Abstract:Background Sleep behavior (eg, sleep duration, sleep quality, and nap) is closely related to many chronic conditions. However, less is known about its association with multiple chronic conditions (multimorbidity), particularly evidence from cohort studies. Methods Data were collected from a cohort of 8 937 individuals aged 45 and older from the China Health and Retirement Longitudinal Study (2011-2018). Sleep duration, sleep quality, and nap duration were collected in 2011 and 2013. Progression of multimorbidity was defined as the first report of 2 or more chronic conditions for participants without multimorbidity or the new report of 1 or more conditions for those with multimorbidity. Cox regression models were performed to calculate the hazard ratios and 95% confidence intervals (CIs) of the associations between sleep behaviors and the progression of multimorbidity. Results Short sleep duration and poor sleep quality were associated with the progression of multimorbidity independently and jointly, especially in those less than 65 years and females. The U-shaped dose-response relationships were observed between nighttime and total sleep duration and the progression of multimorbidity. Persistent short and unsteadily changed sleep behaviors increased the risk of multimorbidity progression. Individuals sleeping <= 5 h/night with 5-7 restless days/week had 1.53 times higher risk of multimorbidity progression (95% CI: 1.37-1.71), compared to those sleeping 7-8 h/night with Conclusions Short sleep duration and poor sleep quality were independently and jointly associated with a higher risk of multimorbidity progression in a mid-to-older population. Optimal sleep duration and sleep quality should be emphasized in multimorbidity prevention and control.

Abstract:BACKGROUND:This study investigated multimorbidity patterns among middle-aged and older Chinese people and whether healthcare costs varied among different multimorbidity patterns.METHODS:Data were from the 2011-2018 waves of the China Health and Retirement Longitudinal Study (CHARLS). We included 20,855 unique observations with information coming from their last wave of interviews and aged at least 45 years or older. Latent class analysis (LCA) was performed to classify individuals with common multimorbidity clusters based on 14 self-reported chronic diseases. Healthcare costs were from participants' self-reports and categorized into outpatient, inpatient, and self-treatment. Two-part regression was performed to analyze the association of multimorbidity patterns with healthcare costs.RESULTS:Five multimorbidity clusters were identified: minimal disease, arthritis, cardiovascular disease (CVD), lung/asthma, and multisystem morbidity. The multisystem morbidity group had the highest use in all three types of healthcare and the highest self-treatment cost. Compared with the minimal disease group, the other four groups did not show significant differences in outpatient costs. Relative to the minimal disease group, the lung/asthma group reported lower inpatient costs.CONCLUSION:Healthcare use and costs varied across multimorbidity patterns among middle-aged and older Chinese people. Implementing an integrated care plan for multimorbidity is suggested to improve the cost-effectiveness of healthcare provision and reduce the financial burden of the healthcare system. Reimbursement policy design should also take multimorbidity patterns into account.

Abstract:An accurate experimental method is proposed for on-spot calibration of the measurement sensitivities of multiple micro-cantilever dynamic modes in atomic force microscopy. One of the key techniques devised for this method is a reliable contact detection mechanism that detects the tip-surface contact instantly. At the contact instant, the oscillation amplitude of the tip deflection, converted to that of the deflection signal in laser reading through the measurement sensitivity, exactly equals to the distance between the sample surface and the cantilever base position. Therefore, the proposed method utilizes the recorded oscillation amplitude of the deflection signal and the base position of the cantilever at the contact instant for the measurement sensitivity calibration. Experimental apparatus along with various signal processing and control modules was realized to enable automatic and rapid acquisition of multiple sets of data, with which the calibration of a single dynamic mode could be completed in less than 1 s to suppress the effect of thermal drift and measurement noise. Calibration of the measurement sensitivities of the first and second dynamic modes of three micro-cantilevers having distinct geometries was successfully demonstrated. The dependence of the measurement sensitivity on laser spot location was also experimentally investigated. Finally, an experiment was performed to validate the calibrated measurement sensitivity of the second dynamic mode of a micro-cantilever.
Multimorbidity patterns in old adults and their associated multi-layered factors: a cross-sectional study

Abstract:Abstract Background Influenced by various factors such as socio-demographic characteristics, behavioral lifestyles and socio-cultural environment, the multimorbidity patterns in old adults remain complex. This study aims to identify their characteristics and associated multi-layered factors based on health ecological model. Methods In 2019, we surveyed a total of 7480 participants aged 60+ by using a multi-stage random cluster sampling method in Shanxi province, China. Latent class analysis was used to discriminate the multimorbidity patterns in old adults, and hierarchical regression was performed to determine the multi-layered factors associated with their various multimorbidity patterns. Results The prevalence of multimorbidity was 34.70% among the old patients with chronic disease. Over half (60.59%) of the patients with multimorbidity had two co-existing chronic diseases. “Degenerative/digestive diseases”, “metabolic diseases” and “cardiovascular diseases” were three specific multimorbidity patterns. Behavioral lifestyles-layered factors had the most explanatory power for the three patterns, whose proportions of explanatory power were 54.00, 43.90 and 48.15% individually. But the contributions of other multi-layered factors were different in different patterns; balanced diet, medication adherence, the size of family and friendship network, and different types of basic medical insurance might have the opposite effect on the three multimorbidity patterns ( p < 0.05). Conclusions In management of old patients with multimorbidity, we should prioritize both the “lifestyle change”-centered systematic management strategy and group-customized intervention programs.

Multimorbidity patterns and associated factors in older Chinese: results from the China health and retirement longitudinal study

Abstract:Background: This study aimed to investigate multimorbidity patterns and their associated factors among elderly population in China. Methods: A total of 10,479 participants aged at least 60 years were drawn from the 2018 wave of the China Health and Retirement Longitudinal Study (CHARLS). Latent class analysis (LCA) was performed to identify distinct multimorbidity classes based on 14 self-reported chronic conditions. The multinomial logit model was used to analyze the associated factors of multimorbidity patterns, focusing on individuals' demographic characteristics, socioeconomic status (SES), and health behaviors. Results: Among the 10,479 participants (mean age [SD]: 69.1 [7.1]), 65.6% were identified with multimorbidity. Five multimorbidity clusters were identified by LCA: relatively healthy class (49.8%), vascular class (24.7%), respiratory class (5.6%), stomach-arthritis class (14.5%), and multisystem morbidity class (5.4%). Multinomial logit analysis with the relatively healthy class as the reference showed that participants of older age and female sex were more likely to be in the vascular class and multisystem morbidity class. The probability of being in the vascular class was significantly higher for those with high SES. Ever smoking was associated with a higher probability of being in the respiratory class and multisystem morbidity class. Physical activity was associated with lower odds of being assigned to the vascular class, respiratory class, and multisystem class. Conclusion: The distinct multimorbidity patterns imply that the prevention and care strategy should target a group of diseases instead of a single condition. Prevention interventions should be paid attention to for individuals with risk factors.

Abstract:Background Multimorbidity presents an enormous problem to societal and healthcare utilization under the context of aging population in low- and middle-income countries (LMICs). Currently, systematic studies on the profile of multimorbidity and its characteristics among Chinese elderly are lacking. We described the temporal and spatial trends in the prevalence of multimorbidity and explored chronological changes of comorbidity patterns in a large elderly population survey. Methods Data were extracted from the Chinese Longitudinal Healthy Longevity Study (CLHLS) conducted between 1998 and 2018 in a random selection of half of the counties and city districts. All the elderly aged 65 and older were included in the survey of eight waves. We used 13 investigated chronic diseases to measure the prevalence of multimorbidity by means of geography, subpopulation, and chronological changes. The patterns of multimorbidity were assessed by computing the value of relative risk (RR indicates the likelihood of certain diseases to be associated with multimorbidity) and the observed-to-expected ratio (O/E indicates the likelihood of the coexistence of a multimorbidity combination). Results From 1998 to 2018, the prevalence of multimorbidity went from 15.60 to 30.76%, increasing in the fluctuation across the survey of eight waves ( p for trend = 0.020). Increasing trends were observed similarly in a different gender group ( p male = 0.009; p female = 0.004) and age groups among female participants ( p ~80 = 0.009; p 81−90 = 0.004; p 91−100 = 0.035; p 101~ = 0.018). The gap in the prevalence of multimorbidity between the north and the south was getting narrow across the survey of eight waves. Hypertension was the highest prevalent chronic condition while diabetes was most likely to coexist with other chronic conditions in the CLHLS survey. The most frequently occurring clusters were hypertension and heart disease, hypertension and cataract, and hypertension and chronic lung disease. And, the cancer, TB, and Parkinson's disease cluster took the domination of O/E rankings over time, which had a higher probability of coexistence in all the multimorbidity combinations. Conclusions The prevalence of multimorbidity has been increasing nationwide, and more attention should be paid to a rapid growth in the southern part of China. It demands the effective diagnosis and treatment adopted to the highly prevalent comorbidities, and strategies and measures were adjusted to strongly relevant clusters.

Trends of Multimorbidity Status among the Older Chinese Population: Findings from the Chinese Longitudinal Healthy Longevity Survey 2000 to 2018

Abstract:Abstract Objectives: This paper aimed to assess temporal trends and risk factors of multimorbidity among older Chinese adults from 2000 to 2018. Methods: A total of 51,326 participants aged 65–105 from the Chinese Longitudinal Healthy Longevity Survey (CLHLS) 2000–2018 were included. Multimorbidity was defined as the co-occurrence of 2 or more (up to 14) chronic conditions in a single person. The adjusted proportion of the number of chronic conditions by follow-up year was examined and described. The generalized estimating equation model including the natural cubic spline was used to predict the prevalence of multimorbidity. Associated factors were identified using generalized estimating equation models from 2000 to 2018. All analyses were conducted separately in 2 age groups: the young-old group (aged 65–79) and the older-old group (aged 80–105). Results: The adjusted multimorbidity prevalence for the young-old group increased from 23.9% in 2002 to 72.5% in 2014 and remained high at 46.3% in 2018; the adjusted prevalence for the older-old group increased from 63.4% in 2000 to 87.2% in 2011 and remained at 73.0% in 2018. Being female, having fewer educational years, a professional or technical occupation, and a history of smoking or drinking were associated with an increased risk for multimorbidity in young-old group; while in older-old group, being female, current smoker or drinker, and less engagement in social and leisure activity were associated with an increased risk for multimorbidity. Conclusion: The prevalence of multimorbidity first increases and then decreases for both age groups. Management of multimorbidity requires more attention from health policymakers, service providers, and educators of health professionals in China. Health systems should prioritize improving the management of older patients, especially women who have lower education levels.

Abstract:(1) Background: Multimorbidity has become one of the key issues in the public health sector. This study aims to explore the patterns and health-ecological factors of multimorbidity in China to propose policy recommendations for the management of chronic diseases in the elderly. (2) Methods: A multi-stage random sampling method was used to conduct a questionnaire survey on 3637 older adults aged 60 and older in Shanxi, China. Association rule mining analysis (ARM) and network analysis were applied to analyze the patterns of multimorbidity. The health-ecological model was adopted to explore the potential associated factors of multimorbidity in a multidimensional perspective. A hierarchical multiple logistic model was employed to investigate the association strengths reflected by adjusted odds ratios and 95% confidence. (3) Results: Multimorbidity occurred in 20.95% of the respondents. The graph of network analysis showed that there were 6 combinations of chronic diseases with strong association strengths and 14 with moderate association strengths. The results of the ARM were similar to the network analysis; six dyadic chronic disease combinations and six triadic ones were obtained. Hierarchical multiple logistic regression indicated that innate personal traits (age, history of genetics, and body mass index), behavioral lifestyle (physical activity levels and medication adherence), interpersonal network (marital status), and socioeconomic status (educational level) were the common predictors of multimorbidity for older adults, among which, having no family history was found to be a relative determinant as a protective factor for multimorbidity after controlling the other covariates. (4) Conclusions: multimorbidity was prevalent in older adults and most disease combinations are associated with hypertension, followed by diabetes. This shows that diabetes and hypertension have a high prevalence among older adults and have a wide range of associations with other chronic diseases. Exploring the patterns and associated factors of multimorbidity will help the country prevent complications and avoid the unnecessary use of the health service, adopting an integrated approach to managing multimorbidity rather than an individual disease-specific approach and implementing different strategies according to the location of residence.

Abstract:Purpose: This study aimed to investigate the prevalence of depressive symptoms and the association between multimorbidity and depressive symptoms among the elderly in eastern China. Patients and methods: A multicenter cross-sectional study was conducted in four cities (Jianggan, Yiwu and Anji in Zhejiang Province and Taixing in Jiangsu Province) in eastern China. We collected the information on 27 chronic conditions through the self-reported medical history and used the Geriatric Depression Scale (GDS) short form to evaluate depressive symptoms. Multivariate Logistic regression was used to evaluate the relationship between multimorbidity and depressive symptoms. Results: Five thousand two hundred and ninety-six participants were included into the current study, among which 2687 (50.74%) were female, with the mean +/- SD age 72.0 +/- 8.1 years old. The overall prevalence of depressive symptoms in eastern China was 23.5%. And the percentage of multimorbidity in depressed participants was higher compared with non-depressed participants (50.8% vs 38.8%, P<0.001). The univariate model and adjusted model suggested that participants with multiple chronic diseases were more likely to have a depressive symptom (adjusted OR=1.42; 95% CIs 1.19-1.70). Conclusion: Depressive symptom was significantly associated with multimorbidity among the community-dwelling elderly population in eastern China.
Association between multimorbidity patterns and disability among older people covered by long-term care insurance in Shanghai, China

Abstract:Abstract Background Functional disability and multimorbidity are common among older people. However, little is known about the relationship between functional disability and different multimorbidity combinations. We aimed to identify multimorbidity patterns and explore the associations between these patterns and functional disability. Methods We investigated a multi-stage random sample of 1871 participants aged ≥60 years and covered by long-term care insurance in Shanghai, China. Multimorbidity was defined as the simultaneous presence of two or more chronic diseases in an individual. Participants completed scales to assess basic and instrumental activities of daily living (BADL and IADL, respectively). Multimorbidity patterns were identified via exploratory factor analysis. Binary logistic regression models were used to determine adjusted associations between functional disability and number and patterns of multimorbidity. Results Multimorbidity was present in 74.3% of participants. The prevalence of BADL disability was 50.7% and that of IADL disability was 90.7%. There was a strong association between multimorbidity and disability. We identified three multimorbidity patterns: musculoskeletal, cardio-metabolic, and mental-degenerative diseases. The cardio-metabolic disease pattern was associated with both BADL (OR 1.28, 95%CI 1.16–1.41) and IADL (OR 1.41, 95%CI 1.19–1.68) disability. The mental-degenerative disease pattern was associated with BADL disability (OR 1.55, 95%CI 1.40–1.72). Conclusions Multimorbidity and functional disability are highly prevalent among older people covered by long-term care insurance in Shanghai, and distinct multimorbidity patterns are differentially associated with functional disability. Appropriate long-term healthcare and prevention strategies for older people may help reduce multimorbidity, maintain functional ability, and improve health-related quality of life.

Multimorbidity Measures Associated with Cognitive Function among Community-Dwelling Older Chinese Adults

Abstract:INTRODUCTIONOlder adults with multimorbidity are at high risk of cognitive impairment development. There is a lack of research on the associations between different multimorbidity measures and cognitive function among older Chinese adults living in the community.METHODSWe used the Chinese Longitudinal Healthy Longevity Survey from 2002 to 2018 and included data on dementia-free participants aged >= 65 years. Multimorbidity measures included condition counts, multimorbidity patterns, and trajectories. The association of multimorbidity measures with cognitive function was examined by generalized estimating equation and linear and logistic regression models.RESULTSAmong 14,093 participants at baseline, 43.2% had multimorbidity. Multimorbidity patterns were grouped into cancer-inflammatory, cardiometabolic, and sensory patterns. Multimorbidity trajectories were classified as "onset-condition," "newly developing," and "severe condition." The Mini-Mental State Examination scores were significantly lower for participants with more chronic conditions, with cancer-inflammatory/cardiometabolic/sensory patterns, and with developing multimorbidity trajectories.DISCUSSIONCondition counts, sensory pattern, cardiometabolic pattern, cancer-inflammatory pattern, and multimorbidity developmental trajectories were prospectively associated with cognitive function.Highlights Elderly individuals with a higher number of chronic conditions were associated with lower MMSE scores in the Chinese Longitudinal Healthy Longevity Survey data. MMSE scores were significantly lower for participants with specific multimorbidity patterns. Individuals with developing trajectories of multimorbidity were associated with lower MMSE scores and a higher risk of mild cognitive impairment.
Prevalence and pattern of multimorbidity in China: a cross-sectional study of 224,142 adults over 60 years old

Abstract:Aim: To examine the prevalence and potential risk factors of multimorbidity among older adult in China. In addition, we investigated the pattern of multimorbidity. Methods: This study is based on data from the fourth Sample Survey of the Aged Population in Urban and Rural China (SSAPUR) in 2015, a comprehensive survey of individuals aged 60 years or older in China. We calculated baseline data and prevalence rates for comorbidities, stratified by household registration, age, sex, education, exercise, and health insurance. Univariate and multivariate logistic regression analyses were conducted to identify potential risk factors for comorbidities. Furthermore, we determined the prevalence rates for the three most frequent disease combinations. Results: A total of 215,040 participants were included in our analysis. The prevalence of multimorbidity was 50.5% among the older adult in China. The prevalence rate was slightly higher in rural areas than in urban areas, with rates of 51.5 and 49.6%, respectively (p < 0.001). Moreover, the prevalence rate was higher in females than in males, with rates of 55.2 and 45.3%, respectively (p < 0.001). Multivariate logistic regression analysis revealed that individuals aged 70-79 years (OR:1.40, 95% CI: 1.38-1.43, p < 0.001) and over 80 years (OR:1.41, 95% CI: 1.38-1.45, p < 0.001) had a higher prevalence of multimorbidity than those aged 60-69 years. The most prevalent pair of comorbidities was hypertension and osteoarthropathy, with 19.6% of the participants having these two conditions, accounting for 5.4% of the total participants. Conclusion: Our findings indicate a high prevalence of multimorbidity among the older adult in China. Increased expenditure on preventive health care, popularization of general medicine and popular medical education may be adopted by the Government to cope with the high prevalence of multimorbidity.
Survival, Disabilities in Activities of Daily Living, and Physical and Cognitive Functioning among the Oldest-Old in China: a Cohort Study.

Abstract:BACKGROUND:The oldest-old (those aged ≥80 years) are the most rapidly growing age group globally, and are most in need of health care and assistance. We aimed to assess changes in mortality, disability in activities of daily living, and physical and cognitive functioning among oldest-old individuals between 1998 and 2008.METHODS:We used data from the Chinese Longitudinal Healthy Longevity Study. Three pairs of cohorts aged 80-89 years, 90-99 years, and 100-105 years (in total, 19 528 oldest-old participants) were examined; the two cohorts in each pair were born 10 years apart, with the same age at the time of the assessment in the 1998 and 2008 surveys. Four health outcomes were investigated: annual death rate, Activities of Daily Living (ADL), physical performance in three tests and cognitive function measured by Mini-Mental State Examination (MMSE). We used different tests and multivariate regression analyses to examine the cohort differences.FINDINGS:Controlling for various confounding factors, we noted that annual mortality among oldest-old individuals was substantially reduced between 0·2% and 1·3% in 1998-2008 compared with individuals of the same age born 10 years previously, and that disability according to activities of daily living had significantly reduced annually between 0·8% and 2·8%. However, cognitive impairment in the later cohorts increased annually between 0·7% and 2·2% and objective physical performance capacity (standing up from a chair, picking up a book from the floor, and turning around 360°) decreased anually between 0·4% and 3·8%. Socioeconomic Inequality in the Multimorbidity Trajectories of Middle-Aged and Older Adults in China: A Prospective Cohort Study

Abstract:Objective The prevalence of multimorbidity is socially patterned, but little is known about how socioeconomic inequality might affect the long-term progression of multimorbidity. This study aimed to identify multimorbidity trajectories and to examine their association with socioeconomic status (SES) among middle-aged and older Chinese adults. Methods A total of 3837 middle-aged and older participants were included from the dynamic cohort of the China Health and Retirement Longitudinal Study, 2011–2018. Multimorbidity trajectories were assessed using the Chinese Multimorbidity-Weighted Index (CMWI), which covers 14 chronic conditions. Group-based trajectory modeling was used to identify multimorbidity developmental trajectories. Education, working status, and total household income were used to construct SES scores. The associations between SES and CMWI trajectories were estimated using multivariable multinomial logistic regression models adjusting for sociodemographic and lifestyle factors. Results Four distinct CMWI trajectories were identified: ‘no multimorbidity’ (16.8 %), ‘new-onset multimorbidity’ (24.3 %), ‘slowly increasing multimorbidity’ (48.7 %), and ‘rapidly increasing multimorbidity’ (10.2 %). Lower SES was associated with higher odds of experiencing the ‘rapidly increasing’ trajectory (P trend < 0.01); for example, compared with the ‘no multimorbidity’ group, participants with low SES had a 96 % (OR, 1.96; 95 % CI, 1.29 to 2.98) increased odds of belonging to the ‘rapidly increasing’ group. Conclusion Socioeconomic inequalities were observed in the CMWI trajectories of multimorbidity in middle-aged and older Chinese adults. The findings suggest effective strategies for preventing and controlling multimorbidity, especially for those of lower SES.

Abstract:Background This study aimed to investigate multimorbidity patterns and their associated factors among elderly population in China. Methods A total of 10,479 participants aged at least 60 years were drawn from the 2018 wave of the China Health and Retirement Longitudinal Study (CHARLS). Latent class analysis (LCA) was performed to identify distinct multimorbidity classes based on 14 self-reported chronic conditions. The multinomial logit model was used to analyze the associated factors of multimorbidity patterns, focusing on individuals' demographic characteristics, socioeconomic status (SES), and health behaviors. Results Among the 10,479 participants (mean age [SD]: 69.1 [7.1]), 65.6% were identified with multimorbidity. Five multimorbidity clusters were identified by LCA: relatively healthy class (49.8%), vascular class (24.7%), respiratory class (5.6%), stomach-arthritis class (14.5%), and multisystem morbidity class (5.4%). Multinomial logit analysis with the relatively healthy class as the reference showed that participants of older age and female sex were more likely to be in the vascular class and multisystem morbidity class. The probability of being in the vascular class was significantly higher for those with high SES. Ever smoking was associated with a higher probability of being in the respiratory class and multisystem morbidity class. Physical activity was associated with lower odds of being assigned to the vascular class, respiratory class, and multisystem class. Conclusion The distinct multimorbidity patterns imply that the prevention and care strategy should target a group of diseases instead of a single condition. Prevention interventions should be paid attention to for individuals with risk factors.
Associating Modifiable Lifestyle Factors with Multimorbidity in Community Dwelling Individuals from Mainland China

Abstract:Aims Lifestyle factors have been well-established as essential targets for fighting individual chronic diseases, but little research has concentrated on multimorbidity from the perspective of multiple lifestyle factors in the Chinese population. Thus, this study aimed to explore the associations of lifestyle factors with the occurrence of multimorbidity. Methods and results Cross-sectional data retrieved from the China Health and Retirement Longitudinal Study were used for analysis. Multimorbidity was calculated on a simple count of self-reported chronic conditions. Lifestyle factors included sleep duration, physical activity, alcohol intake, smoking status, and body mass index. Logistic regression analysis was used to examine the independent and accumulating effects of lifestyle factors on multimorbidity. Latent class analysis was performed to explore the lifestyle patterns. Six thousand, five hundred, and ninety-one valid subjects were included for analysis. Overall, the community dweller's median number of chronic conditions was 1 (range 1-11) and median number of high-risk lifestyle factors was 2 (range 0-5). All lifestyle factors were associated with the occurrence of multimorbidity but varied between genders. We also identified that participants who accumulated more unhealthy lifestyle factors having a higher likelihood of multimorbidity. 'Physical activity and weight', 'smoke and drink', and 'sleep and weight' dominated high-risk lifestyles were the most common lifestyle patterns. Conclusion This study revealed the associations of unhealthy lifestyle factors and their accumulating effect with multimorbidity in Chinese community dwellers. Three common lifestyle patterns indicated that a holistic approach focused on engaging and changing multiple modifiable lifestyle behaviours within an individual might be more effective in managing multimorbidity.

Abstract:Vaidyanathan et al. (this issue) offer provocative proposals for the striking lack of progress, after decades of basic and clinical research, in understanding the etiology of psychopathology. They engage a number of prominent issues. They do not (and need not) take a stand regarding the relative merits of DSM 5’s categorical premise and its almost exclusively psychological characterization of the manifestations of psychopathology vs. the merits of the Research Domain Criteria (RDoC) initiative of the National Institute of Mental Health, with its embrace of dimensional conceptualizations of contributors to psychopathology and of biological phenomena as playing a critical role in understanding psychopathology. Six years after its announcement in 2009, RDoC is gaining traction but is subject to widespread misunderstanding in several critical ways. First, although it champions dimensional conceptualizations of psychopathology, it does not preclude traditional categorical conceptualizations. Second, the widely distributed RDoC matrix does not represent the final set of concepts RDoC seeks. The current matrix is merely a “draft” (http://www.nimh.nih.gov/research-priorities/rdoc/nimh-research-domain-criteria-rdoc.shtml#toc_matrix), an illustrative example, and is designed to be fully extensible as the literature merits. Third, RDoC is not bad news for psychological concepts or for established methods of studying psychopathology. On the contrary, RDoC offers an explicit, systematic antidote to the naive reductionism fostered by the Decades of the Brain that began in 1990 (Miller, 1996, 2010; Miller & Rockstroh, 2013). This is clear from examination of the rows of the proposed matrix, nearly all of which are psychological phenomena. Such dominance of psychology in the RDoC matrix reflects the definitional fact that psychopathology is a set of psychological phenomena, not a set of “brain diseases” as became fashionable to claim in the first Decade of the Brain (e.g., Hyman, 1998; Leshner, 1997). Fourth, in the nomological network, among its columns RDoC positions psychological measurements at the same logical level as biological measurements. Notably, in RDoC, psychological events are not assumed to be reducible to biological events, nor do biological events “underlie” psychological events. Psychological and biological symptoms are not merely at different “levels of analysis”. They are qualitatively different phenomena. Psychological and biological events are coincident (reflecting “contingent identity”), the psychological phenomena are not reducible to or logically the same as the biological phenomena (“necessary identity”, Fodor, 1968; Kozak & Miller, 1982; Miller & Kozak, 1993). RDoC is often characterized as a radical departure from the longstanding dominance of categorical assumptions about psychopathology, and it is, though it merely makes space for dimensional models alongside categorical models. More fundamentally radical is its resuscitation of psychology as an equal partner in the clinical science of psychopathology.
Multimorbidity And Risk Of Mortality In China: Results From The Chinese Electronic Health Records Research In Yinzhou (Cherry) Study

Abstract:Introduction: The prevalence of multimorbidity is increasing in developed countries, while corresponding research in Chinese population is limited. Methods: A population-based cohort in Yinzhou located in an eastern coastal area of China (961,008 adults ≥18 years in 2009; latest mortality follow-up: Nov 2015; 22,637 deaths; 6.53 million person-years at risk) was assembled by linking epidemiological surveys, electronic records for chronic disease management, health administrative and medical records databases. Multimorbidity was defined as the presence of two or more following disorders: hypertension, diabetes, cardiovascular disease or cancer. History of selected diseases was extracted from the electronic records for chronic disease management. Follow-up on fatal events is achieved through records linkage to the regional system of death surveillance. Results: Overall age- and sex-standardized prevalence of one, two or ≥3 disorders were 16.16% (16.09% - 16.23%), 4.11% (4.07% - 4.15%) and0.36% (0.35% - 0.38%) respectively, whereas 41.73%, 14.41% and 1.67% were observed in people aged 60 years and older. The all-cause mortality rate adjusted to the age of 60 was 4.77 per 1000 person-years. Comparing to people without any selected diseases, the age- and sexadjusted HRs for all-cause mortality were 1.17 (1.14, 1.21) in those with one disorder, 1.78 (1.72, 1.86) in those with two morbidities, and 2.97 (2.76, 3.19) in those with ≥3 morbidities. The HRs in those with multimorbidites were higher in younger people and were heterogeneous among different combinations of multimorbidites. Population-attributable fractions (PAF) for all-cause mortality due to one, two or ≥3 disorders were 2.63%, 3.04% and 0.67% respectively (1.28%, 7.69% and 2.34% in people aged 60 years and older). Conclusions: The CHERRY study can serve as a valuable big data resource for scientific research in China. Multimorbidity is becoming acommon condition in Chinese population, especially in older population, and is associated with high mortality. A complementary strategy is required for population health interventions.

Abstract:Background: Both multimorbidity and activities of daily living (ADL) disability and instrument activities of daily living (IADL) disability are common among elderly individuals. ADL/IADL disability may reduce individuals' capacities for independent living and quality of life. This study aimed to examine the association between multimorbidity and ADL/IADL disability.Methods: A multi-stage cluster sample of 2058 residents aged 80 or older was investigated in Shanghai, China. Multimorbidity was defined as the simultaneous presence of two or more chronic diseases with ten common chronic conditions under consideration. Subjects who responded that they "need partial or full assistance" to any ADL/IADL items were defined as having ADL/IADL disability. We examined the association of multimorbidity with ADL/IADL disability, adjusted for socio-demographic characteristics by using logistic regression.Results: Of respondents, 23.23 % had ADL disability, 37.90 % had IADL disability, and 49.17 % had multimorbidity. After adjusted socio-demographic characteristics, a graded association was showed between ADL disability and the quantity of chronic conditions: odds ratio (OR) for 1 condition, 1.53(95 % confidence interval [CI], 1.04-2.24); OR for 2 conditions, 2.06(95 % CI, 1.43-2.96); OR for 3 conditions, 3.23(95 % CI, 2.14-4.86); OR for 4 or more conditions, 5.61(95 % CI, 3.26-9.66). Similar associations were also observed between the quantity of chronic conditions and IADL disability.Conclusions: The quantity of chronic conditions had relatively strong association with both ADL and IADL disability. Initiating prevention of additional chronic conditions and interventions on clusters of diseases may decrease the potential risk of ADL/IADL disability. Additionally, more attention should been given to the older low-income women living with relatives/non-relatives with multimorbidity.
Multimorbidity among Two Million Adults in China

Abstract:To explore the multimorbidity prevalence and patterns among middle-aged and older adults from China. Data on thirteen chronic diseases were collected from 2,097,150 participants aged over 45 years between January 1st 2011 and December 31st 2015 from Beijing Medical Claim Data for Employees. Association rule mining and hierarchical cluster analysis were applied to assess multimorbidity patterns. Multimorbidity prevalence was 51.6% and 81.3% in the middle-aged and older groups, respectively. The most prevalent disease pair was that of osteoarthritis and rheumatoid arthritis (OARA) with hypertension (HT) (middle-aged: 22.5%; older: 41.8%). Ischaemic heart disease (IHD), HT, and OARA constituted the most common triad combination (middle-aged: 11.0%; older: 31.2%). Among the middle-aged group, the strongest associations were found in a combination of cerebrovascular disease (CBD), OARA, and HT with IHD in males (lift = 3.49), and CBD, OARA, and COPD with IHD in females (lift = 3.24). Among older patients, glaucoma and cataracts in females (lift = 2.95), and IHD, OARA, and glaucoma combined with cataracts in males (lift = 2.45) were observed. Visual impairment clusters, a mixed cluster of OARA, IHD, COPD, and cardiometabolic clusters were detected. Multimorbidity is prevalent among middle-aged and older Chinese individuals. The observations of multimorbidity patterns have implications for improving preventive care and developing appropriate guidelines for morbidity treatment.
