Abstract:Background: Data on right ventricular (RV) exercise adaptation following acute intermediate and high-risk pulmonary embolism (PE) remain limited. This study aimed to evaluate the symptom burden, RV functional recovery during exercise and cardiopulmonary exercise parameters in survivors of intermediate and high-risk acute PE. Methods: We prospectively recruited patients following acute intermediate and high-risk PE at four sites in Australia and UK. Study assessments included stress echocardiography, cardiopulmonary exercise testing (CPET) and ventilation-perfusion (VQ) scan at 3 months follow-up. Results: Thirty patients were recruited and 24 (median age: 55 years, IQR: 22) completed follow-up. Reduced peak oxygen consumption (VO2) and workload was seen in 75.0% (n=18), with a persistent high symptom burden (mean PEmb-QoL Questionnaire 48.4±21.5 and emPHasis-10 score 22.4±8.8) reported at follow-up. All had improvement in RV-focused resting echocardiographic parameters. RV systolic dysfunction and RV to pulmonary artery (PA) uncoupling assessed by stress echocardiography was seen in 29.2% (n=7) patients and associated with increased ventilatory inefficiency (V̇E/V̇CO2 slope 47.6 vs 32.4, p=0.03), peak exercise oxygen desaturation (93.2% vs 98.4%, p=0.01) and reduced peak oxygen pulse (p=0.036) compared with controls. Five out of seven patients with RV-PA uncoupling demonstrated persistent bilateral perfusion defects on VQ scintigraphy consistent with chronic thromboembolic pulmonary vascular disease. Conclusion: In our cohort, impaired RV adaptation on exercise was seen in almost one-third of patients. Combined stress echocardiography and CPET may enable more accurate phenotyping of patients with persistent symptoms following acute PE to allow timely detection of long-term complications.

Abstract:BACKGROUND: Prolonged strenuous exercise has a profound effect on cardiac function. The right heart may be more susceptible to this imposition; yet, right-sided chest leads have not been utilised in this setting.METHODS: Thirty highly trained athletes at the 2014 Western States 100-mile Endurance Run from Squaw Valley to Auburn, California (body mass 68 ± 12 kg, age 45 ± 10 years, 57 ± 15 miles per week) were recruited for the study. Pre- and post-race, a right-sided 12-lead ECG was obtained and data were extracted for P, R and S waves, J point, ST segment and T wave amplitude. Data were compared using Students T-test and statistical significance set as P < .05.RESULTS: There was a significant increase in P wave amplitude (29%) and QTc interval (4%) pre- to post-race from standard 12-lead ECG. From the right-sided12-lead ECG, a 23% (P = .01) and 38% (P = .03) increase in J point amplitude in V1R and V2R and a 22% (P = .05) increase in ST segment integral in V2R and V3R were evident. T wave inversion was evident in leads V2R-V6R in 50-90% of athletes, respectively. Close examination revealed marked heterogeneity in individual ECGs.CONCLUSIONS: Completion of a 100-mile ultra-marathon resulted in significant changes in the right-sided ECG alongside more marked responses in specific individuals. P wave, ST segment and T wave changes post-race are indicative of acute exercise-induced right heart electrical adaptation.

Abstract:figure legend The purpose of this study was to characterize the impact of hypoxia on right ventricular (RV) performance during exercise. Ten healthy individuals underwent invasive cardiopulmonary exercise testing with upright cycle ergometry, under normoxic and hypoxic conditions at a fraction of inspired oxygen (FiO2 ) of 0.12. Participants were randomized to exercise with either a Swan–Ganz catheter (to characterize traditional cardiac and pulmonary haemodynamics) or a conductance catheter (to characterize RV performance by generation of real‐time pressure–volume loops). Exercise at an FiO2 = 0.12 was associated with a significant reduction in exercise capacity as determined by maximal oxygen uptake and workload on cycle ergometry. Compared to normoxic exercise, hypoxic exercise led to systemic hypoxaemia, increased pulmonary arterial pressures and pulmonary vascular resistance, and ventilatory inefficiency. RV contractility was maintained during submaximal hypoxic exercise. Ventricular–arterial coupling was also preserved. Acute hypoxia increases pulmonary arterial (PA) pressures, though its effect on right ventricular (RV) function is controversial. The objective of this study was to characterize exertional RV performance during acute hypoxia. Ten healthy participants (34 ± 10 years, 7 males) completed three visits: visits 1 and 2 included non‐invasive normoxic (fraction of inspired oxygen (FiO2 ) = 0.21) and isobaric hypoxic (FiO2 = 0.12) cardiopulmonary exercise testing (CPET) to determine normoxic/hypoxic maximal oxygen uptake (V̇O2max ). Visit 3 involved invasive haemodynamic assessments where participants were randomized 1:1 to either Swan–Ganz or conductance catheterization to quantify RV performance via pressure–volume analysis. Arterial oxygen saturation was determined by blood gas analysis from radial arterial catheterization. Abstract:BACKGROUND: We recently reported a novel observation that many patients with equal resting supine right ventricular(RV) and pulmonary artery(PA) systolic pressures develop an RV outflow tract(RVOT) pressure gradient during upright exercise. The current work details the characteristics of patients who develop such an RVOT gradient.METHODS: We studied 294 patients (59.7±15.5 years-old, 49% male) referred for clinical invasive cardiopulmonary exercise testing, who did not have a resting RVOT pressure gradient defined by the simultaneously measured peak-to-peak difference between RV and PA systolic pressures.RESULTS: The magnitude of RVOT gradient did not correspond to clinical or hemodynamic findings suggestive of right heart failure; rather, higher gradients were associated with favorable exercise findings. The presence of a high peak RVOT gradient (90th percentile, ≥33mmHg) was associated with male sex (70 vs. 46%, p = 0.01), younger age (43.6±17.7 vs. 61.8±13.9 years, p<0.001), lower peak right atrial pressure (5 [3-7] vs. 8 [4-12]mmHg, p<0.001), higher peak heart rate (159±19 vs. 124±26 beats per minute, p<0.001), and higher peak cardiac index (8.3±2.3 vs. 5.7±1.9 L/min/m2, p<0.001). These associations persisted when treating peak RVOT as a continuous variable and after age and sex adjustment. At peak exercise, patients with a high exercise RVOT gradient had both higher RV systolic pressure (78±11 vs. 66±17 mmHg, p<0.001) and lower PA systolic pressure (34±8 vs. 50±19 mmHg, p<0.001).CONCLUSIONS: Development of a systolic RV-PA pressure gradient during upright exercise is not associated with an adverse hemodynamic exercise response and may represent a normal physiologic finding in aerobically fit young people.

Abstract:Introduction Normal pulmonary circulation vasodilates with exercise among normal subjects and most patients with cardiopulmonary disease. However, in heart failure with preserved ejection fraction (HFpEF), some patients have pulmonary vasoconstriction with exercise, which can be identified with an invasive cardiopulmonary exercise test (iCPET). The aim of the study is to assess if this pulmonary vasoconstrictive response correlates with specific cardiac structural features, which can be quantified with cardiac MRI-based feature-tracking strain analysis. Hypothesis In HFpEF, exercise pulmonary vascular resistance (PVR >2.0 Woods unit) leaves an imprint on right ventricular (RV), in the form of significant dysfunction. This can be identified with reduced RV free wall strain and reduced augmentation via left ventricle. Methods In a prospective study, we performed iCPET and cardiac MRI in subjects with unexplained dyspnea (n=25). This included 10 with HFpEF, 5 with pulmonary arterial hypertension, and 10 controls. The current analysis is focused on 8 HFpEF subjects with isolated post-capillary pulmonary hypertension (rest PVR <3.0 WU). Subjects with exercise PVR <2.0 were labeled as normal (n=4) and those with exercise PVR >2.0 were labeled as abnormal (n=4). Results In our analysis of CMR-based strain and iCPET data, we found that there was a statistically and clinically significant difference in RV longitudinal strain (p=0.008), RA strain (p=0.03), and LV circumferential (p=0.004) and radial (p=0.004) strain. Patients with normal exercise PVR response had normal RV longitudinal strain compared to those with abnormal exercise PVR response. Interestingly, no association was found between LA strain and exercise PVR response. Conclusions Abnormal exercise PVR response can manifest on CMR-based strain as reduced right ventricular and right atrial longitudinal strain. Interestingly, left ventricular strains were also reduced in abnormal exercise PVR response, which is possibly related to ventricular interdependence from increased RV wall stress (given left atrial strain is similar in both groups). These findings need validation in larger studies, including our ongoing prospective study.

Abstract:The term exercise-induced cardiac fatigue (EICF) has typically been used to describe a transient reduction in cardiac function following prolonged-strenuous exercise. Recent evidence demonstrates that EICF can occur following only 45 min of high-intensity exercise when assessed using exercising stress echocardiography. This investigation sought to examine whether sprint intervals (SIT; 6 × 30 s Wingate tests), or 90-min moderate-cycling with sprint intervals (MIX; 90 min with 1 × 30 s Wingate test every 15 min) would cause greater EICF than 90 min (CON) or 3 h (LONG) moderate-cycling assessed using stress echocardiography, with a secondary aim to interrogate sex differences in EICF. Seventeen participants (M: 9, F: 8) underwent three cycling sessions with stress-echocardiography performed before-and-after each condition at a target heart rate (HR) of 100 beats·min-1, with the CON testing occurring at the mid-point of the 3 h LONG condition. For all conditions, measures of left ventricular (LV) systolic [stroke volume, ejection fraction (EF), peak longitudinal strain, isovolumetric contraction time, S') and diastolic (E/A, E', isovolumetric relaxation time, longitudinal strain rate) function were reduced after exercise (all P < 0.05). In the right ventricle (RV), systolic function was reduced (tricuspid annular plane systolic excursion, S', peak longitudinal strain and strain rate) following all conditions, and fractional area change was reduced to the greatest degree following SIT (condition × time, P = 0.01). Females demonstrated lesser impairments in LV EF, and elastance (ESP/ESV) compared with males (P < 0.05). Markers of EICF occurred similarly following all cycling loads, suggesting the functional changes may be due to altered loading conditions and reduced stress-echocardiography workload. However, males experienced greater cardiac alterations in some measures, likely due to greater changes in postexercise loading conditions.NEW & NOTEWORTHY This investigation sought to determine the role of exercise intensity on the magnitude of exercise-induced cardiac fatigue using stress echocardiography to maintain loading conditions, with a secondary purpose of assessing sex differences. Unexpectedly, it was found that all cycling loads elicited the same magnitude of functional alteration, which likely represents a common response to exercise and stress echocardiography, rather than intrinsic cardiac impairment. Males demonstrated greater alterations than females, likely due to sex differences in postexercise hemodynamics.
Sex differences in the associations between right heart structure and peak exercise capacity parameters in amateur cyclists

Abstract:Introduction: Right heart changes and their association with exercise capacity, including sex differences, are still being investigated. We analysed right heart structure and its relationship with exercise capacity parameters in amateur cyclists. Materials and methods: A cross-sectional study involving 215 consecutive adult amateur cyclists, who underwent resting transthoracic echocardiography and a cardiopulmonary exercise test (CPET) to exhaustion was performed. Results: The median age of participants was 29 years (IQR 24-37), 71% of them were men. The mean training time was 6 h/week, and 90% participated in vigorous or moderate physical activity. Men had larger right ventricular diameters (basal - RVD1, mid-cavity - RVD2 and longitudinal - RVD3) compared to women (40.9 vs. 37.6 mm, p = 0.0005, 28.7 vs. 26.3 mm, p = 0.03, 92.2 vs. 81.9 mm, p < 0.0001). Indexing for body surface area revealed comparable right atrial volume (RAVI) between sexes (24.1 vs. 22.7 mL/m2). Men achieved higher peak exercise capacity parameters [O2 pulse, oxygen consumption (VO2) and workload] in CPET. Multivariate linear regression models revealed a positive association between peak VO2, workload and O2 pulse with RAVI in women but not with RVD1 or RVD3. Conversely, these parameters showed a positive association with RVD3 and RVD1 but not with RAVI in men. Conclusion: Sex differences exist in the relationship between right heart structural parameters and peak exercise capacity descriptors in amateur cyclists. Better exercise capacity during CPET to exhaustion is associated with greater RAVI in women but a greater RVD1 and RVD3 in men. These findings suggest different mechanisms of right heart adaptation to training in men and women.
Right ventricular remodeling induced by prolonged excessive endurance exercise is mediated by upregulating Wnt/β-catenin signaling in rats

Abstract:Background: The aim of this study was to develop an animal model to investigate whether prolonged intensive endurance exercise induces RV remodeling, taking into account the involvement of Wnt/β-catenin signaling. Methods: Four-week-old male Wistar rats (100 to 125 g) were assigned to four groups (n = 8/group): 1) sixteen weeks of intensive (36 m/min) exercise (INT), 2) twelve weeks of the intensive exercise followed by four weeks of moderate intensity (18 m/min) exercise (INT + MOD), 3) twelve weeks of the intensive exercise followed by four weeks of detraining (INT + DT), and 4) sedentary rats (SED). The exercise protocols were performed five days a week for one h/day. Echocardiography, real-time PCR, western blotting, and histological staining were performed at the end of week sixteen. Results: INT rats developed concentric hypertrophy without diastolic dysfunction compared to SED (p = 0.006) and INT + DT (p = 0.035). Wnt1, β-catenin and CyclinD1 proteins in the training groups were significantly higher than SED rats (p < 0.001). Interestingly, INT rats had higher protein levels than INT + DT and INT + MOD (p < 0.001), with higher gene expression compared to SED rats (p < 0.05). There was also a significant increase in collagen deposition in INT rats compared to SED (p = 0.046) and INT + DT (p = 0.034). Furthermore, INT + MOD and INT + DT rats did not show any adverse structural, functional, or histological changes. Conclusions: Long-term intensive endurance training seems to be associated with increased collagen deposition and wall thickness in the RV through Wnt/β-catenin signaling (which is concentration dependent), without changes in diastolic function. Clinical perspective: Over the past decades, there has been an ongoing debate about whether the structural and functional adaptations of the cardiovascular system in trained endurance athletes are benign physiological responses to training or potentially pathological changes related to disease. While the adaptations of the left heart are well-documented, the remodeling of the right heart remains a subject of discussion. To gain insights into the ability of sustained high-intensity exercise to cause adverse right ventricular (RV) remodeling, we conducted an experimental study in which male rats were trained to run vigorously for 1 h daily over a 16-week period and compared them to a parallel group of sedentary control rats. Our findings revealed that intense long-term exercise induced morphological changes along with fibrosis affecting the RV. These fibrotic changes were a result of the 16-week vigorous exercise training regimen. If these results are confirmed in humans, they suggest that prolonged high-intensity endurance exercise training may lead to adverse cardiac remodeling. Our findings have important potential implications for the assessment of cardiac remodeling in individuals engaged in high-level exercise training.
Right ventricular outflow tract diameter change with exercise: a prospective exercise echocardiography and invasive CPET study

Abstract:While cardiac output reserve with exercise predicts outcomes in cardiac and pulmonary vascular disease, precise quantification of exercise cardiac output requires invasive cardiopulmonary testing (iCPET). To improve the accuracy of cardiac output reserve estimation with transthoracic echocardiography (TTE), this prospective study aims to define changes in right ventricular outflow tract diameter (RVOTd) with exercise and its relationship with invasively measured haemodynamics. Twenty subjects underwent simultaneous TTE and iCPET, with data collected at rest, leg-raise, 25 W, 50 W (n = 16), 75 W (n = 14), and 100 W (n = 6). This was followed by a second exercise study with real-time RV pressure-volume loops at similar stages (except leg-raise). The overall cohort included heart failure with preserved ejection fraction (n = 12), pulmonary arterial hypertension (n = 5), and non-cardiac dyspnoea (n = 3). RVOTd was reverse engineered from the TTE-derived RVOT velocity time integral (VTI) and iCPET-derived stroke volume, using the formula: Fick stroke volume = RVOT VTI × RVOT area (wherein RVOT area = π × [RVOTd/2]2). RVOTd increased by nearly 3-4% at every 25 W increment. Using linear regression models, where each subject is treated as a categorical variable and adjusting for subject intercept, RVOTd was correlated with haemodynamic variables (cardiac output, heart rate, pulmonary artery and RV pressures). Of all the predictor haemodynamic variables, cardiac output had the highest r2 model fit (adjusted r2 = 0.68), with a unit increase in cardiac output associated with a 0.0678 increase in RVOTd (P < 0.001). Our findings indicate that RVOTd increases by 3-4% with every 25 W increment, predominantly correlated with cardiac output augmentation. These results can improve the accuracy of cardiac output reserve estimation by adjusting for RVOTd with graded exercise during non-invasive CPET and echocardiogram. However, future studies are needed to define these relationships for left ventricular outflow tract diameter.

Abstract:Introduction: Prolonged and repeated exercise performed during an ultra-endurance event can induce general and cardiac fatigue known as exercise-induced cardiac fatigue. Our objective was to find a possible correlation between the cardiac function and the autonomic cardiac function. Methods: During a multistage ultra-endurance event, a female well-trained cyclist underwent daily rest echocardiography and heart rate variability measurements to assess the cardiac function and the cardiac autonomic function. Results: The athlete completed 3,345 km at 65% of her maximum heart rate and 39% of her maximum aerobic power. A progressive improvement of the systolic function for both the left ventricle and the right ventricle was observed during the event. Discussion: Alterations were observed on the cardiac autonomic function with an imbalance between sympathetic and parasympathetic, but there was no sign of a significant correlation between the cardiac function and the autonomic cardiac function and no signs of cardiac fatigue either. Further analysis should be performed on a larger sample to confirm the obtained results.

Abstract:Background Healthy young athletes adapt to the increased demands of endurance exercise with symmetric cardiac remodeling. Male veteran endurance athletes have an increased risk of atrial fibrillation (AF), and some athletes seem susceptible to changes mimicking arrhythmogenic cardiomyopathy. Intense exercise puts a disproportionate hemodynamic load on the right-sided heart chambers. Despite this, data describing right heart structure and function in older veteran athletes are scarce. We aimed to investigate structural and functional characteristics of the right heart in veteran athletes with and without AF to contribute to the understanding of exercise-induced cardiac remodeling in this group. Methods Three hundred and two male participants, of whom 151 were veteran skiers (62 with paroxysmal AF) and 151 were controls from the general population (62 with paroxysmal AF) underwent an echocardiographic examination in sinus rhythm to evaluate right atrial (RA) and right ventricular (RV) structure and function. While 87 of the participants had never exercised regularly, 50, 43, and 122 men had practiced regular endurance exercise for 1-20, 20-40, and >40 years, respectively. Results RA volume and RV size increased with cumulative years of exercise (p<0.001), with a disproportionate increase in RV size compared with left ventricular (LV) size, regardless of AF status (p<0.001). RA and RV function assessed by strain remained similar despite lifelong exposure to endurance exercise. AF was associated with reduced RA strain irrespective of exposure to exercise (p<0.001). Conclusion RA and RV size and RV/LV ratio showed a dose-response relationship with cumulative years of endurance exercise, whereas RA and RV function did not. Indicating that increasing RV/LV ratio may represent a physiological adaptation to prolonged endurance exercise. AF was associated with reduced RA function, regardless of exposure to exercise, suggesting RA functional parameters are more closely linked to AF than RA size in veteran athletes. Abstract:Abstract Background The wide-ranging benefits of frequent and moderate exercise are well recorded in the literature. Chronic deleterious remodelling in response to exercise is less well described. We describe an amateur endurance cyclist who, in addition to developing a heart failure syndrome and electrocardiographic evidence of arrhythmia, also developed severe functional tricuspid regurgitation. Case summary After developing palpitations during long distance cycle rides as part of his fitness regimen, a 69-year-old male presented to emergency services but was discharged. While continuing to enjoy long-distance cycling, he began to develop peripheral swelling and presented for a second time to hospital. Subsequent investigation found he had a dilated right heart, exercise-induced arrhythmia, and mid-wall myocardial fibrosis. A diagnosis of exercise-induced cardiomyopathy was made. He was managed with diuretics and immediate cessation of exercise. His symptoms improved and he remains symptom free. Discussion The volume of blood passing through the right heart increases during exercise. In vulnerable individuals undertaking frequent endurance exercise, this can promote structural remodelling and fibrotic change. It is unclear if cessation of exercise can reverse the remodelled heart. There are some early advances in predictive biomarkers and imaging techniques in categorizing those in the population who would be at risk of developing this cardiomyopathy, and those who can undergo intense exercise regimens without concern. If those at risk of developing an exercise-induced cardiomyopathy can be accurately identified, the next dilemma is how can their risk of heart failure or sudden death be acceptably minimized.
Right Ventricular Morphology and Function after Exercise Training in People with Systemic Sclerosis: A Randomized Controlled Pilot Study

Abstract:Background: Vascular dysfunction and its concomitant multi-organ involvement, including cardiac involvement, affects prognosis in systemic sclerosis (SSc) patients. Regular exercise has demonstrated to be able to improve vascular function in SSc. However, the effects of an exercise program on the heart and specifically in right ventricular (RV) morphology and function in SSc have yet to be explored. The study aimed to examine whether a 3-month combined exercise program can affect RV morphology and function in SSc patients. Methods: Twenty-eight SSc patients were randomly allocated to either the exercise training (ET) or the control (CON) group. Baseline and follow-up assessments consisted of a cardiopulmonary exercise test along with both a conventional and a two-dimensional speckle tracking echocardiography (2DSTE) focused on RV morphology and function. Following the baseline assessments, Group ET participated in a supervised combined exercise program for 12 weeks, while group CON received their usual care. Results: The ET group demonstrated increases in peak oxygen consumption by 25.1% (p < 0.001), global RV free wall longitudinal systolic strain by 6.69% (p < 0.03), RV free wall longitudinal systolic strain of the basal segment by 13.5% (p < 0.001), and global RV four-chamber longitudinal systolic strain by 6.76% (p < 0.03) following the exercise program. No differences were observed in group CON. Conclusions: Combined exercise improved cardiorespiratory efficiency and indices of RV systolic function, as assessed by the 2DSTE, in SSc patients.

Abstract:Abstract Background Moderate intensity regular physical activity is a mainstay of cardiovascular prevention. However, it is under debate whether the cumulative effects of intensive endurance exercise induce chronic cardiac damage. It has been proposed that long-term endurance exercise may be associated with myocardial fibrosis detected by late gadolinium-enhancement (LGE) cardiac magnetic resonance (CMR), and/or with right ventricle contractile dysfunction, creating a potential substrate for ventricular arrythmia, but these aspects are not constantly found across studies. Extracellular volume (ECV) determined by CMR, circulating cardiac biomarkers, and longitudinal global 2D strain (LGS) assessed by cardiac echography are relevant tools to explore these potential adverse effects of exercise in senior athletes. Purpose To assess the presence of myocardial fibrosis detected by CMR (LGE and ECV) and biomarkers, and to evaluate ventricles function with LGS, in endurance senior competitive athletes. Methods Thirty-three asymptomatic endurance senior athletes (47±6 years old, 9,6±1,7 h of training per week for 26±6 years), were compared to 18 sedentary controls (49±7 years old). They underwent a CMR protocol including morphological and late gadolinium-enhancement (LGE) analysis, T1 mapping and calculation of ECV using a 1.5 T MRI scanner. A maximal exercise transthoracic echography with LGS analysis was performed. Cardiac biomarkers (N terminal pro brain natriuretic peptide, high-sensitivity troponin T and plasma markers of collagen biosynthesis and degradation procollagen type I N terminal propeptide and procollagen type III NT propeptide) were also analysed. Results Athletes had higher left (84,0±20,8 vs 67,62±12,4 ml/m2; p=0,00593) and right (41,3±9,5 vs 32,9±6,0 ml/m2; p=0,00228) ventricular volumes, higher total biventricular mass (109,61±14,18 vs 98,66±16,25 g/m2; p=0,02038), and higher stroke volume (50,7±12,6 vs 40,1±9,2 ml/m2; p=0,00441) assessed by CMR versus controls. Native T1 (left ventricle: 938±21 vs 940±34 ms; p=0,8098 - right ventricle: 1027±53 vs 1026±114 ms; p=0,92983) and ECV (21,5±1,6 vs 22,0±2,2%; p=0,40868) were not significantly different in athletes compared with controls. LGE was not found in athletes. Peak exercise LGS values were higher in athletes for left (23,19±2,86 vs 20,82±2,05%; p=0,00335) and right (29,25±3,47 vs 26,48±3,22%; p=0,00777) ventricles, compared with control. The levels of cardiac biomarkers were normal in all subjects. Conclusion Athletes showed signs of physiological cardiac remodelling, consistent with previous descriptions of athlete's heart. Despite this important remodelling, there was no evidence of myocardial fibrosis or exercise left or right ventricular dysfunction in athletes. Our results are not supporting the hypothesis of deleterious cardiac effects induced by long term and intensive endurance exercise training.

Abstract:New findings: What is the central question of this study? Right ventricle (RV) dysfunction is highly prevalent in heart failure with preserved ejection fraction (HFpEF), nearly doubling the risk of death: what are the RV functional and structural changes in HFpEF and how does aerobic exercise impact them? What is the main finding and its importance? The HFpEF ZSF1 rat model presents RV structural and functional changes mimicking the human condition. Aerobic exercise prevented the decline in V̇O2max , lowered surrogate markers of RV overload (e.g., higher mean and maximum systolic pressure) and improved diastolic dysfunction (e.g., end-diastolic pressure and relaxation time constant). This emphasizes the importance of using exercise to manage HFpEF. Abstract: Right ventricle (RV) dysfunction is highly prevalent in heart failure with preserved ejection fraction (HFpEF) and is a marker of poor prognosis. We assessed the obese ZSF1 rat model of HFpEF to ascertain if these animals also develop RV dysfunction and evaluated whether aerobic exercise could prevent this. Obese ZSF1 rats were randomly allocated to an aerobic exercise training group (n = 7; treadmill running, 5 days/week, 60 min/day, 15 m/min for 5 weeks) or to a sedentary group (n = 7). We used lean ZSF1 rats (n = 7) as the control group. After 5 weeks, rats were submitted to an exercise tolerance test and invasive haemodynamic evaluation, killed and samples from the RV collected for histological analysis. Obese sedentary ZSF1 rats showed lower V̇O2max , RV pressure overload (e.g., higher mean and maximum systolic pressure) and diastolic dysfunction (e.g., higher minimum and end-diastolic pressure and relaxation time constant), paralleled by RV cardiomyocyte hypertrophy. Except for cardiomyocyte hypertrophy, aerobic exercise prevented these functional changes. Our data support that this model of HFpEF shows functional and structural changes in the RV that resemble the human HFpEF phenotype, reinforcing its utility to understand this pathophysiology and to adress novel therapeutic targets to manage HFpEF. In addition, we showed that aerobic exercise is cardioprotective for the RV. A deeper knowledge of the mechanisms underlying the benefits of aerobic exercise could also lead to the identification of therapeutic targets to be further explored.

Abstract:Abstract Background Cardiac adaptation to exercise is characterized by eccentric remodeling due to increased volume load[1], it is however unclear how systolic function is affected in all chambers. Endurance sports is a known risk factor for later onset atrial fibrillation[2]. Moreover, recent studies of the acute effects of exercise have reported reversable right ventricular (RV) dilation and reduced function in triathletes resembling ARVC, while the left ventricle (LV) was unchanged[3]. These findings have raised concern regarding the long-term adaptations in the thin-walled chambers of the heart. Purpose The aim of the current study was to assess atrial and RV remodeling by echocardiography in elite athletes. Methods A cross-sectional study of 482 healthy active elite athletes from 30 sports was investigated with a standard resting echocardiogram. Using the Mitchell sports classification, athletes were grouped according to levels of endurance demands for their sport into low (Mitchel 1A), intermediate (Mitchel 1B-C, 2A-B-C), and high endurance demand (3A-B-C). All measures were adjusted for body surface area (BSA), sex, and age. Analysis of Covariance models was used to allow for adjustments for categorical variables and continuous covariates. Results Left atrial minimum volume/BSA (LAminV/BSA) was higher in athletes who performed high endurance demanding sports than in the intermediate and low groups (High: 11.3 ml/m2, confidence interval (CI): 10.1-12.5; Int: 8.9 ml/m2, CI: 8.3-9.5, p<0.01; Low: 7.7 ml/m2, CI: 5.8-9.7, p<0.01). We found no difference between the low and intermediate groups. RV systolic area/BSA (RVsysA/BSA) was significantly higher in the high and intermediate groups compared to the low group (high 9.5 ml/m2, CI: 9.1-9.8, p=0,014; Int 9.3 ml/m2, CI: 9.1-9.5, p=0.04; Low: 8.3 ml/m2, CI: 7.7-9.0). The LA/RV minimum size ratio showed a larger size difference between the groups in the LA compared to the RV (High 1.183, CI: 1.063-1.303; Int 0.968, CI: 0.906-1.029, p<0.01, Low 0.921, CI: 0.726-1.116 p=0.02). A comparison of LA and LV size (LAminV/LVsysV) showed a significant decrease between high and intermediate groups, but not the low group (High: 0.366; Int: 0.296, p<0.01; Low: 0.302, p=0.16). The LA/RA size ratio was equal in all groups. LA strain analysis showed reduced contraction in the high and intermediate groups compared to the low group (High -10.2, CI: -10.9-(-9.4), p<0,01, Int -10.6, CI: -11.1-(-10.2), p<0.01, Low: -12.9, CI: -14.2-(-11.6)), the same pattern was not found in RA function, RV freewall and LV GLS (table 1). Conclusion We found a bilateral increase in atrial size while LA (but not RA) also showed reduced contraction in athletes from intermediate and high endurance demanding sports. Athletes from sports with high and intermediate endurance demands exhibited dilated ventricles but no decreased function was detected. Abstract:Right heart enlargement is common in the athletes' heart phenotype; however, few data exist regarding interpretation of normal athletic adaptation during Preparticipation Cardiac Screening (PCS) of Rugby Football League (RFL) athletes. Echocardiography is utilized during PCS and thus the primary aim of this study was to establish the normal right ventricular (RV) phenotype in elite RFL athletes using standard 2‐D echocardiography and myocardial mechanics. The secondary aim was to describe right atrial (RA) structure and function using 2‐D echocardiography.139 male RFL athletes underwent echocardiographic evaluation of the right heart including RV strain (ɛ) and strain rate (SR) imaging using speckle tracking echocardiography (STE). Nonathletic males were used for comparison and allometric scaling was applied for conventional echocardiographic parameters.Scaled RV dimensions were larger in athletes (<i>P</i> P > 0.05) in RV fractional area change (FAC) and RV longitudinal ɛ were observed between groups. Tissue Doppler imaging (TDI)‐indexed parameters and global strain rate (SR) were lower (<i>P</i> P P > 0.05) observed by volume assessment. Reduction in SR and indexed TDI are partly associated with lower HR and increased body mass and are likely to represent normal physiological adaptation in RFL athletes. RA enlargement appears proportional to RV enlargement. These data may aid interpretation of normal athletic adaptation during PCS of RFL athletes.

Abstract:Background: The purpose of the present study was to examine physician’s attitudes and practices toward the use of different lipid-lowering management strategies in patients at increased risk for coronary heart disease (CHD). Aims/Methods: An internet-based questionnaire was completed by 78 general internists and family practitioners (mean age = 49 years; 80% male) affiliated with a large primary care health delivery system in Connecticut. Questions were asked about physician knowledge and use of current national guidelines for lipid-lowering therapy and their treatment practices for patients at varying risk for CHD. Results: Most physicians reported they were very knowledgeable about different interventions to lower serum lipids. Most (92%) indicated that they were aware of and followed national guidelines for the treatment of patients with hyperlipidemia the majority of the time. Physicians were likely to initiate lipid-lowering therapy at lower levels of serum LDL cholesterol in patients at high, as compared to those at moderate, risk for coronary disease. Targeted treatment levels were also reported to be considerably lower for patients at higher risk, than for those at moderate risk, for the development of coronary disease. Diabetes, cigarette smoking, and elevated LDL cholesterol levels were reported to be the three most important risk factors for CHD by the physician sample. Gaps in the recommendation of lifestyle changes to patients with hyperlipidemia were observed. Conclusions: Despite adequate physician knowledge, achieving desirable serum lipid levels in primary care patients remains elusive. Provider education is needed to optimize the care of patients with elevated serum lipids treated in the primary care setting.
Evaluation of Left Ventricular Systolic Function Using Layer-Specific Strain in Rats Performing Endurance Exercise: A Pilot Study

Abstract:OBJECTIVE: The functional characteristics of exercise-induced myocardial hypertrophy were studied in a rat model in conjunction with ultrasound layered strain technique to investigate the hidden changes in the heart brought about by exercise.METHODS: Forty specific pathogen free (SPF) adult Sprague-Dawley rats were selected and randomly divided into two groups of 20 exercise and 20 control rats. The longitudinal and circumferential strain parameters were measured using the ultrasonic stratified strain technique. The differences between the two groups and the predictive effect of stratified strain parameters on left ventricular systolic function were analyzed.RESULTS: The exercise group had significantly higher global endocardial myocardial longitudinal strain (GLSendo), global mid-myocardial global longitudinal strain (GLSmid) and global endocardial myocardial global longitudinal strain (GCSendo) values than the control group (p < 0.05). Even though global mid-myocardial circumferential strain (GCSmid) and global epicardial myocardial circumferential strain (GCSepi) were higher in the exercise group than in the control group, statistical significance was not reached (p > 0.05). Conventional echocardiography parameters were well correlated with GLSendo, GLSmid, and GCSendo (p < 0.05). GLSendo was the best predictor of left ventricular myocardial contractile performance in athletes determined using the receiver operating characteristic curve, with an area under the curve of 0.97, sensitivity of 95% and specificity of 90%.CONCLUSION: Rats performing endurance exercise exhibited subclinical changes in the heart after prolonged high-intensity exercise. A stratified strain parameter, GLSendo, played an important role in the evaluation of LV systolic performance in exercising rats.
