Abstract:Purpose: This study aimed to analyze the domestic and international landscape of imaging diagnostics and treatments, focusing on Japan, to provide current insights for policymaking, clinical practice enhancement, and international collaboration. Methods: Data from 1996 to 2021 were collected from Japan's Ministry of Health, Labor and Welfare database for medical device counts of CT, MRI, PET, mammography, and radiotherapy. The National Database of Health Insurance Claims and Specific Health Checkups of Japan was utilized for examination numbers. An international comparison was made with data from 41 countries using the Organization for Economic Cooperation and Development (OECD) database. Results: The data included a total of 108,596 CT devices, 47,233 MRI devices, 2998 PET devices, 20,641 MMG devices, and 8023 RT devices during the survey period. Upon international comparison, Japan ranked first in CT and MRI devices per million people and second in examination numbers per 1000 people. The number of PET devices per million people exceeded OECD averages; however, the number of examinations per 1000 people was below the OECD average in 2020 (Japan: 4.0, OECD: 4.9). Although Japan exceeded OECD averages in mammography device counts (Japan: 33.8, OECD: 24.5 in 2020), radiotherapy device counts were similar to OECD averages (Japan: 8.3, OECD: 7.9 in 2020). Conclusion: We have analyzed the utilization of equipment in the context of diagnostic imaging and radiotherapy in Japan. Since the initial survey year, all devices have shown an upward trend. However, it is essential not only to increase the number of devices and examinations but also to address the chronic shortage of radiologists and allied health professionals. Based on the insights gained from this study, understanding the actual status of diagnostic imaging and radiation therapy equipment is critical for grasping the domestic situation and may contribute to improving the quality of healthcare in Japan.
Effective Techniques to Reduce Radiation Exposure to Medical Staff during Assist of X-ray Computed Tomography Examination

Abstract:Medical staffs like radiological technologists, doctors, and nurses are at an increased risk of exposure to radiation while assisting the patient in a position or monitor contrast medium injection during computed tomography (CT). However, methods to protect medical staff from radiation exposure and protocols for using radiological protection equipment have not been standardized and differ among hospitals. In this study, the distribution of scattered X-rays in a CT room was measured by placing electronic personal dosimeters in locations where medical staff stands beside the CT scanner gantry while assisting the patient and the exposure dose was measured. Moreover, we evaluated non-uniform exposure and revealed effective techniques to reduce the exposure dose to medical staff during CT. The dose of the scattered X-rays was the lowest at the gantry and at the examination table during both head and abdominal CT. The dose was the highest at the trunk of the upper body of the operator corresponding to a height of 130 cm during head CT and at the head corresponding to a height of 150 cm during abdominal CT. The maximum dose to the crystalline lens was approximately 600 μSv during head CT. We found that the use of volumetric CT scanning and X-ray protective goggles, and face direction toward the gantry reduced the exposure dose, particularly to the crystalline lens, for which lower equivalent dose during CT scan has been recently recommended in the International Commission on Radiological Protection Publication 118.

Abstract:Background: In planning radiotherapy treatments, computed tomography (CT) has become a crucial tool. CT scans involve exposure to ionizing radiation, which can increase the risk of cancer and other adverse health effects in patients. Ionizing radiation doses for medical exposure must be kept "As Low As Reasonably Achievable". Very few articles on guidelines for radiotherapy-computed tomography scans are available. This paper reviews the current literature on radiation dose optimization based on the effective dose and diagnostic reference level (DRL) for head, neck, and pelvic CT procedures used in radiation therapy planning. This paper explores the strategies used to optimize radiation doses, and high-quality images for diagnosis and treatment planning. Methods: A cross-sectional study was conducted on 300 patients with head, neck, and pelvic region cancer in our institution. The DRL, effective dose, volumetric CT dose index (CTDIvol), and dose-length product (DLP) for the present and optimized protocol were calculated. DRLs were proposed for the DLP using the 75th percentile of the distribution. The DLP is a measure of the radiation dose received by a patient during a CT scan and is calculated by multiplying the CT dose index (CTDI) by the scan length. To calculate a DRL from a DLP, a large dataset of DLP values obtained from a specific imaging procedure must be collected and can be used to determine the median or 75th-percentile DLP value for each imaging procedure. Results: Significant variations were found in the DLP, CTDIvol, and effective dose when we compared both the standard protocol and the optimized protocol. Also, the optimized protocol was compared with other diagnostic and radiotherapy CT scan studies conducted by other centers. As a result, we found that our institution's DRL was significantly low. The optimized dose protocol showed a reduction in the CTDIvol (70% and 63%), DLP (60% and 61%), and effective dose (67% and 62%) for both head, neck, and pelvic scans. Conclusions: Optimized protocol DRLs were proposed for comparison purposes.

Abstract:BackgroundIonizing radiation generated during medical imaging procedures is a matter of concern. However, the current status of radiopharmaceutical use in stress myocardial perfusion imaging (MPI) and the radiation exposure from these radiopharmaceuticals is unknown in Japan.Methods and ResultsA nationwide survey was conducted from June through July 2016. The questionnaires on the radiopharmaceuticals used and their administered doses during stress MPI were sent to 641 nuclear medicine facilities. The responses were collected from 431 facilities and the effective dose (ED) for an adult with standard body weight was calculated. Forty-three percent of the facilities used only 201TlCl, 35% used only 99mTc radiopharmaceuticals, and the remaining 22% used both. The two main reasons for using 201TlCl instead of 99mTc radiopharmaceuticals were “more familiarity with the usage of 201TlCl than 99mTc radiopharmaceuticals” and “apprehension about increasing the burden of physicians performing tracer injection twice.” The mean ED was 14.0 ± 5.5 mSv (range, 3.9 to 25.2 mSv), which was higher than that reported in other countries.ConclusionsThe ED of stress MPI radiopharmaceuticals in Japan is probably higher than the world standard because more than 50% of the facilities still use 201TlCl. We recommend revising the routine stress MPI protocol to reduce the effects of ionizing radiation.

Abstract:Cone beam CT is integral to most modern radiotherapy treatments. The application of daily and repeat CBCT imaging can lead to high imaging doses over a large volume of tissue that extends beyond the treatment site. Hence, it is important to ensure exposures are optimised to keep doses as low as reasonably achievable, whilst ensuring images are suitable for the clinical task. This IPEM topical report presents the results of the first UK survey of dose indices in radiotherapy CBCT. Dose measurements, as defined by the Cone Beam Dose Index (CBDIw), were collected along with protocol information for seven treatment sites. Where a range of optimised protocols were available in a centre, a sample of patient data demonstrating the variation in protocol use were requested. Protocol CBDIw values were determined from the average dosimetry data for each type of linear accelerator, and median CBDIw and scan length were calculated for each treatment site at each centre. Median CBDIw values were compared and summary statistics derived that enable the setting of national dose reference levels (DRLs). A total of 63 UK radiotherapy centres contributed data. The proposed CBDIw DRLs are; prostate 20.6 mGy, gynaecological 20.8 mGy, breast 5.0 mGy, 3D-lung 6.0 mGy, 4D-lung 11.8 mGy, brain 3.5 mGy and head/neck 4.2 mGy. However, large differences between models of imaging system were noted. Where centres had pro-active optimisation strategies in place, such as sized based protocols with selection criteria, dose reductions on the 'average' patient were possible compared with vendor defaults. Optimisation of scan length was noted in some clinical sites, with Elekta users tending to fit different collimators for prostate imaging (relatively short) compared with gynaecological treatments (longest). This contrasts with most Varian users who apply the default scan length in most cases.

Abstract:PURPOSE:To develop institutional diagnostic reference levels (IDRL) for coronary CT angiography (CCTA) according to patient size by analyzing radiation dose changes over the past 10 years. MATERIALS AND METHODS:This IRB approved retrospective investigation analyzed radiation dose data from CCTA between 2007 and 2016 at our institution. Annual trends in radiation dose were described for each scanner type and scanning mode. Radiation levels were analyzed for normorhythmic patients, patients with prior coronary artery bypass grafting (CABG), arrhythmia, and according to patient size and tube voltage. Median, and quartile values for volume CT dose index (CTDIvol), dose-length product (DLP), and size-specific dose estimate (SSDE) were calculated. Wilcoxon rank-sum test and Kruskal Wallis test were performed to assess the significance of quantitative data. RESULTS:35,375 examinations from 33,317 patients (median age, 58 [50-66] years; male patients, 21,087 [58.7%]) were analyzed. CTDIvol, DLP, and SSDE significantly decreased by 9.0%, 30.8%, and 40.1% (all P < 0.05) for all examinations, respectively. All radiation dose metrics progressively decreased across scanning modes (especially retrospectively ECG-gated spiral and prospectively ECG-triggered high-pitch spiral acquisition mode), but did not significantly change across scanners in the last 6 years. CTDIvol and DLP increased with patient size when water-equivalent diameters were >19 cm for normorhythmic and CABG patients. In arrhythmic patients, CTDIvol increased progressively with water-equivalent diameters across all groups. CONCLUSION:CCTA radiation dose has progressively decreased in the past decade except in patients with prior CABG and arrhythmia. Size-specific IDRLs may optimize radiation utilization in these patients going forward.

Abstract:The concentrations of alpha-1-microglobulin, beta-2-microglobulin, and retinol binding protein were determined in fetal blood sampled by cordocentesis. The blood values of 126 fetuses without ultrasonographic findings of urinary tract abnormalities as controls were found to be independent of the week of gestation. In nine fetuses affected by a severe bilateral renal dysplasia or agenesis, elevated values of alpha-1-microglobulin but normal values of retinol binding protein were obtained. The authors recommend the determination of alpha-1-microglobulin and, with some restriction, also of beta-2-microglobulin in prenatal renal function diagnosis.

Abstract:Abstract Ob jectives The European Society of Radiology identified 10 common indications for computed tomography (CT) as part of the European Study on Clinical Diagnostic Reference Levels (DRLs, EUCLID), to help standardize radiation doses. The objective of this study is to generate DRLs and median doses for these indications using data from the UCSF CT International Dose Registry. Methods Standardized data on 3.7 million CTs in adults were collected between 2016 and 2019 from 161 institutions across seven countries (United States of America (US), Switzerland, Netherlands, Germany, UK, Israel, Japan). DRLs (75th percentile) and median doses for volumetric CT-dose index (CTDI vol ) and dose-length product (DLP) were assessed for each EUCLID category (chronic sinusitis, stroke, cervical spine trauma, coronary calcium scoring, lung cancer, pulmonary embolism, coronary CT angiography, hepatocellular carcinoma (HCC), colic/abdominal pain, appendicitis), and US radiation doses were compared with European. Results The number of CT scans within EUCLID categories ranged from 8,933 (HCC) to over 1.2 million (stroke). There was greater variation in dose between categories than within categories ( p < .001), and doses were significantly different between categories within anatomic areas. DRLs and median doses were assessed for all categories. DRLs were higher in the US for 9 of the 10 indications (except chronic sinusitis) than in Europe but with a significantly higher sample size in the US. Conclusions DRLs for CTDI vol and DLP for EUCLID clinical indications from diverse organizations were established and can contribute to dose optimization. These values were usually significantly higher in the US than in Europe. Key Points • Registry data were used to create benchmarks for 10 common indications for CT identified by the European Society of Radiology. • Observed US radiation doses were higher than European for 9 of 10 indications (except chronic sinusitis). • The presented diagnostic reference levels and median doses highlight potentially unnecessary variation in radiation dose.

Abstract:Radiation dose management of medical staff has become increasingly important. Particularly, based on the statement by the International Commission on Radiological Protection (ICRP) in 2011, a new lower equivalent dose limit for the eye lens is being established in each country. Although many reports have discussed the occupational radiation dose in interventional radiology (IR), few studies have examined the dose during computed tomography (CT) examinations. This study investigated the radiation dose exposure to medical staff present in the CT room during irradiation, with particular focus on the exposure to eye lens. The radiation dose exposure to those who assist patients during head, chest and upper abdomen CT examination was measured in a phantom study. The radiation dose exposure with scattered radiation was never negligible (i.e., high); Hp(3) was the highest in head CT examination, at 0.44 mSv per examination. Furthermore, the shielding effect of radiation protection glasses was large, and radiation protection glasses are useful tools for the medical staff who are involved in CT examinations. The justification and optimisation should be carefully considered in assistant actions.

Abstract:BACKGROUND: Our study aims to summarize the data of radiation doses collected from consecutive CT examinations by using the Radiometrics software and contributing to the establishment of the region's diagnostic reference levels (DRLs).METHODS: The radiation doses in 158,463 CT examinations performed on 106,275 adults between April 2017 and April 2019 were retrospectively analyzed. The median value and interquartile range (IQR) of volumetric CT dose index (CTDIvol), dose-length product (DLP), effective dose (ED), and size-specific dose estimate (SSDE) were calculated according to the scanning region.RESULTS: The median CTDIvol (mGy) for each scanning region was 42.3 (head), 6.2 (chest), and 9.0 (abdomen). The median DLPs (mGy.cm) for single-phase, multi-phase, and all examinations were as follows: 607, 794, and 641 for the head; 220, 393, and 237 for the chest; 298, 1,141, and 570 for the abdomen. The median EDs (mSv) for single-phase, multi-phase, and all examinations are as follows: 1.6, 2.6, and 1.8 for the head; 5.1, 8.1, and 5.3 for the chest; 5.8, 20.3, and 10.4 for the abdomen.CONCLUSIONS: Our study's results could provide a basis for the evaluation of CT scanning radiation dosage and supply evidence for the establishment of local DRLs in China's Sichuan Province.

Abstract:Abstract A manual radiation dose management system was developed to track the radiation dose and scan parameters of patients for brain computed tomography (CT). Radiation dose in volume computed tomography dose index (CTDIvol) and dose length product (DLP) were monitored to identify procedures that may require optimisation using notification values. The data were analysed and compared with national and international diagnostic reference levels (DRLs). A total of 596 brain CTs were monitored and grouped as <1: 36, 1–<5: 38, 5–<10: 25, 10–<15: 31 and adult: 466. The CTDIvol notification value identified the following number of examinations having high CTDIvol in <1 y: 1, 1–<5: 1, 5–<10: 0, 10–<15: 0 and adult (>15): 11. Furthermore, the DLP notification values identified the following examinations with high DLP in <1 y: 1, 1–<5:1, 5–<10:1, 10–<15: 1 and adults (>15): 18. The established local paediatric DLP DRLs were 2–3 times higher than the international paediatric DLP DRLs. This calls for a total protocol review and optimisation considering the local CT practices for paediatric imaging.

Abstract:Background: JCOG1015A1 is an ancillary research study to determine the organ-specific dose constraints in head and neck carcinoma treated with intensity-modulated radiation therapy (IMRT) using data from JCOG1015. Methods: Individual patient data and dose-volume histograms of organs at risk (OAR) were collected from 74 patients with nasopharyngeal carcinoma treated with IMRT who enrolled in JCOG1015. The incidence of late toxicities was evaluated using the cumulative incidence method or prevalence proportion. ROC analysis was used to estimate the optimal DVH cut-off value that predicted toxicities. Results: The 5-year cumulative incidences of Grade (G) 1 myelitis, ≥ G1 central nervous system (CNS) necrosis, G2 optic nerve disorder, ≥ G2 dysphagia, ≥ G2 laryngeal edema, ≥ G2 hearing impaired, ≥ G2 middle ear inflammation, and ≥ G1 hypothyroidism were 10%, 5%, 2%, 11%, 5%, 26%, 34%, and 34%, respectively. Significant associations between DVH parameters and incidences of toxicities were observed in the brainstem for myelitis (D1cc ≥ 55.8 Gy), in the brain for CNS necrosis (D1cc ≥ 72.1 Gy), in the eyeball for optic nerve disorder (Dmax ≥ 36.6 Gy), and in the ipsilateral inner ear for hearing impaired (Dmean ≥ 44 Gy). The optic nerve, pharyngeal constrictor muscle (PCM), and thyroid showed tendencies between DVH parameters and toxicity incidence. The prevalence proportion of G2 xerostomia at 2 years was 17 versus 6% (contralateral parotid gland Dmean ≥ 25.8 Gy vs less). Conclusions: The dose constraint criteria were appropriate for most OAR in this study, although more strict dose constraints might be necessary for the inner ear, PCM, and brainstem.
Patient-specific Organ and Effective Dose Estimates in Pediatric Oncology Computed Tomography.

Abstract:PurposeEstimate organ and effective doses from computed tomography scans of pediatric oncologic patients using patient-specific information.Materials and MethodsWith IRB approval patient-specific scan parameters and patient size obtained from DICOM images and vendor-provided dose monitoring application were obtained for a cross-sectional study of 1250 pediatric patients from 0 through 20 y-olds who underwent head, chest, abdomen-pelvis, or chest-abdomen-pelvis CT scans. Patients were categorized by age. Organ doses and effective doses were estimated using VirtualDose™ CT based on patient-specific information, tube current modulation (TCM), and age-specific realistic phantoms. CTDIvol, DLP, and dose results were compared with those reported in the literature.ResultsCTDIvol and DLP varied widely as patient size varied. The 75th percentiles of CTDIvol and DLP were no greater than in the literature with the exception of head scans of 16–20 y-olds and of abdomen-pelvis scans of larger patients. Eye lens dose from a head scan was up to 69 mGy. Mean organ doses agreed with other studies at maximal difference of 38% for chest and 41% for abdomen-pelvis scans. Mean effective dose was generally higher for older patients. The highest effective doses were estimated for the 16–20 y-olds as: head 3.3 mSv, chest 4.1 mSv, abdomen-pelvis 10.0 mSv, chest-abdomen-pelvis 14.0 mSv.ConclusionPatient-specific organ and effective doses have been estimated for pediatric oncologic patients from <1 through 20 y-olds. The effect of TCM was successfully accounted for in the estimates. Output parameters varied with patient size. CTDIvol and DLP results are useful for future protocol optimization.
An Image Quality–informed Framework for CT Characterization

Abstract:Background Lack of standardization in CT protocol choice contributes to radiation dose variation. Purpose To create a framework to assess radiation doses within broad CT categories defined according to body region and clinical imaging indication and to cluster indications according to the dose required for sufficient image quality. Materials and Methods This was a retrospective study using Digital Imaging and Communications in Medicine metadata. CT examinations in adults from January 1, 2016 to December 31, 2019 from the University of California San Francisco International CT Dose Registry were grouped into 19 categories according to body region and required radiation dose levels. Five body regions had a single dose range (ie, extremities, neck, thoracolumbar spine, combined chest and abdomen, and combined thoracolumbar spine). Five additional regions were subdivided according to dose. Head, chest, cardiac, and abdomen each had low, routine, and high dose categories; combined head and neck had routine and high dose categories. For each category, the median and 75th percentile (ie, diagnostic reference level [DRL]) were determined for dose-length product, and the variation in dose within categories versus across categories was calculated and compared using an analysis of variance. Relative median and DRL (95% CI) doses comparing high dose versus low dose categories were calculated. Results Among 4.5 million examinations, the median and DRL doses varied approximately 10 times between categories compared with between indications within categories. For head, chest, abdomen, and cardiac (3 266 546 examinations [72%]), the relative median doses were higher in examinations assigned to the high dose categories than in examinations assigned to the low dose categories, suggesting the assignment of indications to the broad categories is valid (head, 3.4-fold higher [95% CI: 3.4, 3.5]; chest, 9.6 [95% CI: 9.3, 10.0]; abdomen, 2.4 [95% CI: 2.4, 2.5]; and cardiac, 18.1 [95% CI: 17.7, 18.6]). Results were similar for DRL doses (all <i>P</i> < .001). Conclusion Broad categories based on image quality requirements are a suitable framework for simplifying radiation dose assessment, according to expected variation between and within categories. © RSNA, 2021 See also the editorial by Mahesh in this issue.

Abstract:In light of recent events involving dose to patients undergoing CT exams, many efforts to record patient dose have been initiated. Many national and international groups (FDA, NIBIB, ACR, IAEA, and NIH intramural programs) have called for recording “dose”, or required that CT equipment report dose. In California, a state law [SB1237] was recently passed that requires (as of July 1, 2012) the reporting of one of the following: “The computed tomography index volume (CTDIvol) and dose length product (DLP), as defined by the International Electrotechnical Commission (IEC) and recognized by the federal Food and Drug Administration (FDA); The dose unit as recommended by the American Association of Physicists in Medicine”. While these reporting/recording efforts are well‐meaning, the only dose metrics currently available for reporting are CTDIvol and DLP. While CTDIvol values are highly standardized and recognized around the world, they actually are measures of scanner output, and as such they do not reflect the actual patient dose or the irradiation burden of the exam . This misconception of CTDI is commonplace and has led to misinterpretation of the dosimetry values being reported at the scanner and recorded into patient records. The purpose of this symposium is to describe methods to more accurately estimate the radiation dose to various organs, and ultimately the radiation risk, to patients undergoing CT exams. The symposium covers methods aimed towards more patient‐specific estimates that take into account: (a) scanner related factors (CT manufacturer and model specifics), (b) exam related specifics (anatomic coverage, use of dose reduction methods such as tube current modulation) and (c) patient related factors such as age, gender, size, and shape. The learning objectives of this symposium include the following: 1. To understand the limitations of current metrics (e.g. CTDIvol) and methods in estimating patient dose 2. To understand the methods to more accurately estimate radiation dose that take into account scanner, exam and patient specific factors 3. To understand the role of Computational Phantoms and Monte Carlo simulation methods in developing patient‐specific organ dose estimates 4. To understand methods to estimate patient‐specific organ doses, effective doses, and radiation risk for comparison and optimization purposes
Establishing Local Diagnostic Reference Levels for Head Computed Tomography Examinations

Abstract:Background/Objectives: Head Computed Tomography (CT) is an essential diagnostic tool for identifying brain pathologies and visualizing blood vessels. However, CT exposes patients to ionizing radiation, making it necessary to establish local diagnostic reference levels (DRLs) to ensure patient safety. This study aimed to establish DRLs for head CT scans and assess the influence of patient characteristics on radiation dose. Methods: A retrospective analysis was conducted on 2043 non-contrast and 488 contrast-enhanced head CT scans performed between 1 July 2023 and 31 March 2024 using a SIEMENS SOMATOM Definition Edge machine. Computed Tomography Dose Index (CTDIvol) and Dose-Length Product (DLP) values were analyzed, with DRLs set at the 75th percentile. The influence of gender, height, and weight on radiation dose was also evaluated. Results: The DRL for both non-contrast and contrast-enhanced scans was 58.18 mGy for CTDIvol and 1018.11 mGy·cm for DLP per acquisition. Total DLP was 2046.09 mGy·cm for contrast-enhanced and 1027.99 mGy·cm for non-contrast scans. No significant correlation was found between patient characteristics and radiation dose, allowing for a uniform DRL to be established. Conclusions: Uniform DRLs were successfully established for head CT scans, ensuring safe radiation doses for both non-contrast and contrast-enhanced studies. The lack of correlation between patient-specific factors and dose supports the use of standardized DRLs, contributing to optimized radiation safety in head CT diagnostics.

Abstract:Purpose: MDCT and kilovoltage CBCT are increasingly used in IGRT. AAPM TG-75 has made a series of recommendations on imaging doses. Recent studies have focused on an inclusion of imaging doses using a radiation treatment planning (RTP) system. This paper describes the use of Monte Carlo methods to calculate imaging dose for a prostate IGRT treatment case. Methods: An IMRT treatment plan per RTOG 0126 for a prostate carcinoma was used involving 28 initial fractions and a boost in 16 fractions. Constraints to rectum bladder and femoral heads were enforced. A total of 40 imaging procedures were considered involving a MDCT or a KVCBCT scanner that is operated at 250 mAs. A GE LightSpeed 16-MDCT scanner and a Varian On-Board Imager (OBI) were modeled with parameters reported in the literature. Planning CT images were used to construct a patient phantom within the MCNPX simulation environment. OARs and background voxels were categorixed into six tissue types. Results: For a total of 40 scans rectum received 69.7 cGy and 68.2 cGy from MDCT and KVCBCT respectively. The bladder received slightly greater doses 73.3 cGy and 69.3 cGy while the femoral heads received much higher doses 161.9 cGy and 125.7 cGy respectively. To investigate the impact MDCT imaging doses are added to those from the original treatment plans and dose-volume histograms evaluated. Among notable findings: the rectum dose after adding the imaging doses may increase to or above the maximum acceptable level. Conclusion: Imaging doses can reach the level that may require the adjustment of original planning in order to still satisfy the constraints. RTP systems may not be suitable for low-energy X-rays especially for bones and lungs. The data are expected to be useful to the newly formed AAPM TG-180.
Age-based diagnostic reference levels and achievable doses for pediatric CT: A survey in Shanghai, China

Abstract:Computed tomography (CT) is extensively utilized in medical diagnostics due to its notable radiographic superiority. However, the cancer risk associated with CT examinations, particularly in children, is of significant concern. The assessment of cancer risk relies on the radiation dose to examinees. Diagnostic reference levels (DRLs) and achievable doses (ADs) were used to assess the level of radiation dose in CT examinations widely. Although the national DRLs of pediatric CT have been explored in China, few local DRLs at the city level have been assessed. To set up the local DRLs and ADs of pediatric CT, we investigated the radiation dose level for pediatric CT in Shanghai. In this survey, a total of 3 061 pediatric CT examinations underwent in Shanghai in 2022 were selected by stratified sampling, and the dose levels in terms of volume CT dose index (CTDIvol) and the dose-length product (DLP) were analyzed by 4 age groups. The DRLs and ADs were set at the 75th and 50th percentile of the distribution and compared with the previous studies at home and abroad. The survey results revealed that, for head scan,theDRLs of CTDIvol were from 25 to 46 mGy, and the levels of DLP were from 340 to 663 mGy·cm. For chest, the DRLs of CTDIvol were from 2.2 to 8.3 mGy, and the levels of DLP were from 42 to 223 mGy·cm. For abdomen, the DRLs of CTDIvol were from 6.3 to 16 mGy, and the levels of DLP were from 181 to 557 mGy·cm. The ADs were about 60 percent lower than their corresponding DRLs. The levels of radiation doses in children-based hospitals were higher than those in other medical institutions. (P<0.001).In conclusion, there was still potential for reducing radiation dose of pediatric CT, emphasizing the urgent need for optimizing pediatric CT dose in Shanghai.

Abstract:Interventional radiology is one of the most widely used treatment options for abdominal abnormality. Due to new dose limitations, radiation is a hazard that needs to be considered when choosing the international treatment technique. The aim of this study was to measure the effective/equivalent doses and to investigate the associated factors correlating with the doses during Transarterial Chemoembolization (TACE) and Percutaneous Transhepatic Biliary Drainage (PTBD) for radiologists, technologists and nurses involved with the treatment. This prospective study reports occupational dose per procedure of 57 cases (TACE = 42, PTBD = 15) in a Thailand hospital spanning one year. All staff included in this study wore 30 nanoDot Optically Stimulated Luminescence Dosimeter for measuring doses at 7 bodily locations. The average dose area product and cumulative doses was 151.3 Gy·cm 2 , 1,185.1 mGy during TACE and 21.3 Gy·cm 2 , 212.2 mGy during PTBD. The radiation exposure of the radiologists was significantly higher than technologists and nurses ( p -value < 0.05). The lead protection equipment was a dose-reducing factor which decreased the dose up to 93%. The present study also contributes to the establishment of an individual dose location report and identifies significant correlations between associated factors and dose values. The left hand (1.2 mSv) and the left eye (1.1 mSv) showed maximum doses during the PTBD procedure. The exceeded dose over the limitation was observed in the left eye. We concluded that the dose feedback was able to increase awareness of the occupation staff. So, dose monitoring, updating education and regular training could be applied simply and effectively to improve skills, while reducing the occupational dose.
