Prognosis of lung cancer with simple brain metastasis patients and establishment of survival prediction models: a study based on real events

Abstract:Objectives The aim of this study was to explore risk factors for the prognosis of lung cancer with simple brain metastasis (LCSBM) patients and to establish a prognostic predictive nomogram for LCSBM patients. Materials and methods Three thousand eight hundred and six cases of LCSBM were extracted from the Surveillance, Epidemiology, and End Results (SEER) database from 2010 to 2015 using SEER Stat 8.3.5. Lung cancer patients only had brain metastasis with no other organ metastasis were defined as LCSBM patients. Prognostic factors of LCSBM were analyzed with log-rank method and Cox proportional hazards model. Independent risk and protective prognostic factors were used to construct nomogram with accelerated failure time model. C-index was used to evaluate the prediction effect of nomogram. Results and conclusion The younger patients (18–65 years old) accounted for 54.41%, while patients aged over 65 accounted for 45.59%.The ratio of male: female was 1:1. Lung cancer in the main bronchus, upper lobe, middle lobe and lower lobe were accounted for 4.91%, 62.80%, 4.47% and 27.82% respectively; and adenocarcinoma accounted for 57.83% of all lung cancer types. The overall median survival time was 12.2 months. Survival rates for 1-, 3- and 5-years were 28.2%, 8.7% and 4.7% respectively. We found female (HR = 0.81, 95% CI 0.75–0.87), the married (HR = 0.80; 95% CI 0.75–0.86), the White (HR = 0.90, 95% CI 0.84–0.95) and primary site (HR = 0.45, 95% CI 0.39–0.52) were independent protective factors while higher age (HR = 1.51, 95% CI 1.40–1.62), advanced grade (HR = 1.19, 95% CI 1.12–1.25) and advanced T stage (HR = 1.09, 95% CI 1.05–1.13) were independent risk prognostic factors affecting the survival of LCSBM patients. We constructed the nomogram with above independent factors, and the C-index value was 0.634 (95% CI 0.622–0.646). We developed a nomogram with seven significant LCSBM independent prognostic factors to provide prognosis prediction.

Abstract:Lung cancer is one of the most common malignancies in the United States, and the common metastatic sites in advanced non-small cell lung cancer (NSCLC) are bone, brain, adrenal gland, and liver, respectively, among which patients with liver metastases have the worst prognosis. We retrospectively analyzed 1963 patients diagnosed with NSCLC combined with liver metastases between 2010 and 2015. Independent prognostic factors for patients with liver metastases from NSCLC were identified by univariate and multivariate Cox regression analysis. Based on this, we developed a nomogram model via R software and evaluated the performance and clinical utility of the model by calibration curve, receiver operating characteristic curves, and decision curve analysis (DCA). The independent prognostic factors for NSCLC patients with liver metastases included age, race, gender, grade, T stage, N stage, brain metastases, bone metastases, surgery, chemotherapy, and tumor size. The area under the curve predicting OS at 6, 9, and 12 months was 0.793, 0.787, and 0.784 in the training cohort, and 0.767, 0.771, and 0.773 in the validation cohort, respectively. Calibration curves of the nomogram showed high agreement between the outcomes predicted by the nomogram and the actual observed outcomes, and the DCA further demonstrated the value of the clinical application of the nomogram. By analyzing the Surveillance, Epidemiology, and End Results database, we established and verified a prognostic nomogram for NSCLC patients with liver metastases, to personalize the prognosis of patients. At the same time, the prognostic nomogram has a satisfactory accuracy and the results are a guide for the development of patient treatment plans.

Abstract:Liver was one of the most common distant metastatic sites in breast cancer. Patients with distant metastasis were identified as American Joint Committee on Cancer (AJCC) stage IV indicating poor prognosis. However, few studies have predicted the survival in females with T1-2N0-1 breast cancer who developed liver metastasis. This study aimed to explore the clinical features of these patients and establish a nomogram to predict their overall survival. 1923 patients were randomly divided into training (n = 1154) and validation (n = 769) cohorts. Univariate and multivariate analysis showed that age, marital status, race, estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor-2 (HER2), chemotherapy, surgery and bone metastasis, brain metastasis were considered the independent prognostic indicators. Abstract:Background The optimal tool for predicting the survival of renal cell carcinoma (RCC) patients with lung metastases remains controversial. Methods We selected patients diagnosed with RCC and lung metastases, from 2010 to 2015, from the Surveillance, Epidemiology, and End Results (SEER) database. After the selection of inclusion criteria and exclusion criterion, the rest of the patients were incorporated into model analysis. Least absolute shrinkage and selection operator (LASSO) regression was used to select the most important features for construction of a nomogram predicting cancer-specific survival. A calibration plot and the concordance index (C-index) were used to estimate nomogram efficacy in a validation cohort. The association between important factors selected by LASSO regression, and prognosis was assessed by the Kaplan-Meier (KM) survival curve. The receiver operating characteristic (ROC) curves were drawn to compare sensitivity and specificity between the nomogram we built and the TNM stage-based model. Results A total of 1,369 patients met the inclusion criteria, but not the exclusion criteria. The LASSO regression model reduced 15 features to seven potential predictors of survival, including tumor grade, the extent of surgery, N and T status, histological profile, and brain and bone metastasis status. Such features had good discrimination in the KM survival curves. The nomogram showed excellent discriminatory power (C-index, 0.71; 95% confidence interval: 0.70 to 0.72) and good calibration in terms of both 1- and 2-year cancer-specific survival. The nomogram showed great discriminatory power (C-index 0.68) and adequate calibration when applied to the validation cohort. The areas under the curve (AUCs) of nomogram were 0.767 and 0.780, respectively, and the AUCs of TNM stage were 0.617 and 0.618 at 1 and 2 years, respectively. Conclusions Our nomogram might play a major role in predicting the cancer-specific survival of RCC patients with lung metastases.

Abstract:BACKGROUND: Lung large cell neuroendocrine carcinoma (L-LCNEC) is a rare and rapidly progressing lung cancer. We aimed to formulate a nomogram model to predict the survival of L-LCNEC patients.METHODS: Clinical data of patients with L-LCNEC, lung large cell cancer (L-LCC) and small cell lung cancer (SCLC) were derived from the Surveillance, Epidemiology, and End Results (SEER) database. The characteristics and prognosis of L-LCNEC were investigated by comparing with that of L-LCC and SCLC, respectively. All L-LCNEC patients were randomly assigned into training group and validation group. A prognostic nomogram model was established for the overall survival (OS) in L-LCNEC patients. Furthermore, we enrolled 112 L-LCNEC patients from our department to validate the nomogram model.RESULT: 3,076 L-LCNEC, 11,163 L-LCC, and 78,097 SCLC patients were collected and enrolled in our analyses. Compared with L-LCC and SCLC, differences were observed in L-LCNEC in age, sex, race, marital status, SEER registry, TNM stage, and treatment. Furthermore, higher proportions of L-LCNEC were located at the upper lobe and unilateral lung compared with SCLC. L-LCNEC has similar survival to L-LCC, but better than SCLC. We identified that the age, gender, T, N, and M classification, and treatment were the independent prognostic predictors. A nomogram model was formulated to predict the OS. Calibration curves were performed to show optimal coherence between predicted probability of survival and actual survival, with a concordance index of 0.775. The external cohort included 112 patients and all of them underwent surgical treatment. The external validation demonstrated the reliability of this model.CONCLUSIONS: The nomogram demonstrated its discrimination capability to predict the OS for L-LCNEC patients.

Abstract:PURPOSE:A nomogram is a useful and convenient tool for individualized cancer prognoses. We sought to develop a clinical nomogram for predicting survival of patients with resected non-small-cell lung cancer (NSCLC).PATIENTS AND METHODS:On the basis of data from a multi-institutional registry of 6,111 patients with resected NSCLC in China, we identified and integrated significant prognostic factors for survival to build a nomogram. The model was subjected to bootstrap internal validation and to external validation with a separate cohort of 2,148 patients from the International Association for the Study of Lung Cancer (IASLC) database. The predictive accuracy and discriminative ability were measured by concordance index (C-index) and risk group stratification.RESULTS:A total of 5,261 patients were included for analysis. Six independent prognostic factors were identified and entered into the nomogram. The calibration curves for probability of 1-, 3-, and 5-year overall survival (OS) showed optimal agreement between nomogram prediction and actual observation. The C-index of the nomogram was higher than that of the seventh edition American Joint Committee on Cancer TNM staging system for predicting OS (primary cohort, 0.71 v 0.68, respectively; P < .01; IASLC cohort, 0.67 v 0.64, respectively; P = .06). The stratification into different risk groups allowed significant distinction between survival curves within respective TNM categories.CONCLUSION:We established and validated a novel nomogram that can provide individual prediction of OS for patients with resected NSCLC. This practical prognostic model may help clinicians in decision making and design of clinical studies.
Construction and Validation of Risk Factors and Prognostic Model for Liver Metastasis of Pancreatic Cancer

Abstract:Background Pancreatic cancer (PC) is a common malignancy that often metastasizes to the liver. The presence of liver metastasis (LM) in PC significantly impacts treatment selection and prognosis, but factors affecting the occurrence and prognosis of pancreatic cancer with liver metastasis (PCLM) are not well described. Methods Patients diagnosed with PC between 2010 and 2015 were selected from the Surveillance Epidemiology and End Results (SEER) database. Independent risk factors for PCLM were identified using univariable and multivariable logistic regression. Independent prognostic factors affecting the overall survival (OS) of PCLM patients were analyzed by univariate and multivariate Cox regression, and two nomograms were constructed to predict the risk and prognosis of PCLM. Nomograms were evaluated by receiver operating characteristic (ROC) analysis, C-index, calibration plots, and decision curve analysis (DCA). Results Multivariate logistic regression showed that age, primary site, grade, histological subtype, N stage, radiotherapy, surgery, bone metastasis, and lung metastasis were independent risk factors for PCLM. Multivariable COX regression showed that age, grade, histological subtype, surgery, radiotherapy, chemotherapy and lung metastasis were independent prognostic factors for PCLM. Diagnostic and prognostic nomograms were constructed based on the ROC curves, C-index, calibration curves and DCA curves, and both nomograms showed good predictive performance and clinical utility. Conclusion The two nomograms constructed in this study exhibit good predictive performance in the risk and prognosis of PCLM and may thus serve as a guide for future clinical management of PCLM.
Influence of unexpected engine start sound of hybrid vehicle in cabin –Comparison between Japanese and German Drivers–

Abstract:In this study, we investigated an impression of engine automatic start sounds for charging electricity of hybrid electric vehicles having idling reduction function. Subjective evaluation test employing various idling sounds of hybrid vehicles were carried out by Japanese and German participants. As a result, uneasiness feeling was found to be increased by the automatic engine start sound in both countries. In addition, upper limit of engine rotational speed and loudness of the engine sound were analyzed not to increase the uneasiness feeling in the cabin. The analytical results indicated the upper limits of the engine start sound were 1400 rpm in Japanese, and 900 rpm in German participants. In addition, 6 sone in Japanese and 4 sone in German participants were analyzed as the upper limit loudness. Consequently, German participants were found to be more sensitive to the engine automatic start sound than Japanese.
A Nomogram for Predicting the Risk of Lymph Node Metastasis in T1–2 Non-Small-cell Lung Cancer Based on PET/CT and Clinical Characteristics

Abstract:Background: Accurately predicting the risk level for a lymph node metastasis is critical in the treatment of non-small cell lung cancer (NSCLC). This study aimed to construct a novel nomogram to identify patients with a risk of lymph node metastasis in T1-2 NSCLC based on positron emission tomography/computed tomography (PET/CT) and clinical characteristics. Methods: From January 2011 to November 2017, the records of 318 consecutive patients who had undergone PET/CT examination within 30 days before surgical resection for clinical T1-2 NSCLC were retrospectively reviewed. A nomogram to predict the risk of lymph node metastasis was constructed. The model was confirmed using bootstrap resampling, and an independent validation cohort contained 156 patients from June 2017 to February 2020 at another institution. Results: Six factors [age, tumor location, histology, the lymph node maximum standardized uptake value (SUVmax), the tumor SUVmax and the carcinoembryonic antigen (CEA) value] were identified and entered into the nomogram. The nomogram developed based on the analysis showed robust discrimination, with an area under the receiver operating characteristic curve of 0.858 in the primary cohort and 0.749 in the validation cohort. The calibration curve for the probability of lymph node metastasis showed excellent concordance between the predicted and actual results. Decision curve analysis suggested that the nomogram was clinically useful. Conclusions: We set up and validated a novel and effective nomogram that can predict the risk of lymph node metastasis for individual patients with T1-2 NSCLC. This model may help clinicians to make treatment recommendations for individuals.

Abstract:Liver metastasis in colorectal cancer (CRC) is common and has an unfavorable prognosis. This study aimed to establish a functional nomogram model to predict overall survival (OS) and cancer-specific survival (CSS) in patients with colorectal cancer liver metastasis (CRCLM). A total of 9,736 patients with CRCLM from 2010 to 2016 were randomly assigned to training, internal validation, and external validation cohorts. Univariate and multivariate Cox analyses were performed to identify independent clinicopathologic predictive factors, and a nomogram was constructed to predict CSS and OS. Multivariate analysis demonstrated age, tumor location, differentiation, gender, TNM stage, chemotherapy, number of sampled lymph nodes, number of positive lymph nodes, tumor size, and metastatic surgery as independent predictors for CRCLM. A nomogram incorporating the 10 predictors was constructed. The nomogram showed favorable sensitivity at predicting 1-, 3-, and 5-year OS, with area under the receiver operating characteristic curve (AUROC) values of 0.816, 0.782, and 0.787 in the training cohort; 0.827, 0.769, and 0.774 in the internal validation cohort; and 0.819, 0.745, and 0.767 in the external validation cohort, respectively. For CSS, the values were 0.825, 0.771, and 0.772 in the training cohort; 0.828, 0.753, and 0.758 in the internal validation cohort; and 0.828, 0.737, and 0.772 in the external validation cohort, respectively. Calibration curves and ROC curves revealed that using our models to predict the OS and CSS would add more benefit than other single methods. In summary, the novel nomogram based on significant clinicopathological characteristics can be conveniently used to facilitate the postoperative individualized prediction of OS and CSS in CRCLM patients.

Abstract:Hazelnut is one of the most commonly consumed tree nuts, being largely used by the food industry in a wide variety of processed foods. However, it is a source of allergens capable of inducing mild to severe allergic reactions in sensitized individuals. Hence, the development of highly sensitive methodologies for hazelnut traceability is essential. In this work, we developed a novel technique for hazelnut detection based on a single-tube nested real-time PCR system. The system presents high specificity and sensitivity, enabling a relative limit of detection of 50 mg/kg of hazelnut in wheat material and an absolute limit of detection of 0.5 pg of hazelnut DNA (1 DNA copy). Its application to processed food samples was successfully achieved, detecting trace amounts of hazelnut in chocolate down to 60 mg/kg. These results highlight the adequacy of the technique for the specific detection and semiquantitation of hazelnut as potential hidden allergens in foods.
Survival Nomogram for Patients with Initially Diagnosed Metastatic Non-Small-cell Lung Cancer: a SEER-based Study

Abstract:Aim: Prognosis of patients with metastatic non-small-cell lung cancer differ widely. Methods: All patients were randomly divided into training or validation cohort. Cox-regression analyses were conducted to select independent predictors. We built a nomogram by R code and evaluated the accuracy and the reliability of the model using C-index, calibration curves and decision curve analyses. We made a risk classification system based on the nomogram. Results: In the validation cohort, C-index was 0.729 and 0.738 for 1- and 2-year overall survival. Calibration plots and decision curve analyses presented great prognostic accuracy and clinical applicability. Its prognostic accuracy preceded the American Joint Committee on Cancer staging with evaluated integrated discrimination improvement. Conclusion: The model can be a practical tool in treatment decision and individual counseling.

Abstract:Objective:We aim to develop and validate an effective nomogram prognostic model for patients with typical lung carcinoid tumors using a large patient cohort from the Surveillance, Epidemiology, and End Results (SEER) database.Materials and Methods:Data from patients with typical lung carcinoid tumors between 2010 and 2015 were selected from the SEER database for retrospective analysis. Univariate and multivariate Cox analysis was performed to clarify independent prognostic factors. Next, a nomogram was formulated to predict the probability of 3- and 5-year overall survival (OS). Concordance indexes (c-index), receiver operating characteristic analysis and calibration curves were used to evaluate the model.Results:The selected patients were randomly divided into a training and a validation cohort. A nomogram was established based on the training cohort. Cox analysis results indicated that age, sex, T stage, N stage, surgery, and bone metastasis were independent variables for OS. All these factors, except surgery, were included in the nomogram model for predicting 3- and 5-year OS. The internally and externally validatedc-indexes were 0.787 and 0.817, respectively. For the 3-year survival prediction, receiver operating characteristic analysis showed that the areas under the curve in the training and validation cohorts were 0.824 and 0.795, respectively. For the 5-year survival prediction, the area under the curve in the training and validation cohorts were 0.812 and 0.787, respectively. The calibration plots for probability of survival were in good agreement.Conclusion:The nomogram brings us closer to personalized medicine and the maximization of predictive accuracy in the prediction of OS in patients with typical lung carcinoid tumors.
Development and validation of a survival prediction model for patients with advanced non-small cell lung cancer based on LASSO regression

Abstract:Introduction: Lung cancer remains a significant global health burden, with non-small cell lung cancer (NSCLC) being the predominant subtype. Despite advancements in treatment, the prognosis for patients with advanced NSCLC remains unsatisfactory, underscoring the imperative for precise prognostic assessment models. This study aimed to develop and validate a survival prediction model specifically tailored for patients diagnosed with NSCLC. Methods: A total of 523 patients were randomly divided into a training dataset (n=313) and a validation dataset (n=210). We conducted initial variable selection using three analytical methods: univariate Cox regression, LASSO regression, and random survival forest (RSF) analysis. Multivariate Cox regression was then performed on the variables selected by each method to construct the final predictive models. The optimal model was selected based on the highest bootstrap C-index observed in the validation dataset. Additionally, the predictive performance of the model was evaluated using time-dependent receiver operating characteristic (Time-ROC) curves, calibration plots, and decision curve analysis (DCA). Results: The LASSO regression model, which included N stage, neutrophil-lymphocyte ratio (NLR), D-dimer, neuron-specific enolase (NSE), squamous cell carcinoma antigen (SCC), driver alterations, and first-line treatment, achieved a bootstrap C-index of 0.668 (95% CI: 0.626-0.722) in the validation dataset, the highest among the three models tested. The model demonstrated good discrimination in the validation dataset, with area under the ROC curve (AUC) values of 0.707 (95% CI: 0.633-0.781) for 1-year survival, 0.691 (95% CI: 0.616-0.765) for 2-year survival, and 0.696 (95% CI: 0.611-0.781) for 3-year survival predictions, respectively. Calibration plots indicated good agreement between predicted and observed survival probabilities. Decision curve analysis demonstrated that the model provides clinical benefit at a range of decision thresholds. Conclusion: The LASSO regression model exhibited robust performance in the validation dataset, predicting survival outcomes for patients with advanced NSCLC effectively. This model can assist clinicians in making more informed treatment decisions and provide a valuable tool for patient risk stratification and personalized management.

Abstract:Background: Breast cancer with liver metastasis consists of a group of heterogeneous diseases, and survival time may be significantly different, ranging from a few months to several years. The present study aimed to develop and externally validate a prognostic model for breast cancer with liver metastasis (BCLM).Methods: In total, 1022 eligible patients from January 2007 to December 2018 were selected from Fudan University Shanghai Cancer Center (FUSCC) and were temporally in the training (n = 715) and validation (n = 307) set. According to regression coefficients found in the multivariate Cox regression analysis, the final results were transformed into the prognostic scores. On the basis of these scores, patients were finally classified into three risk groups, including low-, intermediate-, and high-risk groups. Bootstrapping was used for internal validation. Then, time-dependent receiver operating characteristic (ROC) curves and calibration plots were used to assess discrimination and calibration of this prognostic model in the validation set.Results: Molecular subtypes, metastatic-free interval (MFI), extrahepatic metastasis, and liver function tests were identified as independent prognostic factors in the multivariate analysis. According to risk stratification, intermediate-risk (hazard ratio (HR) 2.12, 95% confidence interval (CI) 1.74–2.58, P < 0.001) and high-risk groups (HR 6.94, 95% CI 5.25–9.16, P < 0.001) had significantly worse prognoses in comparison with the low-risk group regarding overall survival (OS) from the time of metastasis. The median OS in these three groups were 39.97, 21.03, and 8.80 months, respectively. These results were confirmed in the internal and external validation cohorts.Conclusions: Based on molecular classification of tumors, routine laboratory tests, and other clinical information easily accessible in daily clinical practice, we developed a clinical tool for BCLM patients to predict their prognosis. Moreover, it may be useful for identifying the subgroup with unfavorable prognosis and individualization of treatment.

Development and Validation of a Novel Survival Prediction Model in Patients with Spinal Metastasis from Non-small Cell Lung Cancer.

Abstract:Study Design. Retrospective study. Objective. The aim of this study was to develop a model for predicting survival of patients with non-small cell lung cancer (NSCLC) spinal metastasis and compare its accuracy with the existing scoring systems. Summary of Background Data. Spinal metastasis is commonly seen in advanced NSCLC and usually associated with poor survival prognosis. Methods. A total of 376 patients with NSCLC spinal metastases admitted to our institution from January 2010 to October 2016 were enrolled. They were randomly assigned at a 3: 1 ratio to a training cohort (n = 282) and a validation cohort (n = 94). A nomogram for survival prediction was established by identifying and integrating significant prognostic factors, and then subjected to bootstrap validation in both training and validation cohorts. The discrimination was measured by concordance index (Cindex). Predictive accuracy was compared with three existing models by the receiver-operating characteristic curve (ROC) and area under ROC in both training and validation cohorts. Results. Six independent prognostic factors including sex (P = 0.008), the presence of visceral metastasis (P = 0.008), the number of metastases in the vertebral body (P = 0.011), Frankel score (P<0.001), D-dimer (P = 0.002), and sensitive epidermal growth factor receptor mutation (p<0.001) were identified and entered into the nomogram. The calibration curves for probability of 3-, 6-, 12-and, 24-month overall survival showed good agreement between the predictive risk and the actual risk. The C-index of the nomogram was 0.708 (95% confidence interval [CI], 0.674-0.742) in the training cohort and 0.683 (95% CI, 0.619-0.747) in the validation cohort. Model comparison showed that this nomogram had better predictive accuracy than the Tomita et al, Tokuhashi et al, and Schwab et al scoring systems (P<0.05 in the training cohort). Conclusion. We established and validated a novel nomogram that could be used to predict the survival outcome of patients with NSCLC spinal metastasis, thus helping clinicians in decision making and individualized care planning of such patients.
A Prognostic Nomogram of Colon Cancer With Liver Metastasis: A Study of the US SEER Database and a Chinese Cohort

Abstract:Background Among colon cancer patients, liver metastasis is a commonly deadly phenomenon, but there are few prognostic models for these patients. Methods The clinicopathologic data of colon cancer with liver metastasis (CCLM) patients were downloaded from the Surveillance, Epidemiology and End Results (SEER) database. All patients were randomly divided into training and internal validation sets based on the ratio of 7:3. A prognostic nomogram was established with Cox analysis in the training set, which was validated by two independent validation sets. Results A total of 5,700 CCLM patients were included. Age, race, tumor size, tumor site, histological type, grade, AJCC N status, carcinoembryonic antigen (CEA), lung metastasis, bone metastasis, surgery, and chemotherapy were independently associated with the overall survival (OS) of CCLM in the training set, which were used to establish a nomogram. The AUCs of 1-, 2- and 3-year were higher than or equal to 0.700 in the training, internal validation, and external validation sets, indicating the favorable effects of our nomogram. Besides, whether in overall or subgroup analysis, the risk score calculated by this nomogram can divide CCLM patients into high-, middle- and low-risk groups, which suggested that the nomogram can significantly determine patients with different prognosis and is suitable for different patients. Conclusion Higher age, the race of black, larger tumor size, higher grade, histological type of mucinous adenocarcinoma and signet ring cell carcinoma, higher N stage, RCC, lung metastasis, bone metastasis, without surgery, without chemotherapy, and elevated CEA were independently associated with poor prognosis of CCLM patients. A nomogram incorporating the above variables could accurately predict the prognosis of CCLM.

Abstract:Objective: The purpose of this study was to construct a nomogram model based on the general characteristics, histological features, pathological and immunohistochemical results, and inflammatory and nutritional indicators of patients so as to effectively predict the overall survival (OS) and progression-free survival (PFS) of patients with non-small cell lung cancer (NSCLC) after surgery. Methods: Patients with NSCLC who received surgical treatment in our hospital from January 2017 to June 2021 were selected as the study subjects. The predictors of OS and PFS were evaluated by univariate and multivariable Cox regression analysis using the Cox proportional risk model. Based on the results of multi-factor Cox proportional risk regression analysis, a nomogram model was established using the R survival package. The bootstrap method (repeated sampling for 1 000 times) was used to internally verify the nomogram model, and C-index was used to represent the prediction performance of the nomogram model. The calibration graph method was used to visually represent its prediction compliance, and decision curve analysis (DCA) was used to evaluate the application value of the model. Results: Univariate and multivariate analyses were used to identify independent prognostic factors and to construct a nomogram of postoperative survival and disease progression in operable NSCLC patients, with C-index values of 0.927 (907–0.947) and 0.944 (0.922–0.966), respectively. The results showed that the model had high predictive performance. Calibration curves for 1-year, 2-year, and 3-year OS and PFS show a high degree of agreement between the predicted probability and the actual observed probability. In addition, the results of the DCA curve show that the model has good clinical application value. Conclusion: We established a predictive model of survival prognosis and disease progression in patients with non-small cell lung cancer after surgery, which has good predictive performance and can guide clinicians to make the best clinical decision.

Abstract:This paper proposes an environment-dependent vehicle dynamic modeling approach considering interactions between the noisy control input of a dynamic model and the environment in order to make best use of domain knowledge. Based on this modeling, a new domain knowledge-aided moving horizon estimation (DMHE) method is proposed for ground moving target tracking. The proposed method incorporates different types of domain knowledge in the estimation process considering both environmental physical constraints and interaction behaviors between targets and the environment. Furthermore, in order to deal with a data association ambiguity problem of multiple-target tracking in a cluttered environment, the DMHE is combined with a multiple-hypothesis tracking structure. Numerical simulation results show that the proposed DMHE-based method and its extension could achieve better performance than traditional tracking methods which utilize no domain knowledge or simple physical constraint information only.

Abstract:Background The absolute count of lymphocyte subsets (ACLS) is correlated to the prognosis of multiple malignancies. This study aimed to combine the ACLS with the clinicopathological parameters to develop a nomogram to accurately predict the prognosis of non-small cell lung cancer (NSCLC) patients. Methods This retrospective study included a training cohort (n = 1685) and validation cohort (n = 337) with NSCLC patients treated in First Teaching Hospital of Tianjin University of Traditional Chinese Medicine between January 2018 and January 2021. Cox regression were conducted to identify factors associated with overall survival. The nomogram was built based on 10 significant factors, and evaluated by the concordance index (C-index), calibration curve and receiver operating characteristic (ROC) curve. Results In the training cohort, the multivariate cox proportional hazard regression analysis showed that the independent factors for overall survival (OS) included age, brain metastases, hepatic metastases, respiratory system diseases, clinical stages, surgery, absolute count (AC) of CD3 + , CD4 + , CD8 + , and NK cells, which were all applied in the nomogram. The C-index of the nomogram to predict OS was 0.777 (95% CI, 0.751–0.802) in training cohort and 0.822 (95% CI, 0.798–0.846) in validation cohort. The area under the ROC showed a good discriminative ability in both cohorts. Calibration curves presented an excellent consistence between the nomogram predicted probability and actual observation. Conclusions We established a prognostic nomogram to predict OS of the NSCLC patient. This nomogram provided a more quantitative, scientific and objective basis for accurate diagnosis and individual management of NSCLC patients.
