a particular soy allergen and the consequent dynamic change of epitopes during gastrointestinal digestion are unclear. In this study, soy glycinin was isolated and an in vitro dynamic gastrointestinal model was established to investigate the dynamic change of immunoreactivity and peptide profile of unfermented (UG) and fermented glycinin (FG) digestates. Results demonstrated FG intestinal digestate had lower antigenicity (0.08%–0.12%) and IgE-binding capacity (1.49%–3.61%) of glycinin at the early (I-5) and middle (I-30) stages of gastrointestinal digestion, especially those prepared at 2% (w/v) protein concentration. Peptidomic analysis showed that the peptidome of FG digestate was 1.07-, 1.51-, and 1.08-fold that of UG at I-5, I-30, and I-180, respectively. Glycinin subunits G1 and G2 were the preferred ones to release the most abundant peptides, whereas G2, G4, and G5 had an elevated epitope-cleavage rate in FG at stages I-5 and I-30. Among 16 epitopes with more than two records in IEDB epitopes, 11 showed increased cleavage frequency in FG at stages I-5 and I-30 of gastrointestinal digestion. Three-dimensional modeling revealed that fermentation-induced differential degradation epitopes in gastrointestinal digestion were predominantly located in the α-helix and β-sheet structures. They were closely correlated with the reduced immunoreactivity of soy glycinin.

Abstract:αs1-Casein represents one of the major allergens causing cow milk allergy. Few studies have clearly evaluated immunological relationships between IgE- and IgG-binding epitopes of αs1-casein. This study aimed to map IgE- and IgG-binding epitopes of αs1-casein by the serology method, and identify the critical amino acids of αs1-casein by alanine scanning analysis. Our initial data revealed IgE-binding epitopes located in the sequences of AA 126 to 140, AA 6 to 20, AA 171 to 185, and AA 11 to 25. The sequences at AA 21 to 35, AA 56 to 70, and AA 161 to 175 were recognized by IgG antibodies. The alanine scanning analysis showed that IgE- and IgG-binding epitopes share the same critical AA: arginine at position 22 and phenylalanine at position 23. Results obtained from this study will provide necessary information to alter the cDNA to encode a protein with reduced IgE- or IgG-binding capacity.

Abstract:Transglutaminase (TGase)-catalyzed crosslinking has gained substantial traction as a novel strategy for reducing allergenic risk in food proteins, particularly within the realm of hypoallergenic food production. This study explored the impact of TGase crosslinking on conformational changes in a binary protein system composed of soy protein isolate (SPI) and sodium caseinate (SC) at varying mass ratios (10:0, 7:3, 5:5, 3:7 (w/w)). Specifically, the immunoglobulin E (IgE) binding capacity of soy proteins within this system was examined. Prolonged TGase crosslinking (ranging from 0 h to 15 h) resulted in a gradual reduction in IgE reactivity across all SPI-SC ratios, with the order of IgE-binding capability as follows: SPI > SPI5-SC5 > SPI7-SC3 > SPI3-SC7. These alterations in protein conformation following TGase crosslinking, as demonstrated by variable intrinsic fluorescence, altered surface hydrophobicity, increased ultraviolet absorption and reduced free sulfhydryl content, were identified as the underlying causes. Additionally, ionic bonds were found to play a significant role in maintaining the structure of the dual-protein system after crosslinking, with hydrophobic forces and hydrogen bonds serving as supplementary forces. Generally, the dual-protein system may exhibit enhanced efficacy in reducing the allergenicity of soy protein.

[Pre- and post-operative management of cesarean section in a parturient with severe preeclampsia accompanied by hyperdynamic state].

Abstract:A 37-year-old parturient with severe preeclampsia accompanied by pulmonary edema underwent emergency cesarean section. Pulmonary artery (PA) catheter inserted while the patient was awake revealed hyperdynamic state with increased cardiac index and preload, and decreased systemic vascular resistance. Epidural anesthesia and analgesia were provided with a satisfactory outcome. Monitoring of PA pressure and cardiac index was continued postoperatively in ICU for fluid management. We conclude that preoperative PA catheterization provides useful hemodynamic information in severe preeclamptic patients associated with persistent oliguria, pulmonary edema and hyperdynamic state.

Abstract:α-Lactalbumin is one of the major allergens in cow milk. Characterizing the critical amino acids in the epitopes of α-lactalbumin is of great significance for the understanding of the role of immunoglobulin G (IgG) in milk allergy and the development of hypoallergenic dairy products. In this study, the alanine scanning of immunodominant epitopes was used to identify the critical amino acids. First, a series of peptides based on the sequence of α-lactalbumin were identified with IgG in individual sera from cow milk-allergic patients by enzyme-linked immunosorbent assay (ELISA). Then, new peptides were synthesized by sequential replacement of the amino acids acting on the epitopes with alanine, and the critical amino acids were identified with serum pool from cow milk-allergic patients by ELISA. The results showed that IgG-binding epitopes were located in the sequences of aa 6-20, aa 21-35, aa 36-50 and aa 86-100. Our data revealed Phe9, Leu15, Pro24, Trp26and His32were the critical aa in the IgG-binding epitopes.
Simultaneous detection of glycinin and β-conglycinin in processed soybean products by high-performance liquid chromatography-tandem mass spectrometry with stable isotope-labeled standard peptides

Abstract:Glycinin and β-conglycinin are the two main allergic proteins in soybean. Due to their complex structures and lack of protein standards, it is difficult to achieve quantitative determination of these proteins in soybeans. In this study, an HPLC-MS/MS method was developed for the simultaneous determination of five subunits of glycinin (G1, G2, G3, G4, and G5) and three subunits of β-conglycinin (α, α', and β) in processed soybean products based on 8 specific peptides and their stable isotope-labeled peptides. Here, each specific peptide was derived from one of the above 8 subunits. When soy protein was extracted and digested with trypsin, 8 specific peptides, and corresponding stable isotope-labeled peptides were analyzed by HPLC-MS/MS. The linear range for the specific peptides was between 3.2 and 1000 ng/mL (R2 > 0.9955). The recoveries of added peptides ranged from 83.4% to 117.8%, and the intra-day precisions (% CV) were below 17.4%. The limit of quantification of each subunit of glycinin and β-conglycinin in processed soybean products (in terms of protein amount) was between 15.1 and 156.1 g/g. This method was successfully applied to the analysis of 8 subunits of glycinin and β-conglycinin in 68 different processed soybean products, which provides technical support for processed product quality evaluation and monitoring soybean processing technology.

Abstract:Background Soybean protein is used in a number of food products but is also a common cause of food allergy. Soybean glycinin and beta-conglycinin represent up to one-third of protein in the soybean. Many reports have indicated that glycinin and beta-conglycinin have been characterized as major soybean allergens involved in food hypersensitivity.Objective To investigate oral allergy syndrome and anaphylactic reactions in BALB/c mice caused by soybean glycinin and beta-conglycinin with an intragastric feeding protocol without using an adjuvant.Methods BALB/c mice were sensitized by gavages with glycinin and beta-conglycinin, and allergen-specific IgE and IgG1 responses were studied by a passive cutaneous anaphylaxis assay. Serum histamine release and blood pressure were measured according to other methods. Epithelium and mast cell dye used the method of light microscopy.Results Sensitization with soybean allergens induced high levels of antigen-specific IgE and IgG1 and increased serum histamine in BALB/c mice. Percentiles of intact mast cell of small intestine in mice sensitized with glycinin and beta-conglyinin significantly decreased for 28 days. Degranulation of mast cells and damage of the epithelium in the small intestine of mice sensitized with globulins were observed. The level of blood pressure in sensitized mice reached a minimum at 3 h.Conclusion Soybean-specific IgE and IgG1 antibodies increased, with high levels of histamine release, severe degranulation of mast cells and damage of the epithelium of small intestine in mice sensitized with glycinin and beta-conglyinin.
Intestinal Proline is a Potential Anti-Allergy Factor for Allergy Diagnosis and Therapy

Abstract:Allergy has become a public health problem worldwide, but effective diagnostic and therapeutic approaches are limited currently. Amino acids are essential macronutrients that potentially participated in the allergy process. This work aimed to investigate whether amino acids can be applied as a mediator for allergy diagnosis and therapy. Two cohort studies were performed to investigate the correlation between fecal amino acids and allergy responses, and a spleen cell model was used to validate the role of amino acids in regulating allergy. In a cohort study with 193 volunteers, fecal proline was found to be negatively correlated with serum IgE, and detailed data analysis revealed that people with high-IgE-mediated allergy had decreased odds of high intestinal proline. In another cohort study with distinct allergic and non-allergic individuals, proline concentration was significantly lower in the allergic group. Daily diet and metagenomics analysis showed that the proline intake and microbiota amino acid metabolism were not significantly different, implying that the body’s proline metabolism might be different between allergic and non-allergic individuals. Furthermore, the spleen cell model demonstrated that proline specifically targeted Th2 and Treg activity. Overall, this work revealed a tight correlation between gut proline and serum IgE, indicating proline as a promising biomarker and a potential therapeutic method for allergic diseases.

Abstract:Glycinin and beta-conglycinin are major soybean allergens involved in food hypersensitivity. However, the mechanism of immune responses induced by glycinin and -conglycinin has not been fully understood. Balb/c mice were oral sensitised with different doses (0.1, 1.0 and 10 mg/day) of soybean glycinin and -conglycinin for five weeks. Allergen-specific immunoglobulin (Ig), serum histamine and T-cell responses were tested to assess the allergenic activity of glycinin and -conglycinin. Mice sensitised with 0.1 or 1.0 mg/day allergens induced high levels of specific IgE, IgG1 and serum histamine compared with mice treated with saline. Furthermore, specific T-cell proliferation and significant up-regulation of interleukin (IL)-4, IL-5 and interferon (IFN)- were observed in splenocytes from mice gavaged with 0.1 or 1.0 mg/day soybean proteins. Low doses of glycinin or -conglycinin can induce allergic reactions in BALB/c mice, which might be associated with increased IgE and cytokine production.
Comparison of proanthocyanidins A2 and B2 on IgE-reactivity and epitopes in Gly m 6 using multispectral, LC/MS-MS and molecular docking

Abstract:This study comparatively analyzed the changes in IgE-reactivity and epitopes in proanthocyanidins A2- (PA-Gly m 6) and B2-Gly m 6 (PB-Gly m 6) conjugates prepared by alkali treatment at 80 °C for 20 min. Similar to the western blot, ELISA also showed a higher reduced IgE-reactivity in PA-Gly m 6 (70.12 %) than PB-Gly m 6 (63.17 %). SDS-PAGE demonstrated that proanthocyanidins A2 caused more formation of >180 kDa polymers than proanthocyanidins B2. Multispectral analyses revealed that PA-Gly m 6 exhibited more structural alteration (e.g., a decrease of α -helical content and ANS fluorescence intensity) to unfold protein structure than proanthocyanidins B2, improving the accessibility to modify Gly m 6 for shielding or destroying conformational epitopes. LC/MS-MS revealed that PA-Gly m 6 conjugates had a lower abundance of allergens, peptides and linear epitopes than PB-Gly m 6 conjugates. Molecular docking showed that proanthocyanidins A2 and B2 reacted with Gln-317 and Asn-94 of epitopes, respectively. Overall, proanthocyanidins A2 is more effective than proanthocyanidins B2 to decrease the IgE-reactivity of Gly m 6 due to more shielding or destruction of conformational epitopes and lower content allergens and linear epitopes, which was attributed to more protein-crosslinks formation and structural changes in PA-Gly m 6 conjugates.

Abstract:Linear IgE epitopes play essential roles in persistent allergies, including peanut and tree nut allergies. Using chemically synthesized peptides attached to membranes and microarray experiments is one approach for determining predominant epitopes that has seen success. However, the overall expense of this approach and the inherent challenges in scaling up the production and purification of synthetic peptides precludes the general application of this approach. To overcome this problem, we have constructed a plasmid vector for expressing peptides sandwiched between an N-terminal His-tag and a trimeric protein. The vector was used to make overlapping peptides derived from peanut allergens Ara h 2. All the peptides were successfully expressed and purified. The resulting peptides were applied to identify IgE binding epitopes of Ara h 2 using four sera samples from individuals with known peanut allergies. New and previously defined dominant IgE binding epitopes of Ara h 2 were identified. This system may be readily applied to produce agents for component- and epitope-resolved food allergy diagnosis.

Abstract:Trichosanthin (TCS) possesses many biological and pharmaceutical activities, but its strong immunogenicity limits its clinical application. To reduce the immunogenicity of TCS, we modified the reported method for the prediction of antigenic site and identified two crucial amino acid residues (Y55 and D78) for a new epitope. We mutated these two residues into glycine and serine, respectively, and obtained three mutants, Y55G, D78S, and Y55G/D78S. These mutants induced less amount of Ig and IgG antibodies in C57BL/6J mice than wild-type TCS (wTCS) (p<0.01) and almost lost the ability to induce IgE antibody production. The mutants stimulated fewer TCS-specific B cells in C57BL/6J mice than wTCS (p<0.01). Compared with wTCS, Y55G, D78S, and Y55G/D78S lost 26.9%, 17.9%, and 98.7% specific binding ability to anti-TCS monoclonal antibody TCS4E9, respectively. These mutants still retained RNA N-glycosidase activity. In conclusion, Y55 and D78 are two crucial amino acid residues of a new IgE epitope on TCS, and their mutation reduces the immunogenicity of TCS, but still retained the enzymatic activity.
Immunoglobulin E and Immunoglobulin G Cross-Reactive Allergens and Epitopes Between Cow Milk Αs1-Casein and Soybean Proteins

Abstract:Some infants allergic to cow milk-based formula are also sensitive to soybean-based formula. This paper aimed to explore the association of IgE and IgG cross-reactivity between αS1-casein in cow milk (CM) and soybean proteins. The IgE and IgG cross-reactive allergens and epitopes were identified using sera from infants allergic to CM or mice monoclonal antibodies. The AA sequence alignment was performed using bioinformatics software. Finally, the digestion and heating stability of the cross-reactive allergen were explored by sodium dodecyl sulfate (SDS)-PAGE and Western blotting. The results showed that the IgE and IgG cross-reactive allergen was α subunit of β-conglycinin named Gly m Bd 60K. The IgE and IgG epitopes were the sequences at AA 319-341 and AA 164-182. No intact Gly m Bd 60K allergen could be observed after 2 min in simulated gastric fluid by SDS-PAGE. Heating did not change IgE and IgG cross-reactivity by Western blotting. Therefore, the existence of cross-reactivity between CM αS1-casein and soybean proteins possibly contributes to the frequently observed cosensitization for these allergens in cow milk-allergic patients. The same IgE- and IgG-binding epitopes of cross-reactive allergens may provide important information for elucidation of the association between IgG and IgE antibody generation.

Abstract:Allergen G1b33 is an important allergen in rice that can cause allergic reactions such as asthma and atopic dermatitis. However, knowledge of the content in rice is sparse. In the present work, an absolute protein quantification method was established for allergen G1b33 in rice samples using liquid chromatography-tandem mass spectrometry. After extraction of allergen G1b33 from rice grains using salt solution, the isotope-labeled peptide internal standard was added to the extract, followed by enzymatic digestion with trypsin. The signature peptide and its isotope-labeled analogue from the tryptic hydrolysates of allergen Glb33 and the internal standard were detected by liquid chromatography tandem mass spectrometry. The quantitative bias caused by tryptic efficiency and matrix effect was corrected by using two isotope-labeled standard peptides. The method exhibited good linearity in the range of 1-200 nM, with coefficients of determination of le > 0.998. A high sensitivity was observed, with a limit of quantification of 0.97 nM. Mean recoveries obtained from different rice matrices ranged from 82.7%-98.1% with precision <8.5% in intraday trials (n = 6), while mean recoveries were from 75.1%-107.4% with precision <14.6% in interday trials (n = 14). The developed method was successfully applied to the analysis of allergen Glb33 in 24 different rice cultivars.
Establishment of DAS-ELISA for the detection of antigenic changes in glycinin after heat processing

Abstract:In this study, a double antibody sandwich enzyme-linked immunosorbent assay (DAS-ELISA) method was established to detect the antigenic changes of thermally processed products containing glycinin. The proposed DAS-ELISA method used heat-treated antigen-absorbing antiserum as the coating antibody, and horseradish peroxidase (HRP)-labeled rabbit anti-glycinin polyclonal antibody as the detection antibody. The specificity test results which were obtained using the proposed method indicated that good specificity had been achieved. The cut-off value was 0.388, and the LOD was determined to be 19.53 ng/mL. The coefficient of variation was less than 5.25% (intra-day) and 9.50% (inter-day). In this study's milk powder addition test, the recovery rate of the glycinin ranged between 83.65% and 90.13%. The established DAS-ELISA method was also used to detect soybean thermal processing products, such as soy sauce, steamed fish and soy sauce, soybean paste, beef sauce, soy milk powder, and tofu. The results showed that the OD450 values of the aforementioned products were lower than the OD450 values of the glycinin in defatted soybean flour. Therefore, it was indicated that the above products has undergone different degrees of thermal processing. In other words, the majority of the epitopes of glycinin in the products had been destroyed by the thermal processing and could not be combined with heat-treated antigen-absorbing antiserum.

Abstract:Glycinin and beta-conglycinin are major soybean allergens involved in food hypersensitivity. The study was aimed to investigate the soybean protein-induced intestinal immune responses and its possible mechanism. Balb/c mice were sensitised intragastrically with glycinin and beta-conglycinin without an adjuvant for five weeks. Results showed that the sensitised mice displayed diarrhoea symptoms and jejunal morphological changes, including decreased villous height and thickened crypt depth. Both the histamine and immunoglobulin A levels in jejunum were increased in soybean allergen-sensitised mice. Moreover, the number of IgA+B cells and CD4+T cells in the jejunal lamina propria of sensitised mice were significantly increased. The expression levels of interleukin-4 and interferon-gamma were increased, whereas the levels of IL-10 and transforming growth factor-beta (TGF-beta) were decreased in the jejunum mucosa. In conclusion, glycinin and beta-conglycinin induce a mixed Th1/Th2 immune response. The suppression of IL-10 and TGF-beta may play important roles in the development of intestinal allergic reactions.
