Abstract:Racial disparities arise across many vital areas of American life, including employment, health, and interpersonal treatment. For example, one in three Black children lives in poverty (versus one in nine white children), and, on average, Black Americans live four fewer years compared with white Americans. Which disparity is more likely to spark reduction efforts? We find that highlighting disparities in health-related (versus economic) outcomes spurs greater social media engagement and support for disparity-mitigating policy. Further, reading about racial health disparities elicits greater support for action (e.g., protesting) compared with economic- or belonging-based disparities. This occurs in part because people view health disparities as violating morally sacred values, which enhances perceived injustice. This work elucidates which manifestations of racial inequality are most likely to prompt Americans to action.
Increasing Police Presence: Examining Race, Ethnicity, and Perceived Neighborhood Disadvantage as Correlates of Support

Abstract:Given its empirical and public support, increasing police presence seems to be a viable preventive policy option for police agencies. However, the theoretical and empirical literature is not clear on whether this support would vary along race/ethnicity and perceived neighborhood disadvantage, which is relevant given that policies of this kind would likely have a greater impact on disadvantaged communities of color. Using survey data from a national sample, we found that racial and ethnic identity were unrelated to support for increased police presence, while perceived neighborhood disorder was related to greater support for police presence. We also found that neighborhood disorder, social cohesion, and informal social control were indirectly related to support for increased police presence through perceptions of police legitimacy and fear of crime, demonstrating some evidence of the overpolicing-underpolicing paradox in disadvantaged communities. Policy implications of these findings are discussed.

Abstract:Many scholars and activists assume the public would be motivated to fight inequality if only they knew the full extent of existing disparities. Ironically, exposure to extreme disparities can cause people to become more, not less, supportive of the very policies that create those disparities (Hetey & Eberhardt, 2014). Here, we focus on the criminal justice system—policing and incarceration in particular. We argue that bringing to mind racial disparities in this domain can trigger fear and stereotypic associations linking Blacks with crime. Therefore, rather than extending an invitation to reexamine the criminal justice system, the statistics about disparities may instead provide an opportunity to justify and rationalize the disparities found within that system. With the goals of spurring future research and mitigating this paradoxical and unintended effect, we propose three potential strategies for more effectively presenting information about racial disparities: (a) offer context, (b) challenge associations, and (c) highlight institutions.

Abstract:Population-level health outcomes and measures of well-being are often described relative to broad racial/ethnic categories such as White or Caucasian; Black or African American; Latino or Hispanic; Asian American; Native Hawaiian and Pacific Islander; or American Indian and Alaska Native. However, the aggregation of data into these groups masks critical within-group differences and disparities, limiting the health and social services fields' abilities to target their resources where most needed. While researchers and policymakers have recognized the importance of disaggregating racial/ethnic data—and many organizations have advocated for it over the years—progress has been slow and disparate. The ongoing lack of racial/ethnic data disaggregation perpetuates existing inequities in access to much-needed resources that can ensure health and well-being. In its efforts to help build a Culture of Health and promote health equity, the Robert Wood Johnson Foundation has supported activities aimed to advance the meaningful disaggregation of racial/ethnic data—at the collection, analysis, and reporting phases. This special issue presents further evidence for the importance of disaggregation, the technical and policy challenges to creating change in practice, and the implications of improving the use of race and ethnicity data to identify and address gaps in health.

Abstract:Routen and colleagues discuss the need for intersectional approaches in health equity research.1 We identify three additional areas that could help advance health disparities research, integrate findings across studies, and inform clinical relevance.First, using consistent terminology would be helpful. Governing and funding bodies have guidelines to assist with the identification of common terminology. The US National Institutes of Health, for example, require investigators to report ethnicity and race as a combined term, consistent with an intersectional approach.234 Similarly, in the United Kingdom, data are reported in a blended classification format of ethnic and heritage backgrounds.56Second, numerous frameworks and models are available to provide a common conceptual infrastructure for integrating and interpreting findings. Race and healthcare disparities: overcoming vulnerablity

Abstract:The paper summarizes recently published data and recommendations about healthcare disparities experienced by African Americans who have Medicare or other healthcare coverage. Against this background the paper addresses the ethics of such disparities and how disadvantages of vulnerable populations like African Americans are typically maintained in decision making about how to respond to such disparities. Considering how to respond to disparities reveals much that vulnerable populations would bring to the policy-making table, if they can also be heard when they get there. The paper argues that vulnerable populations like African Americans need fair representation in bodies deciding what to do about such disparities and that fairness requires proportional representation at all levels of decisions that affect healthcare--a radical change. In this decision setting, how to provide adequate protection of minorities needs much further attention. The most attractive decision-making model is deliberative democracy. The paper shows that in deliberation, fair representation requires not only having a voice in decisions, but a fair hearing of those voices. Achieving a fair hearing requires changes in norms of communication and training of all to give importance to greetings and other measures of civility and trust building, and to be open to diverse forms of expression. Decisions about how to respond to healthcare disparities would include what programs to initiate for whom, how to evaluate the programs, and what to do in response to such evaluations. Conclusions are that achieving such goals will take a sea change in how healthcare institutions and providers do their business, and that social activism at every level will be needed to effect these changes. The discussion highlights many ethical issues that need much greater attention.

Abstract:Awareness of racial health care inequities is one prerequisite to eliminating them. Although extant research has described awareness of racial health care inequities in the United States, the health impacts of such awareness on communities that are most impacted by these inequities remains unknown. Therefore, we examined associations between awareness of Black-White racial health care inequities and self-rated health for Black and White adults in the United States. We used survey data from non-Hispanic Black and White participants (<i>N</i> = 6,449) who responded to the national American Health Values Survey (2015-2016) to test associations between awareness of Black-White inequities in health care and self-rated health. Accurate awareness of health care inequities was associated with 47% higher odds of poorer self-rated health for Black individuals. Inaccurate awareness was associated with 36% higher odds of poorer self-rated health for White individuals. Accurate awareness may be adaptive, yet place an additional burden on Black individuals. Inaccurate awareness may harm White individuals' health. Health care system changes and alleviation of racism-related stress may be preventive supports for the health of Black individuals. Accurate awareness should be a goal for White individuals, not only to prevent health risks, but to also facilitate structural change for racial equity. Abstract:This study examines whether a key psychosocial factor—perceiving racial discrimination in healthcare—is associated with worse patient activation, communication self-efficacy, and physical health outcomes for Black veterans with chronic pain. Moreover, we explore the role of physician-patient working alliance as a moderator that may alleviate the potential consequences of perceiving racial discrimination. This work is a secondary analysis of baseline data from a clinical trial with 250 U.S. Black veterans with chronic musculoskeletal pain. Participants were recruited from primary care clinics at a Midwestern VA hospital between 2018-2021. Perceiving racial discrimination in healthcare was associated with lower patient activation, lower self-efficacy in communicating with one's physician, higher pain intensity, and lower pain management self-efficacy ( p s<.049), but was unrelated to reports of pain interference or use of pain coping strategies ( p s>.157). Although the relationship between perceived discrimination and patient activation was moderated by working alliance ( p =.014), having a stronger working alliance improved patient activation to varying degrees across levels of perceived discrimination (rather than buffering against negative outcomes when perceiving higher levels of discrimination). Moderation was not significant on any other measures. This study deepens our understanding of the broad range of health outcomes that are (not) associated with perceiving racial discrimination in healthcare. Contrary to prior theorizing, this work also indicates that having a strong working alliance does not attenuate the consequences of perceiving discrimination among Black individuals living with pain. These results highlight the need for system-level interventions to address perceptions of racial mistreatment in healthcare. Perspective This work has important public health implications by identifying the broad range of outcomes associated with perceived discrimination in healthcare among Black Americans. Importantly, having a strong physician-patient relationship did not buffer Black individuals from the consequences of perceiving discrimination. These findings inform intervention targets to mitigate racial health disparities.

Abstract:Summary Societal systems act individually and in combination to create and perpetuate structural racism through both policies and practices at the local, state, and federal levels, which, in turn, generate racial and ethnic health disparities. Both current and historical policy approaches across multiple sectors—including housing, employment, health insurance, immigration, and criminal legal—have the potential to affect child health equity. Such policies must be considered with a focus on structural racism to understand which have the potential to eliminate or at least attenuate disparities. Policy efforts that do not directly address structural racism will not achieve equity and instead worsen gaps and existing disparities in access and quality—thereby continuing to perpetuate a two-tier system dictated by racism. In Paper 2 of this Series, we build on Paper 1's summary of existing disparities in health-care delivery and highlight policies within multiple sectors that can be modified and supported to improve health equity, and, in so doing, improve the health of racially and ethnically minoritised children.

Championing racial equity within mental health systems: Perceptions and experiences among state government organization staff leading the charge.

Abstract:The COVID-19 pandemic and police brutality, especially the murder of George Floyd, have intensified the focus on racial equity within a diverse array of organizations. While a variety of strategies are employed in the pursuit of racial equity in mental health care, there is a notable gap in recognizing the crucial role of individual staff driving change within organizations. Few studies have examined these individuals and the ways they think about their work and its impact. Here, we examine the ways that state health and mental health authority (SH/MHA) staff understand racial equity in mental health care when faced with unusual support for equity reform following the Summer of 2020. Through in-depth qualitative interviews with 58 individuals working in SH/MHAs of 31 states across the United States, we examine the perspectives and experiences of staff engaging in racial equity work when they sense that sociohistoric events perhaps temporarily lifted constraints. Findings confirm the importance of people in the racial equity work process and show that study participants have the characteristics of champions with (a) a conceptualization of (in)equity that involves White systems, including their own organizations, and the need for persistent, sustainable work, (b) an emotional investment toward racial equity, and (c) autonomous motivations of morality and empathy. This study underscores the profound transformative capacity of champions within organizations. Gaining insight into their perceptions and experiences offers a deep understanding of the qualities of a champion and equips us to bolster their pursuits toward racial equity within mental health systems. Promoting Racial/Ethnic Identity in Child and Adolescent Mental Health Treatments

Abstract:Over the last decade, state and city legislative efforts have underscored that racism is a public health crisis. These legislative shifts have been in concert with several professional medical organizations, including the National Academy of Medicine, the United States Department of Health and Human Services, the Centers for Disease Control, and the National Institute of Health, which have collectively called for structural change to address race inequity in health, from research to patient care. The adverse effects of racism (eg, interpersonal, structural, institutional, and internalized) on health have been documented to include negative effects across the lifespan and developmental continuum, particularly for ethnoracially minoritized youth. Indeed, several studies have specifically identified racism's impact on youths' psychosocial functioning and emotional well-being, particularly around anxiety, depression, and academic functioning. The effects of interpersonal racism on mental health in adolescents and, in particular, Black youth is telling. Although the child and adolescent mental health establishment and literature have advocated for strengths-based (eg, cultural assets) and community engaged (eg, community-based participatory research) frameworks to advance evidence-based treatments for diverse communities, the development of culturally responsive and antiracist interventions remain a gap in our treatment armamentarium for ethnoracially minoritized youth. As in other papers, we have highlighted the criticality of health equity, cultural humility, and culturally informed and responsive clinical practices. We have also underscored that, as a field, child mental health practitioners need to move toward being antiracist to truly address well-being, a shift that requires pivoting toward approaches that promote racial/ethnic identity (REI), which includes racial/ethnic connectedness and racial/ethnic pride. Race-conscious interventions, particularly those that focus on racial/ethnic connectedness and racial/ethnic pride, can not only be protective and health promoting in their ability to mitigate the emotional harms of experiencing racism, but can also foster social-emotional functioning and successful academic outcomes among ethnoracially minoritized individuals.

Abstract:Environmental health research in the US has shown that racial and ethnic minorities and members of low-socioeconomic groups, are disproportionately burdened by harmful environmental exposures, in their homes, workplace, and neighborhood environments that impact their overall health and well-being. Systemic racism is a fundamental cause of these disproportionate exposures and associated health effects. To invigorate and inform current efforts on environmental justice and to raise awareness of environmental racism, the National Institute of Environmental Health Sciences (NIEHS) hosted a workshop where community leaders, academic researchers, and NIEHS staff shared perspectives and discussed ways to inform future work to address health disparities.

Abstract:Policy Points In a recently commissioned report on solutions for eliminating racial and ethnic health care inequities entitled Ending Unequal Treatment, the National Academies of Sciences, Engineering, and Medicine found a health workforce that is representative of the communities it serves is essential for health care equity. The Supreme Court decision to ban race‐conscious admission constraints pathways toward health workforce representativeness and equity. This paper draws on the National Academies report's findings that health care workforce representativeness improves care quality, population health, and equity to discuss policy and programmatic options for various participants to promote health workforce representativeness in the context of race‐conscious admissions bans.

Abstract:The Department of Health and Human Services (HHS) recently unveiled the most comprehensive federal commitment yet to reducing racial and ethnic health disparities. The 2011 HHS Action Plan to Reduce Racial and Ethnic Health Disparities not only responds to advice previously offered by stakeholders around the nation, but it also capitalizes on new and unprecedented opportunities in the Affordable Care Act of 2010 to benefit diverse communities. The Action Plan advances five major goals: transforming health care; strengthening the infrastructure and workforce of the nation's health and human services; advancing Americans' health and well-being; promoting scientific knowledge and innovation; and upholding the accountability of HHS for making demonstrable progress. By mobilizing HHS around these goals, the Action Plan moves the country closer to realizing the vision of a nation free of disparities in health and health care.

Abstract:One of the biggest challenges facing racial health disparities research is identifying how and where to implement effective, sustainable interventions. Community-based organizations (CBOs) and community-academic partnerships are frequently utilized as vehicles to conduct community health promotion interventions without attending to the viability and sustainability of CBOs or capacity inequities among partners. Utilizing organizational empowerment theory, this paper describes an intervention designed to increase the capacity of CBOs and community-academic partnerships to implement strategies to improve community health. The Capacity Building project illustrates how capacity building interventions can help to identify community health needs, promote community empowerment, and reduce health disparities.

Abstract:During the COVID-19 pandemic, the American Heart Association created a new 2024 Impact Goal with health equity at its core, in recognition of the increasing health disparities in our country and the overwhelming evidence of the damaging effect of structural racism on cardiovascular and stroke health. Concurrent with the announcement of the new Impact Goal was the release of an American Heart Association presidential advisory on structural racism, recognizing racism as a fundamental driver of health disparities and directing the American Heart Association to advance antiracist strategies regarding science, business operations, leadership, quality improvement, and advocacy. This policy statement builds on the call to action put forth in our presidential advisory, discussing specific opportunities to leverage public policy in promoting overall well-being and rectifying those long-standing structural barriers that impede the progress that we need and seek for the health of all communities. Although this policy statement discusses difficult aspects of our past, it is meant to provide a forward-looking blueprint that can be embraced by a broad spectrum of stakeholders who share the association’s commitment to addressing structural racism and realizing true health equity.

Abstract:Social Psychological and Personality Science, Ahead of Print. High levels of economic inequality are associated with numerous negative individual and societal consequences, and people prefer less of it. Opposition to redistributive policies designed to reduce inequality (e.g., taxing the rich to assist the poor), however, remains persistent. In this research, we propose a simple intervention to boost support for such policies. Specifically, we suggest that describing inequality between babies (in rich or poor homes) makes unequal opportunities more salient than describing inequality between adults. Because unequal opportunities are more difficult to rationalize than unequal outcomes, this results in a shift away from individualistic attributions and toward structural attributions for inequality. Critically, structural problems require structural solutions (e.g., redistributive efforts). We test this account across five preregistered studies (N = 5,800), spanning various presentation modalities (e.g., visual depictions, written descriptions), demographics (e.g., race, gender), inequality-reducing policies (e.g., taxation, food stamps), and a consequential choice underscoring implications for donation behavior.

Changes in Racial and Ethnic Disparities in Use of Mental Health Services under the Affordable Care Act: Evidence from California

Abstract:Background: The Affordable Care Act (ACA) aimed to expand mental health service use in the US, by expanding access to health insurance. However, the gap in mental health utilization by race and ethnicity is pronounced: members of racial and ethnic minoritized groups remain less likely to use mental health services than non-Hispanic White individuals even after the ACA. Aims of the study: This study assessed the effect of the Affordable Care Act (ACA) on mental health services use in one large state (California), and whether that effect differed among racial and ethnic groups. Also, it tested for change in racial and ethnic disparities after the implementation of the ACA, using four measures of mental health care. Methods: Using pooled California Health Interview Survey (CHIS) data from 2011-2018, logistic regression and Generalized Linear Models (GLM) were estimated. Disparities were defined using the Institute of Medicine (IOM) definition. Primary outcomes were any mental health care in primary settings; in specialty settings, any prescription medication for mental health problems, and number of annual visits to mental health services. Results: Findings suggested that the change in Hispanic-non-Hispanic White disparities in prescription medication use under the ACA was statistically significant, narrowing the gap by 7.23 percentage points (p<.05). However, the disparity in other measures was not significantly reduced. Discussion: These findings suggest that the magnitude of the increase in primary and specialty mental health services among racial and ethnic minorities was not large enough to significantly reduce racial and ethnic disparities. One possible explanation is that non-financial factors played a role, such as language barriers, attitudinal barriers from home culture norms, and systemic barriers due to mental health professional shortages and a limited number of mental health care providers of color. Implications for health care provision and use: Integrated approaches that coordinate specialty and primary care mental health services may be needed to promote mental healthcare access for members of racial and ethnic minoritized groups. Implications for health policies: Federal and state policies aiming to improve mental health services use have historically given more weight to financial determinants, but this has not been enough to significantly reduce racial/ethnic disparities. Thus, policies should pay more attention to non-financial determinants. Implications for further research: Assessing underlying mechanisms of non-financial factors that moderate the effectiveness of the ACA is a worthwhile goal for future research. Future studies should examine the extent to which non-financial factors intervene in the relationship between the implementation of the ACA and mental health services use.

Abstract:Atopic Dermatitis (AD) is associated with a number of published disparities, including those related to self-identified racial/ethnic populations. Black/African American populations in the United States experience greater AD prevalence, severity, and persistence compared to other demographic groups, such as those who self-identify as White. There is consistent evidence that healthcare, environmental, and socioeconomic factors affect the prevalence, severity, and/or persistence of AD. Additionally, because of institutional and systemic racism, these same risk factors are more prevalent in racial and ethnic minority populations. Herein, we review the underlying causes of racial/ethnic disparities in AD and potential strategies to improve AD diagnosis and disease control.
