Abstract:OBJECTIVE:To observe the effects of lateral attic wall reconstruction and sealing of attic cell in prevention of recurrence of cholesteatoma and limitation of ossicular chain after tympanoplasty.METHODS:150 cases of chronic tympanitis and mastoiditis were studied. Forty patients with intact ossicular chain after opening mastoidectomy underwent lateral attic wall reconstruction with autologous cartilage of auricular concha (n = 15) or "bone cement plate" made of autologous bone cortex powder adhered by glue of fibrinogen and thrombin (n = 25). Eighty-nine of the 110 cases with damaged ossicular chain underwent reconstruction of ossicular chain and sealing of tympanic cavity with bone powder and glue of fibrinogen and thrombin and fascia of temporal muscle was used to restore the tympanic membrane in 21 of them. Follow-up was conducted for 3 to 5 years to observe the recovery of tympanic membrane, hearing, and whether cholesteatoma recurred.RESULTS:No recurrent cholesteatoma and serious retracted tympanic membrane were found. Eardrum re-perforated in 8 cases (5.33%). The patients with lateral attic wall reconstruction obtained a significant hearing improvement (97.5%). Hearing was improved among 80.8% of the cases that underwent sealing of attic cell and ossiculoplasty.CONCLUSION:Lateral attic wall reconstruction and sealing of attic cell prevent the recurrence of cholesteatoma. Reconstruction of lateral attic wall provides a new space for vibration of ossicular chain.

Abstract:Understanding how T lymphocytes recognize β-cell autoantigens is essential forthe elucidation of the pathogenesis of insulin-dependent diabetes mellitus. The increased and ectopic expression of HLA class I and II molecules detected in human β-cells may facilitate this interaction. T-lymphocyte recognition of surface antigens also involves adhesion accessory molecules: intercellular adhesion molecule 1 (ICAM-1) and lymphocyte function—associated antigen 3 (LFA-3). These molecules not only allow cell contact but can also provide costimulatory signals for T-lymphocyte activation. Levels of ICAM-1 and LFA-3 expression in normal human islet cells and regulation of their expression by cytokines interferon-γ (IFN-γ), tumor necrosis factor α (TNF-α), interleukin 1β (IL-1β), and IL-6 have been studied by two-color immunofluorescence staining of pancreatic cryostat sections and fluorescence-activated cell sorter analysis. Neither ICAM-1 nor LFA-3 could be demonstrated in sections or in fresh cell preparations, but after 18 h of culture, β-, α-, and δ-cells expressed spontaneously moderate levels of ICAM-1 (but not LFA-3). IFN-γ and TNF-α alone or in combination strongly enhanced this spontaneous expressionof ICAM-1 in a time- and/or dose-dependent and additive manner but had no effect on LFA-3. An SV40-transformed islet cell line showed high basal levels of both ICAM-1 and LFA-3, but the response to cytokines followed the same pattern as primary cultures.Theabsence of constitutive expression of adhesion molecules in β-cells supports the concept that they are immunologically silent cells and may also explain the failure of inducing autoimmune diabetes in transgenic mice by the sole expression of class II in theβ-cells via genetic manipulation. The induction of ICAM-1 by the same cytokines that can induce HLA class I and class II expression suggests that, in the diabetic pancreas, remaining β-cells probably express ICAM-1 concomitantly with HLA products.

Abstract:Objective To compare recidivism rates, audiometric outcomes, and postoperative complication rates between soft-wall canal wall reconstruction (S-CWR) versus bony-wall CWR (B-CWR) with mastoid obliteration (MO) in patients with cholesteatoma. Study Design Retrospective chart review. Setting Tertiary neurotologic referral center. Patients Ninety patients aged ≥18 years old who underwent CWR with MO, either S-CWR or B-CWR, for cholesteatoma with one surgeon from January 2011 to January 2022. Patients were followed postoperatively for at least 12 months with or without second-look ossiculoplasty. Intervention(s) Tympanomastoidectomy with CWR (soft vs. bony material) and mastoid obliteration. Main Outcome Measure(s) Recidivism rates; conversion rate to CWD; pre- versus postoperative pure tone averages, speech reception thresholds, word recognition scores, and air-bone gaps; postoperative complication rates. Results Middle ear and mastoid cholesteatoma recidivism rates were not significantly different between B-CWR (17.3%) and S-CWR (18.4%, p = 0.71). There was no significant difference in pre- versus postoperative change in ABG (B-CWR, −2.1 dB; S-CWR, +1.6 dB; p = 0.91) nor in the proportion of postoperative ABGs <20 dB (B-CWR, 41.3%; S-CWR, 30.7%; p = 0.42) between B-CWR and S-CWR. Further, there were no significant differences in complication rates between B-CWR and S-CWR other than increased minor TM perforations/retractions in B-CWR (63% vs. 40%, p = 0.03). Conclusions Analysis of recidivism rates, audiometric outcomes and postoperative complications between B-CWR with MO versus S-CWR with MO revealed no significant difference. Both approaches are as effective in eradicating cholesteatoma while preserving relatively normal EAC anatomy and hearing. Surgeon preference and technical skill level may guide the surgeon’s choice in approach.

Comparison of Total Endoscopic Ear Surgery and Microscopic Postauricular Canal-Wall-Down Approach on Primary Acquired Cholesteatoma

Abstract:This study aimed to compare total endoscopic ear surgery (TEES) and microscopic postauricular canal-wall-down tympanomastoidectomy (CWD) in cholesteatoma surgery in our clinic. This study included 59 patients, of whom 30 and 29 were operated on with CWD in 2016-2018 and TEES in 2019-2021, respectively and compared regarding intraoperative findings, hearing outcomes, long-term outcomes, and recidivism rates between groups. This study excluded patients in stage IV according to the European Academy of Otology and Neurotology/Japan Otological Society Staging System on Middle Ear Cholesteatoma, aged < 18, with congenital cholesteatoma, who underwent revision surgery. Two patients in the TEES group had recidivism (6.9%), with recurrent disease observed in both patients and residual disease in none, whereas 3 patients in the CWD group had recidivism (10%), including recurrent disease in 2 and residual disease in 1 patient. Tympanic membrane perforation occurred in 2 (6.9%) and 1 (3.3%) patients in the TEES and CWD groups, respectively. The 2 groups revealed no significant difference in terms of recidivism and perforation rates (P=1.000, P = .612). The CWD group had a longer mean operation time (225.54 ± 47.86 minutes) than the TEES group (160.55 ± 24.98 minutes) (P < .001). The 2 groups demonstrated no significant difference regarding pre- and postoperative air-bone gap (ABG) and ABG gain (P = .105, P=.329, P=.82, respectively). Total endoscopic ear surgery provides similar results in terms of hearing, recidivism, and long-term outcomes with the microscopic CWD approach. However, the CWD approach is still important, especially in patients in advanced stages.
Canal-wall up cholesteatoma surgery with mastoid obliteration leads to lower rates of disease recurrence without affecting hearing outcomes

Abstract:Objectives The primary objective was to determine whether obliteration of the epitympanic area and mastoid cavity during canal wall up (CWU) cholesteatoma surgery reduces the rate of recurrent and residual cholesteatoma compared to not obliterating the same area. The secondary objective was to compare postoperative hearing outcomes between both techniques. Methods A retrospective cohort study was conducted in a tertiary referral center. One-hundred-fourty-three ears were included of patients (≥18y) who underwent a CWU tympanomastoidectomy for cholesteatoma with or without bony obliteration between January 2015 and March 2020 in the University Medical Center Utrecht. The median follow-up was respectively 1.4 (IQR 1.1–2.2) vs. 2.0 years (IQR 1.2–3.1) ( p = 0.013). Intervention(s) All patients underwent CWU tympanomastoidectomy for cholesteatoma. For 73 ears bone dust, Bonalive® or a combination was used for obliteration of the mastoid and epitympanic area, the rest of the ears ( n = 70) were not obliterated. In accordance with the Dutch protocol, included patients are planned to undergo an MRI scan with diffusion-weighted imaging (DWI) one, three and five years after surgery to detect recurrent or residual cholesteatoma. Main outcome measure(s) The primary outcome measure was recurrent and residual cholesteatoma as evaluated by MRI-DWI and/or micro-otoscopy and confirmed by micro-otoscopy and/or revision surgery. The secondary outcome measure was the postoperative hearing. Results In this cohort, the group treated with canal wall up tympanomastoidectomy with subsequent bony obliteration (73 ears, 51.0%) had significantly lower recurrent (4.1%) and residual (6.8%) cholesteatoma rates than the group without obliteration (70 ears, 25.7% and 20.0%, respectively; p < 0.001). There was no significant difference between both groups in postoperative bone conduction thresholds (mean difference 2.7 dB, p = 0.221) as well as the mean air-bone gap closure 6 weeks after surgery (2.3 dB in the non-obliteration and 1.5 dB in the obliteration group, p = 0.903). Conclusions Based on our results, a canal wall up tympanomastoidectomy with bony obliteration is the treatment of choice, since the recurrent and residual disease rate is lower compared to the group without obliteration. The bony obliteration technique does not seem to affect the perceptive or conductive hearing results, as these are similar between both groups.

[Clinical Analysis of 21 Cases of Cholesteatoma Recidivism after Canal-Wall-up Mastoidectomy with Tympanoplasty].

Abstract:Objective:To discuss the possible reasons for cholesteatoma recidivism after canal-wall-up mastoidectomy with tympanoplasty by analyzing clinical characteristics of patients. Methods:Data of 21 cases who suffered from cholesteatoma recidivism after canal-wall-up surgery were retrospectively reviewed, including preoperative examination, high resolution temporal bone CT, and intraoperative findings. Results:90.5%（19/21） cases had recurrent cholesteatoma with retraction pockets. Among 12 cases with previous operative notes, 66.7%（8/12） had extensive cholesteatoma which was not limited to attic in the original surgery. The intraoperative features of revision surgery in 21 patients including the destruction of reconstructive lateral attic wall and scutumwere found in 19.0%（4/21） cases, the head of malleus left in 19.0%（4/21） cases, the cholesteatoma found in hidden part in 14.3%（3/21） cases, the hadeustachian tube dysfunction in 38.1%（8/21）cases. the sclerotic mastoid in 42.9%（9/21） cases. hadanatomic variations of the temporal bone in 14.3%（3/21） cases and atresia of external auditory canal in 4.8%（1/21） cases. Conclusion:In this group of recidivism cases, most patients had extensive cholesteatoma, which may lead to excessive mucosa loss during lesion clearance, poor ventilation of tympanic isthmus after surgery, and promote the formation of retraction pocket. In addition, some cases had eustachian tube dysfunction, unstable reconstruction of attic lateral wall, and improper selection of the indications, which may also increase the risk of recurrence. Therefore, in order to reduce cholesteatoma recidivism after canal-wall-up surgery, attention should be paid to the striction of surgical indications, comprehensive preoperative evaluation, thorough clearance of lesions and firm reconstruction.
Clinical observation of external canal reconstruction with autogenous bone pate in the micro-otosurgery

Abstract:Objective To observe the therapeutic effect of external canal reconstruction with autogenous bone pate from the mastoid cortex in the micro-otosurgery.Methods Thirteen patients(13 ears) with chronic otitis media comprising cholesteatoma(8 cases) and chronic suppurative otitis media(5 cases) underwent reconstruction of external canal with autogenous bone pate from the mastoid cortex.Of the 13 cases,9 were treated with canal wall down mastoidectomy plus tympanoplasty(CWD),and 4 cases were treated with modified radical mastoidectomy(MRM).The autogenous bone pate was used to obliterate mastoid cavity and reconstruct the external canal in all the 13 cases.The postoperative modality and function of the external auditory canal and the postoperative hearings were observed.Results The mean dry ear time was(32±6.5) days after surgery.The postoperative modality of external auditory canal was normal on the whole,and there was no crust which need to be cleaned usually.All patients were followed up for 4-16 months.The postoperative hearings of 4 cases underwent MRM remained unchanged.The postoperative average air-bone gap of 9 cases underwent CWD was lower than that of the preoperative ones〔(21.3±14.7) dB HL vs(36.3±11.9) dB HL,P<0.05〕.Conclusions External canal reconstruction with autogenous bone pate from the mastoid cortex in the micro-otosurgery is convenient for drawing materials and its therapeutic effect is satisfactory.
The Management of Attic and Mastoid Cavity after Atticotomy and Mastoidectomy in Tympanoplasty

Abstract:Purpose To discuss the application of bone pate from mastoid cortex for construction of attic and mastoid cavity after atticotomy and mastoidectomy in canal wall down tympanoplasty. Methods The data of 58 patients treated by tympanoplasty were retrospectively analyzed. The bone pate collected while drilling the mastoid cortex was used to fill in the attic cell and mastoid cavity after atticotomy and mastoidectomy, and also used to protect the exposed endocranium. The labyrinthine fistuke and naked facial nerve were overlapped with temporalis fascia first and then covered with bone pate. The temporalis fascia was used to repair eardrum and overlap the surface of bone pate. Results Most patients had dry ears with slightly wide and smooth external canal shortly after the operation. The rate of dry ear in three months was 93 A % (54/58) , and rose to 98.3% (57/58) in six months. There were no symptoms of facial paralysis and meningitis in these patients. And the vertigo of patients with labyrinthine fistulae vanished fast. Conclusion Bone pate from mastoid cortex is a good filling and protective material used in attic and mastoid cavity construction in tympanoplasty. It can help to obtain dry ear early.

Abstract:Background: Reconstruction of the posterior canal wall should be planned by the surgeon in canal wall down mastoidectomy to prevent the resulting cavity that is prone to recurrent infection. The surgeon should choose the type of grafts either autologous, homologous or allografts. Furthermore, the surgeon should decide whether to do immediate reconstruction or delayed one. Methods: In this study, the ridge was reconstructed at the same time of mastoidectomy by autologous tissues. We used conchal cartilage graft vascularized by pedicled perichondrial and periosteal flaps. The study included 48 patients (32 males and 16 females) with age ranged from 18-55 years. All patients were with unilateral chronic suppurative otitis media with persistent discharge. They had been operated at our tertiary hospital between Jan. 2012 to march 2014 Results: Successful reconstruction was obtained in all cases with no dehiscence, necrosis or canal stenosis Conclusion: The use of composite conchal cartilage with perichondrial and periosteal flaps in repair of the ridge is highly efficient. Long term outcomes of canal wall up and canal wall down tympanomastoidectomies in pediatric cholesteatoma

Abstract:Cholesteatomas in children have a more aggressive growth pattern compared to adults, which leads to a higher incidence of both residual and recurrent disease. A staged canal wall-up or a canal wall-down tympanomastoidectomy (CWUT and CWDT, respectively) is selected depending on the extent of the disease and condition of the middle ear (ME) cleft and mastoid. Endoscopic ear surgery (EES) has been recently introduced as an adjuvant tool for the treatment of this pathology even in the pediatric population. Objectives: To analyze long term outcomes of CWUT and CWDT in the pediatric population, focusing on residual and recurrence rates of cholesteatoma and hearing results. A literature review including cases treated with EES were discussed. Material and methods: Pediatric patients treated for cholesteatoma involving both the ME and mastoid with a follow-up (FU) of at least 4 years were retrospectively analyzed in a quaternary referral center for otology and lateral skull base surgery. Patients were grouped according to the surgical technique (CWUT versus CWDT). Rates of residual and recurrent cholesteatoma after each surgical technique were reported and compared. Mean Air-Bone Gap (ABG) of 0.5-1-2-4 KHz was measured and reported before the first surgery and at the last post-operative FU. Results: Two-hundred and thirty-six cases fulfilled our inclusion criteria. The mean FU was 100.4 ± 44.2 months (median 89 months). One-hundred and five (44.5%) cases underwent a CWUT, whereas 131 (55.5%) a CWDT. A second stage surgery was performed in 73.5% of CWUT and 58.7% of CWDT. Among the CWUT group, residual cholesteatoma occurred in 22 (21%) ears and recurrence in 24 (22.9%). Patients undergoing CWDT showed lower rates of both residual and recurrent cholesteatoma (7.6% and 2.3%, respectively). ABG improvement was noted for both groups, even though CWUT showed better post-operative hearing results. Conclusions: The CWDT technique offers a definite surgical therapy, with minimal residual and recurrence rates and audiological results comparable to the CWUT technique. EES must still prove its added benefit or equivalence to pure microscopic approaches.
Evaluation of Requirements for Staging the Procedure of Reconstruction of Middle Ear After Canal Wall Down Mastoidectomy

Abstract:Cholesteatoma is a progressive destructive ear disease which can affect any age group. It has been found to be more severe in children and young adults. It erodes the surrounding bone of middle ear, mastoid and ossicles. It causes partial to total deafness, unpleasant smelling discharge, pain, tinnitus, vertigo and facial paralysis. It can even cause meningitis, brain abscess and death. The post-operative outcome of hearing, and the state of the reconstructed middle-ear cavity after concurrent and staged reconstruction of middle ear after canal wall down mastoidectomy was studied in 30 ears with middle-ear cholesteatoma. The reconstructed middle ear was re-aerated in 60.5 % of the cases, which was significantly higher than for the epitympanum (39.5 %). Tympanoplasty was successful in terms of hearing results in 68.9 % of all subjects and in 75.4 % of the ears having a re-aerated tympanic cavity, which was significantly better than the 38.5 % for ears in which the tympanic cavity was not re-aerated. The findings of recurrent cholesteatoma, tympanic atelectasis, and tympanic effusion were observed with significantly (p < 0.03) high incidence in ears with no re-aerated space in their reconstructed mastoid cavities. It was revealed that the post-operative outcome of this surgical technique was significantly related to the state of re-aeration of the reconstructed middle-ear cavity but not with either concurrent or staged reconstruction. Audiological results are same for both concurrent and staged reconstruction following canal wall down tympanomastoidectomy, and hence we reccommend that concurrent reconstruction is preferred in limited disease and staged reconstruction in severe disease.
Atticotomy with reconstruction for limited cholesteatoma

Abstract:Several authors have confirmed that it is entirely reasonable to perform a one-stage atticotomy for attic cholesteatomas which are limited in size. Over a period of 10 years, 195 primary procedures were performed for cholesteatoma, and, out of this total, 65 were operated on by an established technique of cartilage repair. In this group of a limited atticotomy there were only two recurrences (3%) and so the technique is considered reliable and, furthermore, using the author's modification, there is no need to dissect out perichondrial flaps. It is recommended that attic cholesteatomas are operated on while they are as small as possible--no damage to the hearing occurred in 63 (96.9%) of the ears in this series--as the results of ossicular repair in ears damaged by cholesteatoma are unpredictable, especially when there is eustachian tube dysfunction.

Abstract:Background: Aim of the study was to compare the outcomes of a new technique of mastoid cavity obliteration with open mastoid cavity. Methods: 90 patients diagnosed with chronic otitis media (active squamosal variety) and planned to undergo surgery between 2016-2018 were randomly assigned into two groups of 45 patients. One group underwent canal wall down matoidectomy followed by cavity obliteration with posteriorly based vascularised flap with mastoid cortex periosteum plus conchal cartilage composite graft and the other underwent the same surgery but without any obliteration. The two groups were compared in terms of cavity volume, discharge, epithelisation, wax formation, subjective sensation of vertigo and post-operative air-bone gap at 1 year. Results: Cavity obliteration in post-canal wall down setting significantly reduced the post-operative cavity volume and need for cavity debridement with better epithelisation, less incidence of discharge, vertigo on caloric stimulation when compared to open cavity. The post-operative air-bone gap in obliterated cavities was better but not statistically significant. Conclusion: Cavity obliteration has definite advantages over open cavity in terms of healing but no significant differences in long-term audiological outcomes.
Development of Air Cell System Following Canal Wall Up Mastoidectomy for Pediatric Cholesteatoma

Abstract:Background: The development of temporal bone pneumatization is related to the postnatal middle ear environment, where the development of air cells is suppressed with otitis media in early childhood. However, whether air cell formation restarts when mastoidectomy is performed during temporal bone pneumatization remains unclear. Herein, we evaluated temporal bone pneumatization after canal wall up (CWU) tympanomastoidectomy for middle ear cholesteatoma in children. Methods: In total, 63 patients, including 29 patients with congenital cholesteatoma (CC) and 34 patients with acquired cholesteatoma (AC), were assessed using a set of pre- and postoperative computed tomography images. The air cells of the temporal bone were divided into five areas: periantral (anterior), periantral (posterior), periantral (medial), peritubal, and petrous apex. The number of areas with air cells before and after surgery was compared to evaluate temporal bone pneumatization after surgery. Results: A total of 63 patients, comprising 29 with CC and 34 with AC (pars flaccida; 23, pars tensa; 7, unclassified; 4), were evaluated. The median age of patients (18 males and 11 females) with CC was 5.0 (range, 2-15 years), while that of the AC group (23 males and 11 females) was 8 (range, 2-15 years). A significant difference in air cell presence was identified in the CC and AC groups after surgery (Mann-Whitney U, p < 0.001 and p = 0.003, respectively). Between the two groups, considerably better postoperative pneumatization was observed in the CC group. A correlation between age at surgery and gain of postoperative air cell area development was identified in the CC group (Spearman's rank-order correlation coefficient, r = -0.584, p < 0.001). In comparison with the postoperative pneumatization rate of each classified area, the petrous apex area was the lowest in the CC and AC groups. Conclusions: Newly developed air cells were identified in the temporal bones after CWU mastoidectomy for pediatric cholesteatoma. These findings may justify CWU tympanomastoidectomy, at least for younger children and CC patients, who may subsequently develop air cell systems after surgery.
Cholesteatoma surgery in the pediatric population: remaining challenges in the era of mastoid obliteration

Abstract:Abstract Purpose To present the first pediatric study on the safety and efficacy of mastoid obliteration using S53P4 bioactive glass (BAG) for cholesteatoma surgery. Methods A single-center retrospective cohort study was conducted. Inclusion criteria were pediatric cases (≤ 18 years) and at least at least one year of follow-up including non-echo planar diffusion-weighted MRI to assess cholesteatoma recidivism. Both canal wall up (CWU) and canal wall down (CWD) procedures were evaluated. Results A total of 61 cases (56 patients) were included. Most cases had an otologic history before the development of the cholesteatoma. CWU procedure was performed in 18 cases (30%) and CWD procedure in 43 cases (70%). The cholesteatoma recidivism rate was 33% after a mean follow-up period of 58 months. Kaplan–Meier curve estimated a 5-year recidivism rate of 40%. Few complications were seen that were all minor and resolved spontaneously or after local or systemic treatment. Control of the infection (merchant grade 0–1) was achieved in 98% of the cases. Closure of the air–bone gap within 20 dB was achieved in 22% of the cases with complete audiometric evaluation. Conclusion In this MRI-controlled study, we show the safety and efficacy of S53P4 BAG for mastoid obliteration in a pediatric cholesteatoma cohort. Postoperative complications were both rare and minor, and a dry ear was achieved in almost all patients. Nevertheless, persistent hearing loss and the apparent high recidivism rate reflect the challenging nature of pediatric cholesteatoma.

Abstract:Objective To discuss the curative effect and minimal invasive approach of reconstruction of posterior wall of external auditory canal with cartilagein for the surgical treatment of stage Ⅲ external auditory canal cholesteatoma ( EACC ) with specific invasion in mastoid cavity . Methods A retrospective review was made on 11 cases ( 12 ears ) of stage Ⅲ EACC with specific invasion in mastoid cavity from March 2012 to April 2016.Three ears suffered significant bone invasion , and were given traditional canal-wall-down ( CWD ) mastoidectomy and auricular cavity forming .The other nine ears underwent reconstruction of posterior wall of external auditory canal with cartilage after complete elimination of the lesion . Results Dry ear was achieved after surgery in all the 12 ears.The patients got dry ear at 50, 58, and 65 days after operation in the CWD group , and got dry ear at 26-44 days (mean, 33 ±5 days) after reconstruction of posterior wall of external auditory canal .During a follow-up for 6 -53 months (mean, 23 ±15 months), there were no signs of recurrence of cholesteatoma and collapse or stenosis of external auditory canal . Conclusions For the treatment of stage Ⅲ EACC, reconstruction of posterior wall of external auditory canal is a minimal invasive approach with good curative results which effectively eliminates the lesion .Meanwhile , it achieves the reconstruction of shape of the external auditory canal and restores the normal physiological functions .

Abstract:Objective: The objective of the study is to evaluate the outcomes of surgical management options for cholesteatoma using a national database. Study design: Database analysis of the Pediatric Health Information System database to identify children undergoing surgical intervention for cholesteatoma from October 2015 to December 2022. Methods: Patients were categorized by initial surgical modality: tympanoplasty (TM), tympanoplasty with canal wall-up tympanomastoidectomy (TM-CWU), and tympanoplasty with canal wall-down tympanomastoidectomy (TM-CWD). Group comparisons were done on number of surgical revisions and number of patients identified with recommendations for hearing aids. Results: A total of 6304 patients were identified in the database who underwent surgery for cholesteatoma. Of these children, 3405 underwent TM (54.0%), 3116 underwent TM-CWU (49.4%), and 825 underwent TM-CWD (8.2%). The estimated difference in mean number of procedures was significantly higher in the TM-CWU group compared to TM-CWD (-0.34, 95% confidence interval [CI] -0.406,-0.279, P < .0001) and the TM group (9.352, 95% CI 0.315, 0.390, P < .0001). The rate of significant hearing loss necessitating hearing aids was significantly lower in the TM group, but there was no difference between the TM-CWU and TM-CWD groups (1.2%, P < .03, 1.9% vs 2.7%, P = .13). There was no difference in the number of speech delays/therapy diagnoses between TM and TM-CWU or TM-CWU and TM-CWD (3.5%vs 4.4% P = .07, 4.4% vs 5.2%., P = .38). Conclusion: TM and TM-CWD had lower total surgical procedures than the TM-CWU group, and the TM group had a lesser rate of recommendation for hearing aids. The difference between number of procedures is likely due to the complexity of the disease; for instance, TM was likely chosen for small, less severe disease cases, whereas TM-CWD was chosen as more definitive treatment in aggressive cases. TM-CWD is associated with a lower rate of recidivism and recurrence, which also likely contributed to the lower number of procedures.

Abstract:OBJECTIVE: Following cholesteatoma surgery, effective long-term hearing preservation in children is difficult and is not typically expected. Hence, long-term data on hearing outcomes are lacking. The aim of this study was to analyze long-term hearing outcomes in children following cholesteatoma surgery.METHODS: For this study, 49 ears in 47 children (≤16 years) with acquired cholesteatomas following atticotomy-limited mastoidectomy with cartilage reconstruction (inside-out approach) during 1986-2010 were included. Pre- and post-operative recidivism-free audiometric results were compared. Hearing success was defined as a post-operative air conduction (AC) threshold of ≤30 dB (serviceable hearing). Logistic regression analyses were used to evaluate potential prognostic factors that independently contributed to the prediction of hearing success. These factors included stapes condition, pre-operative AC threshold, ossicular chain integrity, disease severity, age, and gender.RESULTS: The mean duration of follow-up was 14.2 years. The post-operative AC (33.55 ± 15.42 dB) and air-bone gap (17.88 ± 12.94 dB) were significantly improved compared with the pre-operative AC (42.90 ± 16.47 dB, p < 0.001) and air-bone gap (30.23 ± 13.68 dB, p < 0.001). The probability of hearing success following surgery (40.8%) was significantly higher than prior to surgery (24.5%, p = 0.008). Multivariate logistic regression analyses revealed a statistically significant correlation between hearing success and stapes integrity only (p = 0.005).CONCLUSIONS: This study provides important information on effective long-term hearing preservation over a mean follow-up of 14 years. In addition, stapes destruction is an independent negative prognostic determinant of achieving hearing success. The prediction model in this study provides otologists with useful pre-operative information to inform patients and parents on expected hearing outcomes and may be useful for post-operative observations.

Abstract:OBJECTIVE To study the reconstruction technique of tympanic membrane and posterior wall of external ear canal with auricular cartilage at same time. METHOD The tympanic membrane and posterior wall of external ear canal of 43 cases of chronic suppurative otitis media were reconstructed with auricular cartilage after mastoidectomy between 1995-2000. The results were compared with that of without reconstruction of the posterior wall of external canal group of 60 ears in same period. RESULT The average follow-up period was 32 months, 3 cases were faced in all of 43 cases, the others were healed in one stage. Postoperative period to be a dry ear was significantly shorter in the reconstruction group than in the canal-wall-down and open group (t-test, t = 3.98, < 0.01). The rezidive results is statistic not significant between this two groups. The patients of both groups have had good hearing results. CONCLUSION The auricular cartilage is a good material for the reconstruction of tympanic membrane and posterior wall of the external auditory canal. It can not only reduce the time of drying ear, improve the hearing results, but also avoid the second operation. The quality of the patients life can be improved.
