Early readmission prediction enables early intervention, which is essential to preventing serious or life-threatening events, and act as a substantial contributor to reduce healthcare costs. Existing works on predicting readmission often focus on certain vital signs and diseases by extracting statistical features. They also fail to consider skewness of class labels in medical data and different costs of misclassification errors. In this paper, we recur to the merits of convolutional neural networks (CNN) to automatically learn features from time series of vital sign, and categorical feature embedding to effectively encode feature vectors with heterogeneous clinical features, such as demographics, hospitalization history, vital signs, and laboratory tests. Then, both learnt features via CNN and statistical features via feature embedding are fed into a multilayer perceptron (MLP) for prediction. We use a cost-sensitive formulation to train MLP during prediction to tackle the imbalance and skewness challenge. We validate the proposed approach on two real medical datasets from Barnes-Jewish Hospital, and all data is taken from historical EMR databases and reflects the kinds of data that would realistically be available at the clinical prediction system in hospitals. We find that early prediction of readmission is possible and when compared with state-of-the-art existing methods used by hospitals, our methods perform significantly better. For example, using the general hospital wards data for 30-day readmission prediction, the area under the curve (AUC) for the proposed model was 0.70, significantly higher than all the baseline methods. Based on these results, a system is being deployed in hospital settings with the proposed forecasting algorithms to support treatment.

Using Machine Learning to Predict Likelihood and Cause of Readmission After Hospitalization for Chronic Obstructive Pulmonary Disease Exacerbation

Abstract:Background: Chronic obstructive pulmonary disease (COPD) is a leading cause of hospital readmissions. Few existing tools use electronic health record (EHR) data to forecast patients' readmission risk during index hospitalizations. Objective: We used machine learning and in-hospital data to model 90-day risk for and cause of readmission among inpatients with acute exacerbations of COPD (AE-COPD). Design: Retrospective cohort study. Participants: Adult patients admitted for AE-COPD at the University of Chicago Medicine between November 7, 2008 and December 31, 2018 meeting International Classification of Diseases (ICD)-9 or -10 criteria consistent with AE-COPD were included. Methods: Random forest models were fit to predict readmission risk and respiratory-related readmission cause. Predictor variables included demographics, comorbidities, and EHR data from patients' index hospital stays. Models were derived on 70% of observations and validated on a 30% holdout set. Performance of the readmission risk model was compared to that of the HOSPITAL score. Results: Among 3238 patients admitted for AE-COPD, 1103 patients were readmitted within 90 days. Of the readmission causes, 61% (n = 672) were respiratory-related and COPD (n = 452) was the most common. Our readmission risk model had a significantly higher area under the receiver operating characteristic curve (AUROC) (0.69 [0.66, 0.73]) compared to the HOSPITAL score (0.63 [0.59, 0.67]; p = 0.002). The respiratory-related readmission cause model had an AUROC of 0.73 [0.68, 0.79]. Conclusion: Our models improve on current tools by predicting 90-day readmission risk and cause at the time of discharge from index admissions for AE-COPD. These models could be used to identify patients at higher risk of readmission and direct tailored post-discharge transition of care interventions that lower readmission risk.

Abstract:Unscheduled 30-day readmissions are a hallmark of Congestive Heart Failure (CHF) patients that pose significant health risks and escalate care cost. In order to reduce readmissions and curb the cost of care, it is important to initiate targeted intervention programs for patients at risk of readmission. This requires identifying high-risk patients at the time of discharge from hospital. Here, using real data from over 7500 CHF patients hospitalized between 2012 and 2016 in Sweden, we built and tested a deep learning framework to predict 30-day unscheduled readmission. We present a cost-sensitive formulation of Long Short-Term Memory (LSTM) neural network using expert features and contextual embedding of clinical concepts. This study targets key elements of an Electronic Health Record (EHR) driven prediction model in a single framework: using both expert and machine derived features, incorporating sequential patterns and addressing the class imbalance problem. We evaluate the contribution of each element towards prediction performance (ROC-AUC, F1-measure) and cost-savings. We show that the model with all key elements achieves higher discrimination ability (AUC: 0.77; F1: 0.51; Cost: 22% of maximum possible savings) outperforming the reduced models in at least two evaluation metrics. Additionally, we present a simple financial analysis to estimate annual savings if targeted interventions are offered to high risk patients.

Abstract:Chronic obstructive pulmonary disease (COPD) is a significantly concerning disease, and is ranked highest in terms of 30-day hospital readmission. Generally, physical activity (PA) of daily living reflects the health status and is proposed as a strong indicator of 30-day hospital readmission for patients with COPD. This study attempted to predict 30-day hospital readmission by analyzing continuous PA data using machine learning (ML) methods. Data were collected from 16 patients with COPD over 3877 days, and clinical information extracted from the patients' hospital records. Activity-based parameters were conceptualized and evaluated, and ML models were trained and validated to retrospectively analyze the PA data, identify the nonlinear classification characteristics of different risk factors, and predict hospital readmissions. Overall, this study predicted 30-day hospital readmission and prediction performance is summarized as two distinct approaches: prediction-based performance and event-based performance. In a prediction-based performance analysis, readmissions predicted with 70.35% accuracy; and in an event-based performance analysis, the total 30-day readmissions were predicted with a precision of 72.73%. PA data reflect the health status; thus, PA data can be used to predict hospital readmissions. Predicting readmissions will improve patient care, reduce the burden of medical costs burden, and can assist in staging suitable interventions, such as promoting PA, alternate treatment plans, or changes in lifestyle to prevent readmissions.

Abstract:While machine learning approaches can enhance prediction ability, little is known about their ability to predict 30-day readmission after hospitalization for Chronic Obstructive Pulmonary Disease (COPD). We identified patients aged ≥40 years with unplanned hospitalization due to COPD in the Diagnosis Procedure Combination database, an administrative claims database in Japan, from 2011 through 2016 (index hospitalizations). COPD was defined by ICD-10-CM diagnostic codes, according to Centers for Medicare and Medicaid Services (CMS) readmission measures. The primary outcome was any readmission within 30 days after index hospitalization. In the training set (randomly-selected 70% of sample), patient characteristics and inpatient care data were used as predictors to derive a conventional logistic regression model and two machine learning models (lasso regression and deep neural network). In the test set (remaining 30% of sample), the prediction performances of the machine learning models were examined by comparison with the reference model based on CMS readmission measures. Among 44,929 index hospitalizations for COPD, 3413 (7%) were readmitted within 30 days after discharge. The reference model had the lowest discrimination ability (C-statistic: 0.57 [95% confidence interval (CI) 0.56–0.59]). The two machine learning models had moderate, significantly higher discrimination ability (C-statistic: lasso regression, 0.61 [95% CI 0.59–0.61], p = 0.004; deep neural network, 0.61 [95% CI 0.59–0.63], p = 0.007). Tube feeding duration, blood transfusion, thoracentesis use, and male sex were important predictors. Abstract:Hospital readmissions consume large amounts of medical resources and negatively impact the healthcare system. Predicting the readmission rate early one can alleviate the financial and medical consequences. Most related studies only select the patient’s structural features or text features for modeling analysis, which offer an incomplete picture of the patient. Based on structured data (including demographic data, clinical data, administrative data) and medical record text, this paper uses deep learning methods to construct an optimal model for hospital readmission prediction, tested on a dataset of heart disease patients’ 30-day readmission. The results show that when only structured data is used, the deep learning model is much better than the Naive Bayes model and slightly better than the Support Vector Machine model. Adding a text model to the deep learning model improves performance, increasing accuracy and F1-score by 2% and 6%, respectively. This indicates that textual information contributes greatly to hospital readmission predictions.

Abstract:Hospital readmission, defined as patients being re-hospitalized shortly after discharge, is a critical concern as it impacts patient outcomes and healthcare costs. Identifying patients at risk of readmission allows for timely interventions, reducing re-hospitalization rates and overall treatment costs. This study focuses on predicting patient readmission within less than 30 days using text mining techniques applied to discharge report texts from electronic health records (EHR). Various machine learning and deep learning methods were employed to develop a classification model for this purpose. A novel aspect of this research involves leveraging the Bio-Discharge Summary Bert (BDSS) model along with principal component analysis (PCA) feature extraction to preprocess data for deep learning model input. Our analysis of the MIMIC-III dataset indicates that our approach, which combines the BDSS model with a multilayer perceptron (MLP), outperforms state-of-the-art methods. This model achieved a recall of 94% and an area under the curve (AUC) of 75%, showcasing its effectiveness in predicting patient readmissions. This study contributes to the advancement of predictive modeling in healthcare by integrating text mining techniques with deep learning algorithms to improve patient outcomes and optimize resource allocation.

Abstract:Background/Objectives: Predicting patient readmission is an important task for healthcare risk management, as it can help prevent adverse events, reduce costs, and improve patient outcomes. In this paper, we compare various conventional machine learning models and deep learning models on a multimodal dataset of electronic discharge records from an Irish acute hospital. Methods: We evaluate the effectiveness of several widely used machine learning models that leverage patient demographics, historical hospitalization records, and clinical diagnosis codes to forecast future clinical risks. Our work focuses on addressing two key challenges in the medical fields, data imbalance and the variety of data types, in order to boost the performance of machine learning algorithms. Furthermore, we also employ SHapley Additive Explanations (SHAP) value visualization to interpret the model predictions and identify both the key data features and disease codes associated with readmission risks, identifying a specific set of diagnosis codes that are significant predictors of readmission within 30 days. Results: Through extensive benchmarking and the application of a variety of feature engineering techniques, we successfully improved the area under the curve (AUROC) score from 0.628 to 0.7 across our models on the test dataset. We also revealed that specific diagnoses, including cancer, COPD, and certain social factors, are significant predictors of 30-day readmission risk. Conversely, bacterial carrier status appeared to have minimal impact due to lower case frequencies. Conclusions: Our study demonstrates how we effectively utilize routinely collected hospital data to forecast patient readmission through the use of conventional machine learning while applying explainable AI techniques to explore the correlation between data features and patient readmission rate.

Abstract:Pediatric asthma is the most prevalent chronic childhood illness, afflicting about 6.2 million children in the United States. However, asthma could be better managed by identifying and avoiding triggers, educating about medications and proper disease management strategies. This research utilizes deep learning methodologies to predict asthma-related emergency department (ED) visit within 3 months using Medicaid claims data. We compare prediction results against traditional statistical classification model - penalized Lasso logistic regression, which we trained and have deployed since 2015. The results have indicated that deep learning model Artificial Neural Networks (ANN) slightly outperforms (with AUC = 0.845) the Lasso logistic regression (with AUC = 0.842). The reason may come from the nonlinear nature of ANN.

Abstract:BACKGROUND: The goals of this study were to develop a model that predicts the risk of 30-d all-cause readmission in hospitalized Medicaid patients diagnosed with COPD and to create a predictive model in a retrospective study of a population cohort.METHODS: We analyzed 2016-2019 Medicaid claims data from 7 United States states. A COPD admission was one in which either the admission diagnosis or the first or second clinical (discharge) diagnosis bore an International Classification of Diseases, Tenth Revision code for COPD. A readmission was an admission for any condition (not necessarily COPD) that occurred within 30 d of a COPD discharge. We estimated a mixed-effects logistic model to predict 30-d readmission from patient demographic data, comorbidities, past health care utilization, and features of the index hospitalization. We evaluated model fit graphically and measured predictive accuracy by the area under the receiver operating characteristic (ROC) curve.RESULTS: Among 12,283 COPD hospitalizations contributed by 9,437 subjects, 2,534 (20.6%) were 30-d readmissions. The final model included demographics, comorbidities, claims history, admission and discharge variables, length of stay, and seasons of admission and discharge. The observed versus predicted plot showed reasonable fit, and the estimated area under the ROC curve of 0.702 was robust in sensitivity analyses.CONCLUSIONS: Our model identified with acceptable accuracy hospitalized Medicaid patients with a diagnosis of COPD who are at high risk of readmission. One can use the model to develop post-discharge management interventions for reducing readmissions, for adjusting comparisons of readmission rates between sites/providers or over time, and to guide a patient-centered approach to patient care.

Abstract:Background: Heart failure (HF) is highly prevalent in the United States. Approximately one-third to one-half of HF cases are categorized as HF with reduced ejection fraction (HFrEF). Patients with HFrEF are at risk of worsening HF, have a high risk of adverse outcomes, and experience higher health care use and costs. Therefore, it is crucial to identify patients with HFrEF who are at high risk of subsequent events after HF hospitalization. Objective: Machine learning (ML) has been used to predict HF-related outcomes. The objective of this study was to compare different ML prediction models and feature construction methods to predict 30-, 90-, and 365-day hospital readmissions and worsening HF events (WHFEs). Methods: We used the Veradigm PINNACLE outpatient registry linked to Symphony Health's Integrated Dataverse data from July 1, 2013, to September 30, 2017. Adults with a confirmed diagnosis of HFrEF and HF-related hospitalization were included. WHFEs were defined as HF-related hospitalizations or outpatient intravenous diuretic use within 1 year of the first HF hospitalization. We used different approaches to construct ML features from clinical codes, including frequencies of clinical classification software (CCS) categories, Bidirectional Encoder Representations From Transformers (BERT) trained with CCS sequences (BERT + CCS), BERT trained on raw clinical codes (BERT + raw), and prespecified features based on clinical knowledge. A multilayer perceptron neural network, extreme gradient boosting (XGBoost), random forest, and logistic regression prediction models were applied and compared. Results: A total of 30,687 adult patients with HFrEF were included in the analysis; 11.41% (3184/27,917) of adults experienced a hospital readmission within 30 days of their first HF hospitalization, and nearly half (9231/21,562, 42.81%) of the patients experienced at least 1 WHFE within 1 year after HF hospitalization. The prediction models and feature combinations with the best area under the receiver operating characteristic curve (AUC) for each outcome were XGBoost with CCS frequency (AUC=0.595) for 30-day readmission, random forest with CCS frequency (AUC=0.630) for 90-day readmission, XGBoost with CCS frequency (AUC=0.649) for 365-day readmission, and XGBoost with CCS frequency (AUC=0.640) for WHFEs. Our ML models could discriminate between readmission and WHFE among patients with HFrEF. Our model performance was mediocre, especially for the 30-day readmission events, most likely owing to limitations of the data, including an imbalance between positive and negative cases and high missing rates of many clinical variables and outcome definitions. Conclusions: We predicted readmissions and WHFEs after HF hospitalizations in patients with HFrEF. Features identified by data-driven approaches may be comparable with those identified by clinical domain knowledge. Future work may be warranted to validate and improve the models using more longitudinal electronic health records that are complete, are comprehensive, and have a longer follow-up time.
Predicting 30-Day Hospital Readmission in Medicare Patients: Insights from an LSTM Deep Learning Model

Abstract:Readmissions among Medicare beneficiaries are a major problem for the US healthcare system from a perspective of both healthcare operations and patient caregiving outcomes. Our study analyzes Medicare hospital readmissions using LSTM networks with feature engineering to assess feature contributions. We selected variables from admission-level data, inpatient medical history and patient demography. The LSTM model is designed to capture temporal dynamics from admission-level and patient-level data. On a case study on the MIMIC dataset, the LSTM model outperformed the logistic regression baseline, accurately leveraging temporal features to predict readmission. The major features were the Charlson Comorbidity Index, hospital length of stay, the hospital admissions over the past 6 months, while demographic variables were less impactful. This work suggests that LSTM networks offers a more promising approach to improve Medicare patient readmission prediction. It captures temporal interactions in patient databases, enhancing current prediction models for healthcare providers. Adoption of predictive models into clinical practice may be more effective in identifying Medicare patients to provide early and targeted interventions to improve patient outcomes.

Abstract:30-day hospital readmission is a long standing medical problem that affects patients' morbidity and mortality and costs billions of dollars annually. Recently, machine learning models have been created to predict risk of inpatient readmission for patients with specific diseases, however no model exists to predict this risk across all patients. We developed a bi-directional Long Short Term Memory (LSTM) Network that is able to use readily available insurance data (inpatient visits, outpatient visits, and drug prescriptions) to predict 30 day re-admission for any admitted patient, regardless of reason. The top-performing model achieved an ROC AUC of 0.763 (0.011) when using historical, inpatient, and post-discharge data. The LSTM model significantly outperformed a baseline random forest classifier, indicating that understanding the sequence of events is important for model prediction. Incorporation of 30-days of historical data also significantly improved model performance compared to inpatient data alone, indicating that a patients clinical history prior to admission, including outpatient visits and pharmacy data is a strong contributor to readmission. Our results demonstrate that a machine learning model is able to predict risk of inpatient readmission with reasonable accuracy for all patients using structured insurance billing data. Because billing data or equivalent surrogates can be extracted from sites, such a model could be deployed to identify patients at risk for readmission before they are discharged, or to assign more robust follow up (closer follow up, home health, mailed medications) to at-risk patients after discharge.
Prediction of COPD Using Machine Learning, Clinical Summary Notes, and Vital Signs

Abstract:Chronic obstructive pulmonary disease (COPD) is a chronic inflammatory lung disease that causes obstructed airflow from the lungs. In the United States, more than 15.7 million Americans have been diagnosed with COPD, with 96% of individuals living with at least one other chronic health condition. It is the 4th leading cause of death in the country. Over 2.2 million patients are admitted to hospitals annually due to COPD exacerbations. Monitoring and predicting patient exacerbations on-time could save their life. This paper presents two different predictive models to predict COPD exacerbation using AI and natural language processing (NLP) approaches. These models use respiration summary notes, symptoms, and vital signs. To train and test these models, data records containing physiologic signals and vital signs time series were used. These records were captured from patient monitors and comprehensive clinical data obtained from hospital medical information systems for tens of thousands of Intensive Care Unit (ICU) patients. We achieved an area under the Receiver operating characteristic (ROC) curve of 0.82 in detection and prediction of COPD exacerbation.

Abstract:Continuity of care (COC) has been shown to possess numerous health benefits for chronic diseases. Specifically, the establishment of its level can facilitate clinical decision-making and enhanced allocation of healthcare resources. However, the use of a generalizable predictive methodology to determine the COC in patients has been underinvestigated. To fill this research gap, this study aimed to develop a machine learning model to predict the future COC of asthma patients and explore the associated factors. We included 31,724 adult outpatients with asthma who received care from the University of Washington Medicine between 2011 and 2018, and examined 138 features to build the machine learning model. Following the 10-fold cross-validations, the proposed model yielded an accuracy of 88.20%, an average area under the receiver operating characteristic curve of 0.96, and an average F1 score of 0.86. Further analysis revealed that the severity of asthma, comorbidities, insurance, and age were highly correlated with the COC of patients with asthma. This study used predictive methods to obtain the COC of patients, and our excellent modeling strategy achieved high performance. After further optimization, the model could facilitate future clinical decisions, hospital management, and improve outcomes.
