Abstract:Background: In patients hospitalised for an exacerbation of COPD (severe AECOPD), elevated blood eosinophil count was associated with a shortened length of stay. Less is known about its association with survival. Aims and objectives: The aim of this study was to evaluate the association of elevated blood eosinophil count at admission for a severe AECOPD with all-cause mortality. Methods: 450 patients of the COMIC cohort study who were admitted for a severe AECOPD were divided in normal or elevated blood eosinophil count, defined as eosinophils ≥2% of the total leukocyte count on day of admission. Outcome parameter was all-cause mortality. Minimum follow-up was 3 years. Results: Elevated blood eosinophil count (N=97) was associated with a significantly lower risk for all-cause mortality in univariate analysis (Figure 1; logrank p=0.004) (HR 1.53; 95%CI 1.15-2.04). [figure1] Patients with elevated blood eosinophil count were younger than patients with normal blood eosinophil count (66.3 (SD 10.4) versus 70.6 (SD 9.6) years, respectively (p Conclusion: The strong association of elevated blood eosinophils on admission for a severe AECOPD with lower risk for mortality in univariate analysis disappeared after correction for age. The association of age with eosinophil count needs further investigation.

Abstract:Journal of International Medical Research, Volume 52, Issue 11, November 2024. ObjectiveIn this multicenter retrospective study, we evaluated the prognostic role of blood eosinophil count on clinical outcomes in hospitalized patients with exacerbations of chronic obstructive pulmonary disease (COPD).MethodsWe included patients aged 20 to 90 years with a COPD diagnosis. Patients were divided into groups with blood eosinophil count ≤300 or >300 cells/μL and then further classified into 1–99, 100–300, or >300 cells/μL. We compared sociodemographic features and clinical outcomes between groups and identified risk factors associated with mortality in hospitalized patients with COPD and blood eosinophil count ≤300 cells/μL.ResultsIn total, 217 patients were included (82% men, average age 64.3±10.3 years). Patients with eosinophil counts ≤300 cells/μL had significantly longer hospital stays, more admissions to the intensive care unit (22.2% vs. 4.3%), and more frequent mechanical ventilation (21.6% vs. 4.3%) than those with eosinophil counts >300 cells/μL. Mortality only occurred in the group with ≤300 cells/μL; patients with COPD who had eosinophil counts >300 cells/μL had significantly better survival rates (17.0% vs. 0%).ConclusionHigh blood eosinophil counts at admission were associated with improved short-term outcomes. Our findings reveal the importance of considering eosinophil counts in clinical decision-making to manage hospitalized patients with COPD.

Abstract:Introduction The acute exacerbation of chronic obstructive pulmonary disease (AECOPD) has a seriously negative impact on patients’ healths condition and disease progression. Bacterial infection is closely related to AECOPD, and antibiotics are frequently used in clinical practice. The lack of specific biomarkers for rational antibiotics use always leads to antibiotics abuse in chronic obstructive pulmonary disease (COPD) flare-ups. Eosinopenia has been considered to be related to increased bacterial load of potentially pathogenic organisms at the onset of COPD exacerbations. Therefore, this study aims to investigate whether eosinopenia could be used as a reference for the use of antibiotics in AECOPD. Methods and analysis In this study, a hospital-based retrospective cohort design will be adopted to analyse the clinical data of inpatients who are primarily diagnosed with AECOPD in West China Hospital of Sichuan University from 1 January 2010 to 31 December 2020. Relevant data will be extracted from the Clinical Big Data Platform for Scientific Research in West China Hospital, including demographic characteristics, blood eosinophil count, procalcitonin, C reactive protein, microbial cultivation, antibiotics use, length of hospital stay, non-invasive ventilation use, intensive care unit transfer and mortality, etc. The collected data will be described and inferred by corresponding statistical methods according to the data type and their distributions. Multiple binary logistic regression models will be used to analyse the relationship between blood eosinophil count and bacterial infection. The antibiotics use, and patient morbidity and mortality will be compared between patients with or without eosinopenia. Ethics and dissemination This study has been approved by the Biomedical Ethics Review Board of West China Hospital of Sichuan University (Approval No. 2020-1056). And the research results will be published in a peer-reviewed journal. Trial registration number ChiCTR2000039379.

Abstract:Background: Individualized management of acute exacerbation of COPD (AECOPD) could optimize patients’ benefit. Blood eosinophils may be associated with bacterial pathogen subtypes and clinical outcomes. Methods: We retrospectively recruited 596 patients hospitalized for AECOPD. The exacerbations were grouped according to blood eosinophil count as the eosinophilic (≥2%) and the eosinopenia (< 2%). Exacerbations were classified as being associated with bacterial infection if either CRP ≧2mg/dl or with at least two of the three symptoms: increase in dyspnea, sputum volume, and sputum purulence, among them, sputum purulence was required. Bacterial culture of sputum and drug sensitivity tests were performed. Bacterial infection, pathogen subtypes, hospital and ICU length of stay, need for mechanical ventilation (MV), mortality during hospitalization were compared. Results: Eosinopenia exacerbation was more strongly associated with bacterial infection (47.7% vs 35.6%, P < 0.01), higher CRP and procalcitonin levels. Sputum culture bacterial profiles were different between the two groups. G+ cocci load, especially Staphylococcus (P = 0.006) and Enterococcus (P = 0.013), was significant higher in the eosinopenia. Low eosinophils were also associated with poor clinical outcomes specifically in longer ICU stay (P = 0.047) and more need for MV (P = 0.033). Conclusions: Low eosinophils could hint more G+ cocci infection in exacerbations of COPD, especially Staphylococcus and Enterococcus. Eosinophils should be integrated into the clinical treatment strategy of AECOPD to achieve individualized and precise treatment.

Abstract:Abstract Background Sepsis is a life-threatening and time-critical medical emergency; therefore, the early diagnosis of sepsis is essential to timely treatment and favorable outcomes for patients susceptible to sepsis. Eosinopenia has been identified as a potential biomarker of sepsis in the past decade. However, its clinical application progress is slow and its recognition is low. Recent studies have again focused on the potential association between Eosinopenia and severe infections. This study analyzed the efficacy of Eosinopenia as a biomarker for diagnosis of sepsis and its correlation with pathophysiology of sepsis. Method The protocol for this meta-analysis is available in PROSPERO (CRD42020197664). We searched PubMed, EMBASE, Web of Science, and Cochrane Central Register of Controlled Trials CENTRAL databases to identify studies that met the inclusion criteria. Two authors performed data extraction independently. The pooled outcomes were calculated by TP (true positive), FP (false positive), FN (false negative), TN (true negative) by using bivariate meta-analysis model in STATA 14.0 software. Meanwhile, possible mechanisms of sepsis induced Eosinopenia was also analyzed. Results Seven studies were included in the present study with a total number of 3842 subjects. The incidence of Eosinopenia based on the enrolled studies varied from 23.2 to 92.7%. For diagnosis of sepsis, the pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and diagnostic odds ratio of Eosinopenia were 0.66 (95%CI [0.53–0.77]), 0.68 (95%CI [0.56–0.79]), 2.09 (95%CI [1.44–3.02]), 0.49 (95%CI [0.34–0.71]) and 4.23 (95%CI [2.15–8.31]), respectively. The area under the summary receiver operator characteristic curve (SROC) was 0.73 (95%CI [0.68–0.76]). Meta-regression analysis revealed that no single parameter accounted for the heterogeneity of pooled outcomes. For each subgroup of different eosinopenia cutoff values (50, 40, ≤25, 100), the sensitivity was 0.61, 0.79, 0.57, 0.54, and the specificity was 0.61, 0.75, 0.83, 0.51, respectively. Conclusions Our findings suggested that Eosinopenia has a high incidence in sepsis but has no superiority in comparison with conventional biomarkers for diagnosis of sepsis. However, eosinopenia can still be used in clinical diagnosis for sepsis as a simple, convenient, fast and inexpensive biomarker. Therefore, further large clinical trials are still needed to re-evaluate eosinopenia as a biomarker of sepsis.

Abstract:Aims and background: Sepsis is a dysregulated host response to an infection that causes organ failure that poses a serious risk to life. Although culture results are not always available right away and the majority of patients continue to test culture negative, microbial culture is still the gold standard for diagnosing sepsis. Therefore, the objective of the current study was to assess absolute eosinophil count as a new marker for diagnosing sepsis and also to assess the prognosis of the patient in relation to Sequential Organ Failure Assessment (SOFA)/quick Sequential Organ Failure Assessment (qSOFA) score. Resources and procedures: In this cross-sectional study, 100 patients with sepsis were enrolled. The other 100 patients without any evidence of sepsis were taken as controls. Absolute eosinophil count (AEC), SOFA/qSOFA scores of all the patients were measured on the 1st, 3rd, and 7th day and data was analyzed statistically. Results: The mean AEC on admission day in sepsis patients was 49.5. The mean AEC among survivors was >50 and nonsurvivors was <50. AEC and SOFA/qSOFA scores exhibit a statistically significant and inverse correlation on the 1st, 3rd, and 7th day of illness. Conclusion: Absolute eosinophil count (AEC) is a simple and cost-effective marker that may be helpful in diagnosis as well as in predicting the prognosis of sepsis as evidenced by its linear inverse correlation with SOFA/qSOFA score.
Normalization of eosinophil count is predictive of oxygen weaning over the course of COVID-19 infection among hospitalized adults during the first wave of 2020 pandemic

Abstract:Background: Understanding COVID-19 outcomes remains a challenge. While numerous biomarkers have been proposed for severity at admission, limited exploration exists for markers during the infection course, especially for the requirement of oxygen therapy. This study investigates the potential of eosinophil count normalization as a predictor for oxygen weaning during the initial wave of the pandemic. Methods: A retrospective study was conducted between March and April 2020 (first wave) among adults admitted directly to a medicine ward. Biological abnormalities, including lymphocyte count, eosinophil count, and C-reactive protein (CRP), were gathered daily during the first week of admission according to oxygen level. In case of worsening, oxygen level was censored at 15 L/min. The primary aim was to assess whether eosinophil count normalization predicts a subsequent decrease in oxygen requirements. Results: Overall, 132 patients were admitted, with a mean age of 59.0 ± 16.3 years. Of the patients, 72% required oxygen, and 20.5% were admitted to the intensive care unit after a median delay of 48 hours. The median CRP at admission was 79 (26–130) mg/L, whereas the eosinophil count was 10 (0–60)/mm 3 . Eosinophil count normalization (≥100/mm 3 ) by day 2 correlated significantly with decreased oxygen needs (<2 L) with hazard ratio (HR) = 3.7 [1.1–12.9] (p = 0.04). Likewise, CRP < 80 mg/L was associated with reduced oxygen requirements (p < 0.001). Predictors, including underlying chronic respiratory disease, exhibited a trend toward a negative association (p = 0.06). Conclusion: The study highlights the relationship between eosinophil count and CRP, with implications for predicting oxygen weaning during COVID-19. Further research is warranted to explore the relevance of these biomarkers in other respiratory infections.

Abstract:Appropriate management of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) is very important because such exacerbation negatively impacts patients' health status, disease progression, and mortality. Exacerbation episodes are mainly triggered by viral and bacterial infections, and one study showed that 55% of hospitalized patients with AECOPD had a bacterial infection.[1] Some analyses have suggested that antibiotic therapy can reduce the risk of short-term mortality and improve the prognosis. However, not all patients with AECOPD benefit from antibiotic therapy. The blood eosinophil count has been used as a biomarker for eosinophil-associated airway inflammation and sepsis. However, the association between the peripheral blood eosinophil count and bacterial pathogens has not been reported. Our aim was to analyze the associations among the eosinophil count, bacterial pathogens, clinical treatments, and prognosis of patients and provide useful and comprehensive references for AECOPD management.

Abstract:Background: Even after the development of vaccines, SARS-CoV-2 continues to cause severe pneumonia all over the world. Consequently, in order to improve the management of patients and optimize the use of resources, predictors of disease severity and lung complications after COVID-19 pneumonia are urgently needed. Blood cell count is an easily available and reproducible biomarker. With this study, we aimed to explore the role of eosinophils in predicting disease behavior and pulmonary sequelae at first follow-up with computed tomography (CT). Methods: we evaluated blood cell count and other inflammatory markers, both at baseline and during hospitalization, in a large population of hospitalized COVID-19 patients. Results: 327 patients were finally enrolled, 214 were classified as low-intensity medical care (LIMC) and 113 as high-intensity medical care. Eosinophils were higher at discharge in the HIMC group [0.1 (0–0.72) vs. 0.05 (0–0.34) × 109/L; p < 0.0001]. Moreover, in the multivariable analysis, age ≥ 62 years (OR 1.76 (1.05–2.8) p = 0.03) and Δ eosinophils ≥ 0.05 (OR 1.75 (1.05–2.9) p = 0.03) were two independent predictors of residual lung abnormalities in the whole patient population at first follow-up. Conclusions: an eosinophil increase during hospitalization could be a potential predictor of pulmonary sequelae in surviving patients after COVID-19 pneumonia.

Abstract:Background: Several studies suggest that, in patients with COPD, Eos is associated with (i) exacerbation frequency and (ii) the effect of inhaled corticosteroid for exacerbation prevention. Aim: To determine whether Eos defines patients9 groups exhibiting attributes of COPD clinical phenotypes, we compared the clinical features and mortality rate between COPD patients with ≥2% (Eos+) and Methods: The following data were collected at inclusion: medical and smoking history, occupational exposures, dyspnea, cough and sputum production, exacerbations in the previous year, history of allergy and asthma, nasal symptoms, BMI, SGRQ score, post-bronchodilator spirometry and plethysmography, comorbidities, CRP and medications. Three-year survival was available for all patients. BOD and ADO scores were calculated. Results: Eos was available in 458 patients (mean age: 62 years, 72% male, mean FEV1: 51%), including 235 Eos+ patients (49%). No difference between Eos+ and Eos- groups was observed in univariate analyses except for diabetes (17% in Eos-, 8% in Eos+, p=0.02) and SGRQ score (mean, 48 units in Eos -, 40 units in Eos+, p=0.007). In particular, there was no difference in term of exacerbations rate (mean, 1.6/patient/year) or history of asthma (15% of the population). Three-year mortality rate was 17% in Eos- vs 13% in Eos+ (NS). Conclusion: Apart from differences in health-related quality of life and frequency of diabetes (which warrant further investigation), Eos+ COPD patients exhibit no specific clinical characteristic, especially regarding symptoms, lung function, exacerbations and prognosis.

Abstract:Background Blood eosinophil counts have been studied in patients with stable chronic obstructive pulmonary disease (COPD) and are a useful biomarker to guide inhaled corticosteroid use. Less is known about eosinophil counts during severe exacerbation. Methods In this retrospective study, 2645 patients admitted consecutively with COPD exacerbation across six UK hospitals were included in the study, and the clinical diagnosis was confirmed by a respiratory specialist. The relationship between admission eosinophil count, inpatient death and 1-year death was assessed. In a backward elimination, Poisson regression analysis using the log-link function with robust estimates, patients’ markers of acute illness and stable-state characteristics were assessed in terms of their association with eosinopenia. Results 1369 of 2645 (52%) patients had eosinopenia at admission. Those with eosinopenia had a 2.5-fold increased risk of inpatient death compared with those without eosinopenia (12.1% vs 4.9%, RR=2.50, 95% CI 1.88 to 3.31, p<0.001). The same mortality risk with eosinopenia was seen among the subgroup with pneumonic exacerbation (n=788, 21.3% vs 8.5%, RR=2.5, 95% CI 1.67 to 2.24, p<0.001). In a regression analysis, eosinopenia was significantly associated with: older age and male sex; a higher pulse rate, temperature, neutrophil count, urea and C reactive protein level; a higher proportion of patients with chest X-ray consolidation and a reduced Glasgow Coma Score; and lower systolic and diastolic blood pressure measurements and lower oxygen saturation, albumin, platelet and previous admission counts. Discussion During severe COPD exacerbation, eosinopenia is common and associated with inpatient death and several markers of acute illness. Clinicians should be cautious about using eosinophil results obtained during severe exacerbation to guide treatment decisions regarding inhaled corticosteroid use.

Abstract:The eosinophil, whether in the blood or sputum, has been receiving increasing attention as a measure of treatment response or failure in patients with chronic obstructive pulmonary disease (COPD). A recent review by Bafadhel et al. [1] summarises the biology of the eosinophil, its role in airway disease, as well as the clinical data supporting the use of blood eosinophils as a biomarker in airway disease, and more specifically, COPD. To paraphrase their conclusions: while the role of eosinophils in COPD needs clarification, there is now the realisation that blood eosinophils may be a useful guide to therapy. There is, however, substantial within-person variation in blood eosinophil counts [2]. Despite this, there is a significant proportion of patients with COPD in whom blood eosinophil counts are consistently high or low: 65% consistently above or below 0.40×109 per L over a 1-year period in the Leicester cohort [1] and 51% at the 2% cut-off over a 3-year period in the ECLIPSE study [3]. Consistency also appears better when using absolute counts and at lower counts. For example, using a cut-off value of <0.15×109 per L, Southworth et al. [4] found 87% to be in the same category at 6 months and a similar proportion at >2 years of follow-up. It has further been suggested that higher blood eosinophil counts are predictive of future eosinophilic exacerbations [5] and this is supported by the better response to oral corticosteroids in patients with exacerbations characterised by higher blood eosinophils at presentation [1]. The broader importance of the eosinophil in the natural history of airway disease at an early stage is suggested by the predictive value of blood eosinophil counts on decline in forced expiratory volume in 1 s (FEV1) and FEV1/forced vital capacity in young adults from the New Zealand birth cohort, an effect that is independent of asthma and cigarette smoking [6]. Abstract:Sepsis remains a major global healthcare problem, indicate as most frequently cause of morbidity and mortality worldwide. The last consensus of sepsis in 2016 defined sepsis as life threatening organ dysfunction caused by a dysregulated host response to infection. Dysfunction of organs can be represented by Sequential [Sepsis-Related] Organ Failure Assessment (SOFA) score. Score2 points or more consequent to the infection. Nowadays, there is ideal biomarkers of sepsis such as procalcitonin (PCT). However, the use of that markers in developing countries are hardly accessible. Eosinopenia is an prepossess biomarker because eosinophil count is always measured in daily practice and considered as a forgotten marker. The study purpose is to determine the validity of absolute eosinopenia in bacterial sepsis patients. This study is a descriptive observational study, collecting 118 patient’s medical record data from the past, diagnosed as sepsis using consensus criteria of Sepsis-3 between January 1st 2018–December 31st 2019. Eosinopenia validity test in sepsis patients showed 92.7% specificity and 71.4% sensitivity. This study also showed significant differences of absolute eosinophil count between positive sepsis patients and negative group with p value <0.001. Eosinopenia had high specificity so it could be used as a marker of diagnostic in septic patients.

Abstract:Objectives: Studies have shown an association between eosinophilia and clinical outcomes in acute exacerbation of chronic obstructive pulmonary disease (AECOPD). However, contradictory findings exist. Our study aims to systematically evaluate the association between elevated peripheral blood eosinophils and clinical outcome of patients with AECOPD. Methods: An electronic search was conducted for relevant studies published from database inception to February 28, 2023, on PubMed, EMBASE, Cochrane Library, and Web of Science. The analysis covered studies on the correlation between EOS AECOPD and mortality, hospital stay duration, readmission and hospitalization rates, and invasive mechanical ventilation. Where applicable, relative risk (RR) and weighted mean difference (WMD) were extracted, pooled, and assessed using meta-analysis. Sensitivity analysis was performed to explore the source of heterogeneity. Results: Fifteen high-quality studies including 14 cohort studies and one case-control study were included in the meta-analysis. Compared with non-eosinophilic AECOPD patients, those with eosinophilic AECOPD had a lower risk of mortality (RR = 0.65, 95 % confidence interval [CI] 0.54, 0.77, P < 0.001), shorter length of hospital stay (WMD = -1.56, 95%CI -2.16, -0.96, P < 0.001), and higher readmission rate (RR = 1.07, 95%CI 1.01,1.13, P = 0.029). No difference was found concerning the rate of hospitalization and invasive mechanical ventilation between the two groups. Conclusion: Individuals diagnosed with eosinophilic AECOPD had a reduced mortality rate, a truncated period of hospitalization, and an insubstantial increase in the probability of readmission relative to their non-eosinophilic AECOPD counterparts. The level of eosinophils in blood has been shown to serve as a potential predictive biomarker for AECOPD patients.

Abstract:BACKGROUND: Blood eosinophil levels are a known marker for the effects of therapy in patients with chronic obstructive pulmonary disease (COPD). This study aimed to clarify the cutoff values for blood eosinophils (EOS) to predict exacerbation risk and prognosis of acute exacerbation COPD (AECOPD) and investigate their correlation using inflammatory indicators and clinical characteristics.METHODS: In this observational study of 174 patients with AECOPD, we assessed the relationship between EOS and COPD. According to the percentage of blood EOS, patients were grouped into two groups (Group 1: EOS <2%, n=98; Group 2: EOS ≥2%, n=76), and Group 2 was further divided into Group A (2%≤ EOS <4%) and Group B (EOS ≥4%) based on a cutoff value of 4%. Patients received standardized treatment after collection of peripheral blood specimen. Associations of EOS with laboratory indicators before any treatment in hospital and with clinical data were compared.RESULTS: Patients in Group 1 showed significantly severe inflammation, worse pulmonary function, longer length of stay (LOS) (P<0.001), higher mMRC score (P<0.05), higher CAT score (P<0.05), higher rates of mortality (P<0.05), and greater noninvasive mechanical ventilation usage (P<0.05) compared with Group 2. Intriguingly, the CRP, total mMRC and CAT scores of patients in Group A were significantly lower than those in Group B (P<0.001; P<0.01; P<0.05, respectively). Pearson correlation analysis showed that a low percentage blood eosinophil level was negatively associated with higher WBC count (r=-0.155, P<0.05), NLR (r=-0.227, P<0.01) and CRP (r=-0.308, P<0.01).CONCLUSIONS: Different cutoff values for percentage blood EOS might be useful biomarkers for predicting the severity of exacerbation and prognosis of inpatients with AECOPD.

Abstract:Background: In recent years, blood eosinophils have been evaluated as a surrogate biomarker for eosinophilic airway inflammation and as a prognostic indicator of the outcomes of hospitalized COPD subjects. During an exacerbation of COPD, eosinopenia has been proposed as a prognostic marker of adverse outcomes. Objectives: The aim of the present post hoc analysis was to elucidate the effectiveness of blood eosinophils for predicting the need of NIV in subjects with COPD exacerbation. Methods: Consecutive subjects admitted to a hospital for COPD exacerbation were included in the analysis. The eosinophil count from the first complete blood count was used to designate the eosinophil groups. The relationship between the clinical characteristics and blood eosinophil counts, as dichotomized using 150 cells/μL, was evaluated. Results Subjects with blood eosinophil number < 150 k/μL had a more severe disease on admission compared to subjects with ≥150 k/μL, regarding pH 7.400 (7.36, 7.44) vs. 7.42 (7.38, 7.45), p = 0.008, PO2/FiO2 levels 238.1 (189.8, 278.6) vs. 276.2 (238.2, 305.6), p < 0.001, CRP (mg/L) levels 7.3 (3.1, 19.9) vs. 3.5 (0.7, 7.8), p < 0.001 and required a longer hospital stay (days) 10.0 (8.0, 14.0) vs. 5.0 (3.0, 7.0) p < 0.001 respectively. The number of blood eosinophils correlated with the levels of CRP upon admission (p < 0.001, r = -0.334), with arterial pH upon admission (p < 0.030, r = 0.121), with PO2/FiO2 (p < 0.001, r = -0.248), and with duration of hospital stay (p < 0.001, r = -0.589). In the multinomial logistic regression analysis, blood eosinophil count < 150 k/μL was an independent predictor of the use of NIV during hospital stay. Conclusion: During COPD exacerbation, low blood eosinophil levels upon admission are related to more severe disease and can be used as a predictor of the need of NIV. Further prospective studies are needed to identify the use of blood eosinophil levels as a predictor of unfavorable outcomes.

Methods: Patient data were extracted from the MIMIC-III V1.4 database. Only the acute exacerbation of chronic obstructive pulmonary disease patients with the first measurement time of blood eosinophil concentrations (%) between 24 hours before admission and 24 hours after admission was included. The logistic regression model was used to analyze the association between eosinophil and outcomes. Results: 1019 patients were included in the study. Two multivariate regression models were built. The adjusted odds ratio of in-hospital mortality, in-ICU mortality, hospital length of stay and ICU length of stay for initial blood eosinophil concentrations in model 1 (adjusted for SAPS II, cardiac arrhythmias, solid tumor, metastatic cancer, liver disease, neutrophils) were 0.792 (95% CI: 0.643– 0.976, p=0.028), 0.812 (95% CI: 0.645– 1.022, p=0.076), 0.847 (95% CI: 0.772– 0.930, p=0.001) and 0.914 (95% CI: 0.836– 1.000, p=0.049) respectively. Meanwhile, in model 2 (adjusted for SOFA score, age, cardiac arrhythmias, solid tumor, metastatic cancer, liver disease, neutrophils) ORs were 0.785 (95% CI: 0.636– 0.968, p=0.024), 0.807 (95% CI: 0.641– 1.016, p=0.068), 0.854 (95% CI: 0.778– 0.939, p=0.001) and 0.917 (95% CI: 0.838– 1.004, p=0.060) respectively. The area under the ROC curve for eosinophil initial was 0.608 (95% CI: 0.559– 0.657). The discriminatory eosinophil thresholds were 0.35% (sensitivity=0.59, specificity=0.61) for in-hospital mortality. Conclusion: Increased blood eosinophils were associated with decreased in-hospital mortality and shorten hospital length of stay in critically ill patients with acute exacerbation of chronic obstructive pulmonary disease. A discriminatory eosinophil threshold of 0.35% for mortality was found, but further studies were needed to verify it. Abstract:Background: Elevated eosinophils in COPD is recognized as a potential risk factor for exacerbations, but the prognostic role of elevated eosinophils during exacerbations of COPD is unclear. We investigated short-term and long-term outcomes in patients with exacerbations of eosinophilic phenotype, compared with patients with low blood eosinophils. Methods: A single-centre retrospective study of all patients admitted for a COPD exacerbation to Bispebjerg University Hospital in 2010-2011 was established by linking inpatient data with national patient and prescription registries, with a three-year follow-up period. Elevated eosinophils were defined as a blood eosinophil level at admission of ≥0.30 × 109 cells/L. Results: A total of 811 patients were included; 13.2% had an eosinophilic exacerbation. The eosinophilic group had less need for non-invasive ventilation, shorter inpatient stay, and lower in-hospital mortality, compared to the non-eosinophilic group. However, the eosinophilic and non-eosinophilic groups showed similar risks of readmission (incidence rate ratio[95], 0.99 [0.73-1.36]). Three-year mortality was high in both groups, although lower in the eosinophilic group (40% vs. 54%, p = 0.006). Conclusions: COPD exacerbations in patients with high blood eosinophil have a better short-term prognosis without higher risk of subsequent exacerbation. Eosinophilic exacerbations have also a lower three-year mortality.
Eosinopenia and neutrophil-to-lymphocyte count ratio as prognostic factors in exacerbation of COPD

Abstract:Exacerbations of Chronic Obstructive Pulmonary Disease (AECOPDs) are one of the most important clinical aspects of the disease, and when requiring hospital admission, they significantly contribute to mortality among COPD patients. Our aim was to assess the role of eosinopenia and neutrophil-to-lymphocyte count (NLR) as markers of in-hospital mortality and length of hospitalization (LoH) among patients with ECOPD requiring hospitalization. We included 275 patients. Eosinopenia was associated with in-hospital deaths only when coexisted with lymphocytopenia, with the specificity of 84.4% (95% CI 79.6-88.6%) and the sensitivity of 100% (95% CI 35.9-100%). Also, survivors presented longer LoH (P < 0.0001). NLR ≥ 13.2 predicted in-hospital death with the sensitivity of 100% (95% CI 35.9-100%) and specificity of 92.6% (95% CI 88.8-95.4%), however, comparison of LoH among survivors did not reach statistical significance (P = 0.05). Additionally, when we assessed the presence of coexistence of eosinopenia and lymphocytopenia first, and then apply NLR, sensitivity and specificity in prediction of in-hospital death was 100% (95% CI 35.9-100) and 93.7% (95% CI 90.1-96.3), respectively. Moreover, among survivors, the occurrence of such pattern was associated with significantly longer LoH: 11 (7-14) vs 7 (5-10) days (P = 0.01). The best profile of sensitivity and specificity in the prediction of in-hospital mortality in ECOPD can be obtained by combined analysis of coexistence of eosinopenia and lymphocytopenia with elevated NLR. The occurrence of a such pattern is also associated with significantly longer LoH among survivors.
