Regenerative medicine in the treatment of specific dermatologic disorders: a systematic review of randomized controlled clinical trials

Abstract:Abstract Aims and objectives The aim of this study is to systematically review randomized controlled clinical trials (RCTs) studying various types of regenerative medicine methods (such as platelet-rich plasma, stromal vascular fraction, cell therapy, conditioned media, etc.) in treating specific dermatologic diseases. Rejuvenation, scarring, wound healing, and other secondary conditions of skin damage were not investigated in this study. Method Major databases, including PubMed, Scopus, and Web of Science, were meticulously searched for RCTs up to January 2024, focusing on regenerative medicine interventions for specific dermatologic disorders (such as androgenetic alopecia, vitiligo, alopecia areata, etc.). Key data extracted encompassed participant characteristics and sample sizes, types of regenerative therapy, treatment efficacy, and adverse events. Results In this systematic review, 64 studies involving a total of 2888 patients were examined. Women constituted 44.8% of the study population, while men made up 55.2% of the participants, with an average age of 27.64 years. The most frequently studied skin diseases were androgenetic alopecia (AGA) (45.3%) and vitiligo (31.2%). The most common regenerative methods investigated for these diseases were PRP and the transplantation of autologous epidermal melanocyte/keratinocyte cells, respectively. Studies reported up to 68.4% improvement in AGA and up to 71% improvement in vitiligo. Other diseases included in the review were alopecia areata, melasma, lichen sclerosus et atrophicus (LSA), inflammatory acne vulgaris, chronic telogen effluvium, erosive oral lichen planus, and dystrophic epidermolysis bullosa. Regenerative medicine was found to be an effective treatment option in all of these studies, along with other methods. The regenerative medicine techniques investigated in this study comprised the transplantation of autologous epidermal melanocyte/keratinocyte cells, isolated melanocyte transplantation, cell transplantation from hair follicle origins, melanocyte–keratinocyte suspension in PRP, conditioned media injection, a combination of PRP and basic fibroblast growth factor, intravenous injection of mesenchymal stem cells, concentrated growth factor, stromal vascular fraction (SVF), a combination of PRP and SVF, and preserving hair grafts in PRP. Conclusion Regenerative medicine holds promise as a treatment for specific dermatologic disorders. To validate our findings, it is recommended to conduct numerous clinical trials focusing on various skin conditions. In our study, we did not explore secondary skin lesions like scars or ulcers. Therefore, assessing the effectiveness of this treatment method for addressing these conditions would necessitate a separate study.

Abstract:Hypertrophic scars (HTSs) result from aberrant wound healing processes, leading to raised, thickened tissue with functional discomfort and cosmetic concerns. Current treatments, including corticosteroid injections and laser therapy, have limitations. Stromal vascular fraction (SVF) therapy and CO2 laser treatment offer promising avenues, with SVF therapy showing regenerative potential and CO2 laser therapy promoting precise tissue removal and wound healing. This study aims to investigate the combined application of SVF therapy and CO2 laser treatment for HTS, aiming to enhance treatment efficacy, tissue remodeling, and aesthetic outcomes, ultimately improving patient satisfaction in HTS management.

Combining scar-modulating agents for the treatment of hypertrophic scars and keloids: A systematic review

Abstract:Injury to the skin can cause abnormal wound healing and continuous inflammation that leads to the formation of hypertrophic scars and keloids. These lesions often cause significant negative impact on a patient's life due to aesthetic, physical, social, and psychological consequences. Numerous treatment modalities exist for these hypertrophic scars and keloids, which include silicone sheeting, pressure garments, intralesional injection/topical application of scar-modulating agents, laser therapy, and surgical excision. Due to increased efficacy, an evolving treatment paradigm encourages the use of multiple treatment modalities instead of one treatment modality. However, no gold standard treatment exists for these lesions, leaving many people with unsatisfactory results. Adding scar-modulating agents such as 5-Fluorouracil, bleomycin, or Botulinum Toxin A to triamcinolone monotherapy has emerged as a potential drug combination for treating hypertrophic scars and keloids. We sought to critically analyze the evidence that exists for the use of more than one scar-modulating agent. This was done by conducting a systematic review to determine the efficacy of these combined drug regimens. We found that many of these combinations show evidence of increased efficacy and fewer/similar adverse events to triamcinolone monotherapy. Triamcinolone and 5-Fluorouracil showed the strongest and most consistent evidence out of all combinations. With this review, we intend to encourage more research into unique drug combinations that may improve outcomes for patients with symptomatic hypertrophic scars or keloids.

Abstract:Aims and Objectives The aim of this study is to examine the efficacy and safety of various regenerative medicine treatments, such as cell therapy, platelet‐rich plasma (PRP), plasma‐poor platelet (PPP), plasma‐rich fibrin (PRF), mesenchymal stem cells, stromal vascular fraction (SVF), exosomes, adipose‐derived stem cells (ADSC), and stem cell‐conditioned media (SC‐CM), for treating vitiligo. Method We conducted a thorough search of major databases such as PubMed, Scopus, and Web of Science, and selected 48 articles based on specific criteria. We used EndNote X8 and Google Sheets to review and extract data from the articles. After analyzing the studies, we categorized them accordingly. Results This systematic review analyzed 48 articles involving 2186 patients with vitiligo to assess the effectiveness of regenerative medicine treatments. Key findings revealed that methods such as autologous non‐cultured melanocyte‐keratinocyte transplantation and platelet‐rich plasma (PRP) injection exhibited significant repigmentation, particularly when combined with modalities like NB‐UVB phototherapy and laser treatments. Notably, the autologous melanocyte‐keratinocyte transplantation achieved over 50% repigmentation within 9 months, while PRP demonstrated an average repigmentation of 58.7%, especially effective with CO2 laser treatment. Hair follicle‐derived cell transplantation also showed impressive response rates, achieving good to excellent results in up to 93.8% of patients. Side effects were noted in 21 of 28 studies, primarily involving pain, with no serious adverse events reported. The risk of bias assessment indicated that 37.21% of studies were low risk, while 48.84% had high risks overall. These findings suggest that while regenerative medicine holds promise for vitiligo treatment, further clinical trials are necessary to explore additional methods like stromal vascular fraction and exosomes. Conclusion We have concluded that regenerative medicine plays an effective role in the treatment of vitiligo lesions. Furthermore, this treatment method is safe and does not cause serious complications. It can be used alone or in combination with other methods for treating vitiligo. To advance the treatment of vitiligo, we recommend conducting clinical trials on the unexplored branches of regenerative medicine.

Hypertrophic Scars and Keloids: Advances in Treatment and Review of Established Therapies

Abstract:Hypertrophic scars and keloids can have significant detrimental effects on patients both psychosocially and functionally. A careful identification of patient risk factors and a comprehensive management plan are necessary to optimize outcomes. Patients with a history of dystrophic scarring should avoid unnecessary procedures and enhance the wound-healing process using various preventive strategies. As there is no single, fully efficacious treatment modality, prevention remains the best approach in reducing aberrant scar formation. When prevention therapies fail, keloids have been shown to be respond to a variety of therapies including topical and injectable corticosteroids, 5-fluorouracil, radiotherapy, lasers, and surgical excision, all with varying efficacies. As such, management should be tailored to the individual patient's risk factors with the use of combination therapies to reduce recurrence rates. Still, keloid and hypertrophic scar therapies are widely diverse with novel treatment modalities providing alternatives for recurring lesions. Laser-assisted drug delivery, skin priming, and novel topical therapies may provide alternative options for the management of hypertrophic scars and keloids.

Abstract:Review question / Objective: The objective of this systematic review and meta-analysis of randomized controlled trials is to evaluate quantitatively the efficacy and safety of extracorporeal shockwave therapies (ESWT) combined comprehensive treatments on hypertrophic scars and keloids compared with comprehensive treatments alone and provide clinicians with an evidence base for their clinical decision making. Information sources: We will search all English and Chinese language articles indexed in PubMed, Medline, the Excerpta Medica database (Embase), Cochrane Central Register of Controlled Trials, the Cochrane Library, Physiotherapy Evidence Database(PEDro), Chinese biomedical literature service system(sinomed) before October 2021. In addition to these databases, Google Scholar and the lists of references will be used to carry out citation tracking of the selected studies for identifying any other eligible studies that could have been missed.

Abstract:A central theme of much of the recent literature on the strategy of the multinational corporation (MNC) is the increasingly important role played by subsidiary companies as contributors to the development of firm-specific advantages. Traditional academic models that viewed subsidiaries as either ‘market access’ providers or as recipients of the parent company’s technology transfers (Vernon, 1966) gave way in the 1980s to richer conceptualizations in which subsidiaries tapped into leading-edge ideas, undertook important research and development work, and became active participants in the formulation and implementation of strategy (Bartlett and Ghoshal, 1986; Hedlund, 1986; Gupta and Govindarajan, 1994). The generation of firm-specific advantages, correspondingly, shifted from being the sole concern of the parent company to a collective responsibility for the corporate network.

Abstract:Six new (erinarols A-F, 1-6) and five known (7-11) ergostane-type sterol fatty acid esters were isolated from the methanol extract of the dried fruiting bodies of Hericium erinaceum. Their chemical structures were elucidated using chemical and physical methods as well as through comparison of NMR and mass spectral data with those reported previously. This is the first comprehensive investigation on ergostane-type sterol fatty acid esters from H. erinaceum. The isolated compounds were evaluated for their PPAR transactivational effects using a luciferase reporter system. Compounds 1 and 2 significantly activated the transcriptional activity of PPARs in a dose-dependent manner, with EC50 values of 8.2 and 6.4 μM, respectively. Moreover, compounds 1 and 2 also activated PPARα and PPARγ transcriptional activity, with stimulation from 1.3- to 3.9-fold at 20 μM concentrations.
San Antonio Breast Cancer Symposium 2007—Adjuvant endocrine therapy update: ATAC 100 highlights

Abstract:The two main classes of adjuvant hormonal therapy used in the treatment of postmenopausal women with hormone receptor–positive breast cancer are selective estrogen receptor modulators (for example, tamoxifen) and the aromatase inhibitors (ais—for example, anastrozole, letrozole, exemestane). The “gold standard” of 5 years of adjuvant tamoxifen has clearly been challenged (and some would argue surpassed) by the accumulating evidence from the adjuvant ai trials. In addition to a tolerable side-effect profile, the ai treatment strategies (upfront, switch, or extended adjuvant) have all demonstrated improved disease-free survival over adjuvant tamoxifen. They also offer a reduced incidence of thromboembolic disease and endometrial pathology, but come at the price of increased musculoskeletal symptoms, osteopenia and osteoporosis, and an associated increased fracture rate. Follow-up data from the Arimidex, Tamoxifen, Alone or in Combination (atac) trial were just presented at the 2007 San Antonio Breast Cancer Symposium (sabcs) and have recently been published1. With a median follow-up of 100 months, atac is the longest-running ai trial to date, and its data continue to demonstrate improved efficacy for 5 years of upfront anastrozole over tamoxifen alone. As was seen in the last atac update and in the tamoxifen overview data, the absolute difference in time to recurrence (ttr) continues to increase even after hormonal treatment is stopped at 5 years—the “carryover effect”: At 5 years: 2.8% ttr (anastrozole 9.7% vs. tamoxifen 12.5%) At 9 years: 4.8% ttr (anastrozole 17.0% vs. tamoxifen 21.8%) Recurrence rates remained lower on anastrozole as compared with tamoxifen after treatment completion [hazard ratio (hr): 0.75; 95% confidence interval: 0.61 to 0.94; p = 0.01] Although a nonsignificant numeric excess of non-breast cancer deaths in patients without a breast cancer recurrence was noted in the anastrozole group, no difference in overall survival was observed (hr: 0.97; p = 0.7). No unexpected toxicity data were reported, and after the intended duration of endocrine therapy had been completed, no “carryover” with respect to increased fracture incidence was seen. Patients actively receiving anastrozole over tamoxifen had a higher annual fracture rate (2.93% vs. 1.90% respectively; incidence rate ratio: 1.55; p < 0.0001), but no difference was observed in those rates after endocrine therapy was complete (1.56% vs. 1.51% respectively; incidence rate ratio: 1.03; p = 0.79). Bisphosphonate use overall—let alone prophylaxis—was low in the trial. The hope is that preemptive measures to identify and treat osteopenia and osteoporosis, as per emerging evidence and evolving clinical guidelines, can further reduce this specific risk, particularly during the active treatment phase. Apart from a continued reduction in endometrial cancer incidence with anastrozole over tamoxifen (0.2% vs. 0.8% respectively), no statistically significant differences in secondary malignancy rates were observed. Also reassuring was the lack of observed differences in cardiovascular morbidity or mortality between the two groups. Although the adjuvant ai trials have provided many answers thus far, many questions remain. No single “ideal” adjuvant endocrine strategy for any given individual—let alone a given population—has emerged. Given differences in trial design, patient populations, medications used, follow-up to date, and lack of a published direct ai strategy comparison, declaring a clear “winner” is difficult. Adding to the complexity, intriguing early follow-up data from the Adjuvant Tamoxifen Longer Against Shorter (atlas) trial presented at sabcs 2007 suggest that longer-term tamoxifen (10 years) may even challenge the original premise of “gold standard” 5 year tamoxifen therapy2. Although trends are suggestive, no single ai trial or adjuvant ai hormonal treatment strategy has yet provided convincing data demonstrating a clear, large, improvement in overall survival beyond that achieved with 5 years of adjuvant tamoxifen alone. All that being said, there is hope. Patients and their clinicians have never had more treatment options at their disposal. And although the therapeutic differences may appear modest, the Early Breast Cancer Trialists’ Collaborative Group (2005–2006) overview dataa and cancer mortality statistics 3,4 demonstrate that successive, modest individual improvements in early detection, local control, and systemic therapy together lead to considerably larger, clinically meaningful differences in efficacy and breast cancer survival, given time. The hope is that, one day, clinicians can offer patients effective, targeted, minimally toxic, individualized therapy based on an integrated understanding of tumoural and patient genetics, pharmacogenetics, and resistance mechanisms, thereby maximizing the predictive and prognostic capabilities of the various technologies available. Until that day materializes, however, the oncology community can anticipate covering large distances with cumulative small steps. As long as we keep heading in the right direction, we are bound to reach our destination.
FSH Stimulation of Leydig Cell Function in the Hypophysectomized Immature Rat

Abstract:Summary FSH treatment of hypophysec-tomized immature rats increased testicular weight, increased the number of LH receptors per testis, and increased the in vitro testicular testosterone production in response to LH. The increase observed in all three parameters was related to the duration of FSH treatment. The testicular response to LH was enhanced to a greater extent than the number of LH receptors. The specificity of the FSH effect was indicated by the observation that administration of LH had no effect on testicular weight or number of LH receptors. LH administration did not increase the in vitro testicular response to LH, but appeared to prevent the decrease in this response observed in saline-injected controls. Intralesional treatment for keloids and hypertrophic scars: a review

Abstract:Background: Although differing in clinical presentation and prognosis, keloids and hypertrophic scars are generally characterized by abnormally proliferative scar tissue and are extremely debilitating. Several intralesional therapies have been studied in attempts to find a universally safe and effective modality, of which there are currently none. Objective: To provide a comprehensive review of current intralesional treatment modalities for keloids and hypertrophic scars. Methods and materials: A PubMed search was performed for literature pertaining to intralesional treatment modalities for keloids and hypertrophic scars. References from retrieved articles were also considered for review. Results and conclusion: Many intralesional therapies for keloids and hypertrophic scars are currently available to physicians and patients. Mechanisms of action and side effect profiles vary between these agents, and new approaches to keloids and hypertrophic scars are frequently being explored. Randomized controlled trials are needed to assess these new and promising modalities fully.
Scarring and Skin Fibrosis Reversal with Regenerative Surgery and Stem Cell Therapy

Abstract:Skin scarring and fibrosis affect millions of people worldwide, representing a serious clinical problem causing physical and psychological challenges for patients. Stem cell therapy and regenerative surgery represent a new area of treatment focused on promoting the body's natural ability to repair damaged tissue. Adipose-derived stem cells (ASCs) represent an optimal choice for practical regenerative medicine due to their abundance, autologous tissue origin, non-immunogenicity, and ease of access with minimal morbidity for patients. This review of the literature explores the current body of evidence around the use of ASCs-based regenerative strategies for the treatment of scarring and skin fibrosis, exploring the different surgical approaches and their application in multiple fibrotic skin conditions. Human, animal, and in vitro studies demonstrate that ASCs present potentialities in modifying scar tissue and fibrosis by suppressing extracellular matrix (ECM) synthesis and promoting the degradation of their constituents. Through softening skin fibrosis, function and overall quality of life may be considerably enhanced in different patient cohorts presenting with scar-related symptoms. The use of stem cell therapies for skin scar repair and regeneration represents a paradigm shift, offering potential alternative therapeutic avenues for fibrosis, a condition that currently lacks a cure.

Using dual-path allocation for partial traffic protection in Elastic Optical Networks

Abstract:Multi-path allocation has been proposed as a simple way to achieve traffic protection in different networking domains. In this work dual-path allocation is proposed for Elastic Optical Networks (EON) assuming dynamic offered traffic, and its performance is quantified by simulation. Indeed, dual-path allocation is compared with partial dedicated path protection assuming different scenarios. In addition to a qualitative comparison where advantages and drawbacks of both schemes are analysed, quantitative results for different backbone network scenarios are presented.
Involvement of norepinephrine in pituitary LH release induced by oestradiol in vitro.

Abstract:The direct effects of oestradiol-17 beta (E2) on pituitary luteinizing hormone (LH) release and the role of norepinephrine (NE) in E2-induced gonadotrophin release were examined in a sequential double chamber perifusion system by perifusing the mediobasal hypothalami (MBH) and/or pituitaries excised from normally cycling female rats. Administration of E2 induced significant release (70-160% increase, P less than 0.05) of LH from the pituitary of rats in pro-oestrus, but not in other stages of the oestrous cycle. When the MBH and the pituitary were perifused in sequence, E2 induced significant release of LH in all stages of the oestrous cycle except oestrus. When the pituitary from rats in dioestrus II was perifused alone with medium containing 200 ng/ml NE, significant release of LH (80-170% increase, P less than 0.05) was observed after the administration of E2. The E2-induced LH release in pro-oestrus was completely abolished by perifusion with medium containing diethyldithiocarbamate, an inhibitor of NE synthesis. These findings suggest that NE may be involved in changes of pituitary responsiveness in LH secretion to oestrogen during the rat oestrous cycle.
Systematic Review: Adipose-Derived Mesenchymal Stem Cells, Platelet-Rich Plasma and Biomaterials as New Regenerative Strategies in Chronic Skin Wounds and Soft Tissue Defects

Abstract:The number of clinical trials evaluating adipose-derived mesenchymal stem cells (AD-MSCs), platelet-rich plasma (PRP), and biomaterials efficacy in regenerative plastic surgery has exponentially increased during the last ten years. AD-MSCs are easily accessible from various fat depots and show intrinsic plasticity in giving rise to cell types involved in wound healing and angiogenesis. AD-MSCs have been used in the treatment of soft tissue defects and chronic wounds, employed in conjunction with a fat grafting technique or with dermal substitute scaffolds and platelet-rich plasma. In this systematic review, an overview of the current knowledge on this topic has been provided, based on existing studies and the authors’ experience. A multistep search of the PubMed, MEDLINE, Embase, PreMEDLINE, Ebase, CINAHL, PsycINFO, Clinicaltrials.gov, Scopus database, and Cochrane databases has been performed to identify papers on AD-MSCs, PRP, and biomaterials used in soft tissue defects and chronic wounds. Of the 2136 articles initially identified, 422 articles focusing on regenerative strategies in wound healing were selected and, consequently, only 278 articles apparently related to AD-MSC, PRP, and biomaterials were initially assessed for eligibility. Of these, 85 articles were excluded as pre-clinical, experimental, and in vitro studies. For the above-mentioned reasons, 193 articles were selected; of this amount, 121 letters, expert opinions, commentary, and editorials were removed. The remaining 72 articles, strictly regarding the use of AD-MSCs, PRP, and biomaterials in chronic skin wounds and soft tissue defects, were analyzed. The studies included had to match predetermined criteria according to the patients, intervention, comparator, outcomes, and study design (PICOS) approach. The information analyzed highlights the safety and efficacy of AD-MSCs, PRP, and biomaterials on soft tissue defects and chronic wounds, without major side effects.

Abstract:Significance: This body of work gives a concise and comprehensive overview for the clinician and scientist on the latest treatment modalities for hypertrophic scars (HTS) and keloids in the pediatric population, as well as the most promising methods of prevention currently being investigated. This review will serve as a guide to the clinician for treatment selection and as an efficient tool for the scientist to achieve a comprehensive overview of the scientific literature to guide their future experiments aimed at pathologic scar prevention. Recent Advances: Current studies in the literature suggest carbon dioxide (CO2) laser and E-light (bipolar radiofrequency, intense pulsed light, and cooling) are two of the most effective treatment modalities for HTS while surgical excision + CO2 laser + triamcinolone injection was one of the most successful treatments for keloids. In animal models, drug impregnated electrospun nanofiber dressings offer encouraging results for HTS prevention while Kelulut honey showed promising results for keloid prevention. Critical Issues: Treatment outcome reproducibility is hindered by small cohorts of patients, inadequate follow up, and variability in assessment tools. Prevention studies show multiple ways of achieving the same result yet fall short of complete prevention. Furthermore, some studies that have purported full prevention have not been validated. Future Directions: In order to establish a standard of care, large clinical trials of the most successful modalities in small cohorts are needed. The key for prevention will be validation in animal models of the most successful methods, followed by translational and clinical studies.

Evidence of Potential Natural Products for the Management of Hypertrophic Scars

Abstract:Hypertrophic scarring is an aberrant wound-healing response to reestablish dermal integrity after an injury and can cause significant abnormalities in physical, aesthetic, functional, and psychological symptoms, impacting the patient's quality of life. There is currently no gold standard for preventing and treating hypertrophic scars. Therefore, many researchers have attempted to search for antihypertrophic scar agents with greater efficacy and fewer side effects. Natural therapeutics are becoming attractive as potential alternative anti-scarring agents because of their high efficacy, safety, biocompatibility, low cost, and easy accessibility. This review demonstrates various kinds of natural product-based therapeutics, including onion, vitamin E, Gotu kola, green tea, resveratrol, emodin, curcumin, and others, in terms of their mechanisms of action, evidence of efficacy and safety, advantages, and disadvantages when used as anti-scarring agents. We reviewed the literature based on data from in vitro, in vivo, and clinical trials. A total of 23 clinical trials were identified in this review; most clinical trials were ranked as having uncertain results (level of evidence 2b; n = 16). Although these natural products showed beneficial effects in both in vitro and in vivo studies of potential anti-scarring agents, there was limited clinical evidence to support their efficacy due to the limited quality of the studies, with individual flaws including small sample sizes, poor randomization, and blinding, and short follow-up durations. More robust and well-designed clinical trials with large-scale and prolonged follow-up durations are required to clarify the benefits and risks of these agents.

Abstract:Abstract Background Striae distensae is a disfiguring atrophic skin condition that impairs the body’s aesthetic image. Despite the variety of conducted studies, there is controversy regarding the best modalities. Human mesenchymal stem cells are considered a rich source for scar treatment. Skin needling is among the most efficient and safe aesthetic and therapeutic devices. This study aimed to evaluate the efficacy of the combination of needling and intradermal injection of mesenchymal stem cells compared to skin needling alone for treating striae distensae. Method This study was a randomized, double-blind clinical trial involving 10 women aged 18–60. Each striae lesion was divided into two parts, with one side receiving needling and intradermal injection of conditioned medium, while the other side received needling and intradermal injection of normal saline. This treatment was administered in three sessions with three-week intervals. Patients were evaluated before the first intervention and three months after the final session. Three months after the completion of the intervention, patients’ lesions were evaluated using biometric criteria, physician evaluation, and patient self-assessment. Results The results demonstrated a significant improvement in dermal and complete thickness and skin density in patients treated with microneedling. All skin ultrasound parameters improved significantly in patients receiving the combination of needling and conditioned medium. When comparing the two groups, significantly higher physician and patient satisfaction was observed in the combination group. However, the comparison of biometric indices improvement wasn’t significant between these groups. Conclusion The combination of human mesenchymal stem cells with microneedling could be considered a novel effective option for stretch marks.
