Introduction:A chronic patellar tendon rupture in adolescent is very rare but can happen in sport-related injury. Presentation at more than 6 weeks after injury might cause a significant disability. Prompt diagnosis and treatment are key factors to a good prognosis. In this case, we use an autologous semitendinosus tendon graft to repair the patellar tendon rupture.Case Report:A 17-year-old male presented with pain at left knee since 6 months ago. The pain started after he landed on his left knee, having jumped while playing volleyball. On physical examination of the left knee, edema and high patella were found. There was also a palpable gap at the inferior pole of the patella. The patient was unable to do active extension of the left lower leg. MRI evaluation of the left knee revealed a patellar tendon rupture, myositis ossificans and a partial ACL rupture. The patient was scheduled for an elective surgery, consisting of patellar tendon reconstruction using an autologous semitendinosus graft along with ossificans removal and debridement.Discussion:Risk factors of tendon rupture should be assessed with extensive evaluation to construct the appropriate surgical repair method. The decision on which method to use depends greatly on the patient's profile and nature of injury. Immobilization period and routine rehabilitation exercises are vital to an enhanced recovery and fully functional knee.Conclusion:An autologous semitendinosus graft provides a good result without the need of a steel wire, as demonstrated in our case. Rehabilitation exercise program after surgical treatment of patellar tendon rupture: A case report

Abstract:Objective: This case report describes in detail a rehabilitation and exercise program following surgical treatment of patellar tendon rupture and offers postoperative therapeutic goals and recommendations, with the aim of expediting the resumption of activities of daily living and sports participation following the surgical procedure. In addition, this report analyzes the cause of the patellar tendon rupture in this case. Design: Case report. Participants: The patient, a 25-year-old male recreational athlete (height: 184 cm; weight: 80 kg; right-hand dominant), experienced a complete rupture of the patellar tendon upon landing on the left lower extremity during an unopposed dribble turn. Rehabilitation exercise program: A well-structured rehabilitation program was implemented. Passive knee flexion range of motion (ROM) exceeded 90° at week 5 and was fully recovered at week 16. A second surgery was performed at week 19 to remove the decompression wires, and the patient progressively resumed basic physical and specialized training at 7 months post-surgery. Main outcome measures: Morphometric and functional tests were performed to measure the effectiveness of rehabilitation throughout the postoperative process. Conclusions: Early surgical repair and an immediate postoperative rehabilitation program have a positive impact on knee ROM, function, and muscle strength. The causes of the patellar tendon rupture in this patient included long-standing uncontrolled patellar tendinopathy, impaired mobility of the ankle and hip joints, poor landing technique, and muscle strength imbalances.

Abstract:INTRODUCTION: The simultaneous rupture of anterior cruciate ligament (ACL) and patellar ligament (PL) is an infrequent condition. Each isolated injury has surgical techniques and rehabilitation protocols that differ widely among each other. Nonetheless, there is no established physical rehabilitation approach when both injuries are associated.OBJECTIVE: The aim of this report is to describe the rehabilitation and the outcomes obtained in the postoperative period of simultaneous rupture of ACL and PL and the follow-up period.CASE REPORT: A 21-year-old male patient suffered the rupture of ACL and PL after landing from a jump while playing soccer. The knee was immediately immobilized, and 10 weeks later, he was operated in a one-stage surgery. He initiated his rehabilitation 3 weeks after the surgical resolution. A three-times a week rehabilitation was implemented to restore range of motion and improve functional status. An extensive evaluation was carried out monthly using dynamic neuromuscular tests and self-reported questionnaires.RESULTS: At the end of the rehabilitation, he presented a complete extension and 130° of knee flexion in passive open kinetic chain. The Single Leg Squat and Landing Error Scoring System showed a good performance, while the Star Excursion Balance Test and the single hops assessments were symmetrical. The International Knee Documentation Committee and Lower Extremity Functional Scale questionnaires yielded values of 90.8% and 77 points, respectively.CONCLUSION: An approach based on mobility exercises and strengthening of the lower limbs and the core muscles, considering the biological healing times of each particular structure, made it possible to obtain satisfactory results in mobility, functional tests, self-reported questionnaires, and patient's satisfaction.

Abstract:In order to obtain some fine structural informations available for the identification of hair samples collected in small amounts or fragments, the preparation method of the samples for the scanning electron microscopy was investigated. Hair samples of human individuals and animals were mounted between two sheets of celluloid using isoamyl acetate and were sectioned to 20 microns thickness. These sections were collected in a test tube for dissolving the celluloid. After centrifuging in acetone, a small mass of sections was embedded in Spurr's epoxy resin. After polymerization, 1 micron thick sections were made on an ultramicrotome. The sections were mounted on glass slide and were surface-etched by exposure to oxygen plasma for 90 seconds. Observations were made in the secondary electron image mode of a scanning electron microscope. After surface-etching, a good resolution of the cross section was obtained. The arrangement of the cortical cells of human hairs was observed to be fairly irregular with conspicuous interdigitations of cell boundaries as compared with that of animal hairs. The medulla of human and animal hairs were filled with cellular remnants of destroyed medullary cells showing fibrous structures. The lamellar structures of cuticular cells became very clearly visible. The number and the overall thickness of cuticular cell layers in human and animal hairs investigated varied considerably between scalp and pubic hairs and from animal to animal.
From Trauma to Recovery: A Comprehensive Management of Achilles Tendon Injury in a Young Female

Abstract:The incidence of the Achilles tendon getting injured has recently increased by 18 in 100,000. Compared to non-surgical treatment, surgical results are superior. The Achilles tendon repaired with surgery has a re-rupture rate of only 5%, while if treated non-operatively, it has a rupture rate of 40%. This case report analyses the traumatic Achilles tendon rupture and subsequent surgical repair in a young woman. In this case study, a 19-year-old female patient's severe Achilles tendon injury was successfully managed by integrating prompt surgical intervention and structure. After rehabilitation, the patient's range of motion (ROM), muscle strength, and gait patterns all significantly improved. Scores on the Lower Extremity Functional Scale (LEFS) and the Dynamic Gait Index (DGI) both significantly improved. This case study reiterates the significance of an integrated healthcare strategy for Achilles tendon injuries. An immediate surgical procedure followed by a specific rehabilitation programme accelerates healing and the return to optimal function. The results emphasize the critical role of physical therapy in assisting surgical interventions and underline the necessity of comprehensive patient care in the treatment of complex orthopaedic problems.

Abstract:Introduction: Posterior cruciate ligament (PCL) injury hampers the rotational stability and stability front to back of the knee joint, seriously affecting the quality of life of patients. Some studies have reported that the peroneus longus tendon (PLT) has sufficient length and strength.Hypothesis: We hypothesized that the PLT can be used as a novel appropriate material for PCL recon-struction therapy. Materials and methods: Herein, we systematically analysed the clinical effect of the double-stranded PLT and the four-stranded hamstring tendon in the reconstruction of the PCL and com-pared the effectiveness and safety of these two surgical approaches in the reconstruction of PCL injury. A total of 48 patients with complete rupture of the PCL were divided into Group A (reconstructed with the double-stranded PLT, 25 cases) and Group B (reconstructed with the four-stranded hamstring tendon, 23 cases).Results: The patients were followed up for more than 1 year. The intraoperative time for tendon extraction was significantly shorter in Group A than Group B. Twenty-four months after operation, patients in the two groups showed the alleviated tibial posterior displacement and the increased IKDC score, Lysholm score and Tegner score. Nevertheless, these scores showed no significant differences between the two groups. Additionally, compared with the therapy using the four-stranded hamstring tendon, therapy using the double-stranded PLT is simpler and safer.Discussion: Both surgical methods are effective in the treatment of PCL injury. The PLT could be a good choice for PCL injury reconstruction material, especially when the four-stranded hamstring tendon is accidentally damaged or ineffective. Abstract:Case: A 22-year-old athlete sustained a traumatic posterior tibial tendon (PTT) rupture with medial malleolus fracture and anterior tibial plafond chondral lesion during practice. He underwent PTT repair, ankle arthroscopy with microfracture, and medial malleolus open reduction and internal fixation with deltoid ligament repair. At 1 year postoperatively, the patient returned to activity excluding high-level competition. Conclusion: Anterior tibial chondral impaction injury with an avulsion fracture of the medial malleolus and PTT tear is effectively treated with a combined arthroscopic and open approach.
Non-union Patella Fracture - Extensor Mechanism Repair with Quadriceps Release: A Case Report

Abstract:Introduction: Non-union patella is rarely seen in everyday outpatient department. Displaced patella fractures usually require surgical treatment for optimum results. The proximal fragment is retracted by the pool of quadriceps muscle. Conservatively managed displaced patella fractures lead to nonunion and impaired knee function. Case report: In our study, a 10-month-old neglected displaced patella fracture with impaired knee function was treated with extensor mechanism repair augmented with bone graft. A good functional outcome was observed at the end of 12 months. A good knee range of motion from full extension to 110° was observed. Conclusion: We conclude that a stable fixation augmented with bone grafting and quadriceps release which augments bone healing gives good functional outcomes.

Abstract:BACKGROUND: This study evaluated a patellar tendon shortening (PTS) surgical procedure that uses an overlapping repair combined with an additional Tycron non-absorbable suture to support the shortening in children with Cerebral Palsy (CP). This study aimed to outline this surgical technique and to evaluate its effectiveness in restoring the knee extensor mechanism.METHODS: The sagittal plane lower limb kinematics, peak knee extensor moment, gait deviation index (GDI), localised movement deviation profile (MDP), temporospatial parameters, passive knee extension ROM, quadriceps lag, and knee extensor strength were calculated pre- and postoperatively. To determine significant differences a robust linear regression model with high breakdown point and high efficiency was fitted to the data.RESULTS: In this retrospective cohort study, a total of 41 patients with CP who were treated with unilateral or bilateral PTS in isolation or as part of single event multilevel surgery (SEMLS), with a mean age of 11.1 years were included. The knee extension angle improved at initial contact (p < 0.0001), and during stance phase (p < 0.0001). The peak internal knee extensor moment decreased during early (p = 0.0014) and late stance phase (p < 0.0001). The quadriceps lag decreased (p < 0.0001) and knee extensor strength increased (p < 0.0001). The GDI improved (p < 0.0001), as well as the localised MDP for sagittal angles (p < 0.0001) and moments (p = 0.0001). Walking speed (p = 1.0) remained unchanged, but the cadence decreased (p = 0.024) and step length increased (p = 0.0001).CONCLUSIONS: The knee extension angle and moment during stance phase improved significantly. The children with CP in this study showed improvements in knee extensor strength and quadriceps lag. Thereby it can be concluded that the PTS procedure was able to restore the knee extensor mechanism effectively.

These patients were reassessed at 10, 24, and 48 weeks post-surgery. The techniques included staged surgery using patellar transosseous traction in the first stage and a modified Kelikian technique in the second stage.ResultsThe study reported satisfactory functional and pain-related outcomes (Lysholm and IKDC scales), reduction in patellar height, and restoration of the full active range of motion in the affected knee.ConclusionThe two-stage surgical technique for chronic patellar tendon rupture, incorporating patellar transosseous traction and a modified Kelikian technique, is simple, reproducible, and yields satisfactory clinical results.

Abstract:The posterior cruciate ligament (PCL), one of the key ligaments in the knee, serves to prevent backward movement of the tibia relative to the femur. A simultaneous occurrence of a PCL avulsion fracture and a femur shaft fracture in a pediatric patient suggests a complex orthopedic injury resulting from significant trauma to the knee and thigh area. This study describes the rehabilitation process of a 12-year-old female involved in a road traffic accident, who suffered both a midshaft femur fracture and a PCL avulsion fracture. Following surgical procedures, the patient underwent a comprehensive physiotherapy regimen utilizing X-Sens inertial sensor technology. The rehabilitation plan comprised multiple stages targeting pain alleviation, muscle strengthening, flexibility exercises, gait retraining, and balance improvement. Various interventions including contrast baths, cryotherapy, patellar mobilization, isotonic and resistance exercises, and progressive gait training were integrated across different phases of the rehabilitation program. Over subsequent follow-up periods, the patient demonstrated significant enhancements in pain management, range of motion, muscle strength, functional capabilities, and gait metrics. This case report underscores the efficacy of a systematic physiotherapy strategy incorporating advanced technology in the successful recovery from intricate lower limb fractures, underscoring the importance of prompt intervention and multidisciplinary collaboration for optimal patient outcomes.
Reinforcement of Chronic Patellar Tendon Repair with High Strength Polyester Sutures

Abstract:Objective To summarize the clinical features of chronic ruptured patellar tendon, and introduce the augmentation technique of repair of chronic ruptured patellar tendon with high strength polyester sutures and postoperative rehabilitation.Methods Six cases of chronic patellar tendon rupture were treated and reinforced with high strength polyester sutures from 2002 to 2007.Patellar tendon ruptures was primary end Ho -end repaired, and reinforced with 4 polyester sutures that were passed through in-traosseous tunnels within the patella and tibial tubercle.The surgery reestablished knee extensor continuity and restored the length of patellar tendon.Augmentation techniques were reliable, and allowed patients to walk and bend the knee, regain adequate quadriceps strength early.Postoperative rehabilitation: on the 1st day after surgery, walking upright and passive flexion of 90° was allowed, and brace protection was unnecessary; 3 weeks later, straight leg raise; 6 weeks later, brisk walking; 12 weeks later, squatting and climbing stairs; 6 months later, sports was allowed, including the jump and kick.Results All patients were followed up from 1 to 5 years.No patient sustained a re-rupture.All patients satisfied with their result.The Lysholm score were 100.Kach knee had restored full range of motion 6 months later, and regained adequate quadriceps strength.Conclusion Augmentation techniques with high strength polyester sutures is reliable and demonstrate good intermediate to long-term results.It can avoid re-operation for removal of the cerclage wire, allow patients to walk and bend the knee, regain adequate quadriceps strength early.

Abstract:Background and purpose: Dashboard injuries with forced lateral rotation of the leg can cause tibial plateau fractures with medial meniscus and ACL injuries. Tibial plateau fractures are difficult to treat and can have devastating effects such as discomfort, deformity, restricted ROM causing early degenerative changes in the knee joint. The purpose of this case report is to explain the physiotherapy intervention options and to assess their success in the conservative management of such injury. Case description: 33-year-old female presented to physical therapy with severe pain over the right knee while standing, unable to bend or straighten the knee and had a feeling of giving away of knee while walking since the time patient experienced a fall from her motorcycle. The condition was managed with supervised and unsupervised physiotherapy for 8 weeks. Intervention consisted of patient’s education about the condition, immobilization, electrotherapeutic modalities, exercise therapy, posture, gait, balance and proprioception training and counselling for 8 weeks, six days a week, for 45–60 minutes each day. Outcome: The patient gained the knee ROM and strength in the lower extremities while achieving all short-term goals. She began ambulating independently and was mobile and autonomous in most situations. Her Oxford knee score improved by 75%. Discussion: It has been demonstrated that physical therapy interventions are helpful in reducing patients' functional limitations. Due to a lack of supporting data, this case report describes specific procedures that were used to reduce functional restrictions caused by a medial meniscus rupture and a lateral tibial plateau fracture. Abstract:Although the surgical reconstruction be the obvious indication for the anterior cruciate ligament (ACL) lesion, there is no consensus on whether the results of surgery are superior to those obtained with nonsurgical management. The objective of this report was to describe a case of nonsurgical treatment for ACL rupture followed by a contralateral rupture. A 28-year-old female practitioner of muay-thai and handball suffered a non-contact ACL rupture in the left knee, and three months after the end of rehabilitation, the patient suffered a second non-contact ACL rupture in the contralateral knee and also received nonsurgical treatment. After both ruptures the patient received a treatment program focused on the strengthening of the quadriceps and hamstring muscles, trunk stabilization, plyometrics exercises, perturbation training, and return-to-sport training. After the treatments the patient exhibited absence of pain; normal muscular function and knee extension and flexion strength; normal range of motion; normal hop tests (<10% difference between members); improvement in the knee functional capacity and total return to normal activities. After two years of follow-up, the patient remained pain free and with normal knee function. The findings demonstrate the physical therapy effects in the nonsurgical treatment of bilateral ACL rupture. The patient could return to sport practice without instability. Further studies with a larger sample are needed to assess the recovery capacity and the full return to sport activities of patients with ACL injury.

Minimal invasive medial patellofemoral ligament reconstruction using quadriceps tendon with de novo technique and arthroscopic guided: A case report

Abstract:Introduction: Medial Patellofemoral ligament (MPFL) reconstruction, the primary procedure for restoring patellar stability and preventing further dislocation, has seen numerous methods and grafts. However, a consensus on the technique and graft remains elusive. This study introduces a novel approach to MPFL reconstruction, utilizing a unique combination of minimal invasiveness, de novo method, and arthroscopic guidance to harvest a quadriceps tendon (QT) graft without the need for a harvesting tool. Case illustration: A 37-year-old female presented to the emergency room with the chief complaints of left knee pain and swelling following the third spontaneous dislocation of the patella. The patient had a few episodes of locking sensation and instability of the left knee while walking, especially while climbing stairs. A knee MRI revealed MPFL tear in the left knee. A modified minimal invasive MPFL reconstruction technique was performed. The IKDC, Lysholm, and Modified Cincinnati scores improved significantly from preoperatively 48.3 %, 49 %, and 51 % to 89.7 %, 90 %, and 95 % consecutively. No recurrent patellar dislocations or swelling were reported during follow-up. Discussion: While quadriceps graft harvesting for MPFL reconstruction is typically highly invasive, using quadriceps tendons has consistently shown excellent outcomes. This study, however, takes it a step further by demonstrating that a minimally invasive procedure for harvesting quadriceps tendons, even without specialized instrumentation, can also yield excellent results. Conclusion: MPFL reconstruction using superficial quadriceps tendon autograft can be achieved using a minimally invasive technique without the use of specialized instrumentation and has shown excellent knee functional improvement.

Abstract:This case report explains the successful management of a rare, combined injury: an undisplaced patellar fracture and a posterior cruciate ligament (PCL) avulsion fracture at the tibial attachment in a 44-year-old male patient following a motorbike accident. While both injuries are frequently seen in orthopedic practice, their concurrent occurrence is uncommon. The patient presented with significant knee swelling, limited range of motion, and pain following the accident. An X-ray revealed a patellar fracture and magnetic resonance imaging (MRI) confirmed an undisplaced fracture, a PCL tear, and a medial meniscus injury. The patient underwent surgical intervention for PCL fixation with a cannulated cancellous (CC) screw under spinal anesthesia. Following surgery, a comprehensive rehabilitation program was implemented, focusing on pain management, reducing swelling, regaining range of motion, and strengthening the surrounding musculature. The program progressed through three phases, steadily increasing the intensity and complexity of exercises. The patient exhibited significant improvement in pain, swelling, range of motion, and muscle strength throughout the rehabilitation program. By week 12, he had achieved near-normal knee function and was able to resume most daily activities.

Abstract:Patellar tendon (PT) rupture following total knee arthroplasty (TKA) is a rare, but devastating complication. These injuries occur most frequently in the acute period following TKA due to trauma to the knee. PT ruptures that disrupt the extensor mechanism create a marked functional deficit, impacting every facet of daily life. In complete ruptures of the PT, repair or reconstruction is typically indicated; however, complication rates following intervention remain high. Operative intervention remains the mainstay of treatment, with only certain specific situations where nonoperative intervention is appropriate. Operative techniques are chosen based on the acuity, location of disruption, and status of the residual soft tissues. Treatment options include repair with or without augmentation or reconstruction. Augmentation does reduce the high risk of complications, bringing rates down from 63 to 25%. Augmentation options include autografts, allografts, synthetic grafts, or synthetic meshes. Despite advancements, outcomes are unpredictable and complications are common, highlighting the need for further research to improve treatment protocols. This article provides an overview of PT ruptures following TKA, the various treatment options, and the recommendations of the M.M., R.D., G.S. for each common type of PT injury encountered. Iterative Rupture of the Patellar Tendon: A Case Report of an Original Technique for Revision Reconstruction Using an Adjustable Loop and an Artificial Ligament

Abstract:Chronic rupture of the patellar tendon is a severe injury that leads to dramatic functional consequences including lack of extension and walking difficulty. Surgery is the gold standard to treat this type of injury, but revision reconstructions are problematic because an ipsilateral graft was often harvested for the initial surgery. Because fibrotic tissues on the patellar tendon need to be debrided, another graft must be added to reinforce the tendon. We reported the case of a former semiprofessional handball player, a 29-year-old man who presented an iterative rupture with the fracture of the transverse patellar tunnel 6 months after reconstruction using a semitendinosus graft and suture repair. We performed revision reconstruction surgery using an artificial ligament placed between the extensor mechanism and the tibia for extra-articular reinforcement to maintain extensor mechanism continuity. Two adjustable loops were also used to repair the patellar tendon tear. At 2-year follow-up, the patient was able to resume the practice of handball at a competitive level with good clinical and functional results. This technique can therefore be used as a salvage procedure for chronic iterative rupture of the patellar tendon.
