Abstract:OBJECTIVES The aim of this study was to assess the feasibility and efficacy of left bundle branch pacing (LBBP) using a novel intraseptal technique to deliver cardiac resynchronization therapy (CRT) in patients with left bundle branch block (LBBB) and nonischemic cardiomyopathy. BACKGROUND His bundle pacing to correct LBBB is a viable alternative approach to achieve CRT but is limited by suboptimal lead delivery and high thresholds. METHODS This was a prospective, multicenter study performed between June 2017 and August 2018 at 6 centers. Patients with nonischemic cardiomyopathy, complete LBBB, and left ventricular ejection fractions (LVEFs) <= 50% who had indications for CRT and/or ventricular pacing in whom LBBP was attempted were included. Success rates, QRS duration, LVEF, left ventricular end-systolic volume, and improvement in functional class were assessed. RESULTS LBBP was successful in 61 of 63 patients (97%, mean age 68 +/- 11 years, 52.4% men). During LBBP, QRS duration narrowed from 169 +/- 16 to 118 +/- 12 ms (p < 0.001). Pacing threshold and R-wave amplitude remained stable at 1-year follow-up compared with implantation values (0.5 +/- 0.15 V/0.5 ms vs. 0.58 +/- 0.14 V/0.5 ms and 11.1 +/- 4.9 mV vs. 13.3 +/- 5.3 mV, respectively). LVEF increased significantly (33 +/- 8% vs. 55 1 10%; p < 0.001), with a reduction in left ventricular end-systolic volume (123 +/- 61 ml vs. 67 39 ml; p < 0.001). LVEF had normalized (>= 50%) in 75% of patients at 1 year. New York Heart Association functional class improved significantly from 2.8 +/- 0.6 at baseline to 1.4 +/- 0.6 at 1 year. No deaths or heart failure hospitalizations were observed during follow-up. CONCLUSIONS LBBP is a feasible and effective method for achieving electric resynchronization of LBBB, with resultant improvements in left ventricular structure and function. Low and stable pacing thresholds may be advantageous over His bundle paring for CRT in patients with LBBB and nonischemic cardiornyopathy. (C) 2020 by the American College of Cardiology Foundation.
Feasibility and Outcomes of Upgrading to Left Bundle Branch Pacing in Patients with Pacing-Induced Cardiomyopathy and Infranodal Atrioventricular Block.

Abstract:His bundle pacing (HBP) can reverse left ventricular (LV) remodeling in patients with right ventricular (RV) pacing-induced cardimyopathy (PICM) but may be unable to correct infranodal atrioventricular block (AVB). Left bundle branch pacing (LBBP) results in rapid LV activation and may be able to reliably pace beyond the site of AVB. Our study was conducted to assess the feasibility, safety, and outcomes of permanent LBBP in infranodal AVB and PICM patients. Patients with infranodal AVB and PICM who underwent LBBP for cardiac resynchronization therapy (CRT) were included. Clinical evaluation and echocardiographic and electrocardiographic assessments were recorded at baseline and follow-up. Permanent LBBP upgrade was successful in 19 of 20 patients with a median follow-up duration of 12 months. QRS duration (QRSd) increased from 139.3 ± 28.0 ms at baseline to 176.2 ± 21.4 ms (P < 0.001) with right ventricular pacing (RVP) and was shortened to 120.9 ± 15.2 ms after LBBP (P < 0.001). The mean LBBP threshold was 0.7 ± 0.3 V at 0.4 ms at implant and remained stable during follow-up. The left ventricular ejection fraction (LVEF) increased from 36.3% ± 6.5% to 51.9% ± 13.0% (P < 0.001) with left ventricular end-systolic volume (LVESV) reduced from 180.1 ± 43.5 to 136.8 ± 36.7 ml (P < 0.001) during last follow-up. LBBP paced beyond the site of block, which results in a low pacing threshold with a high success rate in infranodal AVB patients. LBBP improved LV function with stable parameters over the 12 months, making it a reasonable alternative to cardiac resynchronization pacing via a coronary sinus lead in infranodal AVB and PICM patients.

Abstract:Background Left bundle branch area pacing (LBBAP) aims to capture the cardiac conduction system in area of the left bundle branch. Currently, LBBAP is mainly performed using lumen-less pacing leads (LLLs) with preshaped sheath. However, the data on LBBAP with stylet-driven leads (SDLs) without sheath is limited. Objective This study presents the feasibility, safety, and pacing characteristics of LBBAP using SDLs without the support of sheath. Methods A total of 25 patients with bradycardia indications who received LBBAP implantation with an attempt of SDL (FINELINE II 4471 lead, Boston Scientific, MA, US) between August 2020 and April 2021 at Sir Run Run Shaw Hospital were included in this retrospective cohort study. Twenty of them finally were paced with SDL in priority (SDL-LBBAP group). Twenty propensity score matching patients who underwent LBBAP with LLL (Select Secure 3830 lead, Medtronic, MN, US) and 20 right ventricular septal pacing (RVSP) with regular active fixation lead respectively in the same period (the LLL-LBBAP group and RVSP group) were compared using ECG characteristics, pacing parameters and complications during 6-month follow-up. Results LBBAP was successful with SDL in 23 of 25 patients (92%) and 20 of them were paced with SDL first. In the SDL-LBBAP group, the average age was 70.4 +/- 8.2 years, and 55% of patients were male. Paced QRS duration and the stimulus to peak left ventricular activation time (Sti-LVAT) in SDL-LBBAP group were similar with those in LLL-LBBAP group and significantly shorter than those in RVSP group (126.1 +/- 14.1 ms vs. 124.8 +/- 10.9 ms, p = 1.00; 77.7 +/- 11.2 ms vs. 73.5 +/- 9.3 ms, P = .75; 126.1 +/- 14.1 ms vs. 147.7 +/- 22.5 ms, P<.001; 77.7 +/- 11.2 ms vs. 97.0 +/- 13.2 ms, P<.001). The pacing threshold and R-wave amplitude of SDL-LBBAP group were 0.53 +/- 0.18V and 11.53 +/- 3.63 mV at baseline respectively, which were comparable with the other two groups. During the 6-month follow-up, the pacing parameters remained stable and no lead-related complications were recorded. Conclusion It is feasible and safe to use stylet-directed pacing lead for permanent LBBAP without a delivery sheath. Similar to LLL, LBBAP using SDL showed stable parameters and narrower paced QRS duration compared with RVSP, which could be an alternative to LLL in LBBAP.

Abstract:Background: Left bundle branch area pacing (LBBAP) has been increasingly adopted as an alternative modality to cardiac resynchronization therapy (CRT). The feasibility and safety of using an LBBAP lead to provide sensing of ventricular arrhythmia in patients receiving an implantable cardioverter-defibrillator (ICD) with CRT has been demonstrated recently. Objectives: The purpose of our study was to analyze the feasibility, safety, and short-term follow-up of a traditional defibrillator lead at the LBBAP location. Methods: Patients who underwent successful LBBAP defibrillator using DF-1/DF-4 lead and delivery catheter were included in the study. Defibrillation threshold (DFT) testing was performed after implantation to assess the ability of the LBBAP defibrillator lead to sense and provide appropriate therapy for ventricular arrhythmia. Results: Although the ICD lead could be successfully deployed in the left bundle branch area in 7 of 8 patients, it was repositioned to the right ventricular (RV) apex because of atrial oversensing in 1 patient and cheesy septum in another patient. Acute procedural success was 62.5% (5/8 patients). Mean patient age was 62.6 ± 21.6 years. Mean procedural duration was 115.6 ± 38.1 minutes, with LBBAP defibrillator lead fluoroscopy duration of 10.6 ± 3.5 minutes. Mean capture threshold was 0.58 ± 0.23V/0.4 ms, sensed R-wave amplitude 9.6 ± 2.2 mV, pacing impedance 560 ± 145 Ω, and shock impedance 65.4 ± 5.5 Ω. Defibrillation testing was successful in inducing ventricular fibrillation and could be sensed and reverted promptly by the shock delivered through the lead. During mean follow-up of 3.8 ± 2.2 months, pacing parameters remained stable. No episodes of inappropriate arrhythmia detection or therapy delivery occurred during follow-up. Conclusion: LBBAP defibrillator is feasible, safe, and effective during short-term follow-up. DFT testing at the time of implantation will help to ensure appropriate sensing and treatment of ventricular arrhythmias.
Electrical Synchrony Optimization for Left Bundle Branch Area Pacing in Patients with Bradycardia and Heart Failure.

Abstract:Left bundle branch area pacing (LBBAP) has emerged as a promising physiological pac-ing modality. This study was designed to investigate the acute impact of the atrioventricu-lar delay (AVD) on cardiac electrical characteristics and identify an optimal range of AVDs for LBBAP to achieve electrical atrioventricular and interventricular synchrony. Patients indicated for ventricular or biventricular pacing were studied during routine fol-low-ups at least 3 months after LBBAP implantation. Patients were excluded if they had a complete AV block or persistent atrial fibrillation. AVD was programed from 40 to 240 ms or until intrinsic conduction occurred. Optimal AVD was determined by the elec-trocardiography criteria, including QRS duration, reduced R-wave in lead V1, reduced notching or slurring in lateral leads, and more desirable precordial QRS transition. A total of 38 patients (age 68.7 & PLUSMN; 10.3 years; 16 male (42%); 18 dual-chamber pacemakers and 20 cardiac resynchronization therapy devices; average follow-up period 15.1 & PLUSMN; 10.2 months) were included. The fusion of LBBAP and intrinsic right ventricular conduc-tion occurred in 21 patients with corresponding optimal AVD determined. A great propor-tion (& drangbrac;85%) of the optimal AVDs ranged from 50% to 80% of the observed atrium-to-left bundle branch-sensing (A-LBBS) intervals. The linear correlation between the optimal AVD and corresponding A-LBBS interval (optimal AVD = 0.84 & POUND; [A-LBSs interval] - 36 ms) produced R = 0.86 and p <0.0001. In conclusion, AVD selection during LBBAP greatly impacted the ventricular electrical characteristics and the optimal AVD was line-arly correlated with the corresponding A-LBBS interval.& COPY; 2023 Elsevier Inc. A Multi-Lead ECG Monitoring Combined with a Programmed Intracavitary ECG to Complete LBBaP Pacing: Study Protocol for a Single-center, Prospective Study

Abstract:Abstract Background: Left bundle branch area pacing (LBBaP) has emerged as an alternative method for delivering physiological pacing to provide a near physiological ventricular activation. However, the clinical application and promotion of LBBaP is currently largely depended on the electrophysiological multichannel recorder to record the intracardiac electrogram (EGM). It is difficult for many primary hospitals lack of relevant equipment to carry out such operations. Methods: We aim to recruit 100 patients of bradycardia with a pacemaker indication and nomal ejection fraction(EF>50%), using multi-lead surface electrocardiogram (ECM) monitoring and pacing system analyzer(PSA), combining with the nine-zone pacing method of the LBBaP, to complete LBBaP without an electrophysiological multichannel recorder, only based on the changes of paced QRS morphology on the multi-lead ECG monitoring and the intracavitary electrocardiogram p potential seen in pacing system analyzer. After completing the lead implantation, we will assess multiple parameters to verify whether the LBBaP is successfully achieved and to judge the feasibility and effectiveness of LBBaP using this method without electrophysiological multichannel recorder. Discussion: This study will aim to demonstrate the effectiveness and feasibility to achieve LBBaP combining the multi-lead ECG monitoring and the intracavitary ECG of pacing system analyzer without electrophysiological multichannel recorder. Electrophysiological Insights into Three Modalities of Left Bundle Branch Area Pacing in Patients Indicated for Pacing Therapy

Abstract:Left bundle branch pacing (LBBP) has been adopted as a new pacing therapy whether in routine pacing or patients with heart failure, but the criteria for a completely captured LBBP are too complicated and have a low success rate in routine clinical practice. Consecutive patients with pacing therapy indications were enrolled. Left bundle branch area pacing (LBBAP) was conducted, and the presence of LBB potential, paced QRS duration, stimulus to left ventricular activation time (Stim-LVAT), and LBB potential to left ventricular activation time (LBB po-LVAT) were determined and utilized to characterize LBBAP modalities. Pacing parameters and safety were assessed at 6-month follow-up. LBBAP succeeded in 95.6% of patients (103/106) who completed the 6-month follow-up. Complete LBBP was achieved in 21 (20%) patients, characterized with a short Stim-LVAT equal to LBB po-LVAT. Incomplete LBBP was achieved in 58 (56%) patients with a short Stim-LVAT equal to LBB po-LVAT at a high pacing output and a relatively longer Stim-LVAT at a low pacing output. Deep septal pacing (DSP) characterized with no LBB potential and a longer Stim-LVAT (83.3 +/- 7.7 ms) than that in LBBP (71.37 +/- 7.1 ms, P < 0.01 versus DSP) was observed in 24 (23%) patients. Complete LBBP had a longer total procedure time and longer fluoroscopic time than the other two groups. This study describes the similarities and differences in electrophysiological characteristics and the possible mechanisms of the different types of LBBAP, classified into 3 modalities in routine clinical practice, each with narrow paced QRS duration and stable parameters, indicating LBBAP can be a near-physiological pacing modality.

Abstract:Astacin (EC 3.4.24.21) from the freshwater crayfish (Astacus astacus) is a prototype for the metzincin superfamily and for the astacin family of zinc peptidases, enzymes which are involved in hatching processes, embryonic patterning and tissue remodelling. Here we report on the cloning and overexpression in Escherichia coli of an astacin cDNA which was reverse-transcribed from crayfish midgut-gland mRNA. A cDNA construct based on this clone was generated which comprised the nucleotide sequence encoding mature astacin devoid of the signal and propeptide. This construct was cloned into the pET3a vector and used to transform E. coli BL21(DE3) cells. Recombinant astacin was purified from inclusion bodies and dissolved under reducing conditions. For folding, the protein was diluted into neutral buffer containing l-arginine, GSH and EDTA. Eventually, Zn(2+) was added by dialysis and the fraction of active enzyme was affinity-purified on immobilized Pro-Leu-Gly hydroxamate. As shown by superimposition of the corresponding three-dimensional structures, this inhibitor binds to a region of the active-site cleft that is conserved in most metzincins. Therefore this principle behind this affinity technique, originally introduced for fibroblast collagenase by Moore and Spilburg [Biochemistry (1986) 25, 5189-5195], is applicable throughout the metzincin superfamily of metalloproteases, despite their otherwise differing cleavage specificities. Recombinant astacin is active on gelatine zymograms and in a quenched fluorescence assay, yielding kinetic parameters comparable with those of wild-type astacin purified from crayfish stomach.
The role of the central amygdala in stress and adaption.

Abstract:Recent views on stress emphasise the existence of more than one response pattern to stressful events, and the importance of individual differences in coping with environmental challenges. Therefore, in the evaluation of the specific contribution of the central nucleus of the amygdala (CEA) in stress and adaptation it has to be considered whether the stress is acute or conditioned, and whether it requires active or passive styles of coping. Based on series of our own studies, we propose that the CEA is consistently involved in the organisation of processes of passive coping, reflected in immobile behaviour and parasympathetic activity. Furthermore, a differential regulation of the CEA via its peptidergic neuronal input may underlie the distinct behavioural and physiological stress patterns accompanying the different styles of coping. Additionally, the involvement of the CEA in neuroendocrine control is addressed. The CEA exerts a general, modulatory influence on the neuroendocrine control to acute stressors, whereas this output during conditioned stress seems to be independent of the CEA. The neuroendocrine state as achieved during acute stress is, via feedback to the brain, of importance in learning about the situation, and to consolidate the experience. In this review an attempt is made to provide an integrated model of CEA functioning in relation to stress and adaptation.

Abstract:BACKGROUND Cardiac resynchronization therapy (CRT) via biventricular pacing has demonstrated clinical benefits in patients with heart failure (HF) and ventricular dyssynchrony. Other approaches of CRT is little known. OBJECTIVE The purpose of this study was to assess the feasibility, safety, and efficacy of left bundle branch area pacing (LBBAP) in patients with HF and left bundle branch block (LBBB). METHODS Eleven consecutive patients with HF, reduced left ventricular ejection fraction and LBBB and indicated for CRT were recruited. LBBAP was achieved via transventricular septal approach and characterized by narrower QRS duration, shortened peak left ventricular activation time, and right bundle branch conduction delay on the electrocardiogram. Electrocardiogram, echocardiogram, and cardiac function were evaluated at baseline and follow-up. Interventricular mechanical delay and 3-dimensional tissue synchronization imaging during LBBAP and intrinsic LBBB status were measured by echocardiography at follow-up. RESULTS LBBAP significantly shortened QRS duration (from baseline 180.00 +/- 15.86 ms to 129.09 +/- 15.94 ms; P < .01) and left ventricular activation time (from baseline 108.18 +/- 15.54 ms to 80.91 +/- 9.95 ms; P < .01). Interventricular mechanical delay and the standard deviation of tissue synchronization imaging of 12 left ventricular (LV) segments were significantly shorter during LBBAP than in intrinsic LBBB status (both with P < .01). At a mean follow-up period of 6.7 months, New York Heart Association functional class, plasma level of B-type natriuretic peptide, LV end-systolic diameter, and left ventricular ejection fraction were significantly improved (all with P < .05 vs baseline). CONCLUSION The study demonstrates that LBBAP is clinically feasible in patients with systolic HF and LBBB. LBBAP can be a new CRT technique to correct LBBB, provide ventricular synchrony, and improve clinical symptoms with LV reverse remodeling.

Feasibility and Efficacy of Left Bundle Branch Area Pacing in Patients Indicated for Cardiac Resynchronization Therapy.

Abstract:Aims The present study was to evaluate the feasibility and clinical outcomes of left bundle branch area pacing (LBBAP) in cardiac resynchronization therapy (CRT)-indicated patients. Methods and results LBBAP was performed via transventricular septal approach in 25 patients as a rescue strategy in 5 patients with failed left ventricular (LV) lead placement and as a primary strategy in the remaining 20 patients. Pacing parameters, procedural characteristics, electrocardiographic, and echocardiographic data were assessed at implantation and follow-up. Of 25 enrolled CRT-indicated patients, 14 had left bundle branch block (LBBB, 56.0%), 3 right bundle branch block (RBBB, 12.0%), 4 intraventricular conduction delay (IVCD, 16.0%), and 4 ventricular pacing dependence (16.0%). The QRS duration (QRSd) was significantly shortened by LBBAP (intrinsic 163.6 +/- 29.4 ms vs. LBBAP 123.0 +/- 10.8 ms, P < 0.001). During the mean follow-up of 9.1 months, New York Heart Association functional class was improved to 1.4 +/- 0.6 from baseline 2.6 +/- 0.6 (P < 0.001), left ventricular ejection fraction (LVEF) increased to 46.9 +/- 10.2% from baseline 35.2 +/- 7.0% (P < 0.001), and LV end-diastolic dimensions (LVEDD) decreased to 56.8 +/- 9.7mm from baseline 64.1 +/- 9.9mm (P < 0.001). There was a significant improvement (34.1 +/- 7.4% vs. 50.0 +/- 12.2%, P < 0.001) in LVEF in patients with LBBB. Conclusion The present study demonstrates the clinical feasibility of LBBAP in CRT-indicated patients. Left bundle branch area pacing generated narrow QRSd and led to reversal remodelling of LV with improvement in cardiac function. LBBAP may be an alternative to CRT in patients with failure of LV lead placement and a first-line option in selected patients such as those with LBBB and heart failure.

Long-term data on the safety and performance of LBBAP 1 year postdevice implantation has not been well described. Methods and results: Sixty-five patients (49% females) who received LBBAP for bradycardia indications using the SelectSecure 3830 lead (Medtronic) were retrospectively evaluated. Clinical variables were examined. Lead parameters were obtained at implant and during regular follow-up. Mean age of patients was 75.7 ± 10.1 years with left ventricular ejection fraction 59.8 ± 10.4%. Indications for pacing were atrioventricular block 55%, sinus node dysfunction 19%, tachy-brady syndrome 15%, atrioventricular node ablation 8%, and bail out cardiac resynchronization therapy 3%. Mean baseline QRS measured 120 ± 38 ms, paced QRS duration was 138 ± 22ms. Paced QRS narrowed by 24 ms in those with pre-existing left bundle branch block (BBB), increased by 1 ms in those with pre-existing right BBB, and increased by 42 ms in those with no BBB. LBBAP threshold at implant was 0.521 ± 0.153 V at 0.4 ms, and increased to 0.654 ± 0.186 V at 3 months (+26%), 0.707 ± 0.186 V at 6 months (+36%), and 0.772 ± 0.220 V at 12 months (+48%). Patients with left BBB showed the maximum benefit with QRS narrowing 24 ms. Pacing impedance remained unchanged with no procedure-related complications. Conclusion: LBBAP is a durable form of conduction system pacing with pacing thresholds remaining relatively stable over 12 months post device implantation. Patients with left BBB display the narrowest paced QRS.

Abstract:Background Although left bundle branch area pacing (LBBAP) has been shown to be a feasible option for delivering physiological pacing, data are largely limited to single‐center reports. The aim of this analysis was to systematically assess the safety and efficacy of LBBAP with the Model 3830 lead among primarily bradycardia patients. Methods and Results PubMed, Embase, Cochrane Library, and Google Scholar were searched for full‐text articles on LBBAP using the SelectSecure Model 3830 lumenless lead. Rates and means were estimated using random‐ and mixed‐effects models. Of 3395 articles, 53 met inclusion criteria, representing 6061 patients undergoing an implant attempt. Average patient age was 68.1 years (95% CI: 66.6, 69.6) and 53.1% were male (95% CI: 50.5%, 55.7%). The average implant success rate among bradycardia‐indicated patients was 92.7% (95% CI: 89.5%, 94.9%). The overall estimated procedural adverse event rate was 2.5% (95% CI: 1.1%, 5.4%). The estimated septal perforation rate at implant was 1.6% (95% CI: 1.0%, 2.6%) with no adverse clinical sequelae reported. Pacing thresholds were low at implant (0.67 V [95% CI: 0.64, 0.70]) and remained stable through 12 months (0.76 V [95% CI: 0.72, 0.80]). Among bradycardia‐indicated patients, LVEF remained stable from baseline to post‐implant (59.5% [95% CI: 57.9%, 61.1%] vs. 60.1% [95% CI: 58.5%, 61.7%]). Conclusion This meta‐analysis including 6061 patients implanted with a Model 3830 lead for LBBAP found an average implant success rate of 92.7% and a procedural adverse event rate of 2.5% with stable electrical parameters and LVEF post‐implant.

Abstract:Background Interest in conduction system pacing (CSP) has increased in recent years. For left bundle branch area pacing (LBBAP), 4F and 6F pacing leads have been shown to be effective and safe. No information is available on the potential use of ICD leads for LBBAP. Aim The objective of the study was to demonstrate the feasibility of implanting an ICD lead in the LBBAP position to reduce the number of leads for CRT-D therapy. Patients and Methods We implanted the 7F ICD lead in the LBBAP position through a new 10F CSP delivery sheath in ten consecutive patients. Periprocedural data, QRS morphology and degree of QRS reduction were analyzed. Results Ten consecutive patients (one female) aged 69.5±9.2 years were treated with the above-mentioned method. Eight patients had ischemic cardiomyopathy and two patients had nonischemic cardiomyopathy. All patients had a CRT-D indication. Eight patients had a wide QRS complex with complete LBB and a QRS complex of 184.8±25.8 ms. Two patients had a narrow QRS complex but first-degree AV block with a PR interval greater than 240 ms. Left ventricular ejection fraction was 31.8±8.6%. Operative and fluoroscopy times were 103.6±30.3 minutes and 10.5±6.1 minutes, respectively. On average, 1.7 ± 0.6 screwing attempts were required per patient. The intraprocedural LBBAP threshold was 0.78±0.6V/1.0ms, the impedance was 530.8±86.5 ohm, and the R-wave amplitude was 9±3.3mV. The unipolar paced QRS complex was 123.6±15.9ms, the bipolar paced QRS complex was 129.5±12.8ms, and the bipolar paced QRS complex on the next day was 133.3±11.7. The mean duration from stimulus to R-wave peak in lead V6 was 82.9 ± 19.7 ms. The R-wave interpeak interval between leads V6 and V1 was 43.4 ± 9.7 ms. All but one patient received an effective defibrillation threshold test at 30J (n=8) and 36J (n=1). One patient was not tested during the procedure due to high risk. Conclusion This first-in-human study of ICD lead implantation in the LBBAP position demonstrated its feasibility and efficacy in achieving conduction system stimulation. Left Bundle Branch Area Defibrillator (LBBAD): A First-in-Human Feasibility Study

Abstract:Background: Left bundle branch area (LBBA) pacing is a promising pacing technique. LBBA implantable cardioverter-defibrillator (ICD) lead implantation reduces the number of leads in patients with both pacing and ICD indications, reducing cost and potentially increasing safety. LBBA positioning of ICD leads has not previously been described. Objectives: The goal of this study was to evaluate the safety and feasibility of implanting an LBBA ICD lead. Methods: This prospective, single-center, feasibility study was conducted in patients with an ICD indication. LBBA ICD lead implantation was attempted. Acute pacing parameters and paced electrocardiography data were collected, and defibrillation testing was performed. Results: LBBA defibrillator (LBBAD) implantation was attempted in 5 patients (mean age 57 ± 16.5 years; 20% female) and achieved in 3 (60%). Mean procedural and fluoroscopy duration were 170.0 ± 17.3 minutes and 28.8 ± 16.1 minutes, respectively. Left bundle capture was achieved in 2 patients (66%) and left septal capture in 1 patient. LBBA pacing exhibited a mean QRS duration and V6 R-wave peak time of 121.3 ± 8.3 milliseconds and 86.1 ± 10.0 milliseconds. In all 3 patients, defibrillation testing was successful with mean time to adequate shock delivery of 8.6 ± 2.6 seconds. Acute LBBA pacing threshold and R-wave amplitudes were 0.80 ± 0.60 V at 0.4 milliseconds and 7.0 ± 2.7 mV. No LBBA lead-related complications occurred. Conclusions: This first-in-human evaluation showed the feasibility of LBBAD implantation in a small cohort of patients. With current tools, implantation remains complex and time-consuming. Considering the feasibility reported and the potential benefits, further technological development in this field is warranted with evaluation of long-term safety and performance.

Abstract:BackgroundLeft bundle branch pacing (LBBP) is emerging as an effective alternative to achieve cardiac resynchronization therapy (CRT) and improve heart function. The purpose of our study was to investigate the feasibility and efficacy of LBBP in heart failure patients with left ventricular ejection fraction (LVEF) <50% and left bundle branch block (LBBB).MethodsAll patients with complete LBBB and LVEF <50% were retrospectively included in the study from April 2018 to April 2021 and underwent CRT via LBBP implantation. ECG, pacing parameters, the New York Heart Association (NYHA) functional class, echocardiographic measurements, and complications were recorded and analyzed at implant and during follow-up of 1, 6, and 12 months.ResultsLeft bundle branch pacing was successful in all 34 patients (mean age 65.6 ± 11.2 years, 67.6% men). A significant decrease in QRS duration (QRSd) was observed after the LBBP operation for 1 month (153.2 ± 1.7 vs. 111.9 ± 2.6 ms, p < 0.01). LBB capture threshold and R-wave amplitude remained stable at 12-month follow-up when compared with implantation values (0.62 ± 0.13 V @ 0.4 ms vs. 0.73 ± 0.21 V @ 0.4 ms, 12.02 ± 5.68 mV vs. 8.58 ± 4.09 mV, respectively). LVEF increased significantly (35.28 ± 1.70% vs. 51.09 ± 1.71%, p < 0.01) accompanied with reduced left ventricular end-diastolic dimension (LVEDd; 65.3 ± 1.99 vs. 53.58 ± 2.07 mm, p < 0.01) and left atrial dimension (LAD; 49.03 ± 1.32 vs. 40.67 ± 1.58 mm, p < 0.01). Normalized LVEF (LVEF ≥ 50%) was found in 70.5% of patients at 12 months. The NYHA classification, brain natriuretic peptide (BNP), and 6-minute walk test (6MWT) were significantly improved at follow-up of 12 months (all p < 0.01 vs. baseline). No deaths or heart failure hospitalizations were observed during the follow-up period.ConclusionThe current work suggested that LBBP was feasible with a high success implantation rate and effective to correct LBBB and improved left ventricular structure and function with a low and stable pacing threshold.

Abstract:Background Cardiac resynchronization therapy (CRT) is typically attempted with biventricular pacing. One-third of patients are non-responders. Left bundle branch area pacing (LBBAP) has been evaluated as an alternative means. Objective To assess the feasibility and clinical response of permanent LBBAP as an alternative to biventricular pacing. Methods Of 479 consecutive patients referred with heart failure (HF), 50 with biventricular pacing (BiV-CRT) and 51 with left bundle branch area pacing (LBBAP-CRT) were included in this analysis after study exclusions. Quality-of-Life (QoL) assessments; electrocardiogram and echocardiographic measurements; and New York Heart Association (NYHA) class were obtained at baseline and at 6-monthly Intervals. Results There were no differences in baseline characteristics between groups (P=NS). Clinical outcomes such as left ventricular ejection fraction (LVEF), left ventricular end-systolic volume (LVESV), QoL and NYHA class were significantly improved for both pacing groups compared to baseline. LBBAP-CRT group showed greater improvement in LVEF at 6- (p=0.001) and 12-months (P=0.021), accompanied by greater reduction in LVESV (P=0.007). QRS <120 ms (baseline 160.82±21.35ms vs 161.08±24.48ms) was achieved in 30% in BiV-CRT vs 71% in LBBAP-CRT group (P=<0.001). Improvement in NYHA class (P=0.031) and QoL index was greater (P=0.014). Reduced HF admissions (P=0.003) and healthcare utilisation (P<0.05), and improved lead performance (p<0.001) were observed in LBBAP-CRT group. Conclusions LBBAP-CRT is a feasible and effective cardiac resynchronisation therapy. It results into a meaningful improvement in QoL and reduction in healthcare utilisation. This can be offered as an alternative to BiV-CRT or potentially as first-line therapy.

Abstract:Introduction Left bundle branch area (LBBA) pacing (LBBAP) has been proposed as an alternative therapy option in patients indicated for cardiac pacing to treat bradycardia or heart failure. The aim of the study was to evaluate the safety and effectiveness of LBBAP in patients implanted with a Tendril 2088 stylet‐driven lead. Methods The international retrospective data collection registry included 11 sites from 5 countries globally. Patients with attempted implants of the Tendril lead in the LBBA were followed for at least 6 months post the implant attempt. The primary safety and efficacy endpoints were freedom from LBBAP lead‐related serious adverse events and the composite of LBBA capture threshold of ≤2.0 V and R‐wave amplitudes ≥5 mV (or ≥value at implant), respectively. Results Of 221 patients with attempted implants of the Tendril 2088 lead in the LBBA, 91.4% (202/221) had successful implants for LBBAP. Regardless of the LBBAP implant success, all patients were followed for at least 6 months (8.7 ± 7.3 months). Baseline characteristics: 44% female, 84% ≥65 years old, 34% coronary artery disease, and 86% of primary indications for pacemaker implant. Both primary safety and effectiveness endpoints were met (freedom from LBBAP lead‐related serious adverse device effects of 99.5% and electrical performance composite success rate of 93%). The capture thresholds in LBBAP at implant and 6 months were 0.8 ± 0.3 V@0.4 ± 0.1 ms and 0.8 ± 0.3 V@0.4 ± 0.1 ms. The rate of patients with capture threshold rise ≥1 V was 1.5% through 6 months. The R‐wave amplitudes in LBBAP at implant and 6 months were 9.3 ± 3.2 mV and 10.6 ± 3.0 mV. Conclusions This large multicenter study demonstrates that the stylet‐driven TendrilTM STS 2088 lead is safe and effective for LBBAP with high success and low complication rates.

The Feasibility and Safety of Left Bundle Branch Pacing Vs. Right Ventricular Pacing after Mid-Long-term Follow-Up: a Single-Centre Experience

Abstract:Aims The aim of this study is to prospectively assess the feasibility and safety of left bundle branch pacing (LBBP) when compared with right ventricular pacing (RVP) during mid-long-term follow-up in a large cohort. Methods and results Patients (n = 554) indicated for pacemaker implantation were prospectively and consecutively enrolled and were non-randomized divided into LBBP group and RVP group. The levels of cTnT and N-terminal pro-B type natriuretic peptide were measured and compared within 2 days post-procedure between two groups. Implant characteristics, procedure-related complications, and clinical outcomes were also compared. Pacing thresholds, sensing, and impedance were assessed during procedure and follow-up. Left bundle branch pacing was feasible with a success rate of 94.8% with high incidence of LBB potential (89.9%), selective LBBP (57.8%), and left deviation of paced QRS axis (79.7%) with mean Sti-LVAT of 65.07 +/- 8.58 ms. Paced QRS duration was significantly narrower in LBBP when compared with RVP (132.02 +/- 7.93 vs. 177.68 +/- 15.58 ms, P < 0.0001) and the pacing parameters remained stable in two groups during 18 months follow-up. cTnT elevation was more significant in LBBP when compared with RVP within 2 days post-procedure (baseline: 0.03 +/- 0.03 vs. 0.02 +/- 0.03 ng/mL, P = 0.002; 1 day post-procedure: 0.13 +/- 0.09 vs. 0.04 +/- 0.03 ng/mL, P < 0.001; 2 days post-procedure: 0.10 +/- 0.08 vs. 0.03 +/- 0.08 ng/mL, P < 0.001). The complications and cardiac outcomes were not significantly different between two groups. Conclusion Left bundle branch pacing was feasible in bradycardia patients associated with stable pacing parameters during 18 months follow-up. Paced QRS duration was significantly narrower than that of RVP. Though cTnT elevation was more significant in LBBP within 2 days post-procedure, the complications, and cardiac outcomes were not significantly different between two groups.
