Abstract:Abstract Abstract 3054 Background: Disease relapse, major infection and graft-versus-host disease (GVHD) are the leading causes of death following allogeneic hematopoietic stem cell transplantation (allo-HSCT). Donor T lymphocytes play a critical role in alloimmune recognition and their ability to detect non-self-antigens can lead to GVHD or contribute to prevention of relapse and infection. Three effector pathways have been described for T-cell cytotoxicity: granzyme B/perforin, Fas/Fas ligand (FasL) and secreted molecules such as tumor necrosis factor (TNF)-alpha. Granzyme B is a proapoptotic serine protease. Fas is widely expressed in immunocompetent cells and non-lymphoid tissues. FasL is mainly expressed on activated cytotoxic T lymphocytes (CTLs). When CTLs recognize target cells, they become activated and express FasL which induces apoptosis of target cells. The Fas gene has been found to be silenced in many tumor types. The importance of those effector pathways for the development of acute GVHD (aGVHD) has been well established. In our previous study, we have reported donor and/or recipient TNF-alpha gene polymorphism influenced on the risk of aGVHD. Recently several important polymorphisms have been identified in granzyme B, Fas, and FasL genes and reported to be relevant to allograft rejection in solid organ transplant patients. However, such information is less available in allo-HSCT. This present study was designed to investigate the influence of donor and recipient genetic variations of effector pathway molecules on the abilities of T-cell alloimmune responses including GVHD, cytomegalovirus (CMV) infection within 100 days and disease relapse following allo-HSCT. Methods: The entire study population consisted of 138 pairs of recipients and their unrelated donors, who underwent HSCT in our Center from January 2001 to March 2009. Genomic DNA was extracted from peripheral blood samples obtained from recipients and donors before transplantation. Five single nucleotide polymorphisms (SNPs) including granzyme B +55(A/G), -295(C/T), Fas -1377(G/A), -670(T/C) and FasL -844 (T/C) were analyzed. Results: (1) Patients receiving stem cells from a donor with granzyme B +55 AA genotype developed aGVHD more frequently than those with mutated allele (AG or GG), which has been reported to be associated with low-producing of granzyme B by CTLs and incapable of inducing apoptosis. Multivariate analysis confirmed that donor with the granzyme B +55 AA genotype was an independent risk factor for aGVHD (RR=1.545, 95%CI: 1.012–2.357, P=0.044). (2) Donor with FasL -844 TT genotype, a genotype reported to a reduction in the expression of FasL on T cells, conferred protection against the risk of aGVHD (RR=2.981, 95% CI: 1.320–6.736, P=0.009), however was an independent risk factor for early CMV infection (RR: 3.658, 95% CI: 1.534–8.722, P=0.003). Patients experiencing aGVHD have a higher risk of developing early CMV infection (RR=1.931, 95% CI: 1.242–3.003, P=0.003), early CMV infection also was a significant factor impacting aGVHD (RR=1.630, 95%CI: 1.294–2.053, P<0.001). (3) Recipient with Fas -670 C- allele positive, a mutated allele with higher Fas expression, was associated with a significantly lower risk of relapse (RR=3.453, 95% CI: 1.699–7.091, P=0.001) as well as superior overall survival (84.2% vs 21.2%, P<0.001) in patients with acute myeloid leukemia (AML), but not in patients with other myelogenous or lymphoid malignancies. The polymorphic sites granzyme B -295(C/T), Fas -1377(G/A), did not correlate with the T-cell alloimmune reactivities. Conclusions: Genetic variation, other than HLA polymorphism, regulating the immune response may influence outcomes of patients undergoing allo-HSCT. Transplant patients would thus benefit from accurate risk assessment and individualization of immunosuppressive therapy, taking into account donor and recipient genetic data, as well as other risk factors. This study is the first report of relationship between donor or recipient genetic variations of effector pathway molecules and the abilities of T-cell alloimmune responses. According to our results, special targeting granzyme B and Fas/FasL pathways may be a promising therapeutic strategy in patients with aGVHD, CMV infection and leukemia relapse after allo-HSCT at least in AML patients harboring Fas -670 wild-type genotype. Abstract:Acute lymphoblastic leukemia (ALL) is the most common cancer in children, and the etiology of this aggressive cancer is not fully understood. Common germline polymorphisms in lymphoid development genes and tumor suppressor genes have been associated with ALL susceptibility, although most have modest effects. Only a small fraction of ALL cases are thought to be related to congenital genetic disorders and consequently hereditary predisposition is rarely considered in clinical practice. However, a growing number of rare germline genetic mutations have been discovered in familial ALL (e.g., PAX5, TP53), raising the possibility that the proportion of ALL attributable to inherited predisposition may be higher than currently proposed. In particular, germline ETV6 variations were recently reported in families with hereditary thrombocytopenia and dramatically increased susceptibility to hematologic malignancies (Nat Genet 2015 47: 180 and 535). ETV6 is a transcriptional repressor essential for hematopoiesis and is frequently targeted by somatic genomic aberrations in childhood ALL (e.g., the ETV6-RUNX1 fusion). Therefore, we sought to comprehensively identify ALL predisposition variants in ETV6 and to determine the extent to which these variants contribute to childhood ALL risk in general.

Abstract:Somatic genetic alterations of IKZF1, which encodes the lymphoid transcription factor IKAROS, are common in high-risk B-progenitor acute lymphoblastic leukemia (ALL) and are associated with poor prognosis. Such alterations result in the acquisition of stem cell-like features, overexpression of adhesion molecules causing aberrant cell-cell and cell-stroma interaction, and decreased sensitivity to tyrosine kinase inhibitors. Here we report coding germline IKZF1 variation in familial childhood ALL and 0.9% of presumed sporadic B-ALL, identifying 28 unique variants in 45 children. The majority of variants adversely affected IKZF1 function and drug responsiveness of leukemic cells. These results identify IKZF1 as a leukemia predisposition gene, and emphasize the importance of germline genetic variation in the development of both familial and sporadic ALL.

Abstract:Background There is increasing evidence for an inherited predisposition to pediatric acute lymphoblastic leukemia (ALL). We and others have previously reported rare and highly penetrant variants in hematopoietic transcription factors (PAX5 and ETV6) and tumor suppressor genes (TP53) in both sporadic and familial ALL. IKZF1encodes the founding member of the Ikaros family of zinc finger transcription factors, and is a critical regulator of lymphoid development. IKZF1 is frequently targeted by somatic deletions and mutations in high-risk B-ALL, particularly Ph+ and Ph-like ALL, and is associated with poor outcome. IKZF1 alterations have previously been shown to result in the acquisition of stem cell-like features, overexpression of adhesion molecules causing aberrant cell-cell and cell-stroma interaction, and decreased sensitivity to tyrosine kinase inhibitors. Genome-wide association studies have also identified an association between common polymorphisms at the IKZF1locus and risk of developing ALL, however the nature and effects of germline IKZF1variation in the pathogenesis of ALL are poorly understood. Abstract:Germline variation and somatic mutation are intricately connected and together shape human traits and disease risks. Germline variants are present from conception, but they vary between individuals and accumulate over generations. By contrast, somatic mutations accumulate throughout life in a mosaic manner within an individual due to intrinsic and extrinsic sources of mutations and selection pressures acting on cells. Recent advancements, such as improved detection methods and increased resources for association studies, have drastically expanded our ability to investigate germline and somatic genetic variation and compare underlying mutational processes. A better understanding of the similarities and differences in the types, rates and patterns of germline and somatic variants, as well as their interplay, will help elucidate the mechanisms underlying their distinct yet interlinked roles in human health and biology.

Abstract:Multiple Myeloma (MM) is a hematological malignancy associated with poor prognosis and relapses. MM has a strong inherited component, and while the mutational and transcriptomic landscapes of MM have been characterized extensively, the molecular consequences of germline genetic variation in MM have not been previously explored. Here, we have leveraged matched genotype and expression data from 607 patients to map the germline and somatic regulatory variation in MM. We find that non-coding variation shapes the transcriptional landscape of MM, contributing to overall survival. These regulatory variants alter the binding of transcription factors with known importance in MM, such as IRF4, providing insights into the underlying mechanisms. These data contribute to the increasing understanding of the genetic, molecular, and cellular forces shaping MM risk and outcomes.

Abstract:Significance Our DNA contains a blueprint for phenotypic traits, which include the immune response to tumors. As cancer immunotherapies continue to show clinical promise, it is important to understand how inherited genetic variation may account for variable immune responses, as reflected by gene expression within the tumor. We systematically identified the germline genetic polymorphisms associated with variable tumor tissue gene expression across 24 human cancer types. We showed that expression of major regulators of immunity, such as ICOSLG and ERAP2, was under strong genetic control. Additionally, we defined germline variants associated with the abundance of immune cells that infiltrated the tumor. These data demonstrate that germline genetics is a major player in shaping the tumor environment and immune response.

Abstract:What's new? The genetic predisposition to lymphoma has yet to be fully understood. Here, the authors performed sequencing analyses in 13 lymphoma‐cancer families, in which males tended to develop lymphomas and females to develop cancers. They identified germline variants for 25 DNA repair genes and 14 immune genes in 18 lymphoma patients, with more than one third of these mutations being protein truncating. Genome instability and oncogene activation were verified in lymphoma samples, and somatic mutations mediating immune evasion were also found. These germline defects of DNA repair and immune genes suggest germline double hits in lymphomagenesis. The genetic predisposition to lymphoma is not fully understood. We identified 13 lymphoma‐cancer families (2011–2021), in which 27 individuals developed lymphomas and 26 individuals had cancers. Notably, male is the predominant gender in lymphoma patients, whereas female is the predominant gender in cancer patients (p = .019; OR = 4.72, 95% CI, 1.30–14.33). We collected samples from 18 lymphoma patients, and detected germline variants through exome sequencing. We found that germline protein truncating variants (PTVs) were enriched in DNA repair and immune genes. Totally, we identified 31 heterozygous germline mutations (including 12 PTVs) of 25 DNA repair genes and 19 heterozygous germline variants (including 7 PTVs) of 14 immune genes. PTVs of ATM and PNKP were found in two families, respectively. We performed whole genome sequencing of diffuse large B cell lymphomas (DLBCLs), translocations at IGH locus and activation of oncogenes (BCL6 and MYC) were verified, and homologous recombination deficiency was detected. In DLBCLs with germline PTVs of ATM, deletion and insertion in CD58 were further revealed. Thus, in lymphoma‐cancer families, we identified germline defects of both DNA repair and immune genes in lymphoma patients.

Abstract:Genetic alterations alone cannot account for the diverse phenotypes of cancer cells. Even cancers with the same driver mutation show significant transcriptional heterogeneity and varied responses to therapy. However, the mechanisms underpinning this heterogeneity remain under-explored. Here, we find that novel enhancer usage is a common feature in acute lymphoblastic leukemia (ALL). In particular, KMT2A::AFF1 ALL, an aggressive leukemia with a poor prognosis and a low mutational burden, exhibits substantial transcriptional heterogeneity between individuals. Using single cell multiome analysis and extensive chromatin profiling, we reveal that much transcriptional heterogeneity in KMT2A::AFF1 ALL is driven by novel enhancer usage. Using high resolution Micro-Capture-C in primary patient samples, we also identify patient-specific enhancer activity at key oncogenes such as MEIS1 and RUNX2, driving high levels of expression of both oncogenes in a patient-specific manner. Overall, our data show that enhancer heterogeneity is highly prevalent in KMT2A::AFF1 ALL and may also be a mechanism that drives transcriptional heterogeneity in cancer more generally.

Abstract:Genetic alterations in the RUNX1 gene are associated with benign and malignant blood disorders, particularly of megakaryocyte and myeloid lineages. The role of RUNX1 in acute lymphoblastic leukemia (ALL) is less clear, particularly in terms of how germline genetic variation influences the predisposition to this type of leukemia. Sequencing DNA of 4836 children with B cell ALL (B-ALL) and 1354 with T cell ALL (T-ALL), we identified 31 and 18 germline RUNX1 variants, respectively. RUNX1 variants in B-ALL consistently showed minimal damaging effects. In contrast, 6 T-ALL-related variants resulted in drastic loss of RUNX1 activity as a transcription activator in vitro. Ectopic expression of dominant-negative RUNX1 variants in human CD34(+) cells repressed differentiation into erythroid cells, megakaryocytes, and T cells, while promoting myeloid cell development. Chromatin immunoprecipitation sequencing of T-ALL models showed distinctive patterns of RUNX1 binding by variant proteins. Further whole-genome sequencing identified the JAK3 mutation as the most frequent somatic genomic abnormality in T-ALL with germline RUNX1 variants. Cointroduction of RUNX1 variant and JAK3 mutation in hematopoietic stem and progenitor cells in mice gave rise to T-ALL with the early T cell precursor phenotype. Taken together, these results indicate that RUNX1 is an important predisposition gene for T-ALL and point to biology of RUNX1-mediated leukemogenesis in the lymphoid lineages.

Abstract:We now recognize that with aging, hematopoietic stem and progenitor cells (HSPCs) acquire mutations that confer a fitness advantage and clonally expand in a process now termed clonal hematopoiesis (CH). Because CH predisposes to a variety of health problems, including cancers, cardiovascular diseases, and inflammatory conditions, there is intense interest in the inherited alleles associated with the development of CH. DNA variants near TERT , SMC4 , KPNA4 , IL12A , CD164 , and ATM confer the strongest associations. In this review, we discuss our current state of knowledge regarding germline predisposition to CH.

Abstract:Malignancy is defined by a set of abnormal biological capacities, termed the hallmarks of cancer. Decades of histopathologic assessment and molecular profiling of human tumors have demonstrated there are multiple ways cells can acquire each hallmark. As a result, tumors with the same clinical characteristics can vary dramatically across individuals and these distinct molecular vulnerabilities can have important prognostic and therapeutic implications. It is unclear when these differences originate. Oncogenic aberrations are acquired within the context of germline genomes which differ across individuals at millions of polymorphic sites, but the role of germline variants in somatic evolution remains poorly understood. The most compelling example is that deleterious germline variants in BRCA1 and, to a lesser extent, BRCA2 are preferentially associated with the development of TNBC, implying germline variants sculpt specific subtypes of disease. The mechanistic basis for this preference is incompletely characterized. Additionally, germline variants that upregulate the mTOR pathway are associated with further deregulation of mTOR via somatic PTEN loss-of-function. Moreover, pathogenic germline variants in cancer predisposition genes promote somatic bi-allelic inactivation in a lineage-dependent manner and rare variants across a diverse list of genes can modulate somatic mutational processes. In prostate cancer, germline variants can modulate genomic stability, tumor-specific DNA methylation and gene regulation at the transcriptional and translational levels. Pathogenic germline variants in genes related to telomere and mitotic function increase the risk of sarcoma. Differences in breast cancer subtype frequencies across ancestral populations further suggest germline contributions. These data point to an underappreciated role of the germline genome in somatic tumor evolution. Various lines of evidence suggest that avoidance of the adaptive immune system is another strong determinant of which somatic mutations persist within a tumor. It remains less clear how germline differences influence immunoediting. Levels of interferon signaling and cytotoxic T-cell infiltration are estimated to be 15-20% heritable. Generally, germline variants have not been considered a good source of immunogenic epitopes as cytotoxic response should be dampened by central tolerance. However, non-mutated immunogenic epitopes have been identified in genes such as ERBB2 in breast and ovarian cancer and H4 histone in prostate cancer, amongst others. Antigens with weak binding affinity for MHC receptors can escape central tolerance and elicit an immune response. Tissue-restricted post-translational modifications can also circumvent central tolerance. Peripheral self-reactive T cells are present at similar frequencies to T cells specific to foreign antigens but are held in an anergic state by regulatory T cells (Treg). However, Treg depletion in healthy mouse models led to the natural occurrence of self-reactive CD4+ T cells. Further, there is mounting evidence for innate-like T-cell populations within mouse and tumor malignancies that have increased propensity for self-reactivity. Altogether, these data suggest that under specific circumstances and disruptions to immune homeostasis, a subset of T-cells may respond to germline-derived epitopes during tumorigenesis. Building on these observations, we sought to investigate whether germline variants sculpt somatic evolution by mediating immunoediting. Specifically, we hypothesize that the burden of germline-derived epitopes in recurrently amplified driver genes may select against gene amplification. This is because amplification of a gene with a high burden of germline-derived epitopes would increase epitope availability, likelihood of epitope presentation and immune-mediated cell death. Instead, immune pressures may select for amplification of an alternate driver gene with a lower germline-mediated epitope burden. Motivation for this hypothesis draws upon insights from the immuno-oncology and population genetics fields. First, in a subset of tumors, somatic tumor mutational burden (TMB), specifically clonal TMB, is positively associated with immunotherapy response. Second, recent successes with cancer vaccines developed from non-somatically mutated, tumor-specific overexpressed peptides, i.e. HER2, TERT and WT1, suggests germline epitopes contribute to immune response. Finally, genome-wide association studies have demonstrated the power of aggregating germline-variants with weak effects into a unified risk score, i.e. polygenic risk score (PRS). Thus, we hypothesized aggregating germline-derived epitopes, with weak individual effects, akin to PRS, may provide a powerful metric to assess germline-mediated immunoediting. Abstract:Chronic myeloid leukemia (CML) typically occurs in late adulthood. Pediatric CML is a rare form of leukemia. In all age groups, the characteristic genetic driver of the disease is the BCR :: ABL1 fusion gene. However, additional genomic events contribute to leukemic transformation, which is not yet well-characterized in pediatric CML. We investigated the mutational landscape of pediatric CML to determine whether predisposing germline variants may play a role in early-age disease development. Whole exome sequencing and targeted sequencing were performed in pediatric and adult CML samples to identify age-related germline and somatic variants in addition to the BCR::ABL1 translocation. Germline variants were detected in about 60% of pediatric patients with CML, with predominantly hematopoietic genes affected, most frequently ASXL1 , NOTCH1 , KDM6B , and TET2 . The number of germline variants was significantly lower in adult patients with CML. If only confirmed pathogenic variants were regarded as cancer-predisposing variants, the occurrence was ~ 10% of pediatric CML, which is comparable to other hematological malignancies and most childhood cancer entities in general. We hypothesize that the interaction with the strong oncogene BCR::ABL1 may also favor the development of leukemia by weaker variants in the same genes. In pediatric patients, the germline variants of genes associated with clonal hematopoiesis may increase the likelihood that an incidental BCR::ABL1 translocation triggers the early manifestation of CML.

Molecular pathophysiology of germline mutations in acute myeloid leukemia

Abstract:Germline (GL) predisposition to acute myeloid leukemia (AML) has been established as an independent disease entity in the latest World Health Organization classification. Following the American College of Medical Genetics and Genomics guidelines, GL variants were interpreted as causal if they were classified as "pathogenic." GL predisposition can be divided into three groups with different phenotypes, and play an important role in the pathogenesis of adult-onset AML. The clinical course and age of onset of myeloid neoplasms varied considerably for each gene. For example, patients with GATA2 GL variants develop AML before the age of 30 along with bone marrow failure, whereas those with DDX41 GL variants tend to develop AML after the age of 50 without any preceding hematological abnormalities or organ dysfunction. A comprehensive analysis of adult-onset myelodysplastic syndromes in transplant donors showed a 7% frequency of pathogenic GL variants, with DDX41 being the most frequent gene mutation at approximately 3.8%. Future research on GL predisposition at any age of myeloid neoplasm onset will assist in early and accurate diagnosis, development of effective treatment strategies, and selection of suitable donors for stem cell transplantation.

Abstract:Background: Increasing evidence suggests germline genetic susceptibility influences the risk of developing subsequent neoplasms (SNs)—a major cause of morbidity and mortality—among childhood cancer survivors, but studies have been hampered by sample size and limited gene sets. We pooled two large-scale cohorts of childhood cancer survivors to characterize overall and specific SN risks among individuals carrying germline variants in 88 known cancer susceptibility genes with an autosomal dominant inheritance pattern. Methods: Using whole-genome or whole-exome sequencing data from the Childhood Cancer Survivor Study and St. Jude Lifetime Cohort (median [interquartile range] follow-up, 29.7 [13.8] years), we identified rare, potentially protein-damaging germline variants with SnpEff/ClinVar. Conditional logistic regression evaluated overall SN and SN-specific risk, with up to 100 SN-free controls matched to cases by age, sex, primary childhood cancer type, SN type, radiation dose, chemotherapy, study, and follow-up time. Results: Among 11840 individuals (50.6% female) who survived childhood cancer, 2162 (18.3%) developed at least one SN. The risk of developing any SN was elevated among 288 survivors carrying rare, potentially protein-damaging germline variants in an autosomal dominant cancer susceptibility gene (cases=13.3%; controls=9.6%; odds ratio [OR], 1.48; 95% confidence interval [CI], 1.30-1.70; P=5.12x10-5). A similar proportion of carriers was observed among those who developed common SNs such as basal cell carcinoma (N=107/882, 12.1%), meningioma (N=52/373, 13.9%), breast cancer (N=49/353, 13.9%), and thyroid cancer (N=32/243, 13.2%). Higher frequencies of carriers were observed in less common SNs, specifically sarcoma (N=24/151, 15.9%), colorectal cancer (N=15/72, 20.8%), and glioma (N=16/64, 25.0%). The observed associations were generally consistent regardless of treatment type for first primary childhood cancer. Gene-specific analyses revealed that the risk of developing any SN was most pronounced among individuals carrying variants in TP53 (cases=0.6%; controls=0.04%; OR, 9.93; 95% CI, 4.62-21.34; P=0.001) and FANCM (cases=0.9%; controls=0.5%; OR, 2.45; 95% CI, 1.48-4.03; P=0.018). Conclusion: In large-scale cohorts of childhood cancer survivors with long-term follow-up, individuals carrying potentially protein-damaging germline variants in known autosomal dominant cancer susceptibility genes have substantially increased SN risk, emphasizing the importance of genetic testing and consideration of genetic risk in follow-up guidelines. Abstract:Purpose Germline TP53 variation is the genetic basis of Li-Fraumeni syndrome, a highly penetrant cancer predisposition condition. Recent reports of germline TP53 variants in childhood hypodiploid acute lymphoblastic leukemia (ALL) suggest that this type of leukemia is another manifestation of Li-Fraumeni syndrome; however, the pattern, prevalence, and clinical relevance of TP53 variants in childhood ALL remain unknown. Patients and Methods Targeted sequencing of TP53 coding regions was performed in 3,801 children from the Children's Oncology Group frontline ALL clinical trials, AALL0232 and P9900. TP53 variant pathogenicity was evaluated according to experimentally determined transcriptional activity, in silico prediction of damaging effects, and prevalence in non-ALL control populations. TP53 variants were analyzed for their association with ALL presenting features and treatment outcomes. Results We identified 49 unique nonsilent rare TP53 coding variants in 77 (2.0%) of 3,801 patients sequenced, of which 22 variants were classified as pathogenic. TP53 pathogenic variants were significantly over-represented in ALL compared with non-ALL controls (odds ratio, 5.2; P < .001). Children with TP53 pathogenic variants were significantly older at ALL diagnosis (median age, 15.5 years v 7.3 years; P < .001) and were more likely to have hypodiploid ALL (65.4% v 1.2%; P < .001). Carrying germline TP53 pathogenic variants was associated with inferior event-free survival and overall survival (hazard ratio, 4.2 and 3.9; P < .001 and .001, respectively). In particular, children with TP53 pathogenic variants were at a dramatically higher risk of second cancers than those without pathogenic variants, with 5-year cumulative incidence of 25.1% and 0.7% ( P < .001), respectively. Conclusion Loss-of-function germline TP53 variants predispose children to ALL and to adverse treatment outcomes with ALL therapy, particularly the risk of second malignant neoplasms.

Abstract:Inherited non-coding genetic variants confer significant disease susceptibility in many cancers. However, the molecular processes of by which germline variants contribute to somatic lesions are poorly understood. We performed targeted sequencing in 5,008 patients and identified a key regulatory germline variant in GATA3 strongly associated with Philadelphia chromosome-like acute lymphoblastic leukemia (Ph-like ALL). By creating an isogenic cellular model with CRISPR-Cas9 system, we showed that this variant activated a strong enhancer that significantly upregulated GATA3 transcription, which in turn reshaped the global chromatin accessibility and 3D genome organization. Remarkably, this genotype switch induced a chromatin loop between the CRLF2 oncogene and a distal enhancer, similar to the somatically acquired super-enhancer hijacking event in patients. GATA3 genotype-related alterations in transcriptional control and 3D chromatin organization were further validated in Ph-like ALL patients. Finally, we showed that GATA3 directly regulates CRLF2 and potentiates the oncogenic effects of JAK-STAT signaling in leukemogenesis. Altogether, our results provide evidence for a novel mechanism by which a germline non-coding variant contributes to oncogene activation epigenetic regulation and 3D genome reprogramming.

Abstract:Few risk factors have been established for the plasma cell disorder multiple myeloma, but some of these like African American ethnicity and a family history of B-cell lymphoproliferative diseases suggest a genetic component for the disease. Genetic variation represents the genetic basis of variability in a population. The complex interplay between environment and genes for the development of cancer may therefore be influenced by genetic variations. A genetic variation may change the function of the gene, and if the genetic variation is associated with the risk of disease, that particular gene may be involved in the pathogenesis of disease. Genes of interest are genes involved in the normal development and function of the plasma cell and genes that protect us against exposures from the environment, for example, genes involved in the metabolism of xenobiotics, metabolism of folate and methionine, as well as genes involved in inflammation and DNA repair. Identification of genes with potential influence on cancer risk may help us to establish relevant laboratory studies on exposure and dose-response assessment and may help us to test the hypothesis in epidemiological studies. Knowledge of individual at high risk of cancer may offer promising insight for the prevention of cancer.

Abstract:Recognition of antigens by T cell receptors (TCRs) and B cell receptors (BCRs) is a key step in lymphocyte activation. T and B cells mediate adaptive immune responses, which protect us against infections and provide immunological memory, and also, in some instances, drive pathogenic responses in autoimmune diseases. TCRs and BCRs are encoded within loci that are known to be genetically diverse. However, the extent and functional impact of this variation, both in humans and model animals used in immunological research, remain largely unknown. Experimental and genetic evidence has demonstrated that the complementarity determining regions 1 and 2 (HCDR1 and HCDR2), encoded by the variable (V) region of TCRs and BCRs, also often make critical contacts with the targeted antigen. Thus, knowledge about allelic variation in the genes encoding TCRs and BCRs is critically important for understanding adaptive immune responses in outbred populations and to define responder and non-responder phenotypes.
