Abstract:Objective To evaluate the value of transrectal ultrasound-guided systematic prostate biopsy and magnetic resonance imaging(MRI) suspicious targeted biopsy in the biopsy of prostate cancer.Methods A retrospective analysis was performed in 181 patients with suspected prostate cancer underwent transrectal ultrasound-guided prostate biopsy from September 2011 to September 2013 in the Second Affiliated Hospital of Zhejiang University School of Medicine.In all the 181 patients had the transrectal ultrasound-guided systematic biopsy,116 patients with suspicious MRI had two additional targeted biopsies.Prostate cancers were detected in 97 of 181 patients,and the prostatic hyperplasia were detected in 84 patients.The detection rate of transrectal ultrasound-guided systematic prostate biopsy,suspicious MRI targeted biopsy and combined biopsy in prostate cancer were tested by χ2 test.Compared with pathological results,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of transrectal ultrasound-guided systematic prostate biopsy and suspicious MRI targeted biopsy were calculated and tested by χ2 test.Results Prostate cancer was detected in 97 of 181 patients(53.6%),63(34.8%) was detected by transrectal ultrasound-guided systematic prostate biopsy,and 81(44.8%) was detected by suspicious MRI targeted biopsy.The suspicious MRI targeted biopsy detected more prostate cancers which there had statistical differences compared with transrectal ultrasound-guided systematic prostate biopsy(χ2=34.830,P=0.015).Transrectal ultrasound guided prostate systematic biopsy missed 34 cases of prostate cancer,and suspicious MRI targeted biopsy missed 16 cases,accounting for 35.1%(34/97) and 16.5%(16/97) of prostate cancer.The 181 patients were biopsied a total of 1 896 cores,including transrectal ultrasound guided systematic biopsy 1 448 cores and suspicious MRI targeted biopsy 448 cores.The sensitivity,specifi city,positive predictive value and negative predictive value of suspicious MRI targeted biopsy was 89.8%(280/312),87.4%(1 384/1 584),58.3%(280/480),97.7%(1 384/1 416) compared with the sensitivity of 71.8%(224/312),specifi city of 22.7%(360/1 584),positive predictive value of 15.5%(224/1 448) and negative predictive value of 80.4%(360/448) in transrectal ultrasound-guided systematic prostate biopsy(χ2=14.008,96.037,143.049,170.729,all P < 0.001).Conclusions The detection rate of prostate cancer of suspicious MRI targeted biopsy is higher than transrectal ultrasound-guided systematic prostate biopsy,but can′t replace transrectal ultrasound-guided systematic prostate biopsy.The two methods are complementary to improve the prostate cancer detection rate.

Abstract:ObjectivesProstate cancer (PCa) diagnosis still relies on 12-core systematic biopsy (SBx). More recently, pre-biopsy multiparametric MRI has been introduced in the guidelines as a valuable imaging tool for the detection of significant PCa. However, poor reproducibility and specificity, along with its high cost and limited availability, evidence the need for cost-effective, widespread imaging alternatives. 3D contrast-enhanced ultrasound (CEUS) has shown promise for PCa localization by extraction of quantitative perfusion and dispersion features associated with cancer angiogenesis. Tissue stiffness is an additional PCa biomarker that can be quantified by ultrasound shear-wave elastography (SWE). In this study, the diagnostic potential of multiparametric ultrasound imaging, combining 3D CEUS and SWE features, was evaluated for PCa localization by comparison with the corresponding SBx outcome.MaterialsAfter signing an informed consent, 54 biopsy-naïve patients underwent a 3D CEUS recording using a LOGIQ E9 scanner (GE HealthCare, USA) and a multi-plane (probe sweep) 2D SWE scan using an Aixplorer scanner (SuperSonic Imagine, France) at the Second Affiliated Hospital of Zhejiang University (China). All patients received SBx. Multiple 3D perfusion and dispersion feature maps were extracted from the 3D CEUS acquisitions and further complemented with 3D maps of tissue stiffness, reconstructed based on the multi-plane 2D SWE acquisitions. Subsequently, all the 3D maps were subdivided in 12 regions corresponding to the SBx locations. Gradient Boosting (GB) classification was then implemented together with sequential floating forward feature selection for prediction of the biopsy outcome. The classification accuracy was assessed by a k-fold cross-validation procedure.ResultsOf the 54 patients, 20 had SBx-proven significant PCa. Prediction of biopsy outcome using only CEUS features produced an area under the receiver-operating-characteristic curve (AUC) of 0.81, while using the SWE elasticity alone produced an AUC of 0.66. Their combination yielded an improved AUC of 0.85.ConclusionsThe proposed 3D multiparametric ultrasound imaging approach yields accurate PCa classification results, showing promise for cost-effective PCa localization by multiparametric ultrasound imaging. A multicenter trial has recently started to validate and optimize the classification performance in a larger patient cohort.

Abstract:Objective To address the value of real-time precision targeted prostate biopsy using transrectal ultrasound-magnetic resonance fusion(TRUS/MRI). Methods Twenty four patients with elevated prostate-specific antigen value of 4-20ng/ml,normal or abnormal digital rectum examination,and suspicious mutiparametric magnetic resonance(mpMRI)imaging of T2-weighted or diffusion weighted or dynamic contrast enhanced were enrolled in present study from August 2015 to September 2015.Suspicious areas on mpMRI were delineated and marked by a radiologist.TRUS/MRI fusion biopsy was performed by a urologist and a ultrasonography doctor using Hitachi real-time virtue sonography(RVS)system.At first suspected lesions were biopsied transperineally using RVS system(TB)and then followed by the conventional transperineal systematic biopsy(SB)under general anesthesia.Two cores and 6-12 cores specimens were obtained respectively.The pathological results of TB and SB were compared.The correlation of mpMRI with biopsy results was also evaluated. Results TB revealed prostate cancer(PCa)in 14 of 24men(58.3%),and demonstrated a trend of increasing the cancer detection rate(CDR)compared to SB with a CDR of 37.5%.The positive rate of core in the TB men with PCa was significantly higher than in the SB men(P<0.001).Degree of suspicion on mpMRI had a significant association with CDR(P=0.002). Conclusions TRUS/MRI fusion biopsy provides improved detection of PCa in men with elevated PSA levels and suspicious MR imaging.TB specimens were significant superior to SB.The predictor value of mpMRI has a good concordance with the results of TRUS/MRI fusion biopsy.
Value of cognitive fusion targeted and standard systematic transrectal prostate biopsy for prostate cancer diagnosis

Abstract:Abstract The aim of this study was to compare the accuracies of cognitive fusion-guided targeted biopsy (TB), systematic biopsy (SB), and combined TB+SB for the detection of prostate cancer (PCa) and clinically significant PCa (csPCa) in males with lesions detected by magnetic resonance imaging (MRI). We conducted a retrospective analysis of individuals who underwent prostate biopsy at Peking University People’s Hospital (Beijing, China), with an emphasis on patients with both transrectal TB and SB. The main objective was to determine the precisions of SB, TB, and TB+SB for diagnosing PCa and csPCa. We also evaluated the detection rates of TB, SB, TB+ipsilateral-SB (ipsi-SB), TB+contralateral-SB (contra-SB), and TB+SB for PCa and csPCa in patients with unilateral MRI lesions. We compared the diagnostic yields of the various biopsy schemes using the McNemar’s test. A total of 180 patients were enrolled. The rates of PCa detection using TB, SB, and TB+SB were 52.8%, 62.2%, and 66.7%, respectively, and the corresponding rates for csPCa were 46.1%, 56.7%, and 58.3%, respectively. Among patients with unilateral MRI lesions, the PCa detection rates for TB, SB, TB+ipsi-SB, TB+contra-SB, and TB+SB were 53.3%, 64.8%, 65.6%, 61.5%, and 68.0%, respectively. TB+ipsi-SB detected 96.4% of PCa and 95.9% of csPCa cases. These findings suggest that the combination of TB+SB has better diagnostic accuracy compared with SB or TB alone. For patients with unilateral MRI lesions, the combination of TB+ipsi-SB may be suitable in clinical settings.

Abstract:Background: Prostate cancer (PCa) is one of the most common cancers among men. Various strategies for targeted biopsy based on multiparametric magnetic resonance imaging (mp-MRI) have emerged, which may improve the accuracy of detecting clinically significant PCa in recent years. Aim: To investigate the diagnostic efficiency of a template for cognitive MRI-ultrasound fusion transperineal targeted plus randomized biopsy in detecting PCa. Methods: Data from patients with an increasing prostate-specific antigen (PSA) level but less than 20 ng/mL and at least one lesion suspicious for PCa on MRI from December 2015 to June 2018 were retrospectively analyzed. All patients underwent cognitive fusion transperineal template-guided targeted biopsy followed by randomized biopsy outside the targeted area. A total of 127 patients with complete data were included in the final analysis. A multivariable logistic regression analysis was conducted, and a two-sided P < 0.05 was considered statistically significant. Results: PCa was detected in 66 of 127 patients, and 56 cases presented clinically significant PCa. Cognitive fusion targeted biopsy alone detected 59/127 cases of PCa, specifically 52/59 cases with clinically significant PCa and 7/59 cases with clinically insignificant PCa. A randomized biopsy detected seven cases of PCa negative on targeted biopsy, and four cases had clinically significant PCa. PSA density (OR: 1.008, 95%CI: 1.003-1.012, P = 0.001; OR: 1.006, 95%CI: 1.002-1.010, P = 0.004) and Prostate Imaging-Reporting and Data System (PI-RADS) scores (both P < 0.001) were independently associated with the results of cognitive fusion targeted biopsy combined with randomized biopsy and targeted biopsy alone. Conclusion: This single-centered study proposed a feasible template for cognitive MRI-ultrasound fusion transperineal targeted plus randomized biopsy. Patients with higher PSAD and PI-RADS scores were more likely to be diagnosed with PCa.

Abstract:Activation of the motile apparatus by chemokinetic factors cannot be reliably assessed in cells that are attached to a solid substratum because motility can be totally abolished by excessive adhesion. It is however, necessary to quantify the activation of the motile apparatus in order to analyze and understand chemokinetic responses. It was the purpose of the present work to establish morphological criteria that can be used to quantify motility in nonadherent (floating) neutrophils and to predict the locomotor response under conditions of limited adhesion. The proportion of neutrophils performing crawling-like movements (polarized cells) in suspension correlates very closely with stimulated locomotion at low to optimal concentration of f-Met-Leu-Phe, ie, under conditions of limited adhesion. Reduced locomotion at supraoptimal concentrations of f-Met-Leu-Phe has also morphological correlates. The major feature is the decrease in the proportion of neutrophils performing crawling-like movements and the corresponding appearance of cells that are motile but not polarized in suspension and that do not locomote on the substratum. Concentration-dependent changes in neutrophil length and in the proportion of polarized neutrophils with and without tail were also observed. The locomotor potential of neutrophils under conditions of limited contact with the substratum can be predicted on the basis of their motile behavior, in particular the proportion of cells showing crawling-like movements, in suspension. In combination with measurements of adhesion the procedure should permit a more complete analysis of the regulation of chemokinetic responses.
Magnetic Resonance Imaging - Ultrasound Fusion Targeted Biopsy Outperforms Standard Approaches in Detecting Prostate Cancer: A Meta-Analysis.

Abstract:The aim of the present study was to determine whether magnetic resonance imaging - ultrasound (MRI-US) fusion prostate biopsy is superior to systematic biopsy for making a definitive diagnosis of prostate cancer. The two strategies were also compared regarding their ability to detect clinically significant and insignificant prostate cancer. A literature search was conducted through the PubMed, EMBASE and China National Knowledge Infrastructure databases using appropriate search terms. A total of 3,415 cases from 21 studies were included in the present meta-analysis. Data were expressed as relative risk (RR) and 95% confidence interval. The results revealed that MRI-US fusion biopsy achieved a higher rate of overall prostate cancer detection compared with systematic biopsy (RR=1.09; P=0.047). Moreover, MRI-US fusion biopsy detected more clinically significant cancers compared with systematic biopsy (RR=1.22; P<0.01). It is therefore recommended that multi-parametric MRI-US is performed in men suspected of having prostate cancer to optimize the detection of clinically significant disease, while reducing the burden of biopsies.

Abstract:Background: To evaluate the role of free-hand transperineal targeted prostate biopsy using multiparametric magnetic resonance imaging-transrectal ultrasound (mpMRI-TRUS) fusion in Chinese men with repeated biopsy. Methods: A total of 101 consecutive patients suspicious of prostate cancer (PCa) at the mpMRI scan and with prior negative biopsy and elevated PSA values were prospectively recruited at two urological centers. Suspicious areas on mpMRI were defined and graded using PI-RADS score. Targeted biopsies (TB) were performed for each suspicious lesion and followed a 12-core systematic biopsy (SB). Results of biopsy pathology and whole-gland pathology at prostatectomy were analyzed and compared between TB and SB. The risk for biopsy positivity was assessed by univariate and multivariate logistic regression analysis. Results: Fusion biopsy revealed PCa in 41 of 101 men (40.6%) and 25 (24.8%) were clinically significant. There was exact agreement between TB and SB in 74 (73.3%) men. TB diagnosed 36% more significant cancer than SB (22 vs 13 cases, P = 0.012). When TB were combined with SB, an additional 14 cases (34.1%) of mostly significant PCa (71.4%) were diagnosed (P = 0.036). TB had greater sensitivity and accuracy for significant cancer than SB in 26 men with whole-gland pathology after prostatectomy. PI-RADS score on mpMRI was the most powerful predictor of PCa and significant cancer. Conclusions: Free-hand transperineal TB guided with MRI-TRUS fusion imaging improves detection of clinical significant PCa in Chinese men with previously negative biopsy. PI-RADS score is a reliable predictor of PCa and significant cancer.

Abstract:Objective:To evaluate the efficacy of magnetic resonance imaging/ultrasound (MRI/US) imaging fusion guided transperineal prostate biopsy for the diagnosis of prostate cancer.Method:From September 2014 to March 2016,121 cases were hospitalized for MRI/US imaging fusion guided transperineal prostate biopsy and their data were analyzed retrospectively.Each case underwent systematic 12-core biopsy (SB) add 2-core targeting biopsy (TB) per region of interest (ROI).Pathological findings of each biopsy core was analyzed for Gleason grade and the single-core length of prostate cancer.We recorded the clinical,imaging and pathological data of all cases.We analyzed various data by t test,rank sum test and chi-square test.Result:TB had an obvious higher single-core positive rate for prostate cancer (20.0 ％) and an obvious higher single-core positive rate for high-risk prostate cancer (10.3 ％) than those of SB (12.7 ％ and 5.5 ％),with the statistically significant difference (P =0.001 and P=0.002).TB had a higher single-core length of prostate cancer than that of SB,with the statistically significant difference (P=0.046).Positive cores of TB had a higher primary Gleason grade,secondary Gleason grade and Gleason score compared with positive cores of SB,without statistically significant difference (P＞0.05).Conclusion:When taking MRI/US imaging fusion guided transperineal prostate biopsy,TB can detect high-risk prostate cancer more effectively than SB can.MRI/US imaging fusion guided transperineal prostate biopsy is worth promoting if it is permitted.
Comparison of free-hand transperineal mpMRI/TRUS fusion-guided biopsy with transperineal 12-core systematic biopsy for the diagnosis of prostate cancer: a single-center prospective study in China

Abstract:Objectives To prospectively compare biopsy outcomes between free-hand transperineal mpMRI/TRUS fusion targeted biopsy (TB) and transperineal systematic biopsy (SB) in patients with first prostate biopsy. Patients and methods In all, 224 consecutive patients with the suspicion of PCa were investigated. All patients were evaluated by 3.0-T mpMRI applying the ESUR criteria. All patients underwent free-hand transperineal mpMRI/TRUS fusion TB and additionally a transperineal SB. Pathological findings of TB, SB, and step-sectioned RP specimens were analyzed. Results The median age of the patients was 69 (40–85) years, median PSA level was 10.05 (3.61–78.39) ng/mL, and median prostate volume was 45.5 (22–77) mL. Overall, the PCa detection rate was 50.45% (113/224). TB detected significantly more cancer [44.2% (99/224) vs. 34.8% (78/224); P = 0.001] and clinically significant PCa [75.75% (75/99) vs. 62.82% (49/78); P = 0.005] than SB. For the upgrading of Gleason score, 39.74% (31/78), more clinically significant PCa was detected by using additional TB than by SB alone. Conversely, 5.05% (5/99) more clinically significant PCa was found by SB in addition to that by TB. The location of 96.67% (58/60) and Gleason score of 60% (36/60) of TB-proven ITs were correctly identified, as corroborated by RP specimens. The median IT volume was 1.125 (0.21–19.87) ml on MRI and 1.41 (0.13–9.56) ml in RP specimens. Conclusions Free-hand transperineal mpMRI/TRUS fusion biopsy was associated with a higher detection rate of clinically significant PCa while taking fewer cores. Moreover, this technique can reliably predict the location, and relatively reliably predict cancer volume and Gleason score of ITs.

Abstract:Objective To assess the value of free-hand transperineal multiparametric nagnetic resonance imaging/transrectal ultrasound (mpMRI/TRUS) fusion-guided targeted biopsy (TB) for the diagnosis of prostate cancer(PCa).Methods Patients with elevated PSA level and/or an abnormal DRE finding were recruited prospectively between January 2015 and September 2016.Patients were classified to various scores from 2 to 5 according to prebiopsy mpMRI PI-RADS.Based on free-hand transperineal mpMRI/TRUS fusion-guiding,a 2-cores TB for each cancer-suspicious lesion were carried out and followed 12-cores systematic biopsy (SB) protocol.Pathological findings of biopsy and radical prostatectomy (RP) specimens were analyzed.Results A total of 397 patients were enrolled in this study.The median age of the patients was (68.2 ± 7.4) years old,ranging 42-78 years.The median PSA level was (15.0 ±12.4)ng/ml,ranging 3.0-88.3 ng/ml.DRE showed abnormality in 28 patients(7.1％).The median prostate volume was (41.6 ± 16.4)cm3,ranging 24.6-89.8 cm3.The PCa detection rate of TB was significantly increased compared with SB (44.8 ％ vs.34.8％) (P =0.003),especially in clinically significant PCa (P ＜ 0.001) and intermediate/high-risk PCa (P =0.003),respectively.Of the all 588 mpMRI targeted lesions,277 lesions were positive.A total of 105 index tumors were identified in RP specimens,the locations of TB-proven cancer showed 96.6％ (85/88) in correspondence with the location of the index lesion in RP specimens.Conclusions Free-hand transperineal mpMRI/TRUS fusion-guided TB providing greater detection of intermediate-high risk PCa while limits over detection of low risk PCa.Moreover,TB can reliably predict the location of an index tumor.

Abstract:Abstract Background Multiparametric MRI localizes cancer in the prostate, allowing for MRI guided biopsy (MRI-GB) 43 alongside transrectal ultrasound-guided systematic biopsy (TRUS-GB). Three MRI-GB approaches exist; visual estimation (COG-TB); fusion software-assisted (FUS-TB) and MRI ‘in-bore’ biopsy (IB-TB). It is unknown whether any of these are superior. We conducted a systematic review and meta-analysis to address three questions. First, whether MRI-GB is superior to TRUS-GB at detecting clinically significant PCa (csPCa). Second, whether MRI-GB is superior to TRUS-GB at avoiding detection of insignificant PCa. Third, whether any MRI-GB strategy is superior at detecting csPCa. Methods A systematic literature review from 2015 to 2019 was performed in accordance with the START recommendations. Studies reporting PCa detection rates, employing MRI-GB and TRUS-GB were included and evaluated using the QUADAS-2 checklist. 1553 studies were found, of which 43 were included in the meta-analysis. Results For csPCa, MRI-GB was superior in detection to TRUS-GB (0.83 vs. 0.63 [ p = 0.02]). MRI-GB was superior in detection to TRUS-GB at avoiding detection of insignificant PCa. No MRI-GB technique was superior at detecting csPCa (IB-TB 0.87; COG TB 0.81; FUS-TB 0.81, [ p = 0.55]). There was significant heterogeneity observed between the included studies. Conclusions In patients with suspected PCa on MRI, MRI-GB offers superior rates of csPCa detection and reduces detection of insignificant PCa compared to TRUS-GB. No individual MRI-GB technique was found to be better in csPCa detection. Prospective adequately powered randomized controlled trials are required.

Abstract:BackgroundThis study compared the detection rates for clinically significant prostate cancer (CSPC) between magnetic resonance imaging and ultrasonography (MRI/US)-fusion-targeted biopsy (TB), systematic biopsy (SB) and combination of TB and SB.MethodsThis prospective study evaluated simultaneous TB and SB for consecutive patients with suspicious lesions that were detected using pre-biopsy multiparametric MRI. A commercially available real-time virtual sonography system was used to perform the MRI/US-fusion TB with the transperineal technique. The prostate imaging reporting and data system version 2 (PI-RADS v2) was assigned to categorize the suspicious lesions.ResultsA total of 177 patients were included in this study. The detection rate for CSPC was higher using SB, compared to TB (57.1% vs 48.0%, p = 0.0886). The detection rate for CSPC was higher using the combination of TB and SB, compared to only SB (63.3% vs 57.1%, p = 0.2324). Multivariate analysis revealed that PIRADS v2 category 4 and an age of <65 years were independent predictors for TB upgrading (vs. the SB result).ConclusionsPI-RADS v2 category 4 and an age of <65 years were predictive factors of upgrading the Gleason score by MRI/US-fusion TB. Thus, MRI/US-fusion TB may be appropriate for patients with those characteristics.Trial registrationThis study was retrospectively registered at the University Hospital Medical Information Network (UMINID000025911) in Jan 30, 2017.

Abstract:Objective To evaluate the combined efficacy of multiparametric ultrasonography (mpUS) and multiparametric magnetic resonance imaging/transrectal ultrasound (mpMRI-TRUS) fusion for detecting clinically significant prostate cancer (csPCa). Methods From November 2019 to September 2021, biopsy-naïve patients underwent mpMRI-TRUS fusion imaging combined with mpUS-guided targeted biopsies (TB) and systematic biopsies (SB). To further evaluate the additional diagnostic value of mpUS, the imaging features of 202 focus obtained from fusion imaging were assessed. The diagnostic accuracies of mpMRI-TRUS fusion imaging and the combination of mpMRI-TRUS fusion imaging with mpUS for csPCa were comparatively evaluated. Results A total of 202 prostate lesions (160 patients) were included in the final analysis, of which 105 were csPCa, 16 were ciPCa, and 81 were noncancerous. The median patient age was 69 (65–73) years and the median tPSA was 22.07 (11.22–62.80) ng/mL. For csPCa, the detection rate of TB was higher than that of SB (50.0% vs. 45.5%, p < 0.05). The imaging characteristics of mpUS in the PCa and non-PCa groups were significantly different ( p < 0.001). When compared with mpMRI-TRUS fusion imaging, the positive predictive value, false positive rate, and area under the curve (AUC) of csPCa diagnosis by mpMRI-TRUS fusion imaging combined with mpUS increased by 11.30%, decreased by 19.58%, and increased from 0.719 to 0.770 ( p < 0.05), respectively. Conclusion TB can improve the detection rate of csPCa and hence can be effectively used in the diagnosis and risk assessment of csPCa. The mpUS-enriched valuable diagnostic information for mpMRI-TRUS fusion imaging and their combination showed a higher diagnostic value for csPCa, which can guide subsequent clinical treatment.

Abstract:Systematic prostate biopsy has limitations, such as overdiagnosis of clinically insignificant prostate cancer and underdiagnosis of clinically significant prostate cancer. Magnetic resonance imaging (MRI)-guided biopsy, a promising alternative, might improve diagnostic accuracy. To compare the cancer detection rates of systematic biopsy and combined biopsy (systematic biopsy plus MRI-targeted biopsy) in Asian men, we conducted a retrospective cohort study of men who underwent either systematic biopsy or combined biopsy at two medical centers (Queen Mary Hospital and Tung Wah Hospital, Hong Kong, China) from July 2015 to December 2022. Descriptive statistics were calculated, and univariate and multivariate logistic regression analyses were performed. The primary and secondary outcomes were prostate cancer and clinically significant prostate cancer. A total of 1391 participants were enrolled. The overall prostate cancer detection rates did not significantly differ between the two groups (36.3% vs 36.6%, odds ratio [OR] = 1.01, 95% confidence interval [CI]: 0.81-1.26, P = 0.92). However, combined biopsy showed a significant advantage in detecting clinically significant prostate cancer (Gleason score ≥ 3+4) in patients with a total serum prostate-specific antigen (tPSA) concentration of 2-10 ng ml -1 (systematic vs combined: 11.9% vs 17.5%, OR = 1.58, 95% CI: 1.08-2.31, P = 0.02). Specifically, in the transperineal biopsy subgroup, combined biopsy significantly outperformed systematic biopsy in the detection of clinically significant prostate cancer (systematic vs combined: 12.6% vs 24.0%, OR = 2.19, 95% CI: 1.21-3.97, P = 0.01). These findings suggest that in patients with a tPSA concentration of 2-10 ng ml -1 , MRI-targeted biopsy may be of greater predictive value than systematic biopsy in the detection of clinically significant prostate cancer.

Abstract:Introduction The classic way of diagnosing prostate cancer (PCa) is by conducting the 12-core systematic biopsy (SB). However, it has a low detection rate for clinically significant PCa (csPCa) and can lead to the detection of clinically insignificant PCa (cisPCa). Although MRI-transrectal ultrasound (MRI-TRUS) fusion targeted biopsy (TB) can effectively improve the detection rate of csPCa, it may still miss some cases. Therefore, we propose using a combination of TB and SB methods to enhance the detection rate of csPCa while minimising the detection rate of cisPCa. Methods and analysis This study is a prospective, single-centre investigation that aims to assess and compare the detection rate of csPCa using MRI-TRUS fusion TB combined with SB versus TRUS 12-core SB alone. Biopsy-naïve men with suspected PCa will be subjected to multiparametric MRI. Patients with Prostate Imaging Reporting and Data System (V.2.1) score ≥3 will be enrolled in the TB-SB combination group. The sample size is established as 660 participants, considering a 10% drop-out rate. The primary outcome is the detection rate of csPCa in men without prior biopsy using MRI-TRUS fusion TB combined with the standard TRUS-guided 12-core SB method. CsPCa will be defined as International Society of Urological Pathology Grade ≥2. Ethics and dissemination This study has been approved by the Ethics Committee at the Shanghai Tenth People's Hospital, an affiliated hospital of Tongji University School of Medicine. The research results will be published in a peer-reviewed international journal. Trial registration number ChiCTR2000036089.

Abstract:MRI-guided targeted biopsy (MRGB) was recommended as part of biopsy paradigm of prostate cancers by current guidelines. This study aimed to analyze the diagnostic efficacy of MRGB and systemic biopsy (SB), and to compare diagnostic capabilities within subgroups of MRGB: MRI-cognitive biopsy (MRCB) and MRI-fusion biopsy (MRFB). We retrospectively enrolled patients who underwent MRGB for suspicious malignant lesion(s) identified on MRI in a single tertiary center, sample size was 74 patients. An mpMRI was performed prior to biopsy and reviewed by an experienced radiologist specialized in prostate cancer. Per-person results of MRGB and each concomitant SB were analyzed as independent biopsies for its positive biopsy rate and positive core percentage. Per-lesion results of MRFB and MRCB were compared for the detection rate. Variables of interest were analyzed with t-test, chi-squared test, and logistic regression analysis. Statistical analyses were performed with IBM Statistical Product and Service Solutions (SPSS), Version 23 (IBM, Armonk, New York). Total of 74 patients fulfilled the inclusion criteria and were enrolled. MRFB had higher PCa detection rate comparing to both MRCB and SB (56.1%, 30.3%, and 33.9% respectively, p value = 0.036); clinically significant prostate cancer (csPCa) detection rate was also significantly higher in MRFB group (43.9%, 24.2%, and 16.9% in each group respectively, p value = 0.011). In per-lesion analysis, MRCB and MRFB had no significant difference in PCa and csPCa detection rate (41.0% vs. 26.2% and 29.5% vs. 16.7% respectively, p value = 0.090 and 0.103). In the lesion ≦ 1.3 cm group, MRFB could achieve higher PCa detection rate, comparing to MRCB (36.4% vs. 14.3%, p value = 0.047); there were also higher positive rates for PCa and csPCa per biopsied cores (22.1% vs. 6.8% and 15.6% vs. 2.7%, p value = 0.029 and 0.028, respectively). Further logistic regression of multi-variate analysis in subgroup of lesion ≦ 1.3 cm revealed that PIRADS score and biopsy method were significant predictors of positive biopsy result for PCa (p value = 0.045 and 0.026, respectively) and for csPCa (p value = 0.043 and 0.025, respectively). In patients receiving trans-perineal prostate biopsy, MRFB had higher cancer detection rate than MRCB and SB. In per lesion comparison, MRFB and MRCB had similar diagnostic accuracy. However, in lesions with diameter less than 1.3 cm, MRFB can provided better diagnose value for PCa and csPCa than MRCB.

Abstract:This retrospective study assessed the diagnostic accuracy of targeted biopsy (TB) and unilateral systematic biopsy in detecting clinically significant prostate cancer (csPCa) in 222 men with single magnetic resonance imaging (MRI) lesions (Prostate Imaging Reporting and Data System [PI-RADS] ≥ 3). Patients underwent multiparametric MRI and MRI/ultrasound fusion TB and 12-needle standard biopsy (SB) from September 2016 to June 2021. The study compared the diagnostic performance of TB + iSB (ipsilateral), TB + contralateral system biopsy (cSB) (contralateral), and TB alone for csPCa using the χ 2 test and analysis of variance. Among 126 patients with csPCa (ISUP ≥ 2), detection rates for TB + iSB, TB + cSB, and TB were 100, 98.90, and 100% for lesions, respectively. TB + iSB showed the highest sensitivity and negative predictive value. No significant differences in accuracy were found between TB + iSB and the gold standard for type 3 lesions ( P = 1). For types 4–5, detection accuracy was comparable across methods ( P = 0.314, P = 0.314, P = 0.153). TB had the highest positive needle count rate, with TB + iSB being second for type 3 lesions (4.08% vs 6.57%, P = 0.127). TB + iSB improved csPCa detection rates and reduced biopsy numbers, making it a viable alternative to TB + SB for single MRI lesions.

Abstract:ObjectivesTo report our experience with free-hand transperineal targeted biopsy with real-time transrectal ultrasound (TRUS) and multiparametric magnetic resonance imaging (mpMRI) fusion images for the diagnosis of prostate cancer (PCa).Patients and methodsA total of 62 consecutive patients suspicious of PCa at the mpMRI scan and PSA >4.0 ng/mL were recruited prospectively. Targeted biopsies (TBs) were carried out for each cancer-suspicious lesion and followed a 12-core systematic biopsy (SB) protocol. Pathological findings of TB and SB were analyzed.ResultsThe age of the patients was 68.38 ± 6.57 years (range 51–79 years). The preoperative PSA value was 10.21 ± 5.57 ng/mL (range 4.5–30.1 ng/mL). Preoperative prostate volume was 34.05 ± 9.86 mL (range 19–64 mL). The PCa patients detected by SB and/or TB were 34 (54.8 %). Cancer-detected rates of SB and TB cores were 7.53 and 26.2 %, respectively (P < 0.001). The positive core length of SB and TB cores was 3.71 ± 2.77 mm (range 1–14 mm) and 5.00 ± 3.04 mm (range 2–17 mm), respectively (P = 0.016). The positive core percent of SB and TB cores was 28.77 ± 20.13 % (range 7–100 %) and 35.76 ± 18.73 (range 11–100 %), respectively (P = 0.048). Moreover, clinically significant PCa cores detected by the SB and TB were 19 cores (2.6 %) and 48 cores (18.5 %), respectively (P < 0.001).ConclusionsFree-hand transperineal TB using real-time TRUS and mpMRI fusion imaging has the ability to improve sampling quality and detect more clinically significant PCa compared with SB.
