Abstract:Objective To summarize the clinical manifestations and outcomes of neonatal hepatoblastoma for its improved recognization and more effective treatment.Methods For this retrospective study,8 patients with neonatal hepotoblastoma were admitted from 2007 to 2014.Only neonates and infants aged under 2 months with adequate clinicopathologic data were accepted for review.There were 6 boys and 2 girls with a mean diagnostic age of 24.75 ± 17.62 (2-50) days.The mean birth weight at diagnosis was 3.42 ± 0.47 (2.6-4.0) kg.Seven tumors were derived from right hepatic lobe while another from left hepatic lobe.According to the criteria of PRETEXT,the stages were Ⅰ (n =1),Ⅱ (n =1) and Ⅲ (n =6).According to the histology classification,all tumors were of epithelial type.One patient was of pure fetal histology subtype while the remainder mixed fetal & embryonal subtype.Patients with stages Ⅰ and Ⅱ underwent primary operation while those with stage Ⅲ open biopsy and neoadjuvant chemotherapy.Five patients had delayed operation.The mean interval of preoperative chemotherapy was 4.17 ± 1.16 cycles,median 4 cycles.The mean time of initiating postoperative chemotherapy was 22.00 ± 9.42 days,median 23.5 days.The mean interval of postoperative chemotherapy was 5.00 ± 2.52 cycles,median 4.5cycles.Results Right hepatic duct was injured and subsequently repaired with cholangio-jejunostomy in 1 case.There were no other complications.Two tumors recurred postoperatively.Another 5 patients survived tumor-free.The median period of follow-up was 29.5 months.The 2,5-year overall survivals were 75％ and 60％ respectively.Conclusions Normal term newborns with hepatoblastoma have a good prognosis after standard treatments.Neonatal hepatoblastoma is rare so that it is necessary to conduct a multicenter and multidisciplinary study.
Clinical Characteristics and Outcomes of Neonatal Hepatoblastoma: A Single Center Study

Abstract:Objective: Hepatoblastoma (HB) diagnosed within one month following birth qualifies for a diagnosis of neonatal HB, whose prognosis is reportedly controversial, and its treatment is challenging. This study discussed the diagnosis, treatment, and outcomes of neonatal HB at a single center so as to enhance its overall management in the future. Methods: The clinical information of babies diagnosed with neonatal HB at our center from August 2009 to September 2023 were retrospectively analyzed for demographics, clinical features, therapy, and outcomes. The outcomes were estimated by the Kaplan-Meier analysis method. Results: The study comprised 79 patients aged less than one year old, among which 14 had neonatal HB whereas 65 were non-neonatal HB patients. No differences were found between groups regarding gender, birth weight, delivery details, parental age, clinical signs, or treatment strategies. Neonatal HB patients were more likely to have PRETEXT I-II, smaller tumor size, congenital diseases, and lower risk tumor grade (p < 0.05). Additionally, the AFP levels of all neonatal HB patients were greater than 10,000 ng/ml (p = 0.009) and they had higher levels of ferritin (p = 0.003) and hemoglobin (p = 0.021), but lower levels of serum total proteins (p = 0.001). The three-year survival rate (100% vs 90.8%) and three-year event-free survival rate (100% vs 86.2%) in the neonatal HB group were higher than the non-neonatal HB group. Conclusion: Neonatal HB patients have unique clinical features and can achieve an excellent prognosis following combined treatment with surgery and chemotherapy. Tumor resection, when carefully performed, was safe even in babies younger than one months old. Further and long-term studies are needed from a larger neonatal HB population. A case report of congenital hepatoblastoma

Abstract:Introduction: Reports of congenital hepatoblastoma are rare, and there is limited experience in its management. Case presentation: We present a challenging case of congenital hepatoblastoma that was large at the time of presentation, occupying the first and second hepatic portals and deemed inoperable. Although liver tumors was detected in the child during the mother's pregnancy, the initial diagnosis was hepatic hemangioma. The diagnosis of hepatoblastoma was ultimately confirmed after a biopsy. Neoadjuvant chemotherapy, guided by 3D visual analysis based on enhanced CT, enabled successful block resection of the tumor. Despite a transient cholestatic parcel effusion post-operation, the child achieved good therapeutic outcomes with subsequent drainage and chemotherapy. Discussion: Regular monitoring of alpha-fetoprotein (AFP) levels and performing abdominal ultrasounds are useful for the differential diagnosis of liver tumors; however, pathology remains the gold standard for confirming malignancy. Chemotherapy is safe and effective for treating congenital hepatoblastoma in the perinatal period. 3D visual analysis is valuable tools in performing surgeries on children with large, strategically positioned tumors. Lens culinaris agglutinin-reactive fraction of AFP (AFP-L3) has been assessed for its adjuvant therapeutic efficacy in adult hepatocellular carcinoma, and we have preliminarily investigated its potential role in evaluating the treatment efficacy of congenital hepatoblastoma. Conclusion: Puncture biopsy is a definitive and safe diagnostic method for congenital hepatoblastoma, while neoadjuvant chemotherapy is effective and facilitates subsequent complete tumor resection. Additionally, 3D visual analysis shows significant potential in the surgical treatment of pediatric liver masses.

Abstract:Congenital leukemias are a rare group of hematologic neoplasms with a wide range of clinical signs and symptoms. Here we reported a neonate presenting with jaundice, pleural effusion and ascites. The total protein and serum albumin were markedly low at 48 and 12g/L. Computerized tomography showed the density of liver was asymmetry with several hypoechoic regions. Initial blood routine examination revealed only thrombocytopenia while blood white cells increased to 30.0x10<SU9</SU/L with 17% blast cells several days later. Bone marrow biopsy showed the proportion of blasts and promonocytes increased and she was diagnosed as acute monoblastic leukemia.</.

Abstract:Primary liver tumours in neonates with single-ventricle palliation are exceedingly rare. We present the first reported case of neonatal hepatoblastoma with severe Ebstein's anomaly following Starnes procedure. The patient's postoperative course highlights the challenges and complications in simultaneous management of these diagnoses. Transition from shunted single-ventricle physiology to bidirectional cavopulmonary connection improved end-organ function, permitting more aggressive hepatic malignancy treatment.

Diagnosis and Treatment of Hepatoblastoma in Children

Abstract:Objective To explore the consultation issues of hepatoblastoma in children and the new consideration.Methods From Apr.2006 to Mar.2010,15 children were diagnosed as hepatoblastoma(12 cases were male and 3 cases were female)by pathological examination.The medium age at diagnosis was 1.83 years old(ranged 0.5-11.5 years old).Serum alpha fetoprotein(AFP)level in 14 patients markedly increased,and 1 case had normal serum AFP level.Two cases had this disease of stage Ⅰ,1 case with stage Ⅱ,5 cases with stage Ⅲ,7 cases with stage Ⅳ.Pathological types:4 cases were epithelioid,4 cases were embryonic types,1 case was separate vane type,and 6 cases were composite types.Three patients had primary surgery while 11 patients had pre-operation chemotherapy before surgery,and 1 patient gave up treatment.Twelve cases had operation after 1-4 periods of chemotherapy.AEP/ACP chemotherapy therapy was used totally,1 patient received high dose of chemotherapy combined with autologous peripheral blood stem cell transplantation because of extensive diversion.Results Thirteen patients responded to treatment(86.6%)and 2 cases failed.Eleven patients achieved complete remission after treatment,and 2 cases had partial remission.By Mar.31st,2010,except for 1 dead case and 1 case of missing follow up information were elimintia-ted,8 patients(69.2%,9/13 cases)survived without any event.The overall survival rate was 76.9%(10/13 cases).Conclusions Children with hepatoblastoma are very sensitivie to chemotherapy.High dose of chemotherapy combined with autologous peripheralstem cell transplantation is a good way to treat the patients with stage Ⅳ hepatoblastoma.It can prolong hopefully the survival time hopefully and increase the clinical remission rate.
Two-stage Laparoscopic Resection of Giant Hepatoblastoma in Infants Combined with Liver Partial Partition and Artery Ligation

Abstract:Purpose Laparoscopic resection of giant hepatoblastoma (HB) in children has long been a subject of controversy. Here, a new procedure of two-stage laparoscopic resection of giant HB in infants was firstly reported and the feasibility was discussed. Methods The clinical data of three infants with HB were retrospectively reviewed, all of which received 3–5 cycles of neoadjuvant chemotherapy. Stage 1 laparoscopic selective hepatic artery ligation and liver partial partition were performed. Stage 2 laparoscopic hepatectomy was performed 2 weeks later. Results The results demonstrated that (1) the tumors shrank considerably in size and had relatively clear boundaries after neoadjuvant chemotherapy; (2) after stage 1 surgery, the tumor volume further reduced, while the intratumoral necrosis expanded; (3) 2 weeks later, stage 2 laparoscopic hepatectomy was performed successfully; (4) none of the cases had intraoperative complications such as tumor rupture, air embolism, hemorrhage, biliary fistula, or liver failure, and there was no recurrence or metastasis during follow-up. Conclusions Two-stage laparoscopic hepatectomy associating selective hepatic artery ligation and liver partial partition for HB in infants has the benefits of small invasiveness, fast recovery, improved safety, and high feasibility. However, more cases and longer follow-up are needed to assess its long-term efficacy.
Neuroendocrine Carcinoma in the Extrahepatic Biliary Tract

Abstract:Neuroendocrine carcinoma (NEC) arising from the extrahepatic biliary tracts (EHBTs) is rare, and thus its management and prognosis remain poorly clarified. We herein describe a case of NEC in the perihilar EHBTs, and review the literature, together with a comparison between NECs in the perihilar and distal EHBTs, to elucidate the management strategy and oncological outcome of this rare entity.A 62-year-old Chinese male was admitted with complaints of painless jaundice. Imaging studies revealed a 2-cm mass in the hepatic hilum, regional lymph node involvement, and severe stenosis at the junction of the common hepatic ducts.The histopathological examination of the resected specimen demonstrated small tumor cells with round hperchromatic nuclei and scant cytoplasm. A detailed immunohistochemical analysis showed that the tumor was strongly positive for synaptophysin, CD56 and chromogranin A, with a Ki-67 labeling index greater than 80%. These results led to a diagnosis of NEC in the perihilar bile duct.The patient underwent surgical resection including a left hemihepatectomy, cholecystectomy, lymphadenectomy and Roux-en-Y hepaticojejunostomy.During the two months of follow-up, repeated imaging studies indicated tumor recurrence in the liver. The patient died 6 months after surgery.NEC in the EHBTs is extremely challenging to diagnose preoperatively because of mimicking other bile duct cancers. The prognosis of this disease entity is dismal, and most patients die within 2 years after diagnosis. Subtyping of NECs into perihilar NECs and distal NECs is beneficial for clinical applications, including guiding therapy selection and predicting survival.
Retrospective Analysis of Childhood Hepatoblastoma in a Single Centre in China.

Abstract:Aims: To investigate the protocol efficacy and prognostic factors for paediatric hepatoblastoma in a multidisciplinary model in our centre. Materials and methods: Consecutive hepatoblastoma patients ( <18 years old) treated at Shanghai Children's Medical Center in China from August 2011 to October 2017 were analysed retrospectively for clinical features, chemotherapy courses, surgical treatment and outcomes. Results: One hundred and four cases of paediatric hepatoblastoma (64 males, 40 females; median age at diagnosis 1.64 years) had a median follow-up of 30.68 months (range 8.3-73.3 months). First complete remission was achieved in 95 cases, 85 of which achieved continuous complete remission. Another three cases were lost to follow-up after a median of 24.73 months in complete remission. Seven cases relapsed later, with two achieving a second complete remission and four deaths. Nine cases did not achieve complete remission and five of them died. In general, the 5-year overall survival rate and 5-year event-free survival (EFS) rate were 86.3 +/- 5.0% and 81.8 +/- 4.3%, respectively. Thirty-two cases were classified as standard risk and 72 as high risk with 5-year EFS of 96.8 +/- 3.2% and 75.7 +/- 5.7% (P = 0.029) and 5-year overall survival of 100% and 80.5 +/- 7.0%, respectively. The mean platelet count (P = 0.0036), lactate dehydrogenase (P = 0.0443) and ferritin level (P = 0.0006) at diagnosis were much higher in the high-risk group than in the standard-risk group. Univariate analysis showed that patients <5 years of age (P = 0.018), with higher alpha-fetoprotein (AFP) level ( >100 ng/ml, P = 0.008), without metastases at diagnosis (P = 0.001) and postoperative AFP recovery after no more than three chemocycles (P = 0.014) had better overall survivalin addition, the above factors, except metastases at diagnosis and risk group, were associated with prognosis in the multivariate analysis. Conclusions: The result of this protocol had similar overall survival and EFS rates compared with those in developed countries. Abstract:This study reports a case of hepatoblastoma with onset at 30-weeks' gestation and rapid growth rate. The postnatal enhanced CT confirmed an intrahepatic mass with a size of 8.5 cm × 6.6 cm and a clear boundary accompanied by uneven enhancement, displacement, and narrow lumen of the hepatic vein due to compression. The alpha-fetoprotein (AFP) at birth was 1,002,632 ng/ml (normal level 48,406 [±34,718] ng/ml). A diagnosis of congenital hepatoblastoma was established based on the imaging and laboratory outcomes. The infant received chemotherapy of Cisplatin-5 fluorouracil-Vincristine (C5V) on the fourth day after birth. After four courses of C5V, a complete tumor resection was performed, and the postoperative pathology was consistent with mixed epithelial and mesenchymal hepatoblastoma. Four more courses of C5V and one course of C5VD (C5V plus doxorubicin) followed the surgery. Infectious diarrhea and acute kidney injury (stage I) occurred during chemotherapy, which recovered after anti-infection and symptomatic treatment. The patient is currently 2 years old and still in complete remission. In this case, the onset of hepatoblastoma was early, and the tumor grew rapidly, resulting in an obvious compression effect. Chemotherapy was started early after birth, and the curative effect was satisfactory, suggesting that the hepatoblastoma based on clinical diagnosis with rapid tumor progression and severe dysfunction of surrounding organs caused by compression should undergo chemotherapy as soon as possible if a pathological diagnosis cannot be obtained temporarily, which also plays an important role in improving the complete resection rate of intraoperative tumor and reducing the recurrence rate of postoperative tumor.

Abstract:Hepatoblastoma (HB) remains the most common paediatric liver tumour and survival in children with hepatoblastoma has improved considerably since the advent of sequential surgical regimens of chemotherapy based on platinum-based chemotherapeutic agents in the 1980s. With the advent of modern diagnostic imaging and pathology techniques, new preoperative chemotherapy regimens and the maturation of surgical techniques, new diagnostic and treatment options for patients with hepatoblastoma have emerged and international collaborations are investigating the latest diagnostic approaches, chemotherapy drug combinations and surgical strategies. Diagnosis of hepatoblastoma relies on imaging studies (such as ultrasound, computed tomography, and magnetic resonance imaging), alpha-fetoprotein (AFP) levels, and histological confirmation through biopsy. The standard treatment approach involves a multimodal strategy with neoadjuvant chemotherapy followed by surgical resection. In cases where complete resection is not feasible or tumors exhibit invasive characteristics, liver transplantation is considered. The management of metastatic and recurrent hepatoblastoma poses significant challenges, and ongoing research focuses on developing targeted therapies and exploring the potential of immunotherapy. Further studies are necessary to gain a better understanding of the etiology of hepatoblastoma, develop prevention strategies, and personalize treatment approaches. We aim to review the current status of diagnosis and treatment of hepatoblastoma.
Successful Treatment for Hepatoblastoma in Trisomy 18: A Case Report

Abstract:Children with trisomy 18 tend to develop hepatoblastoma. Since the introduction of appropriate management for organ malfunction, individuals with trisomy 18 have come to have a longer life expectancy. However, the predisposition to hepatoblastoma becomes a significant issue for the quality of a case. Here, we present a rare multifocal hepatoblastoma involving predominantly Couinaud segments 5 and 7 in a 10-month-old boy with trisomy 18. Though the first-line cisplatin monotherapy resulted in unsatisfactory tumor shrinkage, the second-line neoadjuvant chemotherapy administrating irinotecan and vincristine gave rise to significant tumor reduction in volume, leading to the completion of partial resection of the liver without the microscopic residual disease. The patient has been free from recurrence for 44 months. Because anatomical right hepatectomy can cause circulatory instability, including acute onset of pulmonary hypertension in trisomy 18 patients, physicians should balance treatment benefits and potential adverse effects. Our successful experience utilizing a combination of efficacious and less cardiotoxic neoadjuvant chemotherapy followed by the partial hepatectomy encourages physicians to treat a patient with trisomy 18 and tackle hepatoblastoma with a genetic background.

Clinical characteristics and prognosis of 52 cases with Ⅳ stage hepatoblastoma in children

Abstract:Objective To study the diagnostic and treatment characteristics and the prognosis of Ⅳ stage hepatoblastoma(HB) by summing up the clinical data of the disease.Methods Fifty-two cases of Ⅳ stage HB were collected at Beijing Tongren Hospital Affiliated to Capital Medical University from September 2006 to June 2014 and the clinical stage was confirmed according to Children's Cancer Group(Pediatric Oncology Group).There were 30 male cases and 22 female cases.The median age was 1.91 years old (1 month-12 years and 7 months old).The main therapy for hepatoblastoma was surgery and chemotherapy.The regimens of chemotherapy were based on Children Oncology Group (COG) scheme.The clinical efficacy, and relapse rate and recurrence rate and prognosis factors were analyzed by SPSS 19.0 software.Results (1) Age characters : in 52 cases, 0-12 months sick infants in 16 cases (30.76％), 13-36 months old sick infants in 24 cases (46.15％), ＞3-6 years old sick children in 9 cases (17.31％) ,and ＞ 6-12 years old sick children in 3 cases accounting for only 5.78％.(2) Metastasis site included distant organs metastasis in 40 cases (76.92％) ,and adjacent tissues and organs metastasis in 12 cases (23.08％).(3) Follow-up studies were conducted till January 31st,2015 ,the medium follow-up time was 13 months (3-79 months), the 3-year free event survival rate(EFS) was 32.69％ (17/52 cases) ,the 3-year overall survival rate (OS) was 61.53％ (32/52 cases),and the mortality was 38.46％ (20/52 cases).(4)In 52 cases,31 cases had relapse during or after treatment,accounting for 59.61％.Among in relapse cases,the average survival time was (45.23 ± 4.72)months, and 95％ confidence interval was 35.98-54.48 months.2-year EFS was 23.07％ (12/52 cases).The 3-year and 5-year OS in relapse cases was 32.69％ (17/52 cases) ,and 15.38％ (8/52 cases).Conclusions Although the relapse risk of Ⅳ stage HB cases is high and the prognosis is relatively poor, the clinical remission rate can be improved by using personalized and comprehensive treatment methods through evaluating individual conditions.The comprehensive treatment can enhance the treatment confidence, prolong survival time and improve the clinical efficacy.
Primary hepatic neuroendocrine carcinoma: report of two cases and literature review

Abstract:Abstract Background Primary hepatic neuroendocrine carcinoma (PHNEC) is extremely rare. The diagnosis of PHNEC remains challenging—partly due to its rarity, and partly due to its lack of unique clinical features. Available treatment options for PHNEC include surgical resection of the liver tumor(s), radiotherapy, liver transplant, transcatheter arterial chemoembolization (TACE), and administration of somatostatin analogues. Case presentation We report two male PHNEC cases and discuss the diagnosis and treatment options. Both cases presented with abdominal pain; case two also presented with symptoms of jaundice. The initial diagnosis for both cases was poorly differentiated grade 3 small-cell neuroendocrine carcinoma, based on imaging characteristics and the pathology of liver biopsies. Final diagnoses of PHNEC were arrived at by ruling out non-hepatic origins. Case one presented with a large tumor in the right liver lobe, and the patient was treated with TACE. Case two presented with tumors in both liver lobes, invasions into the left branch of hepatic portal vein, and metastasis in the hepatic hilar lymph node. This patient was ineligible for TACE and was allergic to the somatostatin analogue octreotide. This limited treatment options to supportive therapies such as albumin supplementation for liver protection. Patient one and two died at 61 and 109 days, respectively, following initial hospital admission. Conclusions We diagnosed both cases with poorly differentiated grade 3 small-cell PHNEC through imaging characteristics, immunohistochemical staining of liver biopsies, and examinations to eliminate non-hepatic origins. Neither TACE nor liver protection appeared to significantly extend survival time of the two patients, suggesting these treatments may be inadequate to improve survival of patients with poorly differentiated grade 3 small-cell PHNEC. The prognosis of poorly differentiated grade 3 small-cell PHNEC is poor due to limited and ineffective treatment options.

Abstract:Skp1 an essential component of the SCF (Skp1/cullin/F-box) E3 ubiquitin ligases, which target proteins for degradation by the 26S proteasome. We generated a skp1dM mutant strain that is defective for galactose induction of the GAL1 gene and we have found that galactose-induced protein degradation of the repressor Mig2 is defective in this strain. Mig2 degradation was also abolished in cells lacking the protein kinase Snf1 and the F-box protein Das1, suggesting that Snf1 triggers galactose-induced protein degradation of Mig2 by SCFDas1. Chromatin immunoprecipitation showed that Mig2 associates with the GAL1 promoter upon the galactose-induced exit of Mig1 in skp1dM cells, but not in wild-type cells, suggesting that the conditional degradation of Mig2 is required to prevent it from binding to the GAL1 promoter under inducing conditions. A galactose-stable deletion derivative of Mig2 caused a strong Mig (multi-copy inhibition of GAL gene expression) phenotype, confirming that galactose induction of the GAL1 gene requires the degradation of the repressor Mig2. Our results shed new light on the conflicting reports about the functional role of the degradation of transcriptional activators and indicate that gene expression studies interfering with proteasome degradation should take the stabilization of potential repressors into account.

Abstract:Hepatoblastoma (HB) frequently presents at an advanced and unresectable stage. Transcatheter arterial chemoembolization (TACE) had been attempted to improve the feasibility of removing the bulky tumor in the authors' hospital and the results were presented here to evaluate the effectiveness and therapeutic role of TACE in the HB infants. Eight patients (6 boys, 2 girls), ranging in age from 2 months to 12 months, had unresectable HB based on clinical manifestation, B-ultrasound (B-US), chest X-ray film, computed tomography (CT), blood chemistry, and serum alpha-fetoprotein (AFP), and were subjected to TACE 1-3 times. On each TACE, Adriamycin (ADR, 20 mg/m2), vincristine (VCR, 1.5 mg/m2), and cisplatin (CDDP, 40 mg/m2) dispersed in 5-10 mL lipiodol were infused into the tumor, and stainless-steel embolization coils were released into the main feeding artery until completely embolized. Then, all the patients were reexamined once a month. Digital subtractive angiography was performed and the therapeutic strategy of further TACE or surgery was individualized in terms of the changes of tumor stain and the newly forming feeding artery. Six children (75%) had a marked response after the first TACE and were judged as being surgically resectable, but one boy died of pneumonia just before the scheduled operation and another boy preferred further TACE. The other 2 patients had only a partial response and required further TACE before the operation. Thus 6 children eventually underwent complete surgical resection and 1 boy achieved successful disappearance of tumor after 3 episodes of TACE alone. Seven children had an excellent recovery and remained tumor-free for 15-49 months. The results indicate that TACE is an effective and useful preoperative therapeutic choice for unresectable hepatoblastoma, and can improve the resectablity of the bulky tumor and the survival rate of HB patients. Multiple TACE could enhance the therapeutic effect and should be considered if indicated.

Abstract:Background Almost all hepatoblastomas are isolated to the liver. Hepatoblastoma arising from and limited to extrahepatic tissue is an extremely rare clinical entity. Case presentation Here we present a case of a 7-year-old Chinese boy of Han ethnicity with hepatoblastoma originating from the hepatogastric ligament. A complete resection was performed and the entire course was uneventful. He received six cycles of postoperative chemotherapy and had no signs of recurrence for 3 years after surgery. Conclusions Hepatoblastoma arising from extrahepatic tissue is extremely rare. A pedunculated hepatoblastoma is prone to hemorrhage and tumor metastasis. The best treatment for a long-term cure is complete resection of the primary tumor combined with chemotherapy.

Abstract:Purpose: Recently, children with trisomy 18 have been receiving more active treatment for malignancies. We report herein seven cases complete resection was achieved, and discuss multidisciplinary treatment for hepatoblastoma in patients with trisomy 18. Method: The medical records of children with trisomy 18 who were treated at the study center between 2010 and 2023 were reviewed. Result: Six of 69 patients had hepatoblastoma development, and three of these underwent multidisciplinary treatment. In addition, 6 patients had been referred by another hospital for treatment, and four of these underwent multidisciplinary treatment. Among the seven patients who underwent multidisciplinary treatment, three, two, and two were categorized in Pre-treatment Extent of Disease (PRETEXT) classification group I, II, and III, respectively. Neoadjuvant chemotherapy resulting in tumor reduction was performed in three cases. In all the cases, complete resection was achieved with pathologically safe margins. Perioperative complications included circulatory failure in one case and bile leakage in two cases. Adjuvant chemotherapy was administered in four cases. The postoperative observation period ranged from 3 months to 11 years, and all the patients are recurrence-free. Conclusion: Children with trisomy 18 complicated with hepatoblastoma whose cardiopulmonary conditions are stable may be good candidates for chemotherapy and surgery.

Abstract:Abstract Background This study explores and analyzes the clinical characteristics and prognostic factors of hepatoblastoma (HB) in children under 6 years old and establishes a new risk-stratification system for individualized therapy. Methods The clinical data of 382 pediatric patients under 6 years old (231 males and 151 females) who had been diagnosed with HB by pathology between May 2005 and May 2019 were collected. By analyzing the risk factors influencing the survival rate of patients with HB, a new risk-stratification system was established, and it was compared with previous risk-stratification systems by a receiver operating characteristic (ROC) curve. Results According to a Kaplan-Meier survival analysis, the one-year, three-year, and five-year overall survival (OS) was 93.7, 84.0, and 73.9%, respectively, and the event-free survival (EFS) was 90.5, 79.2, and 67.5%, respectively. The independent risk factors influencing prognosis in pediatric patients with HB were alpha-fetoprotein (AFP) 1000 ng/ml (HR = 3.341, P = 0.005); platelet count > 400 × 10 9 /L (pooled hazard ratio [HR] = 2.123, P = 0.026); PRETEXT stage IV (HR = 4.026, P = 0.001); vascular involvement (HR = 2.178, P = 0.019); distant metastasis (HR = 2.634, P = 0.010);and multifocality (HR = 2.215, P = 0.012). A new risk-stratification system was established and divided into three groups: low risk, moderate risk, and high risk. There were statistical differences among the three groups ( P = 0.002). Compared with the previous risk-staging systems, there was no significant difference in the survival rate. Although the effect in the guiding therapy was the same, the area under the curve for the ROC curve was 0.835 (95% CI: 0.784–0.885) for the new stratification system. Conclusion This new risk-stratification system had a better predictive value for the prognosis of pediatric patients with HB than other stratification systems.

Abstract:Background: Cases of large-cell neuroendocrine carcinoma (NEC) concomitant hepatocellular carcinoma (HCC) are very rare. Based on the microscopic characteristics, mixed HCC-NEC tumors can be divided into collision type and combined type. We report a patient with both collision and combined type HCC-NEC tumor at the same time. Case presentation: A 58-year-old man with hepatitis B and cirrhosis was found to have two masses in segment 5 and segment 8 of the liver, respectively. Preoperative imaging diagnosis was primary liver cancer. Indocyanine green retention test (ICG R 15) <10% suggested that the patient can tolerate surgery. Partial hepatectomy was performed under the guidance of 3D reconstruction. Postoperative pathology showed that most of the tumors in S5 were large-cell neuroendocrine carcinoma (90%), and a small part were hepatocellular carcinoma (10%). The tumor in S8 of the liver was diagnosed as HCC combined with immunohistochemistry. After surgery, the patient underwent genetic testing, which indicated mutations in TP53 gene. The test of immune markers of the sample suggest that the patient may benefit little from immune checkpoint inhibitor therapy. The cisplatin and etoposide chemotherapy protocol to the patient following their surgery. Eight month later after the operation, Enhanced CT showed there was no recurrence or metastasis of the tumor. Conclusion: The case at hand augments the understanding of HCC-NEC mixed tumors, offering pivotal insights into their precise diagnosis and treatment modalities. Furthermore, we document a favorable prognosis, marked by an absence of recurrence signs thus far—a rarity in comparable instances. This enlightenment stands to facilitate the handling of ensuing cases and enhance patient prognoses.
