Abstract:Malignant ascites (MA) often occurs in recurrent abdominal malignant tumors, and the large amount of ascites associated with cancerous peritonitis not only leads to severe abdominal distension and breathing difficulties, but also reduces the patient's quality of life and ability to resist diseases, which usually makes it difficult to carry out anti-cancer treatment. The exploration of MA treatment methods is also a key link in MA treatment. This article is going to review the treatment of MA, to provide details for further research on the treatment of MA, and to provide some guidance for the clinical treatment of MA.

Abstract:Malignant ascites is the accumulation of fluid in the peritoneum as a result of advanced cancer and often signifies the terminal phase of the disease. Management of malignant ascites remains a clinical challenge as symptom palliation is the current standard of cure. Previously, studies examining malignant ascites largely focused on ovarian and gastric cancer. In recent years, there has been a significant increase in research on malignant ascites in pancreatic cancer. Malignant ascites is usually diagnosed based on positive cytology, but cytology is not always diagnostic, indicating the need for novel diagnostic tools and biomarkers. This review aims to summarize the current understanding of malignant ascites in pancreatic cancer and the recent advances in the molecular characterization of malignant ascites fluid from patients with pancreatic cancer including analysis of soluble molecules and extracellular vesicles. Current standard of care treatment options such as paracenteses and diuretics are outlined along with new emerging treatment strategies such as immunotherapy and small-molecule based therapies. New potential investigative directions resulting from these studies are also highlighted.

Abstract:Malignant pleural effusion (MPE) is a common complication in the late stage of malignant tumors. The appearance of MPE indicates that the primary tumor has spread to the pleura or progressed to an advanced stage. The survival time of the patients will be significantly shortened, with a median survival of only a few months. There are a variety of traditional treatments, and their advantages and disadvantages are relatively clear. There are still many problems that cannot be solved by traditional methods in clinical work. The most common one is intrapleural perfusion therapy with chemotherapy drugs, but it has a large side effect of chemotherapy. At present, with the development of medical technology, there are a variety of treatment methods, and many innovative, significant and valuable treatment methods have emerged, which also bring hope for the treatment of refractory and recurrent MPE patients. Several clinical trials had confirmed that drug-carrying microparticles has less adverse reactions and obvious curative effect. However, there is still a long way to go to completely control and cure MPE, and the organic combination of clinical work and scientific research results is needed to bring dawn to refractory MPE patients.

The clinical observation of abdominal endostar plus chemotherapy and local hyperthermia treatment of malignant ascites

Abstract:Objective: To observe the efficacy and safety of effusion infusion Endostar combined with the chemotherapy and hyperthermia on advanced cancer with malignant ascites.Methods: 53 advanced cancer patients with malignant ascites confirmed by histopathology or cytopathology.All the patients were divided into A and B groups.Both groups were been given chemotherapy DDP 40～60mg and hyperthermia.Otherwise,A group combined with Endostar 60mg D1,4.All patients were given hyperthermia twice a week D1,4.The regimen was repeated every 7 days.The efficacy was evaluated after 2 cycles according to WHO criteria,and quality of life(QOL) was evaluated according to karnofsky scores.Results:53 cases were capable of safety evaluation.The effective rate of A group was 76%,while 57.1% of the B group.The quality of life(QOL)of A group was improved on 18 cases(72%),while 17 cases of the B group(60.7%).All the patients were of good tolerance as a whole and no severe side effects happened.Conclusion: Intraperitoneal Endostatin combined with DDP and local hyperthermia results in a good response rate and improvement of QOL for the patients with malignant ascites.The combined regimen was low toxic and tolerable.The regimen is worthy of further clinical observation.

Abstract:Abstract In recent years, advances have been made for treating ascites in patients with cirrhosis. Recent studies have indicated that several treatments that have been used for a long time in the management of portal hypertension may have beneficial effects that were not previously identified. Long‐term albumin infusion may improve survival in patients with cirrhosis and ascites while beta‐blockers may reduce ascites occurrence. Transjugular intrahepatic porto‐systemic shunt (TIPS) placement may also improve survival in selected patients in addition to the control with ascites. Low‐flow ascites pump insertion can be another option for some patients with intractable ascites. In this review, we summarize the latest data related to the management of ascites occurring in cirrhosis. There are still unanswered questions, such as the optimal use of albumin as a long‐term therapy, the place of beta‐blockers, and the best timing for TIPS placement to improve the natural history of ascites, as well as the optimal stent diameter to reduce the risk of shunt‐related side‐effects. These issued should be addressed in future studies.

Abstract:In recent years, great progress has been made in the external treatment of traditional Chinese medicine (TCM) for cirrhotic ascites, including TCM umbilical compress therapy, TCM enema, acupuncture, moxibustion, and light therapy. The external treatment has similar effects as oral administration of TCM in relieving the clinical symptoms of cirrhotic ascites, and does not have gastrointestinal adverse reactions. This article reviews the research advances in external treatment of TCM for cirrhotic ascites, and the new research directions in the future are to establish an unified research protocol and expand the scope of application of external treatment of TCM.

Abstract:Liver cirrhosis is a health problem worldwide, and ascites is its principal symptom. Refractory ascites is intractable and occurs in 5%–10% of all patients with ascites due to cirrhosis. Refractory ascites leads to a poor quality of life and high mortality rate. Ascites develops as a result of portal hypertension, which leads to water–sodium retention and renal failure. Various therapeutic measures can be used for refractory ascites, including large-volume paracentesis, transjugular intrahepatic portosystemic shunt, vasoconstrictive drugs, and an automated low-flow ascites pump system. However, ascites generally can be resolved only by liver transplantation. Because not all patients can undergo liver transplantation, traditional approaches are still used to treat refractory ascites. The choice of treatment modality for refractory ascites depends, among other factors, on the condition of the patient.

Abstract:The emergence of malignant ascites (MA) indicates poor prognoses in patients with ovarian, gastrointestinal, breast, and pancreatic cancer. Interleukin-10 (IL-10) is a pleiotropic cytokine with immunoregulatory effects in tumor microenvironment. The level of IL-10 in MA varied across cancer types and patients, influencing cancer progression and outcomes. Originating from various immune and cancer cells, IL-10 contributes to complex signaling pathways in MA. Systemic IL-10 administration, although the evidence of its efficacy on MA is limited, still emerges as a promising therapeutic strategy because it can increase CD8+ T cells cytotoxicity and invigorate exhausted CD8+ tumor infiltration lymphocytes (TILs) directly. IL-10 signaling blockade also demonstrates great potential when combined with other immunotherapies in MA treatment. We reviewed the levels, origins, and functions of IL-10 in malignant ascites and overviewed the current IL-10 signaling targeting therapies, aiming to provide insights for MA treatment.

Abstract:OBJECTIVE:To summarize the present literature about the therapy of advanced gastric cancer.METHODS:The periodic full-text databases of both Medline and CBM disc from January 1991 to September 2009 in the internet were used to retrieve chapters of the corresponding materials.The index words were composed of stomach neoplasms and drug therapy.A total of 1809 aricles were obtained from PubMed and 131 from CBMdisc.The adoptive standards were as the following:1)the chemotherapy of advanced gastric cancer.2)the targeted treatment of advanced gastric cancer.According to the criteria,81 papers were selected and finally 34 were included.RESULTS:The treatment of advanced gastric cancer is essentially palliative.Most gastric cancer responses to chemotherapy are partial and of short duration.There is no standard treatment.Several targeted drugs are currently undergoing clinical trials.CONCLUSION:The present study of the medical treatment of advanced gastric cancer focuses on the following aspects:1)to improve the reqimens of combination chemotherapy;2)to keep on evaluating the effective reqimens nowadays.3)to study the application of the new chemotherapeutics and targeted drugs.

Abstract:Ascites is one of the most common complications of liver cirrhosis associated with a poor prognosis. The first line treatment of ascites requires dietary sodium restriction and the judicious use of di-uretics, such as spironolactone and furosemide. Patients with refractory ascites require second-line treat-ments of repeat large-volume paracentesis (LVP) or the insertion of a transjugular intrahepatic portosystemic shunt (TIPS), and assessment for liver transplantation. The use of albumin as a volume expander is recom-mended for LVP of >5 L to prevent the development of circulatory dysfunction, although the clinical signif-icance of post-paracentesis circulatory dysfunction is still debated. Potential new treatment options for as-cites include the use of vasopressin V2 receptor antagonists and vasoconstrictors, which could possibly im-prove the management of ascites. Clinical pharmacists can optimize treatment plans with doctors and imple-ment pharmaceutical care for patients to promote rational use of drugs.

Abstract:294 Background: Gastric cancer patients with malignant ascites often have poor functional status and malnutrition that precludes receipt of systemic therapies. Thus, these patients have a very poor prognosis. Beginning in 2019, our multidisciplinary gastric cancer disease-oriented team implemented a more aggressive supportive care plan for gastric cancer patients with malignant ascites. The initiative included measures such as supplemental enteral nutrition, ascites drainage, and initiation of chemotherapy on an inpatient basis. We compared outcomes for gastric cancer patients who presented with synchronous malignant ascites treated before and after the implementation of the care plan. Methods: We performed a retrospective review of our institutional database to identify patients diagnosed with gastric adenocarcinoma and synchronous malignant ascites between 2010-2022. We compared survival between patients diagnosed from 2010-2018, which will be referred to as the historical control era and patients diagnosed from 2019-2022, which will be called the aggressive supportive care era. Results: 54 patients were included in our analysis; 31 patients were treated in the historical control time frame and 23 patients treated during the aggressive supportive care era. Demographic, clinical, and pathologic characteristics were similar between groups. 3% of historical controls received supplemental tube feeds at diagnosis as compared to 30% of the aggressive supportive care cohort (p=0.008). 3% of historical controls received their first cycle of chemotherapy in the inpatient setting versus 39% of patients treated during the aggressive supportive care era (p=0.001). The median number of chemotherapy cycles received was 5 among historical controls and 9.5 among aggressive supportive care era patients (p = 0.02). There was no difference in the number of days spent as an inpatient between the two groups. The median overall survival for historical control patients was 5.4 months as compared to 10.4 months for patients treated during aggressive supportive care era (p=0.035). Conclusions: Gastric cancer patients with synchronous malignant ascites treated during a timeframe when our multidisciplinary team implemented more aggressive supportive care measures had improved OS as compared to historic controls.Our results suggest that aggressive supportive measures for these patients with highly challenging clinical issues and poor prognosis can prolong survival. Specifically, initiation of chemotherapy in the inpatient setting and supplemental nutrition should be considered for patients at high risk for treatment intolerance.

Abstract:Pancreatic cancer is a common malignancy of the digestive system. With a high degree of malignancy and poor prognosis, it is called the "king of cancers." Currently, Western medicine treats pancreatic cancer mainly by surgical resection, radiotherapy, and chemotherapy. However, the curative effect is not satisfactory. The application of Traditional Chinese Medicine (TCM) in the treatment of pancreatic cancer has many advantages and is becoming an important facet of comprehensive clinical treatment. In this paper, we review current therapeutic approaches for pancreatic cancer. We also review the protective effects shown by TCM in different models and discuss the potential molecular mechanisms of these.

Abstract:BACKGROUND/AIM: Malignant ascites is a common condition in patients with terminal cancer. Treatments, such as diuretics, percutaneous drainage of ascites, and abdominal vein shunting have been advocated. However, these treatments have not achieved sufficient palliative effects. Therefore, the development of innovative therapies is mandated, especially for new therapies that require the creation of a fluid simulation of malignant ascites. However, there have been no previous studies on the physical properties of malignant ascites, including viscosity, which are necessary for the development of such a fluid. Therefore, we prospectively investigated the physical properties of malignant ascites.PATIENTS AND METHODS: This single-center, prospective, observational study included 30 patients between November 2021 and January 2023. The primary endpoint was the viscosity of the malignant ascites, and the secondary endpoints included other viscosity studies, biochemical tests, and the presence of malignant cells in the ascites.RESULTS: The median viscosity was 1.105 mPa*S. The viscosity of malignant ascites tended to decrease with increasing temperature, which is common for liquids. Malignant ascites fluid containing malignant cells tended to be more viscous than ascites fluid without malignant cells; furthermore, albumin levels tended to be higher in the former than in the latter.CONCLUSION: Malignant ascites' median viscosity was 1.105 mPa*S. Correlation between viscosity and temperature showed a decreasing trend. These findings contribute valuable insights for future malignant ascites management and device development.

Abstract:Objective To study the clinical effect of compound matrine injection combined with homemade xiaogu scattered in the treat-ment of malignant ascites. Methods 90 cases of patients with Qi stagnation,damp type malignant ascites in TCM oncology wards of Qingpu Branch of Zhongshan Hospital from January 2014 to January 2016 were selected,these patients were divided into xiaogu powder group(n=30), combined treatment group( Compound Sophora Injection joint Xiaoguruanjian scattered,n =30 ),control group( Western diuretics,n =30 ). They were given corresponding drug treatment,respectively. The clinical efficacy,quality of life and adverse events of the three groups were statis-tically analyzed. Results The total treatment efficiency of the combined treatment group 93. 3%(28/30),xiaogu powder group,73. 3%(22/30)were significantly higher than that of the control group 50. 0%(15/30)( P ﹤0. 05),while the total treatment efficiency of the combined treatment group was higher than that of the xiaogu powder group( P ﹤0. 05). The combination of traditional Chinese medicine treatment group, Xiaogu San groupˊs life quality improvement rate were significantly higher than that of the control group( P ﹤0. 05). The KPS scores were signifi-cantly higher than that of the control group( P ﹤0. 05),the stable rate and reduction rate were significantly lower than those of the control group ( P ﹤0. 05),and the quality of life of traditional Chinese medicine combined with the improvement rate in the treatment group was significantly higher than that of the Xiaogu powder group( P ﹤0. 05). The stable rate and the decrease rate were significantly lower than the Xiaogu powder group( P ﹤0. 05). The KPS score was significantly higher than that of the Xiaogu powder group( P ﹤0. 05). The combination of traditional Chinese medicine treatment group,Xiaogu San group of patients with adverse reaction rate(13. 3%,10%)were significantly lower than that of the control group(33. 3%),and the difference of adverse reaction between the combination of traditional Chinese medicine treatment group,Xi-aogu San group was not significant( P ﹥0. 05). Conclusion The clinical effect of compound matrine injection combined with homemade xiaogu scattered in the treatment of Qi stagnation,damp type malignant ascites is safe and effective. It can significantly improve the life quality of pa-tients.
Effect of domestic paclitaxel combined with external high frequency thermotherapy on malignant peritoneal effusion

Abstract:Objective To explore the peritoneal infusion chemotherapy of domestic paclitaxel complex external high frequency thermotherapy treating advanced malignant ascites.Methods All 37 malignant abdominal product liquid patients from April 2010 to May 2013 were collected and were divided into observation group(18 cases) and control group(19 cases).The patients in observation group were treated with intraperitoneal perfusion (yew alcohol 60 mg/m2 +0.9％ temperature chloride sodium solution 1 500-2 000 ml),1 times/week,hot pedicures 3 times/week,41-43 ℃/times,1 h/time.The patients in control group were treated with intraperitoneal perfusion (cisplatin 40 mg/m2 + 0.9％ temperature chloride sodium solution 1 500-2 000 ml),heat therapy 3 times/week,41-43 ℃/time,1 h/time,The cycle period was two weeks,and the effect was evaluated every two weeks.The course of treatment was 4 weeks.Results The total effective rate in observation group was higher than that in control group [66.7％ (12/18) vs 36.8％ (7/19),P ＜ 0.05].The scores of KPS in observation before treatment and after treatment were (60 ± 5),(80 ± 5) scores,while in the control group they were (60 ± 5),(70 ± 5)scores,respectively; there were significant differences after treatment in two groups (P ＜ 0.05).The incidence of adverse reaction in observation group was less than that in control group[66.7％ (12/18) vs 94.7％(18/19)] (P ＜ 0.05).Conclusion The peritoneal infusion chemotherapy of domestic paclitaxel complex external high frequency thermotherapy in the treatment of malignant ascites is relatively safe and effective ; it can significantly improve the quality of life.

Abstract:Traditional Chinese medicine (TCM), especially Chinese herbal medicines and acupuncture, has been traditionally used to treat patients with cancers in China and other East Asian countries. Numerous studies have indicated that TCM not only alleviates the symptoms (e.g., fatigue, chronic pain, anorexia/cachexia, and insomnia) of patients with cancer and improves their quality of life (QOL) but also diminishes adverse reactions and complications caused by chemotherapy, radiotherapy, or targeted-therapy. Therefore, Chinese herbal medicines and acupuncture and other alternative therapies need to be understood by TCM physicians and other health care providers. This review mainly summarizes the experimental results and conclusions from literature published since 2010, and a search of the literature as been performed in the PubMed, MEDLINE, Web of Science, Scopus, Springer, ScienceDirect, and China Hospital Knowledge Database (CHKD) databases. Some Chinese herbal medicines (e.g., Panax ginseng, Panax quinquefolius, Astragali radix, Bu-zhong-yi-qi-tang (TJ-41), Liu-jun-zi-tang (TJ-43), Shi-quan-da-bu-tang (TJ-48), and Ban-xia-xie-xin-tang (TJ-14)) and some acupuncture points (e.g., Zusanli (ST36), Zhongwan (CV12), Neiguan (PC6) and Baihui (GV20)) that are commonly used to treat cancer-related symptoms and/or to reduce the toxicity of chemotherapy, radiotherapy, or targeted-therapy are highlighted and summarized. Through a review of literature, we conclude that TCM can effectively alleviate adverse gastrointestinal reactions (including diarrhea, nausea, and vomiting) to these anti-cancer therapies, decrease the incidence of bone marrow suppression, alleviate cardiotoxicity, and protect against chemotherapy-induced peripheral neuropathy and radiation-induced pneumonitis. Moreover, TCM can alleviate epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI)-related acneiform eruptions, diarrhea, and other adverse reactions. The hope is that this review can contribute to an understanding of TCM as an adjuvant therapy for cancer and that it can provide useful information for the development of more effective anti-cancer therapies. However, more rigorously designed trials involving cancer treatment must be conducted in the future, including complete quality control and standardized models at the cellular, organic, animal and clinical levels, in order to study TCM in multiple forms and at multiple levels.

Abstract:Malignant tumors are one of the leading causes of death in the world. Considerable progresses have been made on the treatment of tumors in recent decades, especially in the prevention, early diagnosis and the model changing of therapeutics. But we are still facing tough challenges, including the increasing treatment burden and limited improvement of efficacy. In China, Chinese medicine (CM) provides a powerful arsenal to fight against tumors. CM can be well applied to the onset and progression of tumors in China, bearing the characteristics of multi-target, multi-phase and multi-effect. But there are also many problems demanding urgent attention in the use of CM. Some most debated problems in this field were summarized. We should upgrade our concepts in using CM, find its position scientifically, and establish evidence of its effect by high quality clinical research.

Abstract:Among the various risky complications of liver cirrhosis, refractory ascites is associated with poor survival of cirrhotics and persistently worsens their quality of life (QOL). Major clinical guidelines worldwide define refractory ascites as ascites that cannot be managed by medical therapy either because of a lack of response to maximum doses of diuretics or because patients develop complications related to diuretic therapy that preclude the use of an effective dose of diuretics. Due to the difficulty in receiving a liver transplantation (LT), the ultimate solution for refractory ascites, most cirrhotic patients have selected the palliative therapy such as repeated serial paracentesis, transjugular intrahepatic portosystemic shunt, or peritoneovenous shunt to improve their QOL. During the past several decades, new interventions and methodologies, such as indwelling peritoneal catheter, peritoneal-urinary drainage, and cell-free and concentrated ascites reinfusion therapy, have been introduced. In addition, new medical treatments with vasoconstrictors or vasopressin V2 receptor antagonists have been proposed. Both the benefits and risks of these old and new modalities have been extensively studied in relation to the pathophysiological changes in ascites formation. Although the best solution for refractory ascites is to eliminate hepatic failure either by LT or by causal treatment, the selection of the best palliative therapy for individual patients is of utmost importance, aiming at achieving the longest possible, comfortable life. This review briefly summarizes the changing landscape of variable treatment modalities for cirrhotic patients with refractory ascites, aiming at clarifying their possibilities and limitations. Evolving issues with regard to the impact of gut-derived systemic and local infection on the clinical course of cirrhotic patients have paved the way for the development of a new gut microbiome-based therapeutics. Thus, it should be further investigated whether the early therapeutic approach to gut dysbiosis provides a better solution for the management of cirrhotic ascites.
Stimuli-responsive Hydrogel Microspheres Encapsulated with Tumor-Cell-derived Microparticles for Malignant Ascites Treatment

Abstract:Tumor-cell-derived microparticles (TMPs) have been recognized as chemotherapeutic drug carriers and immunomodulators for anti-tumor therapy. Research in the clinical application of TMPs has been devoted to developing an effective delivery formulation that could enhance their therapeutic effects. Here, we propose thermal-responsive agarose hydrogel microspheres (MTX-TMPs@MSs) with encapsulation of Methotrexate (MTX)-packaging TMPs (MTX-TMPs) and black phosphorus quantum dots (BPQDs) by microfluidic technology for synergistic treatment of malignant ascites. The laden MTX-TMPs, separated from apoptotic tumor cells, could target tumor cells for the delivery of chemotherapy drugs and modulate the tumor immune microenvironment. Under near-infrared (NIR) induced thermal stimulation, MTX-TMPs could be controllably released from the low-melting-point agarose matrix hydrogel microspheres for chemotherapy (CHT) and immunotherapy (IMT). In addition, benefiting from photothermal therapy (PTT)-induced tumor immunogenic death, the anti-tumor immune response triggered by MTX-TMPs was further enhanced. Based on these features, the MTX-TMPs@MSs could remarkably eliminate tumor cells in vitro and obviously suppress tumor growth in vivo through synergistic PTT, CHT, and IMT. Therefore, it is envisaged that this TMPs-integrated microcarrier will have promising applications in clinical tumor therapy. Statement of Significance Primary liver cancer ranks third among the causes of cancer deaths globally, with hepatocellular carcinoma (HCC) being the most common type. In particular, patients with advanced HCC accompanied by malignant ascites, a common complication, indicate tumor metastasis and a poor prognosis. In this paper, we developed stimuli-responsive hydrogel microspheres from microfluidics for the delivery of methotrexate (MTX)-loaded tumor-cell-derived microparticles (MTX-TMPs) for synergistic chemotherapy, photothermal therapy, and immunotherapy. The release of MTX-TMPs from hydrogel microspheres could be on-demand controlled through BPQDs-mediated photothermal stimulus. On the other hand, BPQDs-mediated mild hyperthermia cooperatesss with MTX-TMPs-induced chemotherapy could participate in remodeling the tumor immunosuppressive microenvironment. Thus, the prepared microcarrier system holds great promise for tumor therapy.
