Abstract:Objective This study evaluated vocal fold leukoplakia using i-scan combined with laryngovideostroboscopy for risk assessment prediction. Methods A total of 141 patients with 218 lesions were enrolled in this study. Morphological characteristics of leukoplakia, assessment of the vascular pattern using i-scan, and vocal fold vibratory function were analyzed. Results The number of patients with no, mild, moderate, severe dysplasia, and invasive carcinoma were 68, 40, 17, 46 and 47, respectively. The sensitivity of morphological characteristic, vascular pattern, vibratory function and predictive model were 77.4%, 72%, 69.9%, and 82.8%, respectively. Receiver operating characteristic curve analysis of morphological characteristic, vascular pattern, vibratory function and predictive model were 0.771, 0.824, 0.769, and 0.923, respectively. The results of logistic regression analysis showed that rough morphological types, perpendicular vascular pattern, severe decrease and absence of mucosal waves increased the risk of malignancy (OR = 5.531, 4.973, and 16.992, respectively; P < 0.001). Conclusions I-scan combined with laryngovideostroboscopy can improve the differential diagnosis of low-risk and high-risk vocal fold leukoplakia.
A New Classification of Vocal Fold Leukoplakia by Morphological Appearance Guiding the Treatment

Abstract:OBJECTIVE:To provide a reference for classification and treatment of vocal cords leukoplakia.METHODS:640 cases of patients with vocal cords leukoplakia were divided into three groups based on the appearances. There were respectively 81 smooth flat lesions, 155 smooth hypertrophy lesions and 13 rough lesions were treated with conservative methods and 26, 153 and 212 cases were resected surgically for biopsy.RESULTS:A majority of smooth flat leukoplakia lesions were cured by non-operational methods and no atypical hyperplasia. Most of rough lesions were invalid and even progressed treated with conservative therapy and were severe dysplasia or canceration. Although 67.6% smooth hypertrophy lesions were cured or improved by conservative methods and 47% lesions had no or mild dysplasia, over 30% of lesions were still invalid or progressed and over 50% had moderate, severe dysplasia and canceration. By statistical analysis, the appearance of leukoplakia was concordant with pathological changes.CONCLUSIONS:It is suggested that smooth flat vocal cords leukoplakia could be treated with conservative methods and rough lesions should be resected operationally, while for smooth hypertrophy vocal cords leukoplakia could be first followed-up and then adopt measures based on the appearance changes.
Performance characterization of the Hyperion Imaging Spectrometer instrument

Abstract:This paper presents the techniques and results of Hyperion laboratory characterization. Hyperion is a hyperspectral imager scheduled to fly on the Earth-Orbiter 1 (EO-1) spacecraft for the New Millennium project. The other payloads on the spacecraft are ALI (Advanced Land Imager) and AC (atmospheric corrector). The payloads were integrated into the spacecraft at Goddard Space Flight Center (GSFC). An End-to-End imaging test was conducted at GSFC which demonstrated integrity of Hyperion performance after environmental tests. The performance characterization procedures described here include: crosstrack MTF, spectral and spatial co-alignment, spectral wavelength calibration, signal to noise, polarization, spectral response function and scene generation. The characterization was carried out with the TRW Imaging Spectrometer Characterization Facility which is based on a 250 watt QTH lamp, a monochromator, a collimator and a fine pointing mirror. A selection of narrow slits and a knife edge are illuminated at the exit slit of the monochromator for sub-pixel performance characterization parameters such as MTF. Special attention is devoted to the spectral calibration technique using rare earth doped Spectralon panels. This was the technique used at the End-to-End test to verify spectral performance of Hyperion after GSFC environmental tests. It is a particular useful technique when the optical test setup does not allow for the use of a monochromator.
Association Between the Morphological Classification and Prognosis of Conservative Treatment for Vocal Fold Leukoplakia

Abstract:Objective:To explore the association between the morphological classification and prognosis of conservative treatment for vocal fold leukoplakia. Method:Patients with different types of vocal fold leukoplakia received conservative treatment first. Those who had failed response to conservative treatment or recurred received carbon dioxide laser resection. Then the efficacy and recurrence rate were compared among the three types of vocal fold leukoplakia. Result:Two hundred and thirty-six patients consisted of 78 flat and smooth cases, 146 bulge and smooth cases, and 12 rough cases. The complete response rate of conservative treatment for the three types was 80.8%, 65.1%, and 0%, respectively (P<0.01). Out of 158 patients who had complete response to conservative treatment, 10.1% cases recurred in 1 year (flat and smooth vs. bulge and smooth, 6.3% vs. 12.6%,P=0.283). Totally, 78 patients had no complete response to conservative treatment and 16 patients experienced recurrence, whose complete response rate of carbon dioxide resection was 89.5%, 85.7%, and 75.0%, respectively (P=0.53). The pathological results were significantly different among three types. The complete response rate of conservative treatment combined with surgery for three types was 97.4%, 93.8%, and 75.0%, respectively, and the complete response rate of flat and smooth type was significantly higher than that of rough type (P=0.002). Conclusion:The Conservative treatment could be based on the morphological classification of vocal fold leukoplakia. The treatment modality that conservative treatment combined with surgery had no impact on the prognosis of vocal fold leukoplakia.

Abstract:OBJECTIVE:We assessed the expression of glucose transporter (GLUT)-1 and hexokinase (HK)-Ⅱ in vocal cord leukoplakia (VCL) and investigated its clinical significance. METHODS:We collected 127 archival pathological specimens from patients with VCL, 29 from vocal cord polyps, and 31 with laryngeal carcinoma at our hospital between December 2018 and December 2022. GLUT-1 and HK-II expression was detected using immunohistochemistry. RESULTS:Significant differences were observed between the VCL and vocal cord polyp groups according to sex (P < 0.0001), smoking status (P = 0.001), and alcohol consumption (P = 0.007). GLUT-1 expression was observed in 36.2%, 20.7%, and 90.3% of patients with VCL, vocal cord polyps, and laryngeal carcinoma, respectively. GLUT-1 was highly expressed in severe dysplasia or carcinoma in situ (CIS) compared with vocal cord polyps (P < 0.001). GLUT-1 expression was lower in the VCL group than in the laryngeal carcinoma group (P < 0.001). HK-II expression was observed in 42.5%, 34.5%, and 80.6% of patients with VCL, vocal cord polyps, and laryngeal carcinoma, respectively. HK-II expression was significantly higher in severe dysplasia or CIS compared with vocal cord polyps (P = 0.004). HK-II expression was lower in the VCL group than in the laryngeal carcinoma group (P < 0.001). Both GLUT-1 expression (P < 0.001) and HK-II expression (P = 0.03) increased significantly with the degree of dysplasia. A significant correlation was observed between GLUT-1 and HK-II expression (r = 0.313, P < 0.001). CONCLUSION:GLUT-1 and HK-II are significantly overexpressed in VCL patients. In VCL patients, GLUT-1 and HK-II expression levels increase proportionally with the degree of dysplasia.
Nonsurgical Treatment for Vocal Fold Leukoplakia: an Analysis of 178 Cases

Abstract:Objective To assess the effectiveness and identify vocal fold leukoplakia types appropriate for nonsurgical treatment. Methods The vocal fold leukoplakia in 178 patients was divided by gross appearance into three subtypes: flat and smooth, elevated and smooth, and rough. All patients received nonsurgical treatment including smoking and drinking cessation, voice rest, omeprazole, and Chinese medication therapy. The clinical response of three subtypes was assessed after a 6-month follow-up. Results Vocal fold leukoplakia subtypes included flat and smooth (n = 66; 37.1%), elevated and smooth (n = 103; 57.9%), and rough (n = 9; 5.0%). The rate of complete response was 80.3%, 66.0%, and 0.0% for the 3 lesion types, respectively (rough versus flat and smooth, P < 0.001; rough versus elevated and smooth, P < 0.001, Fisher's exact test). The incidence of carcinoma in rough leukoplakia was significantly higher than that in smooth leukoplakia (44.4% versus 2.4%, P = 0.002, Fisher's exact test). Clinical type was the only significant factor for clinical response of nonsurgical treatment (P = 0.005, ordinal logistic regression). Conclusions The effectiveness of nonsurgical treatment for smooth vocal fold leukoplakia is better in comparison to rough vocal fold leukoplakia. Smooth leukoplakia could be managed with nonsurgical treatment; more aggressive treatments should be considered for rough leukoplakia.

Abstract:BACKGROUND:Leukokeratosis of the vocal cords is a clinical descriptive diagnosis, which includes a group of squamous intraepithelial lesions of the vocal cord mucosa. We investigated the clinical classification and treatment efficacy of leukokeratosis of the vocal cords.METHODS:We conducted a retrospective analysis of the medical history, laryngoscopic examinations, morphological features under a surgical microscope, and pathology results of 360 cases of leukokeratosis of the vocal cords to examine correlations among treatment modalities, therapeutic effects, and clinical features.RESULTS:All cases were divided into four types based on symptoms, examination results, and treatment efficacies as follows: 21 patients had type I inflammatory leukoplakia and their vocal cord morphology and voice quality recovered after conservative therapies; 76 patients had type II frictional polyps and received CO2 laser submucosal cordectomy; 68 patients had type III sulcus vocalis and received mucosal slicing with dredging; and 195 cases had type IV simple leukokeratosis and received partial subligamental cordectomy with CO2 lasers or transmuscular cordectomy. Our treatment achieved a surgical cure rate of 90.9% (308/339), with a recurrence rate of 9.1% (31/339) and malignant transformation rate of 6.5% (22/339). All cancerous transformations occurred in type IV patients.CONCLUSION:Choosing conservative or CO2 laser surgery based on the morphological characteristics of squamous epithelial lesions of keratinized vocal cord mucosa can maximally protect voice quality, reduce complications, and improve the cure rate.
Analysis of prognostic factors for vocal fold leukoplakia based on 344 cases at a two-year follow up

Abstract:Objective: To evaluate risk factors for poor prognosis in vocal fold leukoplakia. Methods: Clinical data were collected for 344 patients with vocal fold leukoplakia who received surgical treatment in our otolaryngology department from October 2010 to June 2019. Univariate and multivariate logistic regression analyses of the relevant factors were conducted. Results: Among the 344 patients, 98 exhibited recurrence and 30 underwent a malignant change. Multivariate logistic regression analysis showed that size of the lesion (p = 0.03, odds ratio = 2.14), form of the lesion under white light (p < 0.001), surgical method (p < 0.001, odds ratio = 0.28) and pathological type (p < 0.001) were independent factors that affected the recurrence of vocal fold leukoplakia. In both univariate and multivariate analyses, the sole independent risk factor for malignant transformation of vocal fold leukoplakia was pathological type (p < 0.001). Conclusion: The outlook for vocal fold leukoplakia depends on several clinical factors, especially pathological type. The more severe the pathological type, the more likely it is to recur or become cancerous.

Abstract:Transdisciplinary scientific collaborations (TDSCs) have the potential to strengthen substance use and misuse research and prevention. Despite its growing prominence as a mode for scientific research, research on TDSC remains in a nascent form and its value to the field of substance use and misuse merits further exploration. The overarching purpose of this article is to examine the potential contributions of transdisciplinary science to research and prevention using conceptualizations, methods, and evidence from a case study of two university-based research centers. The article provides (a) a discussion of the societal context and historical developments that have prompted increasing interest in TDSC; (b) a definition and conceptualization of TDSC; (c) a methodological approach for studying TDSC; (d) initial findings from the case study that reflect instances of transdisciplinary intellectual integration and it examines implications of these methods and findings for future research and policy development relevant to substance use and misuse.
[Clinical analyses of 263 patients with laryngeal leukoplakia]

Abstract:Objective: To investigate the etiology, clinical and pathological characteristics of laryngeal leukoplakia and the predictive risk factors of recurrence and malignant transformation. Methods: Clinical data of 263 patients with laryngeal leukoplakia between January 2000 and December 2015 were analyzed retrospectively. Results: The pathological diagnoses included squamous epithelial hyperplasia (54.4%), mild dysplasia (17.9%), moderate dysplasia (12.2%), severe dysplasia and carcinoma in situ (12.5%), and invasive carcinoma (3.0%). Age and the extent of lesion were statistically different among different pathological groups (P<0.05). Gender, smoking and alcohol consumption did not show statistical differences among different pathological groups (P>0.05). Follow-up of 215 patients, excluding 6 cases of invasive carcinoma. The recurrence rate was 20.6%(43/209), and the malignant transformation rate was 5.3%(11/209). Multivariate analysis showed that pathological classification of moderate to severe dysplasia was the independent risk factor for recurrence and malignant transformation of laryngeal leukoplakia (P<0.05). In patients with severe dysplasia and carcinoma in situ, the recurrence proportion of conservative treatment, vocal cords (partial) resection and radiotherapy were 8/10, 0/10 and 2/11 respectively. Conclusions: Laryngeal leukoplakia occurs frequently in elderly men with long-term smoking history. Pathological diagnoses are different. The grade of dysplasia is the predictive risk factor for the recurrence and malignant transformation of laryngeal leukoplakia. More aggressive treatment and closer follow-up should be warranted for patients with moderate dysplasia, severe dysplasia and carcinoma in situ.
[Clinical diagnosis and treatment of 157 cases of vocal leukoplakia]

Abstract:Objective:To study the clinical characteristics and common etiology of vocal cord leukoplakia, and explore the treatment principle. Method:One hundred and fifty-seven patients with vocal cord leukoplakia were recruited in this study. They were assessed by routine laryngoscope, narrow band imaging （NBI）, stroboscope, reflux finding score （RFs） and reflux symptom index （RSI）, and given conservative treatment （smoking cessation, alcohol, acid suppression, sound cessation, etc.） and/or surgical treatment. Result:Among 157 patients with leukoplakia of vocal cord, 109 （69.4%） had basically improved or cured after conservative treatment. Forty-eight cases underwent operation. The postoperative pathological diagnosis was mild dysplasia in 2 cases （1.3%）, moderate dysplasia in 15 cases （9.6%）, severe dysplasia or carcinoma in situ in 19 cases （12.1%）, and invasive carcinoma in 12 cases （7.6%）. Conclusion:NBI and stroboscopic laryngoscopy showed that most of the leukoplakia of vocal cord was non-malignant in nature, which was not in accordance with the indication of operation. Conservative treatment is effective. Biopsy or operation was indicated in only a few patients. Moreover, most of the patients are accompanied by laryngopharyngeal reflux, and the treatment of acid suppression is effective.
Clinical and Pathological Analysis for 124 Cases of Vocal Cord Leukoplakia

Abstract:Objective To investigate the clinical and pathological features and related factors of vocal cord leukoplakis.Methods One hundred and twenty-four cases of vocal cord leukoplakia from January 2006 to December 2008 were analyzed.All the patients had undergone the surgery of vocal cord leukoplakia resection in retaining laryngoscope under general anesthesia with the specimens pathological examination.Results There were 120 males and 4 females in this study,the age ranged from 29 to 84 years,with the mean age of 49.65±10.13 years old.The causes of 89(71.77%) cases were long-term smoking,while the cause of 67(54.03%) cases were drinking.60.48% of the lesions were located in the forepart to middle part of the bilateral vocal cord,while 23.4% of the lesions were unilateral.The results of pathological examination showed chronic mucosal inflammation in 10 cases(8.06%),squamous epithelial hyperplasia in 88 cases(70.96%),mild dysplasia in 6 cases(4.84%),moderate dysplasia in 5 cases(4.03%),severe dysplasis in 5 cases(4.03%),carcinoma in situ in 6 cases(4.84%),and invasive carcinoma in 4 cases(3.23%).Conclusion Vocal cord leukoplakia is a predominantly male disease.Long-term smoking and drinking is one of common causes.Therefore,the final diagnosis of vocal cord leukoplakia relies on pathology.It is necessary to follow up closely for the dysplasia cases.
The Variation of Peripheral Inflammatory Markers in Vocal Leukoplakia before and after Recurrence and Canceration

Abstract:Background . This retrospective study aims at comparing the variation of peripheral inflammatory markers in recurrent and cancerous vocal fold leukoplakia (VFL) and at exploring the potential connection with pathological outcomes. Methods . The patients undergoing carbon dioxide laser surgery with postoperative pathological diagnosis of recurrent vocal fold leukoplakia in the last 5 years were included. The clinical data were collected, and neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and monocytes-to-lymphocyte ratio (MLR) before and after recurrence and canceration were calculated. Related comparison with two-grade pathological classification was made to evaluate their potential connection with postsurgical histopathology and clinical events. Results . The data of 193 patients were engaged into research, as 111 in the recurrence group (Group A) and 82 in canceration group (Group B). The NLR, PLR, and MLR were significantly increased in canceration event compared to the first ( P = 0.009 , 0.004 , 0.007 , respectively) and penultimate ( P = 0.013 , 0.041 , 0.006 , respectively) time when the previous pathologies were leukoplakia. When redividing the Group A according to the two-grade pathological classification, the high-risk groups showed statistically higher NLR and PLR values than low-risk groups in the subgroups with grade changing ( P = 0.016 , 0.005 , 0.007 , 0.005 , respectively) and subgroups without grade changing ( P = 0.020 , 0.027 , 0.030 , 0.029 , respectively). Conclusions . NLR, PLR, and MLR are reliable biomarkers in the circulation system which show significantly interrelation with the pathological progression of vocal fold leukoplakia. Presurgical evaluation of NLR, PLR, and MLR may have potential values to indicate the following treatment in clinical practice.

Abstract:OBJECTIVE:To study the clinic and pathological features of leukoplakia of the larynx, and reduce canceration rate.METHOD:Seventy-four postoperative patients of leukoplakia of the larynx were followed up for four years by telephone or clinic service.RESULT:None of eighteen cases of pure leukoplakia was recurrence. Fifty-six cases were leukoplakia with epithelial dysplasia histologically. Seven out of fifty-six postoperative cases occurred hoarseness; three out of fifty-six postoperative cases recurred, and accepted twice or more operations successively; three out of fifty-six postoperative cases occurred canceration.CONCLUSION:Operation is the main means for the treatment of leukoplakia of the larynx, and the effect is comparatively well. The patients with histological epithelial dysplasia should be pay great attention because of their higher canceration rate, and enlarged operation ranges appropriately with the patient consent could effectually prevent recurrence or canceration during the early lesions. Close observation is important for these patients.
Comparison Between Early Glottic Carcinoma and Epithelial Dysplastic Lesions of the Vocal Fold Via Voice Analysis

Abstract:Objective: Vocal fold leukoplakia is clinically defined by the presence of white mucosal lesions. Benign and malignant lesions of vocal fold leukoplakia can be distinguished clinically based on pathological biopsy. This study compared the acoustic and aerodynamic parameters of vocal cord carcinoma and dysplasia (mild to severe). Materials and methods: From February 2014 to December 2018, 1,925 voice evaluation assessments were collected before laryngeal microsurgery (LMS). Of 147 patients clinically diagnosed with vocal cord leukoplakia before LMS, 112 male patients were selected for examination. The pathologic findings after LMS were divided into the carcinoma group (56 patients) and dysplasia group (56 patients). Only patients with carcinoma in situ and early glottis cancer were included in the carcinoma group. Analysis of covariance was used to calibrate the age between the two groups. Results: There was no difference in smoking duration between the two groups. F0 (P < 0.00), jitter (P < 0.00), and mean pitch (P = 0.010) were significantly higher, while the mean sound pressure level parameter (P = 0.024) was significantly lower, in the carcinoma group than in the noncarcinoma group. Conclusions: In patients with early glottis cancer, differences in voice analysis parameters may be used to differentiate between early glottic carcinoma and noncarcinoma.
Optimal Pathological Prediction of Vocal Fold Leukoplakia Based on IPCLs and Morphology.

Abstract:OBJECTIVES:To establish a combined classification based on intrapapillary capillary loops (IPCLs) and morphological characteristics to improve the accuracy of pathological prediction of vocal fold leukoplakia (VFL). MATERIAL AND METHODS:A prospective research was conducted of VFL patients diagnosed by IPCLs and morphology. The VFL cases were classified as Type I-III based on IPCLs and morphological characteristics. Type I referred to VFL with dendritic vessels but not IPCLs. Type II defined VFL without any IPCLs or vessels and classified by morphology into two subtypes as non-rough Type IIa and rough Type IIb. Type III referred to VFL with IPCLs and classified into two subtypes as Type IIIa with small IPCLs and Type IIIb with large IPCLs or vascular distortion in or around lesions. Predicting pathology accuracy was analyzed. RESULTS:182 eligible patients were recruited. The prediction accuracy rates of VFL pathology were 81.5% according to the 2019 Ni classification. The combined classification includes 4 cases of Type I, 28 Type IIa, 35 Type IIb, 56 Type IIIa, and 59 Type IIIb VFLs. The prediction accuracy rate of combined classification ranged from 95.1% to 97.3% in three observers. The average sensitivity, specificity, positive predictive value, negative predictive value was 97.8%, 86.2%, 97.4%, 88.2%, respectively. The inter-observer agreement varied from 84.1% to 94.0%, and mean area under curve of receiver-operating curve analysis was 0.954. CONCLUSIONS AND SIGNIFICANCE:The new combined classification based on IPCLs and morphological characteristics could predict pathology of VFL accurately. LEVEL OF EVIDENCE:4 Laryngoscope, 2024.
[Risk prediction and malignant transformation in vocal cord leukoplakia based on electronic laryngoscope]

Abstract:Objective:To investigate the predictive risk factors of vocal fold leukoplakia under electronic laryngoscope and influential factors of malignancy. Methods:A total of 163 cases with vocal cord leukoplakia were analyzed retrospectively. The relationship between the clinical features, including site, size, whether involving the anterior commissure, morphology and pathological grade were analyzed. Then the associations with malignant transformation in vocal cord leukoplakia were evaluated by using multiple Logistic regression analysis. Results:There were one side lesions in 96 cases（58.9%）, and bilateral vocal cord lesions in 67 cases（41.1%）. The vocal cord leukoplakia with extension less than half of the vocal cord was observed in 135 cases（82.8%） and beyond half of the vocal cord was seen in 28 cases（17.1%）. Lesions involving the anterior commissure were 42 cases（25.8%）, without involving the anterior commissure were 121 cases（74.2%）. According to morphological features, 82 patients（50.3%） had a superficial type, followed by exophytic type（n=65, 39.9%）, and ulcerative type（n=16, 9.8%）. The pathological type included squamous epithelial simple hyperplasia in 65 cases（39.9%）, mild hyperplasia in 44 cases（27.0%）, moderate hyperplasia in 31 cases（19.0%）, severe hyperplasia and carcinoma in situ in 23 cases（14.1%）. 140 cases（85.9%） were in low risk group, and 23 cases（14.1%） were in high risk group. The site of the vocal fold leukoplakia was not associated with the pathological grade（P>0.05）, whether the vocal fold leukoplakia range exceeds 1/2 of the total length of the vocal fold, and whether involving the anterior commissure were associated with the pathological grade（both P<0.05）. There were statistically significant differences between the pathological grade and the morphological type（P<0.01）. Ten cases developed larynx cancer, the ratio of malignant transformation was 6.1%. The multiple Logistic regression showed that whether involving the anterior commissure and histopathological classification were the risk factors of malignant transformation in vocal cord eukoplakia. Conclusion:The pathological results of vocal cord leukoplakia can be predicted by laryngoscopy features. Involving of the anterior commissure and histopathological classification are the risk factors of malignant transformation in vocal cord leukoplakia.

Abstract:Abstract Objective To investigate the sensitivity of laryngeal findings in predicting high‐grade dysplasia/carcinoma in situ (CIS) and squamous cell carcinoma (SCC) in patients with vocal fold leukoplakia. Methods A retrospective review of the medical records and video recordings of the laryngeal examination of patients with vocal fold leukoplakia who underwent un‐sedated office‐based laryngeal biopsy in a tertiary referral center between January 2022 and August 2023 was conducted. Laryngeal findings included the size, surface, projection, and edges of the lesion. Vocal fold leukoplakia was classified according to the WHO as benign, low‐grade dysplasia, high‐grade dysplasia/CIS, and squamous cell carcinoma. Results Seventy patients with 100 vocal fold leukoplakia were included. Size was found to have the highest sensitivity with an AUC of 0.730 (95% CI [0.618–0.842], p = 0.002) followed by surface and projection with AUCs of 0.672 (95% CI [0.548–0.795], p = 0.019) and 0.675 (95% CI [0.546–0.804], p = 0.017), respectively. Furthermore, the odds of diagnosing high‐risk lesions (high‐grade dysplasia/CIS and SCC) were the greatest when the lesion was large and rough (OR = 10.28; 95% CI [3.08–34.36]). Conclusion The morphological features of vocal fold leukoplakia may assist the physician in predicting the risk of malignancy. Large and rough lesions were more likely to harbor high‐grade dysplasia/CIS and SCC compared to small and smooth lesions.

NBI Classification Optimization with Morphological Characteristics in Vocal Fold Leukoplakia

Abstract:Objectives: To investigate the pathological contribution of vocal fold leukoplakia (VFL) of type II in narrow-band imaging (NBI) classification and morphological characteristics to improve pathological prediction. Material and Methods: The 59 VFL patients with type II in 2019 Ni classification in NBI were included. The pathological reports were collected and divided following 2005 WHO Blue Book. Low-risk VFL contained non-, mild, moderate dysplasia, high-risk VFL included severe dysplasia. The morphological classification and laryngoscopic scoring system were employed to evaluate leukoplakia for pathological prediction. Results: The pathologies contained 1 case of leukoplakia with non-dysplasia, 12 of mild dysplasia, 15 of moderate dysplasia, 8 of severe dysplasia, and 23 of carcinoma. The 30 smooth VFL contained 1 non-dysplasia, 12 mild dysplasia, 14 moderate dysplasia, 2 severe dysplasia, and 1 carcinoma. The 29 rough cases included 1 moderate dysplasia, 6 severe dysplasia, and 22 carcinomas. Laryngoscopic scoring system revealed irregular texture, large size, and thick lesion as factors in relationship with high-risk leukoplakia in univariate ( P = .002, <.001, <.001) and multivariate ( P = .025, .002, .016) analysis, irregular texture was the most accurate predictor of high-risk VFL pathology. Conclusions and Significance: The pathologies of VFL with type II in NBI classification were hard to be predicted. Morphological irregular/rough texture contributed to predict high-risk pathology in leukoplakia.
Laryngeal Leukoplakia: State of the Art Review

Abstract:Objective: This state-of-the-art article reviews the epidemiology, diagnosis, and management of vocal fold leukoplakia, with focus on recent advances. It focuses on the clinical challenges that otolaryngologists face balancing both oncological efficacy and functional outcomes in leukoplakia and presents the current philosophies and techniques to consider when managing such patients. Data sources: PubMed/MEDLINE. Review methods: We conducted a detailed review of publications related to vocal cord and laryngeal leukoplakia, dysplasia, hyperkeratosis, leukoplakia endoscopy, and leukoplakia management focusing specifically on oncologic outcomes, voice preservation, current and emerging diagnosis, and management techniques. Conclusions: There has been a paradigm shift away from performing "vocal cord stripping" procedures that can cause irreversible hoarseness toward voice preservation surgery while achieving comparable oncologic control. Surgical technical and instrumental developments have been designed to maximally treat superficial disease while preserving underling vibratory mucosa. Recent improvements in histopathological grading systems and advances in biomarker classification may allow for improved oncologic risk stratification. Furthermore, improvements in endoscopic imaging capabilities and contact endoscopy are currently being studied for their potential diagnostic significance. Implications for practice: To optimally manage vocal fold leukoplakia, the otolaryngologist should become familiar with the oncologic implications of the disease and the importance of obtaining pathologic diagnosis to rule out malignancy. In addition, the surgeon should maintain surgical techniques and knowledge of available instruments and lasers that can assist in surgical management while prioritizing the preservation of vibratory tissue and voice quality. Finally, the surgeon and the patient should understand the clinical importance of routine endoscopic surveillance.
