Abstract:The increase in intracellular cyclic GMP concentrations in response to muscarinic-receptor activation in N1E-115 neuroblastoma cells is dependent on extracellular Ca2+ ion. The calcium ionophore A23187 can also evoke an increase in cyclic GMP in the presence of Ca2+ ion. Most (about 85%) of the guanylate cyclase activity of broken-cell preparations is found in the soluble fraction. The soluble enzyme can utilize MnGTP (Km = 55 micrometer), MgGTP (Km = 310 micrometer) and CaGTP (Km greater than 500 micrometer) as substrates. Free GTP is a strong competitive inhibitor (Ki approximately 20 micrometer). The enzyme possesses an allosteric binding site for free metal ions (Ca2+, Mg2+ and Mn2+). The membrane-bound guanylate cyclase is qualitatively similar to the soluble form, but has lower affinity for the metal-GTP substrates. Entry of Ca2+ into cells may increase cyclic GMP concentration by activating guanylate cyclase through an indirect mechanism.

Abstract:Abstract Background Depression is common in older people and is associated with underlying brain change increasing the risk of dementia. Sleep disturbance is frequently reported by those with lifetime depression, however whether circadian misalignment also exists is unclear. We aimed to examine circadian rhythms and sleep associations in older patients with and without lifetime depression. Methods Thirty-four older people meeting DSM-IV criteria for lifetime major depression (mean age = 63.9 years), and 30 healthy controls (mean age = 65.7 years) were recruited. Participants underwent 2-weeks of actigraphy followed by a 3-night protocol including dim light melatonin onset (DLMO) assessment and overnight polysomnography (PSG) for sleep architecture. DLMO and phase angle of entrainment were computed. Results Compared to controls, participants with depression had a significantly longer phase angle of entrainment (6.82 h ± 1.45 vs. 5.87 h ± 1.60, p = 0.02, Cohens-d = 0.62). A small to moderate yet non-significant difference in DLMO times, with earlier DLMO (34 ± 27 min) observed in depression (20:36 ± 1:48 vs. 21:10 ± 1:48, p = 0.22, Cohens-d = 0.32). Individuals with depression had longer sleep latency and latency to rapid eye movement sleep than controls (all p < 0.05). Conclusion Circadian advancement and alterations to the timing of sleep and REM onset are evident in older people with lifetime major depression, despite having only mild residual symptoms. Further research examining the prognostic significance of these changes is warranted as well as chronotherapeutic treatment studies.

Abstract:Abstract Study Objectives In older adults with Alzheimer’s disease, slowing of electroencephalographic (EEG) activity during REM sleep has been observed. Few studies have examined EEG slowing during REM in those with mild cognitive impairment (MCI) and none have examined its relationship with cognition in this at-risk population. Methods 210 older adults (mean age = 67.0, sd = 8.2 years) underwent comprehensive neuropsychological, medical, and psychiatric assessment and overnight polysomnography. Participants were classified as subjective cognitive impairment (SCI; n=75), non-amnestic MCI (naMCI, n=85), and amnestic MCI (aMCI, n=50). REM EEG slowing was defined as (delta + theta) / (alpha + sigma + beta) power and calculated for frontal, central, parietal, and occipital regions. Analysis of variance compared REM EEG slowing between groups. Correlations between REM EEG slowing and cognition, including learning and memory, visuospatial and executive functions, were examined within each subgroup. Results The aMCI group had significantly greater REM EEG slowing in the parietal and occipital regions compared to the naMCI and SCI groups (partial η2 = 0.06, p<0.05 and 0.06, p<0.05, respectively), and greater EEG slowing in the central region compared to SCI group (partial η2 = 0.03, p<0.05). Greater REM EEG slowing in parietal (r = -0.49) and occipital regions (r = -0.38 (O1/M2) and -0.33 (O2/M1) were associated with poorer visuospatial performance in naMCI. Conclusion REM EEG slowing may differentiate older adults with memory impairment from those without. Longitudinal studies are now warranted to examine the prognostic utility of REM EEG slowing for cognitive and dementia trajectories.

Abstract:Abstract Background Sleep disturbance and sleep disorders are potential risk factors for dementia. Adults with mild cognitive impairment (MCI) are at increased risk of developing dementia and many report disturbed sleep. Altered sleep neurophysiology including deficits in sleep spindles and slow wave activity (SWA) during NREM sleep have been reported in MCI, though studies are sparse. These NREM EEG oscillations are crucial for overnight memory consolidation and optimal daytime functioning. Abnormal EEG activity during sleep in MCI may adversely impact sleep‐dependent memory processes and cognitive trajectory. This study is the first to examine sleep microarchitecture using high‐density EEG technology in MCI. Method Older adults with multi‐domain MCI (n = 19, mean age 68.5 years) and cognitively‐intact controls (n = 22, 69 years) had 256‐channel high‐density EEG recordings during overnight polysomnography and resting wakefulness. Declarative (word paired associates) and procedural (finger tapping) memory tasks were administered before and after sleep. Neuropsychological testing and brain MRI scans were also performed. After artefact processing, spectral analysis was performed for high‐density EEG recordings. Topographical power maps were calculated for standard frequency ranges for NREM sleep (N2 and N3 (slow wave sleep)). Maps were compared between groups using normalized power (z‐scores computed for each subject). Result The MCI group had worse declarative (% retention, MCI vs Controls: 84.2 ± 16.6 vs. 94.5 ± 7.8, p = 0.024) and procedural (% early improvement: 92.9 ± 11.0 vs. 103.0 ± 12.0%, p = 0.017) overnight memory consolidation than the controls. Topographic analysis of high‐density EEG data revealed a regional increase in SWA (1‐4.5Hz) EEG power during N3 in a cluster of 18 electrodes overlying the centroparietal cortex in the MCI group (cluster mean t‐value = 3.11, p = 0.0036, uncorrected) and a regional reduction in SWA in the left frontal (5 channels, t = ‐3.75, p<0.001) and right temporal areas (5 channels, t = ‐3.46, p = 0.001), Figure 1. Conclusion Overnight memory consolidation was impaired in this MCI population. Regional differences in SWA during deep sleep were present in distinct brain regions and may indicate pathology. Future analysis exploring associations between EEG abnormalities with underlying brain structures may provide mechanistic insights into the impact of altered sleep neurophysiology on cognitive outcomes in MCI.

Abstract:Abstract Poor sleep health is a modifiable behavior linked to cognitive impairment risk. While prior work has focused primarily on sleep quantity and quality, emerging evidence suggests the importance of sleep timing and regularity. We examined whether sleep timing and regularity were associated with cognitive performance within a diverse cohort of women ages 60.5-72.2 years, and tested whether associations were independent of sleep duration and quality (wake after sleep onset). The sample included 1178 SWAN women who completed 4-7 nights of actigraphy and cognitive testing at the 15th SWAN follow-up in 2015-2016, [Mean age 65.5 (± 2.63 years); 25% Black, 12% Chinese, 6% Hispanic White, 11% Japanese, 46% Non-Hispanic White]. Sleep timing was defined as the midpoint time between sleep onset and waking, averaged across nights and was dichotomized as healthy (2am – 4am) or unhealthy (outside 2am – 4am). Sleep regularity was defined as the standard deviation of the sleep midpoint across nights. Associations of sleep regularity and timing with verbal episodic memory (East Boston Memory immediate (EBMT-I) and delayed (EBMT-D)), and processing speed (Symbol Digit Modalities) were examined using linear regression. Covariates included sleep duration and quality, age, race/ethnicity, education, economic hardship, depression, comorbidities, medications, alcohol use, vasomotor symptoms and testing language. Greater sleep irregularity was associated with worse verbal memory (EBMT-I β= -0.29; p=0.02; EBMT-D β= -0.36; p=0.007). Sleep midpoint outside the healthy range was associated with slower processing speed (β= -1.48; p=0.017). Sleep timing and regularity may be important targets for interventions aimed at preserving cognitive health.

Abstract:Bone mineral metabolism deteriorates gradually beginning from the early stages of chronic kidney disease (CKD). But, the KDIGO-2009 guideline could not provide high quality data regarding the biochemical tests related with bone mineral disorder in CKD. The aim of our study was to analyse the relationship between bone mineral densitometry and clinical and biochemical parameters in chronic peritoneal dialysis (PD) patients. Besides the demographic parameters, routine hematological and biochemical analysis results of PD patients followed up in our clinic were recorded. Bone mineral density (BMD) was measured at lumbar vertebrae and femur neck using DEXA machine. The mean lumbar T- and femur T-score were -1.03±1.20 (minimum:-3.73; maximum:+1.75) and -2.49±1.20 (minimum:-4.63; maximum:-0.51), respectively. Lumbar T score was significantly higher than fe mur T score (p<0.0001). Eight patients had BMD within normal limits; there was osteopenia in 16 and osteoporosis in 29 patients. While there was a negative correlation between femur T-score and age (r=-0.36, p=0.026), no correlation was detected between lumbar T-score and age (r=-0.17, p=0.21). With multivariate analysis of the factors related with femur T-score; age and body mass index (BMI) were the independent determinants while gender, parathyroid hormone levels and use of active vitamin D were not effective. Age was related negatively while BMI was related positively with BMD: BMD measurement at femur is more accurate than that at lumbar vertebrae in PD patients. BMD is low in most of the PD patients; and age and BMI are the major determinative factors.
Long Sleep Duration, Cognitive Performance, and the Moderating Role of Depression: A Cross-Sectional Analysis in the Framingham Heart Study

Abstract:INTRODUCTION: We investigated whether depression modified the associations between sleep duration and cognitive performance. METHODS: Multivariable linear regression models examined the associations between sleep duration and cognition in 1,853 dementia- and stroke-free participants from the Framingham Heart Study. Participants were categorized in four groups: no depressive symptoms, no antidepressants; depressive symptoms without antidepressants use; antidepressant use without depressive symptoms; both depressive symptoms and antidepressant use. RESULTS: Long sleep was associated with reduced overall cognitive function. Strong associations between sleep duration and cognitive performance were found in individuals with depressive symptoms, regardless of antidepressant use. Weaker but significant effects were observed in those without depressive symptoms. No significant associations were observed in participants using antidepressants without depressive symptoms. DISCUSSION: These findings provide new evidence that sleep duration may be a modifiable risk factor for cognitive decline, particularly in individuals with depressive symptoms. Future research should elucidate underlying mechanisms and temporal relationships.

Abstract:Abstract Introduction In adults with Mild Cognitive Impairment (prodromal phase of Alzheimer’s disease; MCI), the level of awareness of cognitive functioning varies significantly. A lack of awareness of cognitive functioning may interfere with monitoring of disease progression and opportunities for intervention. Therefore, understanding mechanisms associated with this relationship is critical. Given age-related changes in sleep and known relationships between sleep health and cognition, this pilot tested whether sleep moderates associations between subjective and objective cognition in older adults with MCI. Methods Older adults with MCI (N=46, Mage=68.2 years, SD=6.1 years, 33 women) completed one week of sleep diaries [averages of sleep onset latency (SOL), total sleep time (TST), sleep efficiency (SE)], Cognitive Failures Questionnaire (sub-scores CFQ-memory, CFQ-distractibility, CFQ-blunders), and cognitive tasks [Cambridge Brain Sciences; Polygons (processing speed), Feature Match (attention), Double Trouble (attention/inhibition), Digit Span (working memory)]. Multiple regressions tested if sleep parameters moderated associations between subjective and objective cognition, covarying for depressive symptoms and number of medical conditions. Results SE moderated associations between Polygons and CFQ-blunders (R2-change=.08, p=.03). Specifically, at highest SE, slower processing speed was associated with more blunder complaints (B=-.10, p=.04). Additionally, SE moderated associations between Digit Span and CFQ-distractibility (R2-change=.06, p=.03) and CFQ-blunders (R2-change=.12, p=.004). Specifically, at lowest SE, worse working memory was associated with less distractibility (B=3.01, p=.002) and blunders (B=2.45, p<.001) complaints, while at highest SE worse working memory was associated with more blunder complaints (B=-2.48, p=.04). Conclusion Preliminary findings suggest in older adults with MCI, lower sleep efficiency may exacerbate discrepancies between objective and subjective cognition, while higher sleep efficiency may converge this relationship. Sleep efficiency should be considered to fully understand the level of awareness of cognitive functioning in older adults with MCI. Critically, these findings may help identify those at risk of further cognitive decline. While future prospective studies are warranted, present findings suggest a potential next step is to examine whether behavioral treatments for insomnia (e.g., Cognitive Behavioral Therapy for Insomnia, cognitive training) may also change the relationship between objective/subjective cognition. Abstract:Background: Depression among older adults is a global concern, contributing to disability and overall illness burden. Understanding its trajectory, associated risk factors, and implications for mortality is essential for effective intervention. Moreover, the relationship between depression, sleep disturbances, and synaptic density in the ageing brain remains complex and poorly understood. Methods: Using data from the University of Manchester Longitudinal Study of Cognition in Normal Healthy Old Age cohort, comprising 6375 participants, we conducted comprehensive assessments of depression trajectories using generalized linear mixed models and mortality risks using Cox mixed-effects models. Generalized structural equation modelling was performed to explore longitudinal associations between sleep duration and depression. Lastly, associations between post-mortem synaptic density and depression were investigated. Results: Our findings revealed that depression rates declined until age 80 before increasing again. Depression was associated with a 10 % increased risk of mortality in older adults. Reduced sleep was correlated with depression, and depression measured early in the study predicted future reduced sleep. Post-mortem analysis showed a global reduction in synaptic density associated with depression, particularly pronounced in the frontal lobe. Limitations: Limitations include recall bias, limiting generalizability due to dominantly including White British participants and difficulty in establishing causation between synaptic density and depression. Conclusion: Our study underscores the significance of addressing depression in older adults, not only for mental health but also for mortality risk and neurobiological health. Early detection and intervention strategies are crucial for improving outcomes in elderly populations, potentially mitigating adverse effects on sleep, synaptic density, cognitive health, and longevity.
Sleep Characteristics in Older Adults with Different Levels of Risk for Dementia: A Cross-sectional Study

Abstract:Background Sleep problems are very prevalent in older adults, especially in those at risk for dementia. But the relationships between sleep parameters and subjective or objective cognitive decline are still inconclusive. Aim The study aimed to investigate the self-reported and objectively measured sleep characteristics in older adults with mild cognitive impairment (MCI) and subjective cognitive decline (SCD). Methods This study adopted a cross-sectional design. We included older adults with SCD or MCI. Sleep quality was measured separately by the Pittsburgh sleep quality index (PSQI) and ActiGraph. Participants with SCD were divided into low, moderate, and high levels of SCD groups. Independent samples T-tests, one-way ANOVA, or nonparametric tests were used to compare the sleep parameters across groups. Covariance analyses were also performed to control the covariates. Results Around half of the participants (45.9%) reported poor sleep quality (PSQI<7), and 71.3% of participants slept less than 7 hours per night, as measured by ActiGraph. Participants with MCI showed shorter time in bed (TIB) (p<0.05), a tendency of shorter total sleep time (TST) at night (p = 0.074) and for each 24-hour cycle (p = 0.069), compared to those with SCD. The high SCD group reported the highest PSQI total score and longest sleep latency than all the other three groups (p<0.05). Both the MCI and high SCD groups had shorter TIB and TST for each 24-hour cycle than the low or moderate SCD groups. Besides, participants with multiple-domain SCD reported poorer sleep quality than those with single-domain SCD (p<0.05). Conclusion Sleep dysregulation is prevalent in older adults with a risk for dementia. Our findings revealed that objectively measured sleep duration might be an early sign of MCI. Individuals with high levels of SCD demonstrated poorer self-perceived sleep quality and deserved more attention. Improving sleep quality might be a potential target to prevent cognitive decline for people with a risk for dementia.
Associations between self-reported sleep patterns and health, cognition and amyloid measures: results from the Wisconsin Registry for Alzheimer's Prevention

Abstract:Previous studies suggest associations between self-reported sleep problems and poorer health, cognition, Alzheimer's disease pathology and dementia-related outcomes. It is important to develop a deeper understanding of the relationship between these complications and sleep disturbance, a modifiable risk factor, in late midlife, a time when Alzheimer's disease pathology may be accruing. The objectives of this study included application of unsupervised machine learning procedures to identify distinct subgroups of persons with problematic sleep and the association of these subgroups with concurrent measures of mental and physical health, cognition and PET-identified amyloid. Dementia-free participants from the Wisconsin Registry for Alzheimer's Prevention (n = 619) completed sleep questionnaires including the Insomnia Severity Index, Epworth Sleepiness Scale and Medical Outcomes Study Sleep Scale. K-means clustering analysis identified discrete sleep problem groups who were then compared across concurrent health outcomes (e.g. depression, self-rated health and insulin resistance), cognitive composite indices including episodic memory and executive function and, in a subset, Pittsburgh Compound B PET imaging to assess amyloid burden. Significant omnibus tests (P < 0.05) were followed with pairwise comparisons. Mean (SD) sample baseline sleep assessment age was 62.6 (6.7). Cluster analysis identified three groups: healthy sleepers [n = 262 (42.3%)], intermediate sleepers [n = 229 (37.0%)] and poor sleepers [n = 128 (20.7%)]. All omnibus tests comparing demographics and health measures across sleep groups were significant except for age, sex and apolipoprotein E e4 carriers; the poor sleepers group was worse than one or both of the other groups on all other measures, including measures of depression, self-reported health and memory complaints. The poor sleepers group had higher average body mass index, waist-hip ratio and homeostatic model assessment of insulin resistance. After adjusting for covariates, the poor sleepers group also performed worse on all concurrent cognitive composites except working memory. There were no differences between sleep groups on PET-based measures of amyloid. Sensitivity analyses indicated that while different clustering approaches resulted in different group assignments for some (predominantly the intermediate group), between-group patterns in outcomes were consistent. In conclusion, distinct sleep characteristics groups were identified with a sizable minority (20.7%) exhibiting poor sleep characteristics, and this group also exhibited the poorest concurrent mental and physical health and cognition, indicating substantial multi-morbidity; sleep group was not associated with amyloid PET estimates. Precision-based management of sleep and related factors may provide an opportunity for early intervention that could serve to delay or prevent clinical impairment.

Abstract:Abstract Introduction Poor sleep health is a proposed risk factor for Alzheimer’s disease (AD). Despite known sex differences in AD prevalence (higher in women), whether sex impacts associations between objective sleep parameters and specific cognitive functions remains underexplored. Such investigations are warranted and will provide information regarding potential sex-specific targets for early intervention and/or markers of AD risk. This study tested whether sex moderates associations between polysomnographic assessed sleep and cognition in older adults. Methods Older adults (Mage=68.2±5.5; 44 women/29 men) completed overnight polysomnography (Sleep Profiler) and cognitive tasks [NIH Toolbox: Pattern Comparison (processing speed), Oral Symbol Digit Test (attention and processing speed), List Sorting (working memory), Auditory Verbal Memory Task (episodic memory), Flanker (inhibition), Dimensional Change Card Sort (cognitive flexibility)]. Multiple regressions examined whether sleep metrics (sleep onset latency, wake time after sleep onset, sleep efficiency, total sleep time), macro-architecture (%N1-N3, %REM) or micro-architecture (sleep spindle duration and index) interacted with biological sex in associations with cognition, controlling for age, education and apnea-hypopnea index. Results Sleep onset latency (R-squared=.06, p=.03) and sleep spindle duration (R-squared=.05, p=.05) interacted with sex in their associations with cognitive flexibility. Specifically, in women (not men), longer sleep onset latency (B=-.01, SE=.002, p=.03) and longer spindle duration (B=-.08, SE=.03, p = .005) were associated with worse cognitive flexibility. Conclusion The link between polysomnographic sleep and cognitive flexibility in older adults may be sex dependent. Sleep onset latency cognitive (rumination/worry) and neural (physiological arousal/pre-frontal activation) mechanisms may interact with female sex-steroid alterations (post-menopausal drops in circulating estrogen and progesterone) and exacerbate its negative impact on cognitive flexibility. We also speculate that longer sleep spindle duration in the presence of dysfunctional cognitive flexibility may reflect compensatory thalamo-prefrontal cortical regulation (greater need for nighttime cognitive/cortical arousal reduction) that is further influenced by prefrontal sex-steroid mechanisms. Longitudinal studies in larger samples of cognitively healthy and impaired older adults are needed to further understand whether polysomnographic sleep metrics represent sex-specific markers of future cognitive decline. Abstract:In GEO (Geostationary Earth Orbit) mobile satellite system, subscribers could not be paged when the MES (Mobile Earth Station) is normally out of coverage area due to additional propagation path losses. In this paper, an orthogonal coded modulation scheme based on BPSK (Binary Phase Shift keying) is proposed for the GEO mobile satellite system for the sake of establishing the call. Compared with the common paging channel modulation method QPSK (Quadrature Phase Shift Keying) and BPSK, the code words are mutual orthogonal, which is convenient for demodulation. And to be emphasized, the simulation results show that this scheme can increase the SNR (Signal Noise Ratio) of actual transmitted information, which is greatly more than the SNR of data in the common paging channel. It allows subscribers to be paged even when the MES is normally out of coverage area due to additional propagation path losses, and to move on an advantageous position.

Abstract:BACKGROUND: Sleep is critical for optimal cognitive function, but as we age both cognitive impairment and sleep problems increase. Longitudinal, population-based studies can be used to investigate temporal relationships between sleep and cognition.METHODS: A total of 2012 cognitively unimpaired individuals 65 years and over were drawn from the MRC Cognitive Function and Ageing Study (CFAS). They answered self-reported measures including: insomnia symptoms and age of onset, night time wakings, snoring, sleep onset latency, napping, daytime sleepiness and duration of night time sleep. Cognition was measured via the Mini-Mental State Examination.RESULTS: It was found that daytime napping at baseline was associated with a lower risk of cognitive decline at two and 10 years, and that obtaining ≤6.5h of night-time sleep and excessive daytime sleepiness at baseline were associated with an increased risk at 10 years.CONCLUSIONS: Daytime napping, night-time sleep duration, and excessive daytime sleepiness may be modifiable behaviours open to intervention strategies, or, clinical indicators of future decline in older individuals.

Abstract:Study objectives: We examined and compared cross-sectional and longitudinal associations between self-reported sleep disturbances and various cognitive domains in five separate Nordic European longitudinal aging studies (baseline N = 5631, mean age = 77.7, mean follow-up = 4.16 years). Methods: Comparable sleep parameters across studies included reduced sleep duration/quality, insomnia symptoms (sleep latency, waking up at night, and early awakenings), short and long sleep duration, and daytime napping. The cognitive domains were episodic memory, verbal fluency, perceptual speed, executive functioning, and global cognition (aggregated measure). A series of mixed linear models were run separately in each study and then compared to assess the level and rate of change in cognitive functioning across each sleep disturbance parameter. Models were adjusted for age, sex, education, hypnotic usage, depressive symptoms, lifestyle factors, cardiovascular, and metabolic conditions. By using a coordinated analytic approach, comparable construct-level measurements were generated, and results from identical statistical models were qualitatively compared across studies. Results: While the pattern of statistically significant results varied across studies, subjective sleep disturbances were consistently associated with worse cognition and steeper cognitive decline. Insomnia symptoms were associated with poorer episodic memory and participants sleeping less or more than 7-8 hours had a steeper decline in perceptual speed. In addition, daytime napping (>2 hours) was cross-sectionally and longitudinally associated with all examined cognitive domains. Most observed associations were study-specific (except for daytime napping), and a majority of association estimates remained significant after adjusting for covariates. Conclusion: This rigorous multicenter investigation further supports the importance of sleep disturbance, including insomnia, long and short sleep duration, and daytime napping on baseline cognitive functioning and rate of change among older adults. These sleep factors may be targeted in future lifestyle interventions to reduce cognitive decline.

Abstract:Abstract Introduction Aging is associated with disruptions in non-rapid eye movement (NREM) sleep expression, but the mechanisms driving this effect remain unclear. Cerebral small vessel disease (CSVD) burden increases with age and is associated with clinical sleep disturbance, but little is known about its relationship with local expression of oscillatory activity during NREM sleep. Here, we explore associations between CSVD burden and local electroencephalography (EEG) measures during NREM sleep in older adults. Methods Fifteen cognitively intact older adults (mean age 71.8±6.3 years, 10 female, Apnea Hypopnea Index (AHI)=7.82±7.95) completed T2-weighted fluid-attenuated inversion recovery magnetic resonance imaging on a 3T scanner to quantify total and lobar white matter hyperintensity (WMH) volumes using a validated semi-automated toolbox and an in-lab overnight polysomnography with 128-channel EEG, separated by 1.95±1.02 years. EEG was preprocessed and multi-tapered to derive absolute spectral power. Topographical correlations between NREM multi-tapered absolute spectra bands were run with 5000-permutation threshold free cluster enhancement. Results Older age was associated with lower absolute posterior total (all p< 0.016, r>-0.61) and slow sigma power (all p< 0.018, r>-0.60) and slow wave activity (SWA) (all p< 0.017, r>-0.61) over centro-posterior EEG derivations. Age was also positively associated with frontal (τ=0.51, p< 0.01) and parietal (τ=0.49, p< 0.01) but not occipital (τ=0.33, p=0.092) or temporal (τ=0.36, p=0.064) WMH burden. Occipital WMH burden was also not associated with AHI (τ=0.038, p=0.843). Occipital WMH burden was associated with global reductions in alpha activity (all p< 0.043, τ>-0.78), centro-posterior reductions in SWA (all p< 0.043, τ>-0.63) and Delta (all p< 0.009, τ>-0.63), frontal reductions in total (all p< 0.043, τ>-0.71) and fast sigma power (all p< 0.004, τ>-0.57), global reductions in slow sigma power (all p< 0.043, τ>-0.71), and central reductions in theta activity (all p< 0.042, τ>-0.57). Conclusion Collectively, these findings indicate that WMH burden in older adults is associated with local disruptions in NREM sleep expression in multiple frequency bands associated with sleep-dependent cognitive functions. Future studies should examine the role of NREM sleep expression in cognitive decline associated with CSVD burden. Abstract:Abstract Background Few studies have examined the trilateral relationships among sleep quality, depression, and cognitive function. Therefore, this study aimed to explore these relationships in an older population. Furthermore, we examined mediation models for sleep quality, depression, and mild cognitive impairment (MCI). Methods From December 2020 to July 2021, a cross-sectional survey was conducted among 5000 older people 60 years and over in Shanghai communities, of whom 4897 (97.9%) provided responses, using a random cluster sampling method. Sleep quality and depression status were assessed using the Pittsburgh Sleep Quality Index (PSQI) and 30-item Geriatric Depression Scale (GDS-30), respectively. The Barthel Index (BADL) and Ascertain Dementia 8 (AD-8) scales were combined to assess cognitive function. T-tests and chi-square tests were used to compare the sociodemographic characteristics of the participants. Pearson’s correlations were used to determine the direction and strength of the relationships between the primary variables. We further assessed the mediating role of sleep quality and depression. Results The prevalence of MCI was 21.7%. After adjusting for covariates, there were bidirectional relationships between sleep quality and depression and between MCI and depression. The significant mediating effect of sleep quality on the association between depressive symptoms and cognition function (95% CI: 0.0011 to 0.0026, p = 0.001) and mediated19.29% of the total effect of depressive symptoms on cognitive function. The mediation analysis showed that sleep quality had an indirect effect on cognitive function through the mediating variable of depression (95% CI: 0.0030 to 0.0050, p = 0.001) and mediated 36.50% of the total effect of sleep quality on cognitive function. Meanwhile, sleep quality partially mediated the relationship between cognitive function and depression symptoms (95% CI: 0.4594 to 0.7643, p = 0.001) and mediated26.77% of the total effect of cognitive function on depression. Conclusion Sleep quality and depression had significant direct (independent) and indirect (as a mediator) effects on MCI. We believe that improving screening for depression and sleep quality among older persons could significantly contribute to their health and well-being.

Abstract:Objectives: This study examined associations between various cognitive domains and sleep discrepancy (self-reported vs objectively measured sleep), and evaluated interactive associations with insomnia status (non-insomnia vs insomnia). Methods: Older adults (N = 65, Mage = 68.72, SD = 5.06, 43 insomnia/22 non-insomnia) aged 60+ reported subjective sleep (7 days of sleep diaries), objective sleep assessment (one-night polysomnography, PSG, via Sleep Profiler during the 7-day period), and completed cognitive tasks (National Institutes of Health Toolbox-Cognition Battery) measuring attention and processing speed, working memory, inhibitory control, cognitive flexibility, and episodic memory. The sleep diary variable corresponding to the same one night of PSG was used to calculate the sleep discrepancy (diary minus PSG parameter) variables for total sleep time (TST), sleep onset latency, wake after sleep onset, and sleep efficiency. Regression analyses determined independent and interactive (with insomnia status) associations between cognition and sleep discrepancy, controlling for age, sex, apnea-hypopnea index, and sleep medication usage. Results: Working memory interacted with insomnia status in associations with sleep discrepancy related to TST and sleep efficiency. In those with insomnia, worse working memory was associated with shorter self-reported TST (p = .008) and lower sleep efficiency (p = .04) than PSG measured. Discussion: In older adults with insomnia, worse working memory may be a contributing factor to sleep discrepancy. Future investigations of underlying neurophysiological factors and consideration of other objective sleep measures (actigraphy) are warranted. Prospective findings may help determine whether sleep discrepancy is a potential marker of future cognitive decline.

Abstract:Abstract Study Objectives To compare sleep and circadian rest/activity rhythms (RARs), quantified by standard and novel actigraphic metrics, between controls and participants with mild cognitive impairment (MCI), and to examine the cross-sectional relationships between these measures and cognition. Methods Actigraphy data were collected in 179 older individuals (mean age = 72.6 years) with normal cognition (n = 153) and MCI (n = 26). Sleep parameters (e.g. sleep efficiency), and standard nonparametric RARs (e.g. interdaily stability) were generated. Functional principal component analysis (fPCA) was used to generate three novel RAR metrics (fPC1, fPC2, and fPC3). Cognitive composite scores reflecting episodic memory and executive function were derived using factor analysis. Regression models compared sleep and RAR parameters between diagnostic groups and their association with cognitive performance. Results Compared to controls, the MCI group exhibited lower levels of the standard RAR parameter: relative amplitude and fPC3—a novel RAR whereby lower scores reflected a lower rhythm peak, as well as greater nighttime activity and less activity in the morning. Across groups, several standard RAR parameters (e.g. interdaily stability) and fPC3 were associated with better episodic memory and executive function performance. Additionally, several standard RAR measures (e.g. relative amplitude) and the novel RAR measure fPC1 (reflecting the total volume of activity and rhythm strength) were associated with better executive function performance. Conclusions Individuals with MCI have altered circadian RARs compared to controls, including the novel RAR metric fPC3, reflecting greater nighttime activity and less activity in the morning compared to mean values. Additionally, these measures are significantly associated with cognitive performance.
