Abstract:Different proteolytic enzymes were tested for their ability to degrade the myelin basic protein of the central nervous system, purified in two different forms, the lipid-free form and the lipid-bound form. As shown by SDS gel electrophoresis only clostripain, a thiol protease, was able to distinguish between the two MBPs since it degraded MBP only in the lipid-free form. The failure to degrade lipid-bound MBP by clostripain could not be ascribed to the presence of lipids, since the other proteolytic enzymes tested degraded both MBPs independently from lipids giving fragments with different size. These results may be related to different conformations of MBPs possibly relevant for the study of myelin structure and antigenic properties of the protein.

Abstract:Abstract In this paper, we propose a new approach to the optimal control problem for linear time invariant systems subject to input constraints. The input constraint is described by means of integral quadratic constraints in order to obtain an accurate model for the nonlinearity. The key point of this work is to use the IQC information on the design stage to improve the closed-loop performance. We apply this strategy to the input saturation problem by using a combination of multipliers and propose an algorithm to tune some parameters associated with the IQC. Numerical examples show that this strategy can significantly improve the performance of the system when compared with the circle and Popov criteria.
[Association between the change in the job facilitating and inhibiting factors of care staff due to the employment of older assistant workers and the emotional exhaustion among care staff]

Abstract:Objectives The mental health condition of care staff in Japan is becoming problematic. Older assistant workers are currently being employed to assist care staff with their jobs and alleviate their job burden. This employment of older assistant workers is drawing attention; however, their influence on the job facilitating and inhibiting factors of care staff and the association with the care staff's emotional exhaustion remains unclear. In this study, we aim to examine how the employment of older care assistant workers relates to the job-facilitating and job-inhibiting factors of care staff and explore that association with the care staff's emotional exhaustion.Methods Data from a mail survey of geriatric health services facilities with older assistant workers were analyzed. Among the answers obtained from the care staff, answers from 5,185 who reported working in facilities that employ older assistant workers (over the age of 60) were analyzed. The Emotional Exhaustion subscale of the Japanese Version of the Burnout Questionnaire was used as the dependent variable. The change in job-facilitating and job-inhibiting factors of care staff due to the employment of older assistant workers (improve, maintain/exacerbate) was explored for nine contents.Results Care staff reported a decrease in the total volume of the task (63.6%), less stress during daily tasks (39.8%), and more concentration on the tasks that require expert care knowledge (38.0%). The results of multiple regression analysis showed that the emotional exhaustion score of care staff was low when the total volume of tasks decreased (β=－0.383, 95%CI=－0.719, －0.047), when less stress was perceived during daily tasks (β=－0.432, 95%CI=－0.796, －0.068), when concentration on tasks that required expert care knowledge increased (β=－0.574, 95%CI=－0.937, －0.210), and when human relationships among staff improved (β=－0.871, 95%CI=－1.263, －0.480). Conversely, an increase in tasks requiring work with regional personnel and organizations (β=0.800, 95%CI=0.162, 1.437) was associated with a high emotional exhaustion score.Conclusion The employment of older care workers was related to the job-facilitating or job-inhibiting factors of care staff, and the change in these factors was associated with lower emotional exhaustion. The employment of older personnel may lower the risk of burnout among care staff.
[Relationship between the perceived benefits of working and emotional exhaustion among older assistant care workers: A cross-sectional study]

Abstract:Objectives This study is intended to clarify the perceived benefits of working among older assistant care workers employed in geriatric health service facilities and examine the relationship between perceived benefits and emotional exhaustion.Methods We analyzed data from a mail survey of older assistant care workers employed in geriatric health service facilities that the Japan Association of Geriatric Health Services Facilities conducted in 2020. In this survey, those aged ≥60 were defined as older assistant care workers. Responses were obtained from 1,606 older assistant care workers at 599 facilities. The dependent variable was the emotional exhaustion subscale of the Japanese version of the Burnout Questionnaire. The independent variables were the seven benefits of working as an assistant care worker: 1) I can contribute to society; 2) I am connected to society; 3) I have a sense of purpose in life; 4) I am earning the income I expected since I started working as an assistant care worker; 5) I am learning about care; 6) It has maintained and improved my health; and 7) I can use my time effectively. Latent class analysis (LCA) was performed using these seven benefits as independent variables. Multiple regression analysis was performed using emotional exhaustion and the perceived benefits as dependent and independent variables, respectively. The missing values were supplemented by the multiple imputation method.Results Overall, 1,601 responses were analyzed after excluding 5 respondents who did not answer all the questions. Four patterns were identified from the LCA results: the "benefit-full type," who perceived all benefits; the "benefit-less type," who perceived few benefits; the "benefit-extroverted type," who perceived social contribution and connection as benefits; and the "benefit-introverted type," who perceived health maintenance and improvement and time utilization as benefits. The multiple regression analysis showed that emotional exhaustion scores were higher for the "benefit-extroverted" and "benefit-less" types than for the "benefit-full" type (b=2.465, P<.001 and b=1.931, P<.001, respectively). No difference was found in the case of the "benefit-introverted" type (b=0.050, P=.851).Conclusion The perceptions of diverse and introverted benefits were associated with lower emotional exhaustion scores among older assistant care workers. Future intervention studies are needed to examine whether obtaining diverse or introverted benefits can decrease the emotional exhaustion score.

Abstract:PURPOSE: To examine demographic characteristics and work conditions of home care aides, nursing home aides, and hospital aides in the late 1980s and late 1990s.DESIGN AND METHODS: This study replicated a previous study which examined the Current Population Survey (CPS) March supplement from 1987 to 1989. The present study examined CPS data from 1997 to 1999 both to obtain a profile of aides in the late 1990s and to compare 1990's work conditions and demographic characteristics with those of aides in the late 1980s.RESULTS: Whereas demographic characteristics and work conditions of hospital and nursing home aides show little change over the decade, home care aides today are generally younger, more educated, more likely to hold full-time positions, and more likely to have health benefits than their counterparts 10 years ago. Yet work conditions are still poorer for home care aides than for the other types of aides.IMPLICATIONS: Work conditions need further improvement, particularly for home care aides. More accurate data to describe paraprofessional care workers are also necessary to address the serious worker shortage problems.

Abstract:The study aimed to understand the main skills of older adult caregivers and find ways to improve these skills. We selected participants using a method called random cluster sampling, where caregivers from 17 different medical and nursing care facilities across seven districts in Hangzhou were chosen. We collected 492 valid questionnaires and conducted interviews with 150 people. To analyze the data, we used T -tests and Analysis of Variance (ANOVA) to identify what factors affect caregivers' skills. We also performed multiple regression analysis to explore these factors in more depth. The analysis showed that age ( p = 0.041), annual income ( p < 0.001), and having a training certificate ( p < 0.001) significantly influence the skills of older adult caregivers. Specifically, caregivers' age and whether they had a training certificate were linked to how competent they were, with income being a very strong factor. The study highlighted a gap between the caregivers' current skills and the skills needed for high-quality care. This gap shows the need for training programs that are specifically tailored to the caregivers' diverse needs and cultural backgrounds. Medical and eldercare facilities should adjust their work and educational programs accordingly. It's also important to look at how caregivers are paid to make sure their salary reflects their skills and the quality of care they provide. Finally, it's crucial to integrate a comprehensive training program that leads to certification within eldercare organizations.

Abstract:In the 1960s the Australian government established the aged care programme to provide subsidies to residential aged care providers. Within a couple of decades, Australia had one of the highest proportions in the world of its older citizens in residential care. In the mid-1980s, to remedy this overdependence on residential care, the Australian government introduced the home and community care (HACC) programme. The HACC programme provided care to the frail aged and disabled; and funded the establishment of many small, stand-alone, non-government service providers. Community care developed separately from residential aged care. The legacy of these policy decisions is two sectors that together constitute one large aged care sector providing complementary and sometimes overlapping services but with separate cultures and workforce traditions. Aged care workers are currently employed by a mix of public, private-for-profit and not-for-profit service providers and, compared with the acute care sector, are distributed across a large number of mostly small to medium sized services, operated by small to large organisations, with most having little individual capacity for workforce planning and development. Over recent years the sector has developed a reputation for staffing shortages and high staffturnover. This diversity of settings, variety of ownership types, range of sizes of services and organisations, and reported workforce issues are, arguably, the outcomes of past government policy. The Living Longer Living Better reforms to the aged care sector announced by the Australian government in 2012 introduce a period of reform in aged care and it is timely to examine the similarities and differences across these sectors and to identify the policies and management practices that are likely to enable the industry to develop the resilient and sustainable workforce needed to meet the predicted demand.The Productivity Commission (2011) has predicted that by 2027 there will be a need for 287,000 residential aged care beds (up from 167,000 in 2007) and about 976,000 HACC individual client services (up from 518,000 in 2007). If current ratios of staffto residents and clients remain constant, this will require a 72% increase in residential aged care staffand an 88% growth in community care staff. In 2012, within the aged care programme, there were approximately 5,300 separate funded services operated by about 1,400 providers. There are approximately 3,300 agencies providing HACC services and seven in 10 HACC organisations employ fewer than 20 workers. Collectively the industry employs approximately 133,000 care staffin residential facilities and an additional 74,000 in community care (Australian Institute of Health and Welfare, 2011). This workforce is predominantly female, a majority work part-time with a quarter employed casually or on contract. They have been paid less than other industries and have a lower level of education than the overall Australian workforce (Australian Institute of Health and Welfare, 2011). Considering this workplace profile and low salaries, it is not surprising that reported workforce issues include a claim of continuing shortages of skilled staff, difficulty in recruitment and high turnover. There is also a continuing discussion on the appropriate staffing mix and arguably a poorly coordinated approach to workforce planning. These workforce claims and the potential strategies to address them are worth examining during this period of aged care reform.The most recent list of skills shortages published by the Australian Government Department of Employment, Education and Workplace Relations (DEEWR) (2012b) suggests that in 2011/2012 the 'social and welfare professions' had the second highest rate (across all occupations surveyed that year) of 'proportion of vacancies filled' (83%; and second only to school teachers at 89%) and nurses and midwives had a vacancy fill rate of 75% (an increase of 12% over the previous year and well above the mean of all occupations listed in the report). …

Abstract:There is a critical shortage of geriatrics-prepared health care professionals. In 2002 more than thirty-five million people were age sixty-five and older, and 23 percent of them reported poor or fair health. Older adults use 23 percent of ambulatory care visits and 48 percent of hospital days, and they represent 83 percent of nursing facility residents. Yet 58 percent of baccalaureate nursing programs have no full-time faculty certified in geriatric nursing. Only three of the nation's 145 medical schools have geriatrics departments, and less than 10 percent of these require a geriatrics course. We argue that every health care worker must have some education in geriatrics and access to geriatrics care experts.

Abstract:This article, drawing on a qualitative study in England, the Netherlands and Taiwan, explores how countries with very different care systems address the challenge of securing sufficient numbers of care workers with appropriate skills in ageing care. The analysis exposes the salient features of care systems, ageing-care markets and the ageing-care workforce in the three countries. To support the analysis, examples of how the countries attempt to recruit and retain ageing-care workers are provided. Key findings are that to secure an adequate number of skilled ageing-care workers, job satisfaction, coherence between care and labour policies and equal working conditions across sectors are vital. A quality ageing-care worker requires regular professional supervision and support, resources for ongoing training and career development opportunities. Most importantly, securing a sufficient number of ageing-care workers and ensuring they have appropriate skills are not isolated challenges but are interlinked and require collaboration between a range of care actors.

Abstract:Background: Japan is one of the countries experiencing a "super-aged society." The government has looked to Southeast Asia for recruiting workers to fill the demand for long-term care (LTC) workers. However, migrant LTC workers have faced many job-related stressors. This study aimed to examine the factors associated with job satisfaction and subjective well-being among Filipino, Indonesian, and Vietnamese LTC workers in Japan and explore the specific factors behind what makes them satisfied in their jobs. Methods: A convergent mixed methods study was conducted. The workers were recruited through snowball and convenience sampling and completed a self-administered questionnaire for the quantitative part. The association of the work environment with the workers' job satisfaction and subjective well-being were analyzed using multiple linear regression analysis. An interpretive phenomenological approach was used to conduct in-depth interviews of the workers, which were analyzed using a deductive and inductive approach for the qualitative part. Quantitative and qualitative results were integrated and interpreted to expand on the findings with new insights. Results: In the final analysis, 122 workers were included (20 Filipino, 43 Indonesian, and 59 Vietnamese). In the quantitative part, having the necessary tools/equipment (Unstandardized Coefficient [B] = 16.1, 95% Confidence Interval [CI] = 6.8, 25.3) and support from work (B = 18.1, 95% CI = 8.6, 27.6) were associated with a higher level of job satisfaction. Having experienced harassment on the job was associated with a lower level of job satisfaction (B = -18.2, p = 0.007, 95% CI = -28.5, -7.8). There is no strong evidence for the association with subjective well-being. In the qualitative part, prominent themes emerged related to cultural sensitivity and an inadequate knowledge of caring for older adults living with dementia. The integrated findings suggested inadequate and unequal Japanese language training across the facilities. Moreover, the importance of human relations in the workplace emerged, with some workers desiring a deeper connection with their Japanese coworkers. Conclusions: A supportive and culturally sensitive work environment may bring about more motivated employees and increase employee retention from migrant LTC workers. Government and facilities should consider policies supporting a culturally sensitive work environment and more equitable Japanese language training across all facilities.
Factors Influencing Formal Care Service Usage: A Comparison of Middle-aged and Older Adults residing in South Korea

Abstract:This study explored factors of formal care service usage, dividing service users between middle-aged (50-64) and older adults (65+). Despite of upsurging care needs of middle-aged adults, previous studies have not studied on their care needs. Therefore, we focused on examining the factors of service uses for understanding their needs for public care services. We used the second data from Care SOS Center in Seoul, South Korea and selected 31,265 people who used the services in 2021. Independent variables were organized into four categories - predisposing, enabling, need, and health care system factors - based on Andersen's theory of service uses, for which many previous studies have adopted. After conducting multiple regression analysis using SPSS 25.0, the results were as follows: First, regarding the predisposing factors, middle-aged adults who have many family members were more likely to use care services, while older adults’ family members were not statistically significant. Second, need factors such as the discharged patients were pivotal in care service uses of both middle-aged and older adults. Third, older adults who had used other social care services before were less likely to use this short-term care SOS services, but the result was not statistically significant for middle-aged adults. Based on the results, we suggested social welfare interventions such as developing intensive care services for the discharged based on community care, interlocking and connecting social care services, making interventions for the excluded who needs imminent services as well as pointed out the importance of universal care services for both older and middle-aged adults.
Elder care and the impact of caregiver strain on the health of employed caregivers

Abstract:OBJECTIVE: As the baby-boom generation moves towards middle age, and their parents toward old age, the number of employees who combine care for an elderly dependant and work will increase in number. These employees are "at risk" of experiencing caregiver strain. This paper advances our understanding of these trends by examining the relationship between caregiver strain and the health of employed caregivers.PARTICIPANTS: Our study involved the analysis of data from the 2001 Canadian National Work, Family and Lifestyle Study (N= 31,517).METHODS: MANOVA was used to determine the relationship between caregiver strain and three situational factors: (1) gender; (2) where the care recipient lives compared to the caregiver; and, (3) family type. Regression was used to determine the relationship between caregiver strain and mental health.RESULTS: We found that caregiver strain depends on gender, family type and location of care. Emotional strain was a significant predictor of mental health.CONCLUSIONS: These findings support the need for organizations to expand their thinking around work-life balance to include employees who have eldercare responsibilities.
An overview of systems for providing integrated and comprehensive care for older people in Japan

Abstract:As longevity occurs, people encounter various risks associated with ageing, including economic uncertainty and health issues. Therefore, in addition to extending healthy life expectancy, it is crucial to create an environment where older people can live better even when their intrinsic capacity declines. Additionally, integrated and comprehensive care for older adults is needed to maintain their functional ability and well-being at higher levels. This review provides an overview of the systems and initiatives in Japan, a forerunner of population ageing that supports the quality of life of older people and summarises their remaining challenges. In Japan, with support for access to necessary care available from social welfare councils and community comprehensive support centres, various health and welfare services are provided to respond to the needs of people with different levels of intrinsic capacity, including medical care, preventive care, long-term care, adult guardianship systems, pensions, and social assistance. Nevertheless, there are challenges for the systems, including the gap between life and healthy life expectancy, moderate accumulation and decumulation of retirement assets, lack of human and financial resources for care, and user-unfriendliness and non-covered needs of the current system. Therefore, integrated and comprehensive care beyond health and long-term care is needed to maintain the well-being of older adults, even with their intrinsic capacity declining.
Well-being of professional older adults' caregivers in Alberta's assisted living and long-term care facilities: a cross-sectional study

Abstract:Background: For the care need of older adults, long-term care (LTC) and assisted living (AL) facilities are expanding in Alberta, but little is known about the caregivers' well-being. The purpose of the study was to investigate the physical health conditions, mental and emotional health (MEH), health behaviour, stress levels, quality of life (QOL), and turnover and absenteeism (TAA) among professional caregivers in Alberta's LTC and AL facilities. Methods: This cross-sectional survey involved 933 conveniently selected caregivers working in Alberta's LTC and AL facilities. Standardised questions were selected from the Canadian Community Health Survey, Patient Health Questionnaire-9, and Short Form-36 QOL survey revalidated and administered to the participants. The new questionnaire was used to assess the caregivers' general health condition (GHC), physical health, health behaviour, stress level, QOL, and TAA. Data were analysed using descriptive statistics, Cronbach alpha, Pearson's correlation, one-way analysis of variance, and multiple linear regression. Results: Of 1385 surveys sent to 39 facilities, 933 valid responses were received (response rate = 67.4%). The majority of the caregivers were females (90.8%) who were ≥ 35 years (73.6%), worked between 20 to 40 h weekly (67.3%), and were satisfied with their GHC (68.1%). The Registered Nurses had better GHC (mean difference [MD] = 0.18, p = 0.004) and higher TAA than the Health Care Aides (MD = 0.24, p = 0.005). There were correlations between caregivers' TAA and each of MEH (r = 0.398), QOL (r = 0.308), and stress (r = 0.251); p < 0.001. The most significant predictors of TAA were the propensity to quit a workplace or the profession, illness, job stress, and work-related injury, F (5, 551) = 76.62, p < 0.001, adjusted R2 = 0.998. Conclusion: Reducing the caregivers' job stressors such as work overload, inflexible schedule, and poor remuneration, and improving their quality of life, health behaviour, and mental, emotional, and physical health conditions may increase their job satisfaction and reduce turnover and absenteeism.

Abstract:Japan is the most aged society in the world. Population aging is one of the greatest challenges the Japanese economy will face during the first half of the 21st century. Because of the aging population, an increasing number of people are unable to live without the support and care of others. In 2000, to manage the challenges brought by the rapid growth in the elderly population, the Japanese government introduced the long-term care insurance system. Before then, the task of caring for the elderly was mainly carried out by their families and hospitals. The introduction of long-term care insurance enabled the development of the care industry. Even with the development of professional care services, however, the burden of caring for the elderly on their families has been increasing. Some caregivers gave up working, and many others shortening their working hours. The present study estimates that, in 2019, the Japanese economy lost 2 percent of its working hours from workers leaving or reducing their workload to provide care for a family member. Given the increasing number of elderly people in Japan, it is highly likely that the Japanese people will need to sacrifice even more working hours for caregiving in the future. The present study forecasts that 3.9 percent of total working hours will be lost for providing care in 2065. The loss of working hours can be avoided or reduced through further development of the long-term care insurance system and care industry. However, the development needs the support of the general public through higher premiums and more government subsidies.
Association between the Kihon Checklist and Working Status among Young-Old Citizens: A Cross-Sectional Study

Abstract:Owing to increasing super-aging societies, older adults will be encouraged to continue working. Although demand exists for older adults to continue working in Japanese society, the enabling factors have not been clarified. This study aimed to clarify (1) the life functions that affect the working status among younger-older adults who continue to work and those who do not via the Kihon Checklist (KCL) and (2) examine whether the number of areas of difficulties in daily functions of the KCL affected older adults' employment status. This cross-sectional study involved 5386 older men and women aged 65 years or older in one city in Japan. Employment status and the seven domains of the Kihon Checklist (KCL) were analyzed. The KCL items related to employment status were the physical (odds ratio = 2.46, p < 0.01), socialization (odds ratio = 1.95, p < 0.01), and mood domains (odds ratio = 1.29, p < 0.01). Furthermore, the odds ratio increased to 2.06 when three or more domains were applicable. To remain employed, one must be physically and mentally healthy. Analysis of job competencies of aged care workers based on Internet recruitment information mining

Abstract:Objective To analyze the competency needs of aged care workers in elderly care institutions, so as to providea reference for accurate construction of nursing staff training program. Methods Aged care worker job information were captured from online job boards with Web Scraper software, and the data were cleaned, then content analysis was used to code the information of job qualifications and job responsibilities with the package of NVivo 12 Plus. Results A total of 1 567 job information were collected, but 925（695 were life care givers related, 230 were integrated medical and support services givers related） were qualified after data cleaning. Four root nodes（knowledge, skills, characteristics and abilities） were extracted by combing and encoding job information. For life care givers, 16 second-level nodes and 37 third-level nodes were extracted, the most needed knowledge and skills were basal living nursing and safety precautions, and the most necessary characteristics and abilities were love, patience, responsibility, hard work and communication and expression ability. For integrated medical and support services givers, 21 second-level nodes and 35 third-level nodes were concluded, the most needed knowledge and skills were disease related and basal living nursing knowledge and skills, and the most necessary characteristics and abilities were the same as those of life care givers. Conclusion The knowledge and skills closely related to the job tasks are most needed for the two types of primary elderly care workers, followed closely by personality characteristics and quality, such as hard working, communication and expression ability. The competency requirement of integrated medical and support services giversis higher than that of life caregivers, information literacy will become an essential quality for caregivers due to the development of e-health services.
Determinants of job continuity among older workers: a mixed-methods research in Japan

Abstract:This study aims to determine the factors that encourage older workers to continue working. This study had an exploratory sequential design using a mixed-methods approach, including interviews and questionnaire surveys. In the interview survey, we targeted 30 workers aged between 60-65 across three manufacturing companies. After using the results of the content analysis in the interviews, we conducted an online questionnaire survey with 1,500 workers aged between 60-89 across the country. We analyzed whether the 15 factors were related to intention to continue working using logistic regression analysis. We identified factors affecting job continuity from three perspectives: individual, company, and life. We determined several factors: health condition, job performance, self-esteem, conservatism, employment system, workload, medical insurance and welfare programs, monetary and non-monetary rewards, relationships, attachment to the organization, distance between living and work, social support, economic situation, and employment policy. In the questionnaire survey, some factors had no relationship with job continuity, including conservatism, employment systems, monetary rewards, and the distance between living and work. Employers and policymakers can use the findings to consider appropriate ways of supporting older workers.
Factors Affecting the Competence of Nursing Assistants in Taiwan Long-Term Care Institutions

Abstract:With the increasing number of people with disabilities caused by an aging global population, the need for long-term care is gradually increasing. Nursing assistants (NAs) are the primary providers of direct care services to older adults with disabilities, whose knowledge, skills, and beliefs affect the quality of care provided. This study aimed to investigate the influential factors affecting NAs' current competences. A total of 255 NAs' valid questionnaires were collected from 20 long-term care institutions in Taiwan through convenience sampling. The questionnaire comprised dimensions of demographics and care competence. The study results indicated that NAs had the greatest care competence in the domain of recognition of patient rights (4.64 ± 0.54 points). The multiple regression indicated that age, religion, job category, disability care experience, the receiving of performance bonuses, and the receiving of year-end bonuses significantly affected the level of care competence (p < 0.05). With the aforementioned findings, the results of this study serve as references for the government in employing long-term care NAs and developing management policies. Training programs for NAs should be developed to improve the quality of care provided to older adults with disabilities.

Abstract:Journal of Applied Gerontology, Ahead of Print. The study compared care source typologies for older adults in China and the United States. Data from the 2014 U.S. Health and Retirement Study and the 2013 China Health and Retirement Longitudinal Study were used. The respondents included community-dwelling older adults aged 65 years or older with at least one limitation in activities of daily living (ADLs) or instrumental ADLs (IADLs) (NChina = 2476, NUS = 2898). Respondents reported whether they received assistance from spouse, child/grandchild, relatives, others, and formal helpers. Latent class analysis and multinomial logistic regression were applied. Four classes were identified in China and the U.S, separately. In both countries, ADLs and IADLs were strong determinants of care source typologies. Care sources were more diverse and included formal assistance among older Americans. Older Chinese relied largely on their spouses and children/grandchildren for support. Policy efforts are needed to expand formal long-term services and supports, particularly in China.
