Abstract:Objective: To systematically review the literature regarding the role of the physical environment in preventing or mitigating aggressive behavior toward healthcare professionals in acute care, outpatient, and psychiatric/behavioral health facilities. Background: Globally, the incidence of violence against healthcare professionals is alarming. Poor environmental design has been identified as a risk factor of violence toward employees. The design of the physical setting in which healthcare is provided may moderate the incidence and severity of violence against healthcare workers. Methods: We conducted electronic database searches of PubMed and CINAHL through November 2018. Result: Findings were organized according to four categories identified in the literature regarding crime prevention through environmental design (CPTED) including natural surveillance, access control, territoriality, and other CPTED elements. Fifteen studies (published between 1991 and 2017) met the inclusion criteria. Of the 15 studies, 4 incorporated environmental interventions. In the 11 remaining studies, physical environment attributes (i.e., layout, location, ambient conditions, equipment) were among the factors affecting violent incidents and staff security. Most study settings were hospital-based (11, with 10 of those specifically focused on emergency departments), followed by behavioral health facilities (4 studies). Design-focused recommendations, such as providing a second door in a triage room and a sub-waiting area inside the treatment zone, were summarized according to CPTED categories. Conclusion: This review suggests that the physical environment in healthcare facilities may affect the incidence of violence by patients or visitors against staff. Further research is needed to identify environmental design strategies that may protect the safety of healthcare professionals.
Threat awareness and counter-terrorism preparedness of Dutch hospitals: A cross-sectional survey

Abstract:Background Hospitals are vulnerable targets for terrorism and extreme violence due to their easy accessibility and the high density of patients, staff and visitors. Nonetheless, little is known about the hospitals' awareness of these risks, and to which extent these facilities protect themselves from violence. Methods This was a cross-sectional survey study among emergency managers of acute care hospitals in the Netherlands. The questionnaire included 42 items across six themes: demographic (hospital) data; general and emergency department (ED) access control; ED security and preparedness; online security and offline transparency; violence, terrorism and warfare; and impact of the COVID-19 pandemic. Results The questionnaire was completed on behalf of 33 out of 71 hospital organizations (46%), representing 38 out of 82 EDs (46%). Hospitals had broadly different policies with regards to patient and visitor registration, the presence of security guards, and vehicle access for the parking lot and ambulance bay. A terrorism paragraph was included in 34% of hospital disaster plans. Eighteen percent of hospitals had previous experience with extreme violence and 55% of hospitals had sustained (attempted) cyberattacks. On a 10-point Likert scale, the likelihood of a cyberattack scored higher (7.3) than a physical terrorist attack (3.6). A significant proportion of emergency managers experienced an increased risk of violence since the COVID-19 pandemic. Conclusion Practice variation with regards to counter-terrorism defence measures in Dutch hospitals is high. The preparedness of hospitals for terrorist attacks or extreme violence could be improved and may benefit from uniform, evidence based hospital security policies.

An innovative approach to training hospital-based clinicians for bioterrorist attacks

Abstract:The recent attacks of September 11, 2001, and the subsequent dissemination event of anthrax in the United States demonstrated the necessity for hospitals to initiate bioterrorism education for clinicians. Events such as the release of sarin gas into the Tokyo subway by the Aum Shinrikyo cult provided some insight into how quickly emergency medical personnel may be overwhelmed by causalities of unconventional weapons. Educational interventions to prepare hospital-based practitioners for such disasters must fit among the demands of patient care, administrative duties, and continuing education within specialties. In addition, the priority placed on the topic, confusion about reputable resources to consult, and concerns of funding for preparedness training mandate the need for an authoritative, comprehensive, and easily accessible approach. A pilot project supported in part by the Agency for Healthcare Research and Quality was developed to facilitate streamlining of preparedness efforts through the implementation of interactive screen savers as an alternative to traditional educational modalities. This report presents the successful application of this model, which was quantified with pretests and posttests given to users of the system.
Terrorist attacks against healthcare facilities involving hostages

Abstract:Introduction: The incidence of terrorist attacks against healthcare facilities has been increasing over recent years. In addition to direct attacks on physical structures, many attacks have involved taking hostages. Hospital and healthcare facilities remain historically underprepared for terrorist attacks, representing vulnerable locations. Yet, studies examining the frequency and reach of hostage-taking incidents within healthcare facilities are limited. Methods: A search of the Global Terrorism Database was performed. A total of 191,465 terrorist attacks were identified. The database search was narrowed down to healthcare-related terrorist attacks (2,322) and then manually analyzed to only include those incidents which involved hospitals and hostage-taking (64). Results: Sixty-four attacks against hospitals involving hostage-taking were identified. A total of 91 victims were injured in these attacks, and 47 were killed. The attacks affected a total of 23 countries worldwide, conducted largely by unidentified terrorist organizations, with approximately half involving firearms. Discussion: This study shows that terrorist attacks against healthcare facilities that involve -hostage-taking have increased in frequency over the past 10 years and have global reach. Systems may still be underprepared for this potentially increasing phenomenon and require preparedness plans with education and simulated practice in place. Healthcare facilities should consider mitigation strategies such as preparedness drills and additional education.

Abstract:Terrorist attacks pose a serious threat to the EU, particularly in public transport (PT) hubs such as railway stations due to their level of accessibility and where the individual passenger security screening methods used in airports are impractical. Consequently, methods of increasing station resilience against terrorist attacks in Europe have been explored as part of the recent SecureStation project. To support this work, PT passengers from across Europe were surveyed to gauge their opinion on current counter-terrorism security measures in stations and the acceptability and perceived effectiveness of potential security enhancements. The results, from a total sample of 489 respondents, provide indications of passenger priorities, their opinions of current station security, the acceptability of different security measures and the degree to which they increase feelings of safety. The results, together with summaries of similar studies, give an overview of passenger acceptance of various counter-terrorism security measures and point to areas for further research.

Abstract:Abstract Purpose The threat of national and international terrorism remains high. Preparation is the key requirement for the resilience of hospitals and out-of-hospital rescue forces. The scientific evidence for defining medical and tactical strategies often feeds on the analysis of real incidents and the lessons learned derived from them. This systematic review of the literature aims to identify and systematically report lessons learned from terrorist attacks since 2001. Methods PubMed was used as a database using predefined search strategies and eligibility criteria. All countries that are part of the Organization for Economic Cooperation and Development (OECD) were included. The time frame was set between 2001 and 2018. Results Finally 68 articles were included in the review. From these, 616 lessons learned were extracted and summarized into 15 categories. The data shows that despite the difference in attacks, countries, and casualties involved, many of the lessons learned are similar. We also found that the pattern of lessons learned is repeated continuously over the time period studied. Conclusions The lessons from terrorist attacks since 2001 follow a certain pattern and remained constant over time. Therefore, it seems to be more accurate to talk about lessons identified rather than lessons learned. To save as many victims as possible, protect rescue forces from harm, and to prepare hospitals at the best possible level it is important to implement the lessons identified in training and preparation.

Abstract:Abstract Technical and organizational steps are necessary to mitigate cyber threats and reduce risks. Human behavior is the last line of defense for many hospitals and is considered as equally important as technical security. Medical staff must be properly trained to perform such procedures. This paper presents the first qualitative, interdisciplinary research on how members of an intermediate care unit react to a cyberattack against their patient monitoring equipment. We conducted a simulation in a hospital training environment with 20 intensive care nurses. By the end of the experiment, 12 of the 20 participants realized the monitors’ incorrect behavior. We present a qualitative behavior analysis of high performing participants (HPP) and low performing participants (LPP). The HPP showed fewer signs of stress, were easier on their colleagues, and used analog systems more often than the LPP. With 40% of our participants not recognizing the attack, we see room for improvements through the use of proper tools and provision of adequate training to prepare staff for potential attacks in the future.

Abstract:Abstract Active shooter incidents represent an increasing threat to American society, especially in commercial and educational buildings. In recent years, a wide variety of security countermeasures have been recommended by public and governmental agencies. Many of these countermeasures are aimed to increase building security, yet their impact on human behavior when an active shooter incident occurs remains underexplored. To fill this research gap, we conducted virtual experiments to evaluate the impact of countermeasures on human behavior during active shooter incidents. A total of 162 office workers and middle/high school teachers were recruited to respond to an active shooter incident in virtual office and school buildings with or without the implementation of multiple countermeasures. The experiment results showed countermeasures significantly influenced participants’ response time and decisions (e.g., run, hide, fight). Participants’ responses and perceptions of the active shooter incident were also contingent on their daily roles, as well as building and social contexts. Teachers had more concerns for occupants’ safety than office workers. Moreover, teachers had more positive perceptions of occupants in the school, whereas office workers had more positive perceptions of occupants in the office.

In order to protect the personal safety of medical staff and prevent the occurrence of medical violence, an indoor wireless location and real-time tracking system based on Wi-Fi was developed for hospital violence. Through the active monitoring and early intervention of hospital violence, we can prevent the occurrence of violence or reduce the damage.

Abstract:A cDNA probe was prepared to investigate the regulation of proenkephalin biosynthesis in the rat. This was necessary because human and bovine proenkephalin cDNA were not sensitive enough for the accurate detection of preproenkephalin mRNA in tissues that contain low copy numbers of this message, such as the adrenal gland. The rat probe was prepared in the following manner. Preproenkephalin mRNA was enriched by sucrose gradient centrifugation of poly(A)-containing mRNA from rat brain and was used as a template for double-stranded cDNA synthesis. The resulting cDNA was inserted into the plasmid pBR322, and recombinant plasmids were used to transform Escherichia coli RR1 cells. A synthetic oligodeoxyribonucleotide (30 bases long) with a sequence that had previously been shown to be identical in bovine and human preproenkephalin cDNA was prepared to screen the clone bank. The plasmid with the longest cDNA insert (about 1200 bases) from the positive clones was isolated, and the sequence of the entire protein coding region was determined. Like the bovine and human gene products, rat preproenkephalin contains four [Met]enkephalin sequences and one copy each of [Leu]enkephalin, [Met]enkephalin-Arg6-Gly7-Leu8, and [Met]enkephalin-Arg6-Phe7. Rat preproenkephalin is 80% and 83% homologous to the bovine and human forms, respectively, at the nucleotide level and is 82% homologous to both species at the amino acid level. Rat preproenkephalin contains 269 amino acid residues, making it larger than the human (267 residues) and bovine (263 residues) precursors. The sensitivity for detection of rat preproenkephalin mRNA with the rat cDNA was several times greater than with the corresponding cDNAs from bovine and human sources.

Abstract:Background: Recognizing and identifying latent safety threats (LSTs) before patient care commences is crucial, aiding leaders in ensuring hospital readiness and extending its impact beyond patient safety alone. This study evaluated the effectiveness of a combination of Simulation-based Clinical Systems Testing (SbCST) with Healthcare Failure Mode and Effect Analysis (HFMEA) with regard to mitigating LSTs within a newly constructed hospital. Methods: Three phases of the combined SbCST and HFMEA approach were implemented across all hospital settings. The scenarios tested system functionalities, team responses, and resource availability. The threats thus identified were categorized into system-related issues, human issues, and resource issues, after which they were prioritized and addressed using mitigation strategies. Reassessment confirmed the effectiveness of these strategies before hospital commissioning. Results: More than 76% of the LSTs were mitigated through the combined approach. System-related issues, such as nonfunctional communication devices and faulty elevators, were addressed by leadership. Human issues such as miscommunication and nonadherence to hospital policy led to improvements in interprofessional communication and teamwork. Resource issues, including missing equipment and risks of oxygen explosion, were addressed through procurement, maintenance, and staff training for equipment preparation. Conclusion: The SbCST and HFMEA were highly effective with regard to proactively identifying and mitigating LSTs across all aspects of hospital preparedness. This systematic and comprehensive approach offers a valuable tool for enhancing patient safety in new healthcare facilities, thereby potentially setting a new standard for proactive hazard identification and risk management in the context of healthcare construction and commissioning.

Abstract:BACKGROUND: Today, considering the importance of bioterrorism, it may be time to assess the risk of bioterrorism as an important priority. Nurses, as the broadest group of therapy group, are very influential in this regard. General aim: To determine the impact of bioterrorism education through two methods of lectures and booklets on the knowledge and attitude of nursing staff. Practical Aim: Minimizing the damage caused by bioterrorism attacks by empowering nursing staff.MATERIALS AND METHODS: This two-group semiexperimental intervention study was performed as a before and after education intervention with the participation of 80 nursing staff of Shams Al-Shomus Nezaja Hospital affiliated to Health Administration of Islamic Republic of Iran Army Ground Forces. The study population included all hospital nursing staff in 2021. The inclusion criteria included having a willingness and informed consent to participate in the study and exclusion criteria included unwillingness to continue cooperation and failure to complete more than 10% of the questionnaire by the subject. This study was performed on 77 nurses in 2021. The nurses were divided into two groups: lecture and booklet. First, the pretest was completed by the nurses. After the educational intervention, both groups completed the posttest. Both groups then completed the questionnaire again after about 2 weeks. First, all the data were measured for normal distribution by the Kolmogorov-Smirnoff test. Statistical description was expressed for the variables with normal distribution as the ratio of geometric mean ± standard error and for abnormal variables as mean ± standard error. To compare the means of the studied quantitative variables, t-test and Wilcoxon matched-paired statistical tests were used. The significance level was considered <0.05. SPSS.16 and Graph Pad Prism 8.0.1 softwares were used to analyze the statistical data.RESULTS: Using both methods is effective in improving the level of knowledge and betterment attitude, but according to the paired t-test in comparison of the two groups, at intervals immediately after training and 2 weeks after training, a significant increase in the average knowledge score of the lecture group Statistically shows (P < 0.001). However, the analysis of attitude scores was performed between the two groups of lectures and booklets, which did not show statistically significant changes between them.CONCLUSIONS: One of the most important issues in the performance of medical staff in bioterrorism attacks is useful, effective, correct, and sufficient training. According to the results of the present study,bioterrorism training leads to increasing the level of knowledge and betterment the attitude of nurses in the field of bioterrorism. Therefore, the inclusion of training courses in medical centers, especially military medical centers, is necessary and sensitive.
Security of Medical Cyber-physical Systems: An Empirical Study on Imaging Devices

Abstract:Recent years have witnessed a boom of connected medical devices, which brings security issues in the meantime. Medical imaging devices, an essential part of medical cyber-physical systems, play a vital role in modern hospitals and are often life-critical. However, security and privacy issues in these medical cyber-physical systems are sometimes ignored. We perform an empirical study on imaging devices to analyse the security of medical cyber-physical systems. To be precise, we design a threat model and propose prospective attack techniques for medical imaging devices. To tackle potential cyber threats, we introduce protection mechanisms, evaluate the effectiveness and efficiency of protection mechanisms as well as its interplay with attack techniques. To scoring security, we design a hierarchical system that provides actionable suggestions for imaging devices in different scenarios. We investigate 15 devices from 9 manufacturers to demonstrate empirical comprehension and real-world security issues.

Abstract:The traditional focus for detecting outbreaks of an epidemic or bio-terrorist attack has been on the collection and analysis of medical and public health data. Although such data are the most direct indicators of symptoms, they tend to be collected, delivered, and analysed days, weeks, and even months after the outbreak. By the time this information reaches decision makers it is often too late to treat the infected population or to react in some other way. In this paper, we explore different sources of data, traditional and non-traditional, that can be used for detecting a bio-terrorist attack in a timely manner. We set our discussion in the context of state-of-the-art syndromic surveillance systems and we focus on statistical issues and challenges associated with non-traditional data sources and the timely integration of multiple data sources for detection purposes.
Emergency mental health management in bioterrorism events

Abstract:The United States has not suffered significant psychosocial or medical consequences from the use of biological weapons within its territories. This has contributed to a "natural" state of denial at the community level. This denial could amplify the sense of crisis, anxiety, fear, chaos, and disorder that would accompany such a bioterrorist event. A key part of primary prevention involves counteracting this possibility before an incident occurs. Doing so will require realistic information regarding the bioterrorism threat followed by the development of a planned response and regular practice of that response. Unlike in natural disasters or other situations resulting in mass casualties, emergency department physicians or nurses and primary care physicians (working in concert with epidemiologic agencies), rather than police, firemen, or ambulance personnel, will be most likely to first identify the unfolding disaster associated with a biological attack. Like community leaders, this group of medical responders must be aware of its own susceptibility to mental health sequelae and performance decrement as the increasing demands of disaster response outpace the availability of necessary resources. A bioterrorist attack will necessitate treatment of casualties who experience neuropsychiatric symptoms and syndromes. Although symptoms may result from exposure to infection with specific biological agents, similar symptoms may result from the mere perception of exposure or arousal precipitated by fear of infection, disease, suffering, and death. Conservative use of psychotropic medications may reduce symptoms in exposed and uninfected individuals, as may cognitive-behavioral interventions. Clear, consistent, accessible, reliable, and redundant information (received from trusted sources) will diminish public uncertainty about the cause of symptoms that might otherwise prompt persons to seek unnecessary treatment. Training and preparation for contingencies experienced in an attack have the potential to enhance delivery of care. Initiating supportive social, psychotherapeutic, and psychopharmacologic treatments judiciously for symptoms and syndromes known to accompany the traumatic stress response can aid the efficient treatment of some patients and reduce long-term morbidity in affected individuals. Preventive strategies and planning must take into account the idea that specific groups within the population are at higher risk for psychiatric morbidity. First responders comprise one group at psychologic risk in this situation, and healthcare providers comprise another. These and other high-risk groups will benefit from the same supportive interventions developed for the community as a whole.
Dentistry and Bioterrorism: A Legitimate Threat

Abstract:Bioterrorism involves the deliberate release of harmful biological agents, such as bacteria and viruses, aimed at causing mass casualties within a population. Often referred to as "poor man's nuclear weapons," chemical and biological weapons pose a significant threat due to their potential for mass destruction. Detecting and preventing bioterrorist attacks is challenging, making them one of the most feared scenarios. Dentistry plays a crucial role in responding to bioterrorism and other catastrophic events, leveraging its personnel and facilities effectively. This paper explores the signs and symptoms of biological agents used in mass destruction, as well as the oral and dental manifestations of both naturally occurring and bioengineered infectious agents. Furthermore, the article stresses the importance of countermeasures against bioterrorism, including deterrence, prevention, surveillance, medical management, and training. Emphasis is placed on implementing surveillance systems, bolstering medical readiness, and conducting training programs to effectively detect, assess, and respond to bioterrorism threats. Ultimately, the article underscores the critical role of dentists and healthcare professionals in collaborative efforts to mitigate the impacts of bioterrorism through proactive measures.
